- • Misdiagnosed and maltreated scaphoid fractures filed to the Swedish National Patient Insurance Company (LÖF) 2011–2018 were analyzed in terms of complications and costs for society. All filed claims are database-registered (altogether 200 000 claims since 2000). This database was assessed in June 2019 through injury ICD10-SWE-diagnoses. Demographics, complications, complaints, corrective surgeries and costs were analyzed. The numbers of claims for scaphoid fractures were reviewed and compared with all claims. • There was a statistically significant trend towards decreasing numbers of notified scaphoid fracture cases during this time. This is not the case compared with the total annually notified injuries to LÖF during the same time, where we instead can see statistically significant increased numbers. • Median age for the 128 patients was 24 years. Men represented 76%. Seventy-eight of the 128 (61%) claims were judged as avoidable, compared with 42% in terms of all notified injuries. Pseudoarthrosis dominated as complication (n = 71). Total numbers of complications were 117, and 47 of the 78 patients had medical invalidity as a consequence. Up to six secondary corrective surgeries per patient were required. Complications and disabilities were more severe if patients needed more than one surgery. The total costs were calculated to €1 226 193. • Level of Evidence: LoE III, Therapeutic

- Antisymmetric, symmetric and standard actuation of acoustofluidic devices were compared in terms of focusability. By imaging the end part of an acoustophoresis separation channel while flowing fluorescent beads through it, we show that beads get more tightly focused by antisymmetric actuation than symmetric and standard actuation at a given electrical input power. This means that antisymmetric actuation is more suitable for single node acoustic focusing as lower input power can be used to reach the same level of performance, or higher throughput can be reached for the same power.

- OBJECTIVES: Piperacillin is a β-lactam antimicrobial frequently used in critically ill patients with acute kidney injury treated with continuous renal replacement therapy (CRRT). However, data regarding piperacillin tissue concentrations in this patient population are limited. A prospective observational study was conducted of free piperacillin concentrations during a single 8-h dosing interval in plasma (8 samples) and subcutaneous tissue (SCT) (13 samples), in 10 patients treated with CRRT following piperacillin 4 g given every 8 h as intermittent administration over 3 min.METHODS: A population pharmacokinetic model was developed using NONMEM 7.4.3, to simulate alternative administration modes and dosing regimens. SCT concentrations were obtained using microdialysis. Piperacillin concentrations were compared to the clinical breakpoint minimum inhibitory concentration (MIC) for Pseudomonas aeruginosa (16 mg/l), with evaluation of the following pharmacokinetic/pharmacodynamics targets: 50% fT > 1 × MIC, 100% fT > 1 × MIC, and 100% fT > 4 × MIC.RESULTS: SCT concentrations were generally lower than plasma concentrations. For the target of 50% free time (fT) > 1 × MIC and 100% fT > 1 × MIC, piperacillin 4 g every 8 h resulted in probability of target attainment (PTA) >90% in both plasma and SCT. PTA > 90% for the target of 100% fT > 4 × MIC was only achieved for continuous infusion.CONCLUSIONS: Piperacillin 4 g every 8 h is likely to provide sufficient exposure in both plasma and SCT to treat P.aeruginosa infections in critically ill patients on CRRT, given that targets of 50% fT > 1 × MIC or 100% fT > 1 × MIC are adequate. However, if a more aggressive target of 100% fT > 4 × MIC is adopted, continuous infusion is needed.

- ABSTRACT: BACKGROUND: Posterior dislocation of the shoulder in brachial plexus birth palsy during the first year of life is rare but the incidence increases with age. The aim was to calculate the incidence of these lesions in children below one year of age. METHODS: The incidence of brachial plexus birth lesion and occurrence of posterior shoulder dislocation was calculated based on a prospective follow up of all brachial plexus patients at an age below one in Malmö municipality, Sweden, 2000-2005. RESULTS: The incidence of brachial plexus birth palsy was 3.8/1000 living infants and year with a corresponding incidence of posterior shoulder dislocation (history, clinical examination and x-ray) during the first year of 0.28/1000 living infants and year, i.e. 7.3% of all brachial plexus birth palsies. CONCLUSION: All children with a brachial plexus birth lesion (incidence 3.8 per thousand) should be screened, above the assessment of neurological recovery, during the first year of life for posterior dislocation of the shoulder (incidence 0.28 per thousand) since such a condition may occur in 7% of children with a brachial plexus birth lesion.

- A phytobezoar is a rare differential diagnosis in the acute abdomen. An 89-year-old woman presented with lower abdominal pain. A computed tomography scan and ultrasound suggested the presence of a bezoar. A phytobezoar was extracted surgically, and a resection was performed of the perforated small bowel segment. The etiology and management of phytobezoars are discussed.

- I den här artikeln argumenterar jag för att vårt tänkande i grunden är metafor- och föreställningsstyrt. Bilder och konceptuella ramverk styr i mångt och mycket hur vi kommunicerar, och därmed i grunden argumenterar och debatterar. Jag hävdar att det finns en utbredd föreställning om att vårt tänkande är mer objektivt förankrat än vad det egentligen är. Som rättssociolog är min poäng att det även tar sig uttryck i hur rätten formuleras, vilket naturligtvis sker i en dialektik med teknologiska utvecklingar, förändrade samhällsförhållanden och så vidare. Rätten adderar dessutom en aspekt av formaliserad makt, helt i linje med sanktionerade metaforer och föreställningar. Den konkreta fallstudien för dessa resonemang utgörs i min artikel av upphovsrättens fokus på exemplarframställning, samt inte minst kontrollen av denna. Jag vill hävda att ”exemplaret” är en konceptuell metafor – som kunde se annorlunda ut. Mediedistribution kan exempelvis konceptualiseras i termer av ett ”flöde” lika mycket som i form av ”exemplar”. Eftersom förutsättningarna för hur digitala medier distribueras har förändrats så väsentligt – från att tidigare ha lagrats i olika varianter av väldefinierat avgränsade plastbitar till att spridas som digitala filer i nätverk – så har exemplarsmetaforen expanderat till att täcka en rad nya fenomen. Det handlar visserligen om fenomen med liknande attribut som de ursprungliga, vilket gäller de flesta metaforiska överföringar. Men någonstans i denna expansion av exemplarsmetaforen kommer det att uppstå svårigheter, det vill säga att metaforen inte är tillräcklig längre. Att den döljer mer än den berättar. De rättsliga följderna vittnar förstås om detta. För att i praktiken visa på problemen med exemplartänkandet i en digital kontext, använder jag i den här artikeln statistik från en fildelningstrackersida (som The Pirate Bay, men inte The Pirate Bay), och de monetära värden per fil som tingsrätten använde i fallet mot The Pirate Bay. Ambitionen är helt enkelt att räkna ut vad en hel sajt som exempelvis The Pirate Bay egentligen skulle vara värd enligt denna etablerade och rättsligt sanktionerade modell. Summan blir naturligtvis enorm – 459 miljarder - vilket sätter detta sätt att värdera datafiler på sin spets. Det är ett sätt att kritisera rätten, förankrat i hur språkliga och tankemässiga processer går till. Även om ett flödesbaserat utbyte inte av nödvändighet behöver upplösa exemplaren i sig, så är det nödvändigt att reflektera utanför det befintliga och här legalt formulerade sättet att tänka.

- Introduction. Restorative surgery for ulcerative colitis with ileal pouch anal anastomosis (IPAA) is frequently accompanied by complications. Volvulus of the ileal pouch is one of the most rarely reported late complications and to our knowledge no report exists on reoperative surgery for this condition. Case Report. A 58-year-old woman who previously had undergone restorative proctocolectomy due to ulcerative colitis with an IPAA presented with volvulus of the pouch. She was operated with a single row pouchopexy to the presacral fascia. Two months later she returned with a recurrent volvulus. At reoperation, the pouch was found to have become completely detached from the fascia. A new pexy was made by firmly anchoring the pouch with two rows of sutures to the presacral fascia as well as with sutures to the lateral pelvic walls. At follow-up after five months she was free of symptoms. Conclusion. This first report ever on reoperative surgery for volvulus of a pelvic pouch indicates that a single row pouchopexy might be insufficient for preventing retwisting. Several rows seem to be needed.

- Purpose Misdiagnosed and maltreated wrist ligament injuries (scapholunate [SL], lunotriquetral, and triangular fibrocartilage complex [TFCC]) filed to the Swedish National Patient Insurance Company (LoF ["regionernas omsesidiga forsakringsbolag"]) 2011 to 2018 were analyzed in terms of complications and costs for society.Methods All filed claims are database registered. The database was assessed in June 2019 through injury International Classification of Diseases 10th Revision-SWE diagnoses. Demographics, reasons for avoidance, type of complication, and costs were assessed. Trend analysis was also used to compare the numbers of filed claims of wrist ligament injuries and total injuries.Results The mean age of the 231 extracted patients was 38 years. Females represented 57%. Ninety-eight (42%) of the claims were judged as avoidable, in accordance with the 40% approved patient injuries of all notified injuries during the same time period. Isolated injuries to the TFCC and SL ligament were the most common (n = 185, 80%). One to seven secondary surgical procedures/patient were needed to treat the condition. Sixty-nine (30%) of the 231 patients had medical invalidity due to the avoidable complication(s). Disability was more severe if more than one surgical procedure was needed. There was a significant trend toward decreasing numbers of filed claims for wrist ligament injuries ( p = 0.002) over time, in contrast to the total number of filed claims to LoF, which has increased by an average of 6% annually for a cumulative increase of 60% from 2011 to 2018.Interpretation The total cost for misdiagnosed and mistreated wrist ligament injuries in Sweden from 2011 to 2018 was euro2,203,467, and costly for both the patients and society at large.Level of Evidence Level III, therapeutic.

I fallstudierna diskuteras framgångsfaktorer, svårigheter och motgångar i öppna och reflekterande texter i avsikt att lära och gemensamt stärka det sociala arbetet inför framtida utmaningar. Fallstudierna ramas in av en teoretisk genomgång och analys av det konkreta mobiliseringsarbetet. Boken riktar sig till yrkesverksamma, studerande och föreningsaktiva, och till alla som är engagerade och intresserade av samhällsarbete i bred bemärkelse.
