    • Obesity is heritable and predisposes to many diseases. To understand the genetic basis of obesity better, here we conduct a genome-wide association study and Metabochip meta-analysis of body mass index (BMI), a measure commonly used to define obesity and assess adiposity, in up to 339,224 individuals. This analysis identifies 97 BMI-associated loci (P < 5 × 10(-8)), 56 of which are novel. Five loci demonstrate clear evidence of several independent association signals, and many loci have significant effects on other metabolic phenotypes. The 97 loci account for ∼2.7% of BMI variation, and genome-wide estimates suggest that common variation accounts for >20% of BMI variation. Pathway analyses provide strong support for a role of the central nervous system in obesity susceptibility and implicate new genes and pathways, including those related to synaptic function, glutamate signalling, insulin secretion/action, energy metabolism, lipid biology and adipogenesis.

    • Body fat distribution is a heritable trait and a well-established predictor of adverse metabolic outcomes, independent of overall adiposity. To increase our understanding of the genetic basis of body fat distribution and its molecular links to cardiometabolic traits, here we conduct genome-wide association meta-analyses of traits related to waist and hip circumferences in up to 224,459 individuals. We identify 49 loci (33 new) associated with waist-to-hip ratio adjusted for body mass index (BMI), and an additional 19 loci newly associated with related waist and hip circumference measures (P < 5 × 10(-8)). In total, 20 of the 49 waist-to-hip ratio adjusted for BMI loci show significant sexual dimorphism, 19 of which display a stronger effect in women. The identified loci were enriched for genes expressed in adipose tissue and for putative regulatory elements in adipocytes. Pathway analyses implicated adipogenesis, angiogenesis, transcriptional regulation and insulin resistance as processes affecting fat distribution, providing insight into potential pathophysiological mechanisms.

    • For the first time in serveral years, the El Nino-Southern Oscillation did not dominate regional climate conditions around the globe. A weak La Ni a dissipated to ENSOneutral conditions by spring, and while El Nino appeared to be emerging during summer, this phase never fully developed as sea surface temperatures in the eastern conditions. Nevertheless, other large-scale climate patterns and extreme weather events impacted various regions during the year. A negative phase of the Arctic Oscillation from mid-January to early February contributed to frigid conditions in parts of northern Africa, eastern Europe, and western Asia. A lack of rain during the 2012 wet season led to the worst drought in at least the past three decades for northeastern Brazil. Central North America also experienced one of its most severe droughts on record. The Caribbean observed a very wet dry season and it was the Sahel's wettest rainy season in 50 years. Overall, the 2012 average temperature across global land and ocean surfaces ranked among the 10 warmest years on record. The global land surface temperature alone was also among the 10 warmest on record. In the upper atmosphere, the average stratospheric temperature was record or near-record cold, depending on the dataset. After a 30-year warming trend from 1970 to 1999 for global sea surface temperatures, the period 2000-12 had little further trend. This may be linked to the prevalence of La Ni a-like conditions during the 21st century. Heat content in the upper 700 m of the ocean remained near record high levels in 2012. Net increases from 2011 to 2012 were observed at 700-m to 2000-m depth and even in the abyssal ocean below. Following sharp decreases in to the effects of La Ni a, sea levels rebounded to reach records highs in 2012. The increased hydrological cycle seen in recent years continued, with more evaporation in drier locations and more precipitation in rainy areas. In a pattern that has held since 2004, salty areas of the ocean surfaces and subsurfaces were anomalously salty on average, while fresher areas were anomalously fresh. Global tropical cyclone activity during 2012 was near average, with a total of 84 storms compared with the 1981-2010 average of 89. Similar to 2010 and 2011, the North Atlantic was the only hurricane basin that experienced above-normal activity. In this basin, Sandy brought devastation to Cuba and parts of the eastern North American seaboard. All other basins experienced either near-or below-normal tropical cyclone activity. Only three tropical cyclones reached Category 5 intensity-all in Bopha became the only storm in the historical record to produce winds greater than 130 kt south of 7 N. It was also the costliest storm to affect the Philippines and killed more than 1000 residents. Minimum Arctic sea ice extent in September and Northern Hemisphere snow cover extent in June both reached new record lows. June snow cover extent is now declining at a faster rate (-17.6% per decade) than September sea ice extent (-13.0% per decade). Permafrost temperatures reached record high values in northernmost Alaska. A new melt extent record occurred on 11-12 July on the Greenland ice sheet; 97% of the ice sheet showed some form of melt, four times greater than the average melt for this time of year. The climate in Antarctica was relatively stable overall. The largest maximum sea ice extent since records begain in 1978 was observed in September 2012. In the stratosphere, warm air led to the second smallest ozone hole in the past two decades. Even so, the springtime ozone layer above Antarctica likely will not return to its early 1980s state until about 2060. Following a slight decline associated with the global 2 emissions from fossil fuel combustion and cement production reached a record 9.5 +/- 0.5 Pg C in 2011 and a new record of 9.7 +/- 0.5 Pg C is estimated for 2012. Atmospheric CO2 concentrations increased by 2.1 ppm in 2012, to 392.6 ppm. In spring 2012, 2 concentration exceeded 400 ppm at 7 of the 13 Arctic observation sites. Globally, other greenhouse gases including methane and nitrous oxide also continued to rise in concentration and the combined effect now represents a 32% increase in radiative forcing over a 1990 baseline. Concentrations of most ozone depleting substances continued to fall.

    • Aim: Our aim was to describe the outcomes of multisystem inflammatory syndrome in children (MIS-C) associated with COVID-19. Methods: This national, population-based, longitudinal, multicentre study used Swedish data that were prospectively collected between 1 December 2020 and 31 May 2021. All patients met the World Health Organization criteria for MIS-C. The outcomes 2 and 8weeks after diagnosis are presented, and follow-up protocols are suggested. Results: We identified 152 cases, and 133 (87%) participated. When followed up 2weeks after MIS-C was diagnosed, 43% of the 119 patients had abnormal results, including complete blood cell counts, platelet counts, albumin levels, electrocardiograms and echocardiograms. After 8weeks, 36% of 89 had an abnormal patient history, but clinical findings were uncommon. Echocardiogram results were abnormal in 5% of 67, and the most common complaint was fatigue. Older children and those who received intensive care were more likely to report symptoms and have abnormal cardiac results. Conclusion: More than a third (36%) of the patients had persistent symptoms 8weeks after MIS-C, and 5% had abnormal echocardiograms. Older age and higher levels of initial care appeared to be risk factors. Structured follow-up visits are important after MIS-C.

    • Methylmercury (MeHg) toxicity may vary widely despite similar levels of exposure. This is hypothetically related to genetic differences in enzymes metabolizing MeHg. MeHg causes oxidative stress in experimental models but little is known about its effects on humans. The aims of the present study was to evaluate the effects of polymorphisms in glutathione (GSH)-related genes (GSTM1, GSTT1, GSTP1 and GCLM) on Hg concentrations in blood and hair, as well as MeHg-related effects on catalase (CAT) and glutathione-peroxidase (GPx) activity and GSH concentrations. Study subjects were from an Amazonian population in Brazil chronically exposed to MeHg from fish. Hg in blood and hair were determined by ICP-MS, CAT, GPx and GSH were determined by spectrophotometry, and multiplex PCR (GSTM1 and GSTT1) and TagMan assays (GSTP1 and GCLM) were used for genotyping. Mean Hg concentrations in blood and hair were 48 +/- 36 mu g/L and 14 +/- 10 mu g/g. Persons with the GCLM-588 IT genotype had lower blood and hair Hg than did C-allele carriers (linear regression for Hg in blood beta = -0.32, p = 0.017; and hair beta = -0.33; p = 0.0090; adjusted for fish intake, age and gender). GSTM1*0 homozygous had higher blood (beta = 0.20; p = 0.017) and hair Hg (hair beta = 0.20; p = 0.013). Exposure to MeHg altered antioxidant status (CAT:beta = -0.086; GSH:beta = -0.12; GPx:beta = -0.16; all p < 0.010; adjusted for gender, age and smoking). Persons with GSTM1*0 had higher CAT activity in the blood than those with GSTM1. Our data thus indicate that some GSH-related polymorphisms, such as GSTM1 and GCLM may modify MeHg metabolism and Hg-related antioxidant effects. (C) 2013 Elsevier BY. All rights reserved.

    • We performed a meta-analysis of 14 genome-wide association studies of coronary artery disease (CAD) comprising 22,233 individuals with CAD (cases) and 64,762 controls of European descent followed by genotyping of top association signals in 56,682 additional individuals. This analysis identified 13 loci newly associated with CAD at P < 5 x 10(-8) and confirmed the association of 10 of 12 previously reported CAD loci. The 13 new loci showed risk allele frequencies ranging from 0.13 to 0.91 and were associated with a 6% to 17% increase in the risk of CAD per allele. Notably, only three of the new loci showed significant association with traditional CAD risk factors and the majority lie in gene regions not previously implicated in the pathogenesis of CAD. Finally, five of the new CAD risk loci appear to have pleiotropic effects, showing strong association with various other human diseases or traits.

    • Formyl peptide receptor 2 (FPR2) is a G protein-coupled pattern recognition receptor sensing both mitochondrial- and bacterial-derived formylated peptides, including the PSMα toxins secreted by community-associated methicillin-resistant Staphylococcus aureus strains. Similar to many other FPR2 agonistic peptides, nanomolar concentrations of both PSMα2 and PSMα3 activate neutrophils to increase the cytosolic concentration of Ca2+ and release NADPH oxidase-derived reactive oxygen species. In addition, the PSMα peptides induce FPR2 homologous desensitization, actin polymerization, and neutrophil reactivation through a receptor cross-talk mechanism. However, in contrast to conventional FPR2 agonistic peptides, including the host-derived formyl peptide MCT-ND4, we found that the PSMα peptides lacked the ability to recruit β-arrestin and induce neutrophil chemotaxis, supporting the previous notion that β-arrestin translocation is of importance for cell migration. Despite the lack of β-arrestin recruitment, the PSMα peptides induced an FPR2-dependent ERK1/2 phosphorylation and internalization. Furthermore, structure-activity relationship analysis with PSMα2 derivatives revealed critical roles of the first 3 aa linked to N-fMet as well as the C terminus of PSMα2 in promoting FPR2 to recruit β-arrestin. In summary, our data demonstrate a novel neutrophil activation pattern upon FPR2 sensing of PSMα peptides, signified by the ability to induce increased intracellular Ca2+, ERK1/2 phosphorylation, internalization, and NADPH oxidase activity, yet lack of β-arrestin recruitment and neutrophil chemoattraction. These novel features adopted by the PSMα peptides could be of importance for S. aureus virulence and might facilitate identification of new therapeutic strategies for treating S. aureus infections.

    • Fungi play major roles in ecosystem processes, but the determinants of fungal diversity and biogeographic patterns remain poorly understood. Using DNA metabarcoding data from hundreds of globally distributed soil samples, we demonstrate that fungal richness is decoupled from plant diversity. The plant-to-fungus richness ratio declines exponentially toward the poles. Climatic factors, followed by edaphic and spatial variables, constitute the best predictors of fungal richness and community composition at the global scale. Fungi show similar latitudinal diversity gradients to other organisms, with several notable exceptions. These findings advance our understanding of global fungal diversity patterns and permit integration of fungi into a general macroecological framework.

  • Around the Screen : Computer activities in children’s everyday lives

    • The present ethnography documents computer activities in everyday life. The data consist of video recordings, interviews and field notes, documenting (i) 16 students in a seventh grade class in a computer room and other school settings and (ii) 22 children, interacting with siblings, friends and parents in home settings. The thesis is inspired by discourse analytical as well as ethnographic approaches, including notions from Goffman (1974, 1981), e.g. those of activity frame and participation framework, which are applied and discussed.The thesis consists of four empirical studies. The first study focuses on students’ illegitimate use, from the school’s point of view, of online chatting in a classroom situation. It is shown that the distinction offline/online is not a static one, rather it is made relevant as part of switches between activity frames, indicating the problems of applying Goffman’s (1981) notions of sideplay, byplay and crossplay to analyses of interactions in which several activity frames are present, rather than one main activity. Moreover, it is shown that online identities, in terms of what is here called tags, that is, visual-textual nicknames, are related to offline phenomena, including local identities as well as contemporary aesthetics. The second study focuses on placement of game consoles as part of family life politics. It is shown that game consoles were mainly located in communal places in the homes. The distinction private/communal was also actualized in the participants’ negotiations about access to game consoles as well as negotiations about what to play, when, and for how long. It is shown that two strategies were used, inclusion and exclusion, for appropriating communal places for computer game activities. The third study focuses on a digital divide in terms of a generational divide with respect to ascribed computer competence, documenting how the children and adults positioned each other as people ‘in the know’ (the children) versus people in apprentice-like positions (the adults). It is shown that this generation gap was deployed as a resource in social interaction by both the children and the adults. The forth study focuses on gaming in family life, showing that gaming was recurrently marked by response cries (Goffman, 1981) and other forms of blurted talk. These forms of communication worked as parts of the architecture of intersubjectivity in gaming (cf. Heritage, 1984), indexing the distinction virtual/‘real’. It is shown how response cries, sound making, singing along and animated talk extended the virtual in that elements of the game became parts of the children’s social interaction around the screen, forming something of an action aesthetic, a type of performative action for securing and displaying joint involvement and collaboration. As a whole, the present studies show how the distinctions master/apprentice, public/private, virtual/real and subject/object are indexicalized and negotiated in computer activities.

    • This article examines territorial negotiations concerning gaming, drawing on video recordings of gaming practices in middle-class families. It explores how private vs public gaming space was co-construed by children and parents in front of the screen as well as through conversations about games. Game equipment was generally located in public places in the homes, which can be understood in terms of parents' surveillance of their children, on the one hand, and actual parental involvement, on the other. Gaming space emerged in the interplay between game location, technology and practices, which blurred any fixed boundaries between public and private, place and space, as well as traditional age hierarchies.

    • The present study focuses on the ways in which response cries (Goffman, 1981) are deployed as interactional resources in computer gaming in everyday life. It draws on a large-scale data set of video recordings of the everyday lives of middleclass families. The recordings of gaming between children and between children and parents show that response cries were not arbitrarily located within different phases of gaming (planning, gaming or commenting on gaming). Response cries were primarily used as interactional resources for securing and sustaining joint attention (cf. Goodwin, 1996) during the gaming as such, that is, during periods when the gaming activity was characterized by a relatively high tempo. In gaming between children, response cries co-occurred with their animations of game characters and with sound making, singing along, and code switching in ways that formed something of an action aesthetic, a type of aesthetic that was most clearly seen in gaming between game equals (here: between children). In contrast, response cries were rare during the planning phases and during phases in which the participants primarily engaged in setting up or adjusting the game.

    • The International Stem Cell Initiative characterized 59 human embryonic stem cell lines from 17 laboratories worldwide. Despite diverse genotypes and different techniques used for derivation and maintenance, all lines exhibited similar expression patterns for several markers of human embryonic stem cells. They expressed the glycolipid antigens SSEA3 and SSEA4, the keratan sulfate antigens TRA-1-60, TRA-1-81, GCTM2 and GCT343, and the protein antigens CD9, Thy1 (also known as CD90), tissue- nonspecific alkaline phosphatase and class 1 HLA, as well as the strongly developmentally regulated genes NANOG, POU5F1 (formerly known as OCT4), TDGF1, DNMT3B, GABRB3 and GDF3. Nevertheless, the lines were not identical: differences in expression of several lineage markers were evident, and several imprinted genes showed generally similar allele-specific expression patterns, but some gene-dependent variation was observed. Also, some female lines expressed readily detectable levels of XIST whereas others did not. No significant contamination of the lines with mycoplasma, bacteria or cytopathic viruses was detected.

  • Acute Myocarditis Associated With Desmosomal Gene Variants

    • BACKGROUND The risk of adverse cardiovascular events in patients with acute myocarditis (AM) and desmosomal gene variants (DGV) remains unknown.OBJECTIVES The purpose of this study was to ascertain the risk of death, ventricular arrhythmias, recurrent myocarditis, and heart failure (main endpoint) in patients with AM and pathogenic or likely pathogenetic DGV.METHODS In a retrospective international study from 23 hospitals, 97 patients were included: 36 with AM and DGV (DGV[+]), 25 with AM and negative gene testing (DGV[-]), and 36 with AM without genetics testing. All patients had troponin elevation plus findings consistent with AM on histology or at cardiac magnetic resonance (CMR). In 86 patients, CMR changes in function and structure were re-assessed at follow-up.RESULTS In the DGV(+) AM group (88.9% DSP variants), median age was 24 years, 91.7% presented with chest pain, and median left ventricular ejection fraction (LVEF) was 56% on CMR (P = NS vs the other 2 groups). Kaplan-Meier curves demonstrated a higher risk of the main endpoint in DGV(+) AM compared with DGV(-) and without genetics testing patients (62.3% vs 17.5% vs 5.3% at 5 years, respectively; P < 0.0001), driven by myocarditis recurrence and ventricular arrhythmias. At follow-up CMR, a higher number of late gadolinium enhanced segments was found in DGV(+) AM. CONCLUSIONS Patients with AM and evidence of DGV have a higher incidence of adverse cardiovascular events compared with patients with AM without DGV. Further prospective studies are needed to ascertain if genetic testing might improve risk stratification of patients with AM who are considered at low risk.   • PERson-centredness in Hypertension management using Information Technology: a randomized controlled trial in primary care

    • OBJECTIVES: To increase the proportion of individuals with hypertension obtaining a blood pressure (BP) of less than 140/90mmHg by improving the management of hypertension in daily life from a person-centred perspective. METHODS: In this unblinded randomized controlled trial, we tested an interactive web-based self-management system for hypertension. A total of 949 patients with hypertension from 31 primary healthcare centres (PHCCs) in Sweden were randomized 1:1 to either the intervention or usual care group. The intervention included daily measurement - via the participant's mobile phone - of BP and pulse and reports of well being, symptoms, lifestyle, medication intake and side effects for eight consecutive weeks. It also included reminders and optional motivational messages. The primary outcome was the proportion of participants obtaining BP of less than 140/90mmHg at 8 weeks and 12months. Significance was tested by Pearson's chi 2 -test. RESULTS: A total of 862 patients completed the trial, 442 in the intervention group and 420 in the control group. The primary outcome (BP <140/90mmHg) at 8 weeks was achieved by 48.8% in the intervention group and 39.9% in the control group ( P =0.006). At 12months, 47.1% (intervention) and 41.0% (control group) had a BP less than 140/90mmHg ( P =0.071). CONCLUSION: The proportion of participants with a controlled BP of less than 140/90mmHg increased after using the interactive system for self-management of hypertension for 8 weeks compared with usual care. Although the trend continued, there was no significant difference after 12months. The results indicate that the effect of the intervention is significant, but the long-term effect is uncertain.     • OBJECTIVE: Few studies address results from use of new technology and patient participation in hypertension management. The PERHIT Study is a multicentre randomised controlled trial with the aim to evaluate the effects of a person-centred approach using a web-based, interactive self-management system through the patient´s own mobile phone on blood-pressure and well-being. Primary aim is the degree of achieved blood pressure (BP) control after eight weeks and one year. In addition, person-centeredness, usefulness, daily life activities in relation to BP values, awareness of risk and health care costs are studied. DESIGN AND METHOD: The PERHIT study was performed in four regions in southern Sweden. Following inclusion, more than 900 patients from 31 primary health care centres were randomised to two groups. In the intervention group (INT), patients were provided with a web-based self-management support system including a home-BP monitor. For eight consecutive weeks, they measured BP and performed self-reports regarding well-being, symptoms, lifestyle, medication intake and side effects every evening via their mobile phone. They could also receive motivational messages and reminders throughout the intervention period. Both patients and professionals had access to graphic feedback of reported values through a secure web portal. Patients in the control (CON) group received standard treatment as usual. RESULTS: The primary outcome (BP < 140/90 mmHg) was achieved by 48.5% and 47.1% in the INT, and by 40.4% and 40.9% in the CON group after 8 weeks (p = 0.016) and 12 months (p = 0.067), respectively. Both patients and professionals experienced the system as a useful resource for communication regarding BP and lifestyle. They described that it could be used to support a constructive and person-centred partnership between patients and professionals. CONCLUSIONS: Blood pressure control was significantly better after eight weeks, but not after one year, following an intervention based on use of mobile phones, feedback and interaction between patients and primary care professionals compared to standard care. The system can be a tool toward a new way of working and help patients reach a controlled BP and play a role in a more person-centred and individually adapted hypertension management.

    • Background: In marine fish species, where pelagic egg and larvae drift with ocean currents, population structure has been suggested to be maintained by larval retention due to hydrographic structuring and by homing of adult fish to natal areas. Whilst natal homing of adults has been demonstrated for anadromous and coral reef fishes, there are few documented examples of philopatric migration in temperate marine fish species. Results: Here, we demonstrate temporally stable genetic differentiation among spawning populations of Atlantic cod (Gadus morhua L.), and present genetic and behavioural evidence for larval drift and philopatric migration in the eastern North Sea-Skagerrak-Kattegat area. We show that juvenile cod collected in the eastern Skagerrak and central Kattegat are genetically similar to cod from offshore spawning areas in the eastern North Sea. Genetic assignment of individual 2-5 year old fish indicates that cod residing at, or migrating towards, spawning areas in Kattegat and the North Sea display philopatric behaviours. Conclusions: Together these findings suggest a loop between spawning, larval drift and adult return-migrations to spawning areas and underlines that both oceanographic processes and migratory behaviour in the adult phase may be important for stock separation and integrity in marine temperate fishes such as Atlantic cod. © 2016 André et al.

    • To further the understanding of climate change adaptation processes, more attention needs to be paid to the various contextual factors that shape whether and how climate-related knowledge and information is received and acted upon by actors involved. This study sets out to examine the characteristics of forest owners' in Sweden, the information and knowledge-sharing networks they draw upon for decision-making, and their perceptions of climate risks, their forests' resilience, the need for adaptation, and perceived adaptive capacity. By applying the concept of ego-network analysis, the empirical data was generated by a quantitative survey distributed to 3000 private forest owners' in Sweden in 2014 with a response rate of 31%. The results show that there is a positive correlation, even though it is generally weak, between forest owner climate perceptions and (i) network features, i.e. network size and heterogeneity, and (ii) presence of certain alter groups (i.e. network members or actors). Results indicate that forest owners' social networks currently serve only a minimal function of sharing knowledge of climate change and adaptation. Moreover, considering the fairly infrequent contact between respondents and alter groups, the timing of knowledge sharing is important. In conclusion we suggest those actors that forest owners' most frequently communicate with, especially forestry experts providing advisory services (e.g. forest owner associations, companies, and authorities) have a clear role to communicate both the risks of climate change and opportunities for adaptation. Peers are valuable in connecting information about climate risks and adaptation to the actual forest property.

    • Adaptation to climate change is becoming more urgent, but the wealth of knowledge that informs adaptation planning and decision-making is not used to its full potential. Top-down approaches to knowledge production are identified as one important reason for the gap between science and practice and are criticized for not meeting the needs of intended users. In response to this challenge, there is a growing interest in the creation of user-oriented and actionable climate services to support adaptation. At the same time, recent research suggests that greater efforts are needed to evaluate the effectiveness of knowledge co-production processes and the best criteria by which to gauge the quality of knowledge outcomes, while also considering different stakeholder perspectives. This paper explores these issues through a critical assessment of the quality of knowledge for adaptation generated from a climate services co-design process in two case studies in Sweden. The study draws on experiences from a 5-year research collaboration in which natural and social science researchers, together with local stakeholders, co-designed climate services to support climate adaptation planning and decision-making. The well-established knowledge quality criteria of credibility, legitimacy, saliency, usability, and usefulness remain relevant, but are not sufficient to capture factors relating to whether and how the knowledge actually is applied by climate change adaptation planners and decision-makers. We observe that case-specific circumstances beyond the scope of the co-design process, including the decision-making context as well as non-tangible outcomes, also play crucial roles that should be accounted for in the knowledge assessment processes.

    • This thesis analyses how societal adaptation processes in public and private sectors at the regional to local level in Sweden are enacted. The thesis pays particular attention to critical factors that constrain or enable adaptation by focussing on: who are the stakeholders, how do different stakeholders perceive their capacity to adapt, and the role of stakeholder interaction in facilitating adaptation processes A combination of two analytical perspectives is used where one is based on key concepts within adaptation literature, and the other draws on boundary crossing and transdisciplinary knowledge production (stakeholders, adaptive capacity, and science-based stakeholder dialogues). The study is conducted within the scope of two overall case studies of local adaptation processes within an urban region, and a land-use based sector, the private forestry sector. The cases are setting the scene for the collection of empirical material which is achieved through qualitative methods, primarily focus groups discussions with local and regional, public and private stakeholders with an interest in, and responsibility for adaptation. The focus groups meetings are organized as a series of meetings to which different participatory techniques are applied. The study also builds on a comprehensive stakeholder mapping. First, the results suggest a systematic method for identifying stakeholders in adaptation research, policy, and planning applicable in both sectors and regions that combines top-down knowledge with experience and knowledge based on bottom-up processes. Second, the analysis of perceived adaptive capacities reveal several facilitating and constraining factors that relates both to the characteristics of climate risks, experience of climate variability and extreme weather events, and responsibility- and decision-making structures. Third, the analysis of the interaction between local experts and scientists show that there is potential for the boundary spanning function of science-based stakeholder dialogues in facilitating adaptation through stimulating questions and sharing different knowledge bases and experiences among the participants. However further attention needs to be taken to the institutional environment and the role of so called anchoring devices that help local experts to contextualise, discus and thus anchor scientific knowledge in their own decision-making context. In conclusion, there are both commonalities between adaptation processes in the two case studies and some marked differences, e.g., regarding the concept of adaptation, what type of adaptation actions that are identified, the perceived opportunities for adaptation and degree of complexity.

  • Drivkrafter för hållbar konsumtion på lokal nivå : Svenska kommuners roll och möjligheter

    • En ökande konsumtion och resursförbrukning utgör idag en av de största globala hållbarhetsutmaningarna och är en viktig anledning till att utsläppen av växthusgaser fortsätter att öka. Såväl internationellt som i Sverige råder en allt större medvetenhet om att vi måste minska den negativa påverkan på klimat, miljö och människors hälsa som vår konsumtion ger upphov till (se till exempel Agenda 2030 och mål 12). För att främja hållbar konsumtion har städer och kommuner en viktig roll att spela genom att både indirekt och direkt främja och skapa incitament för hållbara konsumtionsmönster och beteenden på lokal nivå, såväl i kommunernas egen verksamhet som bland hushållen och andra aktörer.I denna rapport diskuteras de viktigaste insikterna och slutsatserna från projektet Unlock: Drivkrafter för en hållbar konsumtion på lokal nivå1. Syftet med Unlock har varit att bidra med ny kunskap om svenska kommuners roll när det gäller hållbar konsumtion på lokal nivå samt analysera hinder och möjligheter i kommunernas arbete. Rapporten lyfter fram ett antal rekommendationer på åtgärder och strategier för att stärka kommunernas arbete med hållbar konsumtion, och därmed Sveriges möjligheter att etablera ett mer hållbart samhälle.Rapporten utgår från följande forskningsfrågor:Vad kännetecknar kommunernas arbete med hållbar konsumtion idag? Det vill säga vilka konsumtionsområden står i fokus och vilka styrmedel och åtgärder används?Hur uppfattar kommunerna sin roll och sina möjligheter att främja hållbar konsumtion inom ramen för den egna verksamheten, samt gentemot invånare och företag?Hur använder kommunerna data och indikatorer i arbetet med hållbar konsumtion och vilka behov kan identifieras? Genom en tvärvetenskaplig ansats och en kombination av kvantitativa och kvalitativa metoder har projektet fokuserat på att fånga kommunernas drivkrafter, erfarenheter och kunskaper om hållbar konsumtion. Projektets tre huvudsakliga metoder innefattar: systematisk granskning av den vetenskapliga litteraturen om kommunernas roll att främja hållbar konsumtion; nationell webbaserad enkätstudie som genomfördes under hösten 2018 till Sveriges alla kommuner och länsstyrelser, och 3) fördjupade fallstudier av två kommuner, Lund och Upplands Väsby, där en serie fokusgrupper, workshoppar och intervjuer genomfördes med kommunala tjänstepersoner från olika förvaltningar under 2018–2019.Resultaten visar att många kommuner idag arbetar aktivt för att främja hållbar konsumtion med flera strategiska åtgärder på plats. Kommunernas arbete med hållbar konsumtion spänner över flera konsumtionsområden som till exempel livsmedel, upphandling, energi, avfall, energianvändning och transporter. De styrmedel som används är framför allt informativa och administrativa. Samarbete genom nätverk och investeringar är något som verkar öka möjligheterna att nå framgång. Resultat visar även att upphandling betraktas som ett viktigt styrmedel men upplevs svårt på grund av bristande resurser och kunskap. Likaså upplever kommunerna svårigheter att förstå och följa upp miljöbelastningen från sina inköp.Kommunerna visar stark drivkraft i arbetet med hållbar konsumtion men efterfrågar mer vägledning från nationell nivå. Resultaten visar också att politiskt stöd är mycket viktigt för att få tillräckligt med resurser samt stöd för att genomföra åtgärder. Ekonomiska incitament är också en viktig drivkraft medan bristande finansiella resurser är ett hinder för framsteg. För att lyckas i arbetet framhålls även nyckelpersoner, samarbete över förvaltningsgränser samt kompetens och tid som centrala faktorer. När det gäller indikatorer är det tydligt att de kan fungera som ett viktigt incitament för att adressera hållbar konsumtion men att brist på data ofta hämmar kommunernas möjligheter att utvärdera effekterna av olika åtgärder och styrmedel. Resultaten visar också att kommunernas användning av indikatorer hjälper till att fånga upp hur arbetet med hållbar konsumtion utvecklas i delar av arbetet men att de flesta kommuner inte har någon helhetsbild över hur påverkan från konsumtion utvecklas över tid. En förbättrad uppföljning och utvärdering anses vidare vara viktigt för att öka engagemanget för åtgärder som adresserar hållbar konsumtion. Rapportens rekommendationer (se vidare avsnitt 5):Stärk kommunernas möjligheter till kontinuerligt lärande, erfarenhetsutbyte och samarbeten genom etablering av gemensamma mötesplatser.Säkerställ brett politiskt stöd och mer resurser genom att etablera starkare stödjande institutioner, till exempel genom att se över fördelningen av ansvar från lokal till nationell nivå.Ge kommunerna mer stöd genom gemensamma riktlinjer och vägledning från nationell nivå och inom ramen för det kommunala självstyret.     • To improve decisions and awareness considering climate change it is argued that stakeholder interaction and dialogue is essential. Engaging stakeholders in research on adaptation thus requires analysis of stakeholder landscape and identification of relevant actors at different levels in society. The term 'stakeholder' is broad and researchers and practitioners might have both interrelated and contrasting views on who is at stake, the need for adaptation and climate risks. The aim of this paper is to analyse the stakeholder landscape in a Swedish region, as part of increasing the understanding of the adaptation process. The stakeholder analysis has been initiated by the research teams through stakeholder mapping and complemented by local and regional actors' notions of who is, or should be, involved and active stakeholders in adaptation to climate change. The results indicate the importance of careful stakeholder analysis for sustainable adaptation. The actors' expert knowledge of the regions deepens the picture, show important links and gaps between different actors and illuminate unclear relationships and responsibilities as well as identify those actors who have important roles to play.

  • Maternal origin matters : Country of birth as a risk factor for obstetric anal sphincter injuries

    • Objective: Obstetric anal sphincter injuries (OASIS) are severe complications to vaginal births with potential long-term consequences. Maternal origin has been proposed to affect the overall risk, but the association and underlying explanation are uncertain. The objective was to assess the association between maternal country of birth and OASIS. Methods: A Swedish nationwide cohort study including singleton term vaginal births during 2005–2016. Data were extracted from the Swedish Medical Birth Registry and Statistics Sweden. Modified Poisson regression analyses were performed to obtain crude and adjusted risk ratios (RRs). Adjustments were made in four cumulative steps. Sub-analyses were performed to investigate the risk of OASIS associated with female genital circumcision (FGC). Results: In all, 988 804 births were included. The rate of OASIS in Swedish-born women was 3.5%. Women from East/Southeast Asia had an increased risk of OASIS (adjusted RR [aRR] 1.71, 95% confidence interval [CI] 1.60–1.83), as did women born in Sub-Saharan Africa (aRR 1.60, 95% CI 1.49–1.72). The risk remained significantly increased also after adjustment for maternal height. By contrast, women from South/Central America had a decreased risk of OASIS (aRR 0.65, 95% CI 0.56–0.76). FGC was associated with an increased risk of OASIS (aRR 3.05, 95% CI 2.60–3.58). Episiotomy appeared to have an overall protective effect (aRR 0.95, 95% CI 0.92–0.98), but not significantly more protective among women with female genital mutilation. Conclusions: Country of birth plays an important role in the risk of OASIS. Women from East/Southeast Asia and Sub-Saharan Africa are at significantly increased risk as compared with Swedish-born women, whereas women from South/Central America are at lower risk. FGC is also a significant risk factor for OASIS.

    • It is now widely recognized that stakeholder interaction and dialogue is essential to improve decisions about and awareness of climate change. The term ‘stakeholder’ is broad and researchers and practitioners may have interrelated and contrasting views on who is a stakeholder or who is (or should be) responsible for adaptation to climate change. To engage stakeholders in research or other projects on adaptation thus requires a careful mapping of the stakeholder landscape and identification of relevant actors at different levels. Through a case study approach, based on studies of two Swedish urban regions, Stockholm and Gothenburg, this paper proposes a systematic method to analyse and identify roles and responsibilities in the stakeholder landscape. The initial mapping exercise was complemented by participatory studies of local and regional stakeholders’ perceptions of who is, or should be, involved in adaptation and their significance for climate change adaptation in the respective regions. The results indicate the value of careful stakeholder analysis for sustainable, effective, planned adaptation that is flexible, but also systematic enough to fulfil practical and scientific requirements for the study and advancement of ongoing adaptation processes and implementation.

  • Minimizing tearing during vaginal delivery with a perineal protection device : a randomized controlled trial

    • BackgroundThere is a growing body of evidence indicating that second degree tears cause both short-, and long-term consequences. Very few preventative measures have been found to reduce the incidence of these tears.ObjectiveTo investigate if the use of a perineal protection device during vaginal birth reduces severe perineal tearing (≥grade 2 tear) in primiparous women compared to routine manual perineal support.Study designA single center randomized controlled trial was performed in Sweden 2019-2021. Primiparous women at term were randomly allocated to the intervention group (n=43) where a perineal protection device was used, or to receive routine care (n=49). Main outcome measurement was grade of perineal tear. Secondary outcomes were vaginal and labial tearing. Continuous data was analyzed with Student's T-test or Mann-Whitney U-test. Dichotomous data were analyzed with Pearson X2 test, Fisher's Exact test, and ordinal logistic regression.ResultsWomen in the intervention group had a significantly lower risk of sustaining more extensive perineal tearing as compared to the control group. Use of the perineal protection device decreased risk of ≥grade 2 tears, in relation to grade 0-1 tears (OR 0.40 [95% CI 0.17–0.94]). Ordinal logistic regression analyses revealed an OR of 0.36 (0.16–0.81) for one-grade-injury increment, using the perineal protection device vs control group (p=0.013). Number needed to treat using the perineal protection device was 4.3 to avoid one ≥grade 2 tear. Women in the treatment group had less labial tearing (p=0.016). No adverse effects were detected.ConclusionsUse of the perineal protection device reduced the risk of perineal tearing ≥grade 2 tear by 60% as well as labial tearing.

The aim was to determine risk and protective factors for OASIS. Material and methods: We performed a retrospective register-based observational study. A cohort of 988 988 singleton term deliveries 2005–2016 in Sweden were included. Data from the Swedish Medical Birth Registry and Statistics Sweden were extracted to identify cases of OASIS and maternal and fetal characteristics. Modified Poisson Regression analyses were performed to assess risk factors. Results: The rate of OASIS was 3.5% (n = 34 583). Primiparity (adjusted risk ratio [aRR] 3.13, 95% CI 3.05–3.21), vacuum extraction (aRR 2.79, 95% CI 2.73–2.86), forceps (aRR 4.27, 95% CI 3.86–4.72), and high birthweight (aRR 2.61, 95% CI 2.50–2.72) were associated with a significantly increased risk of OASIS. Increasing maternal age and decreasing maternal height also increased the risk of OASIS. Obesity increased the risk of OASIS (aRR 1.04, 95% CI 1.04–1.08), if fetal birthweight was not adjusted for. Smoking (aRR 0.74, 95% CI 0.70–0.79) and low maternal education (aRR 0.87, 95% CI 0.83–0.92) were associated with a decreased frequency of reported OASIS. Previous cesarean section increased the risk of OASIS (aRR 1.41, 95% CI 1.36–1.47). Conclusions: Primiparity, instrumental delivery, and high birthweight significantly increased the risk of OASIS. Obesity, low height, increasing age, and previous cesarean section also increased the risk whereas smoking and low maternal educational level were associated with a lower OASIS rate.

    • This paper examines the science-practice interface in the complex issue of adapting society to climate change. Through analyses of science-based stakeholder dialogues with climate scientists, municipal officers and private individual forest owners in Sweden, this paper looks at how local experts in the forestry and municipal planning sectors share and integrate scientific knowledge and experience into their work strategies and practices. The results demonstrate how local experts jointly conceptualize climate adaptation, how scientific knowledge is domesticated among local experts in dialogue with scientific experts, the emergence of anchoring devices and, in a more general way, the boundary spanning functions that are at work in the respective sectors.

  • Stakeholder perceptions of adaptation space : The relevance of directexperience and perceived ability to adapt to climate change in Sweden

    • This paper analyses perceptions of adaptation space in Swedish forestry and the potential for developing and implementing climate adaptation processes. The empirical material of this study builds on participatory methods and focus group discussions with private forest owners and forest officials in two forest intensive regions in Sweden. This allowed for a qualitative analysis of the many aspects of adaptation processes, with adaptation space as the main analytical framework to understand the capacity for climate adaptation on levels other than the national average, departing from actor’s perceptions rather than statistics and indices. We discuss if and how exposure to extreme weather events and variability in the past, and the prospect of future climate change and extremes, translates into adaptation action, and examine the role of perceived ability to adapt to the multiple effects of climate change.

    •  Denna   rapport   redovisar   delar   av   resultat   och   analyser   från   Mistra-SWECIAs   arbete   i Stockholmsområdet.   Vår   förhoppning   är   att   den   är   av   intresse   för   de   som   arbetar   med anpassningsfrågor inför klimatförändringar på olika sätt i regionen och i andra delar av Sverige.SWECIA står för Swedish Research Programme on Climate, Impacts and Adaptation och finansieras av Stiftelsen  för  miljöstrategisk  forskning,  Mistra.  Forskningsprogrammet  studerar  klimat,  ekonomi, effekter och anpassning med gemensamma socio-ekonomiska förutsättningar, och med hänsyn till kopplingarna   som   finns   mellan   dessa   forskningsområden.   Forskarna   arbetar   vid   Stockholm Environment Institute  (SEI),  Linköpings  universitet, Lunds  universitet,  Stockholms  universitet  och SMHI.  I  Mistra-SWECIA  är  kommunikationen  mellan  forskare  och  avnämare  central  då  dialogen hjälper till att planera forskningen och bidrar dessutom till effektiv omsättning av resultaten. Första programfasen är fyra år (2008–2011). Denna rapport är en första sammanställning av delresultat från den fallstudie som påbörjades i Stockholms län 2008.Inledningsvis   diskuteras   delar   av   den   deltagandestudie   som   genomfördes   med   aktörer   i Stockholmsregionen under hösten 2008. Vi redogör översiktligt för hur deltagandeforskningen har genomförts och varför vi valt detta sätt att bedriva forskning; på vilket sätt socialt lärande bidrar till anpassningsarbetet;     vilka     intressenter     som     är     engagerade     direkt     eller     indirekt     i klimatanpassningsarbetet, och vilka faktorer vi har uppfattat som kritiska och viktiga för anpassning,. Rapporten  avslutas  med  att  presentera  resultat  från  Mistra-SWECIAs  forskning  inom  Lunds universitet  och  SMHI  angående  framtida  flöden  och  vattennivåer  i  Mälaren,  samt  beräknade klimatscenarier för Stockholmsregionen. Dessa har ockå använts som underlag för studien.Författarna är mycket tacksamma för deltagarnas kommentarer och engagemang i studien men eventuella felaktigheter eller missuppfattningar är helt och hållet författarnas egna.   • Infrastructural considerations of implementing gene therapy for hemophilia in the Nordic context

    • Background: Despite improvements in hemophilia care, challenges remain, including treatment burden and impaired quality of life. Gene therapy may overcome these. However, its introduction presents a challenge. Objectives: To outline a function-based gene therapy working model describing critical milestones associated with gene therapy handling, administration, and follow-up to facilitate and implement an effective infrastructure for gene therapy introduction. Design: Literature review and consensus discussion among Hemophilia Comprehensive Care centers (HCCCs) in the Nordic region. Methods: Representatives from six HCCCs sought to pinpoint milestones and key stakeholders for site readiness at the pre-, peri-, and post-infusion stages, including authority and genetically modified organism (GMO) product requirements, awareness, medical eligibility, logistics and product handling for infusion, laboratory monitoring, and follow-up. Results: A gene therapy transit map was developed with key stakeholders identified. The approach to prepare the vector will differ between the Nordic centers, but the contracted pharmacy unit will be a key stakeholder. Therefore, a pharmacy checklist for the implementation of gene therapy was developed. For the future, Advanced Therapy Medicinal Product centers will also be implemented. Patients’ expectations, commitments, and concerns need to be addressed repeatedly and education of patients and the expanded health-care professionals team will be the key to successful and optimal clinical management. Eligibility testing according to the product’s summary of product characteristics and frequent follow-up and monitoring post-infusion according to the World Federation of Hemophilia chart will be crucial. Conclusion: The approach to deliver gene therapy in the Nordic region will differ partly between the hemophilia centers, but the defined road map with checklists for the implementation of this advanced therapy will be applicable to all. The map may also serve as a platform for the use of future GMO product options both within and outside the area of hemophilia.

  • Association between convalescent plasma treatment and mortality in COVID-19 : a collaborative systematic review and meta-analysis of randomized clinical trials

    • Background: Convalescent plasma has been widely used to treat COVID-19 and is under investigation in numerous randomized clinical trials, but results are publicly available only for a small number of trials. The objective of this study was to assess the benefits of convalescent plasma treatment compared to placebo or no treatment and all-cause mortality in patients with COVID-19, using data from all available randomized clinical trials, including unpublished and ongoing trials (Open Science Framework, ). Methods: In this collaborative systematic review and meta-analysis, clinical trial registries (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform), the Cochrane COVID-19 register, the LOVE database, and PubMed were searched until April 8, 2021. Investigators of trials registered by March 1, 2021, without published results were contacted via email. Eligible were ongoing, discontinued and completed randomized clinical trials that compared convalescent plasma with placebo or no treatment in COVID-19 patients, regardless of setting or treatment schedule. Aggregated mortality data were extracted from publications or provided by investigators of unpublished trials and combined using the Hartung-Knapp-Sidik-Jonkman random effects model. We investigated the contribution of unpublished trials to the overall evidence. Results: A total of 16,477 patients were included in 33 trials (20 unpublished with 3190 patients, 13 published with 13,287 patients). 32 trials enrolled only hospitalized patients (including 3 with only intensive care unit patients). Risk of bias was low for 29/33 trials. Of 8495 patients who received convalescent plasma, 1997 died (23%), and of 7982 control patients, 1952 died (24%). The combined risk ratio for all-cause mortality was 0.97 (95% confidence interval: 0.92; 1.02) with between-study heterogeneity not beyond chance (I-2 = 0%). The RECOVERY trial had 69.8% and the unpublished evidence 25.3% of the weight in the meta-analysis. Conclusions: Convalescent plasma treatment of patients with COVID-19 did not reduce all-cause mortality. These results provide strong evidence that convalescent plasma treatment for patients with COVID-19 should not be used outside of randomized trials. Evidence synthesis from collaborations among trial investigators can inform both evidence generation and evidence application in patient care.

  • Media and Basic Desires : An Approach to Measuring the Mediatization of Daily Life

    • The extended indispensability of media can be seen as a key indicator of mediatization, whereby our lives become adapted to media. But even though we can assume that the pervasive character of digital media essentially changes everyday life and the way people experience it, we cannot take consequences of technological transformation for granted. Following from this we argue there is a need for quantitative analyses that can verify mediatization processes in time and space. Here we present a tool that measures the extent to which media reach into the deeper layers of daily human life, used in an empirical study conducted in Sweden in 2017. The results show that perceived media indispensability is played out along three dimensions of daily human life; (1) (re)production; (2) recognition, and (3) civic life, and appears as a socially structured and structuring process, although not in a very strong way. We argue this tool, in diachronic analyses, works as a measurement of mediatization.

There has recently been a formulation of three tasks for mediatization research; historicity, specificity and measurability, needed to empirically verify mediatization processes across time and space. In this article, we present a tool designed to handle these tasks, by measuring the extent to which people experience that media reach into the deeper layers of daily human life. The tool was tested in an empirical study conducted in Sweden in 2017. The results show that perceived media reliance is played out in relation to three types of basic desires: (1) (re)productive desires, (2) recognition desires, and (3) civic desires, and is socially structured and structuring. We argue this tool, in diachronic analyses, can measure one important aspect of mediatization.

    • This study analyzed examples of sustainable ecosystem-based agriculture where management methods supported livelihoods of smallholders while at the same time local ecosystem services were enhanced in Ethiopia, Brazil, and the Philippines. Participation by farmers and collective actions were found to be a crucial driving force, as local specific knowledge and “learning by doing” were main components of the development. Social cohesion, particularly through associations and cooperatives, and improved marketing opportunities were also important drivers. Furthermore, recognition by authorities at all levels was perceived as crucial. Effects of climate change, insecure property rights, and political instability were potential threats. The possibilities of such systems to be scaled up beyond self-sufficiency raised further questions.

    • BackgroundA lack of agreement on definitions and terminology used for nutrition-related concepts and procedures limits the development of clinical nutrition practice and research.ObjectiveThis initiative aimed to reach a consensus for terminology for core nutritional concepts and procedures.MethodsThe European Society of Clinical Nutrition and Metabolism (ESPEN) appointed a consensus group of clinical scientists to perform a modified Delphi process that encompassed e-mail communication, face-to-face meetings, in-group ballots and an electronic ESPEN membership Delphi round.ResultsFive key areas related to clinical nutrition were identified: concepts; procedures; organisation; delivery; and products. One core concept of clinical nutrition is malnutrition/undernutrition, which includes disease-related malnutrition (DRM) with (eq. cachexia) and without inflammation, and malnutrition/undernutrition without disease, e.g. hunger-related malnutrition. Over-nutrition (overweight and obesity) is another core concept. Sarcopenia and frailty were agreed to be separate conditions often associated with malnutrition. Examples of nutritional procedures identified include screening for subjects at nutritional risk followed by a complete nutritional assessment. Hospital and care facility catering are the basic organizational forms for providing nutrition. Oral nutritional supplementation is the preferred way of nutrition therapy but if inadequate then other forms of medical nutrition therapy, i.e. enteral tube feeding and parenteral (intravenous) nutrition, becomes the major way of nutrient delivery.ConclusionAn agreement of basic nutritional terminology to be used in clinical practice, research, and the ESPEN guideline developments has been established. This terminology consensus may help to support future global consensus efforts and updates of classification systems such as the International Classification of Disease (ICD). The continuous growth of knowledge in all areas addressed in this statement will provide the foundation for future revisions.

    • Adaptive immune responses have been studied extensively in the course of mRNA vaccination against COVID-19. Considerably fewer studies have assessed the effects on innate immune cells. Here, we characterized NK cells in healthy individuals and immunocompromised patients in the course of an anti-SARS-CoV-2 BNT162b2 mRNA prospective, open-label clinical vaccine trial. See trial registration description in notes. Results revealed preserved NK cell numbers, frequencies, subsets, phenotypes, and function as assessed through consecutive peripheral blood samplings at 0, 10, 21, and 35 days following vaccination. A positive correlation was observed between the frequency of NKG2C+ NK cells at baseline (Day 0) and anti-SARS-CoV-2 Ab titers following BNT162b2 mRNA vaccination at Day 35. The present results provide basic insights in regards to NK cells in the context of mRNA vaccination, and have relevance for future mRNA-based vaccinations against COVID-19, other viral infections, and cancer.Trial registration: The current study is based on clinical material from the COVAXID open-label, non-randomized prospective clinical trial registered at EudraCT and clinicaltrials.gov (no. 2021–000175-37).     • The consumption of goods and services can be a driver of environmental and social impacts around the world. Understanding the role that the different levels of government can play in incentivising sustainable consumption is therefore critical. Using systematic review techniques, this paper reviews the latest evidence on the importance, effectiveness, successes and failures of local government in advancing sustainable consumption. We find that there is little focus on sustainable consumption in its entirety or whether it is being achieved at the local government level. Important consumption categories like food, procurement, water, waste prevention, clothing, other consumables or services are understudied. Evaluation of the outcome of sustainable consumption interventions was limited, and the assessment that was completed gave mixed results. The most popular policy instruments were of the less coercive administrative and informative type. Multiple barriers to the success of an intervention were identified, the top ones being funding; staff capacity, knowledge or data; lack of flexibility and lock-in to the status quo; lack of guidance or political will; administrative burdens; and lack of regulatory powers or tools. Sustainable consumption interventions by local government were most effective when they had strong leadership, good stakeholder engagement, participatory approaches and extensive consultations.

  • Retracing the footsteps: how do footprint indicators support learning about sustainable consumption among Swedish policymakers?

    • Consumption-based or environmental footprint indicators give a sense of society’s progress towards sustainable consumption. Studies of the role that sustainability indicators play in policy making report that they contribute to learning and conceptual thinking. This literature provides insights into the types of learning outcome that indicators contribute to, such as instrumental, policy-oriented, governmental, political, or societal learning. But few studies have looked specifically at consumption-based indicators or at the learning process itself, of how indicator use supports different actors in changing their ideas, perceptions, and practices – and potentially affects wider social and organizational structures to prompt the desired move to sustainable consumption.To address this, we draw on the theory of expansive learning to investigate the potential for learning about sustainable consumption by Swedish public officials using consumption-based indicators. Data were collected in a series of interviews, focus groups and workshops. The results suggest that consumption-based indicators do help officials to learn about the concept of sustainable consumption and encourage them to push forward the sustainable consumption agenda. This is not, however, so much due to indicators per se, but rather to the creativity and agency of committed government officials. To enhance learning and change in practices further, public officials must be supported by the necessary institutions and authority to promote sustainable consumption.

  • Cost optimization of biofuel production – The impact of scale, integration, transport and supply chain configurations

    • This study uses a geographically-explicit cost optimization model to analyze the impact of and interrelation between four cost reduction strategies for biofuel production: economies of scale, intermodal transport, integration with existing industries, and distributed supply chain configurations (i.e. supply chains with an intermediate pre-treatment step to reduce biomass transport cost). The model assessed biofuel production levels ranging from 1 to 150 PJ a−1 in the context of the existing Swedish forest industry. Biofuel was produced from forestry biomass using hydrothermal liquefaction and hydroprocessing. Simultaneous implementation of all cost reduction strategies yielded minimum biofuel production costs of 18.1–18.2 € GJ−1 at biofuel production levels between 10 and 75 PJ a−1. Limiting the economies of scale was shown to cause the largest cost increase (+0–12%, increasing with biofuel production level), followed by disabling integration benefits (+1–10%, decreasing with biofuel production level) and allowing unimodal truck transport only (+0–6%, increasing with biofuel production level). Distributed supply chain configurations were introduced once biomass supply became increasingly dispersed, but did not provide a significant cost benefit (<1%). Disabling the benefits of integration favors large-scale centralized production, while intermodal transport networks positively affect the benefits of economies of scale. As biofuel production costs still exceeds the price of fossil transport fuels in Sweden after implementation of all cost reduction strategies, policy support and stimulation of further technological learning remains essential to achieve cost parity with fossil fuels for this feedstock/technology combination in this spatiotemporal context. © 2017 The Authors

    • Educational attainment is widely used as a surrogate for socioeconomic status (SES). Low SES is a risk factor for hypertension and high blood pressure (BP). To identify novel BP loci, we performed multi-ancestry meta-analyses accounting for gene-educational attainment interactions using two variables, “Some College” (yes/no) and “Graduated College” (yes/no). Interactions were evaluated using both a 1 degree of freedom (DF) interaction term and a 2DF joint test of genetic and interaction effects. Analyses were performed for systolic BP, diastolic BP, mean arterial pressure, and pulse pressure. We pursued genome-wide interrogation in Stage 1 studies (N = 117 438) and follow-up on promising variants in Stage 2 studies (N = 293 787) in five ancestry groups. Through combined meta-analyses of Stages 1 and 2, we identified 84 known and 18 novel BP loci at genome-wide significance level (P < 5 × 10-8). Two novel loci were identified based on the 1DF test of interaction with educational attainment, while the remaining 16 loci were identified through the 2DF joint test of genetic and interaction effects. Ten novel loci were identified in individuals of African ancestry. Several novel loci show strong biological plausibility since they involve physiologic systems implicated in BP regulation. They include genes involved in the central nervous system-adrenal signaling axis (ZDHHC17, CADPS, PIK3C2G), vascular structure and function (GNB3, CDON), and renal function (HAS2 and HAS2-AS1, SLIT3). Collectively, these findings suggest a role of educational attainment or SES in further dissection of the genetic architecture of BP.

    • A person's lipid profile is influenced by genetic variants and alcohol consumption, but the contribution of interactions between these exposures has not been studied. We therefore incorporated gene-alcohol interactions into a multiancestry genome-wide association study of levels of high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, and triglycerides. We included 45 studies in stage 1 (genome-wide discovery) and 66 studies in stage 2 (focused follow-up), for a total of 394,584 individuals from 5 ancestry groups. Analyses covered the period July 2014-November 2017. Genetic main effects and interaction effects were jointly assessed by means of a 2-degrees-of-freedom (df) test, and a 1-df test was used to assess the interaction effects alone. Variants at 495 loci were at least suggestively associated (P < 1 x 10(-6)) with lipid levels in stage 1 and were evaluated in stage 2, followed by combined analyses of stage 1 and stage 2. In the combined analysis of stages 1 and 2, a total of 147 independent loci were associated with lipid levels at P < 5 x 10(-8) using 2-df tests, of which 18 were novel. No genome-wide-significant associations were found testing the interaction effect alone. The novel loci included several genes (proprotein convertase subtilisin/kexin type 5 (PCSK5), vascular endothelial growth factor B (VEGFB), and apolipoprotein B mRNA editing enzyme, catalytic polypeptide 1 (APOBEC1) complementation factor (A1CF)) that have a putative role in lipid metabolism on the basis of existing evidence from cellular and experimental models.

    • GrassPlot is a collaborative vegetation-plot database organised by the Eurasian Dry Grassland Group (EDGG) and listed in the Global Index of Vegetation-Plot Databases (GIVD ID EU-00-003). GrassPlot collects plot records (releves) from grasslands and other open habitats of the Palaearctic biogeographic realm. It focuses on precisely delimited plots of eight standard grain sizes (0.0001; 0.001;... 1,000 m(2)) and on nested-plot series with at least four different grain sizes. The usage of GrassPlot is regulated through Bylaws that intend to balance the interests of data contributors and data users. The current version (v. 1.00) contains data for approximately 170,000 plots of different sizes and 2,800 nested-plot series. The key components are richness data and metadata. However, most included datasets also encompass compositional data. About 14,000 plots have near-complete records of terricolous bryophytes and lichens in addition to vascular plants. At present, GrassPlot contains data from 36 countries throughout the Palaearctic, spread across elevational gradients and major grassland types. GrassPlot with its multi-scale and multi-taxon focus complements the larger international vegetationplot databases, such as the European Vegetation Archive (EVA) and the global database " sPlot". Its main aim is to facilitate studies on the scale-and taxon-dependency of biodiversity patterns and drivers along macroecological gradients. GrassPlot is a dynamic database and will expand through new data collection coordinated by the elected Governing Board. We invite researchers with suitable data to join GrassPlot. Researchers with project ideas addressable with GrassPlot data are welcome to submit proposals to the Governing Board.

    • Purpose : To investigate the extent and time axis of sarcomere remodeling and of gene expression profile changes following resection surgery in an animal model of strabismus surgery.Methods : The right superior rectus (SR) of 16 adult New Zealand white rabbits was resected 4 mm and reattached to the sclera, with ethical permission and following the animal care directives. The superior rectus muscle of 4 rabbits was collected 1, 2, 4 and 6 weeks after surgery. The SR of 4 control rabbits was also collected. The muscles were divided into two pieces longitudinally and one half was directly frozen for RNA extraction and the other half was stretched, fixed in 2% paraformaldehyde and frozen after sucrose cryoprotection. Serial longitudinal sections were processed for immunohistochemistry with antibodies against desmin. For each muscle section, the area comprising exclusively longitudinally sectioned myofibers was evaluated and the number of dividing sarcomeres present within that area was determined. RNA sequencing was performed with Illumina HiSeq 2500.Results : One week after surgery, the number of sarcomere divisions was 86.5/mm2 (range 30.9-152.7), after two weeks 72.0/ mm2 (42.5-95.9), after 4 weeks 95.7/ mm2 (37.4-161.3). After 6 weeks the number of sarcomere divisions (26.8/ mm2, 9.2-60.7) was similar to that of the control samples (26.0/ mm2, 6.0-66.9). RNA sequencing revealed up to 198 differentially expressed genes and further bioinformatics analysis is ongoing. Preliminary data indicate that the most significantly altered biological processes are those involved in extracellular matrix organization and inflammation, along with regulation of response and production of growth factors involved in muscle repair and regeneration.Conclusions : Signs of sarcomerogenesis were present during the first 4 weeks after resection of the superior rectus, suggesting that sarcomerogenesis plays a role in surgical failure due to recovery of muscle length. We suggest that medical approaches to limit this mechanism may be a desirable complementary therapy to strabismus surgery in the future.
