    • BackgroundCross-sectional and longitudinal reports show that obese adults have more asthma than non-obese adults. A proposed mechanism is via effects of adipokines (leptin and adiponectin) on the immune system. ObjectiveWe wished to measure the associations of asthma and other atopic diseases with serum adipokine levels and to find whether the associations with asthma were strong enough to rule out the possibility that they are secondary to the association of fatness measures with asthma. MethodsThe Global Asthma and Allergy Network of Excellence (GA(2)LEN) clinical follow-up survey is a clinical survey, embedded in a larger multi-centre cross-sectional postal survey, involving, with a case/control design, enrichment of the sample with subjects with asthma and chronic rhinosinusitis (CRS). We recorded serum leptin or adiponectin in 845 men and 1110 women in 15 centres and also anthropometric measures of fatness including body mass index and waist/hip ratio, current asthma, and specific skin prick and IgE sensitisation. We used inverse sampling-probability-weighted rank and regression statistics to measure population associations of disease outcomes with adipokines in males and females, adjusting for confounders (area, age, smoking history, and number of elder siblings) and also mutually adjusting associations with adipokines and fatness measures. ResultsOne thousand nine hundred and fifty-five subjects aged 16-77years had information on leptin or adiponectin levels. Leptin and leptin/adiponectin ratio were positively associated with the level of asthma, especially in females (Somers' D of leptin by asthma score, 0.20; 95% CI, 0.08-0.30; P=0.00079). These associations were attenuated after adjusting for confounders and became non-significant after additionally adjusting for fatness measures and multiple comparisons. Conclusions and Clinical RelevanceAsthma levels are positively associated with serum leptin. However, we cannot rule out the possibility that this association is secondary to associations of both with fatness measures.

  • The Global Methane Budget 2000–2017

    • Understanding and quantifying the global methane (CH4) budget is important for assessing realistic pathways to mitigate climate change. Atmospheric emissions and concentrations of CH4 continue to increase, making CH4 the second most important human-influenced greenhouse gas in terms of climate forcing, after carbon dioxide (CO2). The relative importance of CH4 compared to CO2 depends on its shorter atmospheric lifetime, stronger warming potential, and variations in atmospheric growth rate over the past decade, the causes of which are still debated. Two major challenges in reducing uncertainties in the atmospheric growth rate arise from the variety of geographically overlapping CH4 sources and from the destruction of CH4 by short-lived hydroxyl radicals (OH). To address these challenges, we have established a consortium of multidisciplinary scientists under the umbrella of the Global Carbon Project to synthesize and stimulate new research aimed at improving and regularly updating the global methane budget. Following Saunois et al. (2016), we present here the second version of the living review paper dedicated to the decadal methane budget, integrating results of top-down studies (atmospheric observations within an atmospheric inverse-modelling framework) and bottom-up estimates (including process-based models for estimating land surface emissions and atmospheric chemistry, inventories of anthropogenic emissions, and data-driven extrapolations).For the 2008–2017 decade, global methane emissions are estimated by atmospheric inversions (a top-down approach) to be 576 Tg CH4 yr−1 (range 550–594, corresponding to the minimum and maximum estimates of the model ensemble). Of this total, 359 Tg CH4 yr−1 or ∼ 60 % is attributed to anthropogenic sources, that is emissions caused by direct human activity (i.e. anthropogenic emissions; range 336–376 Tg CH4 yr−1 or 50 %–65 %). The mean annual total emission for the new decade (2008–2017) is 29 Tg CH4 yr−1 larger than our estimate for the previous decade (2000–2009), and 24 Tg CH4 yr−1 larger than the one reported in the previous budget for 2003–2012 (Saunois et al., 2016). Since 2012, global CH4 emissions have been tracking the warmest scenarios assessed by the Intergovernmental Panel on Climate Change. Bottom-up methods suggest almost 30 % larger global emissions (737 Tg CH4 yr−1, range 594–881) than top-down inversion methods. Indeed, bottom-up estimates for natural sources such as natural wetlands, other inland water systems, and geological sources are higher than top-down estimates. The atmospheric constraints on the top-down budget suggest that at least some of these bottom-up emissions are overestimated. The latitudinal distribution of atmospheric observation-based emissions indicates a predominance of tropical emissions (∼ 65 % of the global budget, < 30∘ N) compared to mid-latitudes (∼ 30 %, 30–60∘ N) and high northern latitudes (∼ 4 %, 60–90∘ N). The most important source of uncertainty in the methane budget is attributable to natural emissions, especially those from wetlands and other inland waters.Some of our global source estimates are smaller than those in previously published budgets (Saunois et al., 2016; Kirschke et al., 2013). In particular wetland emissions are about 35 Tg CH4 yr−1 lower due to improved partition wetlands and other inland waters. Emissions from geological sources and wild animals are also found to be smaller by 7 Tg CH4 yr−1 by 8 Tg CH4 yr−1, respectively. However, the overall discrepancy between bottom-up and top-down estimates has been reduced by only 5 % compared to Saunois et al. (2016), due to a higher estimate of emissions from inland waters, highlighting the need for more detailed research on emissions factors. Priorities for improving the methane budget include (i) a global, high-resolution map of water-saturated soils and inundated areas emitting methane based on a robust classification of different types of emitting habitats; (ii) further development of process-based models for inland-water emissions; (iii) intensification of methane observations at local scales (e.g., FLUXNET-CH4 measurements) and urban-scale monitoring to constrain bottom-up land surface models, and at regional scales (surface networks and satellites) to constrain atmospheric inversions; (iv) improvements of transport models and the representation of photochemical sinks in top-down inversions; and (v) development of a 3D variational inversion system using isotopic and/or co-emitted species such as ethane to improve source partitioning.The data presented here can be downloaded from https://doi.org/10.18160/GCP-CH4-2019 (Saunois et al., 2020) and from the Global Carbon Project.

  • The Influence of Remote Aerosol Forcing from Industrialized Economies on the Future Evolution of East and West African Rainfall

    • Past changes in global industrial aerosol emissions have played a significant role in historical shifts in African rainfall, and yet assessment of the impact on African rainfall of near-term (10-40 yr) potential aerosol emission pathways remains largely unexplored. While existing literature links future aerosol declines to a northward shift of Sahel rainfall, existing climate projections rely on RCP scenarios that do not explore the range of air quality drivers. Here we present projections from two emission scenarios that better envelop the range of potential aerosol emissions. More aggressive emission cuts result in northward shifts of the tropical rainbands whose signal can emerge from expected internal variability on short, 10-20-yr time horizons. We also show for the first time that this northward shift also impacts East Africa, with evidence of delays to both onset and withdrawal of the short rains. However, comparisons of rainfall impacts across models suggest that only certain aspects of both the West and East African model responses may be robust, given model uncertainties. This work motivates the need for wider exploration of air quality scenarios in the climate science community to assess the robustness of these projected changes and to provide evidence to underpin climate adaptation in Africa. In particular, revised estimates of emission impacts of legislated measures every 5-10 years would have a value in providing near-term climate adaptation information for African stakeholders.

  • Tension-free mesh versus suture-alone cruroplasty in antireflux surgery : a randomized, double-blind clinical trial

    • BackgroundAntireflux surgery is effective for the treatment of gastro-oesophageal reflux disease (GORD) but recurrence of hiatal hernia remains a challenge. In other types of hernia repair, use of mesh is associated with reduced recurrence rates. The aim of this study was to compare the use of mesh versus sutures alone for the repair of hiatal hernia in laparoscopic antireflux surgery.MethodsPatients undergoing laparoscopic Nissen fundoplication for GORD between January 2006 and December 2010 were allocated randomly to closure of the diaphragmatic hiatus with crural sutures or non-absorbable polytetrafluoroethylene mesh (CruraSoft®). The primary outcome was recurrence of hiatal hernia, as determined by barium swallow study 12 months after surgery. Secondary outcomes were: intraoperative and postoperative complications, use of antireflux medication, postoperative oesophageal acid exposure, quality of life, dysphagia and duration of hospital stay.ResultsSome 77 patients were randomized to the suture technique and 82 patients underwent mesh repair. At 1 year, the hiatal hernia had recurred in six of 64 patients (9 per cent) in the mesh group and two of 64 (3 per cent) in the suture group (P = 0·144). Reflux symptoms, use of proton pump inhibitors and oesophageal acid exposure did not differ between the groups. At 3 years, recurrence rates were 13 and 10 per cent in the mesh and suture groups respectively (P = 0·692). Dysphagia scores decreased in both groups, but more patients had dysphagia for solid food after mesh closure (P = 0·013). Quality-of-life scores were comparable between the groups.ConclusionTension-free crural repair with non-absorbable mesh does not reduce the incidence of recurrent hiatal hernia compared with use of sutures alone in patients undergoing laparoscopic fundoplication.     • The "Ice Giants" Uranus and Neptune are a different class of planet compared to Jupiter and Saturn. Studying these objects is important for furthering our understanding of the formation and evolution of the planets, and unravelling the fundamental physical and chemical processes in the Solar System. The importance of filling these gaps in our knowledge of the Solar System is particularly acute when trying to apply our understanding to the numerous planetary systems that have been discovered around other stars. The Uranus Pathfinder (UP) mission thus represents the quintessential aspects of the objectives of the European planetary community as expressed in ESA's Cosmic Vision 2015-2025. UP was proposed to the European Space Agency's M3 call for medium-class missions in 2010 and proposed to be the first orbiter of an Ice Giant planet. As the most accessible Ice Giant within the M-class mission envelope Uranus was identified as the mission target. Although not selected for this call the UP mission concept provides a baseline framework for the exploration of Uranus with existing low-cost platforms and underlines the need to develop power sources suitable for the outer Solar System. The UP science case is based around exploring the origins, evolution, and processes at work in Ice Giant planetary systems. Three broad themes were identified: (1) Uranus as an Ice Giant, (2) An Ice Giant planetary system, and (3) An asymmetric magnetosphere. Due to the long interplanetary transfer from Earth to Uranus a significant cruise-phase science theme was also developed. The UP mission concept calls for the use of a Mars Express/Rosetta-type platform to launch on a Soyuz-Fregat in 2021 and entering into an eccentric polar orbit around Uranus in the 2036-2037 timeframe. The science payload has a strong heritage in Europe and beyond and requires no significant technology developments.

    • The relevance of considering environmental variability for understanding and predicting biological responses to environmental changes has resulted in a recent surge in variability-focused ecological research. However, integration of findings that emerge across studies and identification of remaining knowledge gaps in aquatic ecosystems remain critical. Here, we address these aspects by: (1) summarizing relevant terms of variability research including the components (characteristics) of variability and key interactions when considering multiple environmental factors; (2) identifying conceptual frameworks for understanding the consequences of environmental variability in single and multifactorial scenarios; (3) highlighting challenges for bridging theoretical and experimental studies involving transitioning from simple to more complex scenarios; (4) proposing improved approaches to overcome current mismatches between theoretical predictions and experimental observations; and (5) providing a guide for designing integrated experiments across multiple scales, degrees of control, and complexity in light of their specific strengths and limitations.

  • Effects of Different Types of Lower Body Resistance Exercise on Upper-body Strength in Men and Women, with Special Reference to Anabolic Hormones

    • Resistance exercise has been shown to induce an acute hormonal response. The purpose of this study was to examine upper-body adaptations and the endocrine response to strength training in men and women when subjected to two different types of lower-body resistance training protocols. Nine males and eight females were assigned to either a Heavy Load (HL) (N = 10) or Mixed Load (ML) (N = 7) training routine three times per week for ten weeks. The HL-group executed low-volume, high-load resistance exercise for both lower and upper-body (4-6 reps at 80-90% of one repetition maximum (1-RM), three-minute inter-set rest). The ML-group performed the HL-protocol for the upper-body, but a high-volume, moderate-load protocol for the lower body (10-15 reps at 60-70% of 1-RM, one-minute inter-set rest). Volume load, 1-RM strength and hormonal measurements were analyzed by repeated-measures linear mixed models. Both groups increased their 1-RM in all assessments (p < 0.01) with no significant difference between groups at any time. Growth hormone (GH), testosterone and bioavailable testosterone (T/SHBG) increased in both groups during a standardized exercise session (p < 0.01) with ML having a greater increase in GH. The notion that acute elevations in anabolic hormones is important for muscle strength adaptation cannot be supported by the present study.

    • The vast majority of gastrin-related gastrointestinal neuroendocrine neoplasms (GI-NENs) develop in the context of chronic atrophic gastritis (type 1), a condition closely related to autoimmune thyroid diseases. These neoplasms are defined as gastric NENs type 1 (GNEN1) and have recently been shown to constitute the commonest GI-NENs in a prospective study. GNEN1s are usually multiple and follow a relative indolent course, raising questions regarding the extent that such patients should be investigated and the appropriate therapeutic interventions needed. Recently, a number of consensus statements and guidelines have been published from various societies dealing with the diagnosis and management of GI-NENs. Endocrinologists are among the many different medical specialties involved in GNEN1s diagnosis and management. However, despite recent advances, few randomized trials are available, and thus existing evidence remains relatively weak compared to other malignancies. The purpose of this review is to provide recent evidence along with currently employed modalities addressing the diagnosis, management, long-term follow-up and potential comorbidities of GNEN1s.

  • Capture of satellites during planetary encounters : A case study of the Neptunian moons Triton and Nereid

    • Context. Single-binary scattering may lead to an exchange where the single object captures a component of the binary, forming a new binary. This has been well studied in encounters between a star-planet pair and a single star. Aims. Here we explore the application of the exchange mechanism to a planet-satellite pair and another planet in the gravitational potential of a central star. As a case study, we focus on encounters between a satellite-bearing object and Neptune. We investigate whether Neptune can capture satellites from that object and if the captured satellites have orbits analogous to the Neptunian moons Triton and Nereid. Methods. Using N-body simulations, we study the capture probability at different encounter distances. Post-capture, we use a simple analytical argument to estimate how the captured orbits evolve under collisional and tidal effects. Results. We find that the average capture probability reaches ~10% if Neptune penetrates the donor planet's satellite system. Most moons grabbed by Neptune acquire highly eccentric orbits. Post-capture, around half of those captured, especially those on tight orbits, can be circularised, either by tides only or by collisions+tides, turning into Triton-like objects. Captures further out, on the other hand, stay on wide and eccentric orbits like that of Nereid. Both moon types can be captured in the same encounter and they have wide distributions in orbital inclination. Therefore, Triton naturally has a ~50% chance of being retrograde. Conclusions. A similar process potentially applies to an exoplanetary system, and our model predicts that exomoons can jump from one planet to another during planetary scattering. Specifically, there should be two distinct populations of captured moons: one on close-in circular orbits and the other on far-out eccentric orbits. The two populations may have highly inclined prograde or retrograde orbits.

    • Histone lysine methylation, mediated by mixed-lineage leukemia (MLL) proteins, is now known to be critical in the regulation of gene expression, genomic stability, cell cycle and nuclear architecture. Despite MLL proteins being postulated as essential for normal development, little is known about the specific functions of the different MLL lysine methyltransferases. Here we report heterozygous variants in the gene KMT2B (also known as MLL4) in 27 unrelated individuals with a complex progressive childhood-onset dystonia, often associated with a typical facial appearance and characteristic brain magnetic resonance imaging findings. Over time, the majority of affected individuals developed prominent cervical, cranial and laryngeal dystonia. Marked clinical benefit, including the restoration of independent ambulation in some cases, was observed following deep brain stimulation (DBS). These findings highlight a clinically recognizable and potentially treatable form of genetic dystonia, demonstrating the crucial role of KMT2B in the physiological control of voluntary movement.

  • Competitive performance prediction of elite alpine skiers

    • Introduction: The overall aim of this doctoral thesis was to identify physiological and anthropometric variables valid for prediction of competitive performance in alpine skiing (indicated by FIS points).Method: Paper I-III in this doctoral thesis followed an experimental, hypothesis-generating design which included both junior and senior elite alpine skiers. In all papers, physiological and anthropometric test results (X-variables) were correlated with FIS points (Y-variables) in order to investigate the predictive power of physiological and anthropometric variables for competitive performance in alpine skiing. The significance of the included test results was examined using bivariate and multivariate data analysis.Results: The results of Paper I show that included aerobic test results, neither alone nor in combination with anthropometric variables, could predict competitive performance of junior elite alpine skiers. Principal component analysis shows that male and female junior alpine skiers could be separated based on test results but that none of the included tests were important for sport-specific performance. The best multivariate models reached R2 = 0.51 to 0.86 and Q2 = -0.73 to 0.18. While several significant regression models could be observed, none of these met the criteria for valid models. The lack of predictive power of observed prediction models was confirmed by cross-validation. The results of Paper II show that included physiological test results from the test battery Fysprofilen could not predict competitive performance of senior elite female alpine skiers. Principal component analysis shows that there is a high correlation between individual physiological test results and their corresponding Fysprofilen score points, indicating that they can be used interchangeably. The Mann-Whitney U test was not significant neither for SL nor for GS. This suggests that Fysprofilen score points (summarized as Fysprofilen Index) and competitive performance (indicated by FIS points) are independent. The best multivariate models for SL and GS reached R2 = 0.27 to 0.43 and Q2 = - 0.8 to - 0.17, indicating low predictive power for competitive performance (as confirmed by cross-validation). The results of Paper III show that included physiological test results from a novel test battery could not predict competitive performance of senior elite female alpine skiers on a group level. When data were analyzed on a group level, the best models for SL and GS reached R2 = 0.39 to 0.40, Q2 = 0.15 to 0.21, indicating low predictive power. In contrast, when data were analyzed on an individual level, valid models with high predictive power (R2 = 0.88 to 0.99 and Q2 = 0.64 to 0.96) were generated. A comparative analysis between individual multivariate models shows that the relative importance of different physiological qualities for athletic performance varies between skiers.Conclusion: When applying tests on alpine skiers, a holistic approach should be considered. This because competitive performance in alpine skiing is the result of a number of interacting dimensions. Before applying physiological tests, the validity and reliability of the test protocols must also be determined. Administering tests that do not meet these criteria will probably waste not only important resources for clubs and ski federations but also risk misleading coaches and athletes when planning and implementing preparatory training.

  • Lack of Predictive Power in Commonly Used Tests for Performance in Alpine Skiing

    • Competitive alpine skiing is a complex sport that requires high physical and technical competence. Testing the physical status of athletes may be important to increase their ability to achieve elite sport-specific performance. This study aimed to investigate the predictive power of the national test battery of the Swedish Olympic Committee (Fysprofilen) and anthropometric variables in the prediction of competitive performance of elite alpine skiers, indicated by Fédération Internationale de Ski points. Data from fourteen Swedish elite female alpine skiers were analyzed using bivariate and multivariate statistical methods. Physiological test results and anthropometric data could not generate significant bivariate or multivariate models for prediction of competitive performance. Multivariate regression (R2) and prediction (Q2) models for Fédération Internationale de Ski Slalom and Giant Slalom rank reached R2=0.27 to 0.43, Q2=+− 0.8 to−0.17, indicating no valid models. The overall interpretation of these and previous findings are that future test batteries must be validated before implemented, and that test results should be treated with caution when it comes to prediction of future competitive results. Applying tests that are not validated against competitive performance risk misleading coaches and training advisors who aim to increase the sports-specific performance of the individual athlete.

    • Viral infections of the respiratory tract are the most common precipitants of acute asthma exacerbations. Exacerbations are only poorly responsive to current asthma therapies and new approaches to therapy are needed. Viruses, most frequently human rhinoviruses (RV), infect the airway epithelium, generate local and systemic immune responses, as well as neural responses, inducing inflammation and airway hyperresponsiveness. Using in vitro and in vivo experimental models the role of various proinflammatory or anti-inflammatory mediators, antiviral responses and molecular pathways that lead from infection to symptoms has been partly unravelled. In particular, mechanisms of susceptibility to viral infection have been identified and the bronchial epithelium appeared to be a key player. Nevertheless, additional understanding of the integration between the diverse elements of the antiviral response, especially in the context of allergic airway inflammation, as well as the interactions between viral infections and other stimuli that affect airway inflammation and responsiveness may lead to novel strategies in treating and/or preventing asthma exacerbations. This review presents the current knowledge and highlights areas in need of further research.

ABSTRACT: A major part of the burden of asthma is caused by acute exacerbations. Exacerbations have been strongly and consistently associated with respiratory infections. Respiratory viruses and bacteria are therefore possible treatment targets. To have a reasonable estimate of the burden of disease induced by such infectious agents on asthmatic patients, it is necessary to understand their nature and be able to identify them in clinical samples by employing accurate and sensitive methodologies. This systematic review summarizes current knowledge and developments in infection epidemiology of acute asthma in children and adults, describing the known impact for each individual agent and highlighting knowledge gaps. Among infectious agents, human rhinoviruses are the most prevalent in regard to asthma exacerbations. The newly identified type-C rhinoviruses may prove to be particularly relevant. Respiratory syncytial virus and metapneumovirus are important in infants, while influenza viruses seem to induce severe exacerbations mostly in adults. Other agents are relatively less or not clearly associated. Mycoplasma and Chlamydophila pneumoniae seem to be involved more with asthma persistence rather than with disease exacerbations. Recent data suggest that common bacteria may also be involved, but this should be confirmed. Although current information is considerable, improvements in detection methodologies, as well as the wide variation in respect to location, time and populations, underline the need for additional studies that should also take into account interacting factors.

  • Airway allergen exposure stimulates bone marrow eosinophilia partly via IL-9

    • BACKGROUND: Interleukin (IL)-9 is a Th2-derived cytokine with pleiotropic biological effects, which recently has been proposed as a candidate gene for asthma and allergy. We aimed to evaluate the therapeutic effect of a neutralizing anti-IL-9 antibody in a mouse model of airway eosinophilic inflammation and compared any such effect with anti-IL-5 treatment. METHODS: OVA-sensitized Balb/c mice were intraperitoneally pretreated with a single dose (100 microg) of an anti-mouse IL-9 monoclonal antibody (clone D9302C12) or its vehicle. A third group was given 50 microg of a monoclonal anti-mouse IL-5 antibody (TRFK-5) or its vehicle. Animals were subsequently exposed to OVA on five days via airways. Newly produced eosinophils were labelled using 5-bromo-2'-deoxyuridine (BrdU). BrdU+ eosinophils and CD34+ cell numbers were examined by immunocytochemistry. After culture and stimulation with OVA or PMA+IC, intracellular staining of IL-9 in bone marrow cells from OVA-exposed animals was measured by Flow Cytometry. The Mann-Whitney U-test was used to determine significant differences between groups. RESULTS: Anti-IL-9 significantly reduced bone marrow eosinophilia, primarily by decrease of newly produced (BrdU+) and mature eosinophils. Anti-IL-9 treatment also reduced blood neutrophil counts, but did not affect BAL neutrophils. Anti-IL-5 was able to reduce eosinophil numbers in all tissue compartments, as well as BrdU+ eosinophils and CD34+ progenitor cells, and in all instances to a greater extent than anti-IL-9. Also, FACS analysis showed that IL-9 is over-expressed in bone marrow CD4+ cells after allergen exposure. CONCLUSIONS: Our data shows that a single dose of a neutralizing IL-9 antibody is not sufficient to reduce allergen-induced influx of newly produced cells from bone marrow to airways. However, in response to allergen, bone marrow cells over-express IL-9. This data suggest that IL-9 may participate in the regulation of granulocytopoiesis in allergic inflammation.
