  • The global burden of cancer attributable to risk factors, 2010-19: a systematic analysis for the Global Burden of Disease Study 2019

    • Background Understanding the magnitude of cancer burden attributable to potentially modifiable risk factors is crucial for development of effective prevention and mitigation strategies. We analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 to inform cancer control planning efforts globally. Methods The GBD 2019 comparative risk assessment framework was used to estimate cancer burden attributable to behavioural, environmental and occupational, and metabolic risk factors. A total of 82 risk-outcome pairs were included on the basis of the World Cancer Research Fund criteria. Estimated cancer deaths and disability-adjusted life-years (DALYs) in 2019 and change in these measures between 2010 and 2019 are presented. Findings Globally, in 2019, the risk factors included in this analysis accounted for 4.45 million (95% uncertainty interval 4.01-4.94) deaths and 105 million (95.0-116) DALYs for both sexes combined, representing 44.4% (41.3-48.4) of all cancer deaths and 42.0% (39.1-45.6) of all DALYs. There were 2.88 million (2.60-3.18) risk-attributable cancer deaths in males (50.6% [47.8-54.1] of all male cancer deaths) and 1.58 million (1.36-1.84) risk-attributable cancer deaths in females (36.3% [32.5-41.3] of all female cancer deaths). The leading risk factors at the most detailed level globally for risk-attributable cancer deaths and DALYs in 2019 for both sexes combined were smoking, followed by alcohol use and high BMI. Risk-attributable cancer burden varied by world region and Socio-demographic Index (SDI), with smoking, unsafe sex, and alcohol use being the three leading risk factors for risk-attributable cancer DALYs in low SDI locations in 2019, whereas DALYs in high SDI locations mirrored the top three global risk factor rankings. From 2010 to 2019, global risk-attributable cancer deaths increased by 20.4% (12.6-28.4) and DALYs by 16.8% (8.8-25.0), with the greatest percentage increase in metabolic risks (34.7% [27.9-42.8] and 33.3% [25.8-42.0]). Interpretation The leading risk factors contributing to global cancer burden in 2019 were behavioural, whereas metabolic risk factors saw the largest increases between 2010 and 2019. Reducing exposure to these modifiable risk factors would decrease cancer mortality and DALY rates worldwide, and policies should be tailored appropriately to local cancer risk factor burden.     • Background In estimating the global burden of cancer, adolescents and young adults with cancer are often overlooked, despite being a distinct subgroup with unique epidemiology, clinical care needs, and societal impact. Comprehensive estimates of the global cancer burden in adolescents and young adults (aged 15-39 years) are lacking. To address this gap, we analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, with a focus on the outcome of disability-adjusted life-years (DALYs), to inform global cancer control measures in adolescents and young adults. Methods Using the GBD 2019 methodology, international mortality data were collected from vital registration systems, verbal autopsies, and population-based cancer registry inputs modelled with mortality-to-incidence ratios (MIRs). Incidence was computed with mortality estimates and corresponding MIRs. Prevalence estimates were calculated using modelled survival and multiplied by disability weights to obtain years lived with disability (YLDs). Years of life lost (YLLs) were calculated as age-specific cancer deaths multiplied by the standard life expectancy at the age of death. The main outcome was DALYs (the sum of YLLs and YLDs). Estimates were presented globally and by Socio-demographic Index (SDI) quintiles (countries ranked and divided into five equal SDI groups), and all estimates were presented with corresponding 95% uncertainty intervals (UIs). For this analysis, we used the age range of 15-39 years to define adolescents and young adults. Findings There were 1.19 million (95% UI 1.11-1.28) incident cancer cases and 396 000 (370 000-425 000) deaths due to cancer among people aged 15-39 years worldwide in 2019. The highest age-standardised incidence rates occurred in high SDI (59.6 [54.5-65.7] per 100 000 person-years) and high-middle SDI countries (53.2 [48.8-57.9] per 100 000 person-years), while the highest age-standardised mortality rates were in low-middle SDI (14.2 [12.9-15.6] per 100 000 person-years) and middle SDI (13.6 [12.6-14.8] per 100 000 person-years) countries. In 2019, adolescent and young adult cancers contributed 23.5 million (21.9-25.2) DALYs to the global burden of disease, of which 2.7% (1.9-3.6) came from YLDs and 97.3% (96.4-98.1) from YLLs. Cancer was the fourth leading cause of death and tenth leading cause of DALYs in adolescents and young adults globally. Interpretation Adolescent and young adult cancers contributed substantially to the overall adolescent and young adult disease burden globally in 2019. These results provide new insights into the distribution and magnitude of the adolescent and young adult cancer burden around the world.     • Background Diabetes, particularly type 1 diabetes, at younger ages can be a largely preventable cause of death with the correct health care and services. We aimed to evaluate diabetes mortality and trends at ages younger than 25 years globally using data from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019. Methods We used estimates of GBD 2019 to calculate international diabetes mortality at ages younger than 25 years in 1990 and 2019. Data sources for causes of death were obtained from vital registration systems, verbal autopsies, and other surveillance systems for 1990-2019. We estimated death rates for each location using the GBD Cause of Death Ensemble model. We analysed the association of age-standardised death rates per 100 000 population with the Socio-demographic Index (SDI) and a measure of universal health coverage (UHC) and described the variability within SDI quintiles. We present estimates with their 95% uncertainty intervals. Findings In 2019, 16 300 (95% uncertainty interval 14 200 to 18 900) global deaths due to diabetes (type 1 and 2 combined) occurred in people younger than 25 years and 73.7% (68.3 to 77.4) were classified as due to type 1 diabetes. The age-standardised death rate was 0.50 (0.44 to 0.58) per 100 000 population, and 15 900 (97.5%) of these deaths occurred in low to high-middle SDI countries. The rate was 0.13 (0.12 to 0.14) per 100 000 population in the high SDI quintile, 0.60 (0.51 to 0.70) per 100 000 population in the low-middle SDI quintile, and 0.71 (0.60 to 0.86) per 100 000 population in the low SDI quintile. Within SDI quintiles, we observed large variability in rates across countries, in part explained by the extent of UHC (r(2)=0.62). From 1990 to 2019, age-standardised death rates decreased globally by 17.0% (-28.4 to -2.9) for all diabetes, and by 21.0% (-33.0 to -5.9) when considering only type 1 diabetes. However, the low SDI quintile had the lowest decline for both all diabetes (-13.6% [-28.4 to 3.4]) and for type 1 diabetes (-13.6% [-29.3 to 8.9]). Interpretation Decreasing diabetes mortality at ages younger than 25 years remains an important challenge, especially in low and low-middle SDI countries. Inadequate diagnosis and treatment of diabetes is likely to be major contributor to these early deaths, highlighting the urgent need to provide better access to insulin and basic diabetes education and care. This mortality metric, derived from readily available and frequently updated GBD data, can help to monitor preventable diabetes-related deaths over time globally, aligned with the UN's Sustainable Development Targets, and serve as an indicator of the adequacy of basic diabetes care for type 1 and type 2 diabetes across nations.     • Background Colorectal cancer is the third leading cause of cancer deaths worldwide. Given the recent increasing trends in colorectal cancer incidence globally, up-to-date information on the colorectal cancer burden could guide screening, early detection, and treatment strategies, and help effectively allocate resources. We examined the temporal patterns of the global, regional, and national burden of colorectal cancer and its risk factors in 204 countries and territories across the past three decades. Methods Estimates of incidence, mortality, and disability-adjusted life years (DALYs) for colorectal cancer were generated as a part of the Global Burden of Diseases, Injuries and Risk Factors Study (GBD) 2019 by age, sex, and geographical location for the period 1990-2019. Mortality estimates were produced using the cause of death ensemble model. We also calculated DALYs attributable to risk factors that had evidence of causation with colorectal cancer. Findings Globally, between 1990 and 2019, colorectal cancer incident cases more than doubled, from 842 098 (95% uncertainty interval [UI] 810 408-868 574) to 2.17 million (2.00-2.34), and deaths increased from 518 126 (493 682-537 877) to 1.09 million (1.02-1.15). The global age-standardised incidence rate increased from 22.2 (95% UI 21.3-23.0) per 100 000 to 26.7 (24.6-28.9) per 100 000, whereas the age-standardised mortality rate decreased from 14.3 (13.5-14.9) per 100 000 to 13.7 (12.6-14.5) per 100 000 and the age-standardised DALY rate decreased from 308.5 (294.7-320.7) per 100 000 to 295.5 (275.2-313.0) per 100 000 from 1990 through 2019. Taiwan (province of China; 62.0 [48.9-80.0] per 100 000), Monaco (60.7 [48.5-73.6] per 100 000), and Andorra (56.6 [42.8-71.9] per 100 000) had the highest age-standardised incidence rates, while Greenland (31.4 [26.0-37.1] per 100 000), Brunei (30.3 [26.6-34.1] per 100 000), and Hungary (28.6 [23.6-34.0] per 100 000) had the highest age-standardised mortality rates. From 1990 through 2019, a substantial rise in incidence rates was observed in younger adults (age <50 years), particularly in high Socio-demographic Index (SDI) countries. Globally, a diet low in milk (15.6%), smoking (13.3%), a diet low in calcium (12.9%), and alcohol use (9.9%) were the main contributors to colorectal cancer DALYs in 2019. Interpretation The increase in incidence rates in people younger than 50 years requires vigilance from researchers, clinicians, and policy makers and a possible reconsideration of screening guidelines. The fast-rising burden in low SDI and middle SDI countries in Asia and Africa calls for colorectal cancer prevention approaches, greater awareness, and cost-effective screening and therapeutic options in these regions.   • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • The experimental methodology achieved in the present study signified that the adsorption and precipitation were main mechanisms occurred together in the removal of nickel from aqueous solution by sorption using cement kiln dust (CKD) byproduct as sorbent. Finding the contribution of each mechanism in the removal process and derivation an analytical model for finding the portion of precipitation were the focal points of this work. Results proved that pure precipitation was increased with the increase of CKD dosage and metal concentration where total removal (adsorption-precipitation) ranged from 45 to 100%. The SEM micrographs of the CKD sorbent before and after sorption process certified that there was crystal precipitates on the surface of the CKD. Also, these graphs in combination with FT-IR tests proved that [Ni(OH2)n]+2 (n=4-6) species were bonded with CKD and insoluble hydroxide species may be precipitated onto the CaO surfaces by co-precipitation, while K–O, Si-O and Ca-O groups enhanced the adsorption mechanism.

    • Wastewater treatment and subsequent effluent recycling for non-drinking purposes such as irrigation contributes to the mitigation of the pressure on freshwater resources. In this study, two vertical sub-surface flow constructed wetland (VSSF-CW) pilot plants were operated to treat municipal wastewater and their effluents were reused for irrigation purposes. One of the wetlands was vegetated with Phragmites australis (Cav.) Trin. ex Steud. (common reed) to compare its efficiency of pollutant removals with the non-vegetated system, which had the same design. COMSOL Multiphysics 3.5a was operated for the Activated Sludge Model 2 (ASM2) to predict the chemical oxygen demand (COD) and ammonia-nitrogen (NH4-N) concentrations. The effluent quality of both treatment systems was assessed for several parameters. Computer simulations show a good compliance between the measured and predicted values of COD and NH4-N for the vegetated system. The calibrated model could be effectively used to predict the behaviours of those parameters as a function of time. Moreover, the effluents of both vegetated (VFp) and non-vegetated (VF) VSSF-CW were significantly (p < 0.05) improved compared to influent. Significant (p < 0.05) effects due to the presence of P. australis were observed for removals of total suspended solids (TSS), 5-day biochemical oxygen demand (BOD5), COD, NH4-N and ortho-phosphate-phosphorus (PO4-P). However, significant increases (p < 0.05) were noted for electrical conductivity (EC), total dissolved solids (TDS), nitrate-nitrogen (NO3-N) and sulphate (SO4) of both effluents compared to the raw wastewater. Except for EC, NH4-N and SO4, all water quality parameters complied with irrigation water standards.

  • Effect of COVID-19 Anthropause on Water Clarity in the Belize Coastal Lagoon

    • The Coronavirus disease 2019 (COVID-19) pandemic halted human activities globally in multiple sectors including tourism. As a result, nations with heavy tourism, such as Belize, experienced improvements in water quality. Remote sensing technologies can detect impacts of “anthropauses” on coastal water quality. In this study, moderate resolution imaging spectroradiometer (MODIS) satellite data were employed along the Belizean coast to investigate impacts of the COVID-19 shutdown on water quality. The attenuation coefficient at 490 nm, Kd(490), was used as an indicator of water quality, with a lower Kd(490) indicating increased water clarity. Four Coastal Management Zones were characterized by marine traffic as high traffic areas (HTAs) and two as low traffic areas (LTAs). Monthly composites for two periods, 2002–2019 (baseline) and 2020 were examined for Kd(490). For months prior to the COVID-19 shutdown in Belize, there was generally no significant difference in Kd(490) (p &gt; 0.05) between 2020 and baseline period in HTAs and LTAs. Through the shutdown, Kd was lower in 2020 at HTAs, but not for LTAs. At the LTAs, the Kd(490)s observed in 2020 were similar to previous years through October. In November, an unusually active hurricane season in 2020 was associated with decreased water clarity along the entire coast of Belize. This study provides proof of concept that satellite-based monitoring of water quality can complement in situ data and provide evidence of significant water quality improvements due to the COVID-19 shutdown, likely due to reduced marine traffic. However, these improvements were no longer observed following an active hurricane season.

    • The present study develops the sorption model for simulating the effects of pH and temperature on the uptake of cadmium from contaminated water using waste foundry sand (WFS) by allowing the variation of the maximum adsorption capacity and affinity constant. The presence of two acidic functional groups with the same or different affinity is the basis in the derivation of the two models; Model 1 and Model 2 respectively. The developed Bi-Langmuir model with different affinity (Model 2) has a remarkable ability in the description of process under consideration with coefficient of determination > 0.9838 and sum of squared error < 0.08514. This result is proved by FTIR test where the weak acids responsible of cadmium ions removal using WFS sorbent can be represented by surface silanol (O-H) functional groups.

    • Background B-cell depleting therapies are highly efficacious in relapsing-remitting multiple sclerosis but one such therapy, rituximab, is not approved for multiple sclerosis and no phase 3 trial data are available. We therefore examined the safety and efficacy of rituximab compared with dimethyl fumarate in patients with relapsing-remitting multiple sclerosis to obtain data that might allow inclusion of rituximab in treatment guidelines. Methods RIFUND-MS was a multicentre, rater-blinded, active-comparator, phase 3, randomised controlled trial done at 17 Swedish university and community hospitals. Key inclusion criteria for participants were: age 18-50 years; relapsing-remitting multiple sclerosis or clinically isolated syndrome according to prevailing McDonald criteria; 10 years or less since diagnosis; untreated or only exposed to interferons or glatiramer acetate; and with clinical or neuroradiological disease activity in the past year. Patients were automatically randomly assigned (1:1) by the treating physician using a randomisation module in the Swedish multiple sclerosis registry, without stratification, to oral dimethyl fumarate 240 mg twice daily or to intravenous rituximab 1000 mg followed by 500 mg every 6 months. Relapse evaluation, Expanded Disability Status Scale rating, and assessment of MRI scans were done by examining physicians and radiologists masked to treatment allocation. The primary outcome was the proportion of patients with at least one relapse (defined as subacute onset of new or worsening neurological symptoms compatible with multiple sclerosis with a duration of more than 24 h and preceded by at least 30 days of clinical stability), assessed in an intention-to-treat analysis using log-binomial regression with robust standard errors. This trial is registered at ClinicalTrials.gov, NCT02746744. Findings Between July 1, 2016, and Dec 18, 2018, 322 patients were screened for eligibility, 200 of whom were randomly assigned to a treatment group (100 assigned to rituximab and 100 assigned to dimethyl fumarate). The last patient completed 24-month follow-up on April 21, 2021. 98 patients in the rituximab group and 97 patients in the dimethyl fumarate group were eligible for the primary outcome analysis. Three (3%) patients in the rituximab group and 16 (16%) patients in the dimethyl fumarate group had a protocol-defined relapse during the trial, corresponding to a risk ratio of 0.19 (95% CI 0.06-0.62; p=0.0060). Infusion reactions (105 events [40.9 per 100 patient-years]) in the rituximab group and gastrointestinal reactions (65 events [47.4 per 100 patient-years]) and flush (65 events [47.4 per 100 patient-years]) in the dimethyl fumarate group were the most prevalent adverse events. There were no safety concerns. Interpretation RIFUND-MS provides evidence that rituximab given as 1000 mg followed by 500 mg every 6 months is superior to dimethyl fumarate in preventing relapses over 24 months in patients with early relapsing-remitting multiple sclerosis. Health economic and long-term safety studies of rituximab in patients with multiple sclerosis are needed.

    • Many researchers are interested in using natural fibres to treat due to recent advancements in polymer characteristics. The mechanical properties of three types of bars are studied in this paper: Cotton Fibre-Reinforced Polymer bars, Cotton/Carbon Fibre-Reinforced Polymer bars, and Cotton/Glass Fibre-Reinforced Polymer bars. The goal of the paper was to create low-cost bars with comparable mechanical performance and corrosion resistance to steel reinforcement. The bars were made using two methods: fibres immersed in polymer and fibres coated with polymer by repeated tension and relaxation of fibres. The second method produced better results in terms of the tensile strength of Cot.CFRP, Cot.GFRP, and Cot.FRP bars, which were 688, 477, and 284 MPa, respectively, and the stress–strain curve revealed brittle behaviour for all bars and modulus of elasticity of 43, 31 and 22 GPa. When sand was put on the bar’s surface, the bars demonstrated a good connection with the concrete. It also showed good resistance to moisture, alkaline solutions and acids, as well as heat resistance at temperatures below 200°C.

    • We aimed to obtain magnesium/iron (Mg/Fe)-layered double hydroxides (LDHs) nanoparticles-immobilized on waste foundry sand-a byproduct of the metal casting industry. XRD and FT-IR tests were applied to characterize the prepared sorbent. The results revealed that a new peak reflected LDHs nanoparticles. In addition, SEM-EDS mapping confirmed that the coating process was appropriate. Sorption tests for the interaction of this sorbent with an aqueous solution contaminated with Congo red dye revealed the efficacy of this material where the maximum adsorption capacity reached approximately 9127.08 mg/g. The pseudo-first-order and pseudo-second-order kinetic models helped to describe the sorption measurements, indicating that the physical and chemical forces governed the removal process.

    • Re-use of the byproduct wastes resulting from different municipal and industrial activities in the reclamation of contaminated water is real application for green projects and sustainability concepts. In this direction, the synthesis of composite sorbent from the mixing of waterworks and sewage sludge coated with new nanoparticles named “siderite” (WSSS) is the novelty of this study. These particles can be precipitated from the iron(II) nitrate using waterworks sludge as alkaline agent and source of carbonate. Characterization tests using X-ray diffraction (XRD), scanning electron microscopy (SEM) and energy dispersive spectroscopy (EDS) mapping revealed that the coating process was correctly achieved and siderite nanoparticles were planted on the surface of the composite sorbent. Interaction between Congo Red dye and composite sorbent was evaluated through a set of batch tests under the variation of agitation time, pH of aqueous solution, and sorbent dosage. The results proved that the prepared sorbent had a high ability in the treatment of water contaminated with Congo Red dye in comparison with previous studies and the maximum adsorption capacity reached to maximum value i.e. 9416 mg/g. The sorption process was governed by electrostatic attractions; however, Sips and Pseudo-second-order models described this process with coefficient of determination greater than 0.99.

  • Active type III translocon assemblies that attenuate Yersinia virulence

    • Type III secretion enables bacteria to intoxicate eukaryotic cells with anti-host effectors. A class of secreted cargo are the two hydrophobic translocators that form a translocon pore in the host cell plasma membrane through which the translocated effectors may gain cellular entry. In pathogenic Yersinia, YopB and YopD shape this translocon pore. Here, four in cis yopD mutations were constructed to disrupt a predicted a-helix motif at the C-terminus. Mutants YopDI262P and YopDK267P poorly localised Yop effectors into target eukaryotic cells and failed to resist uptake and killing by immune cells. These defects were due to deficiencies in host-membrane insertion of the YopD-YopB translocon. Mutants YopDA263P and YopDA270P had no measurable in vitro translocation defect, even though they formed smaller translocon pores in erythrocyte membranes. Despite this, all four mutants were attenuated in a mouse infection model. Hence, YopD variants have been generated that can spawn translocons capable of targeting effectors in vitro, yet were bereft of any lethal effect in vivo. It is therefore probable that an active translocon makes a range of contributions during bacteria-host cell contact that extends beyond effector delivery per se.

    • Pathogenic Yersinia all harbor a virulence plasmid-encoded Ysc–Yop T3SS. In this system, translocator function is performed by the hydrophobic proteins YopB and YopD. With the goal to better understand how YopD orchestrates yop-regulatory control, translocon pore formation and Yop effector translocation, we performed an in silico prediction of coiled-coil motifs in YopD and YopD-like sequences from other bacteria. Of interest was a predicted N-terminal coiled-coil that occurred solely in Yersinia YopD sequences. To investigate if this unique feature was biologically relevant, two in cis point mutations were generated with a view to disrupting this putative structure. Both mutants maintained full T3SS function in vitro in terms of environmental control of Yops synthesis and secretion, effector toxin translocation and evasion of phagocytosis and killing by cultured immune cells. However, these same mutants were attenuated for virulence in a murine oral-infection model. The cause of this tardy disease progression is unclear. However, these data indicate that any structural flaw in this element unique to the N-terminus will subtly compromise an aspect of YopD biology. Sub-optimal T3SSs are then formed that are unable to fortify Yersinia against attack by the host innate and adaptive immune response.

    • Here, batch study for Iraqi bentonite-cadmium aqueous solution interaction was conducted with condi- tions of contact time ::; 120 min, sorbent dosage from 0.05 to 1 g per 100 mL and shaking speed from 50 to 300 rpm for initial concentration of 50 mg/L with initial pH of 3 to simulate the acetogenic phase in the sanitary landfill. The best conditions were 1 h, 0.7 g/100 mL and 250 rpm, respectively to obtain the high- est removal (90%). The sorption data were well formulated by Freundlich and Langmuir models with determination coefficient (R2) 2: 0.98 and sorption capacity of 145.3 mg/g; so, the removal process gov- erned by the physico-chemical forces. The average coefficient of the hydraulic conductivity was found equal to 1.98 x 10-10 m/s and this is suitable for low permeable barrier (LPB). Finally, the predictions of COMSOL package were presented satisfactory simulation for the cadmium concentrations within two-dimensional physical model packed with sand aquifer and Iraqi bentonite. Two configurations of LPB were evaluated for restriction of contaminant front migration and the results proved that the sur- rounding of the location required to protect it from three sides against to the flow direction can be more dependable from continuous configuration.

    • This study investigates the effectiveness of using Iraqi clay as a low-permeability layer to prevent the migration of lead and nickel ions in groundwater-aquifers. Tests of batch operation have been conducted to determine the optimal conditions for removing Pb2+ ions, which were found to be 120 minutes of contact time, a pH of 5, 0.12 g of clay per 100 mL of solution, and an agitation of 250 rpm. These conditions resulted in a 90% removal efficiency for a 50 mg L−1 initial concentration of lead ions. To remove nickel ions with an efficiency of 80%, the optimal conditions were 60 minutes of contact time, a pH of 6, 12 g of clay per 100 mL of solution, and an agitation of 250 rpm. Several sorption models were evaluated, and the Langmuir formula was found to be the most effective. The highest sorption capacities were 1.75 and 137 mg g−1 for nickel and lead ions, respectively. The spread of metal ions was simulated using finite element analysis in the COMSOL multiphysics simulation software, taking into account the presence of a clay barrier. The results showed that the barrier creates low-discharge zones along the down-gradient of the barrier, reducing the rate of pollutant migration to protect the water sources.

    • This study characterized the temporal and spatial distribution of trichloroethylene (TCE) dissolved plume from continuous source in 3D bench-scale sand aquifer. COMSOL Multiphysics 3.5a (2008) software based on the finite element numerical scheme taking into account the sorption process was used to interpreted the experimental results. A conservative tracer is employed for the determination of the longitudinal dispersivity. The sorption characteristics of TCE onto the aquifer sand are independently determined from batch equilibrium data. TCE concentrations at specific downstream locations within the aquifer for different periods of time are measured under flow rates values equal to 20, 40 and 100 ml/min. The predicted TCE concentrations were found to be reasonably agreed with the bench-scale experimental results with mean error <5%.

    • The present study investigates the utilization of the waterworks sludge by-product that generated from water supply treatment plant for the remediation of simulated groundwater contaminated with leachate spilled from the sanitary landfill by permeable reactive barrier (PRB) technology. Batch sorption experiments were conducted for describing the interaction between the acetogenic phase (pH=5.5±0.1) of leachate contaminated with cadmium (Cd(II)), ammonia nitrogen (NH3–N) or dissolved organic matter (COD) and waterworks sludge. Also, conventional sorbents such as the activated carbon and amberlite ion-exchange resin were tested to evaluate their ability in compar-ison with waterworks sludge in the remediation process. Results proved that the Langmuir model describes well the sorption data with maximum sorption capacities of 5.634, 14.908 and 3.938mg/g for Cd(II) onto sludge, NH3–N onto resin and COD onto activated carbon, respectively. The batch and column tests signified that the sludge, resin, and carbon can be used for removing Cd(II), NH3–N, and COD, respectively. The multi-layered bed of sorbents under consideration has a remarkable ability in the remediation of a leachate contaminated with Cd(II), NH3–N, and COD. Finally, the Yan model is more representative than Thomas and Belter models for characterization of the contaminants propagation in the column packed with single sorbent.

    • Background: Cladribine  is  a  deoxyadenosine  analogue  prodrug  that selectively  induces  immune  reconstitution  by  targeting  B-  and  T-lymphocytes. Cladribine  tablets  (CladT)  are  administered  in two courses, 12 months apart, for patients with relapsing multiple sclerosis (RMS). Post-marketing surveillance is important for evaluation  of  long-term  safety  and  effectiveness  in  a  real-world setting. CLADCOMS (CLADribine tablets long-term Control Of MS) is a post-marketing investigator driven study. Here we report one year follow-up data on the first 100 patients included in the study in April 2021.Objective: 1) To  investigate for how long a full dose treatment with Cladribine 10 mg tablets (3.5 mg/kg over two years) offers freedom of disease activity in relapsing MS patients.2) To collect complete data on safety and effectiveness with the help of the Swedish Neuroregistry to enable future assessment on effectiveness and safety in comparison with other in Sweden commonly used disease modifying treatments.Methods: CLADCOMS  includes  patients  with  relapsing  MS  from  the  eight academic  clinics  starting  Cladribine  treatment  after  23rd  of  March  2018. Data  is  collected  in  the  Swedish  Neuroregistry  using  highly  structured  yearly follow-up  routines.   Descriptive   data   on   relapses,   MRI   activity,   Patient   Reported   Outcome   Measures   and   Serious   Adverse   events   (SAEs)  from the  first  100  patient  included  in  the  study  are  obtained from the registry.Results: Up  to  April  2022  1XX  patients  were  included  in  the  study. In April 2021 the first 100 patient entered the study. 40% of patients included were treatment naïve, 29% switched from natalizumab and 13% from rituximab. By April 2022, 5 patients experienced a relapse during the treatment initiation and showed MRT activity with contrast enhancing (CEL)lesions more than six months after initiation of treatment, of which 2 patients showed CEL more than six months after the second treatment course year two. 20% of the patients showed new lesions on the first MRI performed up to 18 months after treatment initiation. Two patients reported SEAs. Analysis of CD19   and   CD27-   B-cells   counts   over   time   will   be   performed.Conclusions: Cladribine treatment demonstrates clinical stability in patients treated ⩾ 12 months. However, continued follow-up is needed to assess the effectiveness and safety of treatment with Cladribine over a longer time to investigate time to disease reactivation after the second treatment course year two has been administered.

    • Type III secretion systems are a prolific virulence determinant among Gram-negative bacteria. They are used to paralyze the host cell, which enables bacterial pathogens to establish often fatal infections—unless an effective therapeutic intervention is available. However, as a result of a catastrophic rise in infectious bacteria resistant to conventional antibiotics, these bacteria are again a leading cause of worldwide mortality. Hence, this report describes a pDM4-based site-directed mutagenesis strategy that is assisting in our foremost objective to better understand the fundamental workings of the T3SS, using Yersinia as a model pathogenic bacterium. Examples are given that clearly document how pDM4-mediated site-directed mutagenesis has been used to establish clean point mutations and in-frame deletion mutations that have been instrumental in identifying and understanding the molecular interactions between components of the Yersinia type III secretion system.

    • Utilizing the waterworks sludge byproduct in the treatment of wastewater contained methylene blue dye is one approach that has been taken in an effort to lessen the difficulties that are associated with managing such byproduct. The prime aim of this work is manufacturing of novel sorbent from co-precipitation of magnesium oxide nanoparticles on the surfaces of waterworks sludge in the existence of cetyl trimethyl ammonium bromide surfactant. Surfactant 0.04 g/50 mL, dose of sludge 2 g/50 mL, and pH 12 were the most efficient preparation parameters to remove 75.31% of adopted dye. The adsorption studies were conducted under various conditions of contact time (0–240 min), concentration of dye (10–300 mg/L), sorbent mass (0.05–1.5 g), and solution pH (3–12). The best values of batch parameters were identical to the highest percentages of contaminant removal. Results proved that the magnesium oxide nanoparticles are attached to the sludge surfaces. Freundlich and pseudo-second-order models have perfectly described sorption results with 59.92 mg/g maximum sorption capacity. The breakthrough curves can be accurately described by the Bohart-Adams model. The outputs of continuous tests have been paved the way for future usage of the prepared sorbent in the field permeable reactive barrier technology.

    • The provision of safe water for people is a human right; historically, a major number of people depend on groundwater as a source of water for their needs, such as agricultural, industrial or human activities. Water resources have recently been affected by organic and/or inorganic contaminants as a result of population growth and increased anthropogenic activity, soil leaching and pollution. Water resource remediation has become a serious environmental concern, since it has a direct impact on many aspects of people’s lives. For decades, the pump-and-treat method has been considered the predominant treatment process for the remediation of contaminated groundwater with organic and inorganic contaminants. On the other side, this technique missed sustainability and the new concept of using renewable energy. Permeable reactive barriers (PRBs) have been implemented as an alternative to conventional pump-and-treat systems for remediating polluted groundwater because of their effectiveness and ease of implementation. In this paper, a review of the importance of groundwater, contamination and biological, physical as well as chemical remediation techniques have been discussed. In this review, the principles of the permeable reactive barrier’s use as a remediation technique have been introduced along with commonly used reactive materials and the recent applications of the permeable reactive barrier in the remediation of different contaminants, such as heavy metals, chlorinated solvents and pesticides. This paper also discusses the characteristics of reactive media and contaminants’ uptake mechanisms. Finally, remediation isotherms, the breakthrough curves and kinetic sorption models are also being presented. It has been found that groundwater could be contaminated by different pollutants and must be remediated to fit human, agricultural and industrial needs. The PRB technique is an efficient treatment process that is an inexpensive alternative for the pump-and-treat procedure and represents a promising technique to treat groundwater pollution.

    • The present work is represented by the derivation of mathematical model and solving the model analytically using the method of separation of variables to describe the migration of the contaminant metal ions through a column packed with bed of permeable reactive barrier (PRB). The validity of the solution can be evaluated through the simulation of cadmium and chromium ions using scrap iron and/or aluminum by-products in the form of wastes that if not utilized to treat waste by waste can impose further burden over the ecosystem. Breakthrough curves proved that the increase of metal ions velocity will decrease the capturing of the ions; therefore, the distribu-tion coefficient and the retardation factor also decrease. Furthermore, the increase of barrier depth will increase the longevity of PRB because this will delay the migration of contaminant. A mathematical model has acceptable ability in the representation of experimental measurements with Nash-€“Sutcliff efficiency coefficients greater than 0.98. The longevity of the PRB was estimated for the field scale to be 210 and 250 d to produce contaminant effluent beyond 100 cm barrier matrix within the environmental permissible concentrations. Although groundwater velocity is highly variable, a proposed velocity of 0.25 cm/min which is assumed to be analogous to the groundwater velocity has revealed prolonged longevity of 7.02 y for the capture of chromium.

    • Preparation of new sorbent from precipitation of nano-sized (Mg/Fe-CTAB)- layered double hydroxide (LDH) on the surfaces of sewage sludge byproduct to remove the anionic and cationic dyes was the focal point of this work. The presence of nanoparticles and enlarged of interlayers by CTAB intercalation have increased the sludge surface area from 5.34 to 10.32 m2/g. The CTAB mass 0.03 g/50 mL, sludge dosage 1 g/50 mL and (Mg/Fe) molar ratio 2 were the best preparation conditions required to obtain effective sorbent with efficiencies exceeded 93% for MB and CR dyes. These efficiencies were obtained under operational conditions for batch study of 0.5 g coated sludge per 50 mL colored dye solution, initial pH 3 (for CR) and 12 (for MB), and time 3 h for 10 mg/L dyes at 200 rpm. Models of Langmuir and pseudo second-order have a high capability in the representation of sorption records with maximum capacities of adsorption 163.6 and 132.6 mg/g for CR and MB dye, respectively. The X-ray diffraction analysis proved that the calcite occurred mainly at 2θ = 29.8° while quartz corresponded to the 21, 26.6, 36.4, 36.9, 50.1, 60.01 and 68.4°. Characterization tests showed that nano-sized particles of magnesium/iron were precipitated on the sludge due to the formation of hydrotalcite-like compounds with an increase in the percentages of Mg and Fe from 0.87 to 1.36 to 4.25 and 3.03%, respectively. The results showed that the electrostatic attraction, intra-particle diffusion and hydrogen bonding were predominant mechanisms for removal of CR and MB onto coated sludge.

    • Purpose: The deregulation of the Hippo pathway results in translocation ofYes-associated protein-1 (YAP1) to the nucleus to exert an oncogenic effect. This effect has been demonstrated in several malignancies, yet, in breast cancer (BC), it remains controversial. The present study aimed to investigate the significance of YAP1 expression in BC, its relation to cancer stem cells (CSCs), and the effect of its inhibition on tumor cell survival. Patients and Methods: We evaluated the expression of YAP1 protein and gene using immunohistochemistry (IHC) and RT-qPCR in FFPE tissue from normal and breast cancer cases. We also studied its association with CSC expression (OCT4, NANOG, and SOX2) and with different clinicopathologic characteristics. Two BC cell lines (MCF7 and MDA-MB -231) were exposed to different concentrations of YAP1 inhibitor "verteporfin" and cell viability was subsequently assessed. Results: YAP1 mRNA was higher in BC compared to the normal breast tissue (p-value=0.040) and was higher in luminal tumors compared to triple-negative breast cancer (TNBC) (p-value= 0.017). Its expression in tumors was significantly associated with the expression of pluripotency markers (OCT4 and NANOG) (p-value= 0.030 and 0.035, respectively) and its inhibition resulted in a significant reduction of CSC expression in both MCF-7 and MDA-MB-231 cells. YAP1 nuclear expression by IHC, which signifies its activation, was more evident in invasive carcinomas compared to normal breast tissue and in-situ foci where the expression was limited to the cytoplasm. The pretreatment of BC cells (MCF7 and MDA-MB-231) with YAP1 inhibitor "verteporfin" resulted in their sensitization to the effect of tamoxifen and doxorubicin, respectively, and significantly decreased tumor cell proliferation and survival. Conclusion: Our results imply that YAP1 is highly expressed and activated in BC and its inhibition could represent a possible novel therapeutic strategy that should be further explored and investigated to improve the outcome of breast cancer patients.

    • This report focuses on Border Management and Migration Control in Iraq through discussing three main issues, these are: First, it discusses Iraq legal framework defining the pre entry measures; procedures for return, detention and readmission; and key actors involved in Border Management and Migration Control. Second, it provides a conceptualization of Iraq decision makers on the importance of Border Management and the process of regulating the status of refugees and asylum seekers. This includes discussing the challenges and fears of Iraqi politicians paralyzing the process of enacting some important legislations aiming at further development of Iraq legal framework. Thirdly, it discusses the applicable policies and practices, and their compliance with the Iraqi legal system and with the international standards. Additionally, the report includes a conclusion part summarising the main findings of the researching team and their recommendations.
