  • The professional relationship forms the base : Swedish child health care nurses' experiences of encountering mothers exposed to intimate partner violence

    • Purpose This study aimed to explore child health care nurses' clinical experiences from encounters with mothers exposed to intimate partner violence (IPV), as little research has explored this topic. Method Nine child health care nurses from two Swedish regions were interviewed. The interviews were analysed using thematic analysis. Results The narratives depicted the nurses' strong commitment to, and professional relationship with, the exposed mothers. The experience of working as a nurse and having encountered IPV in clinical practice made the nurses more confident, which impacted their performance and attitude towards this topic. The ability to uphold the professional relationship was threatened by lack of support and interprofessional collaborations. Conclusions The professional relationship was central to the encounters, yet could impose an emotional burden on the nurses. While the nurses wanted to improve their knowledge of the process around the mother and child, they were happy to pass the primary responsibility over to other professionals. The findings highlight the challenge in establishing sustainable support for nurses, and building a transparent collaboration process between the health care sector and the social services, serving the well-being and safety of the mother and child.

  • Hur föräldrar med psykisk ohälsa upplever sitt behov av stöd i föräldraskapet

    • Det finns ett starkt samband mellan psykisk ohälsa hos föräldern och psykisk ohälsa hos barnet. Barnen tycks även ha en förhöjd risk att utveckla psykisk och fysisk ohälsa som unga vuxna. Eftersom en stor del av patienter med psykisk ohälsa finns i öppenvårdspsykiatrin eller primärvård är andelen barn som lever med föräldrar med psykisk ohälsa  förmodligen mycket större än vad vi tror. Föräldrar med psykisk ohälsa kan behöva stöd i sitt föräldraskap för att bli den bästa föräldren de kan. Syftet med projektet är att identifiera behov av stöd i föräldraskapet som föräldrar eller blivande föräldrar med psykisk ohälsa upplever. Vi genomförde tio intervjuer med föräldrar med psykisk ohälsa och skickade ut en enkät. Slutsatserna från resultatet var att det finns ett behov av stöd i föräldraskapet bland föräldrar med psykisk ohälsa. Vi fann att Familjecentralen lyfts fram som ett gott exempel och här skulle ett samarbete med psykiatrin främja relationen barn - förälder, samt att patient-anhörigförening spelar en viktig roll för föräldrar med psykisk ohälsa. Att synliggöra dessa ännu mer kan bidra till situationen på ett positivt sätt. Slutligen, det behövs inte några komplexa insatser, för många handlar det om att få träffa likasinnade i grupp, eller en samtalskontakt i svåra situationer.

  • A comparison of applicants' and incumbents' mean scores on health constructs and personality constructs. : A follow-up study of military recruits in a selection setting

    • This study looked at the effects of self-enhancement in a real recruitment setting, and included both personality constructs, and the more infrequently studied, health constructs. The participants (N = 202) were assessed first as applicants, and later as incumbents. The result revealed that self-enhancement was more ubiquitous on the personality constructs than on the health constructs. However, the effect sizes were stronger for the latter, and it was twice as likely for the participants to confirm having experienced previous traumatic/stressful events as incumbents compared to as applicants. Implications for self-enhancement on personality and health constructs in military recruitment situations are discussed.

  • A Construct Validation of a Profession-Focused Personality Questionnaire (PQ) Versus the FFPI and the SIMP

    • This study is a construct validation of a profession-focused personality questionnaire (PQ), based on the Big Five, and developed for a military population and military context. The sample (N = 363) consisted of participants selected for international services in the Swedish Armed Forces. The structure of the PQ was modeled by means of confirmatory factor analysis, and its convergent validity was tested against the Five Factor Personality Inventory (FFPI) (Hendricks, Hofstee, & de Raad, 1999, 2000) and the Single-Item Measures of Personality (SIMP) (Woods & Hampson, 2005) as correlations. The emergent structure of the PQ – 41 items and 7 aspects – showed good internal consistency and acceptable convergent validity with both criterion instruments. The only nonconvergence was found between the Agreeableness aspect of the PQ, Concern for Others, and Agreeableness of the SIMP, most likely because of the broader SIMP factor. The structure of PQ was tested on a new sample (N = 274) and was found stable regarding the factor loadings and the relations between the factors. While the PQ needs to be investigated further, it seems as if it may become a useful tool in the research of military teams and contexts.

    • Begreppet hälsa kan betyda olika saker för olika individer och teorier. Det kan handla om antingen fysiologiska eller psykologiska faktorer, men dessa kan också ses som intimt sammanbundna. Det är i det sista perspektivet som denna studie har sitt ursprung, där vi söker en djupare förståelse för ett psykologiskt fenomen som tycks vara svårt att precisera, nämligen en individs känsla av sammanhang (KASAM; eng. Sense of coherence, SOC). KASAM är en generell inställning till livet som tycks påverka förmågan att stå emot påfrestningar och i förlängningen behålla eller till och med förbättra hälsan (Antonovsky, 1993). Det finns en mängd forskning som stärker betydelsen av en hög KASAM för god hälsa och det samma gäller inom det militära. KASAM har tidigare visat sig ha starka samband med veteraners förmåga att återanpassa sig vid hemkomst. KASAM är med andra ord en teori som är av intresse att undersöka vidare.

    • Purpose The purpose of this study is to explore the expectations and experiences regarding a robotic shower, from a dual user perspective. Design/methodology/approach This was an explorative qualitative study in which elderly and personnel were interviewed before the robotic shower was installed and again after four or five months of usage. Findings The elderly participants found the robotic shower empowering. The personnel's experiences encompassed their own work conditions, as well as the user value for the elderly. A shared experience for both user groups was a more independent shower situation for the elderly. Originality/value This study presents a dual user experience of a DAT in an intimate care situation and shows the importance of including both elderly and personnel to fully understand the value of DATs.

    • According to Baxter (1986) every relationship has rules which must be followed. If at least one of these rules is broken, it gives the individual the right to breadup the relationship. Baxter identified eight such rules or criteria. The aim of the present  study was to investigate whether or not the expectations of romantic relationships held by homosexual men can be linked to Baxter's criteria. Data were obtained through a questionnaire addressed to 31 homosexual men and 50 heterosexual men. Results indicated that Baxter's criteria can be linked to expectations of romantic relationships held by men regardless of sexual orientation, and that age and experience are more reliable predictors than is sexual orientation.

  • Improved resiliency and well-being among military personnel in a Swedish Naval Force after a counter-piracy operation off the coast of Somalia

    • The aim of this study was to explore: (1) how the psychological health of the members of the first European Union Naval Force (ME01) was affected by international deployment off the coast of Somalia; and (2) if and how organizational and personal factors (e.g., type of personnel category, previous experiences, and resilience) affected their psychological health and well-being post-deployment. The study had an exploratory longitudinal design, where the participants were assessed both before and after deployment (i.e., T1 and T2). The participants (n=129, 120 men, 9 women) were equally distributed between officers (n=68; 64 men, 4 women) and sailors (n=61; 56 men, 5 women). The members' average age was 31years, ranging from 20 to 61. For the majority (78%) ME01 was their first international deployment and officers were, in general, more experienced than sailors. The overall results showed that the members' reported a positive experience with improved resilience and well-being (e.g., sense of coherence). However, the result also showed that type of personnel category (i.e., officer or sailor) affected their psychological health. Why and how these differences among military personnel arise is discussed, but deserves further attention.

  • Military capacity and Civil adjustment : Assessments of the "re-usable" peacekeeping soldier for development of a selection system

    • This study investigated whether “re-usable” soldiers, that is, those who performed well during operations (Military capacity), and were able to readjust post-deployment (Civil adjustment), could be identified pre-deployment. Participants were 364 UN peacekeeping soldiers. Three hypotheses were posed: (a) the selection system for conscripts cannot identify soldiers with low Military capacity, (b) the selection system for conscripts cannot identify soldiers with poor Civil adjustment after deployment, and (c) the two aspects of “re-usability” (Military Capacity and Civil Adjustment) would be intertwined. Results showed that the selection system for conscripts was unable to identify soldiers’ Military Capacity and Civil Adjustment. Results also showed that these two aspects were unrelated, and did not interact. Indications on possible consequences, and improvements were discussed.

  • Mind over matter : Non-cognitive assessments for the selection of the Swedish voluntary soldier of peace

    • The purpose of this thesis was firstly, to investigate if the current selection system mirrors the task of international deployment and voluntariness. Secondly, to investigate if and how non-cognitive assessments of personality and resilience, individual aspects that seem underrepresented in the current selection system, may increment validity to the current selection system. Since 2012 the Swedish Armed Forces is an All-volunteer Force where young men and women voluntarily can apply for a military service. In contrast to conscription, military service today includes compulsory international deployments with different demands on the personnel’s range of possible abilities and skills as well as selection process—yet the current selection system may not sufficiently correspond to the changes.The thesis comprises four studies (Study I-IV) with relevant military samples, and aside from Study I, a validation of a short version personality questionnaire (PQ) being used in two of the subsequent studies, Study II-IV had a longitudinal design. Study II shows that the former selection system lacked prognostic value of soldiers’ performance during international deployment, and their ability to readjust at homecoming. Additionally, Study II shows that non-cognitive assessments can be used as predictors for readjustment. Study III indicates that international deployment does not need to be harmful for the psychological well-being and that good health seems to be a stable factor across time and situations. Thus, selection of “good health” and resilience may prove fruitful. Study IV suggests that high motivation to serve may have serious consequences for selection decisions and, in the long run, the recruits’ psychological well-being.In sum, this thesis suggests that the current selection system needs adaption to the task of repeated international deployments and to the voluntary applicant pool, and that non-cognitive assessment may increment validity.

    • This study focuses on how fitness technology services can be designed in order to support the co-creation of motivation to sustain a healthy behavior. The study used an exploratory qualitative design in which we interviewed eight seasoned runners about their running routines and their use of fitness trackers. The result shows that the fitness routines require careful planning and deliberation, and that the maintenance of a healthy lifestyle relies more on what happens between the activities than during said activity. Thus, fitness trackers should focus on feedback that allows users to co-create motivation during intermissions between health promoting behaviors.

  • Officer - att vara eller att icke vara : vad påverkar officerares återanvändbarhet för att delta i upprepade internationella insatser?

    • Syftet med den aktuella analysen var att se vilka individvariabler som påverkade officerares fältduglighet och återhämtningsförmåga efter att ha deltagit i internationella insatser, det vill säga deras återanvändbarhet. Återanvändbarheten är en framgångsfaktor i det nya personalförsörjningssystemet då alla anställda och kontrakterade inom Försvarsmakten kan komma att behöva delta i flera internationella insatser under sin tjänstgöringstid. Deltagarna i denna undersökning utgjordes av yrkes- och reservofficerare från två utlandsförband som tjänstgjorde i Kosovo under 2006 och 2007 (N = 111). Utöver självskattningsdata som samlades in vid två tillfällen för respektive förband, före insatsen och efter insatsen, inhämtades också registerdata. Dels mönstringsresultat från Pliktverket eller Krigsarkivet, beroende på när i tiden de hade mönstrat, dels deras vitsord från den aktuella insatsen, från Försvarsmakten. Resultatet visade att mönstringsvariablerna inte hade samband med officerarnas fältduglighet eller återhämtningsförmåga. Överhuvudtaget var det få individvariabler som hade samband med fältdugligheten. Gällande återhämtningsförmåga var det främst Antonovskys formulär Känsla av sammanhang (KASAM) som påvisade effekt, där officerare med låga värden på formuläret också var de som hade svårast att återanpassa sig vid hemkomst. Resultaten går i linje med tidigare analyser på soldatnivå, men behöver replikeras innan långtgående slutsatser kan dras.

  • Personalized Coaching via Texting for Behavior Change to Understand a Healthy Lifestyle Intervention in a Naturalistic Setting : Mixed Methods Study

    • Background: Digital health interventions, such as personalized SMS coaching, are considered affordable and scalable methods to support healthy lifestyle changes. SMS, or texting, is a readily available service to most people in Sweden, and personalizedSMS coaching has shown great promise in supporting behavior changes.Objective: This study aims to explore the effectiveness of highly personalized SMS coaching for behavior change according to the Capability, Opportunity, Motivation–Behavior (COM-B) model on a sample of physically inactive adults in a nonprofit fitness organization in Sweden.Methods: The study used a mixed methods design in which clients acted as their own controls. The participants were clients(n=28) and fitness consultants (n=12). Three types of data were collected: (1) quantitative data at baseline and after the SMS intervention and the waitlist from the clients, (2) qualitative data from semistructured interviews with the fitness consultants, and(3) pseudonymized texting conversations between the fitness consultants and clients.Results: Overall, the results showed that personalized SMS coaching was effective in supporting the clients’ behavior changes. The quantitative analysis showed how the clients’ capabilities (Cohen d=0.50), opportunities (Cohen d=0.43), and relationship with the fitness consultants (Cohen d=0.51) improved during the SMS intervention in comparison with baseline. Furthermore, the qualitative analysis revealed how personalized texts added value to existing work methods (eg, increasing continuity and flexibility) and how the relationship between the clients and fitness consultants changed during the intervention, which helped motivate the clients.Conclusions: Personalized SMS coaching is an effective method for supporting healthy behavior changes. The human connection that emerged in this study needs to be further explored to fully understand the effectiveness of a digital health intervention.

  • Socialt stöd : En explorativ faktoranalys av ett frågeformulär för urvalet till Försvarsmakten

    • AbstractThis study presents an exploratory factor analysis of a questionnaire regar­ding military recruits’ social support from friends and family (i.e., Social support) for their voluntary enlistment, and participation in the basic military training. The participants consisted of the 1290 (1133 men and 157 women) who had been accepted to and started the first rounds of basic military training in the new Swedish Armed Forces All-volunteer Force in 2011. The exploratory factor analysis (Principal Component Analysis, with Oblique-rotation) yielded a simple structure where 11 of the original 13 statements loaded on four related factors. Two of the fac­tors consisted of the statements which were common to all participants, that is, all participants were able to answer them. These two factors re­garded an articulated support for family and friend, and an unspoken dis­approval, respectively. The remaining two factors where specific for the participants who had either been deployed on international operations previously, or were in (or had recently ended) a relationship. The study also explores the relationships between the emerging four factors and two questions regarding the participants’ rating of the importance of social support for their well-being (i.e., Importance of social support), and the prevalence of previous relationship problems (i.e., Relationship problems). However, the structure, and in particular the two latter factors need to be evaluated further, and the results should therefore be interpreted with caution. In addition, the introduction to this study presents a theoretical background to the field of social support, and its importance for military populations.

  • The use of fitness-trackers and the role of motivational intermissions to maintain healthy behaviors : an explorative case study on runners

    • This study explores a user perspective on fitness-trackers and how they are used to maintain healthy behaviors; and how fitness trackers could be designed to better maintain healthy behaviors. A thematic analysis was conducted on the transcriptions from semi-structured interviews with eight seasoned non-professional runners who regularly used fitness trackers in their exercise regime. The main findings are: (a) the maintenance of healthy behaviors rely more on what happens in the intermission – that is, between healthy behaviors – than during the actual behavior itself; and (b), by visualizing the history of healthy behaviors, intermission feedback help motivate the repetition of healthy behavior in a way that mimics broad choice bracketing and behavioral streaks. Thus, fitness-trackers should focus more on the motivational aspects during the intermission between the healthy behaviors rather than encouraging the performance during the target behavior.

    • This study shows that seniors strive to become more independent and that acceptance of welfare technology needs to be understood in the specific situation. User experiences from welfare technology, such as the robotic shower, that serve in a very intimate situation, may not be comparable to the results from previous studies of, for example, surveillance technology.The preliminary results in this paper build on both qualitative and quantitative data, and show that seniors are in general positive to replacing the current shower situation with a more autonomous shower situation as provided by the robotic shower. An important aspect for acceptance was the functionality of the robotic shower. Furthermore, this study showed that the understanding of the shower situation in general can be used for improvement of the current shower situation, to prevent learned helplessness.

    • Presentation om hur införandet av en automatiserad duschlösning skapade olika värden för två användargrupper, äldre och vårdpersonal. Där oberoende var en viktig aspekt för både personal och äldre, men där personalens perspektiv var mer tvetydigt och ambivalent då deras arbetsinsatser inte bara förbättrades utan även i vissa aspekter försvårades. De äldre användarnas perspektiv handlade mer om ett ökat oberoende och upplevelsen av att återfå kontrollen i duschsituationen. Här diskuterades också resultatens implikationer samt lärdomar från studien.

  • The Effectiveness of Child-Parent Psychotherapy on Traumatized Preschoolers and Their Caregivers : A Swedish Multi-Site Study

    • Introduction: Trauma affects a child’s mental health and appears to be associated with challenges in caregiver-child dynamics. Nearly half of child trauma victims are under the age of six, yet few interventions are designed for this age group. Child-Parent Psychotherapy (CPP) is an attachment-based trauma treatment for preschool-aged victims of adversities and their caregivers. This study examines the effectiveness of CPP when implemented in a Swedish naturalistic multi-site clinical setting. Method: 57 children (age 2–6 years) with potentially traumatic experiences and their caregivers participated. They were recruited from 12 mental health services providing child treatment. The study has a one-group, pretest-posttest design. Outcome measures covered posttraumatic stress and general psychological symptoms in children and caregivers and caregivers’ perceptions of their relationship with their child. Paired t-tests were used to calculate within-group differences. Effect sizes were computed as Cohen’s d and associations between child and caregiver outcomes as Pearson’s correlation coefficient (r). A hierarchical regression analysis was tested to explore the correlation analysis. Results: Reductions in posttraumatic stress and general psychological symptoms with small-to-medium effect sizes were found in children and caregivers. Caregivers reported perceived improvements in their relationship with their child, with associations to findings in child posttraumatic stress reduction. Discussion: Findings support earlier results that CPP may be a suitable choice of treatment for preschoolers exposed to potentially traumatic events, such as violence. The dyadic treatment model seems to address the association between child traumatic stress and caregivers’ perceptions of the caregiving relationship. Further dissemination testing is suggested.

    • This chapter focuses on the early preparatory phase of the digital assistive technology (DAT) implementation process, and explores the challenges older people, healthcare personnel, healthcare managers, and IT-staff may experience. The chapter is based on two independent case studies, both part of longitudinal projects on DAT implementation in municipalities in Sweden and Norway. The two cases provide insights into DAT implementation and its challenges, in relation to four different categories: 1) complex innovation characteristics, 2) external policy and incentives, 3) organizational characteristics, and 4) individual characteristics. The chapter demonstrates that DAT does not only transform work practices and daily routines but also the entire healthcare profession, from being primarily concerned with caring and nursing to a profession involving technological skills. DAT implementation is about transformation, and may even be disruptive. Successful digitalization of healthcare needs a new mindset, new work practices, as well as new roles and professions.

    • Military staff performance may be inferior due to several reasons. The purpose of the present survey was to study the impact of stress reactions, personality factors, situation awareness, and maladaptive group dynamics on the quality of the decision-making in Swedish high-level military staffs. Participants were mainly captains and majors, but also lieutenant colonels and colonels took part (n = 256, 61 % response rate). A mainly self-made questionnaire was administered in two staff exercises. Maladaptive group dynamics, stress exposure, lack of situation awareness, and negative stress reactions were the strongest predictors of poor staff performance, while personality had less impact.

  • Personalized digital services to promote healthy behavior changes

    • Purpose and motivation: Research has repeatedly shown that physical activity contributes to physical and mental wellbeing. Nevertheless, the gap between intentions and behavior results in sedentary lifestyles, unhealthy habits, and lower wellbeing persists. This Transformative Service Research (TSR) project focuses on creating novel services that enhance the wellbeing of individuals, regardless of resources.Myriad digital services have been designed to support behavioral changes with the goal of stimulating health, such as starting to exercise, losing weight, or sleeping better. These technological innovations help users integrate resources and manage their value-creation process toward health.One important strategy for supporting behavior change is to provide feedback that helps users regulate their own behavior. Behavior change requires that digital solutions be designed for motivation and not just loaded with technical features. However, few digital solutions are equipped with functions, such as coaching, that can be adapted to fit the individuals’ value-creation process towards a better health and wellbeing. Therefore, the aims of this study were to explore whether an individually tailored digital service solution could support individual behavior changes, and to examine how the coaches experienced their situation.  Methodology: We tested how people in need of behavior changes experienced individually tailored digital support. This was done by implementing a protocol for motivational communication via short text messaging (SMS). The participants in this study were clients and wellness consultants at a non-profit organization that helps individuals who need lifestyle changes regarding, for example, unhealthy dietary habits or sedentary lifestyles. The majority of the clients have been prescribed physical activity by a physician. Data consisted of both interviews with the wellness consultants and the clients, and their SMS conversations. The interviews and SMS conversations were analyzed with the aim of identifying themes in the data. The study was approved by an ethical advisor at Karlstad University. Findings: The preliminary results show that both the wellness consultants and the clients found that the digital solution supported the clients’ motivation for behavior change, and hence goal achievements. The results further indicate that the SMS coaching enhanced the clients’ motivation and maintained an easy rapport between the face-to-face meetings. The added value provided by the SMS coaching to the existing service provision for both wellness consultant and client has led the non-profit organization to permanently offer this service to its clients.   Originality/value: This study contributes to the growing body of TSR by showing how the application of digital solutions and service research perspectives can support individuals’ behavior change processes and wellness promotion.
