    • From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions.

    • Body-mass index (BMI) has increased steadily in most countries in parallel with a rise in the proportion of the population who live in cities(.)(1,2) This has led to a widely reported view that urbanization is one of the most important drivers of the global rise in obesity(3-6). Here we use 2,009 population-based studies, with measurements of height and weight in more than 112 million adults, to report national, regional and global trends in mean BMI segregated by place of residence (a rural or urban area) from 1985 to 2017. We show that, contrary to the dominant paradigm, more than 55% of the global rise in mean BMI from 1985 to 2017-and more than 80% in some low- and middle-income regions-was due to increases in BMI in rural areas. This large contribution stems from the fact that, with the exception of women in sub-Saharan Africa, BMI is increasing at the same rate or faster in rural areas than in cities in low- and middle-income regions. These trends have in turn resulted in a closing-and in some countries reversal-of the gap in BMI between urban and rural areas in low- and middle-income countries, especially for women. In high-income and industrialized countries, we noted a persistently higher rural BMI, especially for women. There is an urgent need for an integrated approach to rural nutrition that enhances financial and physical access to healthy foods, to avoid replacing the rural undernutrition disadvantage in poor countries with a more general malnutrition disadvantage that entails excessive consumption of low-quality calories.

  • Diminishing benefits of urban living for children and adolescents' growth and development

    • Optimal growth and development in childhood and adolescence is crucial for lifelong health and well-being1–6. Here we used data from 2,325 population-based studies, with measurements of height and weight from 71 million participants, to report the height and body-mass index (BMI) of children and adolescents aged 5–19 years on the basis of rural and urban place of residence in 200 countries and territories from 1990 to 2020. In 1990, children and adolescents residing in cities were taller than their rural counterparts in all but a few high-income countries. By 2020, the urban height advantage became smaller in most countries, and in many high-income western countries it reversed into a small urban-based disadvantage. The exception was for boys in most countries in sub-Saharan Africa and in some countries in Oceania, south Asia and the region of central Asia, Middle East and north Africa. In these countries, successive cohorts of boys from rural places either did not gain height or possibly became shorter, and hence fell further behind their urban peers. The difference between the age-standardized mean BMI of children in urban and rural areas was <1.1 kg m–2 in the vast majority of countries. Within this small range, BMI increased slightly more in cities than in rural areas, except in south Asia, sub-Saharan Africa and some countries in central and eastern Europe. Our results show that in much of the world, the growth and developmental advantages of living in cities have diminished in the twenty-first century, whereas in much of sub-Saharan Africa they have amplified.

  • Outcomes after perioperative SARS-CoV-2 infection in patients with proximal femoral fractures: an international cohort study

    • Studies have demonstrated high rates of mortality in people with proximal femoral fracture and SARS-CoV-2, but there is limited published data on the factors that influence mortality for clinicians to make informed treatment decisions. This study aims to report the 30-day mortality associated with perioperative infection of patients undergoing surgery for proximal femoral fractures and to examine the factors that influence mortality in a multivariate analysis.SettingProspective, international, multicentre, observational cohort study.ParticipantsPatients undergoing any operation for a proximal femoral fracture from 1 February to 30 April 2020 and with perioperative SARS-CoV-2 infection (either 7 days prior or 30-day postoperative).Primary outcome30-day mortality. Multivariate modelling was performed to identify factors associated with 30-day mortality.ResultsThis study reports included 1063 patients from 174 hospitals in 19 countries. Overall 30-day mortality was 29.4% (313/1063). In an adjusted model, 30-day mortality was associated with male gender (OR 2.29, 95% CI 1.68 to 3.13, p<0.001), age >80 years (OR 1.60, 95% CI 1.1 to 2.31, p=0.013), preoperative diagnosis of dementia (OR 1.57, 95% CI 1.15 to 2.16, p=0.005), kidney disease (OR 1.73, 95% CI 1.18 to 2.55, p=0.005) and congestive heart failure (OR 1.62, 95% CI 1.06 to 2.48, p=0.025). Mortality at 30 days was lower in patients with a preoperative diagnosis of SARS-CoV-2 (OR 0.6, 95% CI 0.6 (0.42 to 0.85), p=0.004). There was no difference in mortality in patients with an increase to delay in surgery (p=0.220) or type of anaesthetic given (p=0.787).ConclusionsPatients undergoing surgery for a proximal femoral fracture with a perioperative infection of SARS-CoV-2 have a high rate of mortality. This study would support the need for providing these patients with individualised medical and anaesthetic care, including medical optimisation before theatre. Careful preoperative counselling is needed for those with a proximal femoral fracture and SARS-CoV-2, especially those in the highest risk groups.Trial registration numberNCT04323644

    • Being taller is associated with enhanced longevity, and higher education and earnings. We reanalysed 1472 population-based studies, with measurement of height on more than 18.6 million participants to estimate mean height for people born between 1896 and 1996 in 200 countries. The largest gain in adult height over the past century has occurred in South Korean women and Iranian men, who became 20.2 cm (95% credible interval 17.522.7) and 16.5 cm (13.319.7) taller, respectively. In contrast, there was little change in adult height in some sub-Saharan African countries and in South Asia over the century of analysis. The tallest people over these 100 years are men born in the Netherlands in the last quarter of 20th century, whose average heights surpassed 182.5 cm, and the shortest were women born in Guatemala in 1896 (140.3 cm; 135.8144.8). The height differential between the tallest and shortest populations was 19-20 cm a century ago, and has remained the same for women and increased for men a century later despite substantial changes in the ranking of countries.

    • This study describes comprehensive polling of transcription start and termination sites and analysis of previously unidentified full-length complementary DNAs derived from the mouse genome. We identify the 5′ and 3′ boundaries of 181,047 transcripts with extensive variation in transcripts arising from alternative promoter usage, splicing, and polyadenylation. There are 16,247 new mouse protein-coding transcripts, including 5154 encoding previously unidentified proteins. Genomic mapping of the transcriptome reveals transcriptional forests, with overlapping transcription on both strands, separated by deserts in which few transcripts are observed. The data provide a comprehensive platform for the comparative analysis of mammalian transcriptional regulation in differentiation and development.

  • Robust estimation of bacterial cell count from optical density

    • Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data.

    • Free-flowing rivers (FFRs) support diverse, complex and dynamic ecosystems globally, providing important societal and economic services. Infrastructure development threatens the ecosystem processes, biodiversity and services that these rivers support. Here we assess the connectivity status of 12 million kilometres of rivers globally and identify those that remain free-flowing in their entire length. Only 37 per cent of rivers longer than 1,000 kilometres remain free-flowing over their entire length and 23 per cent flow uninterrupted to the ocean. Very long FFRs are largely restricted to remote regions of the Arctic and of the Amazon and Congo basins. In densely populated areas only few very long rivers remain free-flowing, such as the Irrawaddy and Salween. Dams and reservoirs and their up- and downstream propagation of fragmentation and flow regulation are the leading contributors to the loss of river connectivity. By applying a new method to quantify riverine connectivity and map FFRs, we provide a foundation for concerted global and national strategies to maintain or restore them.

  • Terminal Complement Inhibitor Eculizumab in Atypical Hemolytic-Uremic Syndrome

    • Background Atypical hemolytic-uremic syndrome is a genetic, life-threatening, chronic disease of complement-mediated thrombotic microangiopathy. Plasma exchange or infusion may transiently maintain normal levels of hematologic measures but does not treat the underlying systemic disease. Methods We conducted two prospective phase 2 trials in which patients with atypical hemolytic-uremic syndrome who were 12 years of age or older received eculizumab for 26 weeks and during long-term extension phases. Patients with low platelet counts and renal damage (in trial 1) and those with renal damage but no decrease in the platelet count of more than 25% for at least 8 weeks during plasma exchange or infusion (in trial 2) were recruited. The primary end points included a change in the platelet count (in trial 1) and thrombotic microangiopathy event-free status (no decrease in the platelet count of >25%, no plasma exchange or infusion, and no initiation of dialysis) (in trial 2). Results A total of 37 patients (17 in trial 1 and 20 in trial 2) received eculizumab for a median of 64 and 62 weeks, respectively. Eculizumab resulted in increases in the platelet count; in trial 1, the mean increase in the count from baseline to week 26 was 73x10(9) per liter (P<0.001). In trial 2, 80% of the patients had thrombotic microangiopathy event-free status. Eculizumab was associated with significant improvement in all secondary end points, with continuous, time-dependent increases in the estimated glomerular filtration rate (GFR). In trial 1, dialysis was discontinued in 4 of 5 patients. Earlier intervention with eculizumab was associated with significantly greater improvement in the estimated GFR. Eculizumab was also associated with improvement in health-related quality of life. No cumulative toxicity of therapy or serious infection-related adverse events, including meningococcal infections, were observed through the extension period. Conclusions Eculizumab inhibited complement-mediated thrombotic microangiopathy and was associated with significant time-dependent improvement in renal function in patients with atypical hemolytic-uremic syndrome.

  • Efficient prediction of human protein-protein interactions at a global scale

    • Background: Our knowledge of global protein-protein interaction (PPI) networks in complex organisms such as humans is hindered by technical limitations of current methods. Results: On the basis of short co-occurring polypeptide regions, we developed a tool called MP-PIPE capable of predicting a global human PPI network within 3 months. With a recall of 23% at a precision of 82.1%, we predicted 172,132 putative PPIs. We demonstrate the usefulness of these predictions through a range of experiments. Conclusions: The speed and accuracy associated with MP-PIPE can make this a potential tool to study individual human PPI networks (from genomic sequences alone) for personalized medicine.

    • Isoniazid is an important antitubercular molecule identified as a drug of choice in tuberculosis treatment. As such, INH is an inactive prodrug; it acquires an active conformation by forming an adduct with NAD. The adduct targets inhA protein, a reductase responsible for fatty acid chain elongation in the cell wall of Mycobacterium tuberculosis. Resistance to INH is majorly contributed by mutations in inhA, katG and geneic and non-geneic regions associated with efflux genes. Despite being widespread, the mechanism of resistance remains unknown in similar to 15% of INH-resistant strains. Studies report that an intracellular increase in NADH concentration prevents inhA inhibition, leading to INH resistance. In the pursuit of finding possible resistance mechanisms, we set out to find NAD binding proteins to explore similarities in structure and NAD binding property of these proteins with that of inhA. We identified 172 NAD binding proteins, of which 53 were identified to have sequence or structural similarity to inhA. By performing docking analysis on selected proteins, we identified INH-adduct to have good binding affinity despite very minimal structural similarity to inhA. This analysis was further supported by principal component analysis, which identified 65 proteins with NAD binding conformation similar to that of inhA. These findings prompt us to hypothesize that upon exposure to INH, bacteria tries to reduce inhA susceptibility by inducing expression of these NAD binding proteins through increase in NADH concentration. This in turn favours off-target binding and leads to decreased binding and potency of INH, thus contributing indirectly to INH resistance.

    • Excited states in Ta-164 were populated in the Nd-142(Al-27,5n)Ta-164 reaction and investigated using the AFRODITE spectrometer Arran. The yrast rotational decay sequence Lip to spin 21(-) is identified for the first time and assigned to the pi[514]9/2(-) x nu[660]1/2(+) configuration. A less intense. excited hand is also identified. Low-spin signature inversion is observed in the yrast band, and is compared with the extended total Routhian surface calculations. It is likely that quadrupole pairing plays a role in generating the anomalous signature splitting, but an additional mechanism apart front the assumed mean field and quadrupole pairing interaction may be required to account for the experimental data.

    • Long Term Evolution (LTE) is the last step towards the 4th generation of cellular networks. This revolution is necessitated by the unceasing increase in demand for high speed connection on LTE networks. This paper focuses on the performance evaluation of End-to-End delay under variable mobility speed for VoIP (Voice over IP) in the LTE network. In the course of E2E performance evaluation, realizing simulation approach three scenarios have been modeled using OPNET 16.0. The first one is the baseline network while among other two, one consists of VoIP traffic solely and the other consists of FTP along with VoIP. E2E delay has been measured for both scenarios in various cases under the varying mobility speed of the node. Simulation results have been studied and presented in terms of comparative performance analysis of the three network scenarios. In light of the result analysis, the performance quality of a VoIP network (with and without the presence of additional network traffic) in LTE has been determined and discussed. The simulation results for baseline VoIP network (non-congested) congested VoIP network and congested VoIP with FTP network show that as the speed of node is gradually increased, E2E delay slightly increases.

    • The mitochondrial human proteome project (mt-HPP) was initiated by the Italian HPP group as a part of both the chromosome-centric initiative (C-HPP) and the "biology and disease driven" initiative (B/D-HPP). In recent years several reports highlighted how mitochondrial biology and disease are regulated by specific interactions with non-mitochondrial proteins. Thus, it is of great relevance to extend our present view of the mitochondrial proteome not only to those proteins that are encoded by or transported to mitochondria, but also to their interactors that take part in mitochondria functionality. Here, we propose a graphical representation of the functional mitochondrial proteome by retrieving mitochondrial proteins from the NeXtProt database and adding to the network their interactors as annotated in the IntAct database. Notably, the network may represent a reference to map all the proteins that are currently being identified in mitochondrial proteomics studies.

    • Detailed analysis was carried out on proton and a neutron irradiated Nb-containing Zr-alloy to study the evolution of dislocation loop size and densities as well as the formation and evolution of irradiation induced precipitation/clustering. The results obtained here have been contrasted against previously published work on a Nb-free Zr-alloy [1, 21 to investigate the mechanistic reason for the improved resistance to irradiation-induced growth of Nb-containing Zr alloys. The combined use of bright field scanning transmission electron microscopy, ultra-high-resolution energy dispersive spectroscopy and atom probe tomography analysis provides evidence of evenly distributed radiation-induced Nb dusters that have formed during the early stage of proton irradiation and Fe-rich nano-rods near Fe-containing second phase particles. The former seems to have a profound effect on loop and subsequent loop formation, keeping loop size small but number density high while loops seem to initially form at similar dose levels compared to a Nb-free alloy but loop line density does not increase during further irradiation. It is hypothesized that the formation of the Nb nano-precipitates/clusters significantly hinders mobility and growth of loops, resulting in a small size, high number density and limited ability of loops to arrange along basal traces compared to Nb-free Zr-alloys. It is suggested that it is the limited loop arrangement that slows down loop formation and the root cause for the high resistance of Nb-containing Zr-alloys to irradiation-induced growth.

    • The effect of Zn/Gd ratio on dynamic recrystallization (DRX) of Mg-2Gd-xZn (x = 0, 1, 2 and 3 wt%) alloys was investigated by shear punch tests in the temperature range of 623-723 K and shear strain rate range of 1.0 x 10(-2)-1.2 x 10(-1) s(-1). It was observed that at low Zn/Gd ratio, excessive co-segregation of Gd and Zn solute atoms retards recrystallization and provides higher strength. At high Zn/Gd ratios, precipitation reduces the co-segregation, so that the alloy with Zn/Gd = 1.5, experienced the fastest DRX and the lowest strength. In addition, segregation resulted in a weaker texture, by elimination of nucleation and growth of the preferred orientations. rights reserved.

    • In the current study, microstructural evolution and superplasticity of an extruded Mg-2wt% Gd sheet were studied after the constrained groove pressing (CGP) process. Microstructural observations by scanning electron microscopy and electron backscattered diffraction revealed that after 4 cycles of CGP, a rather homogeneous fine-grained microstructure with an average grain size of 4.3 mu m, and a large fraction of high angle grain boundaries was obtained. By performing shear punch tests (SPT) at different temperatures and various shear strain rates, a peak strain rate sensitivity index (m-value) of 0.49 was obtained after 4 cycles of CGP process at 673 K, while peak m-values of 0.31 and 0.36 were obtained for the as-extruded and 2 cycle CGP process conditions, respectively. An m-value of 0.49 and an activation energy of 113 kJ/mol, obtained for the fine-grained material after 4 cycles of CGP, suggest that the dominant deformation mechanism in the superplastic regime is grain boundary sliding (GBS) controlled by grain boundary diffusion.

    • The effect of Zn content on the microstructure, texture, mechanical properties and strain-hardening behavior of extruded Mg-2Gd-xZn (x = 0, 1, 2 and 3 wt%) sheets was investigated. Evaluation of texture revealed that while all of the alloys exhibited weak textures, the texture component was altered from a basal to a non-basal one by the addition of Zn. A typical transverse direction (TD) split texture with basal poles rotated about 40 degrees from the normal direction (ND) toward TD was observed for the Zn-containing alloys, the effect being more pronounced at higher Zn contents. Furthermore, the Mg-2Gd-1Zn alloy exhibited the weakest texture due to solute drag imposed by co-segregation of Zn and Gd atoms at grain boundaries. Addition of Zn also resulted in a general increase in yield stress, ultimate tensile strength and elongation along the extrusion direction from 99 to 172 MPa, 178 to 263 MPa, and 25 to 35% for Mg-2Gd and Mg-2Gd-3Zn alloys, respectively. However, increasing Zn content was accompanied by an initial decrease in anisotropy of mechanical properties and strain-hardening behavior, followed by an increase at higher Zn contents. This was due to the difference of orientation of basal planes with regard to tension direction. As a result, lower yield stress, higher elongation and strain-hardening capacity was obtained along TD (with higher Schmid factor for basal slip) compared to ED. It was concluded that excellent mechanical properties and low anisotropy can be achieved in the Mg-2Gd-1Zn alloy.

    • Thermal stability, structure and mechanical properties of the multi-component Zr 58.5Ti 8.2Cu 14.2Ni 11.4Al 7.7 bulk metallic glass have been studied in detail. The glassy material displays good thermal stability against crystallization and a fairly large supercooled liquid region of 52 K. During heating, the alloy transforms into a metastable icosahedral quasicrystalline phase in the first stage of crystallization. At high temperatures, the quasicrystalline phase undergoes a transformation to form tetragonal and cubic NiZr 2-type phases. Room-temperature compression tests of the as-cast sample show good mechanical properties, namely, high compressive strength of about 1,630 MPa and fracture strain of 3.3%. This is combined with a density of 6.32 g/cm 3 and values of Poisson's ratio and Young's modulus of 0.377 and 77 GPa, respectively. The mechanical properties of the glass can be further improved by cold rolling. The compressive strength rises to 1,780 MPa and the fracture strain increases to 8.3% for the material cold-rolled to a diameter reduction of 10%.

    • The global coronavirus disease 2019 (COVID-19) pandemic has rapidly increased the demand for facemasks as a measure to reduce the rapid spread of the pathogen. Throughout the pandemic, some countries such as Italy had a monthly demand of ca. 90 million facemasks. Domestic mask manufacturers are capable of manufacturing 8 million masks each week, although the demand was 40 million per week during March 2020. This dramatic increase has contributed to a spike in the generation of facemask waste. Facemasks are often manufactured with synthetic materials that are non-biodegradable, and their increased usage and improper disposal are raising environmental concerns. Consequently, there is a strong interest for developing biodegradable facemasks made with for example, renewable nanofibres. A range of natural polymer-based nanofibres has been studied for their potential to be used in air filter applications. This review article examines potential natural polymer-based nanofibres along with their filtration and antimicrobial capabilities for developing biodegradable facemask that will promote a cleaner production.
