    • To investigate the genetic basis of type 2 diabetes (T2D) to high resolution, the GoT2D and T2D-GENES consortia catalogued variation from whole-genome sequencing of 2,657 European individuals and exome sequencing of 12,940 individuals of multiple ancestries. Over 27M SNPs, indels, and structural variants were identified, including 99% of low-frequency (minor allele frequency [MAF] 0.1-5%) non-coding variants in the whole-genome sequenced individuals and 99.7% of low-frequency coding variants in the whole-exome sequenced individuals. Each variant was tested for association with T2D in the sequenced individuals, and, to increase power, most were tested in larger numbers of individuals (> 80% of low-frequency coding variants in similar to ~82 K Europeans via the exome chip, and similar to ~90% of low-frequency non-coding variants in similar to ~44 K Europeans via genotype imputation). The variants, genotypes, and association statistics from these analyses provide the largest reference to date of human genetic information relevant to T2D, for use in activities such as T2D-focused genotype imputation, functional characterization of variants or genes, and other novel analyses to detect associations between sequence variation and T2D.

  • TRY plant trait database - enhanced coverage and open access

    • Plant traits-the morphological, anatomical, physiological, biochemical and phenological characteristics of plants-determine how plants respond to environmental factors, affect other trophic levels, and influence ecosystem properties and their benefits and detriments to people. Plant trait data thus represent the basis for a vast area of research spanning from evolutionary biology, community and functional ecology, to biodiversity conservation, ecosystem and landscape management, restoration, biogeography and earth system modelling. Since its foundation in 2007, the TRY database of plant traits has grown continuously. It now provides unprecedented data coverage under an open access data policy and is the main plant trait database used by the research community worldwide. Increasingly, the TRY database also supports new frontiers of trait-based plant research, including the identification of data gaps and the subsequent mobilization or measurement of new data. To support this development, in this article we evaluate the extent of the trait data compiled in TRY and analyse emerging patterns of data coverage and representativeness. Best species coverage is achieved for categorical traits-almost complete coverage for 'plant growth form'. However, most traits relevant for ecology and vegetation modelling are characterized by continuous intraspecific variation and trait-environmental relationships. These traits have to be measured on individual plants in their respective environment. Despite unprecedented data coverage, we observe a humbling lack of completeness and representativeness of these continuous traits in many aspects. We, therefore, conclude that reducing data gaps and biases in the TRY database remains a key challenge and requires a coordinated approach to data mobilization and trait measurements. This can only be achieved in collaboration with other initiatives.

  • Deep learning-based segmentation of multisite disease in ovarian cancer

    • Purpose: To determine if pelvic/ovarian and omental lesions of ovarian cancer can be reliably segmented on computed tomography (CT) using fully automated deep learning-based methods.Methods: A deep learning model for the two most common disease sites of high-grade serous ovarian cancer lesions (pelvis/ovaries and omentum) was developed and compared against the well-established “no-new-Net” framework and unrevised trainee radiologist segmentations. A total of 451 CT scans collected from four different institutions were used for training (n = 276), evaluation (n = 104) and testing (n = 71) of the methods. The performance was evaluated using the Dice similarity coefficient (DSC) and compared using a Wilcoxon test.Results: Our model outperformed no-new-Net for the pelvic/ovarian lesions in cross-validation, on the evaluation and test set by a significant margin (p values being 4 × 10–7, 3 × 10–4, 4 × 10–2, respectively), and for the omental lesions on the evaluation set (p = 1 × 10–3). Our model did not perform significantly differently in segmenting pelvic/ovarian lesions (p = 0.371) compared to a trainee radiologist. On an independent test set, the model achieved a DSC performance of 71 ± 20 (mean ± standard deviation) for pelvic/ovarian and 61 ± 24 for omental lesions.Conclusion: Automated ovarian cancer segmentation on CT scans using deep neural networks is feasible and achieves performance close to a trainee-level radiologist for pelvic/ovarian lesions.Relevance statement: Automated segmentation of ovarian cancer may be used by clinicians for CT-based volumetric assessments and researchers for building complex analysis pipelines.Key points:The first automated approach for pelvic/ovarian and omental ovarian cancer lesion segmentation on CT images has been presented.Automated segmentation of ovarian cancer lesions can be comparable with manual segmentation of trainee radiologists.Careful hyperparameter tuning can provide models significantly outperforming strong state-of-the-art baselines.   • Somatic mutations affect key pathways in lung adenocarcinoma

    • Determining the genetic basis of cancer requires comprehensive analyses of large collections of histopathologically well-classified primary tumours. Here we report the results of a collaborative study to discover somatic mutations in 188 human lung adenocarcinomas. DNA sequencing of 623 genes with known or potential relationships to cancer revealed more than 1,000 somatic mutations across the samples. Our analysis identified 26 genes that are mutated at significantly high frequencies and thus are probably involved in carcinogenesis. The frequently mutated genes include tyrosine kinases, among them the EGFR homologue ERBB4; multiple ephrin receptor genes, notably EPHA3; vascular endothelial growth factor receptor KDR; and NTRK genes. These data provide evidence of somatic mutations in primary lung adenocarcinoma for several tumour suppressor genes involved in other cancers--including NF1, APC, RB1 and ATM--and for sequence changes in PTPRD as well as the frequently deleted gene LRP1B. The observed mutational profiles correlate with clinical features, smoking status and DNA repair defects. These results are reinforced by data integration including single nucleotide polymorphism array and gene expression array. Our findings shed further light on several important signalling pathways involved in lung adenocarcinoma, and suggest new molecular targets for treatment.

  • Carotid artery stenting compared with endarterectomy in patients with symptomatic carotid stenosis (International Carotid Stenting Study): an interim analysis of a randomised controlled trial

    • Background Stents are an alternative treatment to carotid endarterectomy for symptomatic carotid stenosis, but previous trials have not established equivalent safety and efficacy. We compared the safety of carotid artery stenting with that of carotid endarterectomy. Methods The International Carotid Stenting Study (ICSS) is a multicentre, international, randomised controlled trial with blinded adjudication of outcomes. Patients with recently symptomatic carotid artery stenosis were randomly assigned in a 1:1 ratio to receive carotid artery stenting or carotid endarterectomy. Randomisation was by telephone call or fax to a central computerised service and was stratified by centre with minimisation for sex, age, contralateral occlusion, and side of the randomised artery. Patients and investigators were not masked to treatment assignment. Patients were followed up by independent clinicians not directly involved in delivering the randomised treatment. The primary outcome measure of the trial is the 3-year rate of fatal or disabling stroke in any territory, which has not been analysed yet. The main outcome measure for the interim safety analysis was the 120-day rate of stroke, death, or procedural myocardial infarction. Analysis was by intention to treat (ITT). This study is registered, number ISRCTN25337470. Findings The trial enrolled 1713 patients (stenting group, n=855; endarterectomy group, n=858). Two patients in the stenting group and one in the endarterectomy group withdrew immediately after randomisation, and were not included in the ITT analysis. Between randomisation and 120 days, there were 34 (Kaplan-Meier estimate 4.0%) events of disabling stroke or death in the stenting group compared with 27 (3.2%) events in the endarterectomy group (hazard ratio [HR] 1.28, 95% CI 0.77-2.11). The incidence of stroke, death, or procedural myocardial infarction was 8.5% in the stenting group compared with 5.2% in the endarterectomy group (72 vs 44 events; HR 1.69, 1.16-2.45, p=0.006), Risks of any stroke (65 vs 35 events; HR 1.92, 1.27-2.89) and all-cause death (19 vs seven events; HR 2.76, 1.16-6.56) were higher in the stenting group than in the endarterectomy group. Three procedural myocardial infarctions were recorded in the stenting group, all of which were fatal, compared with four, all non-fatal, in the endarterectomy group. There was one event of cranial nerve palsy in the stenting group compared with 45 in the endarterectomy group. There were also fewer haematomas of any severity in the stenting group than in the endarterectomy group (31 vs 50 events; p=0.0197). Interpretation Completion of long-term follow-up is needed to establish the efficacy of carotid artery stenting compared with endarterectomy. In the meantime, carotid endarterectomy should remain the treatment of choice for patients suitable for surgery.

  • Long-term deglacial permafrost carbon dynamics in MPI-ESM

    • We have developed a new module to calculate soil organic carbon (SOC) accumulation in perennially frozen ground in the land surface model JSBACH. Running this offline version of MPI-ESM we have modelled long-term permafrost carbon accumulation and release from the Last Glacial Maximum (LGM) to the pre-industrial (PI) age. Our simulated near-surface PI permafrost extent of 16.9 x 10(6) km(2) is close to observational estimates. Glacial boundary conditions, especially ice sheet coverage, result in profoundly different spatial patterns of glacial permafrost extent. Deglacial warming leads to large-scale changes in soil temperatures, manifested in permafrost disappearance in southerly regions, and permafrost aggregation in formerly glaciated grid cells. In contrast to the large spatial shift in simulated permafrost occurrence, we infer an only moderate increase in total LGM permafrost area (18.3 x 10(6) km(2)) - together with pronounced changes in the depth of seasonal thaw. Earlier empirical reconstructions suggest a larger spread of permafrost towards more southerly regions under glacial conditions, but with a highly uncertain extent of non-continuous permafrost. Compared to a control simulation without describing the transport of SOC into perennially frozen ground, the implementation of our newly developed module for simulating permafrost SOC accumulation leads to a doubling of simulated LGM permafrost SOC storage (amounting to a total of similar to 150 PgC). Despite LGM temperatures favouring a larger permafrost extent, simulated cold glacial temperatures - together with low precipitation and low CO2 levels - limit vegetation productivity and therefore prevent a larger glacial SOC build-up in our model. Changes in physical and biogeochemical boundary conditions during deglacial warming lead to an increase in mineral SOC storage towards the Holocene (168 PgC at PI), which is below observational estimates (575 PgC in continuous and discontinuous permafrost). Additional model experiments clarified the sensitivity of simulated SOC storage to model parameters, affecting long-term soil carbon respiration rates and simulated ALDs. Rather than a steady increase in carbon release from the LGM to PI as a consequence of deglacial permafrost degradation, our results suggest alternating phases of soil carbon accumulation and loss as an effect of dynamic changes in permafrost extent, ALDs, soil litter input, and heterotrophic respiration.

This paper motivates and discusses the development of a context-aware information push service. A survey is presented motivating the implementation of such a system. Further, key requirements are identified leading to a description of the system architecture, thereby giving insight into how the requirements are tackled. Finally, the rule language which is used to formally declare push processes and the inference engine translating such rules into concrete push messages is explained in detail.     • The focus of this paper is to show that the E-C-A paradigm offers an excellent approach for specifying the behavior of context-aware information push services. Such a service enables its operator to provide the users with tailored messages related to their current situation (context). The paper introduces CAIPS, an implementation of such a service for the tourism domain. The underlying E-C-A rules are presented and the design of the associated rule-engine is described. The engine’s rule-interpreter is based on event-notification services and the object-oriented query-language HQL. The paper further presents a graphical high-level editor which supports business-experts in “writing” the CAIPS E-C-A rules. The presented approach enables the rapid development of new tailored messages (related to the user’s context) without the need to modify the underlying application, i.e. without the trouble of writing new code for new message types.

    • Wetlands of northern high latitudes are ecosystems highly vulnerable to climate change. Some degradation effects include soil hydrologic changes due to permafrost thaw, formation of deeper active layers, and rising topsoil temperatures that accelerate the degradation of permafrost carbon and increase in CO2 and CH4 emissions. In this work we present 2 years of modeled year-round CH4 emissions into the atmosphere from a Northeast Siberian region in the Russian Far East. We use a revisited version of the process-based JSBACH-methane model that includes four CH4 transport pathways: plant-mediated transport, ebullition and molecular diffusion in the presence or absence of snow. The gas is emitted through wetlands represented by grid cell inundated areas simulated with a TOPMODEL approach. The magnitude of the summertime modeled CH4 emissions is comparable to ground-based CH4 fluxes measured with the eddy covariance technique and flux chambers in the same area of study, whereas wintertime modeled values are underestimated by 1 order of magnitude. In an annual balance, the most important mechanism for transport of methane into the atmosphere is through plants (61 %). This is followed by ebullition (similar to 35 %), while summertime molecular diffusion is negligible (0.02 %) compared to the diffusion through the snow during winter (similar to 4 %). We investigate the relationship between temporal changes in the CH4 fluxes, soil temperature, and soil moisture content. Our results highlight the heterogeneity in CH4 emissions at landscape scale and suggest that further improvements to the representation of large-scale hydrological conditions in the model will facilitate a more process-oriented land surface scheme and better simulate CH4 emissions under climate change. This is especially necessary at regional scales in Arctic ecosystems influenced by permafrost thaw.

    • It is important that climate models can accurately simulate the terrestrial carbon cycle in the Arctic due to the large and potentially labile carbon stocks found in permafrost-affected environments, which can lead to a positive climate feedback, along with the possibility of future carbon sinks from northward expansion of vegetation under climate warming. Here we evaluate the simulation of tundra carbon stocks and fluxes in three land surface schemes that each form part of major Earth system models (JSBACH, Germany; JULES, UK; ORCHIDEE, France). We use a site-level approach in which comprehensive, high-frequency datasets allow us to disentangle the importance of different processes. The models have improved physical permafrost processes and there is a reasonable correspondence between the simulated and measured physical variables, including soil temperature, soil moisture and snow. We show that if the models simulate the correct leaf area index (LAI), the standard C3 photosynthesis schemes produce the correct order of magnitude of carbon fluxes. Therefore, simulating the correct LAI is one of the first priorities. LAI depends quite strongly on climatic variables alone, as we see by the fact that the dynamic vegetation model can simulate most of the differences in LAI between sites, based almost entirely on climate inputs. However, we also identify an influence from nutrient limitation as the LAI becomes too large at some of the more nutrient-limited sites. We conclude that including moss as well as vascular plants is of primary importance to the carbon budget, as moss contributes a large fraction to the seasonal CO2 flux in nutrient-limited conditions. Moss photosynthetic activity can be strongly influenced by the moisture content of moss, and the carbon uptake can be significantly different from vascular plants with a similar LAI. The soil carbon stocks depend strongly on the rate of input of carbon from the vegetation to the soil, and our analysis suggests that an improved simulation of photosynthesis would also lead to an improved simulation of soil carbon stocks. However, the stocks are also influenced by soil carbon burial (e.g. through cryoturbation) and the rate of heterotrophic respiration, which depends on the soil physical state. More detailed below-ground measurements are needed to fully evaluate biological and physical soil processes. Furthermore, even if these processes are well modelled, the soil carbon profiles cannot resemble peat layers as peat accumulation processes are not represented in the models. Thus, we identify three priority areas for model development: (1) dynamic vegetation including (a) climate and (b) nutrient limitation effects; (2) adding moss as a plant functional type; and an (3) improved vertical profile of soil carbon including peat processes.

    • Background: Function after acute stroke using the modified Rankin Scale (mRS) is usually assessed at a point in time. The analytical implications of serial mRS measurements to evaluate functional recovery over time is not completely understood. We compare repeated-measures and single-measure analyses of the mRS from a randomized clinical trialMethods and Results: Serial mRS data from AFFINITY (Assessment of Fluoxetine in Stroke Recovery), a double-blind placebo randomized clinical trial of fluoxetine following stroke (n=1280) were analyzed to identify demographic and clinical associations with functional recovery (reduction in mRS) over 12 months. Associations were identified using single-measure (day 365) and repeated-measures (days 28, 90, 180, and 365) partial proportional odds logistic regression. Ninety-five percent of participants experienced a reduction in mRS after 12 months. Functional recovery was associated with age at stroke <70 years; no prestroke history of diabetes, coronary heart disease, or ischemic stroke; prestroke history of depression, a relationship partner, living with others, independence, or paid employment; no fluoxetine intervention; ischemic stroke (compared with hemorrhagic); stroke treatment in Vietnam (compared with Australia or New Zealand); longer time since current stroke; and lower baseline National Institutes of Health Stroke Scale & Patient Health Questionnaire-9 scores. Direction of associations was largely concordant between single-measure and repeated-measures models. Association strength and variance was generally smaller in the repeated-measures model compared with the single-measure model.Conclusions: Repeated-measures may improve trial precision in identifying trial associations and effects.   • Multidecadal trend analysis of in situ aerosol radiative properties around the world

    • In order to assess the evolution of aerosol parameters affecting climate change, a long-term trend analysis of aerosol optical properties was performed on time series from 52 stations situated across five continents. The time series of measured scattering, backscattering and absorption coefficients as well as the derived single scattering albedo, backscattering fraction, scattering and absorption Angstrom exponents covered at least 10 years and up to 40 years for some stations. The non-parametric seasonal Mann-Kendall (MK) statistical test associated with several pre-whitening methods and with Sen's slope was used as the main trend analysis method. Comparisons with general least mean square associated with autoregressive bootstrap (GLS/ARB) and with standard least mean square analysis (LMS) enabled confirmation of the detected MK statistically significant trends and the assessment of advantages and limitations of each method. Currently, scattering and backscattering coefficient trends are mostly decreasing in Europe and North America and are not statistically significant in Asia, while polar stations exhibit a mix of increasing and decreasing trends. A few increasing trends are also found at some stations in North America and Australia. Absorption coefficient time series also exhibit primarily decreasing trends. For single scattering albedo, 52 % of the sites exhibit statistically significant positive trends, mostly in Asia, eastern/northern Europe and the Arctic, 22 % of sites exhibit statistically significant negative trends, mostly in central Europe and central North America, while the remaining 26 % of sites have trends which are not statistically significant. In addition to evaluating trends for the overall time series, the evolution of the trends in sequential 10-year segments was also analyzed. For scattering and backscattering, statistically significant increasing 10-year trends are primarily found for earlier periods (10-year trends ending in 2010-2015) for polar stations and Mauna Loa. For most of the stations, the present-day statistically significant decreasing 10-year trends of the single scattering albedo were preceded by not statistically significant and statistically significant increasing 10-year trends. The effect of air pollution abatement policies in continental North America is very obvious in the 10-year trends of the scattering coefficient - there is a shift to statistically significant negative trends in 2009-2012 for all stations in the eastern and central USA. This long-term trend analysis of aerosol radiative properties with a broad spatial coverage provides insight into potential aerosol effects on climate changes.

The present case study evaluates the effect of unspecified variability of raw material properties upon the quality attributes of granules; produced using a continuous from-powder-to-tablet wet granulation line (ConsiGma (TM) 25). The impact of different material attributes of six samples of microcrystalline cellulose (MCC) was investigated. During a blind study the different samples of MCC were used separately and the resulting granules were evaluated in order to identify the differences between the six samples. Variation in size distribution due to varying water binding capacity of the MCC samples was observed. The cause of this different water binding capacity was investigated and was caused by a different degree of crystallinity. Afterwards, an experimental design was conducted in order to evaluate the effect of both product and process variability upon the granule size distribution. This model was used in order to calculate the required process parameters to obtain a preset granule size distribution regardless of the type of MCC used. The difference in water binding capacity and its effect on granular properties was still present when combining the MCC grades with different binders.

    • Mobile guides (based on PDAs, smart phones, or mobile phones) play an increasingly important role in tourism, giving tourists ubiquitous access to relevant information especially during their trip. Due to a more difficult access to mobile applications in a ubiquitous usage environment, based on time constraints, lighting conditions, bandwidth, etc., user acceptance of mobile applications strongly depends on the application adaptation to the concrete usage context. This article presents a framework for mobile applications in tourism, enabling a flexible implementation of adaptive, context-aware tourism applications. The framework especially provides approaches for user interface adaptation, content adaptation (recommendation), and interaction modality adaptation. The framework has been prototypically instantiated and evaluated in two different application scenarios, a city guide for the city of Innsbruck and a skiing guide for the ski resort DolomitiSuperski. Both application scenarios showed high usage rates and customer satisfaction and proved the applicability and effectiveness of the presented approach for developing adaptive mobile tourism applications.

    • BACKGROUND AND PURPOSE: The AFFINITY trial (Assessment of Fluoxetine in Stroke Recovery) reported that oral fluoxetine 20 mg daily for 6 months after acute stroke did not improve functional outcome and increased the risk of falls, bone fractures, and seizures. After trial medication was ceased at 6 months, survivors were followed to 12 months post-randomization. This preplanned secondary analysis aimed to determine any sustained or delayed effects of fluoxetine at 12 months post-randomization. METHODS: AFFINITY was a randomized, parallel-group, double-blind, placebo-controlled trial in adults (n=1280) with a clinical diagnosis of stroke in the previous 2 to 15 days and persisting neurological deficit who were recruited at 43 hospital stroke units in Australia (n=29), New Zealand (4), and Vietnam (10) between 2013 and 2019. Participants were randomized to oral fluoxetine 20 mg once daily (n=642) or matching placebo (n=638) for 6 months and followed until 12 months after randomization. The primary outcome was function, measured by the modified Rankin Scale, at 6 months. Secondary outcomes for these analyses included measures of the modified Rankin Scale, mood, cognition, overall health status, fatigue, health-related quality of life, and safety at 12 months. RESULTS: Adherence to trial medication was for a mean 167 (SD 48) days and similar between randomized groups. At 12 months, the distribution of modified Rankin Scale categories was similar in the fluoxetine and placebo groups (adjusted common odds ratio, 0.93 [95% CI, 0.76-1.14]; P=0.46). Compared with placebo, patients allocated fluoxetine had fewer recurrent ischemic strokes (14 [2.18%] versus 29 [4.55%]; P=0.02), and no longer had significantly more falls (27 [4.21%] versus 15 [2.35%]; P=0.08), bone fractures (23 [3.58%] versus 11 [1.72%]; P=0.05), or seizures (11 [1.71%] versus 8 [1.25%]; P=0.64) at 12 months. CONCLUSIONS: Fluoxetine 20 mg daily for 6 months after acute stroke had no delayed or sustained effect on functional outcome, falls, bone fractures, or seizures at 12 months poststroke. The lower rate of recurrent ischemic stroke in the fluoxetine group is most likely a chance finding.

The Assessment oF FluoxetINe In sTroke recoverY (AFFINITY) trial aimed to show if daily oral fluoxetine for 6 months after stroke improves functional outcome in an ethnically diverse population.METHODS: AFFINITY was a randomised, parallel-group, double-blind, placebo-controlled trial done in 43 hospital stroke units in Australia (n=29), New Zealand (four), and Vietnam (ten). Eligible patients were adults (aged ≥18 years) with a clinical diagnosis of acute stroke in the previous 2-15 days, brain imaging consistent with ischaemic or haemorrhagic stroke, and a persisting neurological deficit that produced a modified Rankin Scale (mRS) score of 1 or more. Patients were randomly assigned 1:1 via a web-based system using a minimisation algorithm to once daily, oral fluoxetine 20 mg capsules or matching placebo for 6 months. Patients, carers, investigators, and outcome assessors were masked to the treatment allocation. The primary outcome was functional status, measured by the mRS, at 6 months. The primary analysis was an ordinal logistic regression of the mRS at 6 months, adjusted for minimisation variables. Primary and safety analyses were done according to the patient's treatment allocation. The trial is registered with the Australian New Zealand Clinical Trials Registry, ACTRN12611000774921.FINDINGS: Between Jan 11, 2013, and June 30, 2019, 1280 patients were recruited in Australia (n=532), New Zealand (n=42), and Vietnam (n=706), of whom 642 were randomly assigned to fluoxetine and 638 were randomly assigned to placebo. Mean duration of trial treatment was 167 days (SD 48·1). At 6 months, mRS data were available in 624 (97%) patients in the fluoxetine group and 632 (99%) in the placebo group. The distribution of mRS categories was similar in the fluoxetine and placebo groups (adjusted common odds ratio 0·94, 95% CI 0·76-1·15; p=0·53). Compared with patients in the placebo group, patients in the fluoxetine group had more falls (20 [3%] vs seven [1%]; p=0·018), bone fractures (19 [3%] vs six [1%]; p=0·014), and epileptic seizures (ten [2%] vs two [<1%]; p=0·038) at 6 months.INTERPRETATION: Oral fluoxetine 20 mg daily for 6 months after acute stroke did not improve functional outcome and increased the risk of falls, bone fractures, and epileptic seizures.     • Background : The rising global cancer burden has led to an increasing demand for imaging tests such as [18F]fluorodeoxyglucose ([18F]FDG)-PET-CT. To aid imaging specialists in dealing with high scan volumes, we aimed to train a deep learning artificial intelligence algorithm to classify [18F]FDG-PET-CT scans of patients with lymphoma with or without hypermetabolic tumour sites. Methods : In this retrospective analysis we collected 16 583 [18F]FDG-PET-CTs of 5072 patients with lymphoma who had undergone PET-CT before or after treatment at the Memorial Sloa Kettering Cancer Center, New York, NY, USA. Using maximum intensity projection (MIP), three dimensional (3D) PET, and 3D CT data, our ResNet34-based deep learning model (Lymphoma Artificial Reader System [LARS]) for [18F]FDG-PET-CT binary classification (Deauville 1–3 vs 4–5), was trained on 80% of the dataset, and tested on 20% of this dataset. For external testing, 1000 [18F]FDG-PET-CTs were obtained from a second centre (Medical University of Vienna, Vienna, Austria). Seven model variants were evaluated, including MIP-based LARS-avg (optimised for accuracy) and LARS-max (optimised for sensitivity), and 3D PET-CT-based LARS-ptct. Following expert curation, areas under the curve (AUCs), accuracies, sensitivities, and specificities were calculated. Findings : In the internal test cohort (3325 PET-CTs, 1012 patients), LARS-avg achieved an AUC of 0·949 (95% CI 0·942–0·956), accuracy of 0·890 (0·879–0·901), sensitivity of 0·868 (0·851–0·885), and specificity of 0·913 (0·899–0·925); LARS-max achieved an AUC of 0·949 (0·942–0·956), accuracy of 0·868 (0·858–0·879), sensitivity of 0·909 (0·896–0·924), and specificity of 0·826 (0·808–0·843); and LARS-ptct achieved an AUC of 0·939 (0·930–0·948), accuracy of 0·875 (0·864–0·887), sensitivity of 0·836 (0·817–0·855), and specificity of 0·915 (0·901–0·927). In the external test cohort (1000 PET-CTs, 503 patients), LARS-avg achieved an AUC of 0·953 (0·938–0·966), accuracy of 0·907 (0·888–0·925), sensitivity of 0·874 (0·843–0·904), and specificity of 0·949 (0·921–0·960); LARS-max achieved an AUC of 0·952 (0·937–0·965), accuracy of 0·898 (0·878–0·916), sensitivity of 0·899 (0·871–0·926), and specificity of 0·897 (0·871–0·922); and LARS-ptct achieved an AUC of 0·932 (0·915–0·948), accuracy of 0·870 (0·850–0·891), sensitivity of 0·827 (0·793–0·863), and specificity of 0·913 (0·889–0·937). Interpretation : Deep learning accurately distinguishes between [18F]FDG-PET-CT scans of lymphoma patients with and without hypermetabolic tumour sites. Deep learning might therefore be potentially useful to rule out the presence of metabolically active disease in such patients, or serve as a second reader or decision support tool.     • A detailed process-based methane module for a global land surface scheme has been developed which is general enough to be applied in permafrost regions as well as wetlands outside permafrost areas. Methane production, oxidation and transport by ebullition, diffusion and plants are represented. In this model, oxygen has been explicitly incorporated into diffusion, transport by plants and two oxidation processes, of which one uses soil oxygen, while the other uses oxygen that is available via roots. Permafrost and wetland soils show special behaviour, such as variable soil pore space due to freezing and thawing or water table depths due to changing soil water content. This has been integrated directly into the methane-related processes. A detailed application at the Samoylov polygonal tundra site, Lena River Delta, Russia, is used for evaluation purposes. The application at Samoylov also shows differences in the importance of the several transport processes and in the methane dynamics under varying soil moisture, ice and temperature conditions during different seasons and on different microsites. These microsites are the elevated moist polygonal rim and the depressed wet polygonal centre. The evaluation shows sufficiently good agreement with field observations despite the fact that the module has not been specifically calibrated to these data. This methane module is designed such that the advanced land surface scheme is able to model recent and future methane fluxes from periglacial landscapes across scales. In addition, the methane contribution to carbon cycle-climate feedback mechanisms can be quantified when running coupled to an atmospheric model.

    • During different steps in the process of discovering drug candidates for diseases, it can be supportive to identify groups of molecules that share similar properties, i.e. common overall structural similarity. The existing methods for computing (dis)similarities between chemical structures rely on a priori domain knowledge. Here we investigate the clustering of compounds that are applied on embeddings generated from a recently published Mol2Vec technique which enables an entirely unsupervised vector representation of compounds. A research question we address in this work is: do existent well-known clustering algorithms such as k-means or hierarchical clustering methods yield meaningful clusters on the Mol2Vec embeddings? Further, we investigate how far subspace clustering can be utilized to compress the data by reducing the dimensionality of the compounds vector representation. Our first conducted experiments on a set of COVID-19 drug candidates reveal that well-established methods yield meaningful clusters. Preliminary results from subspace clusterings indicate that a compression of the vector representations seems viable.

    • Changes to climate-carbon cycle feedbacks may significantly affect the Earth system's response to greenhouse gas emissions. These feedbacks are usually analysed from numerical output of complex and arguably opaque Earth system models. Here, we construct a stylised global climate-carbon cycle model, test its output against comprehensive Earth system models, and investigate the strengths of its climate-carbon cycle feedbacks analytically. The analytical expressions we obtain aid understanding of carbon cycle feedbacks and the operation of the carbon cycle. Specific results include that different feedback formalisms measure fundamentally the same climate-carbon cycle processes; temperature dependence of the solubility pump, biological pump, and CO2 solubility all contribute approximately equally to the ocean climate-carbon feedback; and concentration-carbon feedbacks may be more sensitive to future climate change than climate-carbon feedbacks. Simple models such as that developed here also provide workbenches for simple but mechanistically based explorations of Earth system processes, such as interactions and feedbacks between the planetary boundaries, that are currently too uncertain to be included in comprehensive Earth system models.

    • Aerosol particles are essential constituents of the Earth's atmosphere, impacting the earth radiation balance directly by scattering and absorbing solar radiation, and indirectly by acting as cloud condensation nuclei. In contrast to most greenhouse gases, aerosol particles have short atmospheric residence times, resulting in a highly heterogeneous distribution in space and time. There is a clear need to document this variability at regional scale through observations involving, in particular, the in situ near-surface segment of the atmospheric observation system. This paper will provide the widest effort so far to document variability of climate-relevant in situ aerosol properties (namely wavelength dependent particle light scattering and absorption coefficients, particle number concentration and particle number size distribution) from all sites connected to the Global Atmosphere Watch network. High-quality data from almost 90 stations worldwide have been collected and controlled for quality and are reported for a reference year in 2017, providing a very extended and robust view of the variability of these variables worldwide. The range of variability observed worldwide for light scattering and absorption coefficients, single-scattering albedo, and particle number concentration are presented together with preliminary information on their long-term trends and comparison with model simulation for the different stations. The scope of the present paper is also to provide the necessary suite of information, including data provision procedures, quality control and analysis, data policy, and usage of the ground-based aerosol measurement network. It delivers to users of the World Data Centre on Aerosol, the required confidence in data products in the form of a fully characterized value chain, including uncertainty estimation and requirements for contributing to the global climate monitoring system.

    • The origin of two large peaks in the atmospheric radiocarbon ((14)C) concentration at AD 774/5 and 993/4 is still debated. There is consensus, however, that these features can only be explained by an increase in the atmospheric (14)C production rate due to an extraterrestrial event. Here we provide evidence that these peaks were most likely produced by extreme solar events, based on several new annually resolved (10)Be measurements from both Arctic and Antarctic ice cores. Using ice core (36)Cl data in pair with (10)Be, we further show that these solar events were characterized by a very hard energy spectrum with high fluxes of solar protons with energy above 100 MeV. These results imply that the larger of the two events (AD 774/5) was at least five times stronger than any instrumentally recorded solar event. Our findings highlight the importance of studying the possibility of severe solar energetic particle events.

    • As mobile tourist guides show huge potentials to support tourists’ information search, executives need to know which mobile systems characterized by which specific functionalities should be provided. The paper proposes an empirical approach combining qualitative and quantitative methods to deduce both, product definition and product marketing information related to mobile tourist guides, respectively. First, a methodology-mix combining qualitative (i.e. explorative survey, focus group interviews) and quantitative techniques (i.e. pilot study, expert interviews) responds to the question ‘Which services should be developed?’ Secondly, again in the pre-prototype phase and based on survey data (N = 705) ideal functionality compositions for promising tourist guides are deduced with regression analysis. Finally, based on technology acceptance theory (Venkatesh et al., 2003a) customer prejudices towards mobile tourist guides are identified. The obtained results support hypotheses indicating strong performance expectations and pronounced effort expectancies towards the deduced concepts designed to support information search by means of mobile tourist guides

    • The increasing number of drugs targeting specific proteins implicated in tumourigenesis and the commercial promotion of relatively affordable genome-wide analyses has led to an increasing expectation among patients with cancer that they can now receive effective personalised treatment based on the often complex genomic signature of their tumour. For such approaches to work in routine practice, the development of correspondingly complex biomarker assays through an appropriate and rigorous regulatory framework will be required. It is becoming increasingly evident that a re-engineering of clinical research is necessary so that regulatory considerations and procedures facilitate the efficient translation of these required biomarker assays from the discovery setting through to clinical application. This article discusses the practical requirements and challenges of developing such new precision medicine strategies, based on leveraging complex genomic profiles, as discussed at the Innovation and Biomarkers in Cancer Drug Development meeting (8th–9th September 2016, Brussels, Belgium).

    • BackgroundIntracerebral hemorrhage (ICH) accounted for 9% to 27% of all strokes worldwide in the last decade, with high early case fatality and poor functional outcome. In view of recent randomized controlled trials (RCTs) of the management of ICH, the European Stroke Organisation (ESO) has updated its evidence-based guidelines for the management of ICH. MethodA multidisciplinary writing committee of 24 researchers from 11 European countries identified 20 questions relating to ICH management and created recommendations based on the evidence in RCTs using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. ResultsWe found moderate- to high-quality evidence to support strong recommendations for managing patients with acute ICH on an acute stroke unit, avoiding hemostatic therapy for acute ICH not associated with antithrombotic drug use, avoiding graduated compression stockings, using intermittent pneumatic compression in immobile patients, and using blood pressure lowering for secondary prevention. We found moderate-quality evidence to support weak recommendations for intensive lowering of systolic blood pressure to <140mmHg within six-hours of ICH onset, early surgery for patients with a Glasgow Coma Scale score 9-12, and avoidance of corticosteroids. ConclusionThese guidelines inform the management of ICH based on evidence for the effects of treatments in RCTs. Outcome after ICH remains poor, prioritizing further RCTs of interventions to improve outcome.

    • Background: Acute pancreatitis (AP) is a frequent disorder with considerable morbidity and mortality. Obesity has previously been reported to influence disease severity. Objective: The aim of this study was to investigate the association of adipose and muscle parameters with the severity grade of AP. Methods: In total 454 patients were recruited. The first contrast-enhanced computed tomography of each patient was reviewed for adipose and muscle tissue parameters at L3 level. Associations with disease severity were analysed through logistic regression analysis. The predictive capacity of the parameters was investigated using receiver operating characteristic (ROC) curves. Results: No distinct variation was found between the AP severity groups in either adipose tissue parameters (visceral adipose tissue and subcutaneous adipose tissue) or visceral muscle ratio. However, muscle mass and mean muscle attenuation differed significantly with p-values of 0.037 and 0.003 respectively. In multivariate analysis, low muscle attenuation was associated with severe AP with an odds ratio of 4.09 (95% confidence intervals: 1.61–10.36, p-value 0.003). No body parameter presented sufficient predictive capability in ROC-curve analysis. Conclusions: Our results demonstrate that a low muscle attenuation level is associated with an increased risk of severe AP. Future prospective studies will help identify the underlying mechanisms and characterise the influence of body composition parameters on AP.

    • AimTo infer a forest carbon density map at 0.01 degrees resolution from a radar remote sensing product for the estimation of carbon stocks in Northern Hemisphere boreal and temperate forests. LocationThe study area extends from 30 degrees N to 80 degrees N, covering three forest biomes - temperate broadleaf and mixed forests (TBMF), temperate conifer forests (TCF) and boreal forests (BFT) - over three continents (North America, Europe and Asia). MethodsThis study is based on a recently available growing stock volume (GSV) product retrieved from synthetic aperture radar data. Forest biomass and spatially explicit uncertainty estimates were derived from the GSV using existing databases of wood density and allometric relationships between biomass compartments (stem, branches, roots, foliage). We tested the resultant map against inventory-based biomass data from Russia, Europe and the USA prior to making intercontinent and interbiome carbon stock comparisons. ResultsOur derived carbon density map agrees well with inventory data at regional scales (r(2)=0.70-0.90). While 40.715.7 petagram of carbon (PgC) are stored in BFT, TBMF and TCF contain 24.5 +/- 9.4PgC and 14.5 +/- 4.8 PgC, respectively. In terms of carbon density, we found 6.21 +/- 2.07kgC m(-2) retained in TCF and 5.80 +/- 2.21kgC m(-2) in TBMF, whereas BFT have a mean carbon density of 4.00 +/- 1.54kgC m(-2). Indications of a higher carbon density in Europe compared with the other continents across each of the three biomes could not be proved to be significant. Main conclusionsThe presented carbon density and corresponding uncertainty map give an insight into the spatial patterns of biomass and stand as a new benchmark to improve carbon cycle models and carbon monitoring systems. In total, we found 79.8 +/- 29.9PgC stored in northern boreal and temperate forests, with Asian BFT accounting for 22.1 +/- 8.3PgC.

    • Aim Information on the amount of carbon stored in the living tissue of tree stems (sapwood) is crucial for carbon and water cycle applications. Here, we aim to investigate sapwood-to-stem proportions and differences therein between tree genera and derive a sapwood biomass map. Location Northern Hemisphere boreal and temperate forests. Time period 2010. Major taxa studied Twenty-five common tree genera. Methods First, we develop a theoretical framework to estimate sapwood biomass for a given stem biomass by applying relationships between sapwood cross-sectional area (CSA) and stem CSA and between stem CSA and stem biomass. These measurements are extracted from a biomass and allometry database (BAAD), an extensive literature review and our own studies. The established allometric relationships are applied to a remote sensing-based stem biomass product in order to derive a spatially continuous sapwood biomass map. The application of new products on the distribution of stand density and tree genera facilitates the synergy of satellite and forest inventory data. Results Sapwood-to-stem CSA relationships can be modelled with moderate to very high modelling efficiency for different genera. The total estimated sapwood biomass equals 12.87 +/- 6.56 petagrams of carbon (PgC) in boreal (mean carbon density: 1.13 +/- 0.58 kgC m(-2)) and 15.80 +/- 9.10 PgC in temperate (2.03 +/- 1.17 kgC m(-2)) forests. Spatial patterns of sapwood-to-stem biomass proportions are crucially driven by the distribution of genera (spanning from 20-30% in Larix to > 70% in Pinus and Betula forests). Main conclusions The presented sapwood biomass map will be the basis for large-scale estimates of plant respiration and transpiration. The enormous spatial differences in sapwood biomass proportions reveal the need to consider the functionally more important sapwood instead of the entire stem biomass in global carbon and water cycle studies. Alterations in tree species distribution, induced by forest management or climate change, can strongly affect the available sapwood biomass even if stem biomass remains unchanged.

    • It is difficult to obtain fossil data from the 10% of Earth's terrestrial surface that is covered by thick glaciers and ice sheets, and hence, knowledge of the paleoenvironments of these regions has remained limited. We show that DNA and amino acids from buried organisms can be recovered from the basal sections of deep ice cores, enabling reconstructions of past flora and fauna. We show that high-altitude southern Greenland, currently lying below more than 2 kilometers of ice, was inhabited by a diverse array of conifer trees and insects within the past million years. The results provide direct evidence in support of a forested southern Greenland and suggest that many deep ice cores may contain genetic records of paleoenvironments in their basal sections.
