  • Manual therapy versus advice to stay active for nonspecific back and/or neck pain : A cost-effectiveness analysis

    • BACKGROUND: Low back and neck pain are the most common musculoskeletal disorders worldwide, and imply suffering and substantial societal costs, hence effective interventions are crucial. The aim of this study was to evaluate the cost-effectiveness of manual therapy compared with advice to stay active for working age persons with nonspecific back and/or neck pain.METHODS: The two interventions were: a maximum of 6 manual therapy sessions within 6 weeks, including spinal manipulation/mobilization, massage and stretching, performed by a naprapath (index group), respectively information from a physician on the importance to stay active and on how to cope with pain, according to evidence-based advice, at 2 occasions within 3 weeks (control group). A cost-effectiveness analysis with a societal perspective was performed alongside a randomized controlled trial including 409 persons followed for one year, in 2005. The outcomes were health-related Quality of Life (QoL) encoded from the SF-36 and pain intensity. Direct and indirect costs were calculated based on intervention and medication costs and sickness absence data. An incremental cost per health related QoL was calculated, and sensitivity analyses were performed.RESULTS: The difference in QoL gains was 0.007 (95% CI - 0.010 to 0.023) and the mean improvement in pain intensity was 0.6 (95% CI 0.068-1.065) in favor of manual therapy after one year. Concerning the QoL outcome, the differences in mean cost per person was estimated at - 437 EUR (95% CI - 1302 to 371) and for the pain outcome the difference was - 635 EUR (95% CI - 1587 to 246) in favor of manual therapy. The results indicate that manual therapy achieves better outcomes at lower costs compared with advice to stay active. The sensitivity analyses were consistent with the main results.CONCLUSIONS: Our results indicate that manual therapy for nonspecific back and/or neck pain is slightly less costly and more beneficial than advice to stay active for this sample of working age persons. Since manual therapy treatment is at least as cost-effective as evidence-based advice from a physician, it may be recommended for neck and low back pain. Further health economic studies that may confirm those findings are warranted. Trial registration Current Controlled Trials ISRCTN56954776.     • Two viridin-related B-norsteroids, B-norviridiol lactone (1) and B-norviridin enol (2), both possessing distinct unprecedented carbon skeletons, were isolated from a liquid culture of the ash dieback-causing fungus Hymenoscyphus pseudoalbidus. Compound 2 was found to degrade to a third B-norsteroidal compound, 1β-hydroxy-2α-hydro-asterogynin A (3), which was later detected in the original culture. The proposed structure of 1 is, regarding connectivity, identical to the original erroneous structure for TAEMC161, which was later reassigned as viridiol. Compound 2 showed an unprecedented 1H-13C HMBC correlation through an intramolecular hydrogen bond. The five-membered B-ring of compounds 1–3 was proposed to be formed by a benzilic acid rearrangement. The known compound asterogynin A was found to be formed from 3 by a β-elimination of water. All compounds were characterized by NMR spectroscopy, LC-HRMS and polarimetry.

    • Nowadays, it is already a truism to state that digital media are effective tools used by citizens, civil society organizations (CSOs) and social movementsto protest, to demand, to push and (sometimes) achieve social and political change. No question, they are. This has been observed and registered time and again in a luminous series of academic contributions (some notable examples include Castells 2012; Cottle 2011; De Luca. Lawson and Sun 2012; Howard and Muzammil 2011; Tremayne 2014; Tufekci 2017). Exactly how these media have been taken up by civic actors in specific contexts has been studied and discussed in fascinating detail (Cammaerts, 2018; Gerbaudo 2012; Treré 2019). This book goes one step further to ask a broader question: Has the use of digital media by civic actors improved (or depleted) the quality of democratic life understood as broad and effective citizen participation in public affairs and decision-making?

    • Introduction: Polymorphonuclear leukocytes (PMN) are main effector cells in the acute immune response. While the specific role of PMN in systemic lupus erythematosus (SLE) and autoimmunity is still unclear, their importance in chronic inflammation is gaining more attention. Here we investigate aspects of function, bone marrow release and activation of PMN in patients with SLE. Methods: The following PMN functions and subsets were evaluated using flow cytometry; (a) production of reactive oxygen species (ROS) after ex vivo stimulation with phorbol 12-myristate 13-acetate (PMA) or Escherichia coli (E. coli); (b) capacity to phagocytose antibody-coated necrotic cell material; (c) PMN recently released from bone marrow, defined as percentage of CD10(-)D16(low) in peripheral blood, and (d) PMN activation markers; CD11b, CD62L and C5aR. Results: SLE patients (n = 92) showed lower ROS production compared with healthy controls (n = 38) after activation ex vivo. The ROS production was not associated with corticosteroid dose or other immunotherapies. PMA induced ROS production was significantly reduced in patients with severe disease. In contrast, neither ROS levels after E. coli activation, nor the capacity to phagocytose were associated with disease severity. This suggests that decreased ROS production after PMA activation is a sign of changed PMN behaviour rather than generally impaired functions. The CD10(-)CD16(low) phenotype constitute 2% of PMN in peripheral blood of SLE patients compared with 6.4% in controls, indicating a decreased release of PMN from the bone marrow in SLE. A decreased expression of C5aR on PMN was observed in SLE patients, pointing towards in vivo activation. Conclusions: Our results indicate that PMN from SLE patients have altered function, are partly activated and are released abnormally from bone marrow. The association between low ROS formation in PMN and disease severity is consistent with findings in other autoimmune diseases and might be considered as a risk factor.

    • Jag har genom åren mött många flickor som av olika anledningar bestämt sig för att matematik inte är något för dem. Syftet med detta arbete är dels att genom litteraturstudier ta reda på om det är just flickor som tycker att matematik är svårt, tråkigt och meningslöst och i så fall ta reda på de bakomliggande orsakerna, dels att genom en egen undersökning ta reda på ”mina” flickors syn på sin situation, sina kunskaper och på den matematikundervisning de har fått. I olika skrifter som jag har tagit del av konstateras att - många flickor slutar läsa matematik när det inte är obligatoriskt. - flickor och pojkar behandlas olika i skolan, har olika förväntningar på sig och sina möjligheter, får olika mycket tid och har olika stort självförtroende. - flickor inte ser nyttan med att lära sig matematik. - flickor verkligen vill förstå vad de gör och lyckas därför bättre med ett mera processorienterat arbetssätt och med cooperative learning. I min undersökning framkommer att påfallande många tycker att de har tillräckliga kunskaper i matematik. De flesta tyckte att matematik var roligt i småskolan men att problem uppstod när matematiken blev abstraktare och de inte längre kunde se någon direkt nytta med de nya kunskaperna. Flickorna är nöjda med den undervisning de har fått men menar att de inte kommer att lyckas med fortsatta matematikstudier eftersom de inte har fått något ”mattesinne”. I min undersökning återkommer flera gånger påståendet att matematik är roligt när man kan. En bra matematikundervisning måste alltså möta varje elev på den nivå den befinner sig. Dessutom framkommer det tydligt att det är viktigt med ett varierat arbetssätt och att matematiken presenteras i sammanhang som kan kännas angelägna för alla elever utifrån deras erfarenheter och intressen.

  • Age and sex determination of mallards Anas platyrhynchos in autumn

    • The mallard is a well-known and important species in migration ecology, game management,and epidemiology. Males and females are generally easily told apart, whereas ageing isproblematic, due to individual timing of the moult cycle and lack of easily defined agecriteria. From examination and photographic documentation of mallards caught within a longtermringing program at Ottenby Bird Observatory (56°12′N, 16°24′E), we describe ninecharacters of plumage and bare parts to be used for ageing mallards in autumn. The reliabilityof these characters was tested by letting experienced bird ringers determine putative age ofbirds from photos. Age determination from any single character proved to be uncertain, as therate of correctly assigned mallard photos of each character was in the range of 51-85% formales and 48-89% for females. For both sexes, the lowest figure represented post-humeralsand the highest represented tertials. Rectrices, tertial coverts, and greater coverts also hadreasonably high scores (71-85%). With all characters at hand, 91% of the males and 95% ofthe females were correctly identified to age. As young mallards, with the progress of prebreedingmoult (completed from October onwards), acquire tail and tertials identical to adults,untypical individuals are better not assigned to an age category.

    • Remote fluorescence imaging of fungal growth on polymeric high-voltage insulators was performed using a mobile lidar system with a laser wavelength of 355 nm. Insulator areas contaminated by fungal growth could be distinguished from clean surfaces and readily be imaged. The experiments were supported by detailed spectral studies performed in laboratory using a fibre-optic fluorosensor incorporating an optical multi-channel analyser system (OMA) and a nitrogen laser emitting radiation at 33 7 nm.

    • A method combining laser-induced fluorescence and principal component analysis to detect and discriminate between algal and fungal growth on insulator materials has been studied. Eight fungal cultures and four insulator materials have been analyzed. Multivariate classifications were utilized to characterize the insulator material, and fungal growth could readily be distinguished from a clean surface. The results of the principal component analyses make it possible to distinguish between algae infected, fungi infected, and clean silicone rubber materials. The experiments were performed in the laboratory using a fiber-optic fluorosensor that consisted of a nitrogen laser and an optical multi-channel analyzer system.

    • Digital media transform news. First, we see this in changing use patterns. Young people today show a decline in interest in traditional news formats and practices, such as watching the evening news on TV or reading a daily newspaper. But digital media also transform production and distribution of news, leading to new ways to conceptualise and understand news. In the light of these profound transformations in audience behaviour many have started to question the concept of news in news research. In the light of such altered production and distribution contexts which are likely to fundamentally impact on audiences’ definitions and perceptions, this article sets out to explore alternative ways to understand and conceptualise news, beyond traditional news research. What is news today, and how can we study it from the perspective of news audiences, without resorting to preconceived notions? We propose a theoretical approach based in classic phenomenology which, we argue, will open up for further reconsideration of the concept as well as providing a potentially fruitful basis for research on digital news consumption. Phenomenology takes human existence as its vantage point and explores how human subjects exist and create meaning in their everyday lives in relation to basic categories such as time, space and (sociocultural) relevance. We argue phenomenology to be particularly relevant for exploring new meanings of news as the basic dimensions of phenomenology not only coincide with the basic dimensions of news consumption, but also of those of news values; time, space, and (sociocultural) relevance.

    • This article provides an analysis of the experiences of user–avatar relations and interaction of people who work in a virtual world. Earlier research often claims that relationships between users and their avatars are, by nature, strong and intense. By analyzing individuals who conducted paid labor in a number of public institutions in a virtual world, this article argues that the frame of work heavily influenced the professional users’ experiences of using an avatar. The user–avatar relationship was mainly related to how and why the user entered the virtual world, their position in their off-line and online workplaces and, as a result, related to aspects of power and control over the framing of the online arena. Because of these factors, many of the professional users regarded their avatar more as a second suit than, as has often been argued, a second self.

    • P>Chalara fraxinea (teleomorph: Hymenoscyphus albidus) is known as a serious pathogen of Fraxinus excelsior, causing massive dieback of trees in Europe. The fungus is able to cause latent infections, and has been previously detected as an endophyte in asymptomatic tissues. Chalara fraxinea is a slow grower in culture, and is thus likely to be overgrown by faster growing fungi whenever pure culture isolations are being attempted. This study reports species-specific ITS primers allowing fast and reliable detection of the pathogen directly from infected tissues of F. excelsior.

  • Dieback of Fraxinus excelsior : biology of ash dieback and genetic variation of the fungus Hymenoscyphus pseudoalbidus

    • Ash dieback caused by Hymenoscyphus pseudoalbidus (anamorph Chalara fraxinea) is a disease that has emerged during the past twenty years. It was first observed in Poland and has expanded over most of the distribution area of European ash (Fraxinus excelsior) in Europe. This thesis comprises four scientific papers. The first reports the production of a phytotoxin, viridiol, by the fungus, and shows that it causes necrotic spots on ash cotyledons. The second paper describes a working tool, a species-specific DNA primer situated in the internal transcribed spacer (ITS) region of the ribosomal DNA, which can be used to detect the fungus in diseased host material and confidently identify fungal cultures. In the third paper, microsatellites and arbitrary primed PCR were used to investigate the genetic population structure of the fungus. The genetic variation was evenly distributed in the whole European population, implying a high level of gene flow. Double alleles in apothecia compared with single alleles in mycelia cultures strongly indicated sexual reproduction. H. pseudoalbidus was clearly distinct from the native closely related Hymenoscyphus albidus and it is unlikely that the disease is derived from a H. albidus ancestry. The low number of microsatellite alleles per locus indicated a recent founder effect in H. pseudoalbidus. In the fourth paper disease development was surveyed in 261 naturally infected ash trees over a 32 month period. A clear seasonal pattern was demonstrated, with lesion activity and growth rate peaking during the summer. A substantial proportion of the lesions ceased to develop, often when the lesion reached a branch base; however the rate at which new lesions emerged was greater than the rate at which lesions entered a resting phase. During the course of the survey a third of the trees died and only a few seedlings remained healthy. In addition, the lower temperature limit for the fungus in culture was estimated to be 0.5°C.

    • This article aims to understand the distinctive mechanisms of digital media use, seen in relation to cultural practices at large. The empirical material is a survey study of university students at the Business Administration, Media and Communication Stud-ies, Political Science and Philosophy departments at Södertörn University, Sweden. The empirical analysis deals with the students’ digital media use and preferences, and how these are related to their broader cultural practices and preferences. Spe-cific attention is paid to the webpages the students mention in the survey, and how these are distributed among the groups. By showing detailed information on these areas, the mechanisms of difference of digital media use are revealed.

  • Ethics and morality beyond the Actor-Network : Doing the right thing in an algorithmic culture

    • The growing governance and authority of algorithms and the rapid emergence of the ‘Internet of Things’, has intensified attention to the relationship between humans and machines, and to the ethics of everyday life with technologies. This raises a lot of questions: How should we live with technologies? Can humans make decisions? The question on the authority of humans is clearly debatable. In this paper I discuss the underlying premises for the ethical standpoint of Actor Network Theory (ANT) and explore other existing alternatives. I begin by looking into the basic premises of ANT, and the assumptions of ‘the human’ and of ethics it comprises. I then explore the notion of a ‘disclosive’ ethics and continue by penetrating the relation between agency and ethics in ANT. Following from this I discuss various different approaches to ethics, and how we can still keep a human notion of ethics in an algorithmic culture.

    • The growing governance of algorithms and the rapid emergence of Artificial Intelligence (AI) and the ‘Internet of Things’, has intensified discussions about the relationship between humans and machines, and the ethics of everyday life with technologies. It raises questions such as: How should we live with technologies? Can humans still make decisions? The earlier taken for granted authority of humans is clearly debatable, and sometimes also dismissed, particularly within Actor Network Theory (ANT). In this paper, I look into the basic premises of the ethics of ANT, explore suggested ethical perspectives within ANT such as ‘disclosive’ ethics, and continue by penetrating the relation between agency and ethics, as well as between morality and ethics in ANT. ANT is further discussed as on the one hand a sociological perspective and on the other hand an ethical approach. Based on this analysis I propose the anthropological perspective of an ‘ethics of the ordinary’ as a possible way to learn from the basic premises of ANT while maintaining a human notion of ethics in a technology‐conflated culture.

    • This article argues for a revised importance of distance, besides the much emphasized closeness, in the debate on and use of ethnographic methods in online environments. When returning to the founding fathers of ethnographic methods, distance is often put forward as a core aspect of ethnographic methods, something widely forgotten, or even rejected, in the current debate in the field. Space has been restructured by digital media technologies, and the spatial and temporal proximity of digital  media cultures present new challenges for research methodologies. Based on the author’s own experiences of ethnographic fieldwork in digital cultures, and with Henri Lefebvre’s theory of everyday life as a rhythm as vantage point, it is here argued that  distance, dialectically interlinked with closeness and proximity, should be given further attention in current research and debate on ethnographic methods used online.

    • This article deals with the moral dimensions of everyday media use. It discusses the values, strategies and norms of the moral economy of media use in everyday life. First, it identifies three different kinds of values connected with media texts and technologies. Second, it discusses different strategies to create a morally correct balance in everyday life. Third, it puts forward the concept of imagined user modes to deepen our understanding of the moral dimensions of everyday media use. Imagined user modes are preconceptions of different technologies and texts and relate to our ideas of how the media affect those who are using them. They are considered when negotiating with ourselves on proper behaviours in specific situations, and therefore they can be different, depending on individual value systems and ideas of a morally correct behaviour.

    • One of the editors of this book was once a participant in a seminar with a famous French sociologist. The professor gave advice about projects and research ideas to the researchers and doctoral students that were participating. Then, someone asked the question of how best to understand one’s contemporaries and contemporary society. Perhaps the person asking the question had expected an answer about innovative research methods or about which social phenomena could say the most about the times we live in. But the sociologist had other advice. He said: stop following the noise of the news, turn off your feeds, and use the time you earn to re-read sociology’s classic texts. There was silence in the room. Would the way to understand the society of the 21st century go via texts written at the end of the 19th century, by people like Karl Marx, Émile Durkheim, Max Weber, and Georg Simmel?.

    • Janice Radway’s Reading the Romance (RTR) is a book that was published at the University of Pennsylvania, United States, in 1984 and soon after republished as a second edition (1987). RTR combines a text analysis with a reception study and an ethnographic study of mass-produced romance literature. The book revolves around a circle of female romance readers in a small American town and the bookseller who merchandises, recommends, and discusses the literature with the readers. The books themselves are also discussed as texts. Radway conducts narrative analyses and a psychoanalytic interpretation of the texts, uses questionnaires, and interviews readers, producers, and distributors. The book is considered a classic within Media Studies, Cultural Studies, Feminist Studies, and Comparative Literature.

    • The extended indispensability of media can be seen as a key indicator of mediatization, whereby our lives become adapted to media. But even though we can assume that the pervasive character of digital media essentially changes everyday life and the way people experience it, we cannot take consequences of technological transformation for granted. Following from this we argue there is a need for quantitative analyses that can verify mediatization processes in time and space. Here we present a tool that measures the extent to which media reach into the deeper layers of daily human life, used in an empirical study conducted in Sweden in 2017. The results show that perceived media indispensability is played out along three dimensions of daily human life; (1) (re)production; (2) recognition, and (3) civic life, and appears as a socially structured and structuring process, although not in a very strong way. We argue this tool, in diachronic analyses, works as a measurement of mediatization.

There has recently been a formulation of three tasks for mediatization research; historicity, specificity and measurability, needed to empirically verify mediatization processes across time and space. In this article, we present a tool designed to handle these tasks, by measuring the extent to which people experience that media reach into the deeper layers of daily human life. The tool was tested in an empirical study conducted in Sweden in 2017. The results show that perceived media reliance is played out in relation to three types of basic desires: (1) (re)productive desires, (2) recognition desires, and (3) civic desires, and is socially structured and structuring. We argue this tool, in diachronic analyses, can measure one important aspect of mediatization.

    • This article proposes and explores the notion of “media micro-generations”. Based on a survey of values and norms in relation to media-related behaviour in Sweden, we identify statistically significant media micro-generations. Through an analysis of the technologies that were introduced during the formative years of different media micro-generations, we propose that media micro-generations are formed with the introduction of new media technologies. Thus, the existence of media micro-generations illustrates how rapid trans- formations of media technologies can shape the moral notions of narrow age groups. It also explains why many earlier studies have detected a rather large span of years (1970-1985, in between the TV generation and the internet generation) during which no generational identity seems to have been formed.

    • The ascomycete fungus Hymenoscyphus pseudoalbidus (anamorph Chalara fraxinea) is responsible for ash dieback currently expanding over large parts of Europe. Our objective was to investigate the genetic structure of H. pseudoalbidus and to examine its relationship to the species H. albidus, known as a saprotroph. The study comprised 181 isolates of H. pseudoalbidus collected within the diseased area, 17 H. albidus isolates from six apothecia, collected outside the diseased area in Norway, and nine apothecia of H. pseudoalbidus collected in Sweden. By analysis of microsatellite markers developed for this study, combined with AP-PCR using the M13 primer, we demonstrated sexual heterothally in H. pseudoalbidus, detected high gene flow and low geographic structure of the H. pseudoalbidus population and found indications of a founder effect. Also, substantial genetic differences were detected between the two species of fungi; only four of seven microsatellite markers developed for H. pseudoalbidus were amplified for H. albidus, and no alleles were shared among the species. Furthermore, AP-PCR banding patterns were distinctly different for the two species. We conclude that even though the two fungi have a similar habitat and are morphologically virtually identical, they do not share a recent common ancestor.     • Ash dieback caused by the infectious fungus Hymenoscyphus pseudoalbidus currently threatens the common ash, Fraxinus excelsior, in Europe. An intriguing aspect is the morphological and ecological similarity between H. pseudoalbidus and the native saprotroph Hymenoscyphus albidus. We revisited four localities where H. albidus apothecia were collected from 1989 to 2005 and established the current relationship of the species in these Danish ash stands based on microsatellites and differences in ITS sequences (used as CAPs marker). Scottish collections from 2010 supported the hypothesis that Danish herbarium samples prior to 2005 are identical to H. albidus still found in Scotland. The markers further revealed that herbarium samples from 1989 to 1994 were all H. albidus, while the latest collection (2005) was H. pseudoalbidus, which coincides with the first Danish symptom observations. Collections from 2010 were purely H. pseudoalbidus. We suggest that expanding H. pseudoalbidus excludes H. albidus from its niche resulting in H. albidus becoming a rare species in Denmark, which is perhaps already locally extinct.   • Seasonal Pattern of Lesion Development in Diseased Fraxinus excelsior Infected by Hymenoscyphus pseudoalbidus

    • Ash dieback is a recent widespread disease on ash (Fraxinus sp.) that is causing important economic and ecological losses throughout Europe. The disease is initiated by the ascomycetous fungus Hymenoscyphus pseudoalbidus (anamorph Chalara fraxinea). The main aim of this study was to investigate seasonal pattern of lesion development associated with ash dieback. We present data on the spread of 324 natural lesions in ash shoots, branches and stems surveyed over a 32 month period. Most lesions were active and showed the greatest rate of growth during the summer; however, lesions were active throughout the year. Tree mortality was high, with more than a third of the surveyed trees dying during the study. Although many lesions permanently ceased to develop, the rate at which new lesions emerged was greater than the rate at which lesions entered a resting phase. The most common cause for a lesion going into a permanent state of rest was that it had encountered a branch-base. Genotype analysis showed that multiple infections can occur in a single tree given that different genotypes were identified in different lesions as well as in single lesions. A weak positive correlation was noted between tree health and tree size and a weak negative correlation was noted between tree overall health and lesion activity. The lower limit for H. pseudoalbidus growth in culture was between 4.0 degrees C and 0.5 degrees C.
