    • The ability to understand, interact and create a caring relationship with the patient is a core component in nursing. A shift in nursing education from traditional classroom teaching towards more experiential approaches should be encouraged as this will support learning that links theory with practice. The aim of this study was to describe nursing students' experiences of learning about nursing through drama. This qualitative study was conducted at a university in Sweden. Four focus group interviews were conducted with a total of 16 nursing students and the data was analyzed using a phenomenographic approach.Three themes with their attendant categories emerged through the analysis: "To explore the future professional self", "To develop an understanding of the patient perspective", and "To reflect on the nature of learning". In conclusion this study shows that the use of drama in nursing education can provide opportunities to explore interactions with others which can increase students' self-awareness and ability to reflect on their future professional identity. Acting in role as a patient can provide an opportunity to experience the patient perspective. Also clear was the importance of commitment and engagement ofthe students as a prerequisite for optimizing this form of learning experience through drama

  • The use and application of drama in nursing education : An integrative review of the literature

    • Background Caring is grounded on universal humanistic values such as kindness, empathy, concern and love for self and others. Nurses need to learn how to implement these values in the care for patients. Nursing students find it hard to assimilate theoretical knowledge in practice. Experiential learning in the form of drama has been used in several studies to enhance nursing students' learning. Aim The aim of this study was to review empirical and theoretical articles on the use and application of drama in nursing education. Design An integrative review of the literature.MethodThe databases CINAHL, PubMed, ProQuest and Academic search elite were searched for articles. Result The search generated 64 articles, with 20 articles meeting the inclusive criteria. Three themes with their attendant subthemes emerged through the analysis. The themes are: The Framing, The Objectives and The Embodiment. Conclusion Drama is effective in entry level courses in nursing education to enhance student learning and can be flexible and adjusted to various contexts. Further research using drama to enroll nursing students as fictive patients and relatives and thereby explore different perspectives is suggested, and more research on the application of drama in nursing education at an advanced level is required.

    • Background: In the health care settings in which nurses work, involvement in some form of conflict is inevitable. The ability to manage conflicts is therefore necessary for nursing students to learn during their education. Method: A qualitative analysis of 43 written group assignments was undertaken using a content analysis approach. Results: Three main categories emerged in the analysis-to approach and integrate with the theoretical content, to step back and get an overview, and to concretize and practicetogether with the overall theme, to learn by oscillating between closeness and distance. Conclusion: Learning about conflict and conflict management through drama enables nursing students to form new knowledge by oscillating between closeness and distance, to engage in both the fictional world and the real world at the same time. This helps students to form a personal understanding of theoretical concepts and a readiness about how to manage future conflicts.

    • This study is a part of a larger project in which the aim is to illuminate the meaning of the caring relationship between patients and nurses in daily nursing practice. Empirical studies in this area inspired from the interpretive phenomenological method are not commonly used. The aim of this paper is to describe how an interpretive phenomenological method was used to illuminate the meaning of the phenomenon caring relationship in daily nursing practice. Data were collected during 16 nursing care proceedings using participant observation with field notes, and in addition to that two interviews, one patient and one nurse. The interpretation moved back and forth between the whole and the parts in a dialectic process. Initial interpretive understanding of interviews and field notes, meaning units and comprehensive understanding were presented. Themes from the patient's interviews were competence, lack of continuity, strain and vulnerability. Themes from the nurse's interviews were competence and striving. Themes from the field notes were interactions towards a goal. The use of interpretive phenomenology offered an opportunity for learning to understand the meaning of the phenomenon caring relationship in daily nursing practice with both strengths and limitations. This study gave an understanding of the phenomenon through the illumination of the patient's and the nurse's thoughts, feelings and actions in the nursing care proceedings that led to a more profound knowledge about how they together create an encounter through their unique competence.

    • The aim of this study was to investigate how the caring relationship is formed in a medical context. The data were collected using participant observation with field notes and analysed by an interpretive phenomenological method. The context circumstances in a medical milieu demanded exacting efficiency and risks to oppress the caring relationship, subsequently causing demands in nursing practice. Three themes of the caring relationship were identified as respect for each other and for themselves, responsibility to reach out to each other and engagement. Patients' and nurses' awareness in encounters drove the forming of a caring relationship that went beyond the individual nurse and patient. This study implicates the importance of an understanding of how context circumstances create the foundation of the caring relationship

    • Studies regarding the caring relationship in out-patient clinics are few. The aim was to study the caring relationships appearance in encounters between patients with long-term illness and nurses in an out-patient clinic, and their experiences of this phenomenon. An interpretive phenomenological method was used; data were collected using participant observation and interviews, which were then interpreted. The findings illuminate the caring relationships appearance in encounters, as well as, patients and nurses experiences. Findings are understood as balancing between vulnerability and dignity, which reflects opportunities for realizing a holistic view in decreasing vulnerability and maintaining dignity.

  • Characterisation of Cancer Support and Rehabilitation Programmes: A Swedish Multiple Case Study

    • Cancer support and rehabilitation are suggested to be an integral part of cancer care strategies. This study focuses on comparativeness of cancer support and rehabilitation programmes. The aim of this study was to analyse available cancer support and rehabilitation programmes in Sweden presented as complementary to cancer rehabilitation at cancer clinics. A multiple case study design was chosen in order to inquire the small number of existing supportive and rehabilitative cancer programmes. Based on the structures, processes and outcomes of the nine included programmes, three types of cancer support and rehabilitation programmes were identified: multimodal rehabilitation, comprehensive cancer support and art therapy. Cancer support and rehabilitation programmes offer a variety of activities and therapies which are highly valuable and relevant for people with cancer. The typology of cancer support and rehabilitation programmes and comparability between programmes need further inquiry.

    • Sjuksköterskeutbildningen ska förbereda framtidens sjuksköterskor för ett yrkesliv som ställer höga krav på professionalitet och kompetens i mötet med patienter, närstående, kollegor och studenter. I dessa möten kan problem, etiska dilemman och konflikter uppstå. Kunskap om bemötande och kommunikation är därför centralt i utbildningen. Det krävs undervisningsformer där studenterna får möjlighet att öva, pröva och reflektera kring olika vårdsituationer i en trygg miljö tillsammans med lärare och andra studenter. Dramapedagogik är en studentaktiv undervisningsform som kan möjliggöra detta. Drama är ett praktiskt och ett teoretiskt ämne som möjliggör en helhetssyn på individen. Drama erbjuder kunskap om hur känsla, tanke och handling samspelar och påverkar varandra i en specifik kontext. En läraktivitet, ett moment med drama ingår i kursen Omvårdnad vid hälsa och ohälsa (30 hp), under termin 3 i sjuksköterskeprogrammet vid Institutionen för vårdvetenskap och hälsa, Sahlgrenska akademin, Göteborgs universitet. Momentet innefattar temat ”kommunikation inom omvårdnad”, med fokus på tolkning och reflektion utifrån vårdsituation, berättelse och begrepp. Momentet har utvecklats till ett program, som består av en heldags dramaworkshop fördelat på sex dramapass, där praktiska dramaövningar och reflektioner varvas för att stödja studenters lärande för att uppnå kursens lärandemål. Reflektioner sker utifrån en omvårdnadsteoretisk grund med avsikten att integrera teori och praktik. Fokusgruppsintervjuer har genomförts med studenter som medverkat i dramaprogrammet. Programmet presenteras i en rapport utgiven av Centrum för Kultur och Hälsa vid Göteborgs universitet.

  • Tensions during a process of change - implementation of the Bologna educational reform in Swedish specialist nursing programmes

    • Background/Objective: This study is an enquiry into the process of change during the implementation of the European educational reform, the Bologna process. The aim was to critically elaborate tensio ns during the process, focusing on the implementation of the degree project into Swedish specialist nursing programmes. Methods: Data were collected in a multi-centre participatory research project during the period August 2007 to January 2011, and consisted of memorandums kept by the project members and written vignettes from questionnaires answered by students. Eighteen faculty members from five universities, 69 male and 268 female students participated. The data were analysed according to a theoretical framework of thesis: the prevailing perspective, antithesis; a challenging perspective, and synthesis: an amalgamated perspective opening for constructive development. Main findings: The findings revealed conflicting views on research-based versus clinical development-based degree projects, and deductive versus inductive didactic development. The tensions consisted of lack of trust and confidence and were expressed as excluding and supercilious behaviour from the defenders of the traditional model. Conclusions: The implementation process, addressing conflicting views, resulted in a synthesized perspective between research and clinical development, paving the way for shared involvement between students, faculty members and clinicians in regard to the students’ degree projects.

    • Det övergripande syftet för avhandlingen har varit att klargöra den vårdande relationens innebörd i dagliga möten mellan patienter med långvarig sjukdom och deras sjuksköterskor. Avhandlingen baseras på deltagande observationer och intervjuer med patienter och sjuksköterskor på en medicinsk vårdavdelning och en medicinsk mottagning på ett mindre västsvenskt sjukhus. Avhandlingen innefattar fyra artiklar. En tolkande fenomenologisk metod har beskrivits och använts för att belysa innebörden av fenomenet vårdande relation i daglig omvårdnad i ett medicinskt sammanhang i vården. Trots krävande inslag och begränsningar visade sig metoden vara lämplig för att klargöra fenomenet. Forskarna arbetade med tolkning i en dialektisk process vilket gav en djupare förståelse för och kunskap om innebörden i den vårdande relationen. Den vårdande relationen framträdde som en balans mellan sårbarhet och värdighet. Denna förståelse kastade ljus på vårdens möjligheter att kunna förverkliga en helhetssyn. Därigenom minskas sårbarhet och värdighet upprätthålls. Det fordrades kravfylld effektivitet som riskerade att undertrycka den vårdande relationen. Patienters och sjuksköterskors medvetenhet drev formandet av denna relation som gick utöver den individuella patienten och sjuksköterskan. Förhållanden i vården skapade specifika förutsättningar för den vårdande relationen. Patienterna skapade relationer med all personal i vårdteamet, men dessa var inte tillräckliga för att ge trygghet. Patienters behov av en personlig vårdande relation framstod som en viktig förutsättning för att kunna möjliggöra trygghet. En tolkad helhetsförståelse av den vårdande relationen visade sig framstå som en dynamik mellan frustration, drivkraft och ömsesidigt omsorgsfullt närmande vilket innebar ett balanserande mellan sårbarhet och värdighet. Frustration uppstod när patienter och sjuksköterskor försökte få en vårdande relation till stånd. De visste att ett ömsesidigt omsorgsfullt närmande var väsentligt i situationen som låg till grund för deras drivkraft vilket skapade frustration i medveten strävan efter trygghet. Denna strävan skapade förutsättningar att förverkliga den helhetssyn som gjorde att vården upplevdes meningsfull vilket genererade kraft. Detta minskade patienters upplevelse av sårbarhet och gav möjlighet att upprätthålla deras värdighet. Detta tillförde tillfredsställelse hos sjuksköterskorna som därigenom fick kraft att stödja och stärka patienters hälsa. Denna avhandling visar genom resultaten och den tolkade helhetsförståelsen att den vårdande relationen mellan patienter med långvarig sjukdom och deras sjuksköterskor var ett ömsesidigt åtagande som skapade förutsättningar för en helhetssyn i vården. Denna behövdes för att minska patienters upplevelse av sårbarhet och upprätthållande av värdighet, vilket genererade kraft genom upplevelse av meningsfullhet. Avhandlingens resultat kan användas som grund för reflektion i unika möten mellan patienter, sjuksköterskor samt all hälso- och sjukvårdspersonal. Dessutom har avhandlingen tillfört kunskap som kan leda till nya forskningsfrågor om den vårdande relationen.     • The extension of advanced home-nursing has increased, and previous research shows that many persons prefer to die in their own homes. Relatives are significant in nursing, but it is emotional and demanding for the relative who needs support. The purpose of the study is to elucidate the relative’s experiences of a palliative home-nursing team. An interview-study with 13 relatives, to patients who have home-nursing until the end of their lives, supported with a palliative home-nursing team. A qualitative content analysis of content is used. The relatives were thankful for the element of security, but experienced exposedness, which appeared as an overall theme. Six categories identified: Trust in the nursing-team. To be invited to/ and get support in caring. To share the responsibility with the team. Not be in agreement with. To experience strain. To get support after the caring period. In spite of the fact that the relatives were very pleased; they experienced security and had trust to the nursing-team, it was found out that the teams resources was not always enough, which is confirmed in former research. Since patients as well as relatives are unique and therefore have different needs of palliative teams, is it important to elucidate this to facilitate security.

    • Background: Providing nursing care for patients and relatives requires a great effort from a nurse. It is therefore important for the nurse to have the opportunity to reflect on the caring acts included in process-oriented nursing supervision (PRONS). The overall objective of the nursing supervision is to support the professional development identity, competences, skills and ethics in different situations in nursing care. Aim: To investigate nurses’ experiences of the model of purging, playing and learning (PPL) according to Eriksson theory in a (PRONS) related to strengthen safe care, quality and professional development. Method: A qualitative study with individual interviews and the data was analyzed using qualitative interpretive content analysis. Participants: All eleven registered participated nurses were from southwestern Sweden and worked in different hospital units. Ethical Considerations: The study carried out in accordance with the ethical guidelines laid down in the Helsinki Dec-laration and according to the recommendations of the regional ethics com-mittee. Findings: Three categories were identified, valuable purging in the process, responsive playing and awareness of learning. The study found that by participating in PRONS the nurses had developed new approaches with different “tools” when difficult situations occurred in daily nursing practice. Conclusion: PRONS with the model PPL has an important role to support nurses in daily nursing practice. The study highlights that there are residual successful effects after PRONS for the nurses in managing care situations that experienced strengthen quality in care and professional development.

    • Background: Recent decades have witnessed the development of qualitative methods as well as intense debate concerning the significance of methodological rigour. Although qualitative research has become an established part of health care, questions have been raised concerning how results can inform practice. Qualitative Outcome Analysis (QOA) is a method derived from qualitative research that allows interventions to be evaluated and that bridges the theory-practice gap on the path from knowledge generation to knowledge implementation. Aim: The aim is to describe Qualitative Outcome Analysis (QOA) by means of examples from an ongoing project about information processing in palliative cancer care and on the basis of our results from phenomenological and conversational analysis studies of person centred information in palliative care for patients with recurrent gastroenterological cancer. The participatory dimensions identified in the project will be related to QOA procedures. Methods: Focus groups with the palliative care team at an oncology outpatient unit were held, and major results from previous studies in the project were used as a framework or guide in order to facilitate discussion and reflection on the team’s professional experiences. In this way patient-reported qualitative findings were merged with professionals’ experience. Group discussion data were analyzed concurrently. A preliminary clinical intervention model was developed and refined by means of small scale clinical tests conducted by the team as well as other focus groups made up of different palliative care teams. Results: The applicability of QOA will be presented. A particular dimension identified in the process is participatory aspects related to the shared commitment between the researchers´ goal with the model and the team’s goal of facilitating clinical development. Such participatory aspects will be elaborated upon and related to the Participatory Action Research tradition. Discussion: The significance of relating QOA to Participatory Action Research will be reflected upon.

  • Academic learning for specialist nurses: a Grounded Theory study

    • The aim was to explore the major concerns of specialist nurses pertaining to academic learning during their education and initial professional career. Specialist nursing education changed in tandem with the European educational reform in 2007. At the same time, greater demands were made on the healthcare services to provide evidence-based and safe patient-care. These changes have influenced specialist nursing programmes and consequently the profession. Grounded Theory guided the study. Data were collected by means of a questionnaire with open-ended questions distributed at the end of specialist nursing programmes in 2009 and 2010. Five universities were included. Further, individual, pair and group interviews were used to collect data from 12 specialist nurses, 5-14 months after graduation. A major concern for specialist nurses was that academic learning should be "meaningful" for their professional future. The specialist nurses' "meaningful academic learning process" was characterised by an ambivalence of partly believing in and partly being hesitant about the significance of academic learning and partly receiving but also lacking support. Specialist nurses were influenced by factors in two areas: curriculum and healthcare context. They felt that the outcome of contribution to professional confidence was critical in making academic learning meaningful.

  • Tensions related to implementation of postgraduate degree projects in specialist nursing education

    • In conjunction with the introduction of the Bologna process in Sweden, specialist nursing education programmes were moved up to the second cycle of higher education with the opportunity to take a one-year master's degree, which also meant that students would undertake a degree project carrying 15 ECTS. The purpose of this study was to examine the introduction of postgraduate degree projects on the second-cycle level into Swedish specialist nursing programmes in accordance with the Bologna process. Five universities were involved and the study design took the form of action research. Problem formulation, planning, evaluation and follow-up with reflection led to new actions over a period of 2 1/2 years. Through a review of local curriculum documents, the implementation of a postgraduate degree project was monitored and these reviews, together with field notes, were analysed by means of constant comparative analysis. The results revealed a variety of tensions that arose when postgraduate degree projects were introduced, taking the form of differing views on the relationship between research, clinical development, specific professional objectives and academic objectives. These tensions were reflected in six areas of change. In summary, it can be noted that implementation of the postgraduate degree projects highlighted tensions related to basic views of learning. © 2010 Elsevier Ltd.

    • Conflicts are common in healthcare settings and affect the nursing care negatively causing suffering among the patients and other costs, such as burnout and higher absenteeism among the nursing staff. The ability to manage conflict is therefore a necessary for nursing students to learn during their education. But previous studies show that nursing students lack in preparation to face and handle conflicts. The aim of this study is to present a teaching structure with the purpose of supporting nursing students learning about conflict and conflict management. The teaching structure combines a theoretical lecture, a drama workshop and a group assignment where the students reflect over their learning. In this study, 43 group assignments are being analyzed using qualitative content analysis. The preliminary analysis consists of three main categories with their attendant sub categories (1) To approach and integrate with the theoretical content, (2) To create a distance and get an overview, and (3) To concretize, practice and apply. The final result as well as the conclusions and implications will be presented at the ICN conference in Barcelona, May 2017.

    • The overall findings of this study for each nurse was that they have new tool to combine their emotions with symbols, which could be one way of own empowerment to prevent burn out. One way in which to raise the quality of nursing in daily practice is to develop and improve the nurses´ reflections on nursing situations. This may be aided by a map that can indicate a possible route to achieving an overview of circumstances and of various choices for improvement, a route to developing one’s own competence and to finding moral courage in the exercise of professional care.

Undervisningsmomentet bygger på styrdokument, forskningsresultat som berör dramaövningar, Forumspel och teorier om konflikthantering, från det internationella DRACON projekt (DRAma and CONflict resolution). DRACON har utvecklats och genomförts i Australien, Malaysia och Sverige. Kursen ges i specialistsjuksköterskeprogrammet, en gång per år, vid Göteborgs universitet, Sahlgrenska Akademin, Institutionen för vårdvetenskap och hälsa. Rapporten belyser undervisningsmomentet konflikthantering i kursen OM4380 Ledarskap och lärande, 15 hp. Kursen ges under sista året av totalt tre år i sjuksköterskeprogrammet vid Göteborgs universitet, Sahlgrenska Akademin, Institutionen för vårdvetenskap och hälsa. Undervisningsmomentet konflikthantering med drama har genomförts sedan 2008.     • Introduction: The European higher education reform known as the “Bologna Process” implies further integration and harmonization of nursing education programs into the higher education system. The consequences for the specialist nurse programs are the following: all courses within the programs are on master’s level, called second cycle, and the programs include a degree project which corresponds to a master’s thesis. A collaborative project was initiated in the beginning of this implementation in order to jointly facilitate the development of structures for students’ degree projects. The purpose of this presentation is to report a tentative didactic model for degree projects in collaboration between universities and clinical practice. Methods: Framed by action research, an inductive multi-case study design was chosen. Five collaborating universities constitute one case respectively. A combination of methods has been used: questionnaires to students, focus groups with faculty members, descriptive analysis of curricula and field notes. Results: A tentative model for degree projects within the specialist nurse programs will be presented: didactic intentions, integration of evidence-based experienced-based and improvement knowledge, balancing generic skills with professional skills, and balancing aims and prerequisites from the university and clinical practice. Four collaborative foci are emphasized: identification of problems for research and development (R&D) in dialogues with clinical practice, development of R&D projects driven by clinical practitioners, thesis supervision by clinical practitioners and researchers, feedback of degree projects’ results to clinical practice. Discussion: The model could serve as a resource for enhancing the collaboration between educational and clinical settings, and thereby, constitute a significant facilitator for students to perform projects as part of clinical R&D projects. This is particularly important in the implementation of the Bologna process in the specialist nurse programs. Since we lack tradition to include theses in clinical R&D projects, we have a tendency to foremost connect students’ projects mostly to university driven research.     • The aim of this study was to gain an understanding of the concept of receiving care, from the perspective of the general public, nursing students and graduate nurses. The need for, and acceptance of care can occur at many different times during a human life and is sometimes necessary for survival. Experiences of receiving care are often retained as a tacit and subconscious awareness and knowledge. It is reasonable to expect that perceptions of receiving care can differ depending on different situation and experiences. Data were gathered by a questionnaire containing an open-ended question, which were presented to and answered by the general public, nursing students and graduate nurses. Content analysis was used to analyse the question posed. Having analysed the data, one main theme could be identified; being of value despite any potential disadvantages which include both edifying and not very edifying aspects. This study demonstrates that if one is in need of receiving care, there are not many choices available. Those requiring care must accept the situation and be prepared to leave themselves in the hands of caregivers. Receiving care highlights the human mode of being, which includes experiences of being exposed, which in turn motivates a seeking for valued and appreciated mutual interactions within the caring process. Within current nursing practice, our findings confirm the necessity of nurses to be fully aware of the importance of mutuality when giving care to patients. It is essential that modern nursing education continuously review the many complex aspects involved in the giving of, and receiving care.

    • The aim was to illuminate the meaning of participating in support and rehabilitation programs described by people diagnosed with cancer. Nineteen persons were interviewed in focus groups and face-to-face. Data were analyzed with a qualitative phenomenological hermeneutical method for researching lived experiences. Interpretation proceeded through three phases: naïve reading, structural analysis, and comprehensive understanding. Three themes were disclosed: receiving support for recovery when being most vulnerable, recapturing capabilities through supportive activities, and searching to find stability and well-being in a changed life situation. Participating in the programs was an existential transition from living in an unpredictable situation that was turned into something meaningful. Recovery did not mean the return to a state of normality; rather, it meant a continuing recovery from cancer treatments and symptoms involving recapturing capabilities and searching for a balance in a forever changed life. This study provides new insights about the experiences of participating in cancer support and rehabilitation programs.

    • Most people who experience an acute stroke survive, but a substantial number die as a result of it. Stroke is, according to the World Health Organization, the second leading cause of death globally. Research in the area of stroke has previously been less focused on situations when patients die due to an acute stroke in favor of research on medical treatments, rehabilitation, and living with the aftermath of stroke. Research focusing on ethical problems in the care of dying patients can be found both in general and in specific care contexts such as palliative, intensive, and non-hospital care, but knowledge concerning ethical problems in the area of stroke, or arising as a result of acute stroke, was lacking. Both these subjects, death due to acute stroke and ethical problems, were combined for the first author's (Å. R.) thesis. Individual interviews with stroke team members were performed guided by their answers in a three-part form filled out before the interviews. This case study provides an account of how qualitative and quantitative content analysis was used in combination to deepen the results from the study utilizing data obtained from both the three-part forms and the interviews performed. The case highlights some challenges faced when combining the two different types of data. It also gives examples of how the analysis was performed when combining qualitative and quantitative content analysis to support and further explore data.

The design of the study is a secondary analysis of qualitative data as proposed by Heaton, from a project on unexpected sudden death by stroke. Material from four interview studies from the project with all in all twelve next of kin of patients who had died due to the acute stroke during hospital stay and twenty-seven stroke team members from stroke units were utilized. Data were analysed with qualitative content analysis according to Krippendorf and Baxter. The analysis reveals communication as the foundation for care and caring. The overarching theme foundation for dignified encounters in care is built up by six themes illuminating the meaning of communication in unexpected sudden death by stroke; creating relationship, information enabling understanding, giving ground for unanimity, personalizing the patient, preconditioning for decisions and giving and experiencing support. Communication and its meaning in unexpected sudden death as experienced by next of kin and stroke team members enables dignified encounters in care. Through the next of kin knowledge about the severely ill patient, possibility to preserve and uphold absolute dignity and dignity of identity is given, expressed through respect for the patient’s will and desire and derived through talks between carers and next of kin. Communication is one key to create dignified encounters in palliative care when unexpected sudden death occurs. It gives ground for unanimity about care and possibility to maintain and uphold dignity in care when acknowledging the next of kin familiarity with the patient. This gives ground for a person centeredness in palliative care despite the patients sever illness.

    • When people die suddenly and unexpectedly ethical issues often come to the fore. The aim of the study was to describe experiences of members of stroke teams in stroke units of ethical problems and how the teams manage the situation when caring for patients faced with sudden and unexpected death from stroke. Data were collected through four focus group interviews with 19 team members in stroke-unit teams, and analysed using interpretive content analysis. Three themes emerged from the analysis characterized by information, decisions about care and support for the next-of-kin in the changing and uncertain situation, with mutual trust as the core. Mutual trust, both within the stroke team and with next-of-kin was seen by the stroke-team members as a way of handling ethical problems by administering care with the patient's best interest at heart.

Syftet med studien var att belysa kommunikationens betydelse vid bråd död med stroke som exempel, så som det upplevs av närstående och stroketeamets medlemmar. Data från fyra tidigare studier med tolv närstående till patienter som dött en bråd död till följd av stroke och sammanlagt tjugosju stroketeammedlemmar användes för sekundäranalys och analyserades med kvalitativ innehållsanalys. Kommunikation stiger fram som vårdens och vårdandets fundament vid bråd död. Det övergripande temat ”fundamentet för värdiga möten i vården” byggs upp av sex teman som belyser kommunikationens betydelse vid bråd död vid stroke; att skapa en relation, information möjliggör förståelse, ge grund för enighet, gör patienten till person, förutsättning för beslut och ge och känna stöd.

Little research is performed on ways or strategies for handling those problems and even less on obstacles to and possibilities of using such strategies. A previous study illuminated stroke team members' experiences of ethical problems and how the teams managed the situation when caring for patients faced with sudden and unexpected death from stroke. These findings have been further explored in this study. Objective: The aim of the study was to illuminate obstacles and possibilities perceived by stroke team members in using strategies for handling ethical problems when caring for patients afflicted by sudden and unexpected death caused by stroke. Research design: A qualitative method with combined deductive and inductive content analysis was utilized. Participants and research context: Data were collected through individual interviews with 15 stroke team members working in stroke units of two associated county hospitals in western Sweden. Ethical considerations: The study was approved by the Regional Ethics Review Board, Gothenburg, Sweden. Permission was also obtained from the director of each stroke unit. Findings: All the studied strategies for handling of ethical problems were found to have both obstacles and possibilities. Uncertainty is shown as a major obstacle and unanimity as a possibility in the use of the strategies. The findings also illuminate the value of the concept the patient's best interests as a starting point for the carers' ethical reasoning. Conclusion: The concept the patient's best interests used as a starting point for ethical reasoning among the carers is not explicitly defined yet, which might make this value difficult to use both as a universal concept and as an argument for decisions. Carers therefore need to strengthen their argumentation and reflect on and use ethically grounded arguments and defined ethical values like dignity in their clinical work and decisions.

We performed a follow-up study on stroke team members' experiences of ethical problems and how the teams managed the situation when caring for patients faced with sudden and unexpected death from stroke. A number of ways for handling ethical problems emerged, which we have now explored further. Data were collected through a three-part form used as base for individual interviews with 15 stroke team members and analyzed using both quantitative and qualitative content analysis. In the analysis, the approaches in the form were condensed into strategies, and the two different ways those strategies were preferred and used by the team members were shown. Hindrances perceived by the team members to impede them from working the preferred way were also revealed and grouped into eight categories.

    • The narrative method is used in healthcare research, mostly in data collection but also in the analysis. Narrative approaches draw attention to how people tell about and create meaning of experiences. The aim of the article was to examine the narrative structure, the elements in the structure and their function and how these can be used in research to gain insights into experiences. Examples are taken from a material of narratives from a study where next of kin were asked to narrate their experiences of sudden and unexpected death from stroke. The narratives had a clear beginning, midpoint and ending. In the beginning, orientation of the narrated events was given. The narrated events were told to have a turning point constituted of complicating actions that lead to a resolution that solved the narrated event. The narratives were built up by multiple recaps into the narrated events and also consisted of asides - side narratives and flashbacks - events back in time. Use of a narrative structure can contribute with valuable information that might be missed with other analysis. The analysis can be used on its own, as a complement to other narrative analysis or even as a complement to other qualitative analysis. © 2013 Nordic College of Caring Science.

Background: Conflicts are common in healthcare settings and affect patient care and the wellbeing of nurses. To be able to manage complex practice situations, conflict management is an essential competence for nurses and should be highlighted during education. The aim of the study was to describe and illuminate conflict situations nursing students encounter during their clinical practice, narrated and performed through Forum Play. Design: The study was designed as a summative qualitative analysis of written group assignments related to nursing students' participation in a drama workshop. Method: A summative qualitative content analysis of written group assignments related to nursing students' participation in a drama workshop, focusing on conflict management. During the workshop the students explored conflict situations they had encountered during clinical practice, through Forum Play. After the workshop, the students handed in a mandatory written group assignment where they described one of the conflict situations. Results: The findings are presented in three categories; Parties; Arenas; Situations, one main theme; "Who knows best" and two subthemes; Difficulties to adapt to the new and Difficulties reaching a mutual understanding. Conclusion: Conflict situations that nursing students encounter during their clinical practice often stem from the health care staff's difficulties in adapting to the new and difficulty reaching a mutual understanding. Conflict management can be implemented as a powerful learning strategy in nursing education.

    • The aim of this article is to discuss challenges in the development of Specialist Nursing Educations as a result of the 2007 Swedish Higher EducationReform: the implementation of the so-called Bologna process. Certain challenges follow this reform, particularly since the specialist nursingprogrammes will be part of the second cycle of the higher education system, and it will be possible to combine the professional degree witha masters degree (one year). Possible strategies in four areas related to the Specialist Nursing Education are discussed: integration of researchbasedknowledge, experienced-based knowledge, improvement knowledge, and strategies for collaboration between university institutions andclinics. Specific didactical issues are raised.

    • Background: Narrative analyses have substantially developed interdisciplinary in several fields during the last decades. In this review we focus on how narratives have been used in the field of palliative care research. Aim: To review how narratives have been used in palliative care research. Methods: The databases PubMed, CINAHL and PsychInfo were used. Search terms were palliative care, terminal care and hospice care in combination with narratives, narrations or storytelling. Theoretical studies and discussion papers were excluded. Results: The majority of the literature that deals with narratives in palliative care research has been published within the last five years. The analysis showed two overlapping fields of application of narratives. The first and most common, comprised studies utilizing narratives as a delimited research tool for data collection, using different forms of narrative interviews without combination of narrative analysis. The second field of application was utilizing narrative designs and analyses which either was performed for pure methodological purposes or combined with theoretical purposes. In the latter case implicitly or explicitly assuming narratives to be an inherent character of practice making narrative inquires significant in order to improve such as communication, personal skills and ethics in palliative care. Discussion: A reception of narratives is clearly recognized in the in the field of palliative care research. However the most common use of narratives was means for data collection. Utilization of narrative designs and analyses are more limited in palliative care research.

    • In an academic programme, completion of a postgraduate degree project could be a significant means of promoting student learning in evidence- and experience-based practice. In specialist nursing education, which through the European Bologna process would be raised to the master’s level, there is no tradition of including a postgraduate degree project. The aim was to develop a didactic model for specialist nursing students’ postgraduate degree projects within the second cycle of higher education (master’s level) and with a specific focus on nurturing shared involvement between universities and healthcare settings. This study embodies a participatory action research and theory-generating design founded on empirically practical try-outs. The 3-year project included five Swedish universities and related healthcare settings. A series of activities was performed and a number of data sources secured. Constant comparative analysis was applied. A didactic model is proposed for postgraduate degree projects in specialist nursing education aimed at nurturing shared involvement between universities and healthcare settings. The focus of the model is student learning in order to prepare the students for participation as specialist nurses in clinical knowledge development. The model is developed for the specialist nursing education, but it is general and could be applicable to various education programmes. © 2011 Springer Science+Business Media B.V.
