Effect of 12 mg vs 6 mg of Dexamethasone on the Number of Days Alive Without Life Support in Adults With COVID-19 and Severe Hypoxemia The COVID STEROID 2 Randomized Trial

- Question What is the effect of 12 mg vs 6 mg of dexamethasone on the number of days alive without life support at 28 days in patients with COVID-19 and severe hypoxemia? Findings In this randomized trial that included 1000 patients with COVID-19 and severe hypoxemia, treatment with 12 mg/d of dexamethasone resulted in 22.0 days alive without life support at 28 days compared with 20.5 days in those receiving 6 mg/d of dexamethasone. This difference was not statistically significant. Meaning Compared with 6 mg of dexamethasone, 12 mg of dexamethasone did not statistically significantly reduce the number of days alive without life support at 28 days. This multicenter randomized clinical trial compares the effects of 12 mg/d vs 6 mg/d of dexamethasone in patients with COVID-19 and severe hypoxemia. IMPORTANCE A daily dose with 6 mg of dexamethasone is recommended for up to 10 days in patients with severe and critical COVID-19, but a higher dose may benefit those with more severe disease. OBJECTIVE To assess the effects of 12 mg/d vs 6 mg/d of dexamethasone in patients with COVID-19 and severe hypoxemia. DESIGN, SETTING, AND PARTICIPANTS A multicenter, randomized clinical trial was conducted between August 2020 and May 2021 at 26 hospitals in Europe and India and included 1000 adults with confirmed COVID-19 requiring at least 10 L/min of oxygen or mechanical ventilation. End of 90-day follow-up was on August 19, 2021. INTERVENTIONS Patients were randomized 1:1 to 12 mg/d of intravenous dexamethasone (n = 503) or 6 mg/d of intravenous dexamethasone (n = 497) for up to 10 days. MAIN OUTCOMES AND MEASURES The primary outcome was the number of days alive without life support (invasive mechanical ventilation, circulatory support, or kidney replacement therapy) at 28 days and was adjusted for stratification variables. Of the 8 prespecified secondary outcomes, 5 are included in this analysis (the number of days alive without life support at 90 days, the number of days alive out of the hospital at 90 days, mortality at 28 days and at 90 days, and >= 1 serious adverse reactions at 28 days). RESULTS Of the 1000 randomized patients, 982 were included (median age, 65 [IQR, 55-73] years; 305 [31%] women) and primary outcome data were available for 971 (491 in the 12 mg of dexamethasone group and 480 in the 6 mg of dexamethasone group). The median number of days alive without life support was 22.0 days (IQR, 6.0-28.0 days) in the 12 mg of dexamethasone group and 20.5 days (IQR, 4.0-28.0 days) in the 6 mg of dexamethasone group (adjusted mean difference, 1.3 days [95% CI, 0-2.6 days]; P = .07). Mortality at 28 days was 27.1% in the 12 mg of dexamethasone group vs 32.3% in the 6 mg of dexamethasone group (adjusted relative risk, 0.86 [99% CI, 0.68-1.08]). Mortality at 90 days was 32.0% in the 12 mg of dexamethasone group vs 37.7% in the 6 mg of dexamethasone group (adjusted relative risk, 0.87 [99% CI, 0.70-1.07]). Serious adverse reactions, including septic shock and invasive fungal infections, occurred in 11.3% in the 12 mg of dexamethasone group vs 13.4% in the 6 mg of dexamethasone group (adjusted relative risk, 0.83 [99% CI, 0.54-1.29]). CONCLUSIONS AND RELEVANCE Among patients with COVID-19 and severe hypoxemia, 12 mg/d of dexamethasone compared with 6 mg/d of dexamethasone did not result in statistically significantly more days alive without life support at 28 days. However, the trial may have been underpowered to identify a significant difference.

- While creativity is often understood as the generation of valuable novelty, we extend that view into a framework based on well-known and established models from the creativity literature. In addition to generation of novelty which forms our frameworks first dimension, we add selection of novelty, and type of creative contribution. Based on interviews with inventors and managers in three large, patent-intensive firms, representing completely different industries, we show how the framework can be employed in order to better understand in what ways different practices exploit and/or support the inventors’ creative abilities. By relating the practices not to creativity in general but to components of creativity (c.f Amabile 1997) the paper enriches the discussion of how inventors’ can be leveraged in technology-based firms.

- Our world is filling up with electronics. High-tech gadgets are integrated everywhere, from smart fridges able to track expiry dates and food usage, to microchip implants that let us unlock doors and pay with our hands. As innovative as they are, these new products and the many more to come impose new requirements on materials and fabrication methods. For instance, emerging electronic technologies that deliver light emission such as smart labels, authenticity features and light-based medical therapies, are often required to be flexible, see-through and low-cost, and in addition sustainable to fabricate, operate and recycle.In response to these challenges, the light-emitting industry is turning to organic electronics for solutions, a field that promises resource-efficient fabrication using environmentally benign materials. An interesting proposal is that of the light-emitting electrochemical cell (LEC), which, thanks to its simple structure, is well suitedfor high-throughput fabrication. The LEC is in many aspects a smart device, able to reorganize itself during operation via the electrochemical action of mobile ions, which create the injection and transport layers that require additional fabrication steps in other technologies. This elegant behavior makes the LEC tolerant to a large array of materials and fabrication methods, and hence a good fit for many applications.Yet the LEC is still today a scientific curiosity rather than an actual commercial solution and among the very few prototypes available on the market, none are able to meet the combined performance, resource efficiency and sustainability criteria. As a matter of fact, of the three layers that make an LEC, i.e., two electrodes surrounding an active material, only the later meet these requirements thanks to a strong recent research effort. In comparison, the electrodes have received little attention and are almost exclusively comprising metals or metal oxides deposited by time- and energy-expensive fabrication methods, making the LEC as a whole unfit for many applications.In an effort to push the LEC toward the untapped commercial niche of low-cost lighting, we tackle the problem of electrode fabrication with resource-efficiency in mind. We first show that up-scalable spray coating of inks under ambient air is a viable mean of fabrication for both active materials and electrodes alike. However, in doing so, we find that we create electrode interfaces that are open to ion transfer; an up-to-now overlooked issue that needs careful consideration when designing solution-processed LECs. Building on our discovery, we demonstrate that it is possible to fabricate an LEC entirely by using spray coating metal-free and organic inks; thereby demonstrating that an all-organic, metal-free and resource-efficient LEC is possible.I hope that our efforts will encourage others to work on solution-processed LECs, electrodes included, and develop ready-to-use products.

- In order to reap competitive advantage from innovation, a firms technology activities should square with its technology strategy - but how do technology strategies relate to activities and financial performance in relevant business areas? This paper investigates this question by means of a comparison between two leading firms in the electrical engineering industry: ABB and General Electric. We show that substantial performance differences between these companies in the power generation field are related to differences in their espoused technology strategies (as indicated by statements in annual reports) and technology activities (as indicated by patenting) and the degree of alignment between these.

- Since the 1960s, governments have sought to encourage technological development to reduce pollution. These efforts now include global greenhouse emissions, especially in sectors such as transport and energy generation. A variety of means are applied: general taxes and trading systems, subsidies and technology-forcing standards. At the macro-level, economists argue that general economic instruments are a more efficient way to regulate emissions than administrative or technology-specific measures. The effectiveness of general economic instruments needs to be examined in relation to their innovation impact in different (sub-) sectors, however. This paper builds on research in the automotive and energy sectors to compare general and specific, economic and administrative, means in terms of their impact on different types of innovation. The review shows that the effectiveness of policy instruments is conditioned by the type of innovation needed (incremental, modular, architectural or radical) and the responding industrial context. General instruments – economic and administrative – encourage development and diffusion of incremental and modular innovation, whereas technology-specific instruments are needed to support the development and diffusion of architectural and radical low-carbon innovations. However, in order to have an effect, instruments have to be connected to a responding industrial context, i.e. networks of firms with requisite resources and capabilities to deploy. Key challenges for policy makers when choosing instruments include issues of selection, stringency, scale and stability.

- This article focuses on technological discontinuities and late shakeouts in mature industries. The empirical case is combined cycle gas turbine technology in the power generation industry, where two of four main incumbents (GE, ABB, Siemens, and Westinghouse) exited the industry after several years of competition. We show that the vast differences in firm performance are strongly related to variation in technological capabilities, such as sourcing and integration of knowledge from related industries and after-launch problem solving. The findings from this case may also be of general interest for studies of dynamics in other mature, complex industries.

- The creative destruction of existing industries as a consequence of discontinuous technological change is a central theme in the literature on industrial innovation and technological development. Established competence-based and market-based explanations of this phenomenon argue that incumbents are seriously challenged only by ‘competence-destroying’ or ‘disruptive’ innovations, which make their existing knowledge base or business models obsolete and leave them vulnerable to attacks from new entrants. This paper challenges these arguments. With detailed empirical analyses of the automotive and gas turbine industries, we demonstrate that these explanations overestimate the ability of new entrants to destroy and disrupt established industries and underestimate the capacity of incumbents to perceive the potential of new technologies and integrate them with existing capabilities. Moreover, we show how intense competition in the wake of technological discontinuities, driven entirely by incumbents, may instead result in late industry shakeouts. We develop and extend the notion of ‘creative accumulation’ as a way of conceptualizing the innovating capacity of the incumbents that appear to master such turbulence. Specifically, we argue that creative accumulation requires firms to handle a triple challenge of simultaneously (a) fine-tuning and evolving existing technologies at a rapid pace, (b) acquiring and developing new technologies and resources and (c) integrating novel and existing knowledge into superior products and solutions.

- This paper addresses the issue of R&D internationalisation of two multinationals in the electro-technical industry (GE and ABB), by means of a patent data analysis. The overwhelming majority of both companies R&D activities are concentrated in Western Europé and North America. The locational overlap between the two firms' activities is small. These results are consistent with findings from earlier studies that (1) there is little evidence to suggest that the 'production' of technology is globalised in a general sense and (2)that tapping knowledge from an industry's global lead location plays a very limited role in foreign R&D investments.

- Various types of policy instruments have been implemented to reduce local and global emissions, but the impact on innovation of different instruments has received less attention. This paper reviews empirical studies of the innovation impact of four main types of policy instruments in two high-emitting sectors. The conclusions are threefold. (1) Policy plays a key role for the development and diffusion of environmental innovation in the studied sectors. (2) Different types of instruments promote different types of innovations: general economic instruments has primarily encouraged incremental innovation, general regulatory instruments has enforced improvements based on modular innovation, and technology-specific instruments appears to have been needed to support the development and deployment of radically new technologies. (3) All types of policy instruments face challenges in design and implementation: understanding the selection impact of the chosen instruments, implementing increasing stringency levels, committing to an appropriate scale, and safeguarding policy stability.

- While the critics of nuclear power previously focused on safety risks, they now point to how modern nuclear power plant constructions are plagued by cost increases and excruciating delays, most recently in Finland's new Olkiluoto 3-plant which was finally commissioned in early 2023. These problems are avoidable, however. In the 1970s and 1980s, Sweden and Finland managed to build 14 new reactors without any costly delays. Stepwise, sequential expansion, continuous improvements, and knowledge transfer based on long-term partnerships were a crucial part of this forgotten achievement, After 14 years of delays and repeated budget overruns, Finland has commissioned its new large nuclear reactor Olkiluoto 3. The many problems that plagued its construction since its start in 2005 have also been highlighted in international media (1). But why was Olkiluoto 3 so late and expensive-and what can other projects and countries learn from this experience? From one per year to 18 years for one For nuclear power critics, the delays and budget overruns are a direct consequence of the complexity of these clear power plants and thus a reason to continue opposing new nuclear power. However, when comparing Olkiluoto 3 with Sweden's nuclear power expansion in 1972-1985 and the earlier Finnish projects Olkiluoto 1 and Olkiluoto 2, a quite different picture emerges.

- During the 1980s and 1990s Finland and Sweden were on the international frontier in telecommunications, pioneering the first-generation cellular system and leading in the development of the second generation. This strength in telecommunications has developed under various regulatory regimes in a complex industrial history, going back to the nineteenth century. To account for this Fenno-Swedish telecom trajectory, the article starts out with Porter's model of industrial competitiveness and theories of public procurement, and then focus the attention and analysis in two directions: (i) the historical role of advanced, research-intensive users and competitive public-private development pairs, and (ii) the emergence and significance of a composite binational clusters and their local agglomerations in the accelerated industry growth in the 1990s.
