    • AIMS AND OBJECTIVES: This study aimed at investigating life satisfaction and its relation to living conditions, overall health, self-care capacity, feeling lonely, physical activities and financial resources among people (65+) with reduced self-care capacity. BACKGROUND: Knowledge about factors related to low life satisfaction among older people with reduced self-care capacity is sparse, although this is important in health care and nursing so that the care is adapted to their needs and perspective. Previous research has mainly focused on isolated aspects such as pain in relation to life satisfaction among older people in general and less among so those with reduced self-care capacity in general. DESIGN AND METHOD: A subsample of 522 persons was selected from a randomly selected cross-sectional survey using a modified form of the Older Americans' Resources Schedule and Life Satisfaction Index Z. RESULTS: The mean age in the total sample was 77.9; women (79.5) were significantly older than men (77.0). Low life satisfaction was found among women, as well as those living in special accommodations. Life Satisfaction Index Z was 15.3 (SD 5.6) in the total sample. Gender and living conditions did not explain life satisfaction whilst poor overall self-reported health and poor financial resources in relation to needs had the strongest explanatory value. Also of significant importance were loneliness, the degree of reduced self-care capacity and feeling worried. CONCLUSION: Life satisfaction in older people with reduced self-care capacity is determined by several factors, with social, physical, mental and financial aspects probably interacting with each other; especially feeling lonely, degree of self-care capacity, poor overall health, feeling worried and poor financial resources in relation to needs. These factors need to be considered in the care of these people to preserve or improve their life satisfaction. RELEVANCE TO CLINICAL PRACTICE: Nursing interventions in terms of preventive home visits, rehabilitation, health education directed towards physical, psychological, social and economic aspects of importance may help to preserve or improve life satisfaction for those with reduced self-care capacity.

  • Assessment of search strategies in literature-review-based candidate theses within a nursing program

    • Background: The majority of candidate theses in baccalaureate nursing programs in Sweden are written as literature studies. Being able to carry out a systematic and structured literature search is an essential part of thesis-related work. Aim: The aim of the current study was to investigate changes in nursing students’ search strategies in candidate theses. Methods: A retrospective, quantitative study design was obtained. Librarians (n = 2) and teachers (n = 4) randomly examined selected candidate theses (every third thesis, n = 89) from the years 2012, 2014, and 2016. Results: The result showed a significant improvement over the years (from 2012 and 2014 to 2016) regarding the use of a sufficient number of synonyms, matching search terms to the respective database, use of the Boolean operator OR, and the use of subject headings and free text searches. Use of the title/abstract search largely disappeared. There was a significant change in the types of searches being done. The searches have become more structured in later years as the use of block searches increased significantly; in other words, more systematic and relevant searches have been done in recent years. Conclusion: The result of this study shows that the quality of the students’ search strategies improved significantly during the studied years. It is recommended that search documents are used in both formative and summative assessments to evaluate students’ search strategies. Educational development in the form of enhanced collaboration between librarians and teachers in nursing programs is recommended because it might help to develop student search strategies in literature-based candidate theses.

    • Collaboration between different professions in community care for older people is often both difficult and complex. In this project, a participatory action research (PAR) was conducted in order to support the professions involved in the care for older people to develop individualized health and social care plans. Cases from daily work were discussed in different professional groups over a period of one year. A key finding was that lack of knowledge regarding the other professions' field of expertise and their underlying professional culture and values was a barrier in their collaboration. However, as the continuous reflective dialogue process progressed, the participants began to reflect more about the importance of collaboration as a prerequisite to achieve the best possible care for the recipient. This process of reflection led to the often complex needs of the care recipients being given a more central position and thus care plans being better tailored to each person's needs.5

    • Aim.  This article is a report on a descriptive study of fathers’ experiences of providing their preterm infants with Kangaroo Mother Care.Background.  During neonatal intensive care, fathers describe the incubator as a barrier and the separation from their infant as stressful. Fathers consider it important to be close to the infant, and performing Kangaroo Mother Care makes them feel an important participant in their infants’ care.Method.  Individual interviews conducted in 2009 with seven fathers who performed Kangaroo Mother Care were analysed using qualitative content analysis.Results.  The fathers’ opportunity for being close to their infants facilitated attainment of their paternal role in the neonatal intensive care unit. Kangaroo Mother Care allowed them to feel in control and that they were doing something good for their infant, although the infant’s care could be demanding and stressful. As active agents in their infant’s care, some fathers stayed with the infant during the whole hospital stay, others were at the neonatal intensive care unit all day long. Despite the un-wished-for situation, they adapted to their predicament and spent as much time as possible with their infants.Conclusion.  Fathers’ opportunities for Kangaroo Mother Care helped them to attain their paternal role and to cope with the unexpected situation. The physical environment and conflicting staff statements influenced their opportunity for, and experience of, caring for their preterm infants.

Det finns studier som tyder på att kvinnor verkar må psykiskt sämre och har sämre reglerad diabetes än män. För att kunna stödja kvinnorna i egenvården behövs kunskap om hur de upplever det att leva med diabetes. Syfte: Undersöka kvinnors upplevese av att leva med typ 2 diabetes. Metod: Litteraturstudie som bygger på nio kvalitativa artiklar. Resultat: Fyra teman hittades; besvikelse och svek, kluvenhet, skuld och skam samt oro för framtiden. Diskussion: Det är svårt att vara kvinna och leva med diabetes typ 2. Sjukdomen kräver daglig egenvård och kombinerat med andra roller som kvinnan har i sitt dagliga liv kan kraven upplevas som stressande för en del av kvinnorna. Många kvinnor med diabetes typ två upplevde att de var i behov av bättre stöd och information för att kunna leva med sin sjukdom.     • Aim: Early parental bonding with preterm babies is particularly important, and the aim of our study was to explore when parents experienced what they regarded as important events for the first time while their infant was in the neonatal intensive care unit (NICU).Methods: The study was part of a longitudinal project on Kangaroo Mother Care at two Swedish university hospitals. The parents of 81 infants completed questionnaires during their infants' hospital stay.Results: Most parents saw and touched their infants immediately after birth, but only a few could hold them skin to skin or swaddle them. Other important events identified by parents included the first time they performed care giving activities and did so independently, interaction and closeness with the infant, signs of the infant's recovery and integration into the family. The timing of the events depended on the physical design of the NICU, whether parents' could stay with their infant round-the-clock and when they were allowed to provide care under supervision and on their own.Conclusion: The design and routines of the NICU dictated when parents first interacted with their infants. Clinical guidelines that facilitate early contact with preterm babies can help parents to make the transition to their parental role.

    • Encountering older people - a person-centered communication training model Introduction: Being recognised as a person is a universal human need and central to the transition to a person-centred elderly care. A prerequisite for acknowledging the person, is to pay attention to the persons emotional and existential concerns. One way to encounter such concerns is through person-centred communication. However, such communication has been described as challenging for nurses caring for older people. To prepare nursing students with this skill, a training model in communication has been introduced into the nursing programmes at Kristianstad University. The model is flexible with the learning activities: attend a filmed lecture; carry out a recorded conversation training in pairs; review and reflect on the recorded conversation; attend filmed general feedback on the conversations and own reflections; reflect on the feedback. The aim of this study was to explore nursing students' experiences of participating in the training. Method: Written reflection tasks (n=301) and semi-structured interviews with students (n=19) were conducted and content analysed. Result: The overall experience was that the skill training was important, challenging, rewarding, and learning. Listening to own recorded conversation made it possible to reflect on own strengths and shortcomings in communicating about emotional and existential concerns. Feedback on the recorded conversations and reflections was instructive. Conclusion: The skill training model is considered as meaningful and important for future work as a nurse in person-centred elderly care. Learning outcomes: Conference participants will gain insight into how to use a model that, despite a large number of students, provides possibility to repeatedly train conversations about emotional and existential issues in person-centred elderly care.

    • The aim of this study was to demonstrate how people with incurable cancer experienced soft massage in a palliative care setting in which massage was used as an established and integrated part of the nursing care. To reach a deep understanding of the experiences of receiving soft massage a qualitative method with a phenomenological approach was chosen. The study was based on interviews with eight patients in an advanced home care setting who had all received soft massage as part of their daily care. Soft massage was experienced by the informants as a way to find inner peace. During the massage the patients felt dignified, while memories from past massage sessions were about becoming free. These experiences of dignity and freedom brought hopes for the future. The conclusion is that soft massage ought to be offered in the ordinary palliative care. More research is needed to understand what is needed to integrate and establish methods such as soft massage in the palliative care.

    • Utveckling av en färdighetsträningsmodell i samtal på personcentrerad grund Bakgrund: I en personcentrerad hälso- och sjukvård är det centralt att personal aktivt lyssnar in och samtalar för att försöka förstå personens tankar, känslor och önskemål och därefter för en dialog om vad som skulle kunna främja personens hälsa. Samtidigt ger hälso- och sjukvårdspersonal uttryck för att de tycker att det är svårt att samtala personcentrerat. Särskilt gällande känslomässiga och existentiella frågor. Litteraturstudier visar att simuleringsträningar under utbildning kan ge ökad kunskap och färdigheter, men att det finns behov av tydlighet om syfte och mål med träningen och en teoretisk förankring. Därtill behövs kvalitativa utvärderingsstudier. Syftet var därför att utveckla en färdighetsträningsmodell i samtal på personcentrerad grund i kurser på grund och avancerad nivå i relation till lärandemålen. Metod: En form av ’experienced based co-design’ har legat till grund för utvecklingen av färdighetsträningen. Den teoretiska grunden för projektet hämtades från Bubers relationsfilosofi, Orlandos interaktionsteori och samtal på fenomenologisk grund samt resultat från ett pedagogiskt projekt. Studenters och lärares erfarenheter vid pilottestningen har därefter tagits tillvara för att vidareutveckla strukturen för färdighetsträningen. Resultat: En modell för färdighetsträningen har arbetats fram med en gemensam struktur som består av fyra delar: 1. filmad föreläsning, 2. färdighetsträning i form av simulerade samtal som ljudinspelas, 3. återkoppling på samtalen, 4. reflektion över färdighetsträning och återkoppling. Färdighetsträningen följs alltså upp med läraråterkoppling och där studenter reflekterar över genomfört samtal och erhållen återkoppling. Slutsats: Färdighetsträningsmodellen är implementerad i kurser och redo att utvärderas avseende genomförbarhet och utfall.

Distriktsnämnden för Nordöstra Skånes sjukvårdsdistrikt har beslutat att äldrevårdsteam ska tillskapas för att kunna erbjuda hembesök till äldre vid akuta/halvakuta sjukvårdsbehov. Syftet med detta arbete var att beskriva processen hur projektet äldrevårdsteam enligt "Hässleholmsmodellen" initierades, planerades och startades upp samt som en del i utvärderingen av projektet undersöka vad olika berörda intressenter ansåg var nyttan med äldrevårdsteam utifrån sitt perspektiv. Metoden som användes var intervjuer med yrkesverksamma och intervjuerna har analyserats med kvalitativ innehållsanalys. I intervjuerna framkom att äldrevårdsteamet kan vara en resurs och ett stöd vid bedömning samt utgöra en möjlighet för förbättrad samverkan och delaktighet kring patienten. Genomgående bedömde informanterna att det innebär en stor nytta för den äldre personen att kunna få vård i hemmet och slippa åka iväg till sjukhuset eller vårdcentralen.   • Joint Handwritten Text Recognition and Word Classification for Tabular Information Extraction

    • In this paper, we present a system for extracting tabular information from loosely structured handwritten documents. The system consists of three parts, (i) a u-net like CNN-based method for text detection and segmentation, (ii) a new attention-based method for simultaneous text recognition and classification of word-parts, and (iii) a method for matching the word parts into a tabular structure for each entry. A key contribution is the observation that the new attention-based recognition and classification module makes it possible for improved spatial analysis of the tabular information. The method is evaluated on a unique historical document: The Swedish Wealth Tax of 1571, consisting of 11,453 pages of hand-written tax records. The evaluation shows that the system provides a significant improvement to the state-of-the-art to the problem of tabular extraction from loosely structured historical documents.

    • We describe a deep learning method to read hand-written records from the 16th century. The method consists of a combination of a segmentation module and a Handwritten Text Recognition (HTR) module. The transformer-based HTR module exploits both language and image features in reading, classifying and extracting the position of each word on the page. The method is demonstrated on a unique historical document: The Swedish Wealth Tax of 1571. Results suggest that the segmentation module performs significantly better than the lay-out analysis implemented in state-of-the art programs, enabling us to trace many more text blocks correctly on each page. The HTR module has a low character error rate (CER), in addition to being able to classify words and help organize them into tabular formats. By demonstrating an automated process to transform loosely structured handwritten information from the 16th century into organized tables, our method should interest economic historians seeking to digitize and organize quantitative material from pre-industrial periods.

    • This thesis explores everyday living with persistent pain from the perspectives of functionally dependent older persons in pain and of staff. A combination of qualitative and quantitative methods was chosen. Data were collected using interviews and tools for assessing pain. The first data collection included 66 persons (65+) living in sheltered accommodation and their corresponding contact nurses. Seventy-five per cent of those who managed to perform a self-assessment of pain were in pain frequently. The staff assessed frequent pain in 57% of persons with impaired cognition. Agreement between assessment by the staff and the older persons was no higher than moderate, in general pain characteristics were underestimated and those with impaired speech were at risk of not having their pain identified. Pain recognition was interpreted as an interactive communicative process that could be facilitated or hindered by characteristics of both the older persons and the staff. Among cognitively impaired older persons, pain was mainly identified by verbal, para-linguistic, facial, and bodily expressions and immobility was viewed as the most common cause of pain. Another data collection included staff (n=52) and persons (n=94, 75+) with persistent pain living in special accommodation or receiving home help in their ordinary homes. One group of older persons felt competent and were proud of their ability to manage life. Others felt confident and serene despite a high level of dependence due to pain. Resignation and feeling sad or misunderstood and inadequately treated characterised other groups. How the pain restricted their daily life was often considered more problematic than the pain itself. The pain was mainly handled by medication, distraction and a balance between rest and mobility. In the 50% of the cases, the older persons had taken prescribed medication although they perceived the effect as not helpful or only somewhat helpful. Non-pharmacological pain-specific methods were seldom employed. A tendency for staff to perceive activities to alleviate pain as more helpful than older persons was marked for ‘medication’, and ‘talking about pain.’ The staff perceived older persons with pain as a heterogeneous group. When the pain was perceived as ‘real’, ‘endured’ and ‘inarticulate’, caring was satisfying, while caring for persons with ‘exaggerated’, ‘care-inflicted’, ‘concealed’ and ‘self-caused’ pain was demanding or frustrating. How the staff perceived the older persons’ pain seemed to influence how the pain was handled. Findings demonstrate that recognising and alleviating older person’s pain is complicated and requires that the older persons’ perspectives are found out and considered.

Interviews with 90 older people receiving home care from nursing auxiliaries in their own homes or in sheltered accommodation were collected from January to June 2000. A typology of older people in persistent pain was developed. Activities for handling pain were examined using content analysis. Findings. Respondents' experiences of themselves and their pain varied. Two groups of older people, considered as 'competent and proud' and 'confident and serene', expressed satisfaction in spite of pain, while the groups 'misunderstood and disappointed' and 'resigned and sad' expressed dissatisfaction. The most common strategies used were medication, rest, mobility, distracting activities and talking about pain. Respondents chose strategies by balancing the advantages of the activities against the disadvantages these brought for their daily living. Conclusion. This study indicates that characteristics of the older people, such as their way of experiencing themselves, how pain affects their daily life and how they perceive effects and side-effects of pain management are areas that need to be identified when staff assess pain and plan pain management. Caring for older people in pain could be improved by listening to and believing their complaints, evaluating effects and side-effects from medications and nonpharmacological pain management and by emphasising the importance of common everyday activities such as mobility and distraction to relieve pain.

    • Pain is a common problem for older persons who need professional help for their daily living. In this study 94 older persons (75+) in persistent pain were compared with 52 care providers concerning the pain management methods they had used/administered during the previous week and how helpful they perceived these methods to be. Interviews were based on 16 items from the original version of the pain management inventory (PMI). Both groups perceived prescribed medication, rest and distraction as the most frequently utilized methods. Specific methods such as transcutaneous electrical nerve stimulation (TENS) or bracing/splinting or bandaging the affected body part were seldom employed, although most users perceived these methods as helpful. Care providers perceived most methods for managing pain as more effective than older persons did. The results imply that care providers need skills in a variety of pharmacological and nonpharmacological methods to manage pain and a need to evaluate effectiveness of the methods in a systematic way.
