  • Signs of dysregulated fibrinolysis precede the development of type 2 diabetes mellitus in a population-based study

    • Background: Diabetic patients experience stimulated coagulation and dysfibrinolysis, which is associated with an increased risk of cardiovascular events. This imbalance may precede the manifest diagnosis. We investigated whether elevated antigen levels of tissue plasminogen activator (tPA), plasminogen activator inhibitor-1 (PAI-1), the tPA/PAI-1 complex, or von Willebrand Factor (VWF) precede type 2 diabetes mellitus (T2DM) diagnosis, and whether this elevation occurs before increased fasting plasma glucose (FPG) or 2-hour plasma glucose (2hPG) in individuals who later develop T2DM.Methods: We conducted a prospective incident case-referent study within the Vasterbotten Intervention Programme. Cardiovascular risk factor data as well as FPG and 2hPG and blood samples for future research were collected at a baseline health examination between 1989 and 2000, (n= 28 736). During follow-up in January 2001, 157 cases had developed T2DM. Referents without T2DM were matched for sex, age, and year of participation (n=277). Subgroup analysis was performed for cases with normal baseline glucose levels (FPG <6.1 mmol/L and 2hPG < 8.9 mmol/L) and cases with elevated levels (FPG 6.1-6.9 mmol/L and/or 2hPG 8.9-12.1 mmol/L).Results: After adjusting for BMI, family history of diabetes, physical activity, smoking, systolic blood pressure and levels of C-reactive protein and triglycerides, independent associations were found between incident T2DM and elevated levels of tPA (OR=1.54, 95% CI 1.06-2.23), PAI-1 (OR=1.61, 95% CI 1.14-2.28), and tPA/PAI-1 complex (OR=2.45, 95% CI 1.56-3.84). In participants with normal glucose levels, PAI-1 (OR=2.06, 95% CI 1.10 - 3.86) exhibited an independent relationship with incident T2DM after the adjustments.Conclusions: Elevated levels of fibrinolytic variables precede the manifestation of T2DM after adjusting for metabolic and cardiovascular risk factors and can be detected several years before changes in glucose tolerance.

    • C-reactive protein (CRP) is a marker of metabolic and cardiovascular disease. To study the effects of lifestyle on CRP in a high-risk population we conducted a randomized controlled trial on 200 obese subjects (BMI > 27 kg m(-2)) with impaired glucose tolerance recruited from primary care settings. They were randomized to either a 1-month stay at a wellness centre focusing on diet, exercise and stress management (intervention group) or 30-60 min of oral and written information on lifestyle intervention (control group). A significant reduction of CRP was observed after 1 month and 1 year in the intervention group. They reduced their CRP levels more than the control group 1 year after intervention (p=0.004). In conclusion lifestyle intervention can decrease CRP in obese individuals with impaired glucose tolerance for up to 1 year. Further research is needed to evaluate whether the CRP level reduction translates into a decreased risk for cardiovascular morbidity.

    • OBJECTIVES: Physical activity has beneficial effects on cardiovascular disease but the mechanisms are still somewhat unclear. One possible pathway may be through the anti-inflammatory effects attributed to regular physical activity. Our primary aim was to study the effects of endurance physical exercise on C-reactive protein (CRP), Interleukin-6 (IL-6), Tumor Necrosis Factor-alpha (TNFalpha) during the acute and recovery phases. Secondarily, we studied the impact of diet on these inflammatory markers.METHODS: Twenty men, aged 18-55 years, participated in a 14 days cross-country skiing tour. They traveled 12-30km per day corresponding to about 10h of heavy physical activity. The participants were randomized to a diet with either 30 or 40% of energy derived from fat. Inflammatory variables were analysed at week 0, after 1 and 2 weeks and during the recovery phase at week 6 and 8.RESULTS: CRP and TNFalpha increased significantly during the two weeks of exercise (1.4-5.0mg/l, p=0.00 and 6.8-8.4pg/ml, p=0.00). CRP levels were significantly lower during recovery (median 0.7mg/l) compared to baseline (median 1.4mg/l) and did not correlate to metabolic variables. There were no significant changes in IL-6 levels during the study period. For dietary groups significant CRP changes were observed only in the high fat group during recovery.CONCLUSIONS: CRP and TNFalpha increased significantly but reacted differently during heavy physical activity while there seemed to be no significant changes in IL-6. No significant differences regarding inflammatory variables were found between the dietary groups.

    • Despite major advances in the treatment and prevention of atherosclerosis the last several decades, cardiovascular disease still accounts for the majority of deaths in Sweden. With the population getting older, more obese and with rising numbers of diabetics, the cardiovascular disease burden may increase further in the future. The focus in cardiovascular disease has shifted with time from calcification and narrowing of arteries to the biological processes within the atherosclerotic plaque. C-reactive protein (CRP) has emerged as one of many proteins that reflect a low grade systemic inflammation and is suitable for analysis as it is more stable and easily measured than most other inflammatory markers. Several large prospective studies have shown that CRP is not only an inflammatory marker, but even a predictive marker for cardiovascular disease. C-reactive protein is associated with several other risk factors for cardiovascular disease including obesity and the metabolic syndrome. Our study of twenty healthy men during a two week endurance cross country skiing tour demonstrated a decline in already low baseline CRP levels immediately after the tour and six weeks later. In a study of 200 obese individuals with impaired glucose tolerance randomised to a counselling session at their health care centre or a one month stay at a wellness centre, we found decreased levels of CRP in subjects admitted to the wellness centre. The effect remained at one, but not after three years of follow-up. In a prospective, nested, case-referent study with 308 ischemic strokes, 61 intracerebral haemorrhages and 735 matched referents, CRP was associated with ischemic stroke in both uni- and multivariate analyses. No association was found with intracerebral haemorrhages. When classifying ischemic stroke according to TOAST criteria, CRP was associated with small vessel disease. The CRP 1444 (CC/CT vs. TT) polymorphism was associated with plasma levels of CRP, but neither with ischemic stroke nor with intracerebral haemorrhage. A study on 129 patients with atrial fibrillation was used to evaluate whether inflammation sensitive fibrinolytic variables adjusted for CRP could predict recurrence of atrial fibrillation after electrical cardioversion. In multivariate iv models, lower PAI-1 mass was associated with sinus rhythm even after adjusting for CRP and markers of the metabolic syndrome. In conclusion, lifestyle intervention can be used to reduce CRP levels, but it remains a challenge to maintain this effect. CRP is a marker of ischemic stroke, but there are no significant associations between the CRP1444 polymorphism and any stroke subtype, suggesting that the CRP relationship with ischemic stroke is not causal. The fibrinolytic variable, PAI-1, is associated with the risk of recurrence of atrial fibrillation after electrical cardioversion after adjustment for CRP. Our findings suggest a pathophysiological link between atrial fibrillation and PAI-1, but the relation to inflammation remains unclear.

    • This paper describes tracer gas measurements at the National Museum of Fine Arts in Stockholm. The building was erected 1850-1860 and the architect was Friedrich August Stüler. He created several museums of Fine Arts, among them Neues Museum in Berlin. The museum building in Stockholm has a volume of almost 90 000 m3 and the number of registered objects of art is well over 600 000. The collection includes one of the world’s most prestigious collections of drawings and graphical prints. In this case study the aim was to determine the infiltration through the main entrance and stairwell, through windows and outside walls. The measurement technique used was the Homogeneous Emission Technique added with a 2-zone set up, by using two different tracer gases. Several interesting results were obtained. One result was that more airtight entrance doors would decrease the infiltration into the main stairwell with 40 %. Another result of interest was that the very dry outdoor air was mixed with the indoor air quickly, and that the mechanical ventilation system with filters and humidifying units has to be upgraded to compensate for the outdoor air infiltration.

    • In this paper we will introduce the general benefits of X-Ray Fluorescence spectrometry (XRF) for assessing the condition of the outer environment, especially where ambient aerosol particles are causing environmental disturbances. Examples from recent environmental studies are presented, and energy dispersive XRF is concluded to be a powerful, nondestructive yet easily applicable tool to supply detailed elemental information of particles collected in different applications. The further development and future potential of the method for detailed analysis of aerosol particles are discussed.

    • In applications of computerized tomography (CT) it is often of interest to reconstruct a function in a proper subset, the region of interest (ROI), of its support from a proper subset of a full CT-scan. In several recent works it has been shown that stable ROI reconstruction is possible in certain limited data situations. In this paper we investigate the limits of what is possible in that direction by proving that reconstruction in an interior ROI is severely unstable for a certain set of data that contains integrals over all lines that intersect the ROI and is large enough for the unknown function to be uniquely determined.

Using closely related methods we study here the relationship between the analytic wave front set for the characteristic function, χD, of a domain D⊂Rn and singularities of the boundary ∂D of the domain. For instance we prove that the boundary surface must be real analytic in a neighborhood of a point z∈∂D∈C1, if the analytic wave front set of χD at z contains no other elements than the conormals to ∂D at z.

    • In this paper we study Schrodinger operators with absolutely integrable potentials on metric graphs. Uniform bounds-i.e. depending only on the graph and the potential-on the difference between the eigenvalues of the Laplace and Schrodinger operators are obtained. This in turn allows us to prove an extension of the classical Ambartsumian Theorem which was originally proven for Schrodinger operators with Neumann conditions on an interval. We also extend a previous result relating the spectrum of a Schrodinger operator to the Euler characteristic of the underlying metric graph.

    • Using a vanishing theorem for microlocally real analytic distributions and a theorem on flatness of a distribution vanishing on infinitely many hyperplanes we give a new proof of an injectivity theorem of Bélisle, Massé, and Ransford for the ray transform on R^n. By means of  an example we show  that this result is sharp. An extension is given where real analyticity is replaced by quasianalyticity.

    • INTRODUCTION: In COMET (Carvedilol or Metoprolol European Trial), carvedilol reduced mortality compared with metoprolol in patients with chronic heart failure. We hypothesized that carvedilol might have greater effects on endothelial derived haemostatic factors than metoprolol. We aimed to study the effects of carvedilol or metoprolol on tissue plasminogen activator (tPA), its inhibitor PAI-1 and Von Willebrand factor (VWF) in patients with heart failure. MATERIAL AND METHODS: We recruited 260 patients (134 on carvedilol, 126 on metoprolol), mean age 66 years and 84% of them men. Plasma mass concentrations of tPA and PAI-1and percent of VWF were measured at baseline and after one and two years of treatment. RESULTS: Plasma tPA, PAI-1 and VWF were similar between treatment groups at baseline and no significant differences between groups emerged after one or two years of treatment. In paired analyses in patients assigned to carvedilol, median PAI-1 level decreased from 37.2 to 32.1 microg/l at two years (p=0.034) and of VWF decreased from baseline to one year (240 vs. 218%, p=0.023) in patients assigned to carvedilol but were not reduced at any time in patients assigned to metoprolol. Plasma tPA increased over time in both treatment groups (p=0.013 and 0.027 respectively). CONCLUSION: We found no significant difference in the effects of carvedilol or metoprolol on tPA, PAI-1 and VWF. Comparison over time within treatment groups suggested that PAI-1 and VWF might have declined on carvedilol but not on metoprolol. Our hypothesis is not proved but this may reflect an inadequate sample size rather than lack of an effect.

  • Healthcare resource utilisation and costs associated with a heart failure diagnosis : A retrospective, population-based cohort study in Sweden

    • Objectives: To examine healthcare resource use (HRU) and costs among heart failure (HF) patients using population data from Sweden.Design: Retrospective, non-interventional cohort study.Setting: Two cohorts were identified from linked national health registers (cohort 1, 2005-2014) and electronic medical records (cohort 2, 2010-2012; primary/secondary care patients from Uppsala and Västerbotten).Participants: Patients (aged ≥18 years) with primary or secondary diagnoses of HF (≥2 International Classification of Diseases and Related Health Problems, 10th revision classification) during the identification period of January 2005 to March 2015 were included.Outcome measures: HRU across the HF phenotypes was assessed with logistic regression. Costs were estimated based on diagnosis-related group codes and general price lists.Results: Total annual costs of secondary care of prevalent HF increased from SEK 6.23 (€0.60) to 8.86 (€0.85) billion between 2005 and 2014. Of 4648 incident patients, HF phenotype was known for 1715: reduced ejection fraction (HFrEF): 64.5%, preserved ejection fraction (HFpEF): 35.5%. Within 1 year of HF diagnosis, the proportion of patients hospitalised was only marginally higher for HFrEF versus HFpEF (all-cause (95% CI): 64.7% (60.8 to 68.4) vs 63.7% (60.8 to 66.5), HR 0.91, p=0.14; cardiovascular disease related (95% CI): 61.1% (57.1 to 64.8) vs 60.9% (58.0 to 63.7), HR 0.93, p=0.28). Frequency of hospitalisations and outpatient visits per patient declined after the first year. All-cause secondary care costs in the first year were SEK 122 758 (€12 890)/patient/year, with HF-specific care accounting for 69% of the costs. Overall, 10% of the most expensive population (younger; predominantly male; more likely to have comorbidities) incurred ~40% of total secondary care costs.Conclusions: HF-associated costs and HRU are high, especially during the first year of diagnosis. This is driven by high hospitalisations rates. Understanding the profile of resource-intensive patients being at younger age, male sex and high Charlson comorbidity index scores at the time of the HF diagnosis is most likely a sign of more severe disease.

    • Vid hjärtsvikt kan vikten av att identifiera bakomliggande orsak aldrig nog betonas. Avancerade undersökningar ger inte alltid diagnos om man inte har en noggrann anamnes och auskultation som grund. Beakta alltid risken av en okritisk förförståelse; ju fler undersökningar som är gjorda, desto större är risken, dvs att man förlitar sig på tidigare satta diagnoser utan att kritiskt grans­ka underlaget. Vid oklara fall behövs tid och utrymme för reflexion och omprövning, vilket måhända kan komma i konflikt med en hårt processdriven och ekonomiskt inriktad sjukvård.

    • The overall aim of this thesis is to explore how the labour market situation appears to persons with different types of disabilities in Sweden. Study I (n=4 359) examined the importance of the person’s type of disability for the possibility of having a job. Study II (n=3 396) examined whether there were differences in occupational attainment between persons with different types of disabilities and between persons with and without a disability (n=19 004). Study III (n=4 359) examined perceived discrimination in the workplace among persons with different types of disabilities. Study IV (n=2 268) examined whatsupport and adaptation persons with different types of disabilities need in their actual workplace or to enter the labour market. In studies I–III the respondents were divided into six disability types, and in study IV the respondents were divided into four disability types.Overall, it can be concluded that a person’s type of disability is of importance for the situation in the labour market, where persons with psychological disabilities encounter barriers to a greater extent than other groups, both difficulties at the present workplace and difficulties entering the labour market. Further, they were subjected to discrimination and they expressed a great need for adaptations to acquire employment and at their actual workplace. It can also be concluded that there are discriminatory mechanisms against women, who have more difficulties than men in acquiring employment, and that men, despite lower education, can more easily progress in the labour market. It is suggested that, in order to achieve inclusion and recognition, factors in the environment, such as existing support systems, need to be changed and developed, and social prejudices vis-à-vis people with disabilities must also be addressed.

    • ABSTRACT: BACKGROUND: Exposure to particulate matter (PM) has been linked to several adverse cardiopulmonary effects, probably via biological mechanisms involving inflammation. The pro-inflammatory potential of PM depends on the particles' physical and chemical characteristics, which again depend on the emitting source. Wood combustion is a major source of ambient air pollution in northern countries during the winter season. The overall aim of this study was therefore to investigate the cellular responses to wood smoke particles (WSPs) collected from different phases of the combustion cycle, and from combustion at different temperatures. RESULTS: WSPs from different phases of the combustion cycle induced very similar effects on pro-inflammatory mediator release, cytotoxicity and cell number, whereas WSPs from medium-temperature combustion were more cytotoxic than WSPs from high-temperature incomplete combustion. Furthermore, comparisons of effects induced by native WSPs with the corresponding organic extracts and washed particles revealed that the organic fraction was the most important determinant for the WSP-induced effects. However, the responses induced by the organic fraction could generally not be linked to the content of the measured polycyclic aromatic hydrocarbons (PAHs), suggesting that also other organic compounds were involved. CONCLUSION: The toxicity of WSPs seems to a large extent to be determined by stove type and combustion conditions, rather than the phase of the combustion cycle. Notably, this toxicity seems to strongly depend on the organic fraction, and it is probably associated with organic components other than the commonly measured unsubstituted PAHs.
