    • Kännetecknade för Gro Dahles bilderböcker är att de inte värjer sig för att gestalta komplexa och kontroversiella teman. Agnes-Margrethe Bjorvand menar i sin analys av boken Sinna mann (2003) att Dahle bidragit till att förnya bilderboksgenren:”[w]hat is new, however, is that such a concrete presentation of physical abuse and violence inside the home finds its way into picturebooks intended to be read to and by relatively small children” (2010, s. 217). Därutöver finns det inte sällan i bilderböckerna en problematiserande ansats som erbjuder flera tolkningsvägar. Ett exempel är Snill (2002) av Dahle (text) och Svein Nyhus (bild) där problemställningen kretsar kring flickan Lussis frigörelse från att inledningsvis vara en snäll, tyst och osynlig tyst flicka till att bli en individ med egen vilja. Eva Maagerø och Guri Lorentzen Østbye (2017) resonerar kring hur Snill i första hand gestaltar Lussis frigörelse men att den avslutande bilden också signalerar andra barns, till exempel pojkars, behov av frigörelse, vilket även är en slutsats som Alfredsson (2021) drar. I föreliggande studie är det Sesam Sesam (2017) av Gro Dahle (text) och Kaia Dahle Nyhus (bild) som är i fokus. Berättelsen handlar kortfattat om hur pojken Al blir påkommen av sin mor när han tittar på pornografi på nätet, moderns reaktion och samtalet mellan Al och hans storebror Kas om vad porr egentligen är. Precis som i Snill och Sinna mann gestaltas ett brett känsloregister ur barnets perspektiv. Ämnet är känslomässigt komplicerat och berättelsen visar hur olika typer av affekter i Silvan S. Tomkins definition som exempelvis nyfikenhet, avsmak och skam, kan ta sig uttryck. I denna artikel är syftet att visa hur Dahles text och Kaia Dahle Nyhus illustrationer samspelar och skapar ett expressivt formspråk genom att framhäva, tona ned eller problematisera olika affekter (Nathanson 1992; Tomkins 1995). Vidare kommer vi att diskutera vilka litteraturdidaktiska möjligheter vår analys rymmer. Detta resonemang är samtidigt en vidareutveckling av ett redan påbörjat projekt med syftet att undersöka möjligheterna med att fläta samman litteraturvetenskapliga och språkvetenskapliga analysmetoder i undervisningen vid svensklärarutbildningen (Dahl & Smedberg Bondesson 2021). Den visuella analysen rymmer tre delar: ideationell, textuell och interpersonell betydelse. Ideationell betydelse avser här det Kress & van Leeuwen benämner narrativa processer och de frågor vi ställer är: Vad händer i bilden och hur gestaltas det? Textuell analys innebär en kartläggning av hur de avbildade är relaterade till varandra i fråga om avstånd och storlek och de betydelser som samspelen mellan de avbildade ger uttryck för. Vi tar fasta på aspekterna avgränsning och sambandsskapande, samt avstånd, visuellt rim och kontrast (Kress & van Leeuwen, s. 201ff). Den interpersonella analysen handlar om de sociala relationer människor etablerar visuellt. Här fokuserar vi på hur bruk av färg realiserar interpersonell betydelse som glädje, sorg, ilska eller rädsla med hjälp av aspekterna färgmättnad, modulation och differentiering (Kress & van Leeuwen, s. 232ff). Vidare kommer vi i analysen och tolkningen med mer litteraturvetenskapliga anspråk att använda tre topi-begrepp, nämligen Bakhtins kronotopi, främst tröskelkronotopin, Foucaults heterotopi, samt lingvistikens diatopi-begrepp, omformulerat och omdefinierat för att tjäna litteraturvetenskapliga syften. Atle Krogstads artikel ”The family house chronotope in three picturebooks by Gro Dahle and Svein Nyhus: idyll, fantasy, and threshold experiences” (2016) konstaterar att familjehuset är den primära scenen för händelserna i Dahles och Nyhus bilderboksuniversum. Vi instämmer med Krogstad i detta liksom i påståendet att Dahles och Nyhus bilderboksberättelser kan betraktas som just tröskelstudier som utforskar, förtydligar och tänjer ut det tillfälle och den tidpunkt då barnet förändras och blir synligt i den vuxna världen. Bakhtins tröskelkronotopi gestaltar existentiellt avgörande förändringar, medan Foucaults heterotopi erbjuder en motplats som ställer övriga världen på ända (Bachtin 1991, s. 157; Foucault 2008, s. 252 f.). Smedberg Bondessons diatopi häver klaustrofobin och innebär en öppning mellan den inre och den yttre världen, en genomsläpplighet och vidgning mot universums oändlighet, i fantasin såväl som i verkligheten (Smedberg Bondesson 2014, s. 50; 2017, s. 208; 217 f; 2018, s. 75). Analysen visar hur visuella val som rör narrativa processer, samband och avstånd samt interpersonella aspekter av färg såsom färgmättnad, differentiering och kontrast förstärker och förtydligar såväl materiella (yttre) som mentala (inre) dimensioner av den skrivna berättelsen. Starka affekter som avsmak, nyfikenhet och skam men också den nödvändiga tryggheten, känslan av att dela världen med någon, det ömsesidiga förtroendet som förutsättning för att klara av att reglera affekterna, kan urskiljas i berättelsens dramaturgi. En slutsats är att Kress & van Leeuwens modell möjliggör en detaljerad multimodal analys av olika affekter i en bilderbok som Sesam Sesam – och vidare ger detta en fingervisning om att man med fördel torde kunna kombinera en litteraturvetenskaplig/litteraturdidaktisk analys med en språkvetenskaplig/sociosemiotisk. Denna kombination är intressant ur många perspektiv, inte minst ett didaktiskt och svenskämnesperspektiv med tanke på hur språk- och litteraturdelen många gånger behandlas som skilda entiteter i klassrummet. Vidare står det i läroplanen att elever ska arbeta med ”texter som kombinerar ord, bild och ljud, och deras språkliga och dramaturgiska komponenter. Hur uttrycken kan samspela med varandra, till exempel i tv-serier, teaterföreställningar och webbtexter.” Här kan man tillägga bilderböcker. Den nordiska bilderboken utmärker sig nämligen på så vis att den inte uteslutande riktar sig till småbarn. Författare som till exempel Gro Dahle eller danskarna Oskar K och Dorte Karrebæk har visat hur gränsen mellan barn- och ungdomslitteratur samt vuxenlitteratur kan tänjas och problematiseras.   • Can Baseline Characteristics Predict Successful Outcomes after Individual, Physiotherapist-Led Rehabilitation in Patients with Chronic Musculoskeletal Pain?

    • BACKGROUND: No strong and consistent variables to predict outcome after pain rehabilitation have been reported in patients with chronic musculoskeletal pain. The aim of the present study was to clarify if baseline variables could predict successful outcome after a unique, individualized, physiotherapist-led rehabilitation of nine sessions.METHODS: In 274 individuals with severe chronic musculoskeletal pain, the risk ratio (RR) and 95% confidence intervals (CIs) were estimated for potentially predictive baseline variables on successful outcomes of pain management, overall health, and pain rating.RESULTS: Statistically significant results show that patients rating moderate or severe baseline pain were in both cases 14% less likely to improve pain management compared to patients rating mild baseline pain (RR = 0.86; 95% CI 0.77-0.97, RR = 0.86; 95% CI 0.74-1.00). Patients with the shortest pain duration were 1.61 times more likely to improve overall health (RR = 1.61; 95% CI 1.13-2.29) compared to patients reporting the longest pain duration (>5 years). Patients reporting anxiety/depression or severe pain were in both cases 1.48 times more likely to improve overall health compared to better baseline presentations (RR = 1.48; 95% CI 1.16-1.88, RR = 1.48; 95% CI 1.03-2.15). Patients with regional/generalized pain were 36% less likely to rate pain reduction (RR = 0.64; 95% CI 0.41-1.00) compared to patients rating localized baseline pain. Of 17 potentially predictive baseline variables, four reached statistical significance for at least one of the three outcomes; although none of them for all three outcomes.CONCLUSIONS: Of 17 potentially predictive baseline variables, mild pain ratings, short pain duration, and localized baseline pain were statistically significantly associated with improvements after individual, physiotherapist-led rehabilitation for patients with chronic musculoskeletal pain. This suggests that this type of rehabilitation probably should be offered early in the pain process. Reporting anxiety/depression or severe pain at the baseline did not hinder the improvements of overall health.

    • BACKGROUND: There is no consensus on best content, set-up, category of involved healthcare professionals or duration of rehabilitation-programs for patients with chronic musculoskeletal pain, and outcomes show varying results. Individual care regimes for sub-groups of patients have been proposed.AIM: To describe the type of interventions used in a physiotherapist-led, rehabilitation-program for patients with chronic musculoskeletal pain, refractory to preceding treatments. A second aim was to report clinical outcomes at 1-year follow-up after the intervention period.METHODS: All patients referred to physiotherapist within a specialist pain-unit due to being refractory to preceding treatments, and deemed fit to undergo physiotherapy-based, individualized rehabilitation during 2014-2018 were consecutively included and followed-up 1 year after ending the program. The inclusion was based on structured 'clinical reasoning' using the referral, examination and on patient-relevant outcome measures. The individual interventions, recorded according to a manual used when reading the patients' medical records, were described. Primary outcomes were clinical results of perceived pain, disability and overall health at start, discharge and 1 year after discharge.RESULTS: In total, 274 patients (mean age 42 years, 71% women) were included, suffering from chronic, severe, musculoskeletal pain (VAS median 7/10, duration median 2.8 years) and moderate disability. The most frequent interventions were education, sensorimotor training, physical activity-advice and interventions for structures/functions (for example manual techniques, stretching) for a median of nine sessions during five months. Despite refractory to preceding treatments, 45% of the patients rated clinically important improvements on pain, 61% on disability and 50% on overall health at discharge and the figures were similar at 1-year follow-up.CONCLUSIONS: A physiotherapist-led, one-to-one, rehabilitation-program of median nine sessions during five months, combining individualized education, sensorimotor training, physical activity-advice and interventions for structures/functions rendered clinically relevant improvements on pain, disability and overall health in half of the patients at 1-year follow-up. Since the cohort consisted of patients refractory to preceding treatments, we believe that these results warrant further studies to identify the subgroups of patients with chronic musculoskeletal pain that will improve from new, distinctive, resource-effective rehabilitation-programs involving individualized rehabilitation.

    • I kapitlet ”Narrativ teknik och känsla i Hemingways ’Hills Like White Elephants’” utgår vi från frågan om det huruvida samtidigt vara känslomässigt berörd av en text och förhålla sig kritiskt till den. I en undersökning av förhållandet mellan estetisk form och känslor argumenterar vi för att kritisk närläsning kan vara ett helt nödvändigt första steg för att uppfatta karaktärers känslor och för att som läsare själv känna känslomässigt engagemang för en text. Analysen fokuserar på de känslor “Hills Like White Elephants” förmedlar genom sin minimalistiska form. Kapitlet diskuterar den effekt formen kan ha på läsaren men snarare än att försöka slå fast vilka känslor som kan eller bör genereras menar vi att pedagogens uppgift främst är att sträva efter att ge studenterna en beredskap för att ta till sig konstverket, det vill säga, arbeta med det som Felski kallar attunement. Vi föreslår tre huvudområden att arbeta med i klassrummet för att uppnå en djupare textförståelse som i sin tur kan leda till attunement. Det första är kulturell och litterär kontext: vad behöver läsaren veta om modernismen för att förstå ”Hills Like White Elephants”? Det andra är analys av miljö, vilket inbegriper bland annat betydelsen av plats men också språkliga konstruktioner och symbolism. Slutligen diskuteras förhållandet mellan karaktärernas känslor och läsarens samt frågan om huruvida det är rimligt att ha som mål att dessa båda parter ska dela känslotillstånd.

    • PURPOSE: We have developed a model for integrated medicines management, including tools and activities for medication reconciliation and medication review. In this study, we focus on improving the quality of the discharge summary including the medication report to reduce medication errors in the transition from hospital to primary and community care. METHODS: This study is a longitudinal study with an intervention group and a control group. The intervention group comprised 52 patients, who were included from 1 March 2006 until 31 December 2006, with a break during summer. Inclusion in the control group was performed in the same wards during the period 1 September 2005 until 20 December 2005, and 63 patients were included in the control group. In order to improve the quality of the medication report, clinical pharmacists reviewed and gave feedback to the physician on the discharge summary before patient discharge, using a structured checklist. Medication errors were then identified by comparing the medication list in the discharge summary with the first medication list used in the community health care after the patient had returned home. RESULTS: By improving the quality of the discharge summary, patients had on average 45% fewer medication errors per patient (P = 0.012). The proportion of patients without medication errors was 63.5% in the control group and 73.1% in the intervention group. However, this increase was not significant (P = 0.319). Patients who used a specific medication dispensing system (ApoDos) had a 5.9-fold higher risk of suffering from medication errors than those without this medication dispensing system (P < 0.001). CONCLUSION: Review and feedback on errors in the discharge summary, including the medication report and a correct medication list, reduced medication errors during the transfer of information from hospital to primary and community care.

    • Objective To avoid negative effects of drug treatment and need for additional medical care, drug treatment must be individualised. Our research group has developed a model for clinical pharmacy which improves several aspects of the patient's drug treatment. This study describes the process behind these improvements, i.e. drug related problems identified by pharmacists within a clinical pharmacy service. Setting Three wards at a department of internal medicine. Method Pharmacists performed systematic interventions during the patient's hospital stay, aiming to identify, solve and prevent drug related problems in the elderly. Identified drug related problems were put forward to the health care team and discussed. Information on identified problems, and their outcomes was collected and analysed. A questionnaire was used to evaluate the health care personnel's attitudes towards the process. Main outcome measure The number of drug related problems identified by the clinical pharmacists, the proportion of problems discussed with the physicians, the proportion of problems adjusted by the physicians and whether pharmacists and physicians prioritised any subgroup of drug related problems when choosing which problems to address. Finally, we wanted to evaluate the health care personnel's attitudes towards the model. Results In total, 1,227 problem were identified in 190 patients. The pharmacists discussed 685 (55.8%) of the identified problems with the physicians who accepted 438 (63.9%) of the suggestions. There was no significant difference in which subgroup to put forward and which to adjust. There was a high response rate (84%) to the questionnaire, and the health care personnel estimated the benefits to be very high, both for the patients and for themselves. Conclusion The process for identifying, solving and preventing drug related problems was good and the different types of problems were considered equally important. The addition of a clinical pharmacy service was considered very useful. This suggests that the addition of our clinical pharmacy service to the hospital setting add skills of great importance.

  • The Enterohepatic Disposition of Rosuvastatin in Pigs and The Impact of Concomitant Dosing with Cyclosporine and Gemfibrozil

    • The hepatobiliary transport and local disposition of rosuvastatin in pig was investigated, along with the impact of concomitant dosing with two known multiple transport inhibitors; cyclosporine and gemfibrozil. 80 mg rosuvastatin was administered as an intrajejunal bolus dose in Treatments I, II and III (TI, TII, and TIII, respectively; n=6 per treatment). Cyclosporine (300 mg) and gemfibrozil (600 mg) were administered in addition to the rosuvastatin dose in TII and TIII, respectively. Cyclosporine was administered as a two hour intravenous infusion and gemfibrozil as an intrajejunal bolus dose. In Treatment IV (TIV, n=4) was 5.9 mg rosuvastatin administered as an intravenous bolus dose. The study was conducted using a pig model, which enabled plasma sampling from the portal (VP), hepatic (VH) and femoral veins and bile from the common hepatic duct. The biliary recovery of the administered rosuvastatin dose was 9.0±3.5% and 35.7±15.6% in TI and TIV, respectively. Rosuvastatin was highly transported into bile as the median AUCbile/AUCVH ratio was 1770 (1640-11300) in TI. Gemfibrozil did not have an effect on the plasma pharmacokinetics of rosuvastatin, most likely because the unbound inhibitor concentrations did not exceed the reported IC50-values. However, cyclosporine significantly reduced the hepatic extraction of rosuvastatin (TI; 0.89±0.06, TII; 0.46±0.13) and increased the AUCVP and AUCVH by 1.6 and 9.1-fold, respectively. In addition, the biliary exposure and fe, bile were reduced by ≈50%. The strong effect of cyclosporine was in accordance with inhibition of sinusoidal uptake transporters, such as members of the OATP-family, rather than canalicular transporters.

    • This work deals with Smart Textiles in interaction with the body. We design textiles and outfits as tools that can influence fashion and textile design. Central to our work is that artistic envisioning can point to new possibilities and values, in which we want to stress the importance of combining traditional materials and methods with contemporary and future functions in order to obtain sustainable ideas. The film documents a performance, where dancers create a link between the body, the textile material and the room surrounding the body. The textile material and the garment are to inspire movement that, in turn, creates development; when a person wears the garment and moves in a certain way or touches other persons, the visual expression of the room changes through an electronic signal. In this case, the colour of the pattern of the textile draping changes to the static pattern that is printed on the person’s outfit. The point of the show was to show possibilities of non-static and dynamic design through scenic expression.

    • I Kristina Lugns sista diktsamling Hej då, ha det så bra! från 2003 drivs den egna poetiken till sin spets. Metaforikens oegentlighet blottas medan bokstavlighetens absurditet tar plats. Enligt Lars Elleström ringar Lugns dikter ”in människans konkret kroppsliga, och därmed också mentala, villkor. [---] När någon människa som står oss nära sårar oss, säger vi att det gör ont, och det är definitivt inte ’bildspråk’” (Elleström 2006: 28). I mitt föredrag gäller detta resonemang inte bara den kroppsliga metaforiken utan hela den bokstavliga språkdräkten, som i sin roll som ”sparkbyxor” rymmer humorns avväpnande sprängkraft. En dikt inleds: ”Jag behöver tystnad / och ensamhet / och en språkdräkt med god passform”. Först det sista ordet välter meningen över ända genom att bokstavliggöra metaforen språkdräkt. Men egentligen välts inget över ända. Vi står tvärtom stadigare än förr, med ett större mått av konkretion i våra slitstarka ”verklighetsunderlag”. Ann-Helén Andersson formulerar i sin avhandling ”Jagär baserad på verkliga personer”. Ironi och röstgivande i Kristina Lugns författarskap att ”skrattet i den humoristiska texten huvudsakligen finns där för sin egen skull, medan det i den satiriska respektive ironiska texten däremot präglas av ett vidare syfte. I Lugns texter kan det komiska ses som ett element inom en dominerande ironisk tendens.” (Andersson 2010: 115). Att bokstavliggöra metaforiska uttryck räknas i den klassiska retoriken till de ironiska troperna. Ändå vill jag i mitt föredrag, med utgångspunkt i Luigi Pirandellos essä L’umorismo från 1908, argumentera för att Lugns bokstavliga språkdräkt kan fungera som en humoristisk och komisk, snarare än ironisk, strategi (inte minst i betydelsen läsarens förhållningssätt till och bruk av den lyriska texten) för att få verkligheten att framträda som aningen mer fattbar och gripbar – i all sin fasansfulla absurditet.     • In this project we create a link between body, textile material and space. Textiles and garments shall inspire to motion that generates variability. When a body moves through a space, touches other bodies and parts of its garment, it affects the visual expression in the room. More specifically the background changes and adapts partially to the pattern of the garments. The base for our investigation is to perform artistic work with the expression of set design in the centre, that shows the possibilities, matters and values of fashion and textile design beyond the traditional boundaries. The tapestry is weawed in cotton, steel and wool. Print in heat sensitive pigment (supplier Variotherm Zenit Konsthantverk AB). The dresses are knitted in cotton and silverthreads.     • Computation and new materials are entering the world of textiles, challenging our view on the textile material. As new techniques and electrically conductive fibres enable the design of textile circuits and computationally active textiles [2], the areas of smart textile design and interaction design start to merge. Wearable computing [cf.1], the notion of moving computational tools directly onto the body, might have been the first approach to bring computation technology closer to the area of clothing.. In an approach to investigate new enhanced forms of expressional interaction through textiles, the relationship between tactile and visual aesthetical properties are explored in the present paper. Textile Dimensions, an interactive set of textiles, shows how clothes and textiles become interfaces themselves, able to sense and react on external stimuli in expressive ways.

    • BACKGROUND: Globally, 700 000 people die every year by suicide. Health care consultation patterns the period before suicide could be one potential way to identify people at risk for suicide. Therefore, this study examines health care patterns up to one year prior to the suicide by age, sex and prior diagnoses and specifically investigates if and how this differs from the general population of Skåne, Sweden.METHODS: This cohort study includes all individuals, aged 15 and older, that died by suicide in Region Skåne, Sweden from 2004 to 2015 (n = 1653). The individuals were identified through the Cause of death register and then linked to the Skåne healthcare register. Health care data was analyzed as proportions consulting different types of health care the month and year preceding the suicide, we also investigated the impact of age, sex and the occurrence of prior psychiatric and pain diagnoses. Additionally, we compared the proportion of consulting care among the suicide victims and the general population of Skåne.RESULTS: In the month before their death, 53% of the suicide victims had any health care consultation, compared with 20% in the general population of Skåne, a given month (p < 0.0001). The corresponding figures for the year prior to suicide was 86% among those who died by suicide, compared to 69% in the general population of Skåne, a given year (p < 0.0001). Women, and those having a documented history of psychiatric diagnosis were more likely to have health care consultations in the month and year preceding suicide (p < 0.001), compared to men and suicide victims without a history of psychiatric disease. Older adults that died by suicide, were less likely to consult psychiatric care compared to the younger suicide victims (p < 0.001).CONCLUSION: A majority of the suicide victims consulted health care in the near time before death and the proportion of seeking health care was significantly higher than in the general population of Skåne and higher among female suicide victims as compared to males. Alternative preventive screening measures should be considered for individuals consulting health care, especially for older people and individuals outside the psychiatric care.

  • Utilization of healthcare and prescription medicines after non-pharmacological interventions for depression - A 3-year register follow-up of an RCT in primary care

    • Depression is a common, recurrent disorder. There is a need for readily available treatments with few negative side effects, that demands little resources and that are effective both in the short- and long term. Our aim was to investigate the long-term effectiveness of two different interventions; physical exercise and internet-based cognitive behavioural therapy (internet-CBT), compared to usual care in patients with mild to moderate depression in a Swedish primary care setting. We performed a register-based 3-year follow-up study of participants in the randomized controlled trial REGASSA (n = 940) using healthcare utilization and dispensed medicines as outcomes. We found no difference between the three groups regarding proportion of participants consulting healthcare due to mental illness or pain during follow-up. Regarding number of consultations, there was no difference between the groups, except for consultations related to pain. For this outcome both treatment arms had significantly fewer consultations compared to usual care, during year 2-3, the risk ratio (RR) for physical exercise and internet-CBT was 0.64 (95% CI = 0.43-0.95) and 0.61 (95% CI = 0.41-0.90), respectively. A significantly lower proportion of patients in both treatment arms were dispensed hypnotics and sedatives year 2-3 compared to the usual care arm, RR for both physical exercise and internet-CBT was 0.72 (95% CI = 0.53-0.98). No other differences between the groups were found. In conclusion, considering long-term effects, both physical exercise and internet-CBT, being resource-efficient treatments, could be considered as appropriate additions for patients with mild to moderate depression in primary care settings.

    • Det övergripande syftet i alla våra gemensamma projekt är att bota svenskämnets schizofreni, det vill säga dess egen inneboende och paradoxalt skarpa gränslinje mellan språk och litteratur, mellan språkvetenskap och litteraturvetenskap. Vi kommer att med olika exempel visa hur en kombination av litteraturvetenskaplig och språkvetenskaplig analys, det dubbla perspektivet (Dahl & Smedberg Bondesson 2021) som vi kallar det, är fruktbara ur ett didaktiskt perspektiv. Våra exempel berör två skönlitterära skildringar: ”Far och jag” (Onda sagor, 1924) av Pär Lagerkvist och bilderboken Sesam Sesam (2017) av Gro Dahle och Kaia Dahle Nyhus. Vad gäller ”Far och jag” är det en empirisk studie där vi undersöker lärarlyftsstudenters uppfattningar om det dubbla perspektivets möjligheter till en fördjupad förståelse för Pär Lagerkvists novell med hjälp av dels en språkvetenskaplig textanalys, dels en litteraturvetenskaplig analys. I det andra exemplet gör vi en multimodal och platsorienterad analys av bilderboken Sesam Sesam och vilka typer av affekter som den rymmer. Här kombineras en sociosemiotisk analysmodell (Kress & van Leeuwen) med en plats- och affektteoretisk litteraturvetenskaplig tolkning där vi använder begrepp från Bakhtin, Foucault och Tomkins. Med utgångspunkt i dessa exempel diskuterar vi vilka didaktiska möjligheter som det dubbla perspektivet rymmer. Vi menar att man undviker en ämnessplittring genom att synliggöra hur språk och litteratur kan samspela på ett produktivt sätt, till exempel hur det dubbla perspektivet ger eleven/studenten förutsättningar för mer nyanserade tolkningar av och därigenom en ökad förståelse för olika slags berättelser och berättande, både i skrift och bild. Detta menar vi stärker elevens/studentens litterära kompetens, kritiska tänkande samt förmåga att problematisera känsliga och kontroversiella ämnen.

    • Pain treatment of lactating bitches is a clinically relevant, but complicated issue. Published scientific studies regarding the excretion of drugs in canine milk are scarce. When considering the risk of side effects in their offspring, lactating bitches have traditionally received very restricted analgesic and anti-inflammatory therapy. Our aim was to quantify the concentrations of carprofen in milk from lactating bitches and relate those to potential risks for the puppies. A second aim was to evaluate the impact mastitis may have on the concentration of carprofen in milk. A population of 100 bitches was enrolled in the study, among which 88 were bitches treated with carprofen after cesarean section (Group CS), eight were bitches with painful inflammatory conditions (Group I) and four were bitches with mastitis (Group M). The patients enrolled in the study received carprofen 4 mg/kg sc at day 1 followed by 2 mg/kg po every 12 h for the following 2-5 days. Owners were instructed to collect milk once a day for five days. The concentration of carprofen in the milk was quantified with ultra-performance liquid chromatography-tandem mass spectrometry. The data obtained were statistically analyzed as repeated-measures data with a mixed-model approach. Data were used to calculate the theoretical maximum total daily intake of carprofen by the puppies in order to perform a computerized simulation of the plasma concentration of carprofen in the puppies. Follow-up telephone interviews to check the status of the enrolled bitches and their litters occurred at one week and three-six months after treatment with car-profen. The major finding of the study was that the concentration of carprofen in the milk was <700 ng/ mL from bitches undergoing CS or suffering painful conditions other than mastitis. In comparison, administration of 2 mg/kg of carprofen sc or po to adult dogs, results in mean maximal plasma con-centrations of 19480 +/- 5420 ng/mL (mean +/- SD). Moreover, data suggests that inflammation of the mammary gland results in a higher concentration of carprofen in milk (up to 1300 ng/mL). In the computerized simulation, the plasma concentrations of carprofen in puppies in group CS and in group I are one tenth of the concentration in adult dogs receiving carprofen at standard doses. Considering the low excretion into milk, carprofen provides an analgesic alternative to lactating bitches without mastitis.(c) 2022 Published by Elsevier Inc.

    • Background: Older people with multiple diagnoses often have problems coping with their daily lives at home because of lack of coordination between various parts of the healthcare chain during the transit from hospital care to the home. To provide good care to those persons who have the most complex needs, regions and municipalities must work together. It is of importance to develop further empirical knowledge in relation to older persons with multiple diagnoses to illuminate possible obstacles to person-centred care during the transition between healthcare institutions and the persons livelihood. The aim of the present study was to describe nurses' experienced critical incidents in different parts of the intended healthcare chain of older people with multiple diagnoses.Methods: The sample consisted of 18 RNs in different parts of the healthcare system involved in the care of older people with multiple diagnoses. Data were collected by semi structured interviews and analysed according to Critical Incident Technique (CIT). A total of 169 critical incidents were identified describing experiences in recently experienced situations.Results: The result showed that organizational restrictions in providing care and limitations in collaboration were the main areas of experienced critical incidents. Actions took place due to the lack of preventive actions for care, difficulties in upholding patients' legal rights to participation in care, deficiencies in cooperation between organizations as well as ambiguous responsibilities and roles. The RNs experienced critical incidents that required moral actions to ensure continued person-centred nursing and provide evidence-based care. Both types of critical incidents required sole responsibility from the nurse. The RNs acted due to ethics, 'walking the extra mile', searching for person-centred information, and finding out own knowledge barriers.Conclusions: In conclusion and based on this critical incident study, home-based healthcare of older people with multiple diagnoses requires a nurse that is prepared to take personal and moral responsibility to ensure person-centred home-based healthcare. Furthermore, the development of in-between adjustments of organizations to secure cooperation, and transference of person-centred knowledge is needed.

    • Background The original project, where older persons received reablement performed by an interprofessional team showed success factors for IHR. However, since there is a lack of knowledge about why some persons do not recover despite receiving IHR, this study follows up patients' experiences of IHR.Aim To describe older persons' perceived dilemmas in the reablement process within the framework of IHR.Method 11 CIT interviews with participants who have previously received IHR, were analysed, interpreted and categorized according to CIT. The study was approved by the Swedish Ethical Review Authority.Results The results showed disease-related dilemmas, fatigue or pain so that participants could not cope with the prescribed exercises. New diseases appeared, as well as medication side effects made exercising difficult, and painkillers became a prerequisite for coping with IHR. Low self-motivation and mistrust towards the staff emerged like lack of trust due to otherness such as sex, cultural background, or language also became critical.Conclusions Interventions that consider individual- and contextual dilemmas are very important. By recognizing critical situations, this study can work as a basis of evidence to further develop interventions for older people living in their own homes and to ensure them to stay there.

    • Aim: This study aims to describe experiences of the implementation of a new integrated healthcare model for older adults with complex care needs due to multimorbidity, living at home, from a health and welfare personnel perspective. The goal was to diminish hospitalization and still carry out high quality care at home for older adults living with multimorbidity. The model was implemented by two organizations working in cooperation, the municipality, and the region that handles interprofessional social care and healthcare in people's homes. Materials and Method: Open-ended group interviews with personnel were carried out, three of the group interviews preimplementations of the model, and three of the group interviews post -implementation. The interviews were audiotaped and analysed according to the procedure of thematic analysis. Results: The quality of the integrated care model was based on care -chain cooperation, shared professionalism, and creating relations with the patient including closeness to next of kin, which was underlined by the participants. Unencumbered time gave the professionals the possibility to develop quality in integrated healthcare as part of integrated and person -centred care. The coproduction of education, research interviews and the follow-up meeting identified successes in diminishing hospitalization rates according to the participants' experiences of the post -implementation interviews. An identified failure was, however, that shared professionalism was not developed over time, rather the different responsibilities were accentuated according to the information retrieved at the follow-up meeting. Conclusion: Quality aspects of the model were identified in the present study. However, when implementation of a new model is completed, the organizations always have their own interpretation of how to further understand the model in question. Plain language summary: The intention of the present study was to follow the process of working with a new model of providing care at home, thus preventing increased numbers of hospital readmissions, based on the professionals point of view of what quality care is for older adults with complex care needs due to multimorbidity, living in their own home. The professionals were interviewed in group settings on several occasions during the implementation. The result showed hopeful expectations expressed by the professionals before the new model was implemented, such as a hope for getting more time for high -quality care for the older adults with multimorbidity. During the teamwork, the conversation within the team members was praised as a key factor that included shared professionalism from professionals with different levels of education and focus on their work. According to the staff, unnecessary hospital stays were reduced, while the interprofessional care -chain cooperation was improved through the work of the integrated care team. For many team members, the positive difference in both work and care satisfaction was highlighted in comparison to regular home care as they were able to use their multi -disciplinary skills and support.

    • AIMS To evaluate the acute haemodynamic effects of a single oral dose of vardenafil and to study the drug concentration in relation to haemodynamic effects in patients with pulmonary hypertension (PH). METHODS Sixteen patients with PH (aged 29-85\ years), received one single oral dose of vardenafil (5, 10 or 20 mg). The haemodynamic effect was assessed over a 60 min period. Vardenafil plasma concentrations were measured after 15, 30, 45 and 60 min using liquid chromatography-tandem mass spectrometry. RESULTS At 60 min a reduction in mPAP with a median % decrease of -20.3% (range -48.3 to 3.0; P < 0.001) and an increase in cardiac output and the cardiac index with a median % change of 10.6% (range -25.0 to 88.1; P = 0.015) and 12.1% (range -24.0 to 94.4; P = 0.01) respectively was observed. The pulmonary vascular resistance (PVR) was reduced with a median % decrease of -28.9% (range -61.5 to -5.9; P < 0.001), and pulmonary selectivity was reflected by a median percent reduction of -16.9% (range -49.0 to 16.5; P = 0.002; n = 14) in the PVR/ systemic vascular resistance ratio. There was a correlation between the plasma concentrations of vardenafil and change in mPAP (r = -0.579, P = 0.019) and between vardenafil concentrations and change in PVR (r = -0.662, P = 0.005). CONCLUSIONS Vardenafil causes rapid changes in cardiopulmonary haemodynamics and there is a correlation between plasma vardenafil drug concentration and the acute changes in mPAP as well as PVR in patients with PH.

    • OBJECTIVE: We investigated whether vardenafil, a phosphodiesterase-5 inhibitor, alters plasma levels of asymmetric dimethylarginine (ADMA), symmetric dimethylarginine (SDMA), and arginine.PATIENTS AND METHODS: ADMA, SDMA, and arginine were measured (0-540min) in 12 patients with pulmonary hypertension after a single oral dose of vardenafil. Invasive hemodynamic data were collected at baseline and after 60min.RESULTS: A reduction in ADMA was observed at 30 and 45min with a median change of -11.1% (P=0.021) and -12.5% (P=0.002). SDMA decreased with a median -5.3% change (P=0.032) at 45min. An increase in arginine, median 40.3% (P=0.002), 45.0% (P=0.010), and 77.1% (P=0.008) was observed at 120, 300, and 540min respectively. An increase in the arginine/ADMA ratio, median 11.7% (P=0.012), 32.5% (P=0.003), 26.5% (P=0.021), 33% (P=0.007), 48.5% (P=0.007), and 63.1% (P=0.008) was observed at 15, 45, 60, 120, 300, and 540min respectively. There was a positive correlation between vardenafil exposure and the percent change in the arginine/ADMA ratio from baseline to 540min (r=0.80; P=0.01). A correlation between baseline mean right atrial pressure (mRAP) and baseline ADMA (r=0.65; P=0.023), and baseline SDMA (r=0.61; P=0.035) was observed. A correlation between the baseline arginine/ADMA ratio and baseline cardiac output (CO) (r=0.59; P=0.045) and baseline cardiac index (CI) (r=0.61; P=0.036) was observed. Baseline arginine/ADMA ratio correlated with baseline mRAP (r=-0.79; P=0.002). A correlation between change (0-60min) in CI and change in arginine (r=0.77; P=0.003) as well as change in the arginine/ADMA ratio (r=0.61; P=0.037) was observed.CONCLUSIONS: Vardenafil induced changes in ADMA, SDMA, arginine, and the arginine/ADMA ratio in patients with PH. An increase in arginine and the arginine/ADMA ratio was associated with improvement in CI.

The over-arching research question is how the poets engage in planning and revision activities during the poetic writing, and how these processes relate to the linguistic content and literate properties of the final poems Expert writing has been characterized as domain specific skill that involves complicated problem-solving processes (Scardamalia & Bereiter, 1991). Expert writers are also described as advanced designers (Maun & Myhill, 2005), and knowledge crafters with abilities to adapt their texts to the audience with less cognitive effort (Kellogg, 2018). The professional creative writing process is however little explored from a cognitive writing perspective (but see Bekius, 2019). This study wanted to capture the full writing process of a poem. For this purpose two professional poets were recruited. The data collection took part in a lab setting, where the writing processes were collected by keystroke logging, combined with eye-tracking. Each poem was elicited by a prompt at the start of the 60-minute recording session. Both poets were satisfied with their poems. The final poems were analyzed linguistically and found to be lexically dense and diverse, with a high usage of low frequent words, sometimes neologisms, and very few conjunctions. The literary analyses show how the final poems are constituted of multi-layered nets, constructed through metaphorical and metonymical connections. The analyses of the writing processes show two dynamic, though different writers. While both spend much of their writing time pausing and revising on a global level, one of them shows a writing pattern consisting of long pauses followed by bursts of text production, the other shows a text production pattern with many immediate revisions, and shorter pauses. The amount of reading differs between the poets, but an extensive global reading of the text is suggested to be a prerequisite for the creation of the dense, metaphorical texts in the poetic genre. In sum, this combination of theoretical and methodological perspectives shows the writing process of two mature, conscious writers who has a global plan for their texts, and who have the domain specific skills to execute the plan.

De bidrag som samlats i den här volymen visar på bredden i konferensens fyra temaområden: - Litterär gestaltning, litterär stämning; Klassrumspraktiker: elevers engagemang under litteraturarbetet; Läsning, undervisning och utbildning; samt Berättande och medier: kognitiva och emotionella utmaningar - men också på hur viktiga litteraturdidaktiska frågor blivit i en tid då skönlitteraturen och läsandet ställs inför många utmaningar. Tillsammans ger bidragen en mångfasetterad bild av litterärt gestaltande av känslor och läsares möjliga känslomässiga reaktioner, liksom av de didaktiska möjligheter detta medför.

    • 1. The metabolite profile of the 5 alpha-reductase type II inhibitor finasteride has been studied in pig plasma, urine and bile using high-resolution mass spectrometry. The porcine biotransformation products were compared to those formed by human liver microsomes and to literature data of recently identified human in vivo metabolites. The objective of this study was to gain further evidence for the validity of using pigs for advanced, invasive drug-drug interaction studies that are not possible to perform in humans. 2. The use of high-resolution mass spectrometry with accurate mass measurements enabled identification of the metabolites by calculation of their elemental compositions as well as their fragmentation patterns. 3. There was an excellent match between the porcine and human metabolic profiles, corroborating the pig as a model of human drug metabolism. The glucuronides of the two recently described human hydroxylated metabolites MX and MY and the carboxylated metabolite M3 were identified as the major biotransformation products of finasteride in pig urine and bile. 4. Furthermore, the CYP enzymes involved in the formation of the hydroxylated metabolites were characterized. Human recombinant CYP3A4 could produce the two major hydroxylated metabolites MX and MY, whereas human recombinant CYP2D6 formed MY only.

    • The objective of this study was to further investigate the metabolism of the 5α-reductase inhibitor, finasteride, and to identify previously unknown phase I and phase II metabolites in vitro and in vivo in human bile and urine. Healthy volunteers were given 5 mg of finasteride, directly to the intestine, and bile and urine were collected for 3 and 24 h, respectively. A single-pass perfusion technique, Loc-I-Gut, was used for drug administration and bile collection from the proximal jejunum, distal to papilla of Vater. Incubations with human liver microsomes/S9 fractions and different cofactors were performed with finasteride and the previously known metabolites, ω-hydroxy finasteride (M1) and finasteride-ω-oic acid (M3). Liquid chromatography coupled to triple quadrupole mass spectrometry (MS) with positive/negative electrospray ionization and ion trap with MSn measurements were used for structural investigations and identification of metabolites. Two hydroxy metabolites of finasteride, other than M1, and one intact hydroxy finasteride glucuronide were identified in vitro and in bile and urine. The glucuronide and at least one of the hydroxy metabolites were previously unidentified. M1 and M3 were glucuronidated in vitro by specific human UDP-glucuronosyltransferases, UGT1A4 and UGT1A3, respectively. M1 glucuronide was not identified in vivo, and M3 glucuronide, an acyl glucuronide, was present in low amounts in bile from a few individuals. In conclusion, previously undescribed metabolites were identified, in vitro and in human urine and bile. Bile collection using the Loc-I-Gut technique followed by sensitive mass spectrometry analysis led to the discovery of novel, both phase I and phase II, finasteride metabolites in human bile.

    • The overall aim of this detailed investigation of finasteride's pharmacokinetics (PK) and metabolism in pigs was to improve the understanding of the in vivo PK for this drug and its metabolites. Specific aims were to examine the effects of ketoconazole co-administration on the PK in three plasma compartments (the portal, hepatic and femoral veins), bile and urine and to utilize these data to in detail study the intestinal absorption, the liver extraction ratio and apply a semi-physiological based PK model to the data. The pigs received an intrajejunal dose of finasteride (0.8 mg/kg) either alone (n=5) or together with ketoconazole (10 mg/kg) (n=5), or an intravenous dose (0.2 mg/kg) (n=3). Plasma, bile and urine (collected from 0-6 hours) were analyzed with ultra performance liquid chromatography tandem mass spectrometry. Ketoconazole increased the bioavailability of finasteride from 0.36±0.23 to 0.91±0.1 (p<0.05) and the terminal half-life from 1.6±0.4 to 4.0±1.1 hours (p<0.05). From deconvolution it was found that the absorption rate from the intestine to the portal vein was rapid and the product of the fraction absorbed and the fraction that escaped gut wall metabolism was high (fa*FG≈1). Interestingly, the apparent absorption rate constant (k(a)) to the femoral vein was lower compared to the portal vein, probably because of binding and distribution within the liver. The liver extraction ratio was time-dependent and varied with the two routes of administration. After intrajejunal administration, from 1 6 hours the liver extraction ratio was significantly (p<0.05) reduced by the ketoconazole treatment from intermediate (0.41±0.21) to low (0.21±0.10).

    • The aim of this study was to investigate what the consequences of induced drug metabolism, caused by St. John's wort (SJW, Hypericum perforatum) treatment, would have on the plasma, biliary and urinary pharmacokinetics of finasteride and its two previously identified phase I metabolites (hydroxy-finasteride and carboxy-finasteride). Twelve healthy men were administered 5mg finasteride directly to the intestine via a catheter with a multi-channel tubing system, Loc-I-Gut, before and after 14 days SJW (300mg b.i.d, hyperforin 4%) treatment. Bile samples were withdrawn via the Loc-I-Gut device from the proximal jejunum. LC-ESI-MS/MS was used to analyze finasteride and its metabolites in plasma, bile and urine. HPLC-UV was used to analyze hyperforin in plasma. The herbal treatment significantly reduced the peak plasma concentration (C(max)), the area under the plasma concentration-time curve (AUC(0-24h)) and the elimination half-life (t(1/2)) of finasteride. The geometric mean ratios (90% CI) were 0.42 (0.36-0.49), 0.66 (0.56-0.79) and 0.54 (0.48-0.61), respectively. Finasteride was excreted unchanged to a minor extent into bile and urine. Hydroxy-finasteride was not detected in plasma, bile or urine. Carboxy-finasteride was quantified in all three compartments and its plasma pharmacokinetics was significantly affected by SJW treatment. Hyperforin concentration in plasma was 21+/-7ng/ml approximately 12h after the last dose of the 14 days SJW treatment. In conclusion, SJW treatment for 2 weeks induced the metabolism of finasteride and caused a reduced plasma exposure of the drug. New knowledge was gained about the biliary and urinary excretion or the drug and its metabolites.

    • OBJECTIVE: Endothelial function, including the nitric oxide (NO)-pathway, has previously been extensively investigated in heart failure (HF). In contrast, studies are lacking on the NO pathway after heart transplantation (HT). We therefore investigated substances in the NO pathway prior to and after HT in relation to hemodynamic parameters.DESIGN: 12 patients (median age 50.0 yrs, 2 females), heart transplanted between June 2012 and February 2014, evaluated at our hemodynamic lab, at rest, prior to HT, as well as four weeks and six months after HT were included. All patients had normal left ventricular function post-operatively and none had post-operative pulmonary hypertension or acute cellular rejection requiring therapy at the evaluations. Plasma concentrations of ADMA, SDMA, L-Arginine, L-Ornithine and L-Citrulline were analyzed at each evaluation.RESULTS: In comparison to controls, the plasma L-Arginine concentration was low and ADMA high in HF patients, resulting in low L-Arginine/ADMA-ratio pre-HT. Already four weeks after HT L-Arginine was normalized whereas ADMA remained high. Consequently the L-Arginine/ADMA-ratio improved, but did not normalize. The biomarkers remained unchanged at the six-month evaluation and the L-Arginine/ADMA-ratio correlated inversely to pulmonary vascular resistance (PVR) six months post-HT.CONCLUSIONS: Plasma L-Arginine concentrations normalize after HT. However, as ADMA is unchanged, the L-Arginine/ADMA-ratio remained low and correlated inversely to PVR. Together these findings suggest that (i) the L-Arginine/ADMA-ratio may be an indicator of pulmonary vascular tone after HT, and that (ii) NO-dependent endothelial function is partly restored after HT. Considering the good postoperative outcome, the biomarker levels may be considered "normal" after HT.
