  • Validation of the EULAR definition of arthralgia suspicious for progression to rheumatoid arthritis

    • Objectives. Recently a EULAR-taskforce defined arthralgia suspicious for progression to RA, in order to allow inclusion of homogeneous sets of arthralgia patients in clinical studies. This longitudinal study aimed (i) to validate this definition in arthralgia patients in whom rheumatologists felt that imminent RA was more likely than other arthralgias [clinically suspect arthralgia (CSA)], that is, the target population fulfilling the entry criterion, and (ii) to explore the performance in arthralgia patients who were referred to secondary care prior to rheumatological evaluation, hence ignoring the entry criterion. Methods. The definition was assessed in 241 Dutch patients identified with CSA by rheumatologists and 113 patients referred to the Umea university hospital with recent-onset arthralgia in small joints. The external reference was arthritis development < 2 years' follow-up. Results. CSA patients with a positive definition (>= 3/7 parameters present) had an increased risk for developing arthritis compared with definition-negative CSA patients (hazard ratio = 2.1, 95% CI: 0.9, 4.7). The sensitivity was 84% and the positive predictive value 30%. In arthralgia patients in whom the definition was applied before rheumatological evaluation, a positive definition was neither sensitive (10%) nor predictive (positive predictive value 3%). Conclusion. The EULAR definition of arthralgia suspicious for progression to RA is sensitive when used to support the rheumatologist's opinion on imminent RA. This validation study shows that the definition, when used as designed, further homogenizes patients that rheumatologists consider at risk for RA. To arrive at a high specificity, the clinical definition needs to be combined with biomarkers.

    • The reactivity of alkylperoxyl radicals and -O3SOO* toward ozone was investigated. The peroxyl radicals were produced by steady-state gamma-radiolysis in the presence Of O-3. The rate constants were extracted from the decay rate of ozone measured during the irradiation. The rate constants vary between 7 x 10(3) and 2 x 10(5) M-1 s(-1) and there is a trend of increasing rate constant with electron-withdrawing substituent. Quantum chemical computations support a mechanism, according to which formation of an alkyl trioxide radical is the rate-determining step. This is followed by rapid expulsion Of O-2 to yield the alkoxyl radical. Conceivably, the alkyl trioxide radical is preceded by an extremely unstable alkyl pentoxide radical in equilibrium with the reactants.

    • In the planning of a new housing area for 100 dwellings, a pre-design has been made for a solar heating plant. The aim of designing a layout, is to compare the solar system with more conventional heating systems. Developers and contractors are invited for turn-key tenders of the different systems. The single family houses, apartments and service premises in two storey, will have a total floor area of 9000 m{sup 2}. The heating demand is estimated to 1100 MWh/year (120 kWh/m{sup 2}) and the power to 450 kW. A new concept system is designed with low temperatures in all essential parts, but heat pumps are not needed. (1) Flat plate solar collectors, mean temperature 60 deg C; (2) Seasonal bore hole heat store in rock, temperature level 30-45 deg C; (3) Heat distribution network, working temperature 20-80 deg C; (4) Floor heating coils, temperature 25-32 deg C; (5) Peak electrical heaters in houses; (6) Solar DHW and auxiliary individual electrical final heaters. The system has only one general heat fluid with a mixture of water and glycol flowing through solar collectors, store, culverts and the floor heating coils. In all operation modes the heat carrier has the same flow direction and even act as a`buffer volume`. The bedrock consist of outcrops of granite and the GWL is at a depth of 4 m below the ground. In a 120 m investigation bore hole, a so called`Response test` is made in situ of the rock and the duct system. The obtained thermal results are: Conductivity{lambda}= 4.1 W/m,K, Capacity C 0.6 kWh/K, m{sup 3}, Resistance total of PEM-tubes and rock mass R= 0.02 K/(W/m). The investment cost have been calculated to 5.4 mil SEK ({approx} 0.7 mil USD) excl. culvert, floor heating system, DHW tanks/heaters. The annual capital and running costs are 0.73 mil SEK (0.1 mil USD), calculated with an interest rate of 6% over 25 years (0.078). The total system heating cost will be 0.68 SEK/kWh (0.1 USD/kWh). With received EU- and -governmental subsides up to 2.0 mil SEK the heating costs drop to 0.54 SEK/kWh (0.07 USD/kWh). Solar energy is by that means cost-effective to conventional alternatives as district heating, bio-fuel block centrals, ground heat pumps or 100% electrical heating. The solar heating project seems in all respects possible to carry through - but the final decision is taken of the market response

  • Synthesis and Mechanistic Studies of Organic Chromophores with Different Energy Levels for p-Type Dye-Sensitized Solar Cells

    • A series of donor-pi-acceptor dyes with different electron-withdrawing groups were designed and synthesized for p-type dye-sensitized Solar cells. The modification of dye structures shows significant influence on the photophysical, electrochemical, and photovoltaic performance of the dyes. DSSCs based on these dyes show maximum 63% and minimum 6% of incident monochromatic photon-to-current conversion efficiencies. The two dyes with the highest (P1) and lowest (P3) efficiencies were Studied by femtosecond transient absorption spectroscopy, which shows a fast injection rate of more than (250 fs)(-1) for both dyes. Such fast injection corresponds to more than 90% injection efficiency. The photoinduced absorption Spectroscopy Study of sensitized NiO films in the presence of electrolyte showed poor regeneration of 113 due to all insufficient driving force. This, together with aggregation of the dye on the NiO film, explained the poor solar cell performance.

  • Scandcleft randomized trials of primary surgery for unilateral cleft lip and palate: dental anomalies in 8-year olds

    • Background: Children born with unilateral cleft lip and palate (UCLP) are reported to display several dental anomalies including agenesis, supernumeraries, as well as variations in dental size, shape, and path of eruption. The extensive sample of individuals with UCLP included in the Scandcleft randomized control trials offers the opportunity to study more rare conditions, which is seldom possible with limited samples. Objectives: The aim was to study dental anomalies at 8 years of age in children born with UCLP included in the Scandcleft randomized control trials. Methods: Panoramic and intraoral radiographs from 425 individuals (279 males and 146 females) with a mean age of 8.1 years were assessed by four orthodontists regarding dental anomalies. Results: Agenesis was found in 52.5 per cent and supernumerary teeth in 16.9 per cent of the participants. The cleft lateral was missing in 43.8 per cent and was found peg shaped in 44.7 per cent.The distribution of ectopic eruption was 14.6 per cent, mainly affecting maxillary first molars, while transposition was found in 3.4 per cent of the individuals. In addition, infraocclusion of one or several primary molars was registered in 7.2 per cent of the participants. Conclusion: We conclude that 8-year-old children born with UCLP display multiple dental anomalies. The Scandcleft sample allowed rarely studied conditions such as infraocclusion of primary molars and transposition to be studied in children born with UCLP.
