  • Workplace intervention for improved risk perception and preventive activity among workers : using hand-held vibrating machines: a pilot study

    • This article presents a pilot study of a 1-h workplace educational intervention implemented among ten construction workers who were highly exposed to hand–arm vibration. The intervention combined risk communication and normative expert advice intended to reinforce preventive behaviour related to vibration, noise, and biomechanical loads. Data for this study comprised intervention notes and interview data from interventionists, and pre- and post-intervention interview data from participating workers.The results suggested that the intervention was sensitive to disturbances and should be directed only to motivated workers possessing sufficient self-efficacy, and only in circumstances in which exposure may be controlled on the local workplace level and by locally implemented measures. Unless these conditions are present, the studied intervention may fail to influence preventive behaviour, and may instead cause cognitive dissonance and frustration among participants and interventionists.

    • The Baltic states, Estonia, Latvia and Lithuania, were occupied no less than three times during the Second World War. Faced with a reign of terror, the threat of deportation, and compulsory conscription, over 30,000 people fled over the Baltic Sea to Sweden. On their arrival in Sweden, most escape boats were confiscated by the authorities and in 1945 were returned to the Soviet Union.This study, which began as an attempt to find out about the Baltic escape boats that ended up in Sweden, is inspired by a foreign boat in a small fishing harbour on the island of Gotland. My curiosity was piqued when I caught sight of the boat and heard that it had probably been an escape boat.The purpose of this thesis is to establish which, if any, Baltic escape boats survive in Sweden, in which contexts they remain, and to review their state of preservation, as well as to answer the question of their significance for the memory of the escape. It is an archaeology of the escape and its aftermath, based on the surviving escape boats – what the boats say about the escape, what happened to them afterwards, and how people relate to them and their historical legacy today.The study looks at 35 boats that have been described as Baltic escape boats in various contexts. It shows that they are found in a multitude of environments and in a range of different states of preservation. The boats illustrate experiences that few other source materials can convey, in a manner pertinent to the archaeological understanding of flight. In addition, the boats are rare traces of an earlier Estonian coastal culture eradicated by the second Soviet occupation.One of the most important outcomes of the study is the documentation from the survey and examinations carried out. Few of the boats were known outside their local context. A national compilation has been lacking, which has impeded research and ultimately our understanding of the breadth and complexity of the Swedish historical landscape when it comes to ship remains.

    • Manual work involving vibrating power tools is associated with symptoms that include vascular, neurological and musculoskeletal disorders. This study examines the vibration exposure of welders to determine the change between 1987 and 2008. Vibration measurements on handheld tools were used to evaluate the acceleration and the daily exposure time was determined by subjective rating. From these data, the 8-hour equivalent vibration exposure A(8), has been calculated. During the period, the A(8) decreased from 3.9 m/s2 to 1.9 m/s2. It was concluded that this decrease is the result of fewer vibrating tools and a decrease in daily exposure time. Although the daily vibration exposure has decreased over the study time, for some welders the daily vibration exposure A(8) is still above the action value set by the EU directive on vibration. This means more effort should be spent to decrease vibration exposure.

    • Objective This study focuses on the acute effects of vibration and how vibrations influence the measures of the thermal perception thresholds during different vibration magnitudes, frequencies, and durations.Methods The fingers of ten healthy subjects, five males and five females, were exposed to vibration under 16 conditions with a combination of different frequency, intensity and exposure time. The vibration frequency was 31.5 and 125 Hz and exposure lasted between 2 and 16 min. The energy-equivalent frequency weighted acceleration, according to ISO 5349-1, for the experimental time of 16 min was 2.5 or 5.0 m/s(2) (r.m.s.), corresponding to a 8-h equivalent acceleration, A(8) of 0.46 and 0.92 m/s(2), respectively. A measure of the thermal perception of cold and warmth was conducted before the different exposures to vibration. Immediately after the vibration exposure the acute effect was measured continuously on the exposed index finger for the first 75 s, followed by 30 s of measures at every minute for a maximum of 10 min. If the subject's thermal thresholds had not recovered, the measures continued for a maximum of 30 min with measurements taken every 5 min.Results For all experimental conditions and 30 s after exposure, the mean changes of the thresholds compared with the pre-test were found to be 0.05 and -0.67C for the warmth and cold thresholds, respectively. The effect of the vibration exposure was only significant on the cold threshold and only for the first minute after exposure when the threshold was decreased. The warmth threshold was not significantly affected at all. The frequency and the exposure time of the vibration stimuli had no significant influence on the perception thresholds for the sensation of cold or warmth. Increased equivalent frequency weighted acceleration resulted in a significant decrease of the subjects' cold threshold, not the warmth. The thresholds were unaffected when changes in the vibration magnitude were expressed as the frequency weighted acceleration or the unweighted acceleration.Conclusion When testing for the thermotactile thresholds, exposure to vibration on the day of a test might influence the results. Until further knowledge is obtained the previous praxis of 2 h avoidance of vibration exposure before assessment is recommended.

    • BACKGROUND: Overcrowding in the emergency department (ED) may negatively affect patient outcomes, so different triage models have been introduced to improve performance. Physician-led team triage obtains better results than other triage models. We compared efficiency and quality measures before and after reorganization of the triage model in the ED at our county hospital.MATERIALS AND METHODS: We retrospectively compared two study periods with different triage models: nurse triage in 2008 (baseline) and physician-led team triage in 2012 (follow-up). Physician-led team triage was in use during day-time and early evenings on weekdays. Data were collected from electronic medical charts and the National Mortality Register.RESULTS: We included 20,073 attendances in 2008 and 23,765 in 2012. The time from registration to physician presentation decreased from 80 to 33 min (P < 0.001), and the length of stay decreased from 219 to 185 min (P < 0.001) from 2008 to 2012, respectively. All of the quality variables differed significantly between the two periods, with better results in 2012. The odds ratio for patients who left before being seen or before treatment was completed was 0.62 (95% confidence interval 0.54-0.72). The corresponding result for unscheduled returns was 0.36 (0.32-0.40), and for the mortality rates within 7 and 30 days 0.72 (0.59-0.88) and 0.84 (0.73-0.97), respectively. The admission rate was 37% at baseline and 32% at follow-up (P < 0.001).CONCLUSION: Physician-led team triage improved the efficiency and quality in EDs.

    • AIMS: To establish if intermittent exposure to hand-transmitted vibration had the same effect as continuous exposure on the temporary response of finger tactile and thermal perception thresholds. METHODS: Two laboratory experiments were conducted. In each, 10 healthy subjects, five males and five females, participated. The subjects' fingers were exposed to vibration under four conditions with a combination of different periods of exposure and rest periods. The vibration frequency was 125 Hz and the frequency-weighted acceleration was 5 m/s(2). A measure of the tactile or thermal perception was conducted before the different exposures to vibration. Immediately after the vibration exposure, the acute effect was measured continuously for the first 75 s. This was followed by regular measures for a maximum of 30 min. RESULTS: The results showed that combinations of vibration with different periods of exposure and rest periods significantly influenced vibrotactile perception, but not thermal perception. CONCLUSIONS: These findings suggest that intermittent exposure to hand-transmitted vibration might be more beneficial for the response of the finger vibrotactile sensation than continuous exposure. This finding is inconsistent with the evaluation methods in ISO 5349-1 for vibrotactile sensation, but accurate for thermal perception.

  • Patient Safety in the Emergency Department : Culture, Waiting, and Outcomes of Efficiency and Quality

    • The overall aim of this thesis was to investigate patient safety in the emergency department (ED) and to determine whether this varies according to patient safety culture, waiting, and outcomes of efficiency and quality variables.I: Patient safety culture was described in the EDs of two different hospitals before and after a quality improvement project. The questionnaire “Hospital Survey on Patient Safety Culture” was used to investigate the patient safety culture. The main finding was that the staff at both hospitals scored more positively in the dimension Team-work within hospital after implementing a new work model aimed at improving patient flow and patient safety in the ED. Otherwise, we found only modest improvements.II: Grounded theory was used to explore what happens in the ED from the staff perspective. Their main concern was reducing patients’ non-acceptable waiting time. Management of waiting was improved either by increasing the throughput of patient flow by structure pushing and by shuffling patients, or by changing the experience of waiting by calming patients and by feinting to cover up.III: Three Swedish EDs with different triage models were compared in terms of efficiency and quality. The median length of stay was 158 minutes for physician-led team triage compared with 243 and 197 minutes for nurse–emergency physician and nurse–junior physician triage, respectively. Quality indicators (i.e., patients leaving before treatment was completed, the rate of unscheduled return within 24 and 72 hours, and mortality rate within 7 and 30 days) improved under the physician-led team triage.IV: Efficiency and quality variables were compared from before (2008) to after (2012) a reorganization with a shift of triage model at a single ED. Time from registration to physician decreased by 47 minutes, and the length of stay decreased by 34 minutes. Several quality measures differed between the two years, in favour of 2012. Patients leaving before treatment was completed, unscheduled return within 24 and 72 hours, and mortality rate within 7 and 30 days all improved despite the reduced admission rate.In conclusion, the studies underscore the need to improve patient safety in the ED. It is important to the patient safety culture to reduce patient waiting because it dynamically affects both patients and staff. Physician-led team triage may be a suitable model for reducing patient waiting time and increasing patient safety.

    • BACKGROUND:The management of emergency departments (EDs) principally involves maintaining effective patient flow and care. Different triage models are used today to achieve these two goals. The aim of this study was to compare the performance of different triage models used in three Swedish EDs. Using efficiency and quality indicators, we compared the following triage models: physician-led team triage, nurse first/emergency physician second, and nurse first/junior physician second.METHODS: All data of patients arriving at the three EDs between 08:00- and 21:00 throughout 2008 were collected and merged into a database. The following efficiency indicators were measured: length of stay (LOS) including time to physician, time from physician to discharge, and 4-hour turnover rate. The following quality indicators were measured: rate of patients left before treatment was completed, unscheduled return within 24 and 72 hours, and mortality rate within 7 and 30 days.RESULTS: Data from 160,684 patients were analysed. The median length of stay was 158 minutes for physician-led team triage, compared with 243 and 197 minutes for nurse/emergency physician and nurse/junior physician triage, respectively (p < 0.001). The rate of patients left before treatment was completed was 3.1 % for physician-led team triage, 5.3 % for nurse/emergency physician, and 9.6 % for nurse/junior physician triage (p < 0.001). Further, the rates of unscheduled return within 24 hours were significantly lower for physician-led team triage, 1.0 %, compared with 2.1 %, and 2.5 % for nurse/emergency physician, and nurse/junior physician, respectively (p < 0.001). The mortality rate within 7 days was 0.8 % for physician-led team triage and 1.0 % for the two other triage models (p < 0.001).CONCLUSIONS: Physician-led team triage seemed advantageous, both expressed as efficiency and quality indicators, compared with the two other models.

  • Relationship between hand-arm vibration exposure and onset time for symptoms in a heavy engineering production workshop

    • OBJECTIVES: This study examined onset time for reported vascular and neurological symptoms in relation to the vibration load in a group of workers exposed to vibration. METHODS: Information on the self-stated year for the first occurrence of symptoms was collected by means of questionnaires. During interviews data were obtained on self-stated estimations of daily exposure time, type of tool, and number of months or years with different exposures. The estimations of the vibration magnitudes of exposure were based on conducted measurements. From these data, the individual vibration exposure at the time of onset of symptoms was calculated. RESULTS: The incidence was 25.6 and 32.9 per 1000 exposure years for vascular and neurological symptoms, respectively, in the group of workers. The first onset of symptoms appeared after an average of 12 years of exposure. For the workers, the symptoms of vascular or neurological disorders started after about the same number of exposure years. The calculated accumulated acceleration correlated best with the onset time of symptoms. CONCLUSIONS: It was concluded that, since the workers' exposure to vibration was below the action level established in the European vibration directive, the results suggest that the action level is not a safe level for avoiding vascular and neurological symptoms.

    • Background: Patient safety is of the utmost importance in health care. The patient safety culture in an institution has great impact on patient safety. To enhance patient safety and to design strategies to reduce medical injuries, there is a current focus on measuring the patient safety culture. The aim of the present study was to describe the patient safety culture in an ED at two different hospitals before and after a Quality improvement (QI) project that was aimed to enhance patient safety. Methods: A repeated cross-sectional design, using the Hospital Survey On Patient Safety Culture questionnaire before and after a quality improvement project in two emergency departments at a county hospital and a university hospital. The questionnaire was developed to obtain a better understanding of the patient safety culture of an entire hospital or of specific departments. The Swedish version has 51 questions and 15 dimensions. Results: At the county hospital, a difference between baseline and follow-up was observed in three dimensions. For two of these dimensions, Team-work within hospital and Communication openness, a higher score was measured at the follow-up. At the university hospital, a higher score was measured at follow-up for the two dimensions Team-work across hospital units and Team-work within hospital. Conclusion: The result showed changes in the self-estimated patient safety culture, mainly regarding team-work and communication openness. Most of the improvements at follow-up were seen by physicians, and mainly at the county hospital.

    • This study clarifies whether the established frequency weighting procedure for evaluating exposure to hand-transmitted vibration can effectively evaluate the temporary changes in vibrotactile perception thresholds due to pre-exposure to vibration. In addition, this study investigates the relationship between changes of the vibrotactile perception thresholds and the normalized energy-equivalent frequency-weighted acceleration. The fingers of 10 healthy subjects, five male and five female, were exposed to vibration under 16 conditions with a combination of different frequencies, intensities, and exposure times. The vibration frequencies were 31.5 and 125 Hz and exposure lasted between 2 and 16 min. According to International Organization for Standardization (ISO) 5349-1, the energy-equivalent frequency-weighted acceleration for the experimental time of 16 min is 2.5 or 5.0 m s(-2) root-mean-square, corresponding to a 8-h equivalent acceleration, A(8), of approximately 0.5 and 0.9 m s(-2), respectively. A measure of the vibrotactile perception thresholds was conducted before the different exposures to vibration. Immediately after the vibration exposure, the acute effect was measured continuously on the exposed index finger for the first 75 s, followed by 30 s of measures every minute for a maximum of 10 min. If the subject's thresholds had not recovered, the measures continued for a maximum of 30 min with measurements taken every 5 min. Pre-exposure to vibration significantly influenced vibrotactile thresholds. This study concludes that the influence on the thresholds depends on the frequency of the vibration stimuli. Increased equivalent frequency-weighted acceleration resulted in a significant change in threshold, but the thresholds were unaffected when changes in the vibration magnitude were expressed as the frequency-weighted acceleration or the unweighted acceleration. Moreover, the frequency of the pre-vibration exposure significantly influenced (up to 25 min after exposure) recovery time of the vibrotactile thresholds. This study shows that the frequency weighting procedure in ISO 5349-1 is unable to predict the produced acute changes in the vibrotactile perception. Moreover, the results imply that the calculation of the 'energy-equivalent' frequency-weighted acceleration does not reflect the acute changes of the vibration perception thresholds due to pre-exposure to vibration. Furthermore, when testing for the vibrotactile thresholds, exposure to vibration on the day of a test might influence the results. Until further knowledge is obtained, the previous practice of 3 h avoidance of vibration exposure before assessment is recommended.

Arkeologins intressen har vidgats från att initialt riktats mot förhistorien till att numera inkludera såväl samtiden som framtiden. På grund av tingens beständighet över tid kan de vara brukade och betydelsebärande under fler epoker än under sin tillkomsttid vilket komplicerar frågan om deras datering.

    • For centuries ships were ballasted with sand, gravel, stone, or rubble to give them stability, and when they no longer needed the extra weight, it was dumped. The result was that huge quantities of ballast were shipped to new places and new continents.In Ballast the archaeologist Mats Burström charts how ship ballast helped to shape the world we live in. Ballast was often reused, sometimes in surprising ways. With the ballast went animal and plant life of all sorts, inadvertently spread to places where they are now so well established that they are thought of as native species. And it was not unknown for ancient artefacts to be found in the ballast too, turning up in the most unexpected places.This is the first comprehensive account of ship ballast, so long overlooked, and now finally recognized for its diverse and exciting history.

    • Barlast är den tyngd som håller fartygen stabila i sjön. Under lång tid användes sand, grus, sten och rivningsmassor för syftet. När den extra vikten inte längre behövdes dumpades den och på så sätt förflyttades enorma mängder material till nya platser och nya kontinenter. Där kom massorna ofta till användning på andra och ibland överraskande sätt.Arkeologen Mats Burström visar i ord och bild hur forna tiders barlast bidragit till att forma den värld vi lever i. Både djur och växter följde oavsiktligt med fartygen och spreds till nya områden. I dag är många av dessa arter så väletablerade att de betraktas som inhemska. Även fornsaker råkade ibland finnas med i barlasten och har därför dykt upp på helt oväntade platser.Detta är den första samlade framställningen om barlast och de spår den lämnat genom århundraden av omfattande sjöfart. Denna förbisedda extralast visar sig rymma massor av tankeväckande historia.

    • During the Second World War, many Estonians buried family possessions before fleeing overseas. Yet their hopes of returning soon to recover them were dashed by the postwar Soviet occupation. During the long years of exile, these possessions were transformed from everyday objects into a kind of repository for memories. One way for exiles to remember their homeland and sustain their dreams of return was to tell stories. Some managed to retrieve their belongings while in exile, others went back to find them after the fall of the Soviet Union. This chapter examines a selection of stories about underground wartime hoards in order to show how family traditions lend a human dimension to grand narratives of the recent past.

    • Ruins, understood as a cultural state in which material objects find themselves, affect people. This applies in particular to ruins of the late modern era. In contrast to how we normally perceive remains from antiquity, many of us do not seem fully mentally prepared to meet the contemporary past in state of ruin. Realizing that even the present and the familiar now are objects of archaeological concern makes you reflect upon the passing of time and your own place in history. Suddenly, the fundamental categories of past and present do not seem relevant. This confusion may indeed be creative and points towards an archaeology beyond modernity, an archaeology that recognizes no chronological borders.

    • From 1933 to 1937 the German National Socialist (Nazi) Party arranged an annual harvest festival at Bückeberg, close to the city of Hamelin. The festival was one of the symbolically most important celebrations in the Third Reich; at its height, more than one million people are reported to have gathered there. A special arena, designed by Albert Speer, was built to handle the large number of participants. Although extensive remains of this arena have survived, local feeling has prevented them from receiving official recognition as a historical monument. This paper presents the Bückeberg site and discusses the responsibilities of heritage professionals towards sites which may have significance as testimony to the past but which are not actively championed by the public.

    • Half a century ago – in October 1962 – the world was on the threshold to the unthinkable: a full-scale nuclear war between the USA and the Soviet Union. The focus of the conflict was a number of Soviet launching sites for medium-range missiles situated in the Cuban countryside. Today, at first glance it is difficult to discover traces at these sites that testify to their important role in twentieth-century history. A closer look, however, reveals not only material remains but also an extensive, peaceful reuse of elements from the former military installations. This photo-essay presents some of the material evidence found during archaeological fieldwork at the missile site at Santa Cruz de los Pinos, also known as San Cristobal 3, situated c. 100 kilometres west of Havana.
