    • A key pathophysiological process and therapeutic target in the critical early post-injury period of traumatic brain injury (TBI) is cell mitochondrial dysfunction; characterised by elevation of brain lactate/pyruvate (L/P) ratio in the absence of hypoxia. We previously showed that succinate can improve brain extracellular chemistry in acute TBI, but it was not clear if this translates to a change in downstream energy metabolism. We studied the effect of microdialysis-delivered succinate on brain energy state (phosphocreatine/ATP ratio (PCr/ATP)) with 31P MRS at 3T, and tissue NADH/NAD+ redox state using microdialysis (L/P ratio) in eight patients with acute major TBI (mean 7 days). Succinate perfusion was associated with increased extracellular pyruvate (+26%, p < 0.0001) and decreased L/P ratio (−13%, p < 0.0001) in patients overall (baseline-vs-supplementation over time), but no clear-cut change in 31P MRS PCr/ATP existed in our cohort (p > 0.4, supplemented-voxel-vs-contralateral voxel). However, the percentage decrease in L/P ratio for each patient following succinate perfusion correlated significantly with their percentage increase in PCr/ATP ratio (Spearman's rank correlation, r = −0.86, p = 0.024). Our findings support the interpretation that L/P ratio is linked to brain energy state, and that succinate may support brain energy metabolism in select TBI patients suffering from mitochondrial dysfunction.

  • Effects of chemotherapy and hormonal therapy for early breast cancer on recurrence and 15-year survival: an overview of the randomised trials

    • Background Quinquennial overviews (1985-2000) of the randomised trials in early breast cancer have assessed the 5-year and 10-year effects of various systemic adjuvant therapies on breast cancer recurrence and survival. Here, we report the 10-year and 15-year effects. Methods Collaborative meta-analyses were undertaken of 194 unconfounded randomised trials of adjuvant chemotherapy or hormonal therapy that began by 1995. Many trials involved CMF (cyclophosphamide, methotrexate, fluorouracil), anthracycline-based combinations such as FAC (fluorouracil, doxombicin, cyclophosphamide) or FEC (fluorouracil, epirubicin, cyclophosphamide), tamoxifen, or ovarian suppression: none involved taxanes, trastuzumab, raloxifene, or modem aromatase inhibitors. Findings Allocation to about 6 months of anthracycline-based polychemotherapy (eg, with FAC or FEC) reduces the annual breast cancer death rate by about 38% (SE 5) for women younger than 50 years of age when diagnosed and by about 20% (SE 4) for those of age 50-69 years when diagnosed, largely irrespective of the use of tamoxifen and of oestrogen receptor (ER) status, nodal status, or other tumour characteristics. Such regimens are significantly (2p=0 . 0001 for recurrence, 2p<0 . 00001 for breast cancer mortality) more effective than CMF chemotherapy. Few women of age 70 years or older entered these chemotherapy trials. For ER-positive disease only, allocation to about 5 years of adjuvant tamoxifen reduces the annual breast cancer death rate by 31% (SE 3), largely irrespective of the use of chemotherapy and of age (<50, 50-69, &GE; 70 years), progesterone receptor status, or other tumour characteristics. 5 years is significantly (2p<0 . 00001 for recurrence, 2p=0 . 01 for breast cancer mortality) more effective than just 1-2 years of tamoxifen. For ER-positive tumours, the annual breast cancer mortality rates are similar during years 0-4 and 5-14, as are the proportional reductions in them by 5 years of tamoxifen, so the cumulative reduction in mortality is more than twice as big at 15 years as at 5 years after diagnosis. These results combine six meta-analyses: anthracycline-based versus no chemotherapy (8000 women); CMF-based versus no chemotherapy (14 000); anthracycline-based versus CMF-based chemotherapy (14 000); about 5 years of tamoxifen versus none (15 000); about 1-2 years of tamoxifen versus none (33 000); and about 5 years versus 1-2 years of tamoxifen (18 000). Finally, allocation to ovarian ablation or suppression (8000 women) also significantly reduces breast cancer mortality, but appears to do so only in the absence of other systemic treatments. For middle-aged women with ER-positive disease (the commonest type of breast cancer), the breast cancer mortality rate throughout the next 15 years would be approximately halved by 6 months of anthracycline-based chemotherapy (with a combination such as FAC or FEC) followed by 5 years of adjuvant tamoxifen. For, if mortality reductions of 38% (age <50 years) and 20% (age 50-69 years) from such chemotherapy were followed by a further reduction of 31% from tamoxifen in the risks that remain, the final mortality reductions would be 57% and 45%, respectively (and, the trial results could well have been somewhat stronger if there had been full compliance with the allocated treatments). Overall survival would be comparably improved, since these treatments have relatively small effects on mortality from the aggregate of all other causes. Interpretation Some of the widely practicable adjuvant drug treatments that were being tested in the 1980s, which substantially reduced 5-year recurrence rates (but had somewhat less effect on 5-year mortality rates), also substantially reduce 15-year mortality rates. Further improvements in long-term survival could well be available from newer drugs, or better use of older drugs.

    • The decay-out process of the yrast superdeformed band in Cu-59 has been investigated. The firm determination of spin, parity, excitation energy, and configuration of the states involved in this process constitutes a unique situation for a detailed understanding of the decay-out mechanism. A theoretical model is introduced that includes a residual interaction and tunneling matrix element between bands, calculated in the configuration-dependent cranked Nilsson-Strutinsky model. This interaction causes the decay to occur via a small number of observed doorway states.

    • High-spin states in Cu-59 were populated using the fusion-evaporation reaction Si-28 + Ca-40 at a beam energy of 122 MeV. The Gammasphere Ge-detector array in conjunction with the 4pi charged-particle detector array Microball allowed for the detection of gamma-rays in coincidence with evaporated light particles. The resulting extensive high-spin decay scheme of Cu-59 is presented, which comprises more than 320 gamma-ray transitions connecting about 150 excited states. Their spins and parities have been assigned via directional correlations of gamma-rays emitted from oriented states. Average quadrupole moments of rotational bands have been determined from the analysis of residual Doppler shifts. Shell model calculations in the fp shell are invoked to study some of the low-spin states, while the experimental characteristics of the rotational bands are analyzed in the configuration-dependent cranked Nilsson-Strutinsky (CNS) approach.

    • We have studied via in-beam gamma-ray spectroscopy Po-196 and Po-198, which are the first neutron-deficient Po isotopes to exhibit a collective low-lying structure. The ratios of yrast state energies and the E2 branching ratios of transitions from non-yrast to yrast states are indicative of a low-lying vibrational structure. The onset of collective motion in these isotopes can be attributed to the opening of the neutron i(13/2) orbital at N approximate to 112 and the resulting large overlap between the two valence protons in the h(9/2) orbital and the valence neutrons in the i(13/2) orbital.

    • Two new features of superdeformed (SD) bands in the A almost-equal-to 190 region emerge from a study of Hg-194 with the Gammasphere detector array. A decrease of the dynamic moment of inertia is observed for rotational frequencies HBARomega greater-than-or-equal-to 0.4 MeV, confirming long standing expectations based on mean field calculations with pairing. Evidence for a small staggering in the SD transition energies is also observed, suggesting the presence of terms with fourfold symmetry in the SD Hamiltonian.

    • The transition quadrupole moments Q(t) for the superdeformed band in Ca-40 have been determined through thin-target Doppler-shift attenuation analyses. A best-fit value of Q(t)=1.30+/-0.05 e b is obtained when a single value is assumed for the entire band. Fitting separate quadrupole moments for in-band transitions decaying from the high-spin states and the presumably admixed low-spin states results in Q(t)(high)= 1.81(-0.26)(+0.41) e b and Q(t)(low)=1.18(-0.05)(+0.06) e b, respectively. Q(t) values extracted for individual transitions in a Doppler-broadened line-shape analysis also indicate smaller Q(t) values at lower spins. These results are consistent with the interpretation of this band as an eight-particle-eight-hole superdeformed band with a significant admixture of less-collective configurations at low spins.

    • The fusion-evaporation reactions Si-28(S-32,1 alpha 2p1n)Fe-53 at 125 MeV and Mg-24(S-32,2p1n)Fe-53 at a 95-MeV beam energy were used to investigate excited states in Fe-53. The combination of the Gammasphere Ge detector array and ancillary devices led to the construction of an extensive level scheme comprising some 90 transitions connecting 40 states. The lifetime of the yrast 25/2(-) state and upper limits for the lifetimes of a number of additional states were determined using the Cologne plunger device coupled to the GASP gamma-ray spectrometer. The experimental results are compared to large-scale shell-model calculations using different sets of two-body matrix elements. In particular, predictions on electromagnetic decay properties such as lifetimes, branching ratios, and mixing ratios are studied in detail.

Spins and tentative positive parities of the band members are assigned based on angular distribution and correlation measurements of transitions, which connect the rotational structure with previously known yrast states. Configuration-dependent cranked Nilsson-Strutinsky calculations suggest a configuration of the band, which comprises one particle in the 1g(9/2) intruder orbit.

    • The level scheme of Mn-51 has been investigated using the Si-28(S-32 2alpha1p)Mn-51 fusion-evaporation reaction at 125 MeV beam energy. The gamma rays were detected in the Ge-detector array Gammasphere, which was coupled to the 4pi-charged-particle detector Microball and the Neutron Shell for the coincident detection of evaporated particles. Over 100 new gamma-ray transitions connecting some 50 excited states have been identified. Most of the states are built on particle-hole excitations across the shell gaps at particle number N=Z=28. The rich experimental data allows for a 'comprehensive comparison of core-excited states with results from large-scale shell-model calculations, which are known to provide excellent predictions for the lower-lying yrast and near-yrast levels in 1f(7/2) nuclei.

    • Tourette syndrome and chronic tic disorder are common, disabling childhood-onset conditions. Guidelines recommend that behavioural therapy should be offered as first-line treatment for children with tics. However, there are very few trained behaviour therapists for tics and many patients cannot access appropriate care. This trial investigates whether an internet-delivered intervention for tics can reduce severity of symptoms.Methods and analysisThis parallel-group, single-blind, randomised controlled superiority trial with an internal pilot will recruit children and young people (aged 9–17 years) with tic disorders. Participants will be randomised to receive 10 weeks of either online, remotely delivered, therapist-supported exposure response prevention behavioural therapy for tics, or online, remotely delivered, therapist-supported education about tics and co-occurring conditions. Participants will be followed up mid-treatment, and 3, 6, 12 and 18 months post randomisation.The primary outcome is reduction in tic severity as measured on the Yale Global Tic Severity Scale total tic severity score. Secondary outcomes include a cost-effectiveness analysis and estimate of the longer-term impact on patient outcomes and healthcare services. An integrated process evaluation will analyse quantitative and qualitative data in order to fully explore the implementation of the intervention and identify barriers and facilitators to implementation. The trial is funded by the National Institute of Health Research (NIHR), Health Technology Assessment (16/19/02).Ethics and disseminationThe findings from the study will inform clinicians, healthcare providers and policy makers about the clinical and cost-effectiveness of an internet delivered treatment for children and young people with tics. The results will be submitted for publication in peer-reviewed journals.     • Nuclear level lifetimes have been measured in two superdeformed bands in Hg-194 using the Doppler-shift attenuation method. Average transition quadrupole moments derived from the life-times of an excited and yrast superdeformed bands are Q(t) = 17.6(30) and 17.2(20) eb, respectively. The Doppler shifts of the excited band relative to the yrast band indicate a slight difference in quadrupole moments [+4(5)%], assuming similar side feeding. These results imply that the second well is stable and rigid with respect to the particle excitation responsible for this excited band.

To detect gamma-rays in coincidence with evaporated particles the Ge-detector array Gammasphere was used in conjunction with the charged-particle detector system, Microball and a 1 pi neutron detector array. Results include a significantly extended level scheme of Ni-56, which is compared to large-scale shell model calculations in the fp shell. The experimental and theoretical results agree to a large extent, with one notable exception; the theoretical model fails to predict the proper sequence of the yrast and yrare 8(+) states.

    • High-spin states have been studied in Kr-72 and Br-72 using the Ca-40 + Ca-40 and Ar-36 + Ca-40 reactions at 164 and 145 MeV, respectively. The properties and configurations of the high-spin bands observed have been interpreted using impaired cranked Nilssou-Strutinsky (CNS), and for Kr-72, paired cranked relativistic Hartree-Bogoliubov (CRHB) calculations. In Kr-72 a new band has been identified that has the properties expected for the doubly aligned S-band configuration. In Br-72 the previously known bands have been extended to higher spin. This has lead to a re-interpretation of the configurations.

    • Level schemes of Cd-99(48)51 and In-101(49)52 nuclei have been extended to high spin. The breakup of the doubly magic Sn-100 core has been observed. Large-scale shell model calculations based on realistic nucleon-nucleon interactions are in good agreement with the experimental data. These results provide a reliable basis to predict nuclear structure properties in Sn-100 and its neighbors. For example, the size of the N=50 shell gap and the energy of the first excited state in Sn-101 have been deduced.

    • The neutron-deficient nucleus La-123 was studied via the Mo-92(Ca-40,2ap) reaction at a beam energy of 184 MeV. Previously known bands were extended to a much higher spin, and in two cases the structures are now observed near 40 (h) over bar. In addition, three new sequences were identified and linked into previously known bands. The lowest (pi,alpha)=(+,-1/2) structure displays characteristics similar to those of analogous bands in La-127,La-129, which have been proposed as examples of smooth band termination. Cranked Nilsson-Strutinsky calculations were compared with the experimental data in La-123 to determine whether this band is approaching a terminating state as well.

    • Three rotational bands in Kr-74 were studied up to (in one case one transition short of) the maximum spin I-max of their respective single-particle configurations. Their lifetimes have been determined using the Doppler-shift attenuation method. The deduced transition quadrupole moments reveal a modest decrease, but far from a complete loss of collectivity at the maximum spin I-max. This feature, together with the results of mean field calculations, indicates that the observed bands do not terminate at I=I-max.

    • High-spin states in Kr-76(36)40 have been populated in the Ca-40(Ca-40,4p)Kr-76 fusion-evaporation reaction at abeam energy of 165 MeV and studied using the Gammasphere and Microball multidetector arrays. The ground-state band and two signature-split negative parity bands of Kr-76 have been extended to similar to 30 (h) over bar. Lifetime measurements. using the Doppler-shift attenuation method show that the transition quadrupole moment of these three bands decrease as they approach their maximum-spin states. Two signatures of a new rotational structure with remarkably rigid rotational behavior have been identified. The high-spin properties of these rotational bands are analyzed within the framework of configuration-dependent cranked Nilsson-Strutinsky calculations.
