  • The three ZNT8 autoantibody variants together improve the diagnostic sensitivity of childhood and adolescent type 1 diabetes

    • Aims: We tested whether autoantibodies to all three ZnT8RWQ variants, GAD65, insulinoma-associated protein 2 (IA-2), insulin and autoantibodies to islet cell cytoplasm (ICA) in combination with human leukocyte antigen (HLA) would improve the diagnostic sensitivity of childhood type 1 diabetes by detecting the children who otherwise would have been autoantibody-negative.Methods: A total of 686 patients diagnosed in 1996–2005 in Skåne were analyzed for all the seven autoantibodies [arginin 325 zinc transporter 8 autoantibody (ZnT8RA), tryptophan 325 zinc transporter 8 autoantibody (ZnT8WA), glutamine 325 Zinc transporter 8 autoantibody (ZnT8QA), autoantibodies to glutamic acid decarboxylase (GADA), Autoantibodies to islet-antigen-2 (IA-2A), insulin autoantibodies (IAA) and ICA] in addition to HLA-DQ genotypes.Results: Zinc transporter 8 autoantibody to either one or all three amino acid variants at position 325 (ZnT8RWQA) was found in 65% (449/686) of the patients. The frequency was independent of age at diagnosis. The ZnT8RWQA reduced the frequency of autoantibody-negative patients from 7.5 to 5.4%—a reduction by 28%. Only 2 of 108 (2%) patients who are below 5 years of age had no autoantibody at diagnosis. Diagnosis without any islet autoantibody increased with increasing age at onset. DQA1-B1*X-0604 was associated with both ZnT8RA (p = 0.002) and ZnT8WA (p = 0.01) but not with ZnT8QA (p = 0.07). Kappa agreement analysis showed moderate (>0.40) to fair (>0.20) agreement between pairs of autoantibodies for all combinations of GADA, IA-2A, ZnT8RWQA and ICA but only slight ( < 0.19) agreement for any combination with IAA.Conclusions: This study revealed that (1) the ZnT8RWQA was common, independent of age; (2) multiple autoantibodies were common among the young; (3) DQA1-B1*X-0604 increased the risk for ZnT8RA and ZnT8WA; (4) agreement between autoantibody pairs was common for all combinations except IAA. These results suggest that ZnT8RWQA is a necessary complement to the classification and prediction of childhood type 1 diabetes as well as to randomize the subjects in the prevention and intervention of clinical trials.

    • Background/Objectives:Alcohol and dietary fat have high energy densities and may therefore be related to body weight and fat deposition. We studied associations between alcohol and macronutrient intake patterns and general and central adiposity.Subjects/Methods:A population-based cross-sectional study of 524 men and 611 women. The participants answered a dietary questionnaire describing habitual food consumption including intake of alcoholic beverages. Macronutrient intake was analysed in relation to anthropometric measures and dual energy X-ray absorptiometry determined body fat.Results:In women, total alcohol intake was negatively associated with body fat percentage (β:-0.67, P<0.01). In men, total alcohol intake was positively associated with sagittal abdominal diameter (SAD) (β: 0.28, P=0.01). In addition, positive associations were found between intake of alcohol from spirits and body fat percentage (β: 1.17, P<0.05), SAD (β: 0.52, P<0.05) and waist circumference (β: 2.29, P=0.01). In men, protein intake was positively associated with body mass index (BMI) (β: 0.03, P=0.001), body fat percentage (β: 0.04, P<0.05), SAD (β: 0.02, P=0.01) and waist circumference (β: 0.09, P<0.01). Also in men only, negative associations between fat intake and BMI (β: -0.03, P<0.01), SAD (β: -0.02, P<0.05) and waist circumference (β: -0.05, P<0.05) were found.Conclusions:Alcohol intake was inversely associated to relative body fat in women whereas spirits consumption was positively related to central and general obesity in men. Macronutrient intakes, particularly protein and fat, were differently associated with obesity indicators in men versus women.     • The splice variant INS-IGF2 entails the preproinsulin signal peptide, the insulin B-chain, eight amino acids of the C-peptide and 138 unique amino acids from an ORF in the IGF2 gene. The aim of this study was to determine whether levels of specific INS-IGF2 autoantibodies (INS-IGF2A) were related to age at diagnosis, islet autoantibodies, HLA-DQ or both, in patients and controls with newly diagnosed type 1 diabetes. Patients (n = 676), 0-18 years of age, diagnosed with type 1 diabetes in 1996-2005 and controls (n = 363) were analysed for specific INS-IGF2A after displacement with both cold insulin and INS-IGF2 to correct for non-specific binding and identify double reactive sera. GADA, IA-2A, IAA, ICA, ZnT8RA, ZnT8WA, ZnT8QA and HLA-DQ genotypes were also determined. The median level of specific INS-IGF2A was higher in patients than in controls (P < 0.001). Irrespective of age at diagnosis, 19% (126/676) of the patients had INS-IGF2A when the cut-off was the 95th percentile of the controls (P < 0.001). The risk of INS-IGF2A was increased among HLA-DQ2/8 (OR = 1.509; 95th CI 1.011, 2.252; P = 0.045) but not in 2/2, 2/X, 8/8, 8/X or X/X (X is neither 2 nor 8) patients. The association with HLA-DQ2/8 suggests that this autoantigen may be presented on HLA-DQ trans-heterodimers, rather than cis-heterodimers. Autoantibodies reactive with both insulin and INS-IGF2A at diagnosis support the notion that INS-IGF2 autoimmunity contributes to type 1 diabetes.

The aim of this study was to examine the influence of parental history of obesity on the association between baseline serum leptin concentrations and subsequent 4-y weight changes. Changes in food intake were also considered in the analysis. Middle-aged, obese women with no obese parent (n = 25) or at least one obese parent (n = 24) were included in the analysis. At baseline, women with no parental history of obesity and women with a parental history of obesity did not differ in body mass index (in kg/m2: 41.2 and 40.2, respectively) or median leptin concentrations (40.8 and 38.8 microg/L, respectively). Four-year weight changes varied widely in both groups combined (from -30 to 24 kg). Stratified regression analysis, adjusted for age, weight, and height, revealed that high leptin concentrations predicted less weight gain (or more weight loss) in women with no obese parent (beta = -21.2, P = 0.0006) but played no significant role in predicting weight gain in women with at least one obese parent (beta = -3.8, P = 0.41). Adding changes in energy and fat intakes to the model reduced the association between leptin and weight change to nonsignificance in the women with no obese parent, indicating that the effect of leptin could be explained largely by dietary changes. In conclusion, serum leptin concentrations predict long-term weight change in obese women with no history of parental obesity, an association largely mediated by changes in food intake. Lauren.Lissner@medfak.gu.se OBJECTIVE: Leptin seems to be involved in the regulation of energy balance, although little is known about the epidemiology of leptin with respect to prediction of weight gain and incidence of obesity-related diseases. The dual aim of this study is to document characteristics of leptin after long-term storage, and to describe its relation to body weight, from birth to old age, in an ongoing prospective study. RESEARCH METHODS AND PROCEDURES: A population-based sample of Swedish women was first examined at the ages of 38 to 60 and re-examined 24 years later. This study used 1358 frozen serum samples that had been stored 29 years for analysis of leptin concentrations and their relation to body weight history. RESULTS: Leptin values obtained from stored samples showed the same correlation with relative weight as that seen in a contemporary sample with similar demographic characteristics. Lower self-reported birth weight was associated with higher leptin levels in adulthood (p = 0.01), controlling for age and adult BMI. Prospective analyses revealed that high leptin in 38 to 46-year-olds predicted subsequent long-term weight gain (p = 0.003), although no significant associations were seen in women initially aged 50 or older. DISCUSSION: It is feasible to use frozen serum for studying leptin in relation to obesity and related developments many years later. High leptin level was a risk factor for subsequent weight gain in 38- and 46-year-old women. Retrospective analyses involving birth weight suggest that leptin resistance in adulthood might have fetal origins. PMID: 10102251 [PubMed - indexed for MEDLINE]

    • Environmental factors, including viral infections, may explain an increasing and fluctuating incidence of childhood type 1 diabetes (T1D). Ljungan virus (LV) isolated from bank voles have been implicated, but it is unclear whether LV contributes to islet autoimmunity, progression to clinical onset, or both, of T1D. The aim was to test whether LV antibodies (LVAb) were related to HLA-DQ and islet autoantibodies in newly diagnosed T1D patients (n = 676) and controls (n = 309). Patients, 0-18 years of age, diagnosed with T1D in 1996-2005 were analyzed for LVAb, HLA-DQ genotypes, and all seven known islet autoantibodies (GADA, IA-2A, IAA, ICA, ZnT8RA, ZnT8WA, and ZnT8QA). LVAb at 75th percentile, defined as cut off, was 90 (range 6-3936) U/mL and 4th quartile LVAb were found in 25% (170/676) of which 64% were < 10 (n = 108, p < 0.0001), and 27% were < 5 (n = 45; p < 0.0001) years old. The 4th quartile LVAb in children < 10 years of age correlated to HLA DQ2/8, 8/8, and 8/X (p < 0.0001). Furthermore, in the group with 4th quartile LVAb, 55% were IAA positive (p = 0.01) and correlation was found between 4th quartile LVAb and IAA in children < 10 years of age (p = 0.035). It is concluded that 1) LVAb were common among the young T1D patients and LVAb levels were higher in the younger age groups; 2) 4th quartile LVAb correlated with IAA; and 3) there was a correlation between 4th quartile LVAb and HLA-DQ8, particularly in the young patients. The presence of LVAb supports the notion that prior exposure to LV may be associated with T1D.

    • BACKGROUND: Obesity is a risk factor for cancer. Intentional weight loss in the obese might protect against malignancy, but evidence is limited. To our knowledge, the Swedish Obese Subjects (SOS) study is the first intervention trial in the obese population to provide prospective, controlled cancer-incidence data. METHODS: The SOS study started in 1987 and involved 2010 obese patients (body-mass index [BMI] >or=34 kg/m(2) in men, and >or=38 kg/m(2) in women) who underwent bariatric surgery and 2037 contemporaneously matched obese controls, who received conventional treatment. While the main endpoint of SOS was overall mortality, the main outcome of this exploratory report was cancer incidence until Dec 31, 2005. Cancer follow-up rate was 99.9% and the median follow-up time was 10.9 years (range 0-18.1 years). FINDINGS: Bariatric surgery resulted in a sustained mean weight reduction of 19.9 kg (SD 15.6 kg) over 10 years, whereas the mean weight change in controls was a gain of 1.3 kg (SD 13.7 kg). The number of first-time cancers after inclusion was lower in the surgery group (n=117) than in the control group (n=169; HR 0.67, 95% CI 0.53-0.85, p=0.0009). The sex-treatment interaction p value was 0.054. In women, the number of first-time cancers after inclusion was lower in the surgery group (n=79) than in the control group (n=130; HR 0.58, 0.44-0.77; p=0.0001), whereas there was no effect of surgery in men (38 in the surgery group vs 39 in the control group; HR 0.97, 0.62-1.52; p=0.90). Similar results were obtained after exclusion of all cancer cases during the first 3 years of the intervention. INTERPRETATION: Bariatric surgery was associated with reduced cancer incidence in obese women but not in obese men.   • Effects of bariatric surgery on mortality in Swedish obese subjects.

    • BACKGROUND: Obesity is associated with increased mortality. Weight loss improves cardiovascular risk factors, but no prospective interventional studies have reported whether weight loss decreases overall mortality. In fact, many observational studies suggest that weight reduction is associated with increased mortality. METHODS: The prospective, controlled Swedish Obese Subjects study involved 4047 obese subjects. Of these subjects, 2010 underwent bariatric surgery (surgery group) and 2037 received conventional treatment (matched control group). We report on overall mortality during an average of 10.9 years of follow-up. At the time of the analysis (November 1, 2005), vital status was known for all but three subjects (follow-up rate, 99.9%). RESULTS: The average weight change in control subjects was less than +/-2% during the period of up to 15 years during which weights were recorded. Maximum weight losses in the surgical subgroups were observed after 1 to 2 years: gastric bypass, 32%; vertical-banded gastroplasty, 25%; and banding, 20%. After 10 years, the weight losses from baseline were stabilized at 25%, 16%, and 14%, respectively. There were 129 deaths in the control group and 101 deaths in the surgery group. The unadjusted overall hazard ratio was 0.76 in the surgery group (P=0.04), as compared with the control group, and the hazard ratio adjusted for sex, age, and risk factors was 0.71 (P=0.01). The most common causes of death were myocardial infarction (control group, 25 subjects; surgery group, 13 subjects) and cancer (control group, 47; surgery group, 29). CONCLUSIONS: Bariatric surgery for severe obesity is associated with long-term weight loss and decreased overall mortality.

    • We constructed a single-chain Fv antibody library that permits human complementarity-determining region (CDR) gene fragments of any germline to be incorporated combinatorially into the appropriate positions of the variable-region frameworks VH-DP47 and VL-DPL3. A library of 2 x 109 independent transformants was screened against haptens, peptides, carbohydrates, and proteins, and the selected antibody fragments exhibited dissociation constants in the subnanomolar range. The antibody genes in this library were built on a single master framework into which diverse CDRs were allowed to recombine. These CDRs were sampled from in vivo-processed gene sequences, thus potentially optimizing the levels of correctly folded and functional molecules, and resulting in a molecule exhibiting a lower computed immunogenicity compared to naive immunoglobulins. Using the modularized assembly process to incorporate foreign sequences into an immunoglobulin scaffold, it is possible to vary as many as six CDRs at the same time, creating genetic and funcfional variation in antibody molecules.

    • Chronically elevated serum levels of serum amyloid A (SAA) are linked to increased risk of cardiovascular disease. However, whether SAA is directly involved in atherosclerosis development is still not known. The aim of this study was to investigate the effects of adipose tissue-derived human SAA on atherosclerosis in mice. hSAA1(+/-) transgenic mice (hSAA1 mice) with a specific expression of human SAA1 in adipose tissue were bred with ApoE-deficient mice. The hSAA1 mice and their wild type (wt) littermates were fed normal chow for 35 weeks. At the end of the experiment, the mice were euthanized and blood, gonadal adipose tissue and aortas were collected. Plasma levels of SAA, cholesterol and triglycerides were measured. Atherosclerotic lesion areas were analyzed in the aortic arch, the thoracic aorta and the abdominal aorta in en face preparations of aorta stained with Sudan IV. The human SAA protein was present in plasma from hSAA1 mice but undetectable in wt mice. Similar plasma levels of cholesterol and triglycerides were observed in hSAA1 mice and their wt controls. There were no differences in atherosclerotic lesion areas in any sections of the aorta in hSAA1 mice compared to wt mice. In conclusion, our data suggest that adipose tissue-derived human SAA does not influence atherosclerosis development in mice.

    • There is a growing interest in the relation between workspace design and innovation. On the one hand, is the idea of designing an “innovation lab” that supports innovation. There are substantial financial investments involved when creating an innovation lab and there is evidence that such spaces can have short useful lifespan and some of them fail because they are not used as intended [Lewis and Moultrie 2005; Fayard and Weeks 2011]. On the other hand, workspaces can be altered by the users for short or long terms to support innovation activities. The users hence become spatial designers themselves. A gap exists in research on the underlying mechanisms, architecture, and dynamics by which organisations can create an environment supporting continuous improvements and radical innovation on both individual and organisational levels [Turner, Swart and Maylor 2013, Turner and Lee-Kelly 2013]. From design research we can contribute with a perspective on the underlying mechanisms and the dynamics in play in the area of workspace design and innovation. We can form the design research for the innovation labs, i.e. utopian specifically designed spaces for innovation, or the relationship between innovation, users and daily workspaces. We have chosen to acknowledge and study the complexity in relations between users, daily workspaces and innovation. Our hypothesis was that photo-elicitation could be a method to study that weave of complexity and research underlying dynamics.In this article we discuss the method of the photo-elicitated interview (PEI), as a tool in human-centred design research with respect to context and workspace. A phenomenological perspective focus on the human experience, examine and clarify situations, events and experiences as they occurs spontaneously in daily life (Seamon, 2000). This article intend to provide background theories from phenomenology and examples from an empirical study to discuss if and how PEI is instrumental in getting information from interviewees about their relation to their workspaces and innovation. Although the phenomenological theoretical perspective is relevant and therefore used here to describe human relation to workspaces and discuss the method, our use of specific notions from phenomenology aims firstly to support the analyse of the method to inform design research, and is not intended develop the phenomenological concepts themselves.

According to aggregated data describingprevalent users compliance to guidelines is fairly high. Objectives:The objective of this study was to investigatechoice and volume (DDD) of substance among incidentusers 75 years and older (75+) of hypnotics (N05C) in Swedenand compare between different county councils andwith incident users below 75 years of age. Methods:Data from the Swedish Prescribed Drug registercovering all dispensed prescriptions in Sweden was analyzedfor incident users of hypnotics (15–44, 45–64, 65–74 and 75+ years of age). Incident users were defined aspeople who filled a prescription (index date) of a hypnotic(N05C) during the period December 2009 - November2010 and had not filled any prescription for a hypnotic duringa wash-out period of 24 months before the index date. Results:Among incident patients 75+ (n= 38,620) zopiclone(52.9%), zolpidem (25.4%), and propiomazine(10.4%) were the most frequently used hypnotics. Therewere however differences between different county councilsin Sweden, with zopiclone ranging from 30 % to 68 %. Theincidence for recommended and appropriate hypnoticsincreased with increasing age, whereas the incidence ofinappropriate hypnotics did not. The prescribed volume ofinappropriate hypnotics was generally larger than the prescribedvolume of recommended hypnotics. Conclusions:The compliance to current guidelines onchoice of hypnotics for the elderly is fairly good, althoughthere is considerable variation between county councils.There are tendencies towards larger mean volumes of inappropriatehypnotics, although further studies are needed toconfirm these findings. Another aspect of quality in treatmentwith sleeping agents is to which extent incident userscontinue their treatment and for how long. This is an issuefor further research.

    • Background Polymorphisms in ABCB1 have the ability to affect both the function and the expression of the transporter protein P-glycoprotein and may lead to an altered response for many drugs including some antidepressants and antipsychotics.Objective The aim of this study was to examine the impact of the ABCB1 polymorphisms 1199Gandgt;A, 1236Candgt;T, 2677Gandgt;T/A, and 3435Candgt;T in deaths by suicide.Patients and methods A total of 998 consecutive Swedish forensic autopsies performed in 2008 in individuals 18 years of age or older, where femoral blood was available and a toxicological screening had been performed, were investigated. Genotypes were assessed with pyrosequencing and information on the cause and manner of each death was obtained from the forensic pathology and toxicology databases.Results There was a significantly higher frequency of the T allele at positions 1236, 2677, and 3435 among the suicide cases compared with the nonsuicide cases.Conclusion Our result from forensic cases suggests that ABCB1 polymorphisms are associated with an increased risk for completed suicides. The biological mechanisms involved and the clinical implications for these findings are largely unknown and need to be examined further.

For example found in Donald Norman’s theory of the design of everyday things. In these examples from design practice, usefulness seems to be put aside for sensual perception. But these desig nobjects also seem to broaden the concept of ’design’. Not, however, placing them in the categories of being aesthetic- or art objects. In this paper, I suggest that these artefacts are playing with ’affordance’, i.e. with the activity called forth through perception of the object.They also actualize design understood as drawing/sketching, as cognitive tools, hence playing with design both understood as process and final object.

    • Aesthetic aspects, along with a vitalisation of traditional discussions on aesthetic matters, can be a key to a better understanding of the design of working places in manufacturing industry of today. In this paper we address a rethinking of aesthetic matters and therefore also the aes-thetic in design practice, while examining examples of superabundant visual communication in industrial environments. The environments are studied from an information design perspec-tive. Theories discussed in the field of aesthetics are, for example, Jauss’ view on the con-cept ’poiesis’. Jauss’ line of thought are put to use, in this paper, as an alternative perspective focusing on the production of artefacts, compared to the more commonly perspective that highlights the concept ’aesthetic’ (Gr. ’aisthesis’) in a focusing on the perception of artefacts. In this paper the industrial environments are analysed as different fundamental forms of in-formation and Marcia J Bates’ notion of the nature of information is put to use in a visual- and spatial context. We argue that instead of underlining the relationship of aesthetic and architec-ture, it is more fruitful for the information design practice in manufacturing industrial envi-ronments, to highlight poiesis as a significant aspect of design. The analysis circle around three examples from industrial environments.

    • Art and the artistic are concepts Friedrich Nietzsche uses in a narrow sense regarding works of art and in a broader sense concerning our lives. He thereby attempts to vitalise the philosophical theme that links life and art. In this dissertation, I suggest that he distinguishes between life-negation and life-affirmation, concerning both the narrow and broader sense of art and the artistic. I firstly approach life-negation and life-affirmation in Nietzsche’s images of life. This part is arranged chronologically and its aim is to present a theme of life-negation and life-affirmation in Nietzsche’s writings and to acknowledge some changes that occur during his production. I consider the Apollonian, the Dionysian and the Socratic type of life and creativity and compare the metaphysical life-affirmation in The Birth of Tragedy with the will to life in Arthur Schopenhauer’s work. I also turn to later images related to life-negation and life-affirmation without the metaphysical perspective from his early thought, for example, the free and the fettered spirit. I moreover address the physiological will to power in Nietzsche’s later writings, in relation to the question of metaphysics and art. I secondly analyse Nietzsche’s notion of art and the artistic in a narrow and a broader sense in association with the theme of a two-fold type of life. This part is arranged thematically and I focus on Nietzsche’s later thought. His emphasis on the perspective of production is demarcated from the aesthetic paradigm that emphasises the reception of art. I then consider Nietzsche’s writings on works of art and the broader sense of art in association with life-negation and life-affirmation. Finally, I compare Nietzsche’s life-affirming type of existence with some contemporary views on the aestheticization of our lives. I suggest that the life-affirming artistic type of being differs in some significant senses from the proposed contemporary aestheticization processes.

    • Mälardalen Univerity is characterised by its close collaboration with companies and with the public sector in the region. A main strategic directions of the university is to develop co-production with partner companies, expressed in the university vision “A Strong MDH – the Coproducing University”. The concept ’co-production’ is in this paper used interchangeably with concept ’co-creation’, emphasising our view of equal participation and interaction with the goal of knowledge, that by the company can be made useful outside the university (see Terblanche, 2014). Based on Mälardalen University’s vision, co-production is a main goal for the Innovation and Product Realization (IPR) environment. IPR is located at the School of Innovation, Design and Engineering and has a common graduate education with three mutually supportive cooperating research groups: Product realization, Innovation management, and Information design. Here ideas from different fields and cultures meet, creating new ideas, possibilities, and knowledge as a result. The approach to develop new insights and knowledge in order to address societal challenges, through working closer between academics and research users, has a potential to improve how research is conducted. Still, there are many questions and challenges in this approach, with need of development: How is research and research education framed and undertaken? What constitutes co-production? What distinguishes reseach in co-production from other forms of research? What are the benefits and barriers of co-production? As we shall see, the follow-up research aim to contribute to how our third level education in a co-production environment can be undertaken.

    • In this paper I address two types of perspectives on the aesthetic that are of relevance for a discussion of contemporary information design. Firstly, the ’aesthetic’ understood as aesthetic perception of beautiful form. Secondly, the ’aesthetic’ recognized as the poietic activity, i.e. the production of an object, in which the recipient also takes part. These perspectives are discussed, in brief, in relation to questions of form and content, and I argue that aesthetic elements in some contemporary information design cannot be understood as aesthetic perception of beautiful form. The aesthetic is still present, but as poietic, creative, elements. Finally, I discuss the poietic activity in the light of McLaughlin’s Heideggerian view, that the passing on of information is never neutral.

    • In information design textbooks, text design is mostly understood as typography and layout. The meaning-making process of language, involving social interaction that affects language, is rarely acknowledged. Instead, texts are supposed to be "clearly" written. In this research article, we argue that the understanding of text design could benefit from also addressing text production and use situated amid social activity. This article presents a study on a text design process partly based on spoken language and owned by assembly operators in a workplace. Capturing the spoken dialogue and transforming it into instructive texts resembling transcripts are essential steps in securing the best practices for the smallest tasks in manual assembly, the minima of working, which is crucial for manufacturing. Our aim within the information design field is 2-fold: To underline the meaning-making process in language as a social phenomenon and to show that the situated design perspective, i.e., an outlook that highlights the uniqueness of the setting, can be important for the production and use of certain texts, such as instructions, and for affecting language. We asked ourselves: What are the consequences for the information design field when meaning-making in a language is understood as being socially situated in an activity? We have studied a design process and used observations, interviews, and text analysis to gather data. The result showed that the workers' ownership of text documents is crucial for the texts' use, yet the texts used do not meet the standard of information design textbooks. Moreover, the design of the text involves a continuous and non-linear collective negotiation that balances standardization in language and work procedures with the incorporation of operators' linguistic improvements. We unfold a case of text design where there is a closeness of designer and user roles, a non-linearity of the process, and an understanding of an information design product as becoming rather than having been finalized for use.

In our research, we have come to focus on the connection between aesthetics and visual-spatial information design; we are addressing the understanding of aesthetic matters in information design and its implications, using examples from the manufacturing sector.

Programmet gick tidigare under namnet Jordbrukets recipientkontroll, JRK. Områden som ingår i programmet finns i ett flertal län i landet och länsstyrelserna är ansvariga för undersökningarna. De områden som väljs ut skall vara representativa för länet. I denna årsredovisning ges en sammanställning av data för typområdet Barlingbo i Gotlands län för det agrohydrologiska året 1994/95. Undersökningarna i området startade 1989. Länsstyrelsen i Gotlands län ansvarar för undersökningarna och avdelningen för vattenvårdslära vis Sveriges landbruksuniversitet har anlitats för sammanställning och redovisning av insamlade data. Denna årsredovisning innefattar bl a årets nederbörd, vattenföring, halter i avrinnande vatten och ämnestransporter. Resultaten kommenteras dessutom översiktligt.

    • According to the United Nation’s Convention of the Rights of the Child, children have the right to participate in their own healthcare and make their opinions heard. The aim of this study was thus to explore the impact of using an eHealth service, Sisom, to gain the children’s perspectives during their healthcare appointments. Data were gathered through individual interviews with a purposeful sample of 16 children, aged 6–13 years old, treated for different diseases and using the eHealth service, Sisom, during their healthcare appointments. The interviews were analysed using a constructivist grounded theory. The results showed that using Sisom made children’s voice heard by creating a communication space in the healthcare setting. This meant that the children got involved in the communication, were acknowledged as an important person who could give the answers to questions and were given time. Implementing the use of Sisom is a way to make children’s needs and preferences explicitly visible for decision-making in practice and thereby supporting the further development of child-centred care in practice. © The Author(s) 2020.

    • The overall aim of the research presented here is to examine challenges in the special needs support tradition and the consequences of these challenges with the aim of increasing knowledge regarding the opportunities and constraints that are created as partof the support. In addition, the overarching study aims to highlight issues of democracyand inclusion in the social practices that are focused here and in what manner theseemerge and are framed. These aims are examined through three separate studies.The first study theoretically examines the central concepts of democracy and inclusionin school policy. The central query here relates to what possible consequences differentmodels of democracy imply vis-à-vis inclusive processes and what possibilities exist forchallenging traditions in special need education? Tradition has often been recognized asan important factor for compensating individuals in a reactive and segregating way.The second study takes an ethnographic point of departure and focuses five languagepre-schools that are instituted with the primary aim of teaching Swedish to children fromhomes where Swedish is not spoken. The institutional intention is to prepare these pupilsfor inclusion in future school arenas. One query here relates to what types ofconsequences such a proactively conceptualized activity gives rise to?A case-study that focuses the analysis of a course to prepare teachers for writingremedial plans constitutes the third research foci. The course is offered by a national stateauthority, the older Institute of Special Education (Specialpedagogiska institutet). Theactivity of working on remedial plans is a challenge for the special educational traditionand its individualization of school related problems. A central issue here relates to whatconsequence such a challenge gives rise to when it is formulated by a school-authority?The results of the studies presented in this thesis indicate that traditions resist changeeven though there is an ambition to challenge these. Old patterns of providing support ininstitutional arenas are difficult to reshape. In both empirical studies individual deficits isa common understanding of school-problems. The results of the empirical studies are alsodiscussed in relation to the conceptual analysis presented in the first article.

    • Hypoxic-ischemic brain injury is often delayed and involves both apoptotic and immunoregulatory mechanisms. In this study, we used a neonatal model of hypoxia-ischemia to examine the effect of the mixed lineage kinase (MLK) inhibitor CEP-1347 on brain damage, apoptosis and inflammation. The tissue volume loss was reduced by 28% (p = 0.019) in CEP-1347-treated versus vehicle-treated rats and CEP-1347 significantly attenuated microgliosis at 7 days (p = 0.038). CEP-1347 decreased TUNEL-positive staining as well as cleaved caspase 3 immunoreactivity. CEP-1347 did not affect the expression of pro-inflammatory cytokines IL-1 beta, IL-6 and MCP-1, nor did it affect the expression of OX-42 (CR3) and OX-18 (MHC I) 24 h after the insult. In conclusion, the MLK inhibitor CEP-1347 has protective effects in a neonatal rat model of hypoxia-ischemia, which is mainly related to reduced apoptosis.

    • BACKGROUND: The tumor promoting or counteracting effects of the immune response to cancer development are thought to be mediated to some extent by the infiltration of regulatory T cells (Tregs ). In the present study we evaluated the prevalence of Treg populations in stromal and epithelial compartments of normal, post atrophic hyperplasia (PAH), prostatic intraepithelial neoplasia (PIN), and tumor lesions in men with and without prostate cancer.METHODS: Study subjects were 102 men consecutively diagnosed with localized prostate cancer undergoing radical prostatectomy and 38 men diagnosed with bladder cancer undergoing cystoprostatectomy without prostate cancer at the pathological examination. Whole mount sections from all patients were evaluated for the epithelial and stromal expression of CD4+ Tregs and CD8+ Tregs in normal, PAH, PIN, and tumor lesions. A Friedmańs test was used to investigate differences in the mean number of Tregs across histological lesions. Logistic regression was used to estimate crude and adjusted odds ratios (OR) for prostate cancer for each histological area.RESULTS: In men with prostate cancer, similarly high numbers of stromal CD4+ Tregs were identified in PAH and tumor, but CD4+ Tregs were less common in PIN. Greater numbers of epithelial CD4+ Tregs in normal prostatic tissue were positively associated with both Gleason score and pT-stage. We observed a fourfold increased risk of prostate cancer in men with epithelial CD4+ Tregs in the normal prostatic tissue counterpart.CONCLUSIONS: Our results may suggest a possible pathway through which PAH develops directly into prostate cancer in the presence of CD4+ Tregs and indicate that transformation of the anti-tumor immune response may be initiated even before the primary tumor is established.

Whether peripheral hyperinsulinemia and GH hypersecretion, which are relevant to the development of vascular complications, result in elevated tissue IGF-I remains unknown. Objective: The purpose of this study was to determine the relationship between whole-body glucose uptake and tissue IGF-I measured by microdialysis. Design: This was a single-blind placebo-controlled crossover study. Setting: The setting was a tertiary pediatric endocrine referral center. Participants: The participants were seven young male adults with type 1 diabetes. Intervention: After an overnight fast, a 6-h lasting euglycemic clamp was performed (constant insulin infusion at 0.5mU/kg x minute and variable glucose infusion rate [GIR]) and a subcutaneous injection of recombinant human (rh) IGF-I (120 mu g/kg) or saline was given after 2 hours. In parallel, tissue IGF-I levels were determined by microdialysis (md-IGF-I). Main Outcome Measures: md-IGF-I levels in muscle and subcutaneous fat, and GIR were determined. Results: md-IGF-I levels were detectable but unchanged after saline. After rhIGF-I, muscle and subcutaneous fat md-IGF-I increased during the second and third hour and then reached a plateau up to 10-fold higher than baseline (P less than .001). GIR was unchanged after saline, whereas it increased 2.5-fold concomitantly with the increase in md-IGF-I (P less than .0001). In contrast, serum IGF-I was increased already at 30 minutes after rhIGF-I and reached a plateau 2-fold above baseline (P less than .0001). Conclusion: We demonstrate that md-IGF-I measurements are valid and physiologically relevant by reflecting rhIGF-I-induced glucose uptake. Future studies should be conducted to elucidate the role of local tissue IGF-I in diabetic vascular complications.

    • AIMS/HYPOTHESIS: To investigate the frequency and characteristics of partial remission in Swedish children with type 1 diabetes and whether insulin delivery method, i.e., continuous subcutaneous insulin infusion (CSII) or multiple daily injections (MDI) affect incidence and duration of this period 2007-2011. Factors that increase the proportion of subjects that enter partial remission and extend this period can improve long-term metabolic control and reduce the risk of severe hypoglycemia, improve quality of life and in the long run reduce late complications.METHODS: Longitudinal data from 2007-2020 were extracted from the Swedish National Quality Register (SWEDIABKIDS) with all reported newly diagnosed children. Data on C-peptide from the participants in the Better Diabetes Diagnosis study (BDD) from 2007-2010 were used. The definition of partial remission was Insulin Dose Adjusted HbA1c (IDAA1c): HbA1c (%)+(4 x total daily insulin dose (U/kg/day)) ≤9.RESULTS: Of the 3,887 patients, 56% were boys. More boys than girls were in partial remission throughout the follow-up period until 24 months after diabetes onset. Fewer children 0-6 years old had partial remission at 3 and 12 months but not at 24 months compared to older age groups. A larger proportion of patients using CSII at 12 and 24 months remained in partial remission compared to those with MDI (37% vs 33%, p=0.02 and 31% vs 27%, p<0.01 respectively). The level of C-peptide was higher in the group with partial remission and mean HbA1c was lower, both p<0.001. Partial remission at 12 months after diabetes onset was associated with CSII (OR:1.39 CI:1.13, 1.71), shorter diabetes duration (OR:0.80 CI:0.76, 0.84) and male sex (OR:1.23 CI:1.04, 1.46)Conclusions/interpretation: Insulin through MDI, longer duration of diabetes, and female sex were associated with lower frequency of partial remission. Use of CSII seem to contribute to longer partial remission among Swedish children with type 1 diabetes.

    • OBJECTIVE: The aim of this study was to evaluate reference genes for expression studies of human adipose tissue. RESEARCH METHODS AND PROCEDURES: Using 52 human adipose tissue expression profiles (HU95), 10 putative reference genes with the lowest variation in expression levels were selected for further studies. Expression stability of these 10 novel and 5 previously established reference genes was evaluated by real-time reverse transcriptase-polymerase chain reaction analysis. For this purpose, 44 adipose tissue biopsies from 27 subjects were chosen to include a wide range of parameters such as sex, age, BMI, depot origin, biopsy procedure, and effects of nutrition. RESULTS: LRP10 was identified as the gene with the least variation in expression levels. The frequently used reference genes RPLP0, 18S rRNA, PPIA, ACTB, and GAPD were ranked as 4, 6, 7, 8, and 10, respectively. DISCUSSION: Our results suggest that LRP10 is a better choice as reference for expression studies of human adipose tissue compared with the most frequently used reference genes.

    • Positioned within the research program and subject area of Innovation and Design, this research focuses on the relationship between tools and joint inquiry. Joint inquiry – the collaborative exploration and definition of problems and possible solutions – plays an important part in Participatory Design processes. Tools, usually involving the making or using of artefacts in workshops, are developed and used to support joint inquiry between different actors. The aim of this research is to contribute to the deliberate design and systematic evaluation of tools for joint inquiry. This work addresses the literature gap comprising a lack of systematic evaluations in Participatory Design research and a need to evaluate designed activities and environments in relation to emerging designer-participant collaborations.This research is a starting point towards developing a systematic approach for designing tools for joint inquiry and introduces two frameworks for this purpose. The first is the Communities of Inquiry framework, from the field of computer-mediated communication in distance education, which is used to identify indicators in joint inquiry. The second framework is a categorisation used to distinguish different aspects of a tool. Combining the two frameworks aids the understanding of the relationships between tools and indicators for joint inquiry. A Research through Design and Research for Design approach is used to study conversations between people and in interaction with tools. Nine design experiments are described, all of which involve the design and testing of tools to support the initiation of joint inquiry. The tools were primarily created and tested within an academic context with design students and researchers, with a focus on the topic of co-production – that is, close collaboration between academia and external actors. One tool was created and used within a museum with youth and researchers.The results from the design experiments show that the tools supported the Communities of Inquiry indicators in the following ways: eliciting metaphors stimulated humour and encouraged contributions, renegotiating artefacts provoked brainstorming, commensality promoted phatic communication and humour, the relationship between material properties and metaphors prompted problem recognition, and formats directed towards personal experiences led to self-disclosure and emotional expression. The tools were a hinderance to the Communities of Inquiry framework when there was dominant participation and when the tools were considered to be inappropriate for certain work contexts. Future research will continue to develop means of systematically evaluating and designing tools that support communicative practices in Participatory Design processes.

    • Participatory design processes can entail involving individuals, groups, and communities in joint inquiry to deliberate on their current situation, imagine alternative futures, and forge possible collaborations. Relational sensitivity – drawing attention to relationships in a given situation – is important when staging (i.e., organising and designing for) and facilitating joint inquiry. It is a valuable concept with which to illuminate and discuss contingent circumstances that may arise and, more generally, to support participation in joint inquiry. This PhD thesis explores and develops views of and a vocabulary for discussing relational sensitivity so as to support practitioners in staging joint inquiry. The thesis includes both a literature review and practice-based studies, applying a research through design approach to discuss relational sensitivity. The practice-based studies examine two participatory design processes: the collaborative designing of a communal space with university students, and the development of shared practices and rituals in a co-working space with professionals from the public sector. The practice-based studies also include nine design experiments in which situations for joint inquiry have been staged in academic contexts and in a museum. Through the literature- and practice-based studies, the thesis addresses two research questions: “How can relational sensitivity be understood when staging situations for joint inquiry?” and “How can the study of relational sensitivity inform practitioners when staging situations for joint inquiry?” Research methods include the use of field notes, journal notes, collective analysis, audio- and video-recordings, and questionnaires.The results include three perspectives from which to understand relational sensitivity when staging joint inquiry: sensitivity to self (e.g., reflexivity, self-awareness, and embodied awareness), sensitivity to intersubjective dynamics (e.g., attunement and responsiveness to group dynamics, affect, emotions, values, trust, and power dynamics), and sensitivity to materiality and process (e.g., the roles of materials, artefacts, and activities in reflection, social cohesion, and diverse contexts, and the role of the body in power dynamics). While relational sensitivity is primarily discussed in relation to practitioners – i.e., those staging joint inquiry – this thesis also discusses the importance of cultivating participants’ awareness of the self and others when staging joint inquiry to collectively support participation.Overall, this research contributes to the research fields of innovation and design and of participatory design by developing knowledge of relational sensitivity and its role in staging situations for joint inquiry. It provides insights into how practitioners can develop, discuss, and study their relational sensitivity and support participants in participatory design processes.
