  • Characteristics of Finnish and Swedish intensive care nursing narratives : a comparative analysis to support the development of clinical language technologies

    • Background: Free text is helpful for entering information into electronic health records, but reusing it is a challenge. The need for language technology for processing Finnish and Swedish healthcare text is therefore evident; however, Finnish and Swedish are linguistically very dissimilar. In this paper we present a comparison of characteristics in Finnish and Swedish free-text nursing narratives from intensive care. This creates a framework for characterising and comparing clinical text and lays the groundwork for developing clinical language technologies. Methods: Our material included daily nursing narratives from one intensive care unit in Finland and one in Sweden. Inclusion criteria for patients were an inpatient period of least five days and an age of at least 16 years. We performed a comparative analysis as part of a collaborative effort between Finnish- and Swedish-speaking healthcare and language technology professionals that included both qualitative and quantitative aspects. The qualitative analysis addressed the content and structure of three average- sized health records from each country. In the quantitative analysis 514 Finnish and 379 Swedish health records were studied using various language technology tools. Results: Although the two languages are not closely related, nursing narratives in Finland and Sweden had many properties in common. Both made use of specialised jargon and their content was very similar. However, many of these characteristics were challenging regarding development of language technology to support producing and using clinical documentation. Conclusions: The way Finnish and Swedish intensive care nursing was documented, was not country or language dependent, but shared a common context, principles and structural features and even similar vocabulary elements. Technology solutions are therefore likely to be applicable to a wider range of natural languages, but they need linguistic tailoring.     • Due to significant developments in cryogenic probe technology and the easy access to inverse detection pulse programmes (HSQC, HMBC), the sensitivity of nitrogen NMR has lately vastly improved. As a consequence, nitrogen NMR has turned into a useful and commonly available tool for solution studies of molecular structure and properties for small organic compounds likewise biopolymers. The high sensitivity of the nitrogen lone pair to changes in the molecular environment, alterations in intra- and intermolecular interactions, and in molecular conformation along with its wide, up to 1200 ppm chemical shift dispersion make nitrogen NMR to an exceptionally sensitive reporter tool. The nitrogen chemical shift has been applied in various fields of chemistry, including for instance the studies of transition metal complexes, chemical reactions such as N-alkylation and N-oxidation, tautomerization, protonation–deprotonation equilibria, hydrogen and halogen bonding, and elucidation of molecular conformation and configuration. The 15N NMR data observed in the investigation of these molecular properties and processes is influenced by the medium it is acquired in. This influence may be due to direct coordination of solvent molecules to transition metal complexes, alteration of tautomerization equilibria, and solvent polarity induced electron density changes of conjugated systems, for example. Thus, the solvent may significantly alter the observed nitrogen NMR shifts. This review aims to provide an overview of solvent effects of practical importance, and discusses selected experimental reports from various subfields of chemistry.

    • Being taller is associated with enhanced longevity, and higher education and earnings. We reanalysed 1472 population-based studies, with measurement of height on more than 18.6 million participants to estimate mean height for people born between 1896 and 1996 in 200 countries. The largest gain in adult height over the past century has occurred in South Korean women and Iranian men, who became 20.2 cm (95% credible interval 17.522.7) and 16.5 cm (13.319.7) taller, respectively. In contrast, there was little change in adult height in some sub-Saharan African countries and in South Asia over the century of analysis. The tallest people over these 100 years are men born in the Netherlands in the last quarter of 20th century, whose average heights surpassed 182.5 cm, and the shortest were women born in Guatemala in 1896 (140.3 cm; 135.8144.8). The height differential between the tallest and shortest populations was 19-20 cm a century ago, and has remained the same for women and increased for men a century later despite substantial changes in the ranking of countries.

    • Background: Neoadjuvant therapy produces high rates of pathological complete response (pCR) and is the standard of care in HER2 positive breast cancer; however, the optimal treatment regimen remains to be established. Methods: In this randomized phase II study patients ≥18 years with HER2 positive breast cancer > 20mm or verified lymph node metastases were randomized to 6 courses of docetaxel, trastuzumab and pertuzumab (DTP, group A) or trastuzumab emtansine (T-DM1, group B), q 21 days. The protocol allowed switch to the competing treatment upon lack of response or drug-related severe toxicity. Patients received postoperative epirubicin+cyclophosphamide, trastuzumab for a total of one year and endocrine therapy. Accrual was completed in October 2018 after randomization of 202 patients, data on pCR were available for 190 at the time for this abstract submission. Median age, 52 years (26-74), menopausal status, histological type and grade were well balanced between the treatment groups. 62.6% of the tumors were hormone receptor (HR) positive. Results: Primary endpoint was pathological objective response. 190 patients completed the protocol-specified preoperative treatment. pCR was achieved in 45.3% of patients, 46.4% in patients treated with DTP and 44.1% with T-DM1 (chi-sq., p = 0.75). In HR-positive tumors, pCR was obtained in 35.3% of patients, 35.9% in group A vs. 34.6% in group B (p = 0.87); in HR-negative tumors, the overall pCR rate was 62.0%, 66.7% in group A vs. 57.9% in group B (p = 0.45). Severe (grade 3/4) toxicity was reported at 68 occasions related to DTP, compared with 16 related to T-DM1, 26 vs. 3 caused by febrile neutropenia. Significantly better quality of life was reported by patients treated with T-DM1. Conclusions: Our data on TDM-1 demonstrates similar efficacy and less toxicity, in particular for patients with HER2 and HR positive cancers, being a potential new standard for neoadjuvant therapy. Clinical trial information: NCT02568839.

    • Objective: To evaluate the effect of intrusive thoughts at diagnosis on quality of life, depressed mood and waking up with anxiety up to two years after radical prostatectomy. Method: The Laparoscopic Prostatectomy Robot Open (LAPPRO) trial was a prospective, longitudinal multicenter study of 4003 patients undergoing radical prostatectomy. Questionnaire data were collected preoperatively, at 3, 12 and 24 months after surgery. Results: The group of patients with intrusive thoughts at diagnosis had a statistically significant higher postoperative prevalence of impaired quality of life, depressed mood and waking up with anxiety as compared with the group of patients with no or minor intrusive thoughts. The highest risk increase for impaired QoL, depressed mood and waking up with anxiety ≥1/week was at 12, 3 and 3 months, respectively, where the three outcomes increased by 38% (RR: 1.38; 95%CI: 1.27–1.49)), 136% (RR: 2.36; 95%CI: 1.74–3.19)) and 165% (RR: 2.65; 95%CI: 2.22–3.17)), respectively. Conclusions: The demonstrated link between intrusive thoughts and quality of life, depressed mood and waking up with anxiety deliver is further evidence to the idea that intrusive thoughts has potential as an endpoint for assessing and predicting psychological distress among men with prostate cancer diagnosis.   • Cardiac Resynchronization Therapy Guided by Echocardiography, MRI, and CT Imaging : A Randomized Controlled Study

    • Objectives: This study evaluated if selecting the left ventricular (LV) target segment by echocardiography-derived late mechanical activation, with access to multimodality imaging for scar and venous anatomy, could help to increase responder rates to cardiac resynchronization therapy (CRT). Background: LV lead placement is important for clinical outcome, but the optimal strategy for LV lead placement in CRT is still debated. Methods: This study conducted a prospective, blinded randomized controlled trial on 102 patients with indication for CRT (27% women, 46% with ischemic cardiomyopathy, 63% in New York Heart Association functional class III, 74% with left bundle branch block, and with mean ejection fraction of 23%). Optimal LV lead location was defined as the latest mechanically activated available segment (free of transmural scar), determined by radial strain echocardiography, cardiac computed tomography, and cardiac magnetic resonance (n = 70). The primary endpoint was reduction of LV end-systolic volume by ≥15% at 6 months post-implantation. Results: Patients were followed for 47 ± 21 months. Based on imaging, optimal or adjacent lead placement was feasible in 96% of all cases and was obtained in 83% of the intervention group versus 80% of the control group. Fifty-six percent of the patients were LV end-systolic volume responders compared with the control group (55%) (p = 0.96), and 71% improved ≥1 New York Heart Association functional class (74% vs. 67%; p = 0.43). Death or heart failure hospitalization within 2 years occurred in 6% (2% of the intervention group vs. 10% of the control group; p = 0.07). Conclusions: Radial strain-guided LV lead placement, in combination with multimodality imaging, did not result in increased clinical or echocardiographic response, nor in a significant reduction of death or heart failure hospitalization.     • Background: Neoadjuvant therapy combining docetaxel, trastuzumab and pertuzumab (DTP) was compared to trastuzumab emtansine (T-DM1) in the randomized phase 2 PREDIX HER2 trial. Patients, ≥18 years with HER2 positive breast cancer, ≥20mm or with verified lymph node metastases, were randomized to six courses of DTP (Standard arm) or T-DM1 (Experimental arm). Primary endpoint was pathological objective response to primary medical therapy at post-treatment surgery. Health related quality of life (HRQoL) was a secondary outcome, and is of specific interest as there was no difference between the randomization groups regarding the main endpoint (results presented in a separate abstract sent to ASCO 2019, Bergh et al.). Methods: Of 202 randomized patients, 190 are available for evaluation at this point. HRQoL was measured, using EORTC QLQ-C30 + EORTC QLQ-BR23, at baseline before randomization and after six courses. Results: No differences between the randomization arms were found at baseline. Results after six courses, based on 163 patients (86%) and adjusted to baseline values, revealed statistical significant differences (p≤0.01), favoring the experimental T-DM1 arm on 7 out of 15 of the EORTC QLQ-C30 variables (Physical functioning, Role functioning, Social functioning, Global quality of Life, Fatigue, Dyspnea, and Diarrhea). For the breast cancer specific questionnaire (EORTC-BR23), the experimental arm scored statistically significantly better on 5 out of 7 subscales (Body image, Sexual functioning, Sexual enjoyment, Systemic therapy side effects and Upset by hair loss). All of the statistical significant differences were of moderate or large clinical significance (≥10 scale scores). No differences between the randomization arms were found for the remaining HRQoL variables. Conclusions: The experimental arm reported better HRQoL than the control arm after six courses. Trastuzumab emtansine may be a useful treatment alternative due to better HRQoL and lower toxicity. Clinical trial information: NCT02568839.

    • Aim: From 1986 to 1996, there was a four-fold increase in coeliac disease among young Swedish children, known as the Swedish coeliac epidemic. Children with type 1 diabetes have an increased risk of developing coeliac disease. We studied whether the prevalence of coeliac disease differed in children with type 1 diabetes born during and after this epidemic.Methods: We compared national birth cohorts of 240 844 children born in 1992–1993 during the coeliac disease epidemic and 179 530 children born in 1997–1998 after the epidemic. Children diagnosed with both type 1 diabetes and coeliac disease were identified by merging information from five national registers.Results: There was no statistically significant difference in the prevalence of coeliac disease among children with type 1 diabetes between the two cohorts: 176/1642 (10.7%, 95% confidence interval 9.2%–12.2%) in the cohort born during the coeliac disease epidemic versus 161/1380 (11.7%, 95% confidence interval 10.0%–13.5%) in the post-epidemic cohort.Conclusion: The prevalence of having both coeliac disease and type 1 diabetes was not significantly higher in children born during, than after, the Swedish coeliac epidemic. This may support a stronger genetic disposition in children who develop both conditions.

    • Until now, there have been few assays to measure C-peptide and proinsulin in the rat. We used a well-established rat insulin ELISA and validated two novel ELISAs for rat C-peptide and rat/mouse proinsulin to examine secretion and content of insulin, proinsulin, and C-peptide from rat islets cultured for 72 h at different glucose concentrations in culture medium. To examine long-term effects in vitro rather than short-term effects of exposure to low, normal, and high glucose, the exposure time to the different glucose concentrations was set to 72 h. The measurement uncertainty of the values obtainable from the ELISAs was determined by calculation of the variation pattern from the intraassay variation generated by unknown samples, and repeatability was determined by analysis of controls. The precision study and the analysis of controls confirm that the validated ELISAs for rat C-peptide and proinsulin would be useful for further studies on the effects of preculture in different glucose concentrations. The higher the glucose concentration used during the 72-h culture period of rat islets, the higher insulin, C-peptide and proinsulin values were obtained in a subsequent short-term glucose-challenge experiment. The proportion of proinsulin to insulin secreted increased, as did islet content, with increasing glucose concentration during preculture. We also observed a nonequimolar, glucose-dependent secretion and content of rat insulin over rat C-peptide after culture at 11.1 and 28 mM glucose.

    • As the prices for housing increase, the question arises whether there is a way to build smaller apartments, yet keeping all the conveniences of a bigger apartment and making it possible to be shared by several people. The concept of Cuber seeks to provide a solution for the problem of having limited space in a shared apartment by enhancing the different activities of the people living in it. Furthermore, it should enable each resident to create their personal space where these activities can be performed. In this paper, a concept is described which makes residents feel in control of their apartment and provides a way to fully adapt the environment to their needs. As a result, a prototype of a tangible cube shaped remote control was implemented that could control lights and sound in a room-like setting and was presented in an exhibition.

    • Obesity shows an increasing prevalence worldwide and a decrease in energy expenditure has been suggested to be one of the risk factors for developing obesity. An increase in resting energy expenditure would have a great impact on total energy expenditure. This study shows that classical music do not influence resting energy expenditure compared to complete silence. Further studies should be performed including other genres of music and other types of stress-inductors than music.

Le développement d’une culture de l'information est également essentiel pour comprendre l'évolution de l'acquisition des connaissances dans le contexte éducatif.

    • This paper unpacks the commercial rhetoric of Google for Education. Through the analysis of information published on the official Google for Education website, the paper seeks to make visible how this service promotes and reproduces certain ways of talking, thinking about and doing education. The aim is to contribute to a critical discussion of the potential implications of allowing major commercial players to take the lead in the development of digital infrastructure in education. Guiding the analysis is the notion of 'problem' understood as central for Google for Education's success story. The case of Sweden, in which Google for Education has become widely used, forms the vantage point for this discussion. The study makes visible how Google for Education, in the commercial rhetoric, is constructed as the solution to problem representations by being positioned as a much-needed bridge, in the shape of digital information infrastructure, between digital policy and educational practice. However, Google for Education is far from simply a practical solution to a set of expensive and urgent problems. To uncritically embrace Google as the information infrastructure of education is to hand over power to one actor, which closes doors to alternative paths of doing and knowing in education.

  • Cell-Mediated Proteomics, and Serological and Mucosal Humoral Immune Responses after Seasonal Influenza Immunization: Characterization of Serological Responders and Non-Responders

    • Immunization against influenza through vaccination is the most effective method with which to prevent infection. To assess protection after immunization, analysing humoral response with a hemagglutinin inhibition assay is the gold standard, but cell-mediated immune response has been shown to better correlate with protection in the elderly. Our aim was to explore the influenza-specific cell-mediated and mucosal humoral responses in serologically defined responders and non-responders. We analysed sera for total immunoglobulins (Ig) A, G, and M and nasal swab samples for influenza-specific IgA. Peripheral blood mononuclear cells were stimulated with trivalent influenza vaccine VaxiGripTetra, and supernatants were analysed for influenza-specific responses with the Olink Immune-Oncology panel using a proximity extension assay. We included 73 individuals, of which 69 completed the study with follow-up sampling at one and six months post-vaccination. Of the 73, 51 (70%) were found to be serological responders and 22 (30%) were non-responders. We did not find any significant differences in sex or mucosal humoral response between responders and non-responders; however, a higher IFN gamma/IL-10 ratio in individuals <= 65 years of age indicates an enhanced cell-mediated immune response in this age group. Characteristics of the non-responders were found to be higher levels of IgM, Granzyme B and Interleukin 12, and lower levels of C-X-C motif chemokine 13 compared with those of the responders. In conclusion, our results did not show any correlation between serological response and age. Furthermore, the majority of influenza-specific cell-mediated immune markers did not differ between responders and non-responders; the immune marker profile of the non-responders and its contribution to protection is of interest but needs to be further explored.

    • Gene expression patterns in ductal carcinoma in situ (DCIS) and invasive and metastatic breast tumors have been determined using serial analysis of gene expression (SAGE). The purpose of this approach was to identify biologically and clinically meaningful subgroups of DCIS with a high risk of progression to invasive disease. The analyses have led to the identification of several differentially expressed genes, such as HIN-1, dermcidin and S100A7 (psoriasin). The aim of the present study was further to delineate the expression profile of S100 genes using information from 22 breast epithelial SAGE libraries. We demonstrated the down-regulation of S100A6 and S100A10 in breast cancer, irrespective of pathological stage. S100P and S100Z were both up-regulated in cancer; whereas S100A7, S100A8 and S100A9 were strongly up-regulated only in DCIS. The hierarchical clustering of S100 gene expression in these 22 libraries revealed two major groups with distinguishable S100 gene expression profiles. One of them was characterized by the high concomitant expression of S100A7, S100A8 and S100A9. Using SAGE informatics, we found 21 genes with a high positive correlation to S100A7 expression in libraries representing different categories of tissues archived at SAGE Genie, suggesting a function of psoriasin that is not tissue specific. Like S100A7, several of these genes displayed cation-binding properties. We also report the strong correlation in the breast epithelial SAGE libraries between the expression of S100A7 and genes reported as being up-regulated in DCIS, as well as in the inflammatory skin disorder, psoriasis; including RGS5, UPK1A, TMPRSS3, S100A9, p53, SCCA1, SCCA2 and KRT17.

    • Lyme borreliosis (LB) is caused by Borrelia burgdorferi and infection may lead to not only a large variety of clinical manifestations but also a subclinical outcome. The aim of the present study was to investigate if there is a constitutional difference in complement activation between individuals with previous subclinical Lyme borreliosis (SB) and patients previously diagnosed with Lyme neuroborreliosis (LNB).Lepirudin plasma for activation studies was collected from 60 SB individuals and from 22 patients pre-diagnosed with LNB. The plasma was incubated with live Borrelia spirochetes of two strains (complement sensitive B. garinii Lu59 and complement resistant B. afzelii ACA1).Complement factor C3 was measured in non-activated lepirudin plasma with immune-nephelometry and C3a and sC5b-9 generated during complement activation were measured by enzyme-linked immunosorbent assay.We found that the complement sensitive Lu59 induced higher complement activation than the complement resistant ACA1 when measuring activation products C3a and sC5b-9 in SB and LNB patients, p < 0.0001. No significant difference was found between SB and LNB patients in systemic levels of C3. Furthermore, SB individuals generated a higher activation of C3 cleavage to C3a (C3a/C3 ratio) than LNB patients after activation with ACA1, p < 0.001, but no significant differences were found in response to Lu59. In conclusion, Lu59 induced higher complement activation than ACA1 and individuals with previous SB showed increased generation of C3a compared with patients with previous LNB. In our study population, this mechanism could lead to less elimination of spirochetes in LNB patients and thereby be a factor contributing to the clinical outcome.

    • This literature review seeks to map the state of research on the effects of digitization on personal financial behavior and management through a bibliometric analysis and a systematic literature review. The findings indicate that current knowledge is primarily based on perspectives of actors in commerce and systems development. More research is needed on how personal financial behavior change in relation to digital technology, the vulnerability of children and adolescents, and the links between changes in credit behavior and indebtedness. Financial counseling could benefit from an awareness of young adults vulnerability as digital consumers and an extended perception of financial literacy that encompasses requirements of digital society. Policymakers need to be aware of the consequences of digital measurability.

    • This dissertation explores how perceived challenges and changing conditions for public libraries are translated and enacted in a local library setting. In 2008, Malmoe City Library, a public library situated in the third largest city of Sweden, initiated an ambitious strategy for renewal and change of their services. The strategy was given the name The Darling Library and expressed a vision to enforce a “paradigm shift”. In recent years, public library sector debates have been coloured by calls for change. By conceptualising Malmoe City Library’s efforts to realize The darling library as an attempt to handle a perceived crisis and demands for change, this study investigates how current challenges are met by local public library services and handled in everyday library work practises. The aim of the study is thus to explore how conceptions of crisis and change are expressed and translated in the transformation process of Malmoe City Library, 2009-2011, and how the changes made to face the perceived challenges are enacted. Thereby it seeks to contribute to a research-based understanding of how the changed conditions for public libraries are handled in a local service an in the everyday work of library employees. The data collection was guided by an ethnographic approach. Drawing on practice theory and ANT, different articulations of the conceptions of crisis and change was followed through a chain of translations, from research about public libraries, through policy and strategy documents, to moments of doing in everyday work practices. The findings show how the enactment of the “paradigm shift” implied that services and everyday work in many respects took on a different shape. Still, several established elements remained significant, for instance the printed book, but were ascribed new roles and meanings. The study shreds light on how enacting change also implies enacting stability.

    • Public libraries are one of several institutions that uphold Swedish democracy. The representative liberal democratic model, expressed in the Library Act, is increasingly being questioned and challenged. Political actors, mainly from the radical right, advocate ademocracy focused on the will of the people at the expense of the rights of individuals. With the notion of plural agonistics, public libraries can be seen as important arenas for debates and meetings between people, offering ways to handle conflicts within democratic institutions. Methodologically, this study employs the perspective of institutional ethnography, and the aim of this paper is to develop knowledge about public libraries’ experiences of political pressure and how this is enacted in a time of political turbulence. This paper reports findings from the first stage of a survey study directed at public library managers in 77 municipalities from the six southernmost regions of Sweden. Based on replies in these surveys, interviews were conducted withseven of the participating library managers. Findings show that the interplay between libraries and the local political level, and between national and local political levels, generally functions without notable opposition. Illegitimate political pressureis uncommon, but when it occurs, it is primarily triggered by issues connected to cultural diversity. Results further indicate that local public libraries tend to respond to illegitimate political pressure by development and use of professional policy documents, but also, in some cases, by avoiding certain activities.

    • This paper reports a study of pupils’ experiences of media and information literacy education in five Swedish schools by answering the following overarching question, what roles do the teaching of information seeking and critical assessment of information play for pupils in their school-work as well as in their everyday life? Pupils in ninth grade were asked to fill in a questionnaire regarding their use of digital technology as well as their thoughts on media and information literacy education. The study shows that many pupils are knowledgeable about the terms of production pertaining to content in most online sources they mention. Still, infrastructural meaning-making that take into consideration issues of personalization, data integrity and surveillance, are largely lacking. The study also shows that the school’s teaching is central to the pupils’ development of a critical stance towards the information that they encounter online. These findings underline the importance of how schools choose to treat media and information literacy education. It is concerning then that infrastructural meaning-making is quite absent in the pupils’ responses.

    • Introduction. In recent years, there has been a growing interest in ethnographic research within the field of library and Information studies. Although ethnography has been used by information researchers for studying a wide variety of phenomena, discussions concerning methodological developments and directions, as well as ethnography’s wider applicability within the field are rare. Our intent is to contribute to the such discussions. Method. The article draws on three on-going research projects to illustrate how the analytical and methodological concepts of following and translation are operationalized. Analysis. Particularly the article addresses the tension between the site specificity traditionally associated with ethnographic methodology, and the fluidity and place transcending but yet situated character of the objects of study in the field. Conclusion. The authors conclude that an ethnography of following and translation is a tool for handling the movability characterizing the phenomena studied in the field, thus turning the necessary uncertainty of inductive research into an advantage. This allows the researcher to stay open and let the object of study lead the way.

    • I denna rapport presenteras resultat från ett två-årigt projekt med målet att undersöka vad vi benämner som det nya svenska bibliotekslandskapets förutsättningar, roller och relationer. Vårt angreppssätt har varit att nå kunskap om detta nya landskap genom att studera styrdokumentens betydelser och gestaltningar i folkbibliotekens arbete. Resultaten som presenteras torde vara av intresse för flera målgrupper som intresserar sig för folkbiblioteken och deras verksamhet. Eftersom vi båda undervisar blivande bibliotekarier har vi dock utformat rapporten med denna målgrupp i särskild åtanke. Vår förhoppning är att rapporten ska kunna fungera som en introduktion till svensk folkbiblioteksverksamhet sett ur ett kulturpolitiskt perspektiv. Studien har finansierats av Svea Bredals stiftelse.

    • Ethnographically inspired methods have in recent years been successfully applied in Information Science (IS) research on different forms of social media related practices (c.f. Sundin & Francke 2009a; 2009b, Kjellberg 2010). This paper aims to contribute to an emerging discussion on the use of this methodological approach for analysing and comprehending consequences of the 2.0-turn in public libraries. Drawing on the experiences of a one-year long ethnographic study of everyday work-related social media practices in a Swedish public library, I suggest that there are significant epistemic benefits of using the ethnographic approach in this field. By shifting focus to everyday practices and routines in local settings, new features of the 2.0-phenomena are accentuated, contributing to different understandings of the role of social media in public library development.

  • Laboratory methods for investigation of the immunological orchestra in response to pathogens

    • Laboratory methods used for investigation of immune response often involve collection of whole blood and analysis of different biomarkers in blood components or generated from pathogen stimulation of whole blood or peripheral blood mononuclear cells (PBMC). Methods used to measure biomarkers are for example enzyme-linked immunosorbent assay (ELISA) which measures one biomarker at a time or multiplex assays for example x-unknown, multi-analyte profiling (xMAP) by Luminex or proximity extension assay (PEA), which can measure up to just over 3000 biomarkers at a time. Analysis of one biomarker at a time are time consuming, costly, and dependent of a large sample size to enable repeated measurements of different analytes. Therefore, multiplex assays that are time saving, more cost effective and measures multiple bi-omarkers at once in a small sample can be applied.   The aim of this thesis was to evaluate multiplex laboratory methods for investigation of the immunological orchestra in response to Borrelia infection and influenza immunisation and if possible, further characterize individuals with different clinical outcomes or serological response, respectively.  In our studies (paper I-III) we included 1113 blood donors of which 66 were found to previously have had a subclinical borreliosis (defined as presence of Borrelia-specific antibodies without recall of previous Lyme borreliosis), of the 66 individuals 60 were available for participation. We also included 22 patients previously diagnosed with Lyme neuroborreliosis (LNB). In paper IV we included in total 73 individuals consisting of healthcare workers and patients attending seasonal influenza vaccination. We applied whole blood, PBMC and plasma stimulations and measured a range of cytokines, chemokines and complement factors with ELISA, nephelometry, xMAP and PEA.   Our results show that subclinical Lyme borreliosis (SB) individuals display the following pattern, low age, male sex, low amount of secreted interleukin (IL)-17, CCL20 and higher secretion of IL-10 by PBMCs stimulated three days with Borrelia garinii compared to patients with previous Lyme neuroborreliosis (LNB). The subclinical individuals also show higher activation of the complement system in response to Borrelia afzelii.   We performed multiplex analysis of complement factors in attempt to further characterize our SB individuals and LNB patient but found the results to deviate largely from reference values retrieved with other standardized methods. This highlights the importance of critical review of generated results from all form of assays. To investigate immune responses after influenza immunisation and further characterize serological responders and nonresponders we included measurement of influenza-specific antibodies and total immunoglobulins (Ig) in blood serum, influenza-specific mucosal IgA (nasal-swabs) and cell-mediated immune response in supernatants from PBMCs stimulated with influenza vaccine using PEA. We found the serological responders to be characterised by lower levels of total IgM, Granzyme B (GZMB) and IL-12 together with higher levels of CXCL13 compared with nonresponders. To conclude, xMAP and PEA are two valuable methods that can be applied together with multivariate statistical methods in the investigation of both innate and adaptive immunity characteristics and association to clinical outcome or serological response after Borrelia infection and influenza immunisation, respectively.

  • Over-Indebtedness and Consumption Patterns in Digital Society – A Research Review

    • This LUii report presents a research review of the field of over-indebtedness and consumption patterns in digital society, which addresses phenomena such as e-commerce, smart phones and their role in consumption and credit, new methods of payment, debt-incurrence and more. The research review has been produced by researchers at Lund University on behalf of the Swedish Enforcement Authority, why it also is in part using Sweden as a case study. The research was conducted during 2014 and primarily includes three sub-studies of somewhat different approaches to provide as comprehensive and accurate an overview of current research in the field as possible: 1) a bibliometric study, 2) a systematic literature review, and 3) additional searches which also include reports written in Swedish, studies and books. A general conclusion drawn from all sub-studies is that research on over-indebtedness and research on digital development are often conducted within different disciplines that share little contact. One consequence of this, with regards to the review materials and results, is that research on potential opportunities created by digital technology in the credit and financial markets is not linked to the emergence of problematic financial situations in the individual. The potential links between these areas therefore deserves further scientific examination. For example, the results indicate that international research on debt and consumer behavior in a digital context is largely conducted from a systemic or business-oriented perspective, and deals only to a minor degree with consumer vulnerability and other negative aspects for the individual. This means that the economic/business-oriented perspective dominates in the research on debt incurrence in digital contexts. Furthermore, young adults comprise the population group that is repeatedly mentioned as particularly vulnerable to the implications of digital technology in consumption and debt. Stefan Larsson, Head of Lund University Internet Institute (LUii), PhD in sociology of law and PhD in spatial planning, has acted as project manager for this research review. Co-researchers for this project were Hanna Carlsson, PhD in information science, and Fredrik Åström, Assoc Prof and bibliometrician.
