    • We performed fine mapping of 39 established type 2 diabetes (T2D) loci in 27,206 cases and 57,574 controls of European ancestry. We identified 49 distinct association signals at these loci, including five mapping in or near KCNQ1. 'Credible sets' of the variants most likely to drive each distinct signal mapped predominantly to noncoding sequence, implying that association with T2D is mediated through gene regulation. Credible set variants were enriched for overlap with FOXA2 chromatin immunoprecipitation binding sites in human islet and liver cells, including at MTNR1B, where fine mapping implicated rs10830963 as driving T2D association. We confirmed that the T2D risk allele for this SNP increases FOXA2-bound enhancer activity in islet- and liver-derived cells. We observed allele-specific differences in NEUROD1 binding in islet-derived cells, consistent with evidence that the T2D risk allele increases islet MTNR1B expression. Our study demonstrates how integration of genetic and genomic information can define molecular mechanisms through which variants underlying association signals exert their effects on disease.

    • Aphids are important agricultural pests and also biological models for studies of insect-plant interactions, symbiosis, virus vectoring, and the developmental causes of extreme phenotypic plasticity. Here we present the 464 Mb draft genome assembly of the pea aphid Acyrthosiphon pisum. This first published whole genome sequence of a basal hemimetabolous insect provides an outgroup to the multiple published genomes of holometabolous insects. Pea aphids are host-plant specialists, they can reproduce both sexually and asexually, and they have coevolved with an obligate bacterial symbiont. Here we highlight findings from whole genome analysis that may be related to these unusual biological features. These findings include discovery of extensive gene duplication in more than 2000 gene families as well as loss of evolutionarily conserved genes. Gene family expansions relative to other published genomes include genes involved in chromatin modification, miRNA synthesis, and sugar transport. Gene losses include genes central to the IMD immune pathway, selenoprotein utilization, purine salvage, and the entire urea cycle. The pea aphid genome reveals that only a limited number of genes have been acquired from bacteria; thus the reduced gene count of Buchnera does not reflect gene transfer to the host genome. The inventory of metabolic genes in the pea aphid genome suggests that there is extensive metabolite exchange between the aphid and Buchnera, including sharing of amino acid biosynthesis between the aphid and Buchnera. The pea aphid genome provides a foundation for post-genomic studies of fundamental biological questions and applied agricultural problems.

  • Robust estimation of bacterial cell count from optical density

    • Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data.

    • We introduce the AusTraits database - a compilation of values of plant traits for taxa in the Australian flora (hereafter AusTraits). AusTraits synthesises data on 448 traits across 28,640 taxa from field campaigns, published literature, taxonomic monographs, and individual taxon descriptions. Traits vary in scope from physiological measures of performance (e.g. photosynthetic gas exchange, water-use efficiency) to morphological attributes (e.g. leaf area, seed mass, plant height) which link to aspects of ecological variation. AusTraits contains curated and harmonised individual- and species-level measurements coupled to, where available, contextual information on site properties and experimental conditions. This article provides information on version 3.0.2 of AusTraits which contains data for 997,808 trait-by-taxon combinations. We envision AusTraits as an ongoing collaborative initiative for easily archiving and sharing trait data, which also provides a template for other national or regional initiatives globally to fill persistent gaps in trait knowledge.

  • An integrated cell atlas of the lung in health and disease

    • Single-cell technologies have transformed our understanding of human tissues. Yet, studies typically capture only a limited number of donors and disagree on cell type definitions. Integrating many single-cell datasets can address these limitations of individual studies and capture the variability present in the population. Here we present the integrated Human Lung Cell Atlas (HLCA), combining 49 datasets of the human respiratory system into a single atlas spanning over 2.4 million cells from 486 individuals. The HLCA presents a consensus cell type re-annotation with matching marker genes, including annotations of rare and previously undescribed cell types. Leveraging the number and diversity of individuals in the HLCA, we identify gene modules that are associated with demographic covariates such as age, sex and body mass index, as well as gene modules changing expression along the proximal-to-distal axis of the bronchial tree. Mapping new data to the HLCA enables rapid data annotation and interpretation. Using the HLCA as a reference for the study of disease, we identify shared cell states across multiple lung diseases, including SPP1 + profibrotic monocyte-derived macrophages in COVID-19, pulmonary fibrosis and lung carcinoma. Overall, the HLCA serves as an example for the development and use of large-scale, cross-dataset organ atlases within the Human Cell Atlas.

  • Strengthening old steel railway bridges: a review

    • Strengthening old bridges is an increasingly relevant strategy for risk prevention and operation continuity in management of infrastructures. Transportation networks are subjected to progressively stricter environmental and load conditions, leading to a growing number of deficient structures, also due to aging and deterioration. However, employable resources are finite, from both economical and environmental points of view. For these reasons, strengthening opportunities should be considered as a viable option, improving bridges behaviour with low economical and environmental impact. With this perspective, a selection of some of the most interesting strengthening techniques for old truss railway bridges is presented. To address effective solutions, the most frequent problems in old truss railway bridges are first presented. Literature analysis and experts’ interviews were conducted and compared to results obtained from a representative bridge cluster. Different solutions addressing highlighted problems are then collected and qualitatively evaluated, in terms of efficacy on structural behaviour and typical construction requirements. Finally, general remarks and recommendations based on collected evidence are presented.

  • Pharmacokinetics in Mouse and Comparative Effects of Frondosides in Pancreatic Cancer

    • The frondosides are triterpenoid glycosides from the Atlantic sea cucumber Cucumaria frondosa. Frondoside A inhibits growth, invasion, metastases and angiogenesis and induces apoptosis in diverse cancer types, including pancreatic cancer. We compared the growth inhibitory effects of three frondosides and their aglycone and related this to the pharmocokinetics and route of administration. Frondoside A potently inhibited growth of pancreatic cancer cells with an EC50 of similar to 1 mu M. Frondoside B was less potent (EC50 similar to 2.5 mu M). Frondoside C and the aglycone had no effect. At 100 mu g/kg, frondoside A administered to CD2F1 mice as an i.v. bolus, the Cp-max was 129 nM, Cl-tb was 6.35 mL/min/m(2), and half-life was 510 min. With i.p. administration the Cp-max was 18.3 nM, Cl-tb was 127 mL/min/m(2) and half-life was 840 min. Oral dosing was ineffective. Frondoside A (100 mu g/kg/day i.p.) markedly inhibited growth cancer xenografts in nude mice. The same dose delivered by oral gavage had no effect. No evidence of acute toxicity was seen with frondoside A. Frondoside A is more potent inhibitor of cancer growth than other frondosides. The glycoside component is essential for bioactivity. Frondoside A is only effective when administered systemically. Based on the current and previous studies, frondoside A appears safe and may be valuable in the treatment of cancer.

Prosjektet er et eksempel på vellykket nordisk samarbeid, og kan ha overføring til fremtidige prosjekter

    • A requirement for heavier vehicular transport on the Norwegian road network has resulted in a demand for increased bearing capacity for many of the older bridges in the country. Many of the bridges that have been found to have insufficient capacity against present-day demands are steel girder bridges with concrete slabs without a shear connection between steel and concrete. There is a large number of bridges of this type in Norway and the paper presents strengthening of two of those, in Aust-Agder county in the south of the country. These bridges are approximately 30 m long, single span. The bearing capacity has been upgraded by installing composite action between the steel girders and the concrete slab using coiled pins, in conjunction with thickening of the bottom flange of the steel girders. To obtain composite action, the pins are fitted to tightly drilled holes through the top flange of girders up into the concrete slab. Coiled pins have not been used much for bridge applications. In the work presented, the method has been found to have advantages in terms of cost and workability. Furthermore, the method has benefits when viewed from an environmental standpoint, since it allows strengthening of existing non-composite bridges using relatively little new material, and minimizes traffic disruptions.

    • Under de senaste 8 åren har vi och våra brokollegor på Ramboll i Luleå fått förmånen att arbeta med tre stora landmärken i Stockholmsområdet. Tre stora broar som var och en varit i funktion i nästan hundra år. Otaliga gånger har de passerats av cyklister, gående, bussresenärer, tågresenärer, spårvagnsresande, bilister och båtar. Med sina silhuetter har de varit en del av stadsbilden och landskapet, någonting blicken söker för att orientera sig med - landmärken. Tusentals minnen har skapats på och omkring dessa broar i olika väder och årstider och i varierande sinnesstämningar. En bro som existerat under så lång tid blir helt naturligt väldigt betydelsefull, inte bara för att den hjälper dig att komma över till andra sidan, utan även på ett annat plan - det går liksom inte att tänka bort den, att den inte längre skulle finnas. När vi konstruerar nya broar ger vi dem en livslängd som de förväntas hålla, vanligen 100 eller 120 år. Alltså något längre än en människas förväntade livslängd. Vad händer när livslängden har uppnåtts, när bron kanske inte orkar mer fast vi inte är beredda att ta farväl? Här en kort beskrivning av broarna som har engagerat så många under årens lopp.

  • PDRs4All : A JWST Early Release Science Program on Radiative Feedback from Massive Stars

    • Massive stars disrupt their natal molecular cloud material through radiative and mechanical feedback processes. These processes have profound effects on the evolution of interstellar matter in our Galaxy and throughout the universe, from the era of vigorous star formation at redshifts of 1-3 to the present day. The dominant feedback processes can be probed by observations of the Photo-Dissociation Regions (PDRs) where the far-ultraviolet photons of massive stars create warm regions of gas and dust in the neutral atomic and molecular gas. PDR emission provides a unique tool to study in detail the physical and chemical processes that are relevant for most of the mass in inter- and circumstellar media including diffuse clouds, proto-planetary disks, and molecular cloud surfaces, globules, planetary nebulae, and star-forming regions. PDR emission dominates the infrared (IR) spectra of star-forming galaxies. Most of the Galactic and extragalactic observations obtained with the James Webb Space Telescope (JWST) will therefore arise in PDR emission. In this paper we present an Early Release Science program using the MIRI, NIRSpec, and NIRCam instruments dedicated to the observations of an emblematic and nearby PDR: the Orion Bar. These early JWST observations will provide template data sets designed to identify key PDR characteristics in JWST observations. These data will serve to benchmark PDR models and extend them into the JWST era. We also present the Science-Enabling products that we will provide to the community. These template data sets and Science-Enabling products will guide the preparation of future proposals on star-forming regions in our Galaxy and beyond and will facilitate data analysis and interpretation of forthcoming JWST observations.

    • Multi-omics datasets are becoming more common, necessitating better integration methods to realize their revolutionary potential. Here, we introduce multi-set correlation and factor analysis (MCFA), an unsupervised integration method tailored to the unique challenges of high-dimensional genomics data that enables fast inference of shared and private factors. We used MCFA to integrate methylation markers, protein expression, RNA expression, and metabolite levels in 614 diverse samples from the Trans-Omics for Precision Medicine/Multi-Ethnic Study of Atherosclerosis multi-omics pilot. Samples cluster strongly by ancestry in the shared space, even in the absence of genetic information, while private spaces frequently capture dataset-specific technical variation. Finally, we integrated genetic data by conducting a genome-wide association study (GWAS) of our inferred factors, observing that several factors are enriched for GWAS hits and trans-expression quantitative trait loci. Two of these factors appear to be related to metabolic disease. Our study provides a foundation and framework for further integrative analysis of ever larger multi-modal genomic datasets.

  • Costs and resource utilization for diagnosis and treatment during the initial year in a European inflammatory bowel disease inception cohort : an ECCO-EpiCom Study

    • BACKGROUND: No direct comparison of health care cost in patients with inflammatory bowel disease across the European continent exists. The aim of this study was to assess the costs of investigations and treatment for diagnostics and during the first year after diagnosis in Europe.METHODS: The EpiCom cohort is a prospective population-based inception cohort of unselected inflammatory bowel disease patients from 31 Western and Eastern European centers. Patients were followed every third month from diagnosis, and clinical data regarding treatment and investigations were collected. Costs were calculated in euros (&OV0556;) using the Danish Health Costs Register.RESULTS: One thousand three hundred sixty-seven patients were followed, 710 with ulcerative colitis, 509 with Crohn's disease, and 148 with inflammatory bowel disease unclassified. Total expenditure for the cohort was &OV0556;5,408,174 (investigations: &OV0556;2,042,990 [38%], surgery: &OV0556;1,427,648 [26%], biologicals: &OV0556;781,089 [14%], and standard treatment: &OV0556;1,156,520 [22%)]). Mean crude expenditure per patient in Western Europe (Eastern Europe) with Crohn's disease: investigations &OV0556;1803 (&OV0556;2160) (P = 0.44), surgery &OV0556;11,489 (&OV0556;13,973) (P = 0.14), standard treatment &OV0556;1027 (&OV0556;824) (P = 0.51), and biologicals &OV0556;7376 (&OV0556;8307) (P = 0.31). Mean crude expenditure per patient in Western Europe (Eastern Europe) with ulcerative colitis: investigations &OV0556;1189 (&OV0556;1518) (P < 0.01), surgery &OV0556;18,414 (&OV0556;12,395) (P = 0.18), standard treatment &OV0556;896 (&OV0556;798) (P < 0.05), and biologicals &OV0556;5681 (&OV0556;72) (P = 0.51).CONCLUSIONS: In this population-based unselected cohort, costs during the first year of disease were mainly incurred by investigative procedures and surgeries. However, biologicals accounted for >15% of costs. Long-term follow-up of the cohort is needed to assess the cost-effectiveness of biological agents.

    • BACKGROUND: A definitive diagnosis of Crohn's disease (CD) or ulcerative colitis (UC) is not always possible and a proportion of patients will be diagnosed as inflammatory bowel disease unclassified (IBDU). The aim of the study was to investigate the prognosis of patients initially diagnosed with IBDU and the disease course during the following five years.METHODS: The Epi-IBD study is a prospective population-based cohort of 1,289 IBD patients diagnosed in centres across Europe. Clinical data were captured prospectively throughout the follow-up period.RESULTS: Overall, 476 (37%) patients were initially diagnosed with CD, 701 (54%) with UC, and 112 (9%) with IBDU. During follow-up, 28 (25%) IBDU patients were changed diagnoses to either UC (n=20, 71%) or CD (n=8, 29%) after a median of six months (IQR: 4-12), while 84 (7% of the total cohort) remained IBDU. A total of 17 (15%) IBDU patients were hospitalized for their IBD during follow-up, while 8 (7%) patients underwent surgery. Most surgeries (n=6, 75%) were performed on patients whose diagnosis was later changed to UC; three of these colectomies led to a definitive diagnosis of UC. Most patients (n=107, 96%) received 5-aminosalicylic acid, while 11 (10%) patients received biologicals, of whom five remained classified as IBDU.CONCLUSIONS: In a population-based inception cohort, 7% of IBD patients were not given a definitive diagnosis of IBD after five years of follow-up. One in four patients with IBDU eventually were classified as CD or UC. Overall, the disease course and medication burden in IBDU patients were mild.

  • Initial Disease Course and Treatment in an Inflammatory Bowel Disease Inception Cohort in Europe : The ECCO-EpiCom Cohort

    • Background:The EpiCom cohort is a prospective, population-based, inception cohort of inflammatory bowel disease (IBD) patients from 31 European centers covering a background population of 10.1 million. The aim of this study was to assess the 1-year outcome in the EpiCom cohort.Methods:Patients were followed-up every third month during the first 12 (3) months, and clinical data, demographics, disease activity, medical therapy, surgery, cancers, and deaths were collected and entered in a Web-based database (www.epicom-ecco.eu).Results:In total, 1367 patients were included in the 1-year follow-up. In western Europe, 65 Crohn's disease (CD) (16%), 20 ulcerative colitis (UC) (4%), and 4 IBD unclassified (4%) patients underwent surgery, and in eastern Europe, 12 CD (12%) and 2 UC (1%) patients underwent surgery. Eighty-one CD (20%), 80 UC (14%), and 13 (9%) IBD unclassified patients were hospitalized in western Europe compared with 17 CD (16%) and 12 UC (8%) patients in eastern Europe. The cumulative probability of receiving immunomodulators was 57% for CD in western (median time to treatment 2 months) and 44% (1 month) in eastern Europe, and 21% (5 months) and 5% (6 months) for biological therapy, respectively. For UC patients, the cumulative probability was 22% (4 months) and 15% (3 months) for immunomodulators and 6% (3 months) and 1% (12 months) for biological therapy, respectively in the western and eastern Europe.Discussion:In this cohort, immunological therapy was initiated within the first months of disease. Surgery and hospitalization rates did not differ between patients from eastern and western Europe, although more western European patients received biological agents and were comparable to previous population-based inception cohorts.

  • Natural Disease Course of Ulcerative Colitis During the First Five Years of Follow-up in a European Population-based Inception Cohort-An Epi-IBD Study

    • Background and Aims: Few population-based cohort studies have assessed the disease course of ulcerative colitis [UC] in the era of biological therapy and widespread use of immunomodulators. The aim of this study was to assess the 5-year outcome and disease course of patients with UC in the Epi-IBD cohort.Methods: In a prospective, population-based inception cohort of unselected patients with UC, patients were followed up from the time of their diagnosis, which included the collection of their clinical data, demographics, disease activity, medical therapy, and rates of surgery, cancers, and deaths. Associations between outcomes and multiple covariates were analysed by Cox regression analysis.Results: A total of 717 patients were included in the study. During follow-up, 43 [6%] patients underwent a colectomy and 163 [23%] patients were hospitalised. Of patients with limited colitis [distal to the left flexure], 90 [21%] progressed to extensive colitis. In addition, 92 [27%] patients with extensive colitis experienced a regression in disease extent, which was associated with a reduced risk of hospitalisation (hazard ratio [HR]: 0.5 95% CI: 0.3-0.8]. Overall, patients were treated similarly in both geographical regions; 80 [11%] patients needed biological therapy and 210 [29%] patients received immunomodulators. Treatment with immunomodulators was found to reduce the risk of hospitalisation [HR: 0.5 95% CI: 0.3-0.8].Conclusions: Although patients in this population-based cohort were treated more aggressively with immunomodulators and biological therapy than in cohorts from the previous two decades, their disease outcomes, including colectomy rates, were no different. However, treatment with immunomodulators was found to reduce the risk of hospitalisation.

    • Background and aims: Anaemia is an important complication of inflammatory bowel disease (IBD). The aim of this study was to determine the prevalence of anaemia and the practice of anaemia screening during the first year following diagnosis in a European prospective population-based inception cohort.Methods: Newly diagnosed IBD patients were included and followed prospectively for one year in 29 European and 1 Australian centre. Clinical data including demographics, medical therapy, surgery and blood samples were collected. Anaemia was defined according to the World Health Organization.Results: A total of 1,871 patients (CD: 686, 88%; UC: 1,021, 87%; IBDU 164. 81%) were included in the study. The prevalence of anaemia was higher in CD than in UC patients and overall, 49% of CD and 39% of UC patients had at least one instance of anaemia during the first 12 months after diagnosis. UC patients with more extensive disease and those from Eastern European countries, and CD patients with penetrating disease or colonic disease location, had higher risks of anaemia. CD and UC patients in need of none or only mild anti-inflammatory treatment had a lower risk of anaemia. In a significant proportion of patients, anaemia was not assessed until several months after diagnosis, and in almost half of all cases of anaemia a thorough work-up was not performed.Conclusions: Overall, 42% of patients had at least one instance of anaemia during the first year following diagnosis. Most patients were assessed for anaemia regularly; however, a full anaemia work-up was frequently neglected in this community setting.

    • The competitiveness of composite bridges depends on several circumstances such as site conditions, local costs of material and staff and the contractor's experience. One major advantage compared to concrete bridges is that the steel girders can carry the weight of the formwork and the wet concrete. Another advantage is the savings in construction time, which saves some money for the contractor but even more so for the road users, a fact that usually is neglected when evaluating alternative bridge designs. A further step to improve the competitiveness of composite bridges is to prefabricate not only the steel girders, but also the concrete deck. In this paper a new concept with dry joints between the elements is described.

    • The European infrastructure is rapidly aging, and steel/composite bridges are noexception to the rule. With thousands of older steel/composite bridges, there is ademand of rational methods to strengthen the older bridges to compensate not onlyfor their age, but also for higher loads and new codes, of which perhaps the newfatigue rules for highway bridges in EC3-2 will be the hardest to meet.Within the frames of the European R&D project Prolife (RFCS-CT-2015-00025) aworkshop was arranged in Stockholm September 28th 2015. Bridge owners,designers and researchers from 12 countries participated, and the similaritiesbetween the countries as well as the variety of technical solutions were highlighted.The contributions are presented in this report and the organizers want to thank allparticipants for making this seminar successful.

    • The resistance of bridge girders with respect to lateral-torsional buckling at support is strongly influenced by the moment gradient. In most design methods this influence is taken into account by the use of a correct critical bending moment in the slenderness parameter λ. This critical moment is influenced by the shape of the moment diagram as well as the distortion of the cross-section and the restraint from the web and stiffeners, if any. In this paper, a method for the calculation of the critical moment is presented. A further effect of the moment gradient is that the stresses due to lateral bending of the flange in connection with lateral-torsional buckling does not coincide with the maximum of stresses caused by bending in the vertical plane. This is taken into account by performing the check for lateral-torsional buckling in a design section at some distance from the support. A design procedure based on this concept has been introduced in Eurocode 3 Part 2: Steel Bridges

    • Bernt Johansson, born in Stockholm in 1942. earned his MSc in Civil Engineering from the Royal Institute of Technology in 1961. He worked as a consultant engineer and continued his studies and presented his PhD thesis in 1976 on the stability of box-girder beams. In 1985 he was recruited to Luleå University of Technology to start research on steel structures. He initiated work with thin-walled structures, high strength steel and composite structures collaborating with industry. He played a pivotal role in the development of Swedish and European design codes. He supervised many master’s, licentiate, and PhD students. He passed away in 2017

    • The present thesis, written in Swedish, deals with the problem of lateral-torsional buckling, especially in gable frames. The aim of the work presented is to suggest an improved design method that takes into account the translational and rotational restraint produced by the roof and wall. A particular question is the behaviour of the comer region. More specific, is it a safe assumption to consider the comer as a lateral support. The study includes a review of literature and design methods, analytical solutions of some stability problems, tests on four frames and a FEM simulation. The design methods for lateral buckling usually start from the elastic critical load which defines a relative slenderness of the structure. This slenderness finally gives the predicted capacity. The lateral buckling theory has to take cross-sectional distortion into account because the rotational restraint is often provided concentratedly by purlins or wall rails. This distortion is important also for rolled beams which was not expected. Closed form solutions are presented for the column and references are given to solutions for the roof beam. In order to get some indication of the stability of the comer of a frame, the inner flanges and the stiffener were modelled as a three rod system. The slope of the roof turned out to be of no significance. The results also indicated that the stability of the comer could be questioned when the critical loads of the column and the girder were close to each other. This conclusion was also reached by studies of elasto-plastic frames carried out by Stoverink, at the RWIH, Aachen. Four frames of IPE 180, of which two were haunched, were tested. The two unhaunched frames reached their plastic capacity, and showed a ductile behaviour although their relative slenderness was above the normally accepted limit for full capacity. The two haunched frames, however, buckled before their plastic capacity was reached. This indicates that haunched frames of this type can not be expected to develop their full plastic capacity under conditions similar to those during testing. Finally, the Swedish standards are compared with experimental results, and improvements are suggested. Guidelines are also drawn up for the design of low-rise frames with respect to lateral buckling.

    • In vielen Ländern der Erde haben sich Brücken in Verbundbauweise zu einer bewährten Ausführungsvariante entwickelt. Die Wettbewerbsfähigkeit einer Verbundbrücke ist von verschiedenen Faktoren abhängig, wie z. B. die örtlichen Material- und Lohnkosten, aber auch die Erfahrungen des ausführenden Unternehmers. Im Vergleich zu Betonbrücken besteht ein großer Vorteil darin, daß die Stahlträger das Gewicht der Schalung und des frischen Betons tragen können, so daß keine temporären Schalungskonstruktionen erforderlich werden. Ein weiterer Vorteil ist die Zeitersparnis während der Montage auf der Baustelle, die zum einen die Ausgaben des Unternehmers reduzieren und zum anderen das Bauwerk für den Straßenverkehr früher nutzbar macht. Letzteres wird normalerweise bei der Beurteilung alternativer Brückenkonstruktionen vernachlässigt. Traditionell wird die Fahrbahnplatte vor Ort auf der Baustelle hergestellt. Dies bedeutet, daß die zugehörigen Arbeiten wie Einschalen und Verlegen der Bewehrung im Freien durchgeführt werden müssen. In Regionen mit langen Winterperioden können diese Arbeiten zu Problemen führen und somit auch die Kosten steigern. Ein weiterführender Schritt zur Erhöhung der Wettbewerbsfähigkeit von Verbundbrücken ist, neben den Stahlträgern auch die Beton-Fahrbahnplatten vorzufertigen. Die überwiegenden Vorteile für dieses Bauverfahren sind im folgenden aufgeführt: der Anteil der Lohnstunden für Arbeiten auf der Baustelle wird herabgesetzt; die Montagezeiten verkürzen sich; die Betonfertigteilelemente werden in der Fertigteilfabrik gefertigt, so daß sich die Ausführungsqualität verbessert; die Schwindkräfte am Bauwerk werden reduziert, da das Schwinden bereits überwiegend vor der Montage der Betonfertigteile auf die Stahlträger abgeschlossen ist; die Arbeitsbedingungen für das Einschalen, Verlegen der Bewehrung und Betonieren der Fahrbahnplatte werden verbessert. Um die Wirtschaftlichkeit dieses Bauverfahrens zu optimieren, müssen allerdings auch noch nicht ausreichend gelöste Problematiken, wie die Verbindungen der Fertigteilplatten, weiterentwickelt werden. Im folgenden wird dies näher erläutert, wobei auch neue Lösungsmöglichkeiten vorgestellt werden.

    • AimThe aim of the study was to examine mortality risk associated with use of antidepressants and antipsychotics classified with torsades de pointes (TdP) risk in elderly. MethodsA matched case-control register study was conducted in people 65 years and older dying outside hospital from 2008-2013 (n=286092) and matched controls (n=1430460). The association between prescription of antidepressants and antipsychotics with various TdP risk according to CredibleMeds () and all-cause mortality was studied by multivariate conditional logistic regression adjusted for comorbidity and several other confounders. ResultsUse of antidepressants classified with known or possible TdP risk, was associated with higher adjusted risk for mortality (OR 1.53, 95% CI 1.51, 1.56 and OR 1.63, 95% CI 1.61, 1.67, respectively) compared with antidepressants classified with conditional TdP risk (OR 1.25, 95% CI 1.22, 1.28) or without TdP classification (OR 0.99, 95% CI 0.94, 1.05). Antipsychotics classified with known TdP risk were associated with higher risk (OR 4.57, 95% CI 4.37, 4.78) than antipsychotics with possible risk (OR 2.58, 95% CI 2.52, 2.64) or without TdP classification (OR 2.14, 95% CI 2.03, 2.65). The following risk ranking was observed for commonly used antidepressants: mirtazapine > citalopram > sertraline > amitriptyline and for antipsychotics: haloperidol > risperidone >olanzapine > quetiapine. ConclusionThe CredibleMeds system predicted drug-associated risk for mortality in the elderly at the risk class level. Among antipsychotics, haloperidol, and among antidepressants, mirtazapine and citalopram, were associated with the highest risks. The results suggest that the TdP risk with antidepressants and antipsychotics should be taken into consideration when prescribing to the elderly.
