    • © 2017 European Society of Clinical Microbiology and Infectious Diseases Objectives Left-sided methicillin-susceptible Staphylococcus aureus (MSSA) endocarditis treated with cloxacillin has a poorer prognosis when the vancomycin minimum inhibitory concentration (MIC) is ≥1.5 mg/L. We aimed to validate this using the International Collaboration on Endocarditis cohort and to analyse whether specific genetic characteristics were associated with a high vancomycin MIC (≥1.5 mg/L) phenotype. Methods All patients with left-sided MSSA infective endocarditis treated with antistaphylococcal β-lactam antibiotics between 2000 and 2006 with available isolates were included. Vancomycin MIC was determined by Etest as either high (≥1.5 mg/L) or low (<1.5 mg/L). Isolates underwent spa typing to infer clonal complexes and multiplex PCR for identifying virulence genes. Univariate analysis was performed to evaluate the association between in-hospital and 1-year mortality, and vancomycin MIC phenotype. Results Sixty-two cases met the inclusion criteria. Vancomycin MIC was low in 28 cases (45%) and high in 34 cases (55%). No significant differences in patient demographic data or characteristics of infection were observed between patients with infective endocarditis due to high and low vancomycin MIC isolates. Isolates with high and low vancomycin MIC had similar distributions of virulence genes and clonal lineages. In-hospital and 1-year mortality did not differ significantly between the two groups (32% (9/28) vs. 27% (9/34), p 0.780; and 43% (12/28) vs. 29% (10/34), p 0.298, for low and high vancomycin MIC respectively). Conclusions In this international cohort of patients with left-sided MSSA endocarditis treated with antistaphylococcal β-lactams, vancomycin MIC phenotype was not associated with patient demographics, clinical outcome or virulence gene repertoire.

    • The application of pyrrole-substituted porphyrin electropolymers for simultaneous optical and electrochemical analysis of red-ox active analytes, namely diazo-conjugated dyes of Sudan family, is presented. Sudan colorants are widely used in many fields, but accurate screening of their consumption is required due to their high toxicity. The inherent electrochemical activity of Sudan dyes, as far as their intense coloration, makes possible to find the appropriate conditions of hybrid optical and electrochemical porphyrin electropolymer based sensor array system application. This approach allowed a significant increase in the chemical information, improving the analytical system performance in terms of selectivity and sensitivity, and permitted the fast and simple monitoring of Sudan dye analytes.

    • The computer screen photo-assisted technique (CSPT) is a measuring method that uses regular computer screens as controlled light sources and web cameras as imaging detectors for optical evaluations of sensing substances, which optical properties respond to target analytes. The fact, that CSPT uses a globally distributed and familiar infrastructure as instrumentation and that is able to serve multiple targets with the same setup, makes it an attractive candidate for home testing. This article reviews the CSPT concept and focuses in two recent studies that demonstrate thorough CSPT determinations using a handy and eventually disposable assay format, with the potential to be as ubiquitous as the CSPT platforms themselves. The implications of such combination of technologies is discussed, and its potential to support the global monitoring of environmental and sanitary parameters, using Internet geography browsers, is analyzed. © 2006 Elsevier B.V.   • Safety and efficacy of coronary sinus narrowing in chronic refractory angina: Insights from the RESOURCE study.

    • Refractory angina (RA) is considered the end-stage of coronary artery disease, and often has no interventional treatment options. Coronary sinus Reducer (CSR) is a recent addition to the therapeutic arsenal, but its efficacy has only been evaluated on small populations. The RESOURCE registry provides further insights into this therapy.The RESOURCE is an observational, retrospective registry that includes 658 patients with RA from 20 centers in Europe, United Kingdom and Israel. Prespecified endpoints were the amelioration of anginal symptoms evaluated with the Canadian Cardiovascular Society (CCS) score, the rates of procedural success and complications, and MACEs as composite of all-cause mortality, acute coronary syndromes, and stroke.At a median follow-up of 502days (IQR 225-1091) after CSR implantation, 39.7% of patients improved by ≥2 CCS classes (primary endpoint), and 76% by ≥1 class. Procedural success was achieved in 96.7% of attempts, with 3% of procedures aborted mostly for unsuitable coronary sinus anatomy. Any complication occurred in 5.7% of procedures, but never required bailout surgery nor resulted in intra- or periprocedural death or myocardial infarction. One patient developed periprocedural stroke after inadvertent carotid artery puncture. At the last available follow-up, overall mortality and MACE were 10.4% and 14.6% respectively. At one, three and five years, mortality rate at Kaplan-Meier analysis was 4%, 13.7%, and 23.4% respectively.CSR implantation is safe and reduces angina in patients with refractory angina.
