  • Circulating Carnosine Dipeptidase 1 associates with weight loss and poor prognosis in gastrointestinal cancer

    • Background: Cancer cachexia (CC) is linked to poor prognosis. Although the mechanisms promoting this condition are not known, several circulating proteins have been proposed to contribute. We analyzed the plasma proteome in cancer subjects in order to identify factors associated with cachexia. Design/Subjects: Plasma was obtained from a screening cohort of 59 patients, newly diagnosed with suspected gastrointestinal cancer, with (n = 32) or without (n = 27) cachexia. Samples were subjected to proteomic profiling using 760 antibodies (targeting 698 individual proteins) from the Human Protein Atlas project. The main findings were validated in a cohort of 93 patients with verified and advanced pancreas cancer. Results: Only six proteins displayed differential plasma levels in the screening cohort. Among these, Carnosine Dipeptidase 1 (CNDP1) was confirmed by sandwich immunoassay to be lower in CC (p = 0.008). In both cohorts, low CNDP1 levels were associated with markers of poor prognosis including weight loss, malnutrition, lipid breakdown, low circulating albumin/IGF1 levels and poor quality of life. Eleven of the subjects in the discovery cohort were finally diagnosed with non-malignant disease but omitting these subjects from the analyses did not have any major influence on the results. Conclusions: In gastrointestinal cancer, reduced plasma levels of CNDP1 associate with signs of catabolism and poor outcome. These results, together with recently published data demonstrating lower circulating CNDP1 in subjects with glioblastoma and metastatic prostate cancer, suggest that CNDP1 may constitute a marker of aggressive cancer and CC.

    • Two of Cassian’s works, the Institutiones and the Collationes, are preserved in both Latin and Greek, and almost all scholars have taken it for granted that he wrote exclusively in Latin. This paper examines two passages from Cassian’s writings, one from the Institutiones (which has a parallel in an Evagrian florilegium) and the other from the Collationes. Arguing against the prevailing view, the paper provides evidence in support of the hypothesis that Cassian originally wrote the Institutiones in Greek. The paper highlights different theological emphases in the Latin and Greek versions and argues that in the translation of Cassian’s works also style, rhetoric, and theology were adapted.

    • This contribution presents a new software, Collectio, which can be used for creating highly complex relational MySQL databases, or more accurately, dynamic libraries. These libraries prove particularly well-suited for texts where the material has been organized in different ways and thus represents a ‘fluid’ textual tradition, or in traditions transmitted in many languages. So far, two libraries have been created using Collectio: APDB (the Apophthegmata Patrum Database) and HIPPO, which contains pre-modern hippiatric material. The sources included in the libraries are mainly in the form of manuscripts, editions and modern translations. Collectio employs a unique input model, built upon .txt and .csv files stored in an archive in the folder of the library. The contents of the database tables in the master database are generated from these documents. Since not only texts are registered but also the detailed structure and parallel text segments in other sources, both texts and structures can be systematically compared and analysed within and across language boundaries. In addition to the advanced research tools for comparing texts and structures, the application contains search options, indexes of names, places and concepts, metadata on the sources, pre-written SQL commands and more. A new way of encoding text, which can be converted into TEI/XML, is also introduced.

    • This volume is a new edition with a parallel English translation of eight Greek stories related to Daniel of Sketis: 1) Abba Daniel from Sketis; 2) Abba Mark the Fool; 3) The blind man; 4AB) A holy and chaste young woman; 5) The woman who pretended to be a drunkard; 6) Eulogios the Stone-cutter; 7) Andronikos the Money-dealer and his wife Athanasia; 8) Anastasia Patrikia. The stories deal with secret saints and holy cross-dressers in sixth-century Egypt. Daniel of Sketis is portrayed as an important monastic authority who knows or discerns secret holiness, and who himself is a secret saint. There are many different redactions of the texts, which are preserved in a great number of MSS. The present edition is based on four related MSS: Scor. R.II.1 (Revilla 21), Par. gr. 919, Mosq. Synod. gr. 345 (Vlad. 342), and Vat. gr. 858. They contain the redactions of two similar Daniel dossiers included in collections of hagiographical material and of the Apophthegmata Patrum. The Introduction discusses, among other things, the diversity of texts mentioning an Abba Daniel associated with Sketis, as well as the theme of secret holiness which is common to all the stories edited here. Details of the contents, as well as textual matters and questions of interpretation and grammar, are discussed in the Commentary. The volume ends with indexes.

    • The effect of a study day on the subject of pain for nurses working at the thorax surgery department The aims of this investigation were: to describe patients' evaluation of pain and the treatment of pain after thorax surgery via sternotomy; to repeat the evaluation with another group of patients following a study day for nurses, featuring pain and pain treatment; and to examine whether the study day influenced the nurses in their treatment of pain. The investigation included daily evaluation of pain using a visual analogue scale (VAS), and an interview with the patients before discharge, where they were asked to review their experience of pain and its treatment. The nurses on the thorax surgery ward and on the intensive care unit (ICU) completed a questionnaire before and after the study day. Finally, a retrospective study of the case notes of the patients taking part was carried out. The results of the investigation showed a low assessment of pain by most patients during the daily evaluation. Asked to recall their pain when interviewed, the rating was higher. A small group of patients had more evident pain than others. When administering opiates the ICU nurses often chose a lower dose than the standing order prescribed. After the study day the nurses gave larger doses of intravenous opioids and the patients experienced less pain.

    • Övergripande syfte Att undersöka kvinnors och mäns smärta efter hjärtkirurgi via sternotomi, med ett större perspektiv på kvinnors smärta samt om en dags utbildning i postoperativ smärta påverkar sjuksköterskors kunskaper och patienters smärta. MetodSåväl kvantitativ som kvalitativ metod har använts.Studie I var en icke-experimentell kvantitativ studie med statistisk analys, och datainsamling före samt tre månader efter utbildningsintervention. I studien ingick daglig smärtskattning med Visuell Analog Skala (VAS), en strukturerad intervju till patienterna för utskrivning för utvärdering av smärtbehandling samt ett frågeformulär om kunskaper och inställning till smärta och smärtbehandling.Studie II är en icke-experimentell och longitudinell studie med en kvalitativ och en kvantitativ del bestående av intervjuer och kvalitativ innehållsanalys, samt uppföljande strukturerad telefonintervju.ResultatSåväl studie I som studie II visar att patienter upplever smärta postoperativt efter hjärtkirurgin och 8% av patienterna (studie I) skattade smärtan som svår. De flesta patienter i såväl studie I som studie II var nöjda med smärtbehandlingen, även vid svår smärta.När patienterna (studie I) skattade smärtan retroaktivt, skattade de smärtan högre jämfört med de dagliga mätningarna, skillnaden var signifikant både före och efter utbildningsinterventionen. Utbildningsinterventionen (studie I) ledde till att patienterna fick större mängd opioider samt en signifikant minskning av smärtan. Sjuksköterskorna från intensivvårds- och vårdavdelningarna fick värdera opioiddoser vilka relaterades till noterade doser enligt patientjournalen. Det framkom att doserna som sjuksköterskorna ansåg att de gav var större jämfört med vad som dokumenterats i journalerna. Doserna enligt journalen var mindre än generella direktiv. I kvinnornas berättelser (studie II) framkom en ovilja gentemot att opioider. Denna ovilja kunde vara generell eller på grund av illamående. I berättelserna var illamåendet ett besvärande symtom, ibland värre än smärtan. Intervjumaterialet visade att kvinnor ansträngde sig för att stå ut med sin smärta genom olika kognitiva eller fysiska strategier. Materialet visade också att inte känt sig respektfullt bemötta.KonklusionStudierna visade att patienter upplever, mild till outhärdlig, smärta efter Thoraxkirurgi via sternotomi. En dags utbildning för sjuksköterskor ledde till signifikant minskning av patienternas skattade smärta. Kvinnorna försökte uthärda svår smärta och det fanns en ovilja gentemot opioider. Illamående var ett betydande problem bland kvinnorna och relaterades ofta till opioidbehandling.   • Chronic pain 1-5 years after lung transplantation

    • Chronic bodily pain after lung transplantation has received little attention. Therefore, the aim was to provide a multidimensional assessment of self-reported chronic pain 1-5 years after lung transplantation and its relationship with self-reported psychological general well-being (PGWB) and self-efficacy. This multicenter, cross-sectional study is a part of the Swedish national study: Self-management after thoracic transplantation (SMATT). In total, 117 lung transplant recipients, all white, due for their yearly follow-up at one (n=35), two (n=28), three (n=23), four (n=20) or five years (n=11) after transplantation were included. Of these, 113 reported their pain on the Pain-O-Meter (POM), which provides information about pain intensity, quality, location, and duration. In addition, they responded to the PGWB instrument and the Self-Efficacy instrument for managing chronic disease. The prevalence of pain was 51% after 1 year, 68 % after 2 years, 69.5 % after 3 years, 75 % after 4 years and 54.5 % after 5 years. Women experienced higher pain intensity and worse sensory and affective burden than men. Psychological general well-being was the main factor that contributed to the experience of pain. Better perceived psychological well-being lowered the odds for pain, while higher self-efficacy reduced the probability of experiencing pain. Many of the lung recipients lacked pain treatment and were uncertain about the reasons behind their pain. Chronic bodily pain is a common and serious symptom up to five years after lung transplantation. Female lung recipients experience more pain and pain related illness than men.

    • RATIONALE AND AIM: Little is known about persistent pain after lung transplantation. Therefore, the aim was to present a multidimensional assessment of self-reported pain 1-5 years after lung transplantation and its relationship with fatigue and transplant-specific well-being.METHODS: This nationwide, cross-sectional cohort study is part of the self-management after thoracic transplantation study. A total of 117 lung recipients, all White, who were due for their annual follow-up at one (n = 35), two (n = 28), three (n = 23), four (n = 20) and 5 years (n = 11) after lung transplantation were included. We used three instruments; the Pain-O-Meter (POM), which provides information about pain intensity, sensation, location and duration, the MFI-19 fatigue instrument and the Organ Transplant Symptom and Well-being Instrument (OTSWI). Permission to carry out this study was granted by the Regional Ethical Review Board in southern Sweden (D-nr 2014-124).RESULTS: The prevalence of pain was 51% after 1 year, 68% after 2 years, 69.5% after 3 years, 75% after 4 years and 54.5% after 5 years. Women experienced more pain than men. Lung recipients with pain reported lower well-being and higher symptom distress but were not more fatigued than those without pain.STUDY LIMITATIONS: The limitations of this study are due to the cross-sectional design. The recruitment of patients during the study period was probably affected by the different conditions regarding staffing at the outpatient lung transplant clinic in the two thoracic transplant centres in Sweden. The slightly different approach to the care of these patients in the pre, peri and postoperative setting contributes to the heterogeneity of the study population.CONCLUSION: Chronic bodily pain up to 5 years after lung transplantation reduces perceived well-being. Lung recipients with pain report higher symptom distress than those without pain.

  • Essentials of Nursing Care in Randomized Controlled Trials of Nurse-Led Interventions in Somatic Care : A Systematic Review

    • Background: Nursing practice has to contribute to evidence pointing out why there is a need for more nurse-designed randomized control trials (RCTs) focusing on evidence-based practice (EBP). How far this EBP has progressed in different health aspects is usually established by systematic reviews of RCTs. Nurse-led RCTs exist but no study has addressed the essentials of nursing care. Aim: The aim was therefore to determine the essentials of nurses’ interventions by means of nurse-led RCTs in somatic care focusing on the stated context, goals, content, strategies as well as the nurse’s role related to effectiveness. Methods: A systematic review was realized according to Cochrane review assumptions to identify, appraise and synthesize all empirical evidence meeting pre-specified eligibility criteria. The PRISMA statement guided the data extraction process (n = 55) from PubMed and CINAHL. Results: Of the RCTs in somatic care, 71% showed a positive effectiveness of nurse-led interventions, of which the nurse had a significant role with regard to being the main responsible in 67% of the studies. Also, 47% of the RCTs presented a theoretical standpoint related to the nurse-led interventions and most prominent were international evidence-based guidelines. Goals were found to have either a patient-centered or a professional-centered ambition. Strategies were based on patient-directed initiatives, nurse-patient-directed initiatives or nurse-directed initiatives, while contents were built upon either a patient-nurse interaction or a nursing management plan. Conclusions: This review underlines the necessity of a holistic view of a person, as nurse-led RCTs comprising a patient-centered ambition, patient-directed initiative and patient-nurse interaction plan showed beneficial nursing care effectiveness, particularly if theory-based. In a nurse-led RCT, a basic theoretical perspective is advantageous as well as to elucidate the role of the nurse in relation to the estimated effects.

    • Background: Teamcare should, like all patient care, also contribute to evidence-based practice (EBP). Randomized controlled trials (RCTs) focusing on teamcare have been performed but no study has addressed its essentials. How far this EBP has progressed in different health aspects is generally established in systematic reviews of RCTs.Aim: The aim is to determine the essentials of teamcare including the nurse profession in RCTs of multi- or interdisciplinary interventions in somatic care focusing on the stated context, goals, strategies, content as well as effectiveness of quality of care.Methods: A systematic review was performed according to Cochrane review assumptions to identify, appraise and synthesize all empirical evidence meeting pre-specified eligibility criteria. The PRISMA statement guided the data selection process of 27 articles from PubMed and CINAHL.Results: Eighty-five percent of RCTs in somatic care showed a positive effectiveness of teamcare interventions, of which interdisciplinary ones showed a greater effectiveness compared with the multidisciplinary approach (100% vs 76%). Also theory-based RCTs presented higher positive effectiveness (85%) compared with non-theory-based RCTs (79%). The RCTs with positive effectiveness showed greater levels for professional-centered ambition in terms of goals and for team-directed initiatives in terms of strategy, and a significantly higher level for patient-team interaction plans in terms of content was shown.Conclusions: Teamcare RCTs are still grounded in the multidisciplinary approach having a professional-centered ambition while interdisciplinary approaches especially those that are theory-based appear to be essential with regard to positive effectiveness and preferable when person-centered careis applied.

    • Collections of sayings of the desert fathers and mothers are extant in manuscripts in many languages and are organized differently. They are ‘fixed-content miscellanies’ (FCM): they include material that belongs to the same genre, but is variable both when it comes to appearance and order. Distance measurement methods are particularly suitable for large text traditions including variable content in the so-called mixed-content miscellanies, such as recipes, anthological compilations of shorter text passages, or catalogues, but can also be suitable for text genres like collections of sayings, that are equally variable in appearance and order of sayings, even though the genre is fixed; hence ‘fixed-content miscellanies’. In the article, collections of sayings in seven languages were compared using four distance measures methods. Each segment of the sayings was given a unique id to be comparable. The first method used, the Jaccard distance measure, disregards the linear order of items and instead considers each collection compared only as a ‘bag of stories’. In two other methods used (Birnbaum and Levenshtein methods), the order in which the narratives of each saying appear is compared. All three methods yielded interesting results, but the collections that were apparently closely related were clustered together so tightly that it was not possible to make more nuanced analyses. In order to remove false negatives, particulars concerning lacunes in the material were taken into account in the proposed modified Levenshtein method, the fixed-content miscellanies (FCM)-Levenshtein method. By applying the FCM-Levenshtein method, previously unknown relations between collections witnessed in different languages could be detected.

    • Background: Repetitive and stressful work tasks have been linked to the development of pain in the trapezius muscle, although the underlying mechanisms still remain unclear. In earlier studies, it has been hypothesized that chronic muscle pain conditions are associated with imbalance in the autonomic nervous system, predominantly expressed as an increased sympathetic activity. This study investigates whether women with chronic trapezius myalgia show higher muscle activity and increased sympathetic tone at baseline and during repetitive low-force work and psychosocial stress, compared with pain-free controls. Methods: Eighteen women with chronic trapezius myalgia (MYA) and 30 healthy female controls (CON) were studied during baseline rest, 100 min of repetitive low-force work, 20 min of psychosocial stress (Trier Social Stress Test, TSST), and 80 min recovery. The subjects rated their pain intensity, stress and energy level every 20 min throughout the experiment. Muscle activity was measured by surface electromyography in the trapezius muscle (EMGtrap) and deltoid muscle (EMGdelt). Autonomic reactivity was measured through heart rate (HR), skin conductance (SCL), blood pressure (MAP) and respiration rate (Resp). Results: At baseline, EMGtrap, stress ratings, and HR were higher in MYA than in CON. Energy ratings, EMGdelt, SCL, MAP and Resp were, however, similar in the two groups. Significant main group effects were found for pain intensity, stress ratings and EMGtrap. Deltoid muscle activity and autonomic responses were almost identical in MYA and CON during work, stress and recovery. In MYA only, pain intensity and stress ratings increased towards the end of the repetitive work. Conclusion: We found increased muscle activity during uninstructed rest in the painful muscle of a group of women with trapezius myalgia. The present study could not confirm the hypothesis that chronic trapezius myalgia is associated with increased sympathetic activity. The suggestion of autonomic imbalance in patients with chronic local or regional musculoskeletal pain needs to be further investigated.

    • This study investigated differences in HPA axis function, measured as salivary cortisol concentrations, between 18 women with chronic trapezius myalgia (MYA) and 30 healthy female controls (CON). In addition, the interactions between HPA axis reactions to psychosocial stress and aspects of pain, health and psychological symptoms were analyzed. Salivary cortisol was measured both in daily life, to assess the circadian profile, and in the laboratory during light repetitive work and standardized psychosocial stress (Trier Social Stress Test, TSST). MYA and CON exhibited similar circadian rhythms and comparable salivary cortisol response magnitudes after TSST. In subjects defined as responders to the TSST, the mean peak time point of the cortisol response after TSST differed significantly between MYA and CON. Furthermore, negative psychological states and higher pain intensity were related to a slower HPA axis response to TSST. Low circadian variations in cortisol and smaller cortisol responses to TSST were found among subjects scoring high on anxiety sensitivity. Thus, a relatively favorable sample of female chronic trapezius myalgia patients exhibited normal circadian rhythm and normal salivary cortisol response magnitudes after a psychosocial stress test. In the subgroup of responders, the MYA group showed indications of a slower salivary cortisol response to psychosocial stress. Further studies are needed to elucidate the possibility of altered HPA axis activity in terms of a slower salivary cortisol response. (c) 2009 Elsevier Ltd. All rights reserved.
