    • Six different lipid matrices (the intact lipid (IL), four lipid fractions with different polarity, and the free fatty acids (FFAs) obtained by hydrolysis of the triacylglycerol (TAG) containing fraction) were obtained from salmon (Salmo salar) and eel (Anguilla anguilla), each collected at a contaminated and a comparatively uncontaminated catch site along the coast of Scandinavia. The lipid matrices were studied in toxicological test systems representing various biological functions of different organ systems from several species and trophic levels. The results were evaluated with particular respect to the concentrations of extractable organically bound chlorine (EOCl) in the matrices tested. In some test systems, the specimens with a higher EOCl concentration appeared to be more toxic. For example, the TAG containing fraction (F2) from Idefjord eel, having a higher EOCl content than F2 from Oslofjord eel, reduced the number and hatchability of eggs laid by zebrafish. Both IL and F2 of Idefjord eel increased mortality and reduced the oxygen/nitrogen-ratio in blue mussels. Non-polar compounds (F1) from Bothnian Sea salmon induced 7-ethoxyresurofin O-deethylase (EROD) activity in rainbow trout hepatocytes, whereas F1 from Senja salmon did not. F1 from Bothnian Sea salmon also reduced the number of T-cells in foetal mouse thymus anlagen in vitro compared with the cell number in anlagen exposed to F1 from Senja salmon. A positive correlation between EOCl concentration and test response was found for EROD activity in rainbow trout hepatocytes and for ATP-leakage in Erlich ascites tumour cells when testing the phospolipid containing fraction (F4). However, in most test systems the fish oils, irrespective of EOCl content, were of low toxicity, and the observed effects need to be verified in future studies.

  • How much synthetic oxytocin is infused during labour? A review and analysis of regimens used in 12 countries

    • Objective To compare synthetic oxytocin infusion regimens used during labour, calculate the International Units (IU) escalation rate and total amount of IU infused over eight hours. Design Observational study Setting Twelve countries, eleven European and South Africa. Sample National, regional or institutional-level regimens on oxytocin for induction and augmentation labour Methods Data on oxytocin IU dose, infusion fluid amount, start dose, escalation rate and maximum dose were collected. Values for each regimen were converted to IU in 1000ml diluent. One IU corresponded to 1.67 mu g for doses provided in grams/micrograms. IU hourly dose increase rates were based on escalation frequency. Cumulative doses and total IU amount infused were calculated by adding the dose administered for each previous hour. Main Outcome Measures Oxytocin IU dose infused Results Data were obtained on 21 regimens used in 12 countries. Details on the start dose, escalation interval, escalation rate and maximum dose infused were available from 16 regimens. Starting rates varied from 0.06 IU/hour to 0.90 IU/hour, and the maximum dose rate varied from 0.90 IU/hour to 3.60 IU/hour. The total amount of IU oxytocin infused, estimated over eight hours, ranged from 2.38 IU to 27.00 IU, a variation of 24.62 IU and an 11-fold difference. Conclusion Current variations in oxytocin regimens for induction and augmentation of labour are inexplicable. It is crucial that the appropriate minimum infusion regimen is administered because synthetic oxytocin is a potentially harmful medication with serious consequences for women and babies when inappropriately used. Estimating the total amount of oxytocin IU received by labouring women, alongside the institution's mode of birth and neonatal outcomes, may deepen our understanding and be the way forward to identifying the optimal infusion regimen.

    • Background: The aim of this 12-month study in pre-pubertal children was to evaluate the effect of school transportation on gain in lean and fat mass, muscle strength and physical performance. Methods: Ninety-seven girls and 133 boys aged 7-9 years from the Malmö Pediatric Osteoporosis Prevention Study were included. Regional lean and fat mass were assessed by dual energy X-ray absorptiometry, isokinetic peak torque of knee extensors and flexors by a computerised dynamometer and physical performance by vertical jump height. Level of physical activity was assessed by accelerometers. The 12-month changes in children who walked or cycled to school were compared with changes in those who travelled by bus or car. Results: There were no differences in baseline or annual changes in lean or fat mass gain, muscle strength or physical performance between the two groups. All children reached the internationally recommended level of 60 minutes per day of moderate or high physical activity by accelerometers. Conclusion: The choice of school transportation in pre-pubertal children seems not to influence the gain in lean and fat mass, muscle strength or functional ability, probably as the everyday physical activity is so high that the mode of school transportation contributes little to the total level of activity.

  • Outcome measures in studies on the use of oxytocin for the treatment of delay in labour: A systematic review

    • Objectives: to identify primary and secondary outcome measures in randomised trials, and systematic reviews of randomised trials, measuring effectiveness of oxytocin for treatment of delay in the first and second stages of labour, and to identify any positive health-focussed outcomes used. Design: eight relevant citation databases were searched up to January 2013 for all randomised trials, and systematic reviews of randomised trials, measuring effectiveness of oxytocin for treatment of delay in labour. Trials of active management of labour or partogram action lines were excluded. 1918 citations were identified. Two reviewers reviewed all citations and extracted data. Twenty-six individual trials and five systematic reviews were included. Primary and secondary outcome measures were documented and analysed using frequency distributions. Findings: most frequent primary outcomes were caesarean section (n=15, 46%), length of labour (n=14, 42%), measurements of uterine activity (n=13, 39%) and mode of vaginal birth (n=9, 27%). Maternal satisfaction was identified a priori by one review and included as a secondary outcome by three papers. No further positive health-focussed outcomes were identified. Key conclusions: outcomes used to measure the effectiveness of oxytocin for treatment of delay in labour are heterogeneous and tend to focus on adverse events. Implications for practice: it is recommended that, in future randomised trials of oxytocin use for delay in labour, some women-centred and health-focussed outcome measures should be used, which may instil a more salutogenic culture in childbirth. © 2014 The Authors.

    • Fibroblast growth factor 23 (FGF23) is a phosphaturic hormone that in recent years has been reported to have significant effects on numerous tissues. Chronic obstructive pulmonary disease (COPD) is associated with hypophosphatemia but the evidence for elevated plasma levels of FGF23 in COPD subjects is ambiguous. Recently, FGF23 has even been shown to be involved in the inflammatory pathways activated in COPD, so FGF23 could be a novel biomarker for COPD and impairment of pulmonary function. The purpose was thus to explore the association of FGF23 with COPD and measures of pulmonary function. This was a cross sectional study of 450 subjects who underwent spirometry, body plethysmography, determination of diffusing capacity (DL,CO) and biomarker analysis of FGF23, interleukin (IL)-1 receptor antagonist, IL-6 and IL-8. Forty-four participants were excluded due to missing data or renal impairment (eGFR <45 mL/min/m2). Spirometry identified 123 subjects with COPD. FGF23 levels were elevated in COPD subjects compared to non COPD subjects, and this remained significant after adjustment for age, sex and smoking habits (OR = 1.6, p = 0.02). Linear regression showed significant relationships between FGF23 and FEV1 (β = −0.15, p = 0.003), RV/TLC (β = 0.09, p = 0.05) and DL, CO (β = −0.24, p < 0.001). In conclusion we found that plasma levels of FGF23 are elevated in COPD subjects even when adjusting for traditional risk factors. Furthermore, FGF23 is associated with impairment in lung function as measured by FEV1 and DL,CO. Further studies are needed to establish whether FGF23 could serve as a novel biomarker of COPD and emphysema development.

  • Matrix Metalloproteinases in COPD and atherosclerosis with emphasis on the effects of smoking

    • Background Matrix metalloproteinases (MMP´s) are known biomarkers of atherosclerosis. MMP´s are also involved in the pathophysiological processes underlying chronic obstructive pulmonary disease (COPD). Cigarette smoking plays an important role in both disease states and is also known to affect the concentration and activity of MMP´s systemically. Unfortunately, the epidemiological data concerning the value of MMP´s as biomarkers of COPD and atherosclerosis with special regards to smoking habits are limited. Methods 450 middle-aged subjects with records of smoking habits and tobacco consumption were examined with comprehensive spirometry, carotid ultrasound examination and biomarker analysis of MMP-1, -3, -7, -10 and -12. Due to missing data 33 subjects were excluded. Results The remaining 417 participants were divided into 4 different groups. Group I (n = 157, no plaque and no COPD), group II (n = 136, plaque but no COPD), group III (n = 43, COPD but no plaque) and group IV (n = 81, plaque and COPD). Serum levels of MMP-1,-7,-10-12 were significantly influenced by smoking, and MMP-1, -3, -7 and-12 were elevated in subjects with COPD and carotid plaque. This remained statistically significant for MMP-1 and-12 after adjusting for traditional risk factors. Conclusion COPD and concomitant plaque in the carotid artery were associated with elevated levels of MMP-1 and -MMP-12 even when adjusting for risk factors. Further studies are needed to elucidate if these two MMP´s could be useful as biomarkers in a clinical setting. Smoking was associated with increased serum levels of MMP´s (except for MMP-3) and should be taken into account when interpreting serum MMP results.

    • Introduction: Long-term intense training leads to structural, functional, and electrical remodeling of the heart. How different sports affect the heart has not been fully investigated, particularly for female athletes. The aim of the present study was to investigate the morphology of 12-lead resting electrocardiogram (ECG) in elite female handball players compared to non-athlete female subjects. Potential changes will be explored to see if they could be explained by differences in cardiac dimensions and exercise hours.Materials and methods: A cross-sectional study of 33 elite female team handball players compared to 33 sex and age-matched, non-athletic controls (age range 18-26 years) was performed. All participants underwent a resting 12-lead ECG and an echocardiographic examination. ECG variables for left ventricular hypertrophy and durations were evaluated and adjusted for cardiac dimensions and exercise hours using ANCOVA analysis. A linear regression analysis was used to describe relation between echocardiographic and ECG measures and exercise hours.Results: The female handball players had larger cardiac dimensions and significantly lower heart rate and QTc duration (Bazett's formula) as well as increased QRS and QT durations compared to controls. The 12-lead sum of voltage and the 12-lead sum of voltage & lowast; QRS were significantly higher among handball players. Changes in ECG variables reflecting the left ventricle could in part be explained by left ventricular size and exercise hours. Correlation with exercise hours were moderately strong in most of the echocardiographic measures reflecting left ventricular (LV), left ventricular mass (LVM), left atrium (LA) and right atrium (RA) size. Poor to fair correlations were seen in the majority of ECG measures.Conclusions: Female team handball players had altered ECGs, longer QRS and QT durations, higher 12-lead sum of voltage and 12-lead sum of voltage & lowast; QRS as well as shorter QTc (Bazett's formula) duration compared to non-athletic controls. These findings could only partly be explained by differences in left ventricular size. Despite larger atrial size in the athletes, no differences in P-wave amplitude and duration were found on ECG. This suggest that both structural, and to some degree electrical remodeling, occur in the female team handball players' heart and highlight that a normal ECG does not rule out structural adaptations. The present study adds knowledge to the field of sports cardiology regarding how the heart in female team handball players adapts to this type of sport.

    • Summary: The study investigates the effect of physical activity (PA) on a composite score for fracture risk in pre-pubertal children. Low PA in children is related to the composite score for fracture risk and the pre-pubertal years seem to be a period when PA positively affects the score. Introduction: This study evaluates if PA in children is related to clustering of risk factors for fracture. Research questions are the following: (i) What is the effect of physical activity (PA) on single traits and a composite score for fracture risk? (ii) Could this score be used to identify the level of PA needed to reach beneficial effects? Methods: This prospective population-based study included 269 children, aged 7–9 years at baseline while 246 attended the 2-year follow-up. We estimated duration of PA by questionnaires and measured traits that independently predict fractures. We then calculated gender specific Z-scores for each variable. The mean Z-score of all traits was used as a composite score for fracture risk. We tested correlation between duration of PA, each trait, and the composite score and group differences between children in different quartiles of PA. Results: At baseline, we found no correlation between duration of PA and any of the traits or the composite score. At follow-up, we found a correlation between PA and the composite score. Physical activity had an effect on composite score, and children in the lowest quartiles of PA had unbeneficial composite score compared to children in the other quartiles. Conclusion: Low PA in children is related to clustering of risk factors for fracture, and the pre-pubertal years seem to be a period when PA positively affects the composite score.

    • Polybrominated diphenyl ethers (PBDEs) are commonly used flame retardants in various consumer products. Pre- and postnatal exposure to congeners of PBDEs disrupts normal brain development in rodents. Two-dimensional difference gel electrophoresis (2D-DIGE) was used to analyze concentration-dependent differences in protein expression in cultured cortical cells isolated from rat fetuses (GD 21) after 24 h exposure to PBDE-99 (3, 10, or 30 muM). Changes on a post-translational level were studied using a 1 h exposure to 30 muM PBDE-99. The effects of 24 h exposure to 3 and 30 muM PBDE-99 on mRNA levels were measured using oligonucleotide microarrays. A total of 62, 46, and 443 proteins were differentially expressed compared to controls after 24 h of exposure to 3, 10, and 30 muM PDBE-99, respectively. Of these, 48, 43, and 238 proteins were successfully identified, respectively. We propose that the biological effects of low-concentration PBDE-99 exposure are fundamentally different than effects of high-concentration exposure. Low-dose PBDE-99 exposure induced marked effects on cytoskeletal proteins, which was not correlated to cytotoxicity or major morphological effects, suggesting that other more regulatory aspects of cytoskeletal functions may be affected. Interestingly, 0.3 and 3 muM, but not 10 or 30 muM increased the expression of phosphorylated (active) Gap43, perhaps reflecting effects on neurite extension processes.

    • Recent studies have indicated that diets rich in digestible carbohydrates and dietary fibre might be beneficial in the regulation of type II non insulin dependent diabetes (NIDD). Addition of the gel forming type of dietary fibre such as pectin and guar gum to meals or glucose solutions reduces post-prandial glucose and insulin response. Addition of cereal fibres in the form of bran seems to have long term beneficial effect improving glucose tolerance. Little is known, however, concerning effects of dietary fibre naturally occurring in food on postprandial glucose and hormone response. In the present study we prepared two breakfast meals which were similar regarding digestible carbohydrates but differed in their dietary fibre content. One of the meals, including whole grain bread and whole apples, contained 8.4 g of dietary fibre, and the other one, containing white bread and apple juice, 3.1 g. When given to eight NIDD, the fibre rich breakfast gave significantly lower blood glucose increment during the three hours following ingestion. The results indicate that foods rich in dietary fibre might be useful in the regulation of type II diabetes.

    • Marine borers constitute a great danger to historical shipwreck in marine environments as they are able to decompose wood material in just a few years. Recently, there have been indications that the marine borer Teredo navalis is spreading into the brackish Baltic sea, where thousands of invaluable historical wrecks for centuries have had unique preservation conditions. The WreckProtect project was a coordination and support action funded by the European Commission within the 7th Framework Program. The main objective of the project was to develop tools for predicting the spread of marine borers into the Baltic and to evaluate methods for in situ protection of the historical wreck and submerged settlements. This paper gives a summary final report of the project and an overview of results.

    • Aim:Aerobic capacity, defined as peak oxygen uptake (VO2PEAK), is generally considered to be the best single marker for aerobic fitness. We assessed if VO2PEAK is related to different cardiac dimensions in healthy young children on a population base. Methods: In a cross-sectional study, 245 children (137 boys and 108 girls) aged 8-11 years, were recruited from a population based cohort. VO2PEAK (ml/min-1/kg-1) was assessed by indirect calorimetry during a maximal exercise test. DXA-scan was used to measure lean body mass (LBM) and total fat mass (TBF). Echocardiography, with 2-dimensional guided M-mode, was performed in accordance with current guidelines. Left ventricular end-diastolic diameter (LVDD) and left atrial end-systolic diameter (LA) were measured, and left ventricular mass (LVM) was calculated. Results: Univariate correlations were found between VO2PEAK versus LVDD r=0.44 and LA r=0.27 (both P<0.05) and LVM r=-0.06 (NS) in boys. Corresponding values for girls were; 0.55, 0.34 (both P<0.05) and 0.11 (NS). Multiple regression analysis with VO2PEAK as dependent variable and inclusion of LBM, TBF, sex, age, Tanner stage, and maximal heart rate as independent variables showed that 67% of the total variance of VO2PEAK could be explained by these variables. Including LVDD or LA in the model, added 1% additional explained variance. Conclusion: Findings from this population based cohort of young healthy children show that multiple cardiac dimensions at rest are related to VO2PEAK. However, the different cardiac dimensions contributed very little to the added explained variance of VO2PEAK.

    • This study evaluates associations between objectively measured daily physical activity vs aerobic fitness and body fat in children aged 8-11 years. A cross-sectional study of 225 children aged 7.9-11.1 years was performed. Abdominal fat mass (AFM) and total body fat (TBF) were quantified by dual-energy x-ray absorptiometry. TBF was calculated as percentage of total body mass (BF%). Body fat distribution was calculated as AFM/TBF. Aerobic fitness was measured by indirect calorimetry during a maximal cycle ergometer exercise test. Daily physical activity was assessed by accelerometers for 4 days and daily accumulation of moderate-to-vigorous and vigorous activity was calculated. Significant relationships (P<0.05) existed for vigorous activity vs ln BF% (r=-0.40), ln AFM (r=-0.35), TBF/AFM (r=-0.22) and aerobic fitness (r=0.38), whereas moderate-to-vigorous activity displayed weaker relationships (-0.22, -0.18, -0.12 NS, and 0.25). Multiple regression analyses with inclusion of possible confounders concluded that vigorous activity was independently related to aerobic fitness and ln BF% or ln AFM. Moderate-to-vigorous activity was only independently related to aerobic fitness. In this population, low daily accumulation of vigorous activity was, already in children aged 8-11 years, associated with more body fat and lower aerobic fitness. A similar relation was not found for daily accumulation of moderate-to-vigorous activity.

    • Objective To evaluate the association between objectively measured daily Study design Cross-sectional, observational, study of 248 children aged 7.9 to 11.1 years. Abdominal fat mass and total body fat mass were measured by dual-energy X-ray absorptiometry. Daily physical activity was assessed by accelerometers for 4 days. Results Total body fat expressed as a percentage of body mass was inversely related to minutes of vigorous physical activity per day, for all children r = -0.38 (P < .05). Children, both boys and girls, in the highest quartile of body fat performed on average 12 minutes less vigorous activity per day compared with their counterparts in the lowest quartile. Multiple regression analysis revealed that independent factors for body fat were number of minutes of vigorous activity per day and sex. Conclusion Low physical activity can be a contributing factor in childhood obesity. Only longitudinal studies, however, can give more definitive information about the relation between daily physical activity and obesity.

    • Training studies in children have suggested that endurance training can give enlargement of cardiac dimensions. This relationship has not been studied on a population-based level in young children with objective methods. A cross-sectional study was made of 248 children (140 boys and 108 girls), aged 8-11 years, from a population-based cohort. Left ventricular end-diastolic diameter (LVDD) and left atrial end-systolic diameter (LA) were measured with echocardiography and indexed for body surface area (BSA). Physical activity was assessed by accelerometry, and the duration of vigorous physical activity per day (VPA) was calculated. Acceptable accelerometer and echocardiography measurements were obtained in 228 children (boys=127, girls=101). Univariate correlations between VPA and LVDD were indexed for BSA in boys (r=0.27, P<0.05) and in girls (r=0.10, NS). Multiple regression analysis showed that independent factors for LVDD, indexed for BSA for boys, were age and VPA. LA indexed for BSA was not related to physical activity variables in either gender. No clear relationship exists between cardiac size and daily physical activity in children aged 8-11 years. This suggests that significant cardiac remodelling due to volume exposure secondary to a high amount of physical activity begins later in life.

    • The pseudo-spectra (or spectral instability) of non-selfadjoint operators is a topic of current interest in applied mathematics. For example, in computational fluid dynamics it affects the study of the stability of laminar flows. In fact, even for the most basic flows, the computations entirely fails to predict what is observed in the experiments. The explanation is that for non-normal operators the resolvent could be very large far away from the spectrum, which makes computation of the eigenvalues impossible. The occurence of ``false eigenvalues'' is due to the existence of quasi-modes, i.e., approximate local solutions to the eigenvalue problem. The quasi-modes appear since the Nirenberg-Treves condition (Psi) is not satisfied for topological reasons.

    • The paper studies the local solvability and subellipticity for square systems of principal type. These are the systems for which the principal symbol vanishes of first order on its kernel. For systems of principal type having constant characteristics, local solvability is equivalent to condition (Ψ) on the eigenvalues. This is a condition on the sign changes of the imaginary part along the oriented bicharacteristics of the real part of the eigenvalue. In the generic case when the principal symbol does not have constant characteristics, condition (Ψ) is not sufficient and in general not well defined. Instead we study systems which are quasi-symmetrizable, these systems have natural invariance properties and are of principal type. We prove that quasi-symmetrizable systems are locally solvable. We also study the subellipticity of quasi-symmetrizable systems in the case when principal symbol vanishes of finite order along the bicharacteristics. In order to prove subellipticity, we assume that the principal symbol has the approximation property, which implies that there are no transversal bicharacteristics.

    • The relationship between birth quarter distribution and physiological characteristics related to athletic skills, in adolescent sport academy students has not been fully investigated. In a cross-sectional study, we recruited 86 boys and 52 girls aged 12-14 years during their first term at a sport academy school. We measured body size, cardiac size, pulmonary function, body composition, lower body power, cardiorespiratory fitness parameters, and running endurance by standard methods and analyzed these estimates in relation to birth quarter by ANOVA. Birth quarter distribution in our cohort was compared with birth quarter distribution in the same ages in the whole of Sweden and analyzed by logistic regression. The academy had an overrepresentation of students born in the first quartile of the year compared to those born in the last quartile (odds ratio 2.3 (95% CI: 1.1-4.7)). When comparing the physiological characteristics between birth quarters, uniformity is prominent since out of 26 performed physiological and anthropometric tests only four showed statistically significant group differences. We thus believe that the selection process to the sport academy favours athletes with higher chronological age, i.e., a so-called relative age effect is present. © 2020 MDPI (Basel, Switzerland).

    • Summary Background: Real-time perfusion (RTP) adenosine stress echocardiography (ASE) can be used to visually evaluate myocardial ischaemia. The RTP power modulation technique, provides images for off-line parametric perfusion quantification using Qontrast((R)) software. From replenishment curves, this generates parametric images of peak signal intensity (A), myocardial blood flow velocity (beta) and myocardial blood flow (Axbeta) at rest and stress. This may be a tool for objective myocardial ischaemia evaluation. We assessed myocardial ischaemia by RTP-ASE Qontrast((R))-generated images, using 99mTc-tetrofosmin single-photon emission computed tomography (SPECT) as reference. Methods: Sixty-seven patients admitted to SPECT underwent RTP-ASE (SONOS 5500) during Sonovue((R)) infusion, before and throughout adenosine stress, also used for SPECT. Quantitative off-line analyses of myocardial perfusion by RTP-ASE Qontrast((R))-generated A, beta and Axbeta images, at different time points during rest and stress, were blindly compared to SPECT. Results: We analysed 201 coronary territories [corresponding to the left anterior descendent (LAD), left circumflex (LCx) and right coronary (RCA) arteries] from 67 patients. SPECT showed ischaemia in 18 patients. Receiver operator characteristics and kappa values showed that A, beta and Axbeta image interpretation significantly identified ischaemia in all territories (area under the curve 0.66-0.80, P = 0.001-0.05). Combined A, beta and Axbeta image interpretation gave the best results and the closest agreement was seen in the LAD territory: 89% accuracy; kappa 0.63; P<0.001. Conclusion: Myocardial isachemia can be evaluated in the LAD territory using RTP-ASE Qontrast((R))-generated images, especially by combined A, beta and Axbeta image interpretation. However, the technique needs improvements regarding the LCx and RCA territories.

Cohen's d effect sizes, paired t-tests and ANOVA were used to study differences between conditions. A mixed linear regression model analyzed the relationship between heart rate (HR) and oxygen uptake (V̇O2) with load and between sexes.ResultsWearing CG resulted in, for both sexes, a decreased time to exhaustion (−11 min), lower V̇O2peak (L/min) ES = 0.56; VO2peak (mL//kg/min) ES = 2.44, both p < 0.001, a net decrease in minute ventilation (ES = 3.53) and no change in HRmax. No sex-difference were present except for absolute V̇O2peak. The VO2 and HR relationship showed a cardiorespiratory reduction wearing CG vs. NL. Added load was equal between sexes, although female soldiers' CG relative to body mass was higher (25%) than male soldiers’ (20%), p < 0.01.ConclusionWearing CG reduces soldiers’ cardiorespiratory capacity and exercise performance level, although the reduction cannot be explained solely based on the added load of CG, instead CG seems to restrict the capacity to fully ventilate. No sex differences were found in relative cardiorespiratory responses to wearing CG compared to NL.

    • Introduction: Previous studies have shown that risk of physical fatigue increases if prolonged average work intensity exceeds 50% of oxygen uptake (VO2). In order to avoid persistent fatigue in a work setting, it is important to conduct suitable work capacity analyses. In physically demanding jobs where wearing protective gear and/or external load is mandatory, monitoring of cardiovascular demands through heart rate (HR) is one way to track the workers’ relative effort.  There are limited studies examining effects of load carrying on cardiovascular capacity where it appears that VO2peak differ when soldiers and firefighters are tested with work-related equipment/clothing compared to light clothing. The aim of this pilot study was to investigate effects of load (combat gear) on HR, VO2 and muscle oxygen saturation (SmO2) compared to no load in soldiers during a graded treadmill protocol.Methods: Eight volunteer army soldiers (1 woman, 7 men) performed a graded treadmill test until exhaustion. All soldiers performed the test twice, once with light clothes and no load (NL) and once with added load, their personal combat gear (CG), with at least 48 h between sessions. The treadmill protocol stages included supine and standing positions, followed by marching speeds of 5.4 km/h and 8 km/h at 0 incline (all 5 min) immediately followed by a set marching speed of 8 km/h with a starting incline of 2 % increasing the incline 2 % every third minute until voluntary exhaustion. Measurements of HR, VO2 and SmO2 were collected continuously and the last 30 s of each stage were averaged and used for statistical analyses (paired t-tests).Results: The mean added load for all soldiers with CG was 16.8 ± 1.1 kg. All soldiers completed at least 6 stages (range 6-11 stages) with both NL and CG, where time to exhaustion with NL was longer (19.1 ± 3.2 min) compared to CG (9.1 ± 2.9 min; p <0.01). Submaximal HR and VO2 were both significantly higher with CG compared to NL (at absolute intensities) at all marching speeds all soldiers completed (5.4 km/h 0 % grade - 8 km/h 4% grade; all p<0.05). For SmO2, marching with CG compared to NL resulted in increased muscle oxygen utilization, at submaximal stages 8 km/h 0% -4% grade (all p<0.05). For values at maximal effort the CG had a significantly lower VO2peak (3.7 ± 0.5 L/min) compared to NL (4.1 ± 0.6 L/min, p <0.01), whereas there was no difference in HRpeak or the lowest value of SmO2 between CG (193.1 ± 7.2 bpm; 42.4 ± 30.3%) and NL (195.4 ± 8.9 bpm; 47.0 ± 29.2%).Conclusions: This pilot study suggests that assessment of aerobic capacity in soldiers should be conducted with combat gear to help determine their actual work capacity during combat and other load carrying tasks. These results suggest that if soldiers’ work performance is determined without added load it overestimates their aerobic capacity (VO2peak) in tasks wearing combat gear, which might lead to added fatigability and deleterious effect on performance.

    • Purpose: The aim of the present study was to identify the most physically demanding work tasks for Swedish ground combat soldiers through the development and application of a questionnaire survey. This is the first in a series of studies aiming to describe the development process and validation of physical selection standards in the Swedish armed forces.Methods: Based on procedural documentation, combat manuals and job analyses, a questionnaire was developed that defined and rated the perceived physical strain of 30 work tasks for ground combat soldiers. To assess validity, an expert focus group was used and psychometric analysis performed. The questionnaire was then distributed to 231 ground combat soldiers, of whom 165 responded (71%).Results: The questionnaire was validated in three steps to achieve face and content validity, and internal consistency was acceptable (Chronbach's alpha ≥0.95). Of the 30 work tasks included in the survey, transport of wounded was rated as the most demanding task for both aerobic capacity and strength. Other highly demanding tasks for aerobic capacity included combat movement (low/high crawl), dismounted attack in close country, urban and rough terrain and carrying heavy loads. There were no gender differences for either aerobic or strength demands in the top five most challenging tasks based on proportions.Conclusions: This study identified the most physically demanding tasks performed in the Swedish ground combat forces. Almost all the physically demanding tasks found in the present study contain elements of lifting and carrying, which require muscular strength and muscular endurance, with no gender differences. © 2019 Elsevier Ltd.     • INTRODUCTION: Military tasks place considerable physiological demands on the soldier. It is therefore important to know the energy expenditure of soldiers while solving tasks in different environments. The purpose of this study was to describe the cardiorespiratory demands of certain movements and activities on ground combat soldiers during military field operations using body sensors and simulated combat.MATERIALS AND METHODS: Movement characteristics and cardiorespiratory responses were assessed in 42 soldiers (three women) in the Swedish Army. The different posts assessed were commander, combat engineer, driver, and gunner. The military field exercises examined were urban operations and retrograde operations in rough terrain. Measurements included (1) body mass, (2) heart rate (HR) including maximal (HRmax), (3) velocity, (4) accelerations/decelerations, and (5) distance moved. Maximal aerobic capacity (V̇O2peak, mL·kg-1·min-1) was tested in a laboratory setting when wearing combat gear and body armor.RESULTS: There was a weak positive correlation (r = 0.41 and 0.28, both P < .05) between VO2peak and percentage of time over 40% and 50% of maximal aerobic capacity during simulated combat. No differences were found for the different posts in time spent over 40% or 50% of maximal aerobic capacity and 76% of their HRmax (P > .05). Wearing combat gear and additional load while solving tasks resulted in mean HR varying between 98 and 111 beats·min-1, corresponding with 50-57% of the soldiers HRmax. Studying all exercises, mean HR was 105 ± 11 beats min-1, 54 ± 5% of HRmax corresponding to light work intensity. Soldiers performed between 2.8 and 4.9 accelerations/min in the different exercises. A significant correlation between V̇O2peak (mL kg-1 min-1) and acceleration and m/min were found, implying that soldiers with good aerobic capacity were able to cope better with tasks requiring quick movements.CONCLUSION: Conducting military operations in urban terrain and retrograde operations in rough terrain strains ground combat soldiers' cardiorespiratory system, with work intensities close to 40% of maximal aerobic capacity in 15-33% of mission time. Tasks with external load carriage include change of direction, accelerations, bounds, and jumping over obstacles, and physical fitness tests should replicate this. Findings in this study also add objective data to the physiological demands of work performed by combat soldiers while conducting urban operations and retrograde operations in rough terrain. These findings could be used to develop a model for classifying work demands for ground combat forces. © The Association of Military Surgeons of the United States 2022.

The aim of the present study was to identify the most physically demanding work tasks for Swedish ground combat soldiers and to study gender differences in perceived difficulty of the tasks.Methods: Based on procedural documentation, field manuals and job analyses, a questionnaire was developed that defined 30 work tasks for ground combat soldiers. To assess face and content validity, an expert focus group was used. The questionnaire was distributed to 231 ground combat soldiers, 165 of whom responded (71.4%), rating the perceived physical strain of the identified work tasks.Results: Of the 30 item work tasks included in the survey, five were selected as the most physically demanding. Transport of wounded was rated as the most demanding task both for aerobic endurance and strength. Other highly demanding tasks for aerobic capacity were low/high crawl, dismounted attack in close country, urban and rough terrain, and carrying heavy loads. For muscle strength demands there were a total of 11 different work task ranked for hand-, arm-, leg-, and core strength although many of them only occurred in one of the areas of interest. Carrying heavy loads were ranked in second place except for leg and core strength there it was in third place, instead high and low crawl was ranked very strenuous for leg and core strength. Care of wounded were ranked high for hand strength but not so hard for the rest of the areas.Conclusions: Swedish ground combat soldiers rate many tasks as physically demanding. Almost all the identified tasks were physically challenging and contains elements of lifting and carrying, which demand personnel’s muscular strength and muscular endurance capabilities for the soldiers. There were no significant differences in ranked physical requirements between male and female soldiers although they sometimes ranked the tasks in different order.

    • SenseWear Armband (SW) is a multisensor monitor to assess physical activity and energy expenditure. Its prediction algorithms have been updated periodically. The aim was to validate SW in children, adolescents, and adults. The most recent SW algorithm 5.2 (SW5.2) and the previous version 2.2 (SW2.2) were evaluated for estimation of energy expenditure during semi-structured activities in 35 children, 31 adolescents, and 36 adults with indirect calorimetry as reference. Energy expenditure estimated from waist-worn ActiGraph GT3X+ data (AG) was used for comparison. Improvements in measurement errors were demonstrated with SW5.2 compared to SW2.2, especially in children and for biking. The overall mean absolute percent error with SW5.2 was 24% in children, 23% in adolescents, and 20% in adults. The error was larger for sitting and standing (23%-32%) and for basketball and biking (19%-35%), compared to walking and running (8%-20%). The overall mean absolute error with AG was 28% in children, 22% in adolescents, and 28% in adults. The absolute percent error for biking was 32%-74% with AG. In general, SW and AG underestimated energy expenditure. However, both methods demonstrated a proportional bias, with increasing underestimation for increasing energy expenditure level, in addition to the large individual error. SW provides measures of energy expenditure level with similar accuracy in children, adolescents, and adults with the improvements in the updated algorithms. Although SW captures biking better than AG, these methods share remaining measurements errors requiring further improvements for accurate measures of physical activity and energy expenditure in clinical and epidemiological research.

    • INTRODUCTION: Children and adolescents are encouraged to maintain a habitually active lifestyle because of the known health benefits associated with regular physical activity, but there are some reports that a high level of activity may be associated with increased fracture risk. This prospective controlled exercise intervention study in pre-pubertal children evaluated if a school-based exercise intervention could enhance growth related gains in muscle strength and muscular function without affecting fracture risk. METHODS: Fractures were registered in 417 girls and 500 boys aged 7-9 years in the intervention and in 836 age-matched girls and 872 boys. The intervention included 40 minutes/day of school physical education for two years whereas the controls achieved 60 minutes/week. In a subsample consisting of 49 girls and 80 boys in the intervention and 50 girls and 53 boys in the control group, body composition was measured by dual X-ray absorptiometry (DXA), muscle strength by isokinetic Peak Torque (PT) of the knee extensors and flexors at 60 and 180 °/seconds by a computerized dynamometer and neuromuscular performance by Vertical Jump Height (VJH). RESULTS: The rate ratio [RR (95% confidence interval)] for children in the intervention group to sustain a fracture was 1.07 (0.66, 1.68). The annual gain in knee extensor PT at 180°/seconds was significantly higher for both girls (p<0.001) and boys (p<0.01) in the intervention compared to the control group. Boys in the intervention group also had a greater annual gain in knee flexion PT at 180 °/seconds (p<0.001) and girls a greater gain in VJH (p<0.05). CONCLUSIONS: An increase in school-based physical education from 60 to 200 min/week enhanced muscle strength in pre-pubertal children without affecting fracture risk.

    • BackgroundTo provide a comprehensive, systematic evaluation of the literature on experiences of psychological interventions for postpartum depression (PPD) in women. Depression is one of the most common postpartum mental disorders. Studies have identified that psychological interventions reduce depressive symptoms. However, less is known about the experiences of women who have received such treatments.MethodsA systematic review of the literature was conducted by searching five databases (CINAHL, Cochrane Library, EMBASE, Medline, PsycINFO), in August 2022. Studies with qualitative methodology examining women's experiences of professional treatment for PPD were included and checked for methodological quality. Eight studies (total N = 255) contributed to the findings, which were synthesized using thematic synthesis. Confidence in the synthesized evidence was assessed with GRADE CERQual.FindingsThe women had received cognitive behavioral therapy (5 studies) or supportive home visits (3 studies). Treatments were individual or group-based. Two main themes were identified: Circumstances and expectations, and Experiences of treatment, with six descriptive themes. Establishing a good relationship to their health professional was important for the women, regardless of treatment model. They also expressed that they wanted to be able to choose the type and format of treatment. The women were satisfied with the support and treatment received and expressed that their emotional well-being had been improved as well as the relationship to their infant.ConclusionThe findings can be helpful to develop and tailor patient-centered care for women who are experiencing postnatal depression.

    • Aim: Predictive equations for estimating body composition from bioelectrical impedance analysis (BIA) among Scandinavian children are lacking. In the present study, equations for estimation of fat-free body mass (FFM) and lean tissue mass (LTM) were developed and cross-validated from BIA using dual-energy X-ray absorptiometry (DXA) as the reference measurement of body composition. Methods: The study population consisted of 49 girls and 52 boys aged 9-11 years from Malmo, Sweden. Bioelectrical impedance was measured between hand and foot at 50 kHz. Predictive equations were developed by multiple linear regression and cross-validated against DXA measurements of body composition. Results: FFM was predicted from BIA and anthropometric variables with an adjusted R-2 = 0.95 and root mean square error (RMSE) = 0.84 kg, and LTM was predicted with an adjusted R-2 = 0.95 and RMSE = 0.87 kg. Cross-validation revealed a mean RMSE = 0.95 kg FFM and a mean RMSE = 0.96 kg LTM. Prediction of body composition from equations developed in previous literature was mixed when applied to the present cohort of children. Conclusions: FFM and LTM are predicted with sufficient accuracy at the population level. We recommend that the predictive equations developed in the present study are used in prepubescent European children aged 9-11 years only in order to minimize confounding of results because of possible differences in population samples.

    • Amyotrophic lateral sclerosis (ALS) affects anterior horn cells of the spinal cord causing an indolent slow and steady deterioration of muscle strength leading inevitably to death in respiratory failure. ALS is a model condition for neurodegenerative disorders. Exposure to different agents dispersed in the environment has been suggested to cause neurodegeneration but no convincing evidence for such a link has yet been presented. Respiratory exposure to metallic mercury (Hg0) from different sources may be suspected. Body distribution of metallic mercury is fast and depends on solubility properties. Routes of transport, metabolism, excretion and biological half-life determine the overall toxic effects. Inhalation experiments were performed in 1984 where small marmoset monkeys (Callithrix jacchus) were exposed to 203Hg0 vapour mixed into the breathing air (4–5 μg/l). After 1 hr of exposure, they were killed and whole body autoradiograms prepared to study the distribution of mercury within organs. Autoradiograms showed that Hg was deposited inside the spinal cord. Areas of enhanced accumulation anatomically corresponding to motor nuclei could be observed. This study describes a reinvestigation, with new emphasis on the spinal cord, of these classical metal exposure data in a primate, focusing on their relevance for the causation of neurodegenerative disorders. A comparison with more recent rodent experiments with similar findings is included. The hypothesis that long-time low-dose respiratory exposure to metals, for example, Hg, contributes to neurodegenerative disorders is forwarded and discussed.

    • © 2016 The Author(s).Background: The Childbirth Experience Questionnaire (CEQ) was originally designed to study women's perceptions of labour and birth. The main objective of our study was to adapt the CEQ to the Spanish context and determine its psychometric properties. This would provide an opportunity to evaluate women's experiences in order to improve evidence in the Spanish context as recommended by national guidelines. Methods: The CEQ was translated into Spanish using a standard forward and back translation method (CEQ-E). A convenience sample of 364 women was recruited from 3 Spanish hospitals; all participants were able to read and write in Spanish. Mothers with high risk pregnancies or preterm deliveries were excluded from the study. A self-administered questionnaire on sociodemographic variables was completed by participants before discharge. Data on childbirth variables were obtained from maternity records. Between 1 and 3 months postpartum a postal CEQ-E questionnaire was sent. The CEQ-E structure was examined by a confirmatory factor analysis of polychoric correlations using a diagonally weighted least squares estimator. Reliability was assessed using Cronbach's alpha. Construct validity was conducted by testing differences in CEQ-E scores between known-groups (to differ on key variables). Results: 226 (62.1%) of the recruited participants completed the postal questionnaire. The CEQ-E factor structure was similar to the original one. The Spanish version showed fit statistics in line with standard recommendations: CFI = 0.97; NNFI = 0.97; RMSEA = 0.066; SRMS = 0.077. The internal consistency reliability of the CEQ-E was good for the overall scale (0.88) and for all subscales (0.80, 0.90, 0.76, 0.68 for "own capacity", "professional support", "perceived safety" and "participation", respectively) and similar to the original version. Women with a labour duration ≤ 12 h, women with a labour not induced, women with a normal birth and multiparous women showed higher overall CEQ-E scores and "perceived safety" subscale scores. Women with a labour duration ≤ 12 h and those with previous experience of labour obtained higher scores for the "own capacity" and "participation" subscales. Conclusions: The results of this study indicate that the CEQ-E can be considered a valid and reliable measure of women's perceptions of labour and birth in Spain.

    • Glucagon-like peptide-1 (GLP-1) is a gut peptide that regulates food intake and glucose metabolism. GLP-1 is also produced and released in the brain, and GLP-1 receptors are expressed in brain regions important for alcohol and drug reward, and for the development of addiction. GLP-1 receptor agonists can decrease alcohol intake acutely in rodents. However, alcohol use disorder is a chronic condition that requires treatments to be effective in promoting abstinence from excessive alcohol consumption over time. Here, we assessed the effect of daily treatment with the GLP-1 receptor agonist Exendin-4 in an assay of relapse-like drinking in socially housed mice. Male C57BL/6NTac mice were allowed continuous access to alcohol without tastant in the home cage for 37 days. Then, alcohol bottles were removed and Exendin-4 (1.5 μg/kg/day) or saline was administered subcutaneously for 8 days during alcohol deprivation. Treatment continued for 8 additional days after reintroducing access to alcohol. A high-precision automated fluid consumption system was used to monitor intake of alcohol and water, drinking kinetics, and locomotor activity. Exendin-4 prevented the deprivation-induced increase in alcohol intake observed in control mice, without significantly affecting total fluid intake, body weight, or locomotor activity. The reduced alcohol intake was caused by a protracted latency to the first drink of alcohol and a reduced number of drinking bouts, while bout size and duration were not affected. The effect was maintained undiminished throughout the treatment period. These findings support the possible use of GLP-1 receptor agonists in the treatment of alcohol use disorder. © 2017 Elsevier Inc.
