  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Friend, Foe or Frenemy? Traditional Journalism Actors’ Changing Attitudes towards Peripheral Players and Their Innovations

    • This study synthesises two analytical frameworks—journalistic strangers and agents of media innovation—to examine how perceptions among newsworkers towards new entrants to their field shape the normalisation of innovations in a digital- first legacy news organisation over three years. Based on two rounds of interviews, it finds that peripheral players are gradually recognised for their contributions to journalism by traditional actors. Nonetheless, as barriers between the two groups lower, tensions involving dissonant professional perspectives, practices, and jurisdictions surface and are negotiated. The findings indicate a growing salience of hybrid roles in newsrooms that serve as linchpins to connect divergent professional fields, and more importantly, as bridges between tradition and innovation. Based on the increasing importance of collaboration and hybrid roles, this study makes a theoretical and practical contribution to research and media management by proposing that four forms of proximity—physical, temporal, professional, and control—are crucial in operationalising the impact that peripheral players have on innovation in news organisations.

    • This presentation showcases an ongoing three-year study that examines the interrelationships between newsworkers’ attitudes towards non-traditional journalism actors and the normalisation of innovations into everyday news practices in media organisations over time. It explores whether and to what extent have newsworkers’ attitudes towards non-traditional journalism actors and their contributions to innovations changed over time and considers if non-traditional journalism actors are more valued if they are involved in the core journalistic function of newsgathering and reporting, than if their input is related to the auxiliary journalistic functions (design, distribution, research, commerce and promotion). Congruently, would non-traditional journalism actors who come from within the newsroom be more accepted than those with comparable skills from outside? This research is guided by a synthesis of two analytical frameworks—the agents of media innovation (Westlund & Lewis, 2014) and the typology of strangers in journalism (Holton & Belair-Gagnon, 2018), and adopts a case study approach (Yin, 2018) to examining a legacy newspaper in Singapore. The findings indicate that as barriers between traditional and non-traditional journalism groups of actors are gradually lowered, new tensions involving dissonant professional perspectives, practices and jurisdictions begin to surface and are being negotiated. Additionally, there is a growing salience of hybrid roles within newsrooms that serve as lynchpins to connect divergent professional fields, and more importantly, as bridges between tradition and change.

  • Cold comfort: Lessons for the 21st-century newspaper industry from the 20th-century ice industry

    • First cut from frozen lakes and later manufactured on an industrial scale, ice was, in the nineteenth and early twentieth century, big business. The arrival of domestic refrigeration spelled the end of this industry. The rise and fall of the ice industry offers lessons to the modern newspaper industry, which faces a similar threat. Drawing an analogy between the ice and newspaper industries in terms of innovation and organizational learning, we look at how both are/were hampered by investment in technology and identity, and affected by devolution of control into consumers’ hands. Based on this, we suggest that the newspaper industry would benefit from links with heterogeneous rather than isomorphic partners to counterbalance routines that obstruct change, and just as refrigeration forced a reconceptualization of the concrete “ice” into the abstract “cold,” newspaper organizations can re-imagine themselves around the services and values they provide, rather than the product of “news.”

  • Generation and characterization of a novel candidate gene therapy and vaccination vector based on human species D adenovirus type 56

    • The vectorization of rare human adenovirus (HAdV) types will widen our knowledge of this family and their interaction with cells, tissues and organs. In this study we focus on HAdV-56, a member of human Ad species D, and create ease-of-use cloning systems to generate recombinant HAdV-56 vectors carrying foreign genes. We present in vitro transduction profiles for HAdV-56 in direct comparison to the most commonly used HAdV-5-based vector. In vivo characterizations demonstrate that when it is delivered intravenously (i.v.) HAdV-56 mainly targets the spleen and, to a lesser extent, the lungs, whilst largely bypassing liver transduction in mice. HAdV-56 triggered robust inflammatory and cellular immune responses, with higher induction of IFNγ, TNFα, IL5, IL6, IP10, MCP1 and MIG1 compared to HAdV-5 following i.v. administration. We also investigated its potential as a vaccine vector candidate by performing prime immunizations in mice with HAdV-56 encoding luciferase (HAdV-56-Luc). Direct comparisons were made to HAdV-26, a highly potent human vaccine vector currently in phase II clinical trials. HAdV-56-Luc induced luciferase 'antigen'-specific IFNγ-producing cells and anti-HAdV-56 neutralizing antibodies in Balb/c mice, demonstrating a near identical profile to that of HAdV-26. Taken together, the data presented provides further insight into human Ad receptor/co-receptor usage, and the first report on HAdV-56 vectors and their potential for gene therapy and vaccine applications.

    • Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection fatality rate (IFR) doubles with every 5 y of age from childhood onward. Circulating autoantibodies neutralizing IFN-α, IFN-ω, and/or IFN-β are found in ∼20% of deceased patients across age groups, and in ∼1% of individuals aged <70 y and in >4% of those >70 y old in the general population. With a sample of 1,261 unvaccinated deceased patients and 34,159 individuals of the general population sampled before the pandemic, we estimated both IFR and relative risk of death (RRD) across age groups for individuals carrying autoantibodies neutralizing type I IFNs, relative to noncarriers. The RRD associated with any combination of autoantibodies was higher in subjects under 70 y old. For autoantibodies neutralizing IFN-α2 or IFN-ω, the RRDs were 17.0 (95% CI: 11.7 to 24.7) and 5.8 (4.5 to 7.4) for individuals <70 y and ≥70 y old, respectively, whereas, for autoantibodies neutralizing both molecules, the RRDs were 188.3 (44.8 to 774.4) and 7.2 (5.0 to 10.3), respectively. In contrast, IFRs increased with age, ranging from 0.17% (0.12 to 0.31) for individuals <40 y old to 26.7% (20.3 to 35.2) for those ≥80 y old for autoantibodies neutralizing IFN-α2 or IFN-ω, and from 0.84% (0.31 to 8.28) to 40.5% (27.82 to 61.20) for autoantibodies neutralizing both. Autoantibodies against type I IFNs increase IFRs, and are associated with high RRDs, especially when neutralizing both IFN-α2 and IFN-ω. Remarkably, IFRs increase with age, whereas RRDs decrease with age. Autoimmunity to type I IFNs is a strong and common predictor of COVID-19 death.

    • Rationale: Asthma has substantial morbidity and mortality and a strong genetic component, but identification of genetic risk factors is limited by availability of suitable studies. Objectives: To test if population-based cohorts with self-reported physician-diagnosed asthma and genome-wide association (GWA) data could be used to validate known associations with asthma and identify novel associations. Methods: The APCAT (Analysis in Population-based Cohorts of Asthma Traits) consortium consists of 1,716 individuals with asthma and 16,888 healthy controls from six European-descent population-based cohorts. We examined associations in APCAT of thirteen variants previously reported as genome-wide significant (P < 5x10(-8)) and three variants reported as suggestive (P < 5 x 10(-7)). We also searched for novel associations in APCAT (Stage 1) and followed-up the most promising variants in 4,035 asthmatics and 11,251 healthy controls (Stage 2). Finally, we conducted the first genome-wide screen for interactions with smoking or hay fever. Main Results: We observed association in the same direction for all thirteen previously reported variants and nominally replicated ten of them. One variant that was previously suggestive, rs11071559 in RORA, now reaches genome-wide significance when combined with our data (P = 2.4x10(-9)). We also identified two genome-wide significant associations: rs13408661 near IL1RL1/IL18R1 (PStage1+Stage2 = 1.1x10(-9)), which is correlated with a variant recently shown to be associated with asthma (rs3771180), and rs9268516 in the HLA region (PStage1+Stage2 = 1.1x10(-8)), which appears to be independent of previously reported associations in this locus. Finally, we found no strong evidence for gene-environment interactions with smoking or hay fever status. Conclusions: Population-based cohorts with simple asthma phenotypes represent a valuable and largely untapped resource for genetic studies of asthma.

    • Through an analysis of relevant literature and open-ended survey responses from 2501 Singaporeans, this article proposes a conceptual framework to understand how individuals authenticate the information they encounter on social media. In broad strokes, we find that individuals rely on both their own judgment of the source and the message, and when this does not adequately provide a definitive answer, they turn to external resources to authenticate news items.

  • Optimal age-specific vaccination control for COVID-19 : An Irish case study

    • The outbreak of a novel coronavirus causing severe acute respiratory syndrome in December 2019 has escalated into a worldwide pandemic. In this work, we propose a compartmental model to describe the dynamics of transmission of infection and use it to obtain the optimal vaccination control. The model accounts for the various stages of the vaccination, and the optimisation is focused on minimising the infections to protect the population and relieve the healthcare system. As a case study, we selected the Republic of Ireland. We use data provided by Ireland's COVID-19 Data-Hub and simulate the evolution of the pandemic with and without the vaccination in place for two different scenarios, one representative of a national lockdown situation and the other indicating looser restrictions in place. One of the main findings of our work is that the optimal approach would involve a vaccination programme where the older population is vaccinated in larger numbers earlier while simultaneously part of the younger population also gets vaccinated to lower the risk of transmission between groups. We compare our simulated results with those of the vaccination policy taken by the Irish government to explore the advantages of our optimisation method. Our comparison suggests that a similar reduction in cases may have been possible even with a reduced set of vaccinations available for use.

  • Hypoxia Due to Cardiac Arrest Induces a Time-Dependent Increase in Serum Amyloid β Levels in Humans

    • Amyloid β (Aβ) peptides are proteolytic products from amyloid precursor protein (APP) and are thought to play a role in Alzheimer disease (AD) pathogenesis. While much is known about molecular mechanisms underlying cerebral Aβ accumulation in familial AD, less is known about the cause(s) of brain amyloidosis in sporadic disease. Animal and postmortem studies suggest that Aβ secretion can be up-regulated in response to hypoxia. We employed a new technology (Single Molecule Arrays, SiMoA) capable of ultrasensitive protein measurements and developed a novel assay to look for changes in serum Aβ42 concentration in 25 resuscitated patients with severe hypoxia due to cardiac arrest. After a lag period of 10 or more hours, very clear serum Aβ42 elevations were observed in all patients. Elevations ranged from approximately 80% to over 70-fold, with most elevations in the range of 3-10-fold (average approximately 7-fold). The magnitude of the increase correlated with clinical outcome. These data provide the first direct evidence in living humans that ischemia acutely increases Aβ levels in blood. The results point to the possibility that hypoxia may play a role in the amyloidogenic process of AD.
