Diagnostic assays are needed to rapidly discriminate between causes, because this will guide decisions on appropriate treatment. Therefore, a quantitative real-time PCR (qPCR) assay with duplex reactions targeting eight bacteria and six viruses was developed. Technical performance was examined with linear plasmids. Upper and Lower respiratory tract specimens were used to compare the qPCR assay with standard microbiological methods. The limit of detection was 5 to 20 DNA template copies with approximately 1000-fold differences in concentrations of the two competing templates. SDs for positive controls were <5%. The use of the qPCR assay resulted in 113 positive identifications in 94 respiratory specimens compared with 38 by using standard diagnostics. Diagnostic accuracy of the qPCR assay varied between 60% positive agreement with standard tests for Streptococcus pneumoniae and 100% for Mycoplasma pneumoniae, Moraxella catarrhalis, and Staphylococcus aureus. Negative percentage of agreement was >95% for M. pneumoniae, Streptococcus pyogenes, respiratory syncytial virus, and influenza A virus; whereas it was only 56% for Haemophilus influenzae. Multiple microbial agents were identified in 19 of 44 sputum and 19 of 50 nasopharynx specimens. We conclude that in parallel qPCR detection of the targeted respiratory bacteria and viruses is feasible. The results indicate good technical performance of the assay in clinical specimens.

  • Improved diagnosis and prediction of community-acquired pneumonia

    • Community-acquired pneumonia (CAP) is a major cause of morbidity and mortality worldwide. Although there is wide variation in the microbial etiology, CAP may manifest with similar symptoms, making institution of proper treatment challenging. Therefore, etiological diagnosis is important to ensure that correct treatment and necessary infection control measures are instituted. This provides a challenge for conventional microbial diagnostic methods, typically based on culture and direct antigen tests. Moreover, existing molecular biomarkers have poor prognostic value. Few studies have investigated the global metabolic response during infection and virtually nothing is known about early responses after the start of antimicrobial treatment. The aim of this work was to improve diagnostic and predictive methods for CAP.In paper I, a qPCR panel targeting 15 pathogens known to cause CAP was developed and evaluated. It combined identification of bacterial pathogens and viruses in the same diagnostic platform. The method proved to be robust and the results consistent with those obtained by standard methods. The panel approach, compared to conventional, selective diagnostics, detected a larger number of pathogens. In Paper II, whole blood samples from 65 patients with bacteremic sepsis were analyzed for metabolite profiles. Forty-nine patients with symptoms of sepsis, but later attributed to other diagnoses, were matched according to age and sex and served as a control group. Six metabolites were identified, all of which predicted growth of bacteria in blood culture. One of the metabolites, myristic acid, alone predicted bacteremic sepsis with a sensitivity of 100% and a specificity of 95%. Paper III and IV were based on a clinical study enrolling 35 patients with suspected CAP in need of hospital care. The aim was to study the metabolic response during the early phase of acute infection. The qPCR panel developed in Paper I was used to obtain the microbial etiological diagnosis. Paper IV focused on the global metabolic response and highlighted the dynamics of changes in major metabolic pathways during early recovery. A specific metabolite pattern for M. pneumoniae etiology was found. Four metabolites accurately predicted all but one patient as either M. pneumoniae etiology or not. Paper III looked at phospholipid levels during the first 48 hours after hospital admission. It was found that all major phospholipid species, especially the lysophosphatidyl-cholines, were pronouncedly decreased during acute infection. Levels started to increase the day after admission, reaching statistical significance at 48 hours. Paper II-IV showed that metabolomics might be used to study a number of different aspects of infection, such as etiology, disease progress and recovery. Knowledge of the metabolic profiles of patients may not only be utilized for biomarker discovery, as proposed in this work, but also for the future development of targeted therapies and supportive treatment.

  • The Swedish youth labor market in boom and depression

    • This paper is concerned with the labor market experience of Swedish youths during the 1980s and the 1990s. The first objective is to portray early economic attainment among young Swedes. The second objective of the paper is to examine the impact of labor market programs on youth employment. We find that the slump in the 1990s has been associated with dramatic increases in youth unemployment and youth participation in active labor market programs. The impact on unemployment rates by age and education has been roughly proportional, however. The evolution of employment and unemployment does not offer much ground for the popular hypothesis that the recent rise in unemployment is driven by large and pervasive shifts in the demand for labor by skill attributable to technological innovation. The employment crisis has been met by an unprecedented increase in active labor market programs, in large part targeted at unemployed youths. There is a risk that these programs may crowd out regular youth employment, a hypothesis that is supported in our empirical investigation of regular youth employment in Swedish municipalities.

In a prospective study, whole blood samples from 65 patients with bacteremic sepsis and 49 ER controls were compared. The blood samples were analyzed using gas chromatography coupled to time-of-flight mass spectrometry. Multivariate and logistic regression modeling using metabolites identified by chromatography or using conventional laboratory parameters and clinical scores of infection were employed. A predictive model of bacteremic sepsis with 107 metabolites was developed and validated. The number of metabolites was reduced stepwise until identifying a set of 6 predictive metabolites. A 6-metabolite predictive logistic regression model showed a sensitivity of 0.91(95% CI 0.69-0.99) and a specificity 0.84 (95% CI 0.58-0.94) with an AUC of 0.93 (95% CI 0.89-1.01). Myristic acid was the single most predictive metabolite, with a sensitivity of 1.00 (95% CI 0.85-1.00) and specificity of 0.95 (95% CI 0.74-0.99), and performed better than various combinations of conventional laboratory and clinical parameters. We found that a metabolomics approach for analysis of acute blood samples was useful for identification of patients with bacteremic sepsis. Metabolomics should be further evaluated as a new tool for infection diagnostics.

  • Phospholipid Levels in Blood during Community-Acquired Pneumonia

    • Phospholipids, major constituents of bilayer cell membranes, are present in large amounts in pulmonary surfactant and play key roles in cell signaling. Here, we aim at finding clinically useful disease markers in community-acquired pneumonia (CAP) using comprehensive phospholipid profiling in blood and modeling of changes between sampling time points. Serum samples from 33 patients hospitalized with CAP were collected at admission, three hours after the start of intravenous antibiotics, Day 1 (at 12–24 h), Day 2 (at 36–48 h), and several weeks after recovery. A profile of 75 phospholipid species including quantification of the bioactive lysophosphatidylcholines (LPCs) was determined using liquid chromatography coupled to time-of-flight mass spectrometry. To control for possible enzymatic degradation of LPCs, serum autotaxin levels were examined. Twenty-two of the 33 patients with a clinical diagnosis of CAP received a laboratory-verified CAP diagnosis by microbial culture or microbial DNA detection by qPCR. All major phospholipid species, especially the LPCs, were pronouncedly decreased in the acute stage of illness. Total and individual LPC concentrations increased shortly after the initiation of antibiotic treatment, concentrations were at their lowest 3h after the initiation, and increased after Day 1. The total LPC concentration increased by a change ratio of 1.6–1.7 between acute illness and Day 2, and by a ratio of 3.7 between acute illness and full disease resolution. Autotaxin levels were low in acute illness and showed little changes over time, contradicting a hypothesis of enzymatic degradation causing the low levels of LPCs. In this sample of patients with CAP, the results demonstrate that LPC concentration changes in serum of patients with CAP closely mirrored the early transition from acute illness to recovery after the initiation of antibiotics. LPCs should be further explored as potential disease stage biomarkers in CAP and for their potential physiological role during recovery.

    • BACKGROUND: The long-term sequelae after COVID-19 constitute a challenge to public health and increased knowledge is needed. We investigated the prevalence of self-reported persistent symptoms and reduced health-related quality of life (HRQoL) in relation to functional exercise capacity, 6 months after infection, and explored risk factors for COVID-19 sequalae.METHODS: This was a prospective, multicenter, cohort study including 434 patients. At 6 months, physical exercise capacity was assessed by a 1-minute sit-to-stand test (1MSTST) and persistent symptoms were reported and HRQoL was evaluated through the EuroQol 5-level 5-dimension (EQ-5D-5L) questionnaire. Patients with both persistent symptoms and reduced HRQoL were classified into a new definition of post-acute COVID syndrome, PACS+. Risk factors for developing persistent symptoms, reduced HRQoL and PACS+ were identified by multivariable Poisson regression.RESULTS: Persistent symptoms were experienced by 79% of hospitalized, and 59% of non-hospitalized patients at 6 months. Hospitalized patients had a higher prevalence of self-assessed reduced overall health (28 vs. 12%) and PACS+ (31 vs. 11%). PACS+ was associated with reduced exercise capacity but not with abnormal pulse/desaturation during 1MSTST. Hospitalization was the most important independent risk factor for developing persistent symptoms, reduced overall health and PACS+.CONCLUSION: Persistent symptoms and reduced HRQoL are common among COVID-19 survivors, but abnormal pulse and peripheral saturation during exercise could not distinguish patients with PACS+. Patients with severe infection requiring hospitalization were more likely to develop PACS+, hence these patients should be prioritized for clinical follow-up after COVID-19.

    • This paper reexamines the link between career interruptions and subsequent wages. Using a rich new Swedish dataset, we are able to disaggregate time out of work into several components. Regressing log wages on aggregate total time out leads to the standard result, i.e., a negative coefficient on time out. However, we find that different types of time out have different effects on wages and that these effects vary by gender. This casts doubt on the usual human capital depreciation interpretation that has been placed on the negative coefficient of total time out in the wage equation. We propose a simple signaling model as an alternative interpretation.

    • Universities need to reconfigure and rethink existing engineering beliefs in order to keep promoting students that can target and capitalize on tomorrow’s opportunities. This put pressure on promoting the best possible Engineering Education, which means continuant upgrades and revisions to existing curricula’s and faculties’ pedagogical methods and processes. This paper summarizes the experiences and lessons learned from a nationwide initiative to rethink and redesign existing engineering programs towards more traceable innovative practices. The Swedish Product Innovation Engineering Program (PIEp) and the Royal Institute of Technology (KTH) in particular have a long tradition of successful exchanges involving research and education. PIEp is committed to a system change towards innovation and entrepreneurship in institutes of higher education and research. From PIEp an organized network of senior researchers, PhD students, lecturers and students is seen as the seed for this change. Activities are conducted in three areas; research in product innovation, education for product innovation and industrial collaboration for product innovation. Turning away from one-timer and mere embryonic attempts, PIEp visions a systems shift through long term dedication to influence higher engineering education curricula design. KTH is currently performing a revision of all engineering program to fit the European Bologna higher education restructuring process. Encompassing both undergraduate and master level studies, the integration of engineering syllabus imperatives strive to converge with the internationally recognized CDIO standards and the new Swedish national degree specifications. The paper aims to summarize the initiative provided between PIEp, KTH and Stanford to stimulate Swedish Engineering faculty to embrace methods and tools for integrating creativity and innovation. Ultimately, building on the long experiences of successful workshops held by PIEp and KTH the overall ambition is to establish a change in mindsets, and by so influencing key participants to directly leave endurable footprints onto their respective Swedish Engineering Education Program. The paper has a descriptive character blending ‘best-of-both-worlds’ concepts as it reveals how a nationwide initiative has set up a learning hub overseas together with Stanford University. Utilizing this source of entrepreneurial and inspiring environment the ambition is to equip Swedish faculty with experiences, success stories, lessons learned, personal opinions, to provoke and challenge existing program and curricula design. In summary, the full paper version entails the set-up, reflections and actions outline by Swedish university representatives to address implementation of more transferability between innovation characteristics in respective education programs.

For students to excel in the exploration of early-phaseinnovation, it is important to identify when, how and to what extent leaningelements can be reinforced. Thestrengthened understanding by students is mirrored in improved ability to takeaction and apply relevant knowledge in distinct learning situations.     • Potato tuber necrosis in the form of spraing symptoms is caused by infection with Potato mop-top virus (PMTV) or Tobacco rattle virus (TRV); spraing has become more important in the Swedish potato crop production. In this study, the presence in Sweden of three potato-infecting viruses associated with necrotic symptoms in tubers was demonstrated: PMTV, TRV and Tobacco necrosis virus (TNV). This study shows that both PMTV and TRV are frequent in Swedish potato fields. PMTV was found in all Swedish counties, except the two most northern ones. TRV was present at several locations in southern and up to the central parts of Sweden. Both viruses were found further north than observed in earlier surveys. PMTV and TRV were analysed in tubers with spraing symptoms and it was not possible to decide visually if the symptoms were caused by either PMTV or TRV. Furthermore, a high occurrence of symptomless tuber infections was observed for several potato cultivars. A unique observation from this study was a mixed infection of both PMTV and TRV in tubers of cv. Berber, where no visual differences on symptom development were detectable for these tubers compared to single infections. During the survey, tubers of cv. Melody were found to display necrotic symptoms in the skin that are characteristic for the ABC disease. This suggested infection by TNV, which also could be confirmed.

    • BACKGROUND: Severe COVID-19 increases the risk for long-term respiratory impairment, but data after mild COVID-19 are scarce. Our aims were to determine risk factors for reduced respiratory function 3-6 months after COVID-19 infection and to investigate if reduced respiratory function would relate to impairment of exercise performance and breathlessness.METHODS: Patients with COVID-19 were enrolled at the University Hospitals of Umeå and Örebro, and Karlstad Central Hospital, Sweden. Disease severity was defined as mild (nonhospitalized), moderate (hospitalized with or without oxygen treatment), and severe (intensive care). Spirometry, including diffusion capacity (DLCO ), was performed 3-6 months after hospital discharge or study enrollment (for nonhospitalized patients). Breathlessness (defined as ≥1 according to the modified Medical Research Council scale) and functional exercise capacity (1-min sit-to-stand test; 1-MSTST) were assessed.RESULTS: Between April 2020 and May 2021, 337 patients were enrolled in the study. Forced vital capacity and DLCO were significantly lower in patients with severe COVID-19. Among hospitalized patients, 20% had reduced DLCO , versus 4% in nonhospitalized. Breathlessness was found in 40.6% of the participants and was associated with impaired DLCO . A pathological desaturation or heart rate response was observed in 17% of participants during the 1-MSTST. However, this response was not associated with reduced DLCO .CONCLUSION: Reduced DLCO was the major respiratory impairment 3-6 months following COVID-19, with hospitalization as the most important risk factor. The lack of association between impaired DLCO and pathological physiological responses to exertion suggests that these physiological responses are not primarily related to decreased lung function.

  • Effects of the COVID-19 pandemic on population mobility under mild policies : Causal evidence from Sweden

    • Sweden has adopted far less restrictive social distancing policies than most countries following the COVID-19 pandemic (1–7). This paper uses data on all mobile phone users, from one major Swedish mobile phone network, to examine the impact of the Coronavirus outbreak under the Swedish mild recommendations and restrictions regime on individual mobility and if changes in geographical mobility vary over different socio-economic strata. Having access to data for January-March in both 2019 and 2020 enables the estimation of causal effects of the COVID-19 outbreak by adopting a Difference-in-Differences research design. The paper reaches four main conclusions: (i) The daytime population in residential areas increased significantly (64 percent average increase); (ii) The daytime presence in industrial and commercial areas decreased significantly (33 percent average decrease); (iii) The distance individuals move from their homes during a day was substantially reduced (38 percent decrease in the maximum distance moved and 36 percent increase in share of individuals 2 who move less than one kilometer from home); (iv) Similar reductions in mobility were found for residents in areas with different socioeconomic and demographic characteristics. These results show that mild government policies can compel people to adopt social distancing behavior.

  • Interplay between soft and hard processes in quantum chromodynamics

    • In QCD, the theory of strong interactions, hard processes with large momentum transfers can be calculated using perturbation theory, while soft processes with small momentum transfers are poorly understood. In this thesis, phenomenological models of soft and hard QCD processes are developed and tested.A new model is presented for the distribution of quarks and gluons in hadrons. They are derived from a spherically symmetric, Gaussian, distribution of the quark and gluon momenta in the hadron rest frame. The proton structure function F2 is calculated from the model and evolved using next-to-leading order QCD, resulting in a successful comparison-with deep inelastic scattering data.A unified description is given of the hadronic final states observed in deep inelastic electron-proton scattering at HERA, regarding both the large forward energy flow and the rapidity gap events with no particles in a large forward angular region. The model is based on perturbative QCD complemented with a new "soft colour interaction" mechanism which affects the hadronic final state, and provides a continuous description of the transition from diffractive to non-diffractive hard scattering.The model for soft colour interactions also describes the surprisingly high production rate of heavy quarkonia observed in proton-antiproton collisions at the Tevatron.

    • Could differences in risk attitudes explain parts of the gender wage gap? We present estimates on the association between labor market outcomes and financial risk-taking using individual level administrative data on individual wealth portfolios and wage rates from year 2000, when high-quality wealth data were available in Sweden. The individual's share of risky to total financial assets is significantly and positively associated with the wage rate. However, it turns out that our risk measure explains only a small part of the observed gender difference in wages.
