    • The ABC effect-a puzzling low-mass enhancement in the pi pi invariant mass spectrum, first observed by Abashian, Booth, and Crowe-is well known from inclusive measurements of two-pion production in nuclear fusion reactions. Here we report on the first exclusive and kinematically complete measurements of the most basic double-pionic fusion reaction pn -> d pi(0)pi(0) at beam energies of 1.03 and 1.35 GeV. The measurements, which have been carried out at CELSIUS-WASA, reveal the ABC effect to be a (pi pi)(I=L=0) channel phenomenon associated with both a resonancelike energy dependence in the integral cross section and the formation of a Delta Delta system in the intermediate state. A corresponding simple s-channel resonance ansatz provides a surprisingly good description of the data.

    • Exclusive measurements of the reactions pd -> He-3 pi(+)7 pi(-) and pd -> He-3 pi(0)pi(0) have been carried out at T-p = 0.893 GeV at the CELSIUS storage ring using the WASA detector. The pi(+)pi(-) channel evidences a pronounced enhancement at low invariant pi pi masses-as anticipated from previous inclusive measurements of the ABC effect. This enhancement is seen to be even much larger in the isoscalar pi(0)pi(0) channel. The differential distributions prove this enhancement to be of scalar-isoscalar nature. Delta Delta calculations give a good description of the data, if a boundstate condition is imposed for the intermediate Delta Delta system.

    • The ABC effect - a puzzling threshold enhancement in the ππ invariant mass spectrum of double-pionic fusion to nuclear bound states - has been investigated since 40 years by inclusive measurements without providing a conclusive interpretation. First exclusive measurements to this topic have now been carried out at CELSIUS-WASA. They confirm a huge ππ threshold enhancement, prove it to be of scalar-isoscalar nature, i.e., a σ-channel phenomenon and reveal the theoretically predicted high-mass enhancement in inclusive spectra to be of πππ rather than of ππ nature - falsifying thus corresponding model predictions for the ABC effect. From the kinematically complete data samples we infer that the interaction between the two Δ particles created in this process is of crucial importance.

    • The CELSIUS-WASA setup is used to measure the 3π0 decay of η mesons produced in pp interactions with beam kinetic energies of 1.36 and 1.45 GeV. The efficiency-corrected Dalitz plot and density distributions for this decay are shown, together with a fit of the quadratic slope parameter α yielding α = −0.026 ± 0.010(stat) ± 0.010(syst). This value is compared to recent experimental results and theoretical predictions.

    • A search for rare lepton decays of the eta meson was performed using the WASA detector at CELSIUS. Two candidates for double Dalitz decay eta -> e(+)e(-)e(+)e(-) events are reported with a background of 1.3 +/- 0.2 events. This allows to set an upper limit to the branching ratio of 9.7x10(-5) (90% CL). The branching ratio for the decay eta -> e(+)e(-)gamma is determined to (7.8 +/- 0.5(stat)+/- 0.8(syst))x10(-3) in agreement with world average value. An upper limit (90% CL) for the branching ratio for the eta -> e(+)e(-) decay is 2.7x10(-5) and a limit for the sum of the eta ->mu(+)mu(-)mu(+)mu(-) and eta ->pi(+)pi(-)mu(+)mu(-) decays is 3.6x10(-4).

    • Purpose To assess the relationship of age-related cataract with all-cause mortality in a Swedish population. Methods Cox regression analyses were performed in a cohort of 746 residents 65-74 years of age, examined in a population survey in the rural district of Tierp, Sweden, 1984-86. To expand the sample size, 1,071 people were recruited by means of glaucoma case records established at the Eye Department in Tierp from 1978 to 2007. In this way, the cohort comprised 1,817 subjects, representing nearly 27,000 person-years at risk. The presence of cataract was determined based on retroillumination with lens opacities evident on slit-lamp examination. Information on deaths was obtained from the local population register. Results By the conclusion of the study in April 2020, 1,633 deaths had been reported. Of these cases, 694 were affected by lens opacities or had history of cataract surgery at baseline. In multivariate analysis, including cataract, age, sex, smoking habits, cancer, diabetes, hypertension and ischemic heart disease, no association was found between cataract and mortality (hazard ratio 0.99; 95% confidence interval 0.90-1.10). Adjustment for participation in the population survey had no effect on the estimate. Conclusion In this long-term follow-up study of subjects 65-74-years-old in Sweden, cataract was not associated with all-cause mortality.

    • Background: Open-angle glaucoma (OAG) and Alzheimer's disease (AD) are two age-related neurodegenerative diseases of significant public health importance. Epidemiological studies have indicated that there might be an association between the disorders.Methods: Predictors of AD, including mixed and unspecified dementia, were analysed in a cohort of 712 residents aged 65-74 years, examined in a population survey in the rural district of Tierp, Sweden, from 1984 to 1986. To expand the sample size, 821 people were recruited by means of glaucoma case records established at the Eye Department in Tierp from 1978 to 2007. In this way, the cohort comprised 1,533 people, representing more than 21,000 person-years at risk. Medical records were reviewed to identify subjects diagnosed with dementia. Those with a follow-up duration shorter than 2 years were excluded.Results: By the conclusion of the study, in August 2020, 307 subjects had received a diagnosis of AD, including mixed and unspecified dementia. Of these cases, 55 were affected with definite OAG at baseline. Higher age and ischemic heart disease were the only predictors of AD identified. In multivariate analysis, adjusting for age, participation in the population survey and competing events, no association was found between OAG and AD (hazard ratio 1.08; 95% confidence interval: 0.80-1.47).Conclusion: In this long-term follow-up study of subjects aged 65-74 years old in Sweden, OAG was not associated with AD.

    • Purpose: To estimate the incidence of pseudoexfoliation (PEX) and its associated risk factors in a defined population in Sweden.Methods: The development of PEX was studied in a cohort of 489 residents aged 65-74 years, examined in a population survey in the municipality of Tierp 1984-1986. To expand the sample size, 576 people were recruited by means of glaucoma case records established at the Eye Department in Tierp in 1978-2007. In this way, the cohort comprised 1065 subjects, representing nearly 10 500 person-years at risk. Incidence rates with 95% confidence intervals (CIs) were calculated. Survival analyses were performed to identify predictors for PEX. Those with a follow-up time shorter than 1 year were excluded.Results: By the end of the study in November 2018, 179 new cases of PEX had been found. Of these, 78 had participated in the population survey, equal to an incidence rate of 14.8 per 1000 person-years (95% CI 11.5-18.1). The development of PEX was associated with higher age (hazard ratio 1.08 per year; 95% CI 1.03-1.13) and female gender (hazard ratio 1.59; 95% CI 1.16-2.18).Conclusion: In this study on a Swedish population, the incidence of PEX was high. Increasing age and female gender were the only predictors for PEX identified.

  • Incidence of open-angle glaucoma in central Sweden

    • PURPOSE: To estimate the incidence rate of open-angle glaucoma (OAG) in a population in which pseudoexfoliation is a common finding. METHODS: A population-based survey of 760 people aged 65-74 years was conducted in the municipality of Tierp, in central Sweden, during 1984-86. Visual fields (VFs) were tested with the Competer 350 automated perimeter. The present report concerns 711 people, all of whom participated in the survey and were not diagnosed with glaucoma. Since 1988, a follow-up study of the 530 individuals in this cohort with normal and reliable VFs has been in progress. Incident cases of OAG were identified among study participants and patients seeking medical attention at the Eye Department in Tierp. Records of prescriptions for anti-glaucoma drugs were used to search for unidentified patients. A time-weighted mean intraocular pressure (IOP) for all visits was calculated. RESULTS: By the end of the study in May 2006, 51 new cases of definite OAG had been identified, 31 with capsular and 20 with chronic simple glaucoma. Of these, 46 cases were detected in the follow-up study, corresponding to an incidence of 7.11 (95% confidence interval 5.06-9.17) per 1000 person-years. Twelve of the 51 cases were classified as normal-tension glaucoma, all of which were found in the follow-up study. Open-angle glaucoma with a mean IOP > or = 23 mmHg tended to emerge closer to the baseline examination than OAG with a mean IOP < 23 mmHg. CONCLUSIONS: The incidence of OAG was high compared with that reported for other studies conducted in White populations.

  • Prevalence of open-angle glaucoma in Central Sweden : The Tierp Glaucoma Survey

    • In 1984‐86, a glaucoma survey was carried out in a municipality in central Sweden, comprising 760 subjects 65–74 years of age. Since 1988, a follow‐up study of those 527 subjects who had normal and reliable visual fields has been in progress. A visual field defect was required for a diagnosis of chronic open‐angle glaucoma. By December 1991, 10 cases of capsular glaucoma and 3 cases of chronic simple glaucoma had been found. The standardized relative risk for chronic open‐angle glaucoma was 3.4 times (95% confidence interval: 1.1 ‐ 11) greater in subjects with a baseline intraocular pressure < 21 mmHg, using the highest pressure in either eye, than in those with an intraocular pressure < 21 mmHg. Pseudoexfoliation increased the standardized relative risk 9.8‐fold (95% confidence interval: 2.5 – 38). There was indication of effect modification between intraocular pressure and pseudoexfoliation. Optic disc haemorrhages were associated with a 4.8‐fold (95% confidence interval: 0.63 – 37) increased unadjusted risk. The study supports the pressure theory of optic nerve damage in glaucoma. Pseudoexfoliation was recognized as an independent risk factor for the development of chronic open‐angle glaucoma.

    • PURPOSE: To study the relationship between pseudoexfoliation (PEX) and aortic aneurysm in a population with a high prevalence of PEX.METHODS: Survival analyses were performed in a cohort of 735 residents aged 65-74 years, examined in a population survey in the municipality of Tierp, Sweden, 1984-1986. To expand the sample size, 1040 people were recruited by means of glaucoma case records established at the Eye Department in Tierp in 1978-2007. In this way, the cohort comprised 1775 subjects, representing more than 25 400 person-years at risk. Medical records and autopsy reports were reviewed to identify subjects diagnosed with aneurysm. Those with a follow-up time shorter than 1 year were excluded.RESULTS: By the end of the study in September 2017, 60 new cases of aortic aneurysm had been found. Of these cases, 23% (14 subjects) were affected by PEX at baseline, compared with 28% among subjects without aneurysm. No association between PEX and aortic aneurysm was found (hazard ratio 0.97; 95% confidence interval 0.53-1.77).CONCLUSION: In this population-based study, we were unable to verify a relationship between PEX and the development of aortic aneurysm.

To expand the cohort, participants in other studies in Tierp were enrolled. Additionally, subjects were recruited by means of glaucoma case records established at the Eye Department in Tierp in 1978-2007. In total, the cohort comprised 1471 individuals, representing more than 15 900 person-years of risk. Information on cataract surgery was obtained from the glaucoma case records and from medical records.RESULTS:By the end of the study in August 2014, 564 subjects had undergone cataract surgery. Of these cases, 224 were affected by PEX at baseline. In multivariate analyses, PEX was the second most important predictor for cataract surgery after lens opacities, accounting for a 2.38-fold (95% confidence interval 2.01-2.84) increased risk.CONCLUSION:The results strongly suggest that PEX is a predictor for cataract surgery in the population under study.

    • PURPOSE: To estimate the risk of open-angle glaucoma (OAG) associated with exposure to pseudoexfoliation (PEX) and increased intraocular pressure (IOP). METHODS: In 1984-86, a cross-sectional, population-based survey was conducted in the municipality of Tierp, central Sweden. Its target population comprised 2429 residents aged 65-74 years. In addition to a sample of 760 people, patients previously diagnosed with glaucoma were examined. The prevalence of OAG in the target population was estimated from the prevalence in the sample and patients already diagnosed. A review of prevalent cases in 1984-86 was undertaken in 2006. RESULTS: Definite OAG was established in 77 cases, corresponding to a prevalence of 5.3% (95% confidence interval [CI] 4.4-6.2). Of these, 23 represented newly detected cases. The prevalence of PEX was 17.2% (95% CI 14.6-19.9), calculated from 134 cases in the population sample. When adjusting for gender, PEX was associated with a 4.7-fold (95% CI 2.2-9.4) increased risk of OAG. For clinical cases only, the risk was 16-fold (95% CI 4.8-56) greater in subjects with PEX, compared with those without PEX. In individuals without a previous diagnosis of glaucoma, an IOP > or = 20 mmHg was associated with a 9.7-fold (95% CI 3.7-27) increased risk, but PEX alone was not a risk factor for OAG (adjusted odds ratio = 0.96). CONCLUSIONS: Pseudoexfoliation was associated with OAG only in people previously diagnosed with the disease. In cases detected in the population-based survey, increased IOP was a serious risk factor.

From 1988 to 2006, a follow-up study of the 530 people with normal visual fields has been in progress. To increase the cohort, 273 ophthalmic outpatients were enroled. Reliable visual fields were available for 679 people, representing 6 126 person-years at risk. A time-weighted mean intraocular pressure (IOP) for all visits was calculated.Results: Sixty-four subjects developed definite OAG, 29 of whom were exposed to PEX. Risk factors associated with OAG were higher age, a positive family history, increased IOP and PEX. The age-standardized rate ratio (SRR) was 14.8 times (95% confidence interval [CI] 7.92-27.8) greater in subjects with mean IOP >/=20 mmHg than in those with mean IOP /=20 mmHg.Conclusion: Increased IOP and PEX were serious risk factors for incident OAG. The effect of PEX was mediated by increased IOP.

    • Purpose To study predictors of open-angle glaucoma (OAG) in newly diagnosed patients detected in clinical practice in a defined population and to estimate the frequency of normal-tension glaucoma (NTG).Methods Predictors of OAG were analysed in a case-control study, nested in a cohort of 481 people, 55-84 years of age, in two rural districts in Sweden, examined at the Eye Department in Tierp from 1988 to 2003. To make the sample complete, 63 residents first examined at the Uppsala University Hospital (UUH), 60 of whom were identified with the help of registers, were invited to attend the study. In this way, the cohort included 544 individuals. Automated perimetry was used to find cases of OAG.Results In people first examined in Tierp, incident OAG was found in 99 subjects. Of these cases, six were diagnosed with NTG. Increased intraocular pressure (IOP), higher age, male sex, a positive family history, pseudoexfoliation (PEX), and haemorrhages of the optic disc were associated with OAG. In a logistic regression model including PEX, every increase in IOP by 5 mmHg increased the risk for OAG four-fold (odds ratio 4.04; 95% confidence interval 2.91-5.62). The effect of PEX was essentially mediated by increased pressure. The inclusion of the 63 subjects first examined at the UUH had no impact on the results.Conclusion In this study on patients aged 55-84-years old in clinical practice in Sweden, increased IOP was strongly associated with incident OAG, while NTG was a rare finding.

    • Glaucoma is a common disease in the elderly population. Open-angle glaucoma (OAG) is the predominant form of glaucoma. Chronic simple glaucoma and capsular glaucoma, characterized by the occurrence of pseudoexfoliation in the anterior eye segment, are the most frequent types of OAG. The purpose of the present thesis was to study the epidemiology of OAG in the municipality of Tierp, whose population has a high exposure to pseudoexfoliation.In a case-finding study, the prevalence of known cases of OAG by December 31, 1983 was estimated to 1.4% in people ≥45 years of age. Sixty-three percent of all cases had capsular glaucoma. Patients with advanced glaucoma were older, had had the disease for longer, had higher mean initial intraocular pressure, and had more extensive visual field defects at the time of diagnosis.A population survey of people 65–74 years of age was conducted in 1984–86. The prevalence of OAG was 5.3%. Pseudoexfoliation was found in 17%, being more common in females. Pseudoexfoliation was associated with OAG only in people previously diagnosed with the disease (odds ratio = 16). In cases detected at the survey, an intraocular pressure ≥20 mmHg was a serious risk factor of having OAG (odds ratio = 9.7).In a 5-year follow-up study of participants in the population survey, increased intraocular pressure and pseudoexfoliation were recognized as independent risk factors for the development of OAG (standardized risk ratios = 3.4 and 9.8, respectively). Interaction between increased intraocular pressure and pseudoexfoliation was indicated. By May 2006, the incidence of OAG was estimated to 7.1 per 1,000 person-years. The incidence of capsular glaucoma was more than twice that of chronic simple glaucoma.The prevalence and incidence of OAG was higher than that reported from other studies conducted on Caucasian populations. The probable explanation for this finding is exposure to pseudoexfoliation.

    • PAX6 gene mutations cause a variety of eye and central nervous system (CNS) abnormalities. Aniridia is often accompanied by CNS abnormalities such as pineal gland atrophy or hypoplasia, leading to disturbed circadian rhythm and sleep disorders. Less is known on the coincidence of narcolepsy in this patient group. We aimed to find out whether the circadian rhythm or sleep-wake structure was affected in patients with aniridia. Four members of a family segregating with congenital aniridia in two generations were included in the study. The patients were subjected to genetic testing for a PAX6 mutation, multiple sleep latency test, whole-brain magnetic resonance imaging (MRI), hypocretin-1 in cerebrospinal fluid, and Human Leukocyte Antigen DQ beta1*06:02. All four members were heterozygous for the pathogenic c.959-1G>A mutation in the PAX6 gene. Sleep disturbance was observed in all family members. The index patient was diagnosed with narcolepsy. MRI showed a hypoplastic pineal gland in all members. We describe the first case of a patient with PAX6 haploinsufficiency, aniridia and pineal gland hypoplasia diagnosed with narcolepsy type-1, suggesting a complex sleep disorder pathogenesis.

    • The Uppsala university coin cabinet harbours arather unusual numismatic treasure, a ‘screw-thaler’ from Augsburg, produced in the secondhalf of the 17th century. The object combines twothalers from Augsburg, one from 1632 portrayingthe Swedish king Gustavus Adolphus, and theother one coined 1658 displaying the city ofAugsburg. One side of both coins is polished aswell as hollowed out, and the rim remade intoscrew threads. Thereby the thalers are convertedinto a little coin-shaped box. Inside the screw-thaler you find two watercolours on paper,one showing a man, the other a woman, bothwearing contemporary clothing. Besides thewatercolours the ’box’ contains eight picturespainted on thin sheets made of mica. The mica-sheets are transparent except where they aredecorated with male and female clothing suitablefor the watercolours. These painted sheets couldthen be placed on top of the man or woman andthereby showing those wearing dif-ferent dresses.Screw-thalers were popular gifts and tokens inthe 17th century, and could besides these mica-sheets mentioned above, contain written notes,portraits, or paintings illustrating pro-fane orreligious content. The screw-thaler is typical forearly modern art- and craft production meetingthe cravings for shimmering objects with hiddenfunctions.

    • Background: Increased intraocular pressure (IOP) and pseudoexfoliation (PEX) are major risk factors for open-angle glaucoma (OAG), an age-related neurodegenerative disease of significant importance for public health. There are few studies on the distribution of IOP in populations where PEX is a common finding.Methods: The distribution of IOP was studied in 733 subjects 65-74 years of age, examined in a population survey in the rural district of Tierp, Sweden, 1984-86. The difference between the right and left eye and the effect of which eye was measured first were examined. Odds ratios, adjusted for age and sex, according to Mantel-Haenszel (ORMH), were calculated to estimate predictors of increased IOP, defined as a pressure >= 20 mm Hg in either eye. The pressure was measured with Goldmann applanation tonometry. Automated perimetry was used to identify OAG.Results: The distribution of IOP was close to that of other European- derived populations. The pressure in the first measured eye was higher than in the second measured eye. Increased IOP was related to OAG and PEX, ORMH 8.97 (95% confidence interval [CI] 3.84-20.9) and 2.40 (95% CI 1.53-3.76), respectively. An IOP >= 20 mm Hg increased the risk of having been diagnosed with diabetes (ORMH 1.83; 95% CI 1.08-3.09).Conclusion: In this study of subjects 65-74-years- old in Sweden, the distribution of IOP was close to that of other European-derived populations. Although the difference was small, the pressure in the first measured eye was higher than in the second eye. Increased IOP was strongly related to untreated OAG and PEX.
