  • Design and Baseline Characteristics of the Finerenone in Reducing Cardiovascular Mortality and Morbidity in Diabetic Kidney Disease Trial

    • <b><i>Background:</i></b> Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. <b><i>Patients and</i></b> <b><i>Methods:</i></b> The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m<sup>2</sup> and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. <b><i>Conclusions:</i></b> FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.   • EISCAT_3D - a next-generation European radar system for upper atmosphere and geospace research

    • The EISCAT Scientifi c Association, together with a number of collaborating institutions, has recently completed a feasibility and design study for an enhanced performance research radar facility to replace the existing EISCAT UHF and VHF systems. This study was supported by EU Sixth-Framework funding. The new radar retains the powerful multi-static geometry of the EISCAT UHF, but will employ phased arrays, direct-sampling receivers, and digital beamforming and beam steering. Design goals include, inter alia, a tenfold improvement in temporal and spatial resolution, an extension of the instantaneous measurement of full-vector ionospheric drift velocities from a single point to the entire altitude range of the radar, and an imaging capability to resolve small-scale structures. Prototype receivers and beamformers are currently being tested on a 48-element, 224 MHz array (the "Demonstrator") erected at the Kiruna EISCAT site, using the EISCAT VHF transmitter as an illuminator.

  • Teachers’ Social and Emotional Competencies : A Lever for Social and Emotional Learning in Schools

    • The present paper focuses on social and emotional learning, and highlights empirical findings on its importance for teachers, students and schools. The importance of social and emotional learning in school settings has been a central focus of research for the last 30 years. The development of teachers’ social and emotional competencies has been an essential factor for improving social and emotional learning of students. Only when teachers are able to create a school climate in which students feel included and welcome, students can develop and enhance their social and emotional competencies. In this paper, we outline the main conceptual frameworks on social and emotional learning, emphasizing the CASEL framework, relevant for both young people and adults. We proceed by focusing on teachers’ social and emotional competencies, as teachers are one of the key figures for fostering and developing social and emotional competencies in school settings. Afterwards, we address the issue of development of teachers’ social and emotional competencies in various settings. The last part of the paper links teachers’ and students’ social and emotional competencies. We stress the importance of teachers’ social and emotional competencies for healthy teacher-student relationships, and students’ well-being and academic achievement. As teachers also need support for the development of these competencies, we advocate for comprehensive school-wide approach, in order to ensure the successful implementation of social and emotional learning in classrooms.

  • Sox5 regulates beta-cell phenotype and is reduced in type 2 diabetes

    • Type 2 diabetes (T2D) is characterized by insulin resistance and impaired insulin secretion, but the mechanisms underlying insulin secretion failure are not completely understood. Here, we show that a set of co-expressed genes, which is enriched for genes with islet-selective open chromatin, is associated with T2D. These genes are perturbed in T2D and have a similar expression pattern to that of dedifferentiated islets. We identify Sox5 as a regulator of the module. Sox5 knockdown induces gene expression changes similar to those observed in T2D and diabetic animals and has profound effects on insulin secretion, including reduced depolarization-evoked Ca 2+-influx and β-cell exocytosis. SOX5 overexpression reverses the expression perturbations observed in a mouse model of T2D, increases the expression of key β-cell genes and improves glucose-stimulated insulin secretion in human islets from donors with T2D. We suggest that human islets in T2D display changes reminiscent of dedifferentiation and highlight SOX5 as a regulator of β-cell phenotype and function.

  • SCORE2 risk prediction algorithms: new models to estimate 10-year risk of cardiovascular disease in Europe

    • Aims The aim of this study was to develop, validate, and illustrate an updated prediction model (SCORE2) to estimate 10-year fatal and non-fatal cardiovascular disease (CVD) risk in individuals without previous CVD or diabetes aged 40-69 years in Europe. Methods and results We derived risk prediction models using individual-participant data from 45 cohorts in 13 countries (677 684 individuals, 30 121 CVD events). We used sex-specific and competing risk-adjusted models, including age, smoking status, systolic blood pressure, and total- and HDL-cholesterol. We defined four risk regions in Europe according to country-specific CVD mortality, recalibrating models to each region using expected incidences and risk factor distributions. Region-specific incidence was estimated using CVD mortality and incidence data on 10 776 466 individuals. For external validation, we analysed data from 25 additional cohorts in 15 European countries (1 133 181 individuals, 43 492 CVD events). After applying the derived risk prediction models to external validation cohorts, C-indices ranged from 0.67 (0.65-0.68) to 0.81 (0.76-0.86). Predicted CVD risk varied several-fold across European regions. For example, the estimated 10-year CVD risk for a 50-year-old smoker, with a systolic blood pressure of 140 mmHg, total cholesterol of 5.5 mmol/L, and HDL-cholesterol of 1.3 mmol/L, ranged from 5.9% for men in low- risk countries to 14.0% for men in very high-risk countries, and from 4.2% for women in low-risk countries to 13.7% for women in very high-risk countries. Conclusion SCORE2-a new algorithm derived, calibrated, and validated to predict 10-year risk of first-onset CVD in European populations-enhances the identification of individuals at higher risk of developing CVD across Europe.

  • IDOL regulates systemic energy balance through control of neuronal VLDLR expression

    • Liver X receptors limit cellular lipid uptake by stimulating the transcription of inducible degrader of the low-density lipoprotein receptor (IDOL), an E3 ubiquitin ligase that targets lipoprotein receptors for degradation. The function of IDOL in systemic metabolism is incompletely understood. Here we show that loss of IDOL in mice protects against the development of dietinduced obesity and metabolic dysfunction by altering food intake and thermogenesis. Unexpectedly, analysis of tissue-specific knockout mice revealed that IDOL affects energy balance, not through its actions in peripheral metabolic tissues (liver, adipose tissue, endothelium, intestine, and skeletal muscle) but by controlling lipoprotein receptor abundance in neurons. Single-cell RNA sequencing of the hypothalamus demonstrated that IDOL deletion altered gene expression linked to the control of metabolism. Finally, we identified very low-density lipoprotein receptor (VLDLR) rather than low-density lipoprotein receptor (LDLR) as the primary mediator of the effects of IDOL on energy balance. These data identify a role for the neuronal IDOL-VLDLR pathway in metabolic homoeostasis and diet-induced obesity.

    • Observations obtained by the Freja satellite at altitudes around 1700 km in the high-latitude magnetosphere are used to study ion energization perpendicular to the geomagnetic field. Investigations of ions, electrons, plasma densities, electric and magnetic wave fields, and field-aligned currents are used to study O+ heating mechanisms. Three ion heating events are studied in detail, and 20 events are used in a detailed statistical study. More than 200 events are classified as belonging to one of four major types of ion heating and are ordered as a function of magnetic local time. The most common types of ion heating are associated with broadband low-frequency electric wave fields occurring at all local times. These waves cover frequencies from below one up to several hundred hertz and correspond to the most intense O+ energization. Heating by these waves at frequencies of the order of the O+ gyrofrequency at 25 Hz seems to be the important energization mechanism, causing O+ ion mean energies up to hundreds of eV. The broadband waves are associated with Alfven waves with frequencies up to at least a few hertz and with field-aligned currents. Other types of O+ energization events are less common. During these events the ions are heated by waves near the lower hybrid frequency or near half the proton gyrofrequency. These waves are generated by auroral electrons or in a few cases by precipitating ions.

    • We present an investigation of dispersion map optimization for-span single channel 28 GBaud QPSK transmission systems with phase-sensitive amplifiers (PSAs). In experiments, when the PSA link is operated in a highly nonlinear regime, a 1.4 dB error vector magnitude (EVM) improvement is achieved compared to a one-span optimized dispersion map link due to improved nonlinearity mitigation. The two-span optimized dispersion map of a link differs from the optimized dispersion map of a dispersion managed phase-insensitive amplifier (PIA) link, Simulations show that the performance of the two-span dispersion map optimized PSA link does not improve by residual dispersion optimization. Further, by using the two-span optimized dispersion maps repeatedly in a long-haul PSA link instead of one-span optimized maps, the maximum transmission reach can he improved 1,5 times, (C) 2017 Optical Society of America

  • Fracture Risk After Gastric Bypass Surgery : A Retrospective Cohort Study

    • Objectives: Gastric bypass surgery constitutes the most common and effective bariatric surgery to treat obesity. Gastric bypass leads to bone oss but fracture risk following surgery has been insufficiently studied. Our objective was to investigate if gastric bypass surgery in obese patients, with and without diabetes, was associated with fracture risk, and if the fracture risk was associated with post-surgery weight loss or insufficient calcium and vitamin D supplementation.Methods: Using large databases, 38 971 obese patients undergoing gastric bypass were identified, 7758 with diabetes and 31 213 without. Through multivariable 1:1 propensity score matching, well-balanced controls were identified. The risk of fracture and fall injury was investigated using Cox proportional hazards and flexible parameter models. Fracture risk according to weight loss and degree of calcium and vitamin D supplementation one year post-surgery was investigated.Results: 77 942 patients had a median and total follow-up time of 3.1 (IQR 1.7-4.6) and 251 310 person-years, respectively. Gastric bypass was associated with increased risk of any fracture, in patients with diabetes and without diabetes using a multivariable Cox model (HR 1.26, 95%CI 1.05-1.53 and HR 1.32, 95%CI 1.18-1.47, respectively). The risk of fall injury without fracture was also increased after gastric bypass, both in patients with (HR 1.26 95%CI 1.04-1.52) and without diabetes (HR 1.24 95%CI 1.12-1.38). Weight loss or degree of calcium and vitamin D supplementation after gastric bypass were not associated with fracture risk.Conclusions: Gastric bypass was associated with an increased risk of fracture and fall injury. Weight loss or calcium and vitamin D supplementation following surgery were not associated with fracture risk. These findings indicate that gastric bypass increases fracture risk, which could at least partly be due to increased susceptibility to falls.

  • Overview of Cherenkov Telescope on-board EUSO-SPB2 for the Detection of Very-High-Energy Neutrinos

    • We present the status of the development of a Cherenkov telescope to be flown on a long-duration balloon flight, the Extreme Universe Space Observatory Super Pressure Balloon 2 (EUSO-SPB2). EUSO-SPB2 is an approved NASA balloon mission that is planned to fly in 2023 and is a precursor of the Probe of Extreme Multi-Messenger Astrophysics (POEMMA), a candidate for an Astrophysics probe-class mission. The purpose of the Cherenkov telescope on-board EUSOSPB2 is to classify known and unknown sources of backgrounds for future space-based neutrino detectors. Furthermore, we will use the Earth-skimming technique to search for Very-High-Energy (VHE) tau neutrinos below the limb (E > 10 PeV) and observe air showers from cosmic rays above the limb. The 0.785 m2 Cherenkov telescope is equipped with a 512-pixel SiPM camera covering a 12.8° x 6.4° (Horizontal × Vertical) field of view. The camera signals are digitized with a 100 MS/s readout system. In this paper, we discuss the status of the telescope development, the camera integration, and simulation studies of the camera response.

    • Between May and December 1998, tick-associated febrile illness was prospectively studied in Southeast Sweden in order to assess the occurrence of human granulocytic ehrlichiosis (HGE). Inclusion criteria were fever (greater than or equal to 38.0degreesC), with or without headache, myalgia or arthralgia in patients with an observed tick bite or tick exposure within 1 month prior to onset of symptoms. Patients with clinical signs of Lyme borreliosis were included. Of the 27 patients included, we identified 4 cases of HGE. Three of the patients had coinfection with Lyme borreliosis, which presented as erythema migrans. All 27 patients presented with a 2-5 d history of fever. None of the clinical signs or laboratory parameters monitored was helpful in predicting ehrlichiosis in this group with tick-associated fever conditions. Within the HGE-negative group (n = 23), 12 patients had clinical or laboratory signs of Lyme borreliosis. For 11 patients, the actiology of the fever remained unclear. Our results suggest that HGE is common in tick-infested areas of Southeast Sweden, and may occur as a coinfection of Lyme borreliosis. Granulocytic ehrlichiosis should be suspected in patients who present with tick-associated fever, with or without erythema migrans. Ehrlichia serology and PCR should be employed to confirm the diagnosis.

    • Some of the common soil microorganisms are actinomycetes, Gram-positive bacteria with high GC content. Because of their mycelial habit they were initially believed to be fungi, which reected in their name (mucus (lat.) means fungus). In 1939, one year before rediscovery of penicillin by Florey and Chain, soil microbiologist Waksman has set his lab on a quest for new antimicrobial drugs. From the previous studies he knew that actinomycetes can inhibit the growth of other soil bacteria through secretion of bioactive compounds, which he named “antibiotics” (anti (lat.) against, bio (lat.) life). Systematic search for antibiotics produced by actinomycetes resulted in the discovery of actinomycin (1940), clavacin, and streptothricin (1942), all of them sadly turned out to be toxic in animal tests. In 1943 Waksman’s student Schatz isolated streptomycin-producing strain of Streptomyces griseus.1 Streptomycin was not particularly toxic to animals and humans, but remarkably was the rst compound active against tuberculosis bacteria. Many pharmaceutical companies and research laboratories started to collect soil samples from all over the world in search of antibiotics-producing organisms. Most of the discoveries were made in the rst ten years of the “hunt,” the larger part involved Streptomyces species. Streptomyces is a genus in the genera of actinomycetes, many of these bacteria produce volatile compounds that give the earth its characteristic odor. Streptomyces proved to be an excellent source of secondary metabolites, including antibiotics, anticancerous agents, antihelmintic drugs, and other useful compounds (Table 24.1). At present more than half of antibiotics in clinical use are produced in Streptomyces species.

    • Hybrid systems combining distributed Raman amplification and phase-sensitive amplifiers (PSAs) are investigated in numerical simulations. We focus on the mitigation of fiber nonlinearities and the impact of the span power map which is also important in systems employing optical phase conjugation or phase-conjugated twin waves.We simulate multi-span PSA links with and without distributed Raman amplification and show that by including distributed Raman amplification, the transmission distance increases more at optimum launch power than in the linear regime. For a 5-channel WDM QPSK PSA-amplified system, we observe a transmission reach increase by a factor of 8.1 by including ideal distributed Raman amplification.

    • We demonstrate long-haul transmission using a hybrid amplifier approach combining distributed Raman amplification and lumped phase-sensitive amplification. Aside from the well-known resulting SNR improvement, distributed Raman amplification is included in an effort to improve the nonlinearity mitigation capability of the phase-sensitive amplifiers. When changing from phase-insensitive operation to phase-sensitive operation in a link employing distributed Raman amplification, the transmission reach at BER = 10−3is increased from 15 to 44 spans of length 81 km while simultaneously increasing the optimal launch power by 2 dB.

    • Extremely low-frequency (ELF) magnetic and electric field plasma wave emissions were recorded on 2 October 1993 on auroral field lines by the Magnetic Field Experiment during Freja orbit 4770. The ELF wave frequencies were below the local oxygen gyrofrequency (25 Hz) and between the helium and proton gyrofrequencies (100 to 400 Hz). The ELF waves, interpreted as electromagnetic ion cyclotron (EMIC) waves, were observed in a region of inverted-v-type electron precipitation. The EMIC waves were correlated over time with auroral and lower energy (≈ 100 eV) electrons, which are both possible sources of free energy, and also with transversely accelerated oxygen ions. The waves above the helium gyrofrequency were more closely correlated with the transverse oxygen ion acceleration than the waves below the oxygen gyrofrequency. These observations are consistent with a scenario in which electron beams generate EMIC waves, which then produce transverse oxygen ion acceleration through a gyroresonant interaction.

    • Butadienyl dyes of the benzothiazole series with various fragments of benzocrown ethers 1a-c were synthesized for the first time. The structures and spectral properties of crown-containing butadienyl dyes and their complexes with alkali and alkaline-earth metal cations were studied by X-ray diffraction analysis and 1H-NMR, UV/VIS, and resonance Raman spectroscopy. To interpret the experimental results, quantum-chemical calculations were performed.     • This study investigated the effects of rosuvastatin on plaque progression and in vivo coronary artery function in apolipoprotein E-knockout (ApoE-KO) mice, using noninvasive high-resolution ultrasound techniques. Eight-week-old male ApoE-KO mice ( n = 20) were fed a high-fat diet with or without rosuvastatin (10 μmol·kg−1·day−1) for 16 wk. When compared with control, rosuvastatin reduced total cholesterol levels ( P < 0.05) and caused significant retardation of lesion progression in the brachiocephalic artery, as visualized in vivo using an ultrasound biomicroscope ( P < 0.05). Histological analysis confirmed the reduction of brachiocephalic atherosclerosis and also revealed an increase in collagen content in the statin-treated group ( P < 0.05). Coronary volumetric flow was measured by simultaneous recording of Doppler velocity signals and left coronary artery morphology before and during adenosine infusion. The hyperemic flow in response to adenosine was significantly greater in left coronary artery following 16 wk of rosuvastatin treatment ( P < 0.001), whereas the baseline flow was similar in both groups. In conclusion, rosuvastatin reduced brachiocephalic artery atherosclerotic plaques in ApoE-KO mice. Coronary artery function assessed using recently developed in vivo ultrasound-based protocols, also improved.

    • Adipose tissue dysfunction is considered an important contributor to systemic insulin resistance and Type 2 diabetes (T2D). Recently, a novel family of endogenous lipids, palmitic acid hydroxy stearic acids (PAHSAs), was discovered. These have anti-diabetic and anti-inflammatory effects in mice and are reduced in serum and adipose tissue of insulin resistant humans. In the present study, we investigate if adipose tissue dysfunction is associated with reduced PAHSA levels in human subjects and if PAHSAs influence adipocyte differentiation. Our results show that low expression of adipocyte GLUT4 and adipocyte hypertrophy, markers of adipose tissue dysfunction, are associated with reduced expression of key enzymes for de novo lipogenesis and adipose tissue levels of PAHSAs in human subjects. We also show that GLUT4 is not only a marker of adipose tissue dysfunction, but may be causally related to the observed impairments. PAHSAs may also act locally in the adipose tissue to improve adipogenesis through a mechanism bypassing direct activation of peroxisome proliferator-activated receptor (PPAR.). The discovery of PAHSAs and our current results provide novel insights into positive effects of lipid species in adipose tissue and mechanisms by which dysfunctional adipose tissue is associated with insulin resistance and risk of developing T2D.

    • Background and aims: To study the effects of obesity treatment with gastric bypass surgery on hospitalization for heart failure (HF) and atrial fibrillation (AF) in patients with type 2 diabetes (T2D) and obesity. We also studied the effects of gastric bypass surgery on mortality in a subgroup of individuals with preexisting heart failure.Materials and methods: In this register-based nationwide cohort study we compared individuals with T2D and obesity that underwent Roux-en-Y gastric bypass surgery (RYGB) with matched individuals with T2D and obesity that did not undergo surgery. Data was gathered by linking the Swedish National Diabetes Register and Scandinavian Obesity Surgery Registry. Matching of individuals for age, gender, BMI and calendar time was done using a time updated propensity score. The main outcome measures were hospitalization for HF and/or AF, and mortality in patients with preexisting HF. The risk for heart failure, AF and death were assessed using a Cox-proportional hazards regression model that addressed measured confounding.Results: We identified 5321 individuals with T2D and obesity that had undergone RYGB between January 2007 and December 2013 and 5321 matched controls. The individuals includedwere between 18 and 65 years old and had a BMI > 27.5 kg/m2. The follow-up time for hospitalization was until the end of 2015 (mean 4.5 years) and the end of 2016 for death. Our results show a 73% lower risk for HF (HR 0.27 (0.19, 0.38) p<0.001), 41% for AF (HF 0.59 (0.44, 0.78) p < 0.001), and 77% for concomitant AF and HF (HR0.23 (0.12, 0.46) p < 0.001) in the surgically treated group. In patients with preexisting HF we observed significantly lower mortality in the group that underwent surgery (HR0.23 (0.12, 0.43) p < 0.001).Conclusion: Bariatric surgery may reduce risk for HF and AF in patients with T2D and obesity, speculatively via positive cardiovascular and renal effects. Obesity treatment with surgery may also be a valuable alternative in selected patients with T2D and HF.

    • Lyme borreliosis and human granulocytic ehrlichiosis are tick-borne diseases caused by Borrelia burgdorferi and Anaplasma phagocytophilum, respectively. Infection with A. phagocytophilum has been observed to induce immunosuppression and animal studies suggest that the bacteria might also have prolonged inhibitory effects on immune cells. The aim of this study was to investigate the cytokine secretion in patients exposed previously to A. phagocytophilum and currently infected with B. burgdorferi compared with patients infected with B. burgdorferi and seronegative for A. phagocytophilum. Eight patients with erythema migrans and antibodies against A. phagocytophilum, 15 patients with erythema migrans and negative A. phagocytophilum serology and 15 non-exposed healthy individuals were included in the study. Blood mononuclear cells were stimulated with Borrelia-antigen and the number of cytokine [interleukin (IL)-4, IL-5, IL-12, IL-13 and interferon (IFN)-γ]-secreting cells was detected by enzyme-linked immunospot (ELISPOT). This study shows that patients with a previous exposure to A. phagocytophilum and a current infection with B. burgdorferi have a lower number of Borrelia-specific cells secreting IL-12 compared to Ap seronegative patients infected with B. burgdorferi (P < 0·001), indicating impairment in the ability to mount strong Th1-responses. We suggest that this mirrors a reduced Th1 response caused by A. phagocytophilum which could influence the outcome of the Borrelia infection and, speculatively, may also have implications in other conditions.
