    • Background: We investigate whether differences in breast cancer survival in six high-income countries can be explained by differences in stage at diagnosis using routine data from population-based cancer registries. Methods: We analysed the data on 257 362 women diagnosed with breast cancer during 2000-7 and registered in 13 population-based cancer registries in Australia, Canada, Denmark, Norway, Sweden and the UK. Flexible parametric hazard models were used to estimate net survival and the excess hazard of dying from breast cancer up to 3 years after diagnosis. Results: Age-standardised 3-year net survival was 87-89% in the UK and Denmark, and 91-94% in the other four countries. Stage at diagnosis was relatively advanced in Denmark: only 30% of women had Tumour, Nodes, Metastasis (TNM) stage I disease, compared with 42-45% elsewhere. Women in the UK had low survival for TNM stage III-IV disease compared with other countries. Conclusion: International differences in breast cancer survival are partly explained by differences in stage at diagnosis, and partly by differences in stage-specific survival. Low overall survival arises if the stage distribution is adverse (e. g. Denmark) but stage-specific survival is normal; or if the stage distribution is typical but stage-specific survival is low (e. g. UK). International differences in staging diagnostics and stage-specific cancer therapies should be investigated.

  • Chest x ray films from construction workers: International Labour Office (ILO 1980) classification compared with routine readings

    • The extent of agreement between International Labour Office (ILO) and clinical readings of chest x ray films from construction workers was studied. From a survey of 5898 workers 258 subjects with a profusion of small opacities of > or = 1/1 and a stratified sample of subjects with profusion < 1/1 were selected. Only 41% of the films classified as ILO profusion category > or = 1/1 were clinically recorded as non-normal for the parenchyma. The proportion of films recorded as pneumoconiotic (or possibly so) was especially low for irregular opacities (22%), but increased with the profusion category (both rounded and irregular) as well as with the size of rounded opacities (p 3/11, q 12/25, r 3/4). Only with the profusion category > or = 2/1 were most of the films recorded as pneumoconiotic. The specificity and sensitivity were highest in the geographical areas where a few clinical readers had assessed many films each. The proportion of false negative clinical reports was low for circumscribed pleural thickening of the chest wall (9%) and diaphragmatic pleural thickening (6%). For calcified pleural changes and for the combination of diffuse pleural thickening and obliteration of the costophrenic angle, false negative reports were absent. The present study shows an unsatisfactory sensitivity for clinical compared with ILO readings as a means for screening the parenchyma of workers with a risk of pneumoconiosis.

  • Impact of exposure to insulation wool on lung function and cough in Swedish construction workers

    • OBJECTIVES: To investigate whether application of insulation wool adversely affects lung volumes and increases the occurrence of symptoms of airway irritation. METHODS: Data from nationwide health check ups in 1981-93 of male construction workers born in 1955 or later were used to investigate cross sectional (n = 96,004) and longitudinal (n = 26,298) associations between lung volumes, vital capacity (VC), and forced expiratory volume in one second (FEV1) and exposure to insulation wool by combining a job exposure matrix (JEM) and self reported exposure. Data on 12 month prevalence of persistent cough not associated with the common cold was available for the period 1989-92. Potential confounding from smoking, exposure to asbestos, silica, and isocyanates, was considered in the analyses. RESULTS: For those in the highest exposure category (self reported duration of exposure of > or = 11 years, and high exposure according to the JEM) VC was on average 2.5 cl lower (95% CI -6.5 to 1.5) than in those with no exposure. The corresponding figures for FEV1 was -2.4 cl (95% CI -6.1 to 1.3). In the longitudinal analyses, the yearly change in VC between the first and last spirometry for those in the highest exposure category was 0.50 cl (95% CI -0.97 to 1.98) less than in the unexposed category. The corresponding figure for FEV1 was 0.89 cl (95% CI - 0.70 to 2.06). High exposure to insulation wool, asbestos, or silica, during the 12 months preceding the check up was associated with increased odds ratios (ORs) for persistent cough of the same magnitude as current smoking. CONCLUSIONS: The results indicate no effects on VC or FEV1 from exposure to insulation wool. Recent exposure to insulation wool, asbestos, and silica was associated with an increased prevalence of persistent cough.

    • In this work we present a compact, nanosecond pulsed, single frequency, single stage Yb-doped fiber amplifier by using an overall fiber core diameter of 20 μm. The key component is a custom made, compact, ultra-low noise, single frequency ring-cavity solid state laser (SSL) at 1064 nm used as a master oscillator. The SSL can be designed to provide nanosecond pulses with pulse energies in the sub-mJ range. Our ultimate goal is to develop a compact linearly polarized, single frequency, nanosecond pulsed laser source in an all-fiber format. Short (less than 1m), highly Yb-doped fibers have been used in order to suppress non-linear effects. © 2014 SPIE.

    • High-power, single-frequency, pulsed fiber amplifiers are required in light detection and ranging, coherent laser detection, and remote sensing applications to reach long range within a short acquisition time. However, the power-scaling of these amplifiers is limited by nonlinearities generated in the optical fibers, in particular by stimulated Brillouin scattering (SBS). In this regard, the use of multicomponent phosphate glasses maximizes the energy extraction and minimizes nonlinearities. Here, we present the development of a single-stage, hybrid, pulsed fiber amplifier using a custom-made multicomponent Yb-doped phosphate fiber. The performance of the phosphate fiber was compared to a commercial Yb-doped silica fiber. While the latter showed SBS limitation at nearly 6.5 kW for 40 cm length, the maximum achieved output peak power for the multicomponent Yb-doped phosphate fiber was 11.7 kW for 9 ns pulses using only 20 cm with no sign of SBS.

    • Analyses of data from cancer registries have shown a 10% unit difference in 5-year relative survival between Danish and Swedish patients with breast cancer. This study investigates the effect of age and patho-anatomic variables on this survival difference. Hospital records were collected for women over 40 years of age diagnosed in 1989 or 1994 in east Denmark and south Sweden; patho-anatomical variables and survival were compared between 2289 Danish and 1715 Swedish women. Tumours were smaller, node-negative axillae more frequent and well-differentiated tumours almost 10% more frequent in Sweden. A superior 5-year relative survival in Sweden was found in the 50- to 79-year age group. The adjusted hazard rate ratio between countries was 1.7 in 1989 and 1.3 in 1994. Conditional survival after surviving the first 5 years was similar for the two countries. Adjusting for patho-anatomical variables reduced but did not eliminate the higher risk of death among the Danish patients. Higher population death rates could explain some but not all of the residual elevated risk for Danish women. (C) 2004 Elsevier Ltd. All rights reserved.

    • The 5-year relative survival from breast cancer in Denmark is 10 percentage points lower than in Sweden. This difference has been demonstrated previously as being caused partly by more involved lymph nodes and larger tumours in Denmark. Sweden has had nationwide mammography-screening coverage since 1991, whereas this is still in its infancy in Denmark. In the search for an explanation for the remaining survival difference, patient delay was a likely candidate. This study compared patient delay and mammography-detection between two national regions. Data on patient delay and mammography were obtained from hospital records from 1989 and 1994, and analysed using Cox proportional hazard analysis of death within the first 5 years, with the factors age, country, delay/mammography detection and established patho-anatomic variables. A comparison of patient delay and mammography detection in 1989 and 1994 showed more mammography-detected tumours in south Sweden and more women with long delay in east Denmark. Mammography detection, but not long patient delay, had a significant effect on the death hazard when adjusting for patho-anatomic risk factors. The hazard ratio was not eliminated in 1989, but in 1994, the hazard ratio between east Denmark and south Sweden was reduced from 1.3 to 1.1. In conclusion, patient delay did not appear to have any effect on 5-year survival when adjusting for patho-anatomic factors, but tumour detection by mammography affected survival favourably and partly explained the survival difference between east Denmark and south Sweden.     • Celestial mechanical simulations from a purely classical point of view of the solar system, including our Moon and the Mars moons {Phobos and Deimos {are carried out for 2 millions of years before present. Within the classical approximation, the results are derived at a very high level of accuracy. Effects from general relativity for a number of variables are investigated and found to be small. For climatic studies of about 1 Myr, general relativity can safely be ignored. Three different and independent integration schemes are used in order to exclude numerical anomalies. The converged results from all methods are found to be in complete agreement. For verification, a number of properties such as spin axis precession, nutation, and orbit inclination for Earth and Mars have been calculated. Times and positions of equinoxes and solstices are continously monitored. As also observed earlier, the obliquity of the Earth is stabilized by the Moon. On the other hand, the obliquity of Mars shows dramatic variations. Climatic influences due to celestial variables for the Earth and Mars are studied. Instead of using mean insolation as in the usual applications of Milankovitch theory, the present approach focuses on the instantaneous solar radiation power (insolation) at each summer solstice. Solar radiation power is compared to the derivative of the icevolume and these quantities are found to be in excellent agreement. Orbital precessions for the inner planets are studied as well. In the case of Mercury, it is investigated in detail.

    • Background As musculoskeletal disorders are common in general populations and they therefore constitute a heavy burden to society, the identification of their determinants is important. The aim of this study was to identify dose-response associations for location-specific prevalences of musculoskeletal disorders with various physical exposures and psychosocial factors in a large group of workers. Methods The study was based on cross-sectional data from self-administered questionnaires filled out by 85 191 male employees in the Swedish construction industry. In addition to symptoms from nine musculoskeletal locations, the questionnaire addresses physical exposures and psychosocial factors. Logistic regression methods were used to study disorder prevalence in relation to scales of these exposures and factors. Results Compared with working rarely in a stooping or twisted posture, the odds ratio (OR) for lower back disorder with respect to often working in this posture was 3.05 [95% confidence interval (95% CI) 2.73-3.42]. The OR values for work with hands above the shoulders and for neck and shoulder disorders were 2.93 (95% CI 2.65-3.24) and 3.66 (95% CI 3.32-4.04), respectively. Among the psychosocial factors, sleeping problems and hurrying without reason were associated with symptoms in all of the body locations, the highest OR being found for neck symptoms and sleeping problems, 2.44 (95% CI 2.15-2.78). Conclusions Work in awkward postures has strong location-specific dose-response associations. In contrast, some psychosocial factors, primarily those reflecting characteristics of individual persons, are strongly associated with symptoms in all body locations. Symptoms seem not to be associated with factors like support from workmates or supervisors or control of the work situation. All location-specific prevalence rates seem to increase strongly with age.

    • A structural investigation of a synthesized precursor in a silica glass matrix is performed. Silica soot samples are doped with the heterobimetallic precursor ErNb2(OPri)13 by using a conventional solution doping technique and heat treatments to different temperatures. The precursor has also been introduced into a silica fiber preform by using the modified chemical vapor deposition technique. Analyses are made by using ultraviolet–visible–near infrared absorption spectroscopy, scanning electron microscopy, energy dispersive spectroscopy and powder X-ray diffraction. It is concluded that an immiscible system of ErNbO4 crystallites and Nb2O5 is formed in the silica soot samples at high temperatures. Colloidal particles of ErNbO4 are also formed in the silica glass fiber preform showing interesting features.

    • Breast cancer survival differs 9 percentage points between the neighbouring countries of Denmark and Sweden. The authors' aim was to analyse whether this was caused by early detection in Sweden. The extent of disease and outcome was compared in two population-based breast cancer cohorts in 1983-1989. Breast cancer management was decentralized in Denmark without mammography screening whereas treatment in Sweden was centralized and the population partly screened. Ten- and 15-year relative survival was 15% and 6% higher in Sweden (p <0.001) with corresponding differences in crude and disease-specific survival. Stage distribution was significantly more favourable in the Swedish cohort. In multivariate analysis age, tumour size, extent of axillary surgery, and spread affected survival; however, the impact of region persisted (p <0.001). Reanalysis without screening-detected patients only slightly affected the impact of region. It was concluded that early detection had significant impact on survival but other regional differences might be of importance.

    • INTRODUCTION: Breast cancer follow-up programmes consume large resources and despite the indications that several alternative approaches could be used effectively, there is no coherent discussion about costs and/or cost-effectiveness of follow-up programmes. PATIENT AND METHODS: In a prospective trial there were 264 breast cancer patients, stage I and II, randomised to two different follow-up programmes- PG (physician group) and NG (nurse group). The trial period was 5 years. The women in the two intervention groups did not differ in anxiety and depression, their satisfaction with care, their experienced accessibility to the medical centre or their medical outcome as measured by recurrence or death. The analyses were done from different lists representing costs at three hospitals in Sweden according to the principles of a cost minimization study. RESULTS: The cost per person year of follow-up differed between the groups, with 630 euro per person year in PG compared to 495 euro per person year in NG. Thus, specialist nurse intervention with check-ups on demand was 20% less expensive than routine follow-up visits to the physician. The main difference in cost between the groups was explained by the numbers of visits to the physician in the respective study arms. There were 21% more primary contacts in PG than NG. DISCUSSION: The difference in cost per year and patient by study arm is modest, but transforms to nearly 900 euro per patient and 5-year period, offering a substantial opportunity for reallocating resources since breast cancer is the most prevalent tumour worldwide.

    • The value of routine follow-up with frequent visits to a breast cancer specialist - both in terms of detection of recurrence and patient satisfaction - has been questioned. The aim of this study was to compare nurse-ted follow-up on demand versus physician follow-up after breast cancer treatment with regards to patients’ well-being, satisfaction, access to medical care and medical safety. Two hundred and sixty-four consecutively selected women with newly diagnosed breast cancer, classified as UICC stage I or stage II, were randomised to follow-up at two hospitals in Sweden, either by routine medical follow-up, the physician group (PG, n = 131), or on demand by a specialist nurse, the nurse group (NG, n = 133). Measures were done at baseline and twice a year over a period of 5 years by means of a questionnaire containing the Hospital Anxiety and Depression Scale (HAD), and the Satisfaction and Accessibility (SaaC) scale. Number of contacts with the health care services, number of diagnostic procedures, and time to recurrence or death were monitored. The ratings of HAD and SaaC did not show any statistically significant differences between the groups. The levels of anxiety and depression were generally low and levels of patient satisfaction high. There were no differences between the groups concerning time to recurrence or death. This study indicates that women with breast cancer in stages I to II can be followed up by a specialist nurse with high patient satisfaction and good medical safety. © 2004 Published by Elsevier Ltd.

    • To evaluate the effect of the Danish Sun Safety Campaign 2007–2015 on the prevalence of sunbed use and to model future effects on the skin cancer incidences 2007–2040.DesignThe study has a repeated, cross-sectional design.SettingExposure to ultraviolet radiation is the main risk factor for skin cancer. Denmark has the highest prevalence of sunbed use reported and one of the highest incidences of skin cancer worldwide.ParticipantsDuring 2007–2015, survey data were collected for 37 766 Danes, representative of the Danish population with regards to age, gender and region.InterventionsIn 2007, an ongoing long-term antisunbed campaign was launched in Denmark.Primary and secondary outcome measuresSunbed use was evaluated by annual cross-sectional surveys. Skin cancer incidence was modelled in the Prevent programme, using population projections, historic cancer incidence, sunbed use exposure and relative risk of sunbed use on melanoma.ResultsThe prevalence of recent sunbed use in Denmark was reduced from 32% and 18% to 13% and 8% for women and men, respectively. The campaigns results during 2007–2015 are estimated to reduce the number of skin cancer cases from more than 5000 (746malignant melanoma, 1562 SCC, 2673 BCC) totally during 2007–2040. Keeping the 2015 level of sunbed use constant by continued campaign pressure or introduction of structural interventions would potentially prevent more than 750 skin cancer cases annually in 2040 and 16 000 skin cancer cases in total during 2007–2040.ConclusionWe have shown the value of prevention and of long-term planning in prevention campaigning. Sunbed use was reduced significantly during 2007–2015 and further reductions are possible by structural interventions. Consequently, significantly fewer skin cancer cases are anticipated during 2007–2040. The Danish Parliament has population support to enforce structural interventions to avoid a large burden of this disease.
