  • Impact of post-hepatectomy liver failure on morbidity and short- and long-term survival after major hepatectomy

    • Background Post-hepatectomy liver failure (PHLF) is one of the most serious postoperative complications after hepatectomy. The aim of this study was to assess the impact of the International Study Group of Liver Surgery (ISGLS) definition of PHLF on morbidity and short- and long-term survival after major hepatectomy. Methods This was a retrospective review of all patients who underwent major hepatectomy (three or more liver segments) for various liver tumours between 2010 and 2018 at two Swedish tertiary centres for hepatopancreatobiliary surgery. Descriptive statistics, regression models, and survival analyses were used. Results A total of 799 patients underwent major hepatectomy, of which 218 patients (27 per cent) developed ISGLS-defined PHLF, including 115 patients (14 per cent) with ISGLS grade A, 76 patients (10 per cent) with grade B, and 27 patients (3 per cent) with grade C. The presence of cirrhosis, perihilar cholangiocarcinoma, and gallbladder cancer, right-sided hemihepatectomy and trisectionectomy all significantly increased the risk of clinically relevant PHLF (grades B and C). Clinically relevant PHLF increased the risk of 90-day mortality and was associated with impaired long-term survival. ISGLS grade A had more major postoperative complications compared with no PHLF but failed to be an independent predictor of both 90-day mortality and long-term survival. The impact of PHLF grade B/C on long-term survival was no longer present in patients surviving the first 90 days after surgery. Conclusions The presently used ISGLS definition for PHLF should be reconsidered regarding mortality as only PHLF grade B/C was associated with a negative impact on short-term survival; however, even ISGLS grade A had clinical implications. The aim was to assess the ISGLS criteria for post-hepatectomy liver failure (PHLF) in a cohort of patients with major hepatectomy. The presently used ISGLS definition for PHLF should be reconsidered regarding mortality as only PHLF grade B/C was associated with a negative impact on short-term survival.

  • Wood and fibre mechanics related to the thermomechanical pulping process

    • The main objective of this thesis was to improve the understanding of some aspects on wood and fibre mechanics related to conditions in the thermomechanical pulping process. Another objective was to measure the power distribution between the rotating plates in a refiner.   The thesis comprises the following parts: –A literature review aimed at describing fracture in wood and fibres as related to the thermomechanical pulping process –An experimental study of fracture in wood under compression, at conditions similar to those in feeding of chips into preheaters and chip refiners –An experimental study of the effect of impact velocity on the fracture of wood, related to conditions of fibre separation in the breaker bar zone in a chip refiner –A micromechanical model of the deterioration of wood fibres, related to the development of fibre properties during the intense treatment in the small gap in the refining zone –Measurements of the power distribution in a refiner.   The fracture in wood under compression was investigated by use of acoustic emission monitoring. The wood was compressed in both lateral and longitudinal directions to predict preferred modes of deformation in order to achieve desired irreversible changes in the wood structure. It was concluded that the most efficient compression direction in this respect is longitudinal. Preferable temperature at which the compression should be carried out and specific energy input needed in order to achieve substantial changes in the wood structure were also given.   The fibre separation step and specifically the effect of impact velocity on the fracture energy were studied by use of a falling weight impact tester. The fracture surfaces were also examined under a microscope. An increase in impact velocity resulted in an increase in fracture energy. In the thermomechanical pulping process the fibres are subjected to lateral compression, tension and shear which causes the creation of microcracks in the fibre wall. This damage reduces the fibre wall stiffness. A simplified analytical model is presented for the prediction of the stiffness degradation due to the damage state in a wood fibre, loaded in uni-axial tension or shear. The model was based on an assumed displacement field together with the minimum total potential energy theorem. For the damage development an energy criterion was employed. The model was applied to calculate the relevant stiffness coefficients as a function of the damage state. The energy consumption in order to achieve a certain damage state in a softwood fibre by uniaxial tension or shear load was also calculated. The energy consumption was found to be dependent on the microfibril angle in the middle secondary wall, the loading case, the thicknesses of the fibre cell wall layers, and conditions such as moisture content and temperature. At conditions, prevailing at the entrance of the gap between the plates in a refiner and at relative high damage states, more energy was needed to create cracks at higher microfibril angles. The energy consumption was lower for earlywood compared to latewood fibres. For low microfibril angles, the energy consumption was lower for loading in shear compared to tension for both earlywood and latewood fibres. Material parameters, such as initial damage state and specific fracture energy, were determined by fitting of input parameters to experimental data. Only a part of the electrical energy demand in the thermomechanical pulping process is considered to be effective in fibre separation and developing fibre properties. Therefore it is important to improve the understanding of how this energy is distributed along the refining zone. Investigations have been carried out in a laboratory single-disc refiner. It was found that a new developed force sensor is an effective way of measuring the power distribution within the refining zone. The collected data show that the tangential force per area and consequently also the power per unit area increased with radial position. The results in this thesis improve the understanding of the influence of some process parameters in thermomechanical pulping related wood and fibre mechanics such as loading rate, loading direction, moisture content and temperature to separate the fibres from the wood and to achieve desired irreversible changes in the fibre structure. Further, the thesis gives an insight of the spatial energy distribution in a refiner during thermomechanical pulping.

  • Zygoma fixture in the management of advanced atrophy of the maxilla: technique and long-term results.

    • Despite refinements in surgical technique, including bone grafting and sophisticated prosthetic reconstructions, there are limitations to what can be achieved with bone-anchored fixed prostheses in patients with advanced atrophy of the maxillae. A new approach was suggested by a long-term study on onlay bone grafting and simultaneous placement of a fixture based on a new design: the zygoma fixture, and the aim of this study was to assess its potential. Twenty-eight consecutive patients with severely resorbed edentulous maxillae were included, 13 of whom had previously had multiple fixture surgery in the jawbone that had failed. A total of 52 zygoma fixtures and 106 conventional fixtures were installed. Bone grafting was deemed necessary in 17 patients. All patients have been followed for at least five years, and nine for up to 10 years. All patients were followed up with clinical and radiographic examinations, and in some cases rhinoscopy and sinoscopy as well. Three zygoma fixtures failed; two at the time of connection of the abutment and the third after six years. Of the conventional fixtures placed at the time of the zygoma fixture, 29 (27%) were lost. The overall prosthetic rehabilitation rate was 96% after at least five years of function. There were no signs of inflammatory reaction in the surrounding antral mucosa. Four patients with recurrent sinusitis recovered after inferior meatal antrostomy. To conclude, the zygoma fixture seems to be a valuable addition to our repertoire in the management of the compromised maxilla.

  • Improved refining energy efficiency in thermo-mechanical pulping by means of collimated wood chipping – from solid mechanics to full scale evaluation

    • The wood chipping process was never optimized with regard to high yield pulping processes as thermomechanical pulping (TMP) and chemithermomechanical pulping (CTMP). It is generally believed that wood chips for pulping should be produced in such a way that the degree of damage is minimized and that the chip dimensional distribution should be as narrow as possible. Since the TMP and CTMP processes were developed in the 60-ies and 70-ies, compression screw as well as roll nip equipment have been developed to pretreat wood chips as a way to reduce refining energy consumption to given fiber and pulp properties and also in order to improve impregnation. The general conclusions are that a combination of shear and compression in the tangential or radial direction of the wood initiates cracks that later in the refiner will enhance and optimize fiber separation and also fiber property development. The idea with the collimated chipping technology is to utilize the wood chipper as a tool, combining cutting of wood logs to wood chips with a pretreatment of the chips by creating cracks that would enhance fiber separation, fiber surface development as well as chip-impregnation. In this case the compression is performed in the wood fiber direction, in which direction wood actually is weakest when it comes to compression induced cracking. The maximization of the amount of cracks in wood-chips is performed by optimizing the knife angle (or spout angle) in the chipper, to what we call collimated chipping (according to a patent owned by CCT AB). This presentation describes a theoretical background and two demonstration scale studies performed by SCA Forest Products at their Ortviken mill. One conclusion drawn, based on two-month test period with three weeks of collimated chipping, was that specific refining energy reduction was around 100 kWh/adt of the 1400 kWh/adt used in primary stage double disc refining. Most probably the potential is higher if the whole system is optimized. Tests were performed at constant production rate and energy was reduced by reducing power to constant freeness, leading to similar tensile and light scattering levels. The deliberately increased forces created in the wood chipper by means of an optimized (increased) edge angle caused more problems with knife holder equipment than normally, as well as increased vibrations. These problems will have to be solved for future long-term implementation of the technique.

    • This patent application describes a method to produce wood chips with the intention of reducing the energy consumption in the subsequent process steps for pulp production. With the present method wood chipping is done in a wood chipper where the chipping tool (3) has an angle γ (4) within the interval of 75° to 105° between the fibre direction of the log and the side of the tool which faces the chip (2). Angles in this interval will cause an axially directed compression of the chip which will cause a cracking of the wood during chipping.

    • It has recently been shown that it is possible to improve the energy efficiency during first stage TMP (thermomechanical pulp) refining by utilizing a modified chipping method (collimated chipping). A patent application regarding this method has been filed and is pending. The modification consists in that the spout angle i.e. the angle between the fibre direction of the wood specimen and the cutting plane, is increased. This paper reports the differences in properties of IMP refined from chips produced at two different spout angles i.e. 30 and 50 with and without the addition of sodium bisulphite (NaHSO3) to the dilution water. It was found that the specific energy input for a certain CSF (Canadian Standard Freeness) value was lower for chips produced at spout 50 but that the addition of chemicals to the dilution water had no influence on the specific energy value for a given CSF value. However, the tensile index and specific light scattering coefficient was substantially higher for handsheets made of the pulp refined from chips produced at spout angle 50 and with NaHSO3 added compared to handsheets from pulp made from 30 and 50 chips without chemicals added.

    • Spruce woodchipswere produced under well-controlled conditions in a laboratory woodchipper at spout angles of 30°, 40°, and 50° at a cutting rate of 20 m s-1 and with a nominalchip length of 25 mm. Thechips were then refined under thermomechanical pulp (TMP) conditions in a pilot refiner plant. The pulpproperties such as freeness, average fiber length, and shives content were determined and evaluated as a function of specific energy consumption. For a first stage refining and for a freeness value of 350 ml, a decrease in specific electrical energy consumption could be achieved by performing thewood chipping at a spout angle of 50° as compared to 30° which is the spout angle commonly used. A patent application regarding this method has been filed and is pending. It is realized that a freeness value is not directly indicative of any quality measure, such as, for example tensile index and light scattering coefficient but the obtained results can be interpreted to be promising. Further studies are needed regarding the impact of the modified chipping process.

  • Distributions Of Fiber Characteristics As A Tool To Evaluate Mechanical Pulps

    • Mechanical pulps are used in paper products such as magazine or news grade printing papers or paperboard. Mechanical pulping gives a high yield; nearly everything in the tree except the bark is used in the paper. This means that mechanical pulping consumes much less wood than chemical pulping, especially to produce a unit area of printing surface. A drawback of mechanical pulp production is the high amounts of electrical energy needed to separate and refine the fibers to a given fiber quality. Mechanical pulps are often produced from slow growing spruce trees of forests in the northern hemisphere resulting in long, slender fibers that are well suited for mechanical pulp products. These fibers have large varieties in geometry, mainly wall thickness and width, depending on seasonal variations and growth conditions. Earlywood fibers typically have thin walls and latewood fibers thick. The background to this study was that a more detailed fiber characterization involving evaluations of distributions of fiber characteristics, may give improved possibilities to optimize the mechanical pulping process and thereby reduce the total electric energy needed to reach a given quality of the pulp and final product. This would result in improved competitiveness as well as less environmental impact. This study evaluated the relation between fiber characteristics in three types of mechanical pulps made from Norway spruce (Picea abies), thermomechanical pulp(TMP), stone groundwood pulp (SGW) and chemithermomechanical pulp (CTMP). In addition, the influence of fibers from these pulp types on sheet characteristics, mainly tensile index, was studied. A comparatively rapid method was presented on how to evaluate the propensity of each fiber to form sheets of high tensile index, by the use of raw data from a commercially available fiber analyzer (FiberLabTM). The developed method gives novel opportunities of evaluating the effect on the fibers of each stage in the mechanical pulping process and has a potential to be applied also on‐line to steer the refining and pulping process by the characteristics of the final pulp and the quality of the final paper.The long fiber fraction is important for the properties of the whole pulp. It was found that fiber wall thickness and external fibrillation were the fibercharacteristics that contributed the most to tensile index of the long fiber fractions in five mechanical pulps (three TMPs, one SGW, one CTMP). The tensile index of handsheets of the long fiber fractions could be predicted by linear regressions using a combination of fiber wall thickness and degree of external fibrillation. The predicted tensile index was denoted BIN, short for Bonding ability INfluence. This resulted in the same linear correlation between BIN and tensile index for 52 samples of the five mechanical pulps studied, each fractionated into five streams(plus feed) in full size hydrocyclones. The Bauer McNett P16/R30 (passed 16 meshwire, retained on a 30 mesh wire) and P30/R50 fractions of each stream were used for the evaluation. The fibers of the SGW had thicker walls and a higher degree of external fibrillation than the TMPs and CTMP, which resulted in a correlation between BIN and tensile index on a different level for the P30/R50 fraction of SGW than the other pulp samples. A BIN model based on averages weighted by each fiber´s wall volume instead of arithmetic averages, took the fiber wall thickness of the SGW into account, and gave one uniform correlation between BIN and tensile index for all pulp samples (12 samples for constructing the model, 46 for validatingit). If the BIN model is used for predicting averages of the tensile index of a sheet, a model based on wall volume weighted data is recommended. To be able to produce BIN distributions where the influence of the length or wall volume of each fiber is taken into account, the BIN model is currently based on arithmetic averages of fiber wall thickness and fibrillation. Fiber width used as a single factor reduced the accuracy of the BIN model. Wall volume weighted averages of fiber width also resulted in a completely changed ranking of the five hydrocyclone streams compared to arithmetic, for two of thefive pulps. This was not seen when fiber width was combined with fiber wallthickness into the factor “collapse resistance index”. In order to avoid too high influence of fiber wall thickness and until the influence of fiber width on BIN and the measurement of fiber width is further evaluated, it is recommended to use length weighted or arithmetic distributions of BIN and other fiber characteristics. A comparably fast method to evaluate the distribution of fiber wall thickness and degree of external fibrillation with high resolution showed that the fiber wallthickness of the latewood fibers was reduced by increasing the refining energy in adouble disc refiner operated at four levels of specific energy input in a commercial TMP production line. This was expected but could not be seen by the use of average values, it was concluded that fiber characteristics in many cases should be evaluated as distributions and not only as averages.BIN distributions of various types of mechanical pulps from Norway spruce showed results that were expected based on knowledge of the particular pulps and processes. Measurements of mixtures of a news‐ and a SC (super calendered) gradeTMP, showed a gradual increase in high‐BIN fibers with higher amounts of SCgrade TMP. The BIN distributions also revealed differences between the pulps that were not seen from average fiber values, for example that the shape of the BINdistributions was similar for two pulps that originated from conical disc refiners, a news grade TMP and the board grade CTMP, although the distributions were on different BIN levels. The SC grade TMP and the SC grade SGW had similar levels of tensile index, but the SGW contained some fibers of very low BIN values which may influence the characteristics of the final paper, for example strength, surface and structure. This shows that the BIN model has the potential of being applied on either the whole or parts of a papermaking process based on mechanical or chemimechanical pulping; the evaluation of distributions of fiber characteristics can contribute to increased knowledge about the process and opportunities to optimize it.

    • An important aspect of precision medicine is to probe the stability in molecular profiles among healthy individuals over time. Here, we sample a longitudinal wellness cohort with 100 healthy individuals and analyze blood molecular profiles including proteomics, transcriptomics, lipidomics, metabolomics, autoantibodies andimmune cell profiling, complementedwith gut microbiota composition and routine clinical chemistry. Overall, our results show high variation between individuals across different molecular readouts, while the intra-individual baseline variation is low. The analyses show that each individual has a unique and stable plasma protein profile throughout the study period and that many individuals also show distinct profiles with regards to the other omics datasets, with strong underlying connections between the blood proteome and the clinical chemistry parameters. In conclusion, the results support an individual-based definition of health and show that comprehensive omics profiling in a longitudinal manner is a path forward for precision medicine.

    • Nonwoven fibrous materials with reticular support of an interconnected fiber network and a tortuous airflow pathway have been commonly used in filtration applications. To meet the criteria of filter efficiency and performance, the filter materials are recommended to contain different types of fibers such as mechanical pulp fibers, microfibrillated cellulose, cellulose nanofibers, and other polymer or synthetic fibers with a range of dimensions, i.e., length and diameter. Cellulose fibers in filter media possess irregular and complex structures with hollow or collapsed lumen structures owing to their refinement or pulping method. The development of an appropriate filter media model requires information on actual fiber characteristics. In this study, a simulation method was used to investigate the complex microstructures of filter media. The physical parameters such as fiber wall thickness, diameter, length, cross-section shapes, and curliness were obtained from fiber analyzers and scanning electron microscopy. Based on the experimental findings, GeoDict database comprising different types of common fiber models was constructed. 3-Dimensional fibrous models corresponding to the wet-laid binderless filter material were generated. Using the GeoDict modules, the pore size distributions, average pore sizes, air permeability, pressure drop and initial filter efficiency simulations were performed. The simulation results appear to be in close agreement with the experimental results. The incorporation of cellulose nanofibers resulted in reduced average pore sizes and air permeability of the filter material, thus enhancing the initial filter efficiency. The filter media developed a biobased material derived from pulp fibers for advanced applications such as medical facemask, and air filtration purposes.

    • This study describes a novel sustainable concept for the scalable direct fabrication and functionalization of nanocellulose from wood pulp with reduced energy consumption. A central concept is the use of metal-free small organic molecules as mediators and catalysts for the production and subsequent versatile surface engineering of the cellulosic nanomaterials via organocatalysis and click chemistry. Here, organoclick chemistry enables the selective functionalization of nanocelluloses with different organic molecules as well as the binding of palladium ions or nanoparticles. The nanocellulosic material is also shown to function as a sustainable support for heterogeneous catalysis in modern organic synthesis (e.g., Suzuki cross-coupling transformations in water). The reported strategy not only addresses obstacles and challenges for the future utilization of nanocellulose (e.g., low moisture resistance, the need for green chemistry, and energy-intensive production) but also enables new applications for nanocellulosic materials in different areas.

    • Background In Western countries the prevalence of Helicobacter pylori (H. pylori) infection may be declining but there is a lack of recent longitudinal population studies. We evaluated the changing epidemiology over a 23-year period in Sweden.Materials and methods In 1989, the validated Abdominal Symptom Questionnaire (ASQ) was mailed to a random sample of inhabitants (ages 22-80 years) in a Swedish community, and 1097 (87%) responded. H. pylori serology was analysed in a representative subsample (n=145). Twenty-three years later, the ASQ was mailed again using similar selection criteria, and 388 out of 1036 responders had an upper endoscopy with assessment of H. pylori and corpus atrophy status.Results The prevalence of positive H. pylori serology decreased from 37.9% (1989) to 15.8% (2012), corresponding to a decrease in odds of 75% per decade (odds ratio (OR): 0.25; 95% confidence interval (CI): 0.11-0.59, p=0.001) independent of age, gender, body mass index (BMI) and level of education, with a pattern consistent with a birth cohort effect. The prevalence increased with increasing age (p=0.001). The prevalence of H. pylori on histology in 2012 was 11.4% (95% CI 8.6-15.0). The prevalence of corpus atrophy on serology and/or histology in 2012 was 3.2% (95% CI 1.8-5.5); all cases were 57 years old.Conclusion The stomach is healthier in 2012 compared with 1989. H. pylori prevalence in adults has decreased over the last two decades to a level where clinical management might be affected.

    • Fabrication of paper-based packaging materials is increasing and the challenge is developing a sustainable process to manufacture the materials that can compete with plastics. Employing stronger fiber in production of fiber-based materials improves the efficiency of fabrication process by using a reduced amount of biomass. Cationic starch is a well-known polysaccharide that has been introduced to paper and paperboard fibers to improve the mechanical properties of lignocellulosic fibers. The polyelectrolyte (PE) multilayer method has been popularized as a new and interesting technique to enhance the adsorption of cationic starch on the fiber for improving the strength properties of chemi-thermomechanical pulp (CTMP), chemical and kraft pulps. We have shown in our previous work that the synergistic combination of organocatalysis and PE complexes improved the mechanical properties of CTMP and TMP. In this work, we chose to expand this concept by integrating it with low-dose sulfite pretreatment of wood chips in preparation of CTMP. Thus, CTMP produced by initial sulfite pre-treatment was next surface engineered by synergistic combination of organocatalysis and PE complexes using organic acids as catalysts. The CTMP pulps, which contains 0.1-0.24 wt.% sulfur, produced by our novel pulp-engineering strategy shows a dramatic strength increase (Z- strength: up to 100 %) as compared to no surface engineering. While only sulfite pre-treatment and PE-complex surface engineering were able to improve the strength properties, it was only when the organic catalysts was present that the highest strength improvements were reached. Thus, a clear synergistic effect of the catalyst was observed.

  • Evaluation of two hydrocyclone designs for pulp fractionation

    • The process conditions and fractionation efficiency of two hydrocyclone designs, a novel and a conventional conical design, were evaluated. The novel design comprised a modified inlet section, where the pulp suspension had to pass a narrow ring-shaped opening, and a very compact fractionation zone. The influence of feed concentration and fine fraction mass ratio was studied. The trials were performed with never-dried, unrefined bleached chemical softwood pulp. Fractionation efficiency was evaluated in terms of change of surface roughness of handsheets made out of the fractions and the feed pulp respectively.The fractionation efficiency increased considerably with decreasing fine fraction mass ratio, especially at higher feed concentrations. This finding prompted a hypothesis on the existence of a radial gradient in the composition of the suspension inside the novel hydrocyclone. Using the novel hydrocyclone in a feed-forward fractionation system would therefore prove to be more favourable as a larger total fine fraction of better properties can be obtained. A three-stage feed-forward fractionation system was evaluated in laboratory scale. Here, it was indeed possible to extract fine fractions with improved surface properties in each of the three consecutive stages. All three fine fractions had about the same surface roughness.The fractionation performance of the novel design was benchmarked against that of a conventional, best available technology (BAT) design. In terms of fractionation efficiency, the BAT design performed better. However, the fractions produced with the novel hydrocyclone had a much smaller difference in concentration, implying a much less pronounced enrichment of fines in the fine fraction. It is unclear, to what extent the lower share of latewood fibres and the increased fines content, respectively, contributed to the improved surface roughness of the fine fractions. However, it is clear that the lower enrichment of fines in the novel hydrocyclone makes it easier to install it in industrial applications without a need for auxiliary equipment to redistribute large water flows.

Specific light scattering coefficient was higher after DD-LC refining than SD-SD refining, while the development in second stage refining was better for SD-SD pulp than for DD-LC pulp. Shive level was significantly lower for DD-LC pulp than for SD-SD pulp.

    • The aim of this study was to investigate the influence of long fibre concentration on loadability and pulp properties during LC refining of mechanical pulp. Long fibre concentration was adjusted to three different levels by screen fractionation of the pulp. The three pulps were refined in a single disc pilot scale LC refiner at similar process conditions. Increased long fibre concentration supported a larger refiner gap and resulted in less fibre cutting at a given specific energy consumption. The higher long fibre concentration probably contributed to a stronger fibre network that maintained a larger refining gap at certain specific energy consumption. Increased long fibre concentration also enabled a higher tensile index increase in the LC refiner at certain fibre length reduction. The study supports a process combining LC refining with screen fractionation, where the long fibre fraction is recycled to the refiner feed. This enables a higher loadability and a more effective utilisation of the LC refiner. By using this technology, overall specific energy consumption can be reduced if a larger share of the refining is performed in LC rather than HC refining.

Sulphite chip pre-treatment iniiicombination with LC refining in second stage, yielded a pulp without screeningand reject refining with tensile index and shives content that were similar to nonpre-treated final pulp after screening and reject refining.A pilot scale study was performed to investigate the influence of fibreconcentration on pulp properties in LC refining of mechanical pulps. MarketCTMP was utilised in all trials and fibre concentrations were controlled by meansof adjustments of the pulp consistency and by screen fractionation of the pulp. Inaddition, various refiner parameters were studied, such as no-load, gap and baredge length. Pulp with the highest fibre concentration supported a larger refinergap than pulp with low fibre concentration at a given gross power input. Fibreshortening was lower and tensile index increase was higher for long fibre enrichedpulp. The results from this study support the interesting concept of combiningmain line LC refining and screening, where screen reject is recycled to the LCrefiner inlet.It has been observed that the rotor in two-zoned refiners is not always centred,even though pulp flow rate is equal in both refining zones. This leads to unequalplate gaps, which renders unevenly refined pulp. Trials were performed in millscale, using the 72-inch TwinFlo, to investigate differences in pulp properties androtor positions by means of altering the pressure difference between the refiningzones. In order to produce homogenous pulp, it was found that uneven plate gapscan be compensated for in LC refiners with dual refining zones. Results from thedifferent flow rate adjustments indicated that the control setting with similar plategap gave the most homogenous pulp.

    • Earlier studies have shown that plate gaps are sometimes unequal in two-zoned low consistency refiners and that unequal gaps render unevenly refined pulp. It is also known that optimisation of plate gap in low consistency refining leads to improved energy efficiency. In this work, trials were made in mill scale in a modern TMP line equipped with a prototype 72 inch TwinFlo low consistency refiner in second stage. The study was designed to investigate the development of pulp properties from different rotor positions by means of altering the outlet flow rate ratio. The specific energy consumption was calculated for each refining zone and setting, based on flow rate and temperature increase. In order to produce homogenous pulp, it was found that uneven plate gaps need to be compensated in low consistency refiners with dual refining zones. Results from the different flow rate adjustments indicated that the control setting with similar plate gap gave the most homogenous pulp. However, further studies are needed to find an adequate rotor control strategy. The temperature increase in each refining zone seems to correlate well with the applied specific energy consumption in each refining zone.

    • The aim of this investigation was to find new approaches to evaluate the performance of a full sized two-zoned low-consistency refiner i.e. a refiner with two stators and one rotor in between. Data from a paper mill producing TMP from Norway spruce was used in order to find a possible way to calculate the power split between the two zones. An assumption of equal amount of fibres captured between overlapping bars was found successful in order to develop equations for the power split. The equations predicted equal power in both zones at equal disc gaps. The power was found to correlate approximately linearly with the disc gap. The power split was essential to know for calculating refining intensity expressed as specific edge load and forces on fibres in the two zones. The reduction in fibre length was about 5% at 0.17 mm disc gap corresponding to 0.03 N force on fibres and 0.7 J/m specific edge load. Disc gap, force on fibres and specific edge load was found to predict the fibre shortening with approximately equal sufficiency upon changes in power and flow rate through the refiner.

    • Chemithermomechanical pulp (CTMP) is often used in central layers of multiply paperboards due to its high bulk and strength. Such a CTMP should consist of well-separated undamaged fibres with sufficient bonding capacity. The basic objective of this work is to optimize process conditions in low-consistency (LC) refining, i. e. to select or ultimately develop new optimal LC refiner filling patterns, in order to produce fibrillar fines and improve the separation of fibres from each other while preserving the natural fibre morphology as much as possible. Furthermore, the aim is to evaluate if this type of work can be done at laboratory-scale or if it is necessary to run trials in pilot- or mill-scale in order to get relevant answers. First stage CTMP made from Norway spruce (Picea abies) was LC refined in mill-, pilot- and laboratory-scale trials and with different filling patterns. The results show that an LR1 laboratory refiner can favourably be used instead of larger refiners in order to characterize CTMP with regard to tensile index and z-strength versus bulk. A fine filling pattern resulted in CTMP with higher tensile index, z-strength and energy efficiency at maintained bulk compared to a standard filling pattern.

  • Production and application of fine fractions made of chemical pulp for enhanced paperboard strength

    • For all kinds of paperboard packages, the bending stiffness of the paperboard is a crucial property. In multi-ply folding boxboard (FBB) grades, this is obtained by placing different stocks in the outer and centre plies of the board. In the outer plies, a stock with a high tensile stiffness is used, typically made from refined kraft pulp fibres. In the middle ply/plies a stock with more bulky properties is placed, typically comprising of a high proportion of CTMP (chemi-thermomechanical pulp). CTMP fibres are stiffer and more inflexible with poor bonding abilities resulting in low strength properties. To increase the bonding strength in the middle ply, broke, containing chemical pulp is added, and sometimes refined chemical kraft pulp as well. Both fibres and fines, i.e. smaller fibre fragments, in a pulp have a significant contribution to the properties of the product. Fines produced during refining of chemical pulp are especially beneficial for increasing the strength.To achieve pulp fraction with higher fines content the pulp can be fractionated with a micro-perforated screen basket; a fine fraction produced from a screen with very small holes will contain a large proportion of fines. By adding such a fine fraction to a middle ply stock, the bulk properties of the main pulp, for example a CTMP, can be conserved as less refining of this pulp is required to achieve the targeted strength properties. However, a drawback is that the fine fraction usually has a very low mass concentration after the screening process as a lot of water pass through the screen together with the fines and fibre fragments. The excess water must be removed to maintain the water balance of the papermaking process. Further, the larger volumes require extra pumping capacity. A resource-efficient production of a fine fraction must target a high fine fraction mass concentration and a high content of fines and short fibre fragments in order to be implemented industrially.The focus of the present work was on separation efficiency (i.e. the difference in fibre length distribution caused by screening) and process efficiency (i.e. the concentration of the fine fraction) for production of a fine fraction of chemical pulp by screening, and the utilisation of the fine fraction as strength agent.Pilot-scale fractionation trials with a pressure screen with different microperforated screen baskets were performed in order to evaluate how the separation efficiency and process efficiency were affected by parameters such as feed concentration, pulp type (hardwood or softwood kraft pulp), hole size of the screen, and refining treatment prior to screening. The trials were evaluated using fibre length distributions, flow rates and concentrations of viii the feed flow and the fractions. Here, two complementary quantitative measures, Proportion in fine fraction (for process efficiency) and Fine fraction enrichment (for separation efficiency), were developed. To evaluate the strength enhancing effect of the obtained fine fraction, a lab scale study was performed where the fine fraction of a highly refined pulp was compared with the highly refined pulp as strength agent for a CTMP. The results of this study were verified in a pilot paper machine trial. In a second pilot paper machine trial, sheets with different CTMP proportions in the middle ply were studied in order to find out if the bulk could be increased while maintaining strength, by using a fine fraction made from refined chemical pulp.Regarding process efficiency, it was found that the most important parameter to obtain a high fine fraction concentration was a high feed concentration. Further, a higher fine fraction concentration for a given screening process was also obtained when using hardwood pulp and refining the pulp prior to the screening process. A higher feed concentration also had a positive effect on the separation efficiency. Small holes and a smooth surface of the screen basket were also important to improve the separation efficiency.It was shown that, when used as a strength agent in a CTMP pulp, the fine fraction of highly refined kraft pulp was twice as efficient as the highly refined kraft pulp, when added at equal mass proportion. However, both in the lab and pilot trial the strength increase was accompanied by a decreased bulk. This was expected, and to avoid this the proportion of the bulky CTMP had to be increased. The pilot paper machine trial with an increased CTMP proportion in the middle ply and a fine fraction of refined kraft pulp as strength agent demonstrated that it was possible to produce sheets with an increased bulk and maintained z-strength.

    • The objective in this work was to obtain a fine fraction of kraft pulp, with as high concentration as possible, in a pilot-scale fractionation with micro-perforated screen baskets. The influence of screen basket surface, hole size, feed concentration, pulp type and refining segment design was investigated. The results showed that a smooth screen basket surface improved the fractionation efficiency of the unrefined pulp compared to a profiled screen basket, despite a larger hole size. A significantly higher fine fraction concentration was obtained when using refined hardwood pulp compared to when using softwood pulp, which was explained with its lower average fibre length and narrower and thus more flexible fibre fragments. The pilot trials also showed that the screening process could be operated at feed concentrations similar to those directly after a refiner, 30-40 g/l. This was demonstrated in a process layout with partial recirculation where a refiner and a micro-perforated screen basket were operated in series in pilot scale.

    • In this study, fines-enriched pulp (FE-pulp)-the fine fraction of highly-refined kraft pulp-was benchmarked against highly-refined kraft pulp (HRK-pulp) as a strength agent in eucalyptus chemithermomechanical pulp (CTMP). Both the FE-pulp and the HRK-pulp were produced from unbleached softwood kraft pulp, and equal amounts of those strength agents were added to the original CTMP, as well as to washed CTMP, where most of the fines had been removed. The effects of the added strength agents were evaluated with laboratory handsheets. The FE-pulp proved to be twice as effective as HRK-pulp. Both HRK-pulp and FE-pulp increased the strength of the CTMP handsheets. The bulk of the handsheets decreased, however, as well as the drainability. The addition of 5% FE-pulp resulted in the same strength increase as an addition of 10% HRK-pulp, as well as the same decrease in bulk and CSF. For the handsheets of washed CTMP, the strengths were not measurable; the CTMP lost the sheet strength when the CTMP-fines content was reduced through washing. The reduced strength properties were compensated for by the addition of chemical pulp fines that proved to be an efficient strength agent. The addition of 5% FE-pulp restored the strength values, and at a higher bulk and higher drainability. Application: In this study, we show how the strength of a CTMP sheet can be improved by adding fine material from kraft pulp.

    • In a mechanical pulping process, (TMP) wood is refined to pulp in a process with very high wood utilization. However, the power demand in the process is high. Thus efficient energy recovery, especially steam recov-ery, is very important. In high consistency (HC) refining the pulp wood is refined at high temperature (140°C) and pressure. The high temperature makes it possible to recover process heat with usable steam properties. One strategy to decrease the power consumption is to split the refining into two stages, one HC-stage and one low consistency (LC) refining stage. This kind of sys-tem is quite common today. One drawback with LC-refining is that it operates at a low temperature normally below 100°C. Hence, the steam recovery potential from conventional LC-refining is limited. In this project, we analyse three concepts of steam re-covery in LC-refining by increasing the temperature in the LC-stage. Two base cases: Conventional HC refin-ing only and conventional HC/LC refining is compared with three steam recovery cases: Pulp/Pulp heat ex-changing, Screw Press Dewatering combined with proc-ess water re-circulation and finally Pulp/Water Heat Exchanging. The study shows that it is possible to recover steam from the LC-stage and, hence, increase the energy effi-ciency of a combined HC/LC refining system. The screw press case has the highest steam recovery poten-tial of the HC/LC configurations. An initial economic estimate indicates that steam recovery in LC-refining is profitable compared to a conventional HC/LC-configuration.

I detta projekt analyseras möjligheten att öka ångåtervinningenfrån en mekanisk massaprocess där raffinering vid hög massakoncentration (HCraffinering)kombineras med raffinering vid låg massakoncentration (LC-raffinering).Vid HC-raffinering finns sedan många år väl utvecklad teknik för att återvinna ånga.Vid konventionell LC-raffinering är det inte möjligt att återvinna ånga eftersom temperaturnivåernaär för låga, lägre än 100°C. Genom att värmeväxla producerad massa ellerpressat från processen kan man öka temperaturen i LC-steget så att det blir möjligt attåtervinna processånga med användbara ångdata, i detta fall 2,7 bar och 130°C.I projektet har tre alternativa konfigurationer för ångåtervinning analyserats(A) Ångåtervinning i kombination med massa/massa värmeväxling (B) Ångåtervinningi kombination med en trycksatt skruvpress och (C) Ångåtervinning i kombination medmassa/vatten värmeväxling.En inledande lönsamhetsberäkning visar att ångåtervinning från LC-steget kan ha enmycket god lönsamhet. Fall B, ångåtervinning i kombination med trycksatt skruvpressvisar bäst potential för fortsatta studier. Det beror på att värmeåtervinningen sker genominblandning av pressat i spädvattnet och värmeväxling av vatten med en konventionellvärmeväxlare. Det innebär att de tekniska riskerna i detta alternativ är låga.Utvecklingen av de värmeväxlare som ingår i fallen A och B och som kan hantera massamed 5% massakoncentration innebär en stor teknisk utmaning och associerad risk.Det finns idag inga sådana växlare på marknaden och det är osäkert om man kan konstrueraen växlare som kan hantera massa med hög viskositet och som har tillräckligthög temperaturverkningsgrad.Projektet har uppfyllt målet att visa att det teoretiskt är energitekniskt möjligt att modifieraen LC-process så att ånga kan återvinnas även från detta steg. Detta innebär enförbättring av den konventionella LC-processens energieffektivitet. Projektets resultatkan vara ett underlag för beslut om fortsatt forskning inom området energieffektiv LCraffinering.

  • A MicroRNA Gene Panel Predicts the Vaginal Microbiota Composition

    • The vaginal microbiota plays an essential role in vaginal health. The vaginas of many reproductive-age women are dominated by one of the Lactobacillus species. However, the vaginas of a large number of women are characterized by the colonization of several other anaerobes. Notably, some women with the non-Lactobacillus-dominated vaginal microbiota develop bacterial vaginosis, which has been correlated with sexually transmitted infections and other adverse outcomes. However, interactions and mechanisms linking the vaginal microbiota to host response are still under investigation. There are studies suggesting a link between human microRNAs and gut microbiota, but limited analysis has been carried out on the interplay of microRNAs and vaginal microbiota. In this study, we performed a microRNA expression array profiling on 67 vaginal samples from young Swedish women. MicroRNAs were clustered into distinct groups according to vaginal microbiota composition. Interestingly, 182 microRNAs were significantly elevated in their expression in the non-Lactobacillus-dominated community, suggesting an antagonistic relationship between Lactobacillus and microRNAs. Of the elevated microRNAs, 10 microRNAs displayed excellent diagnostic potential, visualized by receiver operating characteristics analysis. We further validated our findings in 34 independent samples where expression of top microRNA candidates strongly separated the Lactobacillus-dominated community from the non-Lactobacillus-dominated community in the vaginal microbiota. Notably, the Lactobacillus crispatus-dominated community showed the most profound differential microRNA expression compared with the non-Lactobacillus-dominated community. In conclusion, we demonstrate a strong relationship between the vaginal microbiota and numerous genital microRNAs, which may facilitate a deeper mechanistic interplay in this biological niche. IMPORTANCE Vaginal microbiota is correlated with women's health, where a non-Lactobacillus-dominated community predisposes women to a higher risk of disease, including human papillomavirus (HPV). However, the molecular relationship between the vaginal microbiota and host is largely unexplored. In this study, we investigated a link between the vaginal microbiota and host microRNAs in a group of young women. We uncovered an inverse correlation of the expression of microRNAs with the abundance of Lactobacillus species in the vaginal microbiota. Particularly, the expression of microRNA miR-23a-3p and miR-130a-3p, displaying significantly elevated levels in non-Lactobacillus-dominated communities, predicted the bacterial composition of the vaginal microbiota in an independent validation group. Since targeting of microRNAs is explored in the clinical setting, our results warrant investigation of whether microRNA modulation could be used for treating vaginosis recurrence and vaginosis-related diseases. Conversely, commensal bacteria could be used for treating diseases with microRNA aberrations.

    • A sustainable strategy for the neat production and surface functionalization of nanocellulose from wood pulp is disclosed. It is based on the combination of organocatalysis and click chemistry (organoclick chemistry) and starts with nanocellulose production by organic acid catalyzed hydrolysis and esterification of the pulp under neat conditions followed by homogenization. This nanocellulose fabrication route is scalable, reduces energy consumption and the organic acid can be efficiently recycled. Next, the surface is catalytically engineered by organoclick chemistry, which allows for selective and versatile attachment of different organic molecules (e.g. fluorescent probes, catalyst and pharmaceuticals). It also enables binding of metal ions and nanoparticles. This was exemplified by the fabrication of a heterogeneous nanocellulose palladium nanoparticle catalyst, which is used for Suzuki cross-coupling transformations in water. The disclosed surface functionalization methodology is broad in scope and applicable to different nanocelluloses and cellulose based materials as well.

  • Building a precision medicine infrastructure at a national level : The Swedish experience

    • Precision medicine has the potential to transform healthcare by moving from one-size-fits-all to personalised treatment and care. This transition has been greatly facilitated through new high-throughput sequencing technologies that can provide the unique molecular profile of each individual patient, along with the rapid development of targeted therapies directed to the Achilles heels of each disease. To implement precision medicine approaches in healthcare, many countries have adopted national strategies and initiated genomic/precision medicine initiatives to provide equal access to all citizens. In other countries, such as Sweden, this has proven more difficult due to regionally organised healthcare. Using a bottom-up approach, key stakeholders from academia, healthcare, industry and patient organisations joined forces and formed Genomic Medicine Sweden (GMS), a national infrastructure for the implementation of precision medicine across the country. To achieve this, Genomic Medicine Centres have been established to provide regionally distributed genomic services, and a national informatics infrastructure has been built to allow secure data handling and sharing. GMS has a broad scope focusing on rare diseases, cancer, pharmacogenomics, infectious diseases and complex diseases, while also providing expertise in informatics, ethical and legal issues, health economy, industry collaboration and education. In this review, we summarise our experience in building a national infrastructure for precision medicine. We also provide key examples how precision medicine already has been successfully implemented within our focus areas. Finally, we bring up challenges and opportunities associated with precision medicine implementation, the importance of international collaboration, as well as the future perspective in the field of precision medicine.

    • The manufacture of pulp and paper is an energy intensive process configured of several unit processes that shape a network of flows of wood chips, chemical pulp, mechanical pulp, paperboard, steam and other important components. Improved energy efficiency supports sustainability of the process and the products. With the purpose of monitoring and controlling, information from multiple process and quality variables is continuously collected in the process data system. This data may be of time-varying nature and the variability might potentially span from seasonal to time-wise shorter variations and there are in some cases a need for predicting certain properties.By applying models based on process data there is a potential to increase the knowledge of the process characteristics, investigate the applicability of predictive models and identify optimization opportunities. Based on data from an evaporation and a CTMP plant, process models have been developed with the aim of improving the energy efficiency and studying process variability.

Storage in white water gave extensive discolouration with a shoulder in the absorption spectrum at around 550-650 nm. Most of the colour was associated with pulp fines or filler but some colour was also found in smaller fractions and in the water phase. The addition of ferric ions increased the light absorption coefficient during storage, initially at short wavelengths and then over the whole spectrum, but could not explain the increased absorption at 550-650 nm and could not be the only cause of the darkening in the mill system. A cationic basic violet dye gave a shoulder in the absorption spectrum similar to that in the mill system, but the absorption in this area did not increase during storage. Model calculations indicate that ferric ions together with violet and red dyes could explain a major part, but not all, of the colour observed in the mill system after storage. The darkening not accounted for at longer wavelengths and around 550-650 nm is suggested to be related to fines and fillers including dissolved and colloidal substances associated with these particles.

  • Storage of Hydrogen Peroxide Bleached Mechanical Pulp : Reduction in Reflectance over the Visible Spectrum

    • The objective of this thesis is to determine possible causes of the darkening of hydrogen peroxide bleached mechanical pulp over the visible spectrum and their relative contributions. It focuses on both process conditions and the composition of the pulp and the dilution water, including additions or losses of material along the process line from the bleach tower to the paper machine.A mapping of the optical properties of the pulp along the process showed that the fine fraction of the pulp darkened more than the long fibre fraction. Simulation of retention times of different fractions showed that the main part of the fine material is retained in the paper within a few hours, a small part might circulate for considerably longer time and may therefore be strongly coloured.Storage trials were mainly performed using a hydrogen peroxide bleached mechanical pulp intended for SC paper made of Norway spruce (Picea abies), sampled on one occasion and stored in a freezer. Unwashed or well-washed pulp was stored in distilled water or in different process waters. Some complementary trials were included, e.g. unbleached pulp.Time and temperature were the process variables that gave the strongest darkening of the pulp, as expected, both in a clean and a more process-like system, whereas pH only had an effect in the presence of process waters; the highest brightness stability was seen at a pH around 5.5–6.0.The darkening was due to an increase in the light absorption coefficient (k) beginning at short wavelengths, but after longer storage times the increase in kλ also became noticeable at longer wavelengths. The colour (CIE L*, a*, b*) of the pulp changed towards red and yellow, initially more towards red and then more towards yellow. These changes were clearly visible.Washing of the bleached pulp made it less sensitive to storage; possibly due to the removal of extractives, lignin-like substances, metals and pulp fines. This washing had little effect before storage and the amount of material removed was small.The pulp darkened more when stored in process waters compared to distilled water. Apart from fibres, most of the colour was associated with pulp fines or filler but some colour was also found in the dissolved and colloidal fractions. At an increased pulp consistency, the increase in k460 was smaller.Storage in white water from the paper machine gave extensive discolouration with a shoulder in the absorption spectrum around 550–650 nm, which increased with time. The addition of ferric ions increased the light absorption coefficient during storage, but could not explain the increased absorption at 550–650 nm nor could it be the only cause of the darkening in the mill system. A cationic basic violet dye gave a shoulder in the absorption spectrum similar to that of the mill system, but the absorption of the dye did not increase during storage. The new method makes it possible to follow changes in light absorption and light scattering coefficients over the visible range of wavelengths. It is approximately six times faster than standard methods, reduces the risk of additional darkening of the sample and can be used with small pulp quantities.The deviation from the expected linear behaviour of the light scattering coefficient, s, at wavelengths corresponding to strong light absorption has been studied using the Kubelka-Munk model and the angular resolved DORT2002 radiative transfer solution method. The decrease in s could not be explained by errors introduced in the Kubelka-Munk modelling by anisotropic scattering.

    • To evaluate the causes behind the formation of colour in mill systems, the effect of different process waters on the optical properties of hydrogen-peroxide bleached Norway spruce mechanical pulp during storage was studied. The influence of process water type, temperature, time, pulp consistency and pH was evaluated over the visible range of wavelengths. The darkening was more pronounced when pulp was stored in process waters compared to in distilled water. Increased temperature and prolonged storage times increased the darkening as expected, whereas a higher storage consistency gave less darkening than a lower storage consistency (15% compared to 5%). The pH value that gave the lowest brightness reduction and a minimum in Delta k(460) was found to be 5.5-6.0. Storage of pulp in white water from the paper machine resulted in a broad shoulder in the k spectra indicating colour formation related to the particles in this water. Part of this shoulder in absorption spectra was related to added dyes, but the increase in absorption with time is likely to be caused by some other colour formation. The colour measured as a*, b* showed that the storage changed the colour of the pulp towards red and yellow, and initially more towards red.

    • BACKGROUND: Limited evidence exists to guide the management of patients with liver metastases from squamous cell carcinoma (SCC). The aim of this retrospective multicentre cohort study was to describe patterns of disease recurrence after liver resection/ablation for SCC liver metastases and factors associated with recurrence-free survival (RFS) and overall survival (OS).METHOD: Members of the European-African Hepato-Pancreato-Biliary Association were invited to include all consecutive patients undergoing liver resection/ablation for SCC liver metastases between 2002 and 2019. Patient, tumour and perioperative characteristics were analysed with regard to RFS and OS.RESULTS: Among the 102 patients included from 24 European centres, 56 patients had anal cancer, and 46 patients had SCC from other origin. RFS in patients with anal cancer and non-anal cancer was 16 and 9 months, respectively (P = 0.134). A positive resection margin significantly influenced RFS for both anal cancer and non-anal cancer liver metastases (hazard ratio 6.82, 95 per cent c.i. 2.40 to 19.35, for the entire cohort). Median survival duration and 5-year OS rate among patients with anal cancer and non-anal cancer were 50 months and 45 per cent and 21 months and 25 per cent, respectively. For the entire cohort, only non-radical resection was associated with worse overall survival (hazard ratio 3.21, 95 per cent c.i. 1.24 to 8.30).CONCLUSION: Liver resection/ablation of liver metastases from SCC can result in long-term survival. Survival was superior in treated patients with liver metastases from anal versus non-anal cancer. A negative resection margin is paramount for acceptable outcome.

  • The Resection Rate of Synchronously Detected Liver and Lung Metastasis from Colorectal Cancer Is Low-A National Registry-Based Study

    • Simple Summary Real-life data on the occurrence and treatment of synchronously detected liver and lung metastases from colorectal cancer are lacking. Through the merging of several Swedish nationwide patient quality registries, we aimed to answer these questions. We found that synchronous liver and lung colorectal metastases are rare and that a minority undergo resection of both metastatic sites, but if they do, they have an excellent survival. It is likely that a larger proportion of patients could be offered treatment that leads to a prolonged overall survival. We also found differences in regional treatment approaches across Sweden, but the reasons for this are unknown, which warrants further studies. Population-based data on the incidence and surgical treatment of patients with colorectal cancer (CRC) and synchronous liver and lung metastases are lacking as are real-life data on the frequency of metastasectomy for both sites and outcomes in this setting. This is a nationwide population-based study of all patients having liver and lung metastases diagnosed within 6 months of CRC between 2008 and 2016 in Sweden identified through the merging of data from the National Quality Registries on CRC, liver and thoracic surgery and the National Patient Registry. Among 60,734 patients diagnosed with CRC, 1923 (3.2%) had synchronous liver and lung metastases, of which 44 patients had complete metastasectomy. Surgery of liver and lung metastases yielded a 5-year OS of 74% (95% CI 57-85%) compared to 29% (95% CI 19-40%) if liver metastases were resected but not the lung metastases and 2.6% (95% CI 1.5-4%) if non-resected, p < 0.001. Complete resection rates ranged from 0.7% to 3.8% between the six healthcare regions of Sweden, p = 0.007. Synchronous liver and lung CRC metastases are rare, and a minority undergo the resection of both metastatic sites but with excellent survival. The reasons for differences in regional treatment approaches and the potential of increased resection rates should be studied further.

The present invention makes this possible and comprises a method wherein at first an advancing pulp suspension is screened or vortex cleaned, (3) leading to the formation of an accept pulp fraction (4) and a reject pulp fraction (5) and that the reject pulp fraction is cleaned and divided, and that accepted material (pulp fibres and valuable fine material) is brought to further treatment and/or use. The invention is characterized in that the cleaning and division of the reject pulp suspension is carried out so that substantially all ray cells are present in the apex fraction of a fractionating cyclone (6) (if that is the kind of device used) and in that said fraction as such constitutes a very limited material stream, or in that a very limited material stream, predominantly containing ray cells is selected from the apex fraction, and in that this very limited material stream is brought to a disposal stage.

    • A research program funded by the Swedish Energy Agency, the Swedish Knowledge Foundation and the Research Council of Norway was initiated by the companies; Holmen, Norske Skog, SCA and Stora Enso during 2008 with the objective to demonstrate techniques to reduce electrical energy demand by 50% for production of TMP and CTMP within a 10-year period starting 2011, while retaining similar final product characteristics of printing paper and paperboard. A gap analysis was performed to summarize different options to reach the goal at similar final product quality properties. The gap analysis indicated that some different approaches based on earlier research work both in pilot and demonstration scale together with improved process and product quality control should make it possible to reach the very tough goal. The very thorough benchmarking study was performed during 2012 including 16 TMP and CTMP-lines in Norway and Sweden showed the best production lines had the following electrical energy demands to certain final product properties; 900 kWh/t for CTMP optimized for paperboard, 1800 kWh/t for TMP optimized for newsprint and 2800 kWh/t for TMP optimized for SC-paper. The best results reached in industrial demonstration tests until 2015 showed potential to reduce the energy demand from 900 to 700 kWh/t for paperboard CTMP, from 1800 to 1400 kWh/t for newsprint TMP and from 2800 to 2400 for SC-paper TMP. In addition the results from other research within the program in the form of pilot and demonstration trials indicates that it could be possible to reach the goal of 50% reduced energy demand within a 10-year period provided that the industry and the research funds will continue to finance a continuation of the research program.

There are several suggestions torelatively large modifications in design of refiner platepatterns, chip pre-treatment and chip feed strategies toexisting refiners, but these suggestions are difficult toimplement, as the knowledge of the mechanismsprevalent in the refiner gap is insufficient.To solve this problem FSCN and CIT have startedresearch project, “Filling the Gap”, together with thecompanies; Dametric, Holmen, Metso, Norske Skog,Pöyry, SCA and Stora-Enso co-financed by theSwedish authorities; Vinnova and Swedish EnergyAgency. The objective of this research project is toshow how to improve the electric energy efficiency ofchip refining by means of utilizing the fundamentalknowledge of wood material properties relevant forchip refining i.e. refining hypotheses in combinationwith output variable knowledge from new andimproved refining zone measurement methods as;exact gap distance, temperature-, force- and fibrematerial radial distributions combined with thetraditional out/in-put variables used. These data will beutilized in two ways:1) Optimize refining conditions in a static way, i.e. ofconditions to maximize energy efficiency to reach thefunctional fibre properties aimed for.2) Maximize process stability and minimize qualityvariations to these functional fibre properties.This paper reports a general overview from thepreliminary results of these evaluations.
