    • Botulism is a severe neuroparalytic disease that affects humans, all warm-blooded animals, and some fishes. The disease is caused by exposure to toxins produced by Clostridium botulinum and other botulinum toxin-producing clostridia. Botulism in animals represents a severe environmental and economic concern because of its high mortality rate. Moreover, meat or other products from affected animals entering the food chain may result in a public health problem. To this end, early diagnosis is crucial to define and apply appropriate veterinary public health measures. Clinical diagnosis is based on clinical findings eliminating other causes of neuromuscular disorders and on the absence of internal lesions observed during postmortem examination. Since clinical signs alone are often insufficient to make a definitive diagnosis, laboratory confirmation is required. Botulinum antitoxin administration and supportive therapies are used to treat sick animals. Once the diagnosis has been made, euthanasia is frequently advisable. Vaccine administration is subject to health authorities' permission, and it is restricted to a small number of animal species. Several measures can be adopted to prevent or minimize outbreaks. In this article we outline all phases of management of animal botulism outbreaks occurring in wet wild birds, poultry, cattle, horses, and fur farm animals. © 2013, Mary Ann Liebert, Inc.

  • Gas analysis in gasification of biomass and waste : Guideline report: Document 1

    • Gasification is generally acknowledged as one of the technologies that will enable the large-scale production of biofuels and chemicals from biomass and waste. One of the main technical challenges associated to the deployment of biomass gasification as a commercial technology is the cleaning and upgrading of the product gas. The contaminants of product gas from biomass/waste gasification include dust, tars, alkali metals, BTX, sulphur-, nitrogen- and chlorine compounds, and heavy metals. Proper measurement of the components and contaminants of the product gas is essential for the monitoring of gasification-based plants (efficiency, product quality, by-products), as well as for the proper design of the downstream gas cleaning train (for example, scrubbers, sorbents, etc.). In practice, a trade-off between reliability, accuracy and cost has to be reached when selecting the proper analysis technique for a specific application. The deployment and implementation of inexpensive yet accurate gas analysis techniques to monitor the fate of gas contaminants might play an important role in the commercialization of biomass and waste gasification processes.This special report commissioned by the IEA Bioenergy Task 33 group compiles a representative part of the extensive work developed in the last years by relevant actors in the field of gas analysis applied to(biomass and waste) gasification. The approach of this report has been based on the creation of a team of contributing partners who have supplied material to the report. This networking approach has been complemented with a literature review. The report is composed of a set of 2 documents. Document 1(the present report) describes the available analysis techniques (both commercial and underdevelopment) for the measurement of different compounds of interest present in gasification gas. The objective is to help the reader to properly select the analysis technique most suitable to the target compounds and the intended application. Document 1 also describes some examples of application of gas analysis at commercial-, pilot- and research gasification plants, as well as examples of recent and current joint research activities in the field. The information contained in Document 1 is complemented with a book of factsheets on gas analysis techniques in Document 2, and a collection of video blogs which illustrate some of the analysis techniques described in Documents 1 and 2.This guideline report would like to become a platform for the reinforcement of the network of partners working on the development and application of gas analysis, thus fostering collaboration and exchange of knowledge. As such, this report should become a living document which incorporates in future coming progress and developments in the field.

    • This paper presents work in progress for development of an on-line method based on PID (Photo Ionization Detector) for quantitative measurement of tar from biomass gasification. To calibrate the method the PID signals are compared to quantitative data of individual tar compounds obtained by an established reference method. The measured response factors for the model tar compounds demonstrated very good linearity. The PID approach was tested on-line with real producer gases from an atmospheric fluidized bed gasifier operated at 800-900 °C. The results suggest that PID can be used for continuous on-line tar measurement of product gases from biomass gasification.

  • Cardiovascular effects of dobutamine stress testing in women with suspected coronary artery disease

    • Objective. To investigate the central and peripheral haemodynamic response to dobutamine stress testing in women with suspected ischaemic heart disease and to seek an explanation for the hypotension phenomenon. Design. 18 women aged 54-73 years were investigated with Doppler echocardiography and venous occlusion plethysmography during intravenous infusion of dobutamine 5-10 d after an episode of unstable angina. Results. An average peak dose of 33 (SD 9.7) μg/kg/min was given. Heart rate and cardiac output increased by 49% and 59%, respectively (P < 0.001). Total and leg peripheral vascular resistance decreased by 44% and 26%, respectively (P < 0.001). Four patients developed hypotension (decrease in systolic blood pressure > 10 mm Hg), one of whom had a paradoxical bradycardia and two a low increase in cardiac output. Patients with hypotension had a more pronounced decrease in total peripheral vascular resistance but a similar change in leg peripheral vascular resistance compared with patients without hypotension. Conclusions. Dobutamine infusion leads to marked peripheral vasodilatation and an increase in cardiac output. Some patients experience hypotension during the test for reasons which include paradoxical vasovagal reactions and diminished capacity for adequate increase in cardiac output. There is also a disparity between the pattern of total and leg peripheral vascular resistance in patients with hypotension which might reflect a baroreceptor mediated compensatory increase in vasoconstrictor tone of muscle vessels not matched in other vascular territories.

  • Exercise echocardiography : a methodological study comparing peak‐exercise and post‐exercise image information

    • It is unclear whether echocardiography at peak bicycle exercise adds information to registrations obtained recumbent immediately after the test and what factors influence image quality. Therefore, exercise echocardiography was performed consecutively and prospectively in 66 men, unselected with regard to echocardiography, one month after an episode of unstable coronary artery disease. Of 594 segments (9 × 66), 569 (96%) were adequately visualized recumbent at rest. The corresponding figures recumbent directly after exercise, seated before exercise, and seated at peak exercise were 544 (92%), 474 (80%), and 428 (72%), respectively. In the majority of our patients, acceptable images at peak exercise were obtained for the septal region, while for the anterior, lateral, and inferior segments the success rate varied from 50 to 70%. Recumbent after exercise, the success rate was acceptable for most segments, possibly with the exception of the apical and lateral segments. Fifty‐five patients developed new wall motion abnormalities or worsening of wall motion in connection with exercise. Echocardiography at peak exercise provided more information than afterwards in patients with images of good quality. However, in patients with inferior image quality, the registrations obtained recumbent after the test revealed wall motion abnormalities which were not obtained seated at peak exercise. Patients with worse image quality had significantly higher respiratory rate and weight, and rated a higher degree of dyspnoea at peak exercise than those with good quality. We conclude that in middle aged men with coronary artery disease, image acquisition at peak bicycle exercise and immediately after exercise are of complementary value.

    • To compare modern, digital exercise echocardiography and thallium 201 single-photon emission computed tomography (SPECT) in patients with unstable coronary artery disease, 65 men unselected with regard to echocardiography were prospectively investigated 1 month after an episode of unstable angina or non-Q-wave myocardial infarction. Exercise echocardiography and 201Tl SPECT were performed on consecutive days in connection with a standard symptom-limited upright bicycle test and analyzed in a 9-segment model. Coronary angiography was performed in all but 1 patient and 60 patients had significant coronary lesions. Wall motion abnormalities were seen in 53 patients (81%) at rest and perfusion defects in 57 patients (88%) at the redistribution images. New or worsening of wall motion abnormalities were seen in 55 patients, either seated at peak exercise or recumbent after exercise, and 43 patients had reversible or partially reversible 201Tl scintigraphic defects (P = .02). The segmental agreement between wall motion abnormalities and scintigraphic defects was low (58%). The additional value of exercise echocardiography and 201Tl SPECT to exercise test was greatest in patients with one-vessel disease. Thus, 1 month after an episode of unstable coronary artery disease in men, there is a high incidence of significant coronary stenoses as well as signs of ischemia shown both by wall motion abnormalities during exercise echocardiography and by postexercise studies with 201Tl SPECT. Exercise echocardiography gives a higher diagnostic yield regarding occurrence of reversible ischemia.

  • Time-resolved alkali release during steam gasification of char in a fixed bed reactor

    • In this study time-resolved char conversion and alkali release under steam gasification conditions were investigated using a fixed bed reactor. The behaviour of an industrial char and chars produced from straw and furniture waste was investigated. For woody chars, an increase in gasification reactivity is observed together with a notable alkali release as the gasification approaches completion (degree of conversion > 0.8). In contrast, straw char exhibited a decrease in conversion rate and alkali release throughout the gasification process, attributed to the formation of catalytically inactive potassium silicates inhibiting the catalytic role of alkali. Aerosol particles in the 0.01–22 µm size range are emitted during the char conversion. A fraction is formed by nucleation of alkali compounds and other condensable gases. A wide particle distribution that extends over the whole size range is also observed, and the particles are likely to consist of solid char fragments. The study concludes on the importance of alkali release, illustrating the difference in alkali release pattern for high and low ash char.

    • Objective: To evaluate diagnostic instruments for assessment of nutritional status in patients with rheumatoid arthritis (RA) in relation to objective body composition data.Subjects and methods: Study subjects include 60 in-ward patients (83% women, median age 65 years). Anthropometric measures and the nutritional tools Mini Nutritional Assessment (MNA), Subjective Global Assessment (SGA), Malnutrition Universal Screening Tool (MUST) and Nutritional Risk Screening tool 2002 (NRS-2002). Body composition was determined by dual-energy X-ray absorptiometry and fat-free mass index (FFMI; kg/m(2)) and fat mass index (FMI; kg/m(2)) were calculated.Results: Mean body mass index (BMI) for RA women and men were 24.4 and 26.9 kg/m(2), respectively. Twelve per cent of the women and none of the few men had BMI<18.5 kg/m(2), that is, the cutoff value for malnutrition. FFMI indicated 52% of the women and 30% of the men to be malnourished. The sensitivity and specificity for BMI to detect malnutrition according to FFMI were 27 and 100%, whereas for arm muscle circumference the sensitivity was 36% and the specificity 89% and for triceps skin fold 43 and 93%, respectively. For MNA, sensitivity was 85% and specificity 39% and for SGA 46 and 82%. Both MUST and NRS-2002 had sensitivity of 45% and specificity of 19%.Conclusion: A large proportion of in-ward RA patients had reduced FFMI. Concurrent elevation of fat mass made BMI a non-reliable tool to detect malnutrition. Of the tested clinical evaluation tools, MNA might be used as a screening instrument, but because of its low specificity it should be followed by body composition determination.

    • Septal systolic strain measurements showed reduced longitudinal function but its localized nature failed to demonstrate radial disturbances in patients with pathologically thickened septum. No difference was found in systolic strain between patients according to the etiology of septal thickness. This limitation might be either technical or is explained by the maintained radial function in all patient groups.

    • OBJECTIVES: To examine the impact of inflammation, insulin-like growth factor (IGF-1) and its regulating binding protein (IGFBP-1) on lean body mass (LBM) in patients with rheumatoid arthritis (RA). METHODS: In 60 inpatients (50 women), inflammatory activity was measured by Disease Activity Score 28 (DAS28), C-reactive protein (CRP), and interleukin (IL)-6, and physical disability by the Health Assessment Questionnaire (HAQ). LBM was assessed by dual-energy X-ray absorptiometry (DXA) and fat free mass index (FFMI; kg/m(2)) and fat mass index (FMI; kg/m(2)) were calculated. RESULTS: Median age was 65 years and disease duration 13 years. Fifty per cent of the patients had FFMI below the 10th percentile of a reference population and 45% had FMI above the 90th percentile, corresponding to the condition known as rheumatoid cachexia (loss of muscle mass in the presence of stable or increased FM). DAS28, CRP, and IL-6 correlated negatively with LBM (p = 0.001, 0.001, and 0.018, respectively), as did HAQ (p = 0.001). Mean (confidence interval) IGF-1 was in the normal range, at 130 (116-143) microg/L. IGFBP-1 levels were elevated in patients (median 58 microg/L in women and 59 microg/L in men) compared with a normal population (33 microg/L in women and 24 microg/L in men). The ratio IGF-1/IGFBP-1, which reflects bioavailable IGF-1, was low (2.0 microg/L) and was positively correlated with LBM (p = 0.015). In multiple regression analysis, 42% of the LBM variance was explained by IGF-1/IGFBP-1, HAQ score, and DAS28. CONCLUSION: A large proportion of RA inpatients, mainly women, had rheumatoid cachexia. The muscle wasting was explained by inflammatory activity and physical disability as well as low bioavailable IGF-1.

    • Alkali-associated problems are key issues for the efficient use of straw that is available as a major renewable energy resource worldwide. The effects of six bed materials commonly used in fluidized bed reactors on straw pyrolysis and char gasification were evaluated using online monitoring of alkali release and thermogravimetric analysis. Scanning electron microscopy with energy dispersive spectroscopy was used to determine the elemental composition of the char surface. In the straw pyrolysis stage, alkali release is reduced by the addition of dolomite and silica due to alkali adsorption on the bed materials, and enhanced by the addition of alumina because of its high sodium content. In the char gasification stage, silica, sea sand, olivine, and ilmenite reduce the char reactivity and alkali release, which is attributed to transfer of Si and Ti from the bed materials to the char and reaction with alkali to form stable and catalytically inactive compounds. Alumina also reduces the char conversion rate by transfer of Al to the char and formation of K-Al-Si and Ca-Al-Si compounds, while alkali release from the straw and alumina blend remains high due to the high Na content in alumina. Dolomite initially appears to increase the char gasification reactivity, but the results are affected by conversion of volatile matter that deposited on the dolomite in the straw pyrolysis stage. Dolomite also significantly increases the alkali release, which is attributed to Ca reactions with aluminosilicate compounds that allow potassium to remain in volatile form. Fresh bed materials are concluded to have significant effects on straw conversion depending on their chemical composition, and the results can contribute to the understanding required for efficient use of straw in commercial applications of biomass thermochemical conversion.

  • Effects of used bed materials on char gasification : Investigating the role of element migration using online alkali measurements

    • Online alkali measurements using surface ionization are employed to study alkali release during heating of used industrial fluidized bed materials and gasification of biomass-based char and bed material mixtures. The alkali release from the bed materials starts at 820 °C and increases with temperature, the time a bed material has experienced in an industrial process, and in the presence of CO2. Online alkali measurement during heating of char mixed with used bed material shows significant alkali uptake by the char. Complementary SEM-EDS studies confirm the alkali results and indicate that other important inorganic elements including Si, Mg, and Ca also migrate from the bed material to the char. The migration of elements initially enhances alkali release and char reactivity, but significantly reduces both during the final stage of the gasification. The observed effects on char gasification become more pronounced with increasing amount of bed material and increasing time the material experienced in an industrial process. The ash-layer on the used bed material is concluded to play an important role as a carrier of alkali and other active components. The char and bed material systems are closely connected under operational conditions, and their material exchange has important implications for the thermal conversion.

    • Bed materials provide efficient heat transfer and catalytic function in the thermochemical conversion of biomass, but their interactions with the fuel remain incompletely understood. In this study, the effects of bed materials on alkali release and fuel conversion during wood pyrolysis and CO2 gasification are investigated by online alkali detection combined with thermogravimetric analysis. The investigated bed materials include silica, sea sand, alumina and the natural ores olivine, ilmenite and dolomite. Only dolomite has a significant effect on fuel mass loss and alkali release during wood pyrolysis, while all bed materials influence char reactivity and alkali release during gasification. Sea sand, alumina and dolomite enhance the char gasification during the whole or most of the gasification process, which is related to alkali migration from the bed materials. All bed materials affect char reactivity and alkali release when the conversion approaches completion, and small amounts of some bed materials reduce the alkali release by an order of magnitude. The findings can be understood based on the chemical composition of the different materials. Silicon-rich materials reduce the levels of catalytically active alkali by formation of stable alkali silicates, and a similar explanation applies for ilmenite that captures alkali efficiently. Magnesium and calcium in contrast promote alkali release through their influence on alkali silicate chemistry. Analysis of char surfaces using scanning electron microscopy with energy dispersive spectroscopy indicates that low amounts of several elements are transferred from the bed material to the char where they may be directly involved in the char conversion process. The transferred elements are specific for each bed material and relates to their chemical composition. Mechanisms for material exchange between bed material and char are discussed.

    • Fuel blends may be used to meet several operational needs in thermal conversion of biomass waste, including optimization of ash properties and fuel conversion efficiency. In this study, online alkali measurements using surface ionization are employed to study synergistic effects produced by inorganic elements during co-pyrolysis/gasification of wood and straw waste. Synergistic effects on the fuel conversion behavior are not observed during co-pyrolysis, while alkali migration from straw to wood is clearly observed above 600 °C by online alkali monitoring. In contrast, synergistic effects on char conversion and alkali release are substantial during co-gasification. Positive effects on char reactivity during most of the gasification process are attributed to alkali migration from the straw to the wood char, and the most pronounced effect occurs at a gasification temperature of 900 °C and a straw content of 25%. Negative effects on char reactivity are observed at the final gasification stage, which is associated with a significantly reduced alkali release from fuel blends compared to pure wood char. The effect is attributed to the migration of silicon, phosphorus, and aluminum to the wood char, as revealed by scanning electron microscopy with energy dispersive spectroscopy, where the elements react with alkali to form catalytically inactive compounds. The mixing of biofuels is concluded to result in substantial effects on the fuel conversion efficiency, which should be taken into consideration in thermochemical conversion of biomass.

    • Potassium and sodium compounds play both positive and negative roles during biomass gasification, but the detailed behavior of alkali metal compounds remain incompletely understood. In this study, alkali release during CO2 gasification of biochar is characterized online with a surface ionization method in combination with thermogravimetric analysis of the char samples undergoing gasification. For wood chars, the alkali release rate follows a slowly decreasing trend as the char conversion proceeds, but increases by up to two orders of magnitude when the conversion approaches completion. In contrast, the alkali release from straw char is 40-50 times higher than observed for wood char and decreases continuously during the whole gasification process. A high temperature and a high CO2 concentration enhance both alkali release and char reactivity. The char preparation method also influences the alkali release from pine char, while the char reactivity is less affected. Alkali release and char reactivity are linked, but other factors including mineral content, surface area and char structure may play important roles for the observed reactivity. The results provide a basis for understanding of alkali behavior during gasification and may help optimize catalytic effects and reduce detrimental issues in biomass gasification.

    • BackgroundElevated body weight in adolescence is strongly associated with early cardiovascular disease, but whether this association is traceable to weight in early adulthood, or to weight gain with subsequent high adult weight is not known. Using data from the Swedish CArdioPulmonary bioImage Study (SCAPIS), we investigated the association between weight at age 20, weight gain to midlife and coronary artery calcium score (CACS) at midlife.MethodsIn the first 15,810 participants in SCAPIS (mean age 58 years, 52% women), data on CACS at midlife, self-reported body weight at age 20 and weight at examination in SCAPIS were recorded.ResultsCACS in midlife was significantly higher with increasing weight at age 20 (p<0.001 for both sexes), and then increased with weight gain until midlife at all levels of body weight at age 20 after adjusting for age, height, smoking, alcohol intake, education level, exercise levels and LDL cholesterol. However, the association with weight gain was only significant in men (p = 0.047), not in women (p=0.474). No significant interaction was seen between weight at age 20 and midlife weight with CACS. The effect of weight at age 20 on CACS was significantly more marked in men than in women, as was the effect of weight gain (p<0.001 for both interactions).ConclusionWeight at age 20 and weight gain to midlife were both related to CACS, but much more markedly so in men than in women, indicating a generally larger effect of both early adult weight and further weight gain until midlife on CACS in men, compared to women.

    • Sexually transmitted disease (STD) case management was evaluated through observation and interviews at 2 urban and 4 rural health centres and 2 district hospital STD clinics in one urban and 2 rural districts in Central Province, Zambia. The analysis was limited to 59 patients (42 men and 17 women) who paid first visits for their disease and were managed by a clinical officer. The evaluation suffered from the lack of a standard for case management. Results showed that the patients engaged in risky sexual behaviour without being aware of the risks. At the health institutions, few patients were informed about condoms, the risk of HIV, and abstinence from sex during treatment and few were asked to notify their partners. Clinical officers with special STD training performed better than others but sill informed only one-fifth of the patients. Few clinical officers managed patients according to the syndromic approach recommended by the STD control programme.

    • BACKGROUND: In search of a proposed viral aetiology of childhood acute lymphoblastic leukaemia (ALL), the common species C adenoviruses were analysed in Guthrie cards. METHODS: Guthrie cards from 243 children who later developed ALL and from 486 matched controls were collected and analysed by nested polymerase chain reaction for the presence of adenovirus DNA. RESULTS: Adenovirus DNA was reliably detected from only two subjects, both of whom developed ALL. CONCLUSION: Adenovirus DNA is detected in Guthrie card samples at too low a frequency to reveal an association between adenovirus and the development of leukaemia.

    • The ability to measure and quantify myocardial motion and deformation provides a useful tool to assist in the diagnosis, prognosis and management of heart disease. The recent development of magnetic resonance imaging methods, such as harmonic phase and displacement encoding with stimulated echoes (DENSE), make detailed non-invasive 3D transmural kinematic analyses of human myocardium possible in the clinic and for research purposes. As data acquisition technologies improve, quantification methods for cardiac kinematics need to be adapted and validated on the new types of data. In the present paper, a previously presented polynomial method for cardiac strain quantification is extended to quantify 3D strains from DENSE magnetic resonance imaging data. The method yields accurate results when validated against an analytical standard, and is applied to in vivo data from a healthy  human heart. The polynomial field is capable of resolving the measured material positions from the in vivo data, and the obtained in vivo strains agree

A robust analysis method is required, however.MethodsWe propose to estimate strain using a polynomial function which produces local models of the displacement field obtained with DENSE. Given a specific polynomial order, the model is obtained as the least squares fit of the acquired displacement field. These local models are subsequently used to produce estimates of the full strain tensor.ResultsThe proposed method is evaluated on a numerical phantom as well as in vivo on a healthy human heart. The evaluation showed that the proposed method produced accurate results and showed low sensitivity to noise in the numerical phantom. The method was also demonstrated in vivo by assessment of the full strain tensor and to resolve transmural strain variations.ConclusionsStrain estimation within a 3D myocardial volume based on polynomial functions yields accurate and robust results when validated on an analytical model. The polynomial field is capable of resolving the measured material positions from the in vivo data, and the obtained in vivo strains values agree with previously reported myocardial strains in normal human hearts.
