    • Since 2008 advanced home healthcare agencies (ASiH) in a larger Swedish county council has underwent a transformation, to become part of a coming concept: networked healthcare (NVS). NVS means that intermediate multi-organizational healthcare (IMV) will be produced often in the home, and from 2013 to an increasing number of patients in different age groups with different diagnoses and medical conditions - in large variability of needs. At the same time IMV has proved to be not simply practical to implement in a resource-efficient and patientsafe way. Based on theories from Command and Control Science the safetyproblem that arise in connection with IMV is a sign of the less known increasing need of the direction and coordination support that IMV requires.With a casestudy based research approach with interactive elements, different qualitative methods has been used in two phases between 2008 - 2013. The first phase is characterized by a phenomenological approach, while the second phase has a critical hermeneutic approach. Research methods includes fieldvisits with informal discussions, in-depth interviews, validation with respondents and two different methodologies for textanalysis.The main result shows that practical aggravating circumstances for safe care consists of lesser known and from 2013 increasing problems with direction and coordination, through expanded advanced IMV in the home as a part of NVS concept. This also as a result of inadequate and inappropriate direction and coordination support for IMV.The thesis concludes that the NVS represents a resource intensive health care concept, which requires a new view on the management issue and a network-related methodology for direction and coordination. This is to promote ethical, equitable, patientsafe and dignified advanced IMV so an optimized use of resources can be implemented, through shared responsibility and coordination in patientuniquely designed networkconstellations as a given work model.

    • A novel data mining method was developed to gauge the experience of the drug Sitagliptin (trade name Januvia) by patients with diabetes mellitus type 2. To this goal, we devised a two-step analysis framework. Initial exploratory analysis using self-organizing maps was performed to determine structures based on user opinions among the forum posts. The results were a compilation of user's clusters and their correlated (positive or negative) opinion of the drug. Subsequent modeling using network analysis methods was used to determine influential users among the forum members. These findings can open new avenues of research into rapid data collection, feedback, and analysis that can enable improved outcomes and solutions for public health and important feedback for the manufacturer.

  • A Novel Method to Intelligently Mine Social Media to Assess Consumer Sentiment of Pharmaceutical Drugs

    • This thesis focuses on the development of novel data mining techniques that convert user interactions in social media networks into readable data that would benefit users, companies, and governments. The readable data can either warn of dangerous side effects of pharmaceutical drugs or improve intervention strategies. A weighted model enabled us to represent user activity in the network, that allowed us to reflect user sentiment of a pharmaceutical drug and/or service. The result is an accurate representation of user sentiment. This approach, when modified for specific diseases, drugs, and services, can enable rapid user feedback that can be converted into rapid responses from consumers to industry and government to withdraw possibly dangerous drugs and services from the market or improve said drugs and services.Our approach monitors social media networks in real-time, enabling government and industry to rapidly respond to consumer sentiment of pharmaceutical drugs and services.

    • Depression is a global health concern. Social networks allow the affected population to share their experiences. These experiences, when mined, extracted, and analyzed, can be converted into either warnings to recall drugs (dangerous side effects), or service improvement (interventions, treatment options) based on observations derived from user behavior in depression-related social networks. Our aim was to develop a weighted network model to represent user activity on social health networks. This enabled us to accurately represent user interactions by relying on the data's semantic content. Our three-step method uses the weighted network model to represent user's activity, and network clustering and module analysis to characterize user interactions and extract further knowledge from user's posts. The network's topological properties reflect user activity such as posts' general topic as well as timing, while weighted edges reflect the posts semantic content and similarities among posts. The result, a synthesis from word data frequency, statistical analysis of module content, and the modeled health network's properties, has allowed us to gain insight into consumer sentiment of antidepressants. This approach will allow all parties to participate in improving future health solutions of patients suffering from depression.

    • Intelligently extracting knowledge from social media has recently attracted great interest from the Biomedical and Health Informatics community to simultaneously improve healthcare outcomes and reduce costs using consumer-generated opinion. We propose a two-step analysis framework that focuses on positive and negative sentiment, as well as the side effects of treatment, in users' forum posts, and identifies user communities (modules) and influential users for the purpose of ascertaining user opinion of cancer treatment. We used a self-organizing map to analyze word frequency data derived from users' forum posts. We then introduced a novel network-based approach for modeling users' forum interactions and employed a network partitioning method based on optimizing a stability quality measure. This allowed us to determine consumer opinion and identify influential users within the retrieved modules using information derived from both word-frequency data and network-based properties. Our approach can expand research into intelligently mining social media data for consumer opinion of various treatments to provide rapid, up-to-date information for the pharmaceutical industry, hospitals, and medical staff, on the effectiveness (or ineffectiveness) of future treatments.

    • Samverkan kan leda till innovation, konkurrenskraftiga företag, förstklassig forskning samt välfungerande myndigheter och institutioner. I den politiska debatten idag finns en förväntan att Sverige ska upprätthålla sin konkurrenskraft och bemöta samhällets utmaningar genom innovation och att vägen till innovation går via samverkan. Avhandlingen bygger på en studie av ett samverkansprojekt vars syfte var att skapa innovation för att minska antalet vårdrelaterade infektioner i Sverige. Projektet som studerats ses som en transdisciplinär ansats med aktörer som representerade akademi, industri samt hälso- och sjukvård.Syftet med avhandlingen är att vidareutveckla kunskapen om interorganisatorisk samverkan för innovation. Detta görs genom ett tredelat bidrag, till teoribildningen kring samverkan för innovation som börjat växa fram, till den samverkande praktiken inom både privat och offentlig sektor samt till politiker och beslutsfattare som styr fördelning av statliga anslag till forskning och innovation.Fallstudien som ligger till grund för avhandlingen är baserad på en etnografiskt inspirerad studie. Empiriskt material samlades in och skapades tillsammans med aktörerna i projektet under drygt två års tid genom intervjuer och deltagande observation.Studien visar att interorganisatorisk samverkan består av flera dimensioner och kan förstås på flera nivåer. Interorganisatorisk samverkan innebär inte bara att det är olika organisationer som ska göra en gemensam ansträngning. Organisationerna består av olika människor med olika discipliner och professioner vilka bygger på olika utgångspunkter och sätt att se på världen. Samverkan kan ses som ett sätt att fylla mellanrummen mellan organisationer istället för att bygga broar över gränser. I de organisatoriska mellanrummen kan aktörer från olika organisationer, med olika discipliner och professioner mötas utan institutionaliserade roller, i en receptiv kontext där innovation kan skapas.

    • The Swedish Agency for Health Technology Assessment and Assesment of Social Services (SBU) is an independent national authority, tasked by the government with assessing methods used in health, medical and dental services and social service interventions from a broad perspective, covering medical, economic, ethical and social aspects. The language in SBU's reports are adjusted to a wide audience. SBU's Board of Directors has approved the conclusions in this report. The systematic review showed the following graded results:There is limited scientific evidence that the triad (Three components of a whole. The triad associated with SBS usually comprises subdural haematoma, retinal haemorrhages and encephalopathy.) and therefore, its components can be associated with traumatic shaking (low-quality evidence).There is insufficient scientific evidence on which to assess the diagnostic accuracy of the triad in identifying traumatic shaking (very low-quality evidence).Limited scientific evidence (low-quality evidence) represents a combined assessment of studies of high or moderate quality which disclose factors that markedly weaken the evidence. It is important to note that limited scientific evidence for the reliability of a method or an effect does not imply complete lack of scientific support. Insufficient scientific evidence (very low-quality evidence) represents either a lack of studies or situations when available studies are of low quality or show contradictory results. Evaluation of the evidence was not based on formal grading of the evidence according to GRADE but on an evaluation of the total scientific basis.

    • Nailfold capillaroscopy is one of the various noninvasive bioengineering methods used to investigate skin microcirculation. It is an effective examination for assessing microvascular changes in the peripheral circulation; hence it has a significant role for the diagnosis of Systemic sclerosis with the classic changes of giant capillaries as well as the decline in capillary density with capillary dropout. The decline in capillary density is one of microangiopathic features existing in connective tissue disease. It is detectable with nailfold capillaroscopy. This parameter is assessed by applying quantitative measurement. In this article, we reviewed a common method for calculating the capillary density and the relation between the number of capillaries as well as the existence of digital ulcers, pulmonary arterial hypertension, autoantibodies, scleroderma patterns and different scoring system.

    • Video nailfold capillaroscopy (NFC), considered as an extension of the widefield technique, allows a more accurate measuring andstoring of capillary data and a better defining, analyzing, and quantifying of capillary abnormalities. Capillaroscopic study is oftenperformed on the patients suspected of having microcirculation problems such as Raynaud’s phenomenon as the main indicationfor nailfold capillaroscopy. Capillaroscopic findings based on microcirculation studies can provide useful information in the fieldsof pathophysiology, differential diagnosis, and monitoring therapy. Nailfold capillaroscopy provides a vital assessment in clinicalpractices and research; for example, its reputation in the early diagnosis of systemic sclerosis is well established and it is also usedas a classification criterion in this regard. This review focuses on the manner of performing video nailfold capillaroscopy and on acommon approach for measuring capillary dimensions in fingers and toes.

    • Objectives: The aim of this study was to determine capillaroscopic changes in patients with systemic lupus erythematosus (SLE) and their predictors. Methods: Fifty-nine SLE patients and 31 controls were enrolled in a cross-sectional study. Nailfold capillaroscopy examinations were performed, and qualitative semi-quantitative and quantitative parameters were evaluated in all fingers. Demographic features and lupus characteristics, such as renal involvement, medications, laboratory data, disease activity (SLEDAI) and damage, were recorded. The predictors of capillaroscopic abnormalities were obtained by backward stepwise regression analysis. Results: Capillary numbers of right hands were significantly lower in patients than in controls [8.74 (1.66) vs. 9.63 (1.80), P = 0.0001]. Capillaries were wider in patients than in controls in right [56.32 pm (16.76) vs. 50.43 mu m (10.16), P = 0.002] and left hands [54.40 (15.02) vs. 49.71 (9.77), P = 0.005]. Capillaries were shorter in SLE patients than in controls. Multivariate analysis revealed that the main associative factors of microvascular abnormalities were gender, drinking tea and hydroxychloroquine use for giant capillaries, SLEDAI and low C3 for avascularity and age, lupus nephritis and corticosteroid use for ramification. Conclusion: Most nailfold capillaroscopic abnormalities were more common in SLE patients than in controls. Hydroxychloroquine, corticosteroids, SLEDAI, low complement and lupus nephritis may be the major prognostic factors for microvascular changes in SLE patients.

    • In this study, the Homo-Separation of Variables method is introduced to find the exact solution of somecoupled nonlinear partial differential equations. This analytical method is a new combination of two powerfuland popular methods; namely the homotopy perturbation method (HPM) and the separation of variables method.The exact solution is constructed by choosing an appropriate initial approximation in addition to retaining onlythe first term of the series obtained by HPM. The proposed method is capable of reducing the computational loadconsiderably when compared to other classical method.

    • Capillaries play a crucial role in the microcirculatory system by exchanging metabolic substrates and waste products between blood and various tissues. The behavior of capillaries is affiliated with the number of capillaries per unit volume of tissue. Among the various noninvasive techniques available for analyzing skin microcirculation, nailfold capillaroscopyis considered to be a simple and easy-to-perform technique that allows a direct in-vivo visualization of the capillary network.Capillary density is one of the most important parameters in the studies involving capillaroscopy images. Capillary density in most of studies is defined as the number of capillaries in a one-millimeter span of the distal row in each finger or toe. This definition is silent about counting or excluding the number of the capillary with different shapes. However, there is no single standard for counting the number of capillaries in a span of one millimeter. In this paper, a novel method is proposed for determining the nailfold capillary density. This method is a modified combination of two existing techniques: the direct observation and the 90◦ method.Compared to the two existing approaches, the proposed method is more straightforward and easy to use for cases in which the capillaries have different shapes and sizes. Through different examples, we have shown how this method can be used to select the apex point of the capillary and subsequently count the number of capillaries with several papillae.

    • Nailfold capillaroscopy is a practical method for identifying and obtaining morphological changes in capillaries which might reveal relevant information about diseases and health. Capillaroscopy is harmless, and seems simple and repeatable. However, there is lack of established guidelines and instructions for acquisition as well as the interpretation of the obtained images; which might lead to various ambiguities. In addition, assessment and interpretation of the acquired images are very subjective. In an attempt to overcome some of these problems, in this study a new modified technique for assessment of nailfold capillary density is introduced. The new method is named elliptic broken line (EBL) which is an extension of the two previously known methods by defining clear criteria for finding the apex of capillaries in different scenarios by using a fitted elliptic. A graphical user interface (GUI) is developed for pre-processing, manual assessment of capillary apexes and automatic correction of selected apexes based on 90° rule. Intra- and inter-observer reliability of EBL and corrected EBL is evaluated in this study. Four independent observers familiar with capillaroscopy performed the assessment for 200 nailfold videocapillaroscopy images, form healthy subject and systemic lupus erythematosus patients, in two different sessions. The results show elevation from moderate (ICC = 0.691) and good (ICC = 0.753) agreements to good (ICC = 0.750) and good (ICC = 0.801) for intra- and inter-observer reliability after automatic correction of EBL. This clearly shows the potential of this method to improve the reliability and repeatability of assessment which motivates us for further development of automatic tool for EBL method.

    • Background: Nailfold capillaroscopy (NC) is a diagnostic imaging technique that is used to assess the blood capillary network in the nailfold area. NC is routinely used for patients with microcirculation problems, such as systemic sclerosis and other connective tissue diseases.  Experts commonly use subjective evaluation as a reference point in images of nailfold video capillaroscopy, so it is important to reduce the inherent ambiguities in human judgment and diagnosis. Image quality is an important factor that affects measurement error and assessment time of NC images.Objective: In this study, a new image enhancement technique was introduced and evaluated subjectively.Methods: In total, 475 nailfold video capillaroscopy images from 18 healthy subjects and 41 systemic lupus erythematosus patients were used. The images were randomly divided into two sets, one each with 275 and 200. Eight independent observers who were familiar with the capillaroscopy technique participated in this study. The set of 275 images was evaluated by three observers with the forced–choice pairwise comparison method. Elliptic broken line (EBL) was used to count the number of capillaries. The intra- and inter-observer reliability of the original and enhanced images was evaluated on 200 images by five observers.Result: Except for eight images, all observers preferred the enhanced images in the visual quality comparison method. The intra-class correlation coefficient (ICC) of intra- and inter-observer reliability increased from 0.76-0.84 to 0.82-0.89, respectively, when using the enhancement method.Conclusion: By improving the image quality, more capillary details will be visible, and an observer can document more details that may not be visible in the original image and can do so more efficiently.

    • Nailfold capillaroscopy (NC) is an invasive imaging technique that is used to assess the blood capillary network in the nailfold area. NC is routinely used for patients with microcirculation problems, such as systemic sclerosis and other connective tissue diseases. NC is repeatable and appears to be simple and harmless. However, there is a lack of established guidelines [QCE1] and instructions for both the acquisition and interpretation of the images obtained, which may lead to various ambiguities.The general aim of this thesis was to reduce the inherent ambiguities in human judgment and diagnosis by introducing novel techniques for assessing the nailfold capillary density, which is an important parameter in NC. To achieve this goal, a toolkit was developed that includes a method to measure the capillary density, an image enhancement technique and a graphical user interface (GUI).In the first study, a summary of the nailfold videocapillaroscopy procedure was presented, and the common techniques used to evaluate capillaroscopic parameters were reviewed. In the second study, common methods for calculating the capillary density were reviewed, and a survey of the relationship between the number of capillaries as well as the existence of digital ulcers, pulmonary arterial hypertension, autoantibodies, scleroderma patterns and the different scoring systems was performed. In the third study, a novel method was proposed to determine the nailfold capillary density, and this method is referred to as the elliptic broken line (EBL) method. In the fourth study, the EBL method for measuring capillary density was evaluated. In the fifth study, a new image enhancement technique was introduced and evaluated both subjectively and objectively. Finally, clinical applications of the EBL method and enhancement techniques were demonstrated in the sixth study. The result verifies the potential of the proposed EBL method to improve the reliability and repeatability of assessments. Additionally, improving the image quality, i.e., using the proposed enhancement method, can reveal more capillary details for an observer compared to the raw original image, and the results motivate the future development of automatic tools for the EBL method, e.g., automatic segmentation and capillary detection and characterization.Please note that the font changes here.     • Nailfold capillaroscopy (NC) is a highly sensitive, safe, and non-invasive technique to assess involvement rate of microvascularity in dermatomyositis and systemic sclerosis. A large number of studies have focused on NC pattern description, classification, and scoring system validation, but minimal information has been published on the accuracy and precision of the measurement. The objective of this review article is to identify different factors affecting the reliability and validity of the assessment in NC. Several factors can affect the reliability of the examination, e.g., physiological artifacts, the nailfold imaging instrument, human factors, and the assessment rules and standards. It is impossible to avoid all artifacts, e.g., skin transparency, physically injured fingers, and skin pigmentation. However, minimization of the impact of some of these artifacts by considering some protocols before the examination and by using specialized tools, training, guidelines, and software can help to reduce errors in the measurement and assessment of NC images. Establishing guidelines and instructions for automatic characterization and measurement based on machine learning techniques also may reduce ambiguities and the assessment time.

    • A new method proposed and coined by the authors as the homo-separation of variables method is utilized to solve systems oflinear and nonlinear fractional partial differential equations (FPDEs). The new method is a combination of two well-establishedmathematical methods, namely, the homotopy perturbation method (HPM) and the separation of variables method. Whencompared to existing analytical and numerical methods, the method resulting from our approach shows that it is capable ofsimplifying the target problem at hand and reducing the computational load that is required to solve it, considerably.The efficiencyand usefulness of this new general-purposemethod is verified by several examples, where different systems of linear and nonlinearFPDEs are solved.

This is although, it is well known that,access to quality education and scientific knowledge is essential for creating economic rowth and sustainable human development, including poverty alleviation and improvement of human health. In all countries and in the developing countries in particular, there is a need to mploy Information and Communication Technology, ICT to gain global access to learning. ICT an address issues of educational equity, social exclusion and can deliver a more effective and ccessible educational opportunities. It can also reduce cost of reaching and educating many ural students who are deprived of creative education due to lack of qualified teaching force.In Bangladesh, the education curriculum has been modernized to meet an international standard. For various reasons, qualified teachers are not keen to move to rural areas. The same is true in areas of health sectors where qualified medical doctors are not willing to move to rural areas. Therefore, we find enormous potentials for contribution of e-Learning and e-Health in empowering the rural educators as well as health care providers. A pilot project to test the potentials of e-Learning is implemented at a village Nohata in Magura district in Bangladesh (www.nuhat.org). Started in 2006, it has been using ICT tools to communicate, learn, and access international quality educational content. International quality teachers have been conducting teaching using videoconference system. Various relevant e-Learning aids have been developed to meet the local needs and conditions, you tube programmes covering different topics are carefully selected to meet the appropriate requirement of different target groups. The links are downloaded so that the students, the teachers, health workers, patients can follow the links off line, as many times as they want and discuss among themselves. The ICT tools are also being used to improve access to health care; enhance the quality of service delivery; improve the effectiveness of public health and primary care interventions; improve the shortage of health professionals through collaboration and training. ICT tools are offering solutions for emergency medical assistance, long-distance consultation, supervision quality assurance, and education and training for healthcare professionals and providers. The main challenges are to empower the rural people through creating locally relevant content for improving proficiency in English language, Mathematics, Science and Health care application and services considering the sociocultural factors, to achieve health, education and economic development. Through careful selection and creation of relevant e-Learning materials, we intend to develop the rural community using the potential of rural people and adopting participatory approaches for building  knowledge, skills and capacity.Keywords: e-learning in developing countries, rural empowerment, relevant content, e-health, rural health quality improvement

    • Most of the present time outdoor lighting investments seem to be focused on road and automotive traffic facilities or oriented toward safety, security and efficiency. We can identify this in many of the world cities, as a globally spread common practice, where lighting is still highly perceived as a pure matter of quantities’ distribution. Considering the tendency for the Human being to become an Urban-being, the future of human quality of life will, most probably, depend on the fortune of so called sustainable cities. By lighting design, and promoting the return of the city lights to the Human scale, is argued that the Urban Planning approach may not correspond best to the person viewpoint, due to scale. Will be explored, this way, by positioning the perspective at human eye level and not zenith bird-view, the traditional perspective over the city, where from not a soul ever experiences urbanity. The method definition will be based mostly on observations and analysis of the Light and Lighting transitions taking place in cities, experienced by its inhabitants, from daytime to night time.

    • Point-of-care (POC) diagnostics is playing an increasingly important role in public health, environmental monitoring, and food safety analysis. Smartphones, alone or in conjunction with add-on devices, have shown great capability of data collection, analysis, display, and transmission, making them popular in POC diagnostics. In this article, the state-ofthe- art advances in smartphone-based POC diagnostic technologies and their applications in the past few years are outlined, ranging from in vivo tests that use smartphone’s built-in/external sensors to detect biological signals to in vitro tests that involves complicated biochemical reactions. Novel techniques are illustrated by a number of attractive examples, followed by a brief discussion of the smartphone’s role in telemedicine. The challenges and perspectives of smartphonebased POC diagnostics are also provided.
