  • Exome-wide association study to identify rare variants influencing COVID-19 outcomes: Results from the Host Genetics Initiative

    • Host genetics is a key determinant of COVID-19 outcomes. Previously, the COVID-19 Host Genetics Initiative genome-wide association study used common variants to identify multiple loci associated with COVID-19 outcomes. However, variants with the largest impact on COVID-19 outcomes are expected to be rare in the population. Hence, studying rare variants may provide additional insights into disease susceptibility and pathogenesis, thereby informing therapeutics development. Here, we combined whole-exome and whole-genome sequencing from 21 cohorts across 12 countries and performed rare variant exome-wide burden analyses for COVID-19 outcomes. In an analysis of 5,085 severe disease cases and 571,737 controls, we observed that carrying a rare deleterious variant in the SARS-CoV-2 sensor toll-like receptor TLR7 (on chromosome X) was associated with a 5.3-fold increase in severe disease (95% CI: 2.75–10.05, p = 5.41x10-7). This association was consistent across sexes. These results further support TLR7 as a genetic determinant of severe disease and suggest that larger studies on rare variants influencing COVID-19 outcomes could provide additional insights.

  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Design and development of a graphene-based reconfigurable patch antenna array for THz applications

    • This paper presents a graphene-based antenna array for terahertz (THz) applications. The suggested antenna array has four radiating square shaped patches fed by a coplanar waveguide (CPW) technique. The proposed antenna array operates at the three frequencies with operational bandwidths of 1.173-1.210 THz (at 1.19 THz), 1.270-1.320 THz (at 1.3 GHz), and 1.368-1.346 THz (at 1.4 GHz). The total area of the antenna array is reported as 1000 × 1000 μm2, printed on a Silicon substrate with a thickness of 20 μm and a dielectric constant of ϵ r = 11.9. To enhance the structure's performance and optimize the feeding network, a parametric analysis was performed using the FDTD technique. Furthermore, the key properties of the proposed antenna array, such as resonance frequency, peak gain, and radiation efficiency, may be changed by adjusting the chemical potentials of the graphene in the antenna array. The use of graphene's chemical potential tuneability yields exceptional results comparing to the recent research outputs, with a peak gain and radiation efficiency of 10.45 dB and 70%, respectively. These results show the performance of the suggested design for constructing antenna arrays for use in THz applications.

  • Incidence and factors associated with burnout in radiologists : A systematic review

    • Rationale and objectives: Burnout among physicians has a prevalence rate exceeding 50%. The radiology department is not immune to the burnout epidemic. Understanding and addressing burnout among radiologists has been a subject of recent interest. Thus, our study aims to systematically review studies reporting the prevalence of burnout in physicians in the radiology department while providing an overview of the factors associated with burnout among radiologists. Materials and methods: The search was conducted from inception until November 13th, 2022, in PubMed, Embase, Education Resources Information Center, PsycINFO, and psycArticles. Studies reporting the prevalence of burnout or any subdimensions among radiology physicians, including residents, fellows, consultants, and attendings, were included. Data on study characteristics and estimates of burnout syndrome or any of its subdimensions were collected and summarized. Results: After screening 6379 studies, 23 studies from seven countries were eligible. The number of participants ranged from 26 to 460 (median, 162; interquartile range, 91–264). In all, 18 studies (78.3%) employed a form of the Maslach Burnout Inventory. In comparison, four studies (17.4%) used the Stanford Professional Fulfillment Index, and one study (4.3%) used a single-item measure derived from the Zero Burnout Program survey. Overall burnout prevalence estimates were reported by 14 studies (60.9%) and varied from 33% to 88%. High burnout prevalence estimates were reported by only five studies (21.7%) and ranged from 5% to 62%. Emotional exhaustion and depersonalization prevalence estimates were reported by 16 studies (69.6%) and ranged from 11%−100% and 4%−97%, respectively. Furthermore, 15 studies (65.2%) reported low personal accomplishment prevalence, ranging from 14.7% to 84%. There were at least seven definitions for overall burnout and high burnout among the included studies, and there was high heterogeneity among the cutoff scores used for the burnout subdimensions. Conclusion: Burnout in radiology is increasing globally, with prevalence estimates reaching 88% and 62% for overall and high burnout, respectively. A myriad of factors has been identified as contributing to the increased prevalence. Our data demonstrated significant variability in burnout prevalence estimates among radiologists and major disparities in burnout criteria, instrument tools, and study quality.

    • Open channels are very important to deliver water from main sources to laterals especially for developing countries. Production is subjective by the way that the water is scheduled, and this scheduling is subject to several irrigation constraints. In open channel projects, for instance, maximum discharge of the laterals and main channels, depending on the size of their dimensions and the water requirements for fields. The current paper shows how efficient water scheduling, regarding the delivering water from the main channel to laterals in consequent time slots, can be done by utilizing a genetic algorithm optimisation technique. This research is intended to be applied for scheduling the Shattulhilla River in Babylon City and has broad applications for open channel projects in Iraq. The obtained results clarify how the genetic algorithm optimisation modelling is a sophisticated tool which operators of irrigation projects could now utilize to timetable open channels of irrigation systems.

    • Context: Extensive research conducted within past years revealed that curcumin is a highly pleiotropic molecule that interacts with a diverse range of molecular targets and hence it possess anti-proliferative activities against tumor cells.The great similarities between curcumin analogs and chalcones inspired their testing against tubulin enzyme activity as recent research revealed that chalcones possess cytotoxic activity associated with tubulin inhibition and interference with microtubule formation, which is essential in mitosis and cell replication. Objective: Novel Curcumin analogs were designed, synthesized and tested for their antitumor activities. Also in silico and in vitro studies has been performed to predict the binding affinity of the target compounds and to test their ability to inhibit tubulin assembly and act as microtubule destabilizing agents. Methods: Six novel curcumin analogs were designed & synthesized with 3,5-dibenzylidenepiperidin-4-one core moiety. Results: Compounds showed interaction energy comparable to or within the range of podophyllotoxin itself when docked into the colchicine binding site of tubulin using the podophyllotoxin-tubulin complex (PDB 1SA1). Conclusion: Compounds showed moderate anticancer activity and moderate ability to destabilize microtubules and thus inhibiting tubulin polymerization, as a result; these compounds could be used for further future development to obtain more potent analogs.     • Novel curcumin analogs with 4-piperidone ring were designed, synthesized, and evaluated for their cytotoxic activities against five different cancer cell lines. 3,5-bis(4-Hydroxy-3-methoxybenzylidene)-4-oxo-N-phenylpiperidine-1-carbothioamide (XIIe) exhibited considerable cytotoxic activity with IC50 values in 1-2.5m range. In silico and in vitro, studies were also performed to predict the binding affinity of the target compounds to the -chain of tubulin receptor (PDB code 1SA1) and their abilities to affect microtubules polymerization cycle. 3,5-bis(3-Iodo-5-methoxy-4-propoxybenzylidene)-N-acetylpiperidin-4-one (VIIa) was found to exert 93.3% inhibition of tubulin and destabilization of microtubules in vitro compared to vincristine while, 3,5-bis(3,4,5-trimethoxybenzylidene)-N-benzoylpiperidin-4-one (XIIc) showed high potency in a differentway where it exerted 94.8% stabilization of microtubules in vitro compared to positive control paclitaxel.

    • Nano-composite Ba1-xSr(x)TiO3 (BST), where x=0.01-0.50 and doped with different concentrations of iron Ba(1-x-y)Sr(x)TiFe(y)O3 (BSTF), where x=0.01 and y=0.01-0.05 powders were prepared by sol-gel method. The effect of increasing the iron and strontium concentrations substituted in Barium strontium titanate system will be studied. The prepared samples have a tetragonal crystalline phase after sintering for 1 h at 750 degrees C in air. The structural and the morphological features of the systems have been studied using X-ray diffraction (XRD), transmission electron microscope (TEM) and scanning electron microscope (SEM). The dielectric properties of the BST and BSTF systems have been investigated as a function of temperature and frequency. The dielectric measurements are carried out in the frequency range 42 Hz-1 MHz, at a temperature ranging between 25 and 250 degrees C. The results showed a decrease in T-c temperature giving the following values 90 and 85 degrees C for B10ST and B10ST5F prepared powder samples, respectively, implying tetragonal, feroelectric phase at lower temperature into cubic, para-electric phase at temperature higher than T-c.
