    • Hereditary angioedema (HAE), a rare but life-threatening condition, manifests as acute attacks of facial, laryngeal, genital, or peripheral swelling or abdominal pain secondary to intra-abdominal edema. Resulting from mutations affecting C1 esterase inhibitor (C1-INH), inhibitor of the first complement system component, attacks are not histamine-mediated and do not respond to antihistamines or corticosteroids. Low awareness and resemblance to other disorders often delay diagnosis; despite availability of C1-INH replacement in some countries, no approved, safe acute attack therapy exists in the United States. The biennial C1 Esterase Inhibitor Deficiency Workshops resulted from a European initiative for better knowledge and treatment of HAE and related diseases. This supplement contains work presented at the third workshop and expanded content toward a definitive picture of angioedema in the absence of allergy. Most notably, it includes cumulative genetic investigations; multinational laboratory diagnosis recommendations; current pathogenesis hypotheses; suggested prophylaxis and acute attack treatment, including home treatment; future treatment options; and analysis of patient subpopulations, including pediatric patients and patients whose angioedema worsened during pregnancy or hormone administration. Causes and management of acquired angioedema and a new type of angioedema with normal C1-INH are also discussed. Collaborative patient and physician efforts, crucial in rare diseases, are emphasized. This supplement seeks to raise awareness and aid diagnosis of HAE, optimize treatment for all patients, and provide a platform for further research in this rare, partially understood disorder.

  • Ultra-deep Large Binocular Camera U-band Imaging of the GOODS-North Field : Depth Versus Resolution

    • We present a study of the trade-off between depth and resolution using a large number of U-band imaging observations in the GOODS-North field from the Large Binocular Camera (LBC) on the Large Binocular Telescope (LBT). Having acquired over 30 hr of data (315 images with 5-6 minutes exposures), we generated multiple image mosaics, starting with the best atmospheric seeing images (FWHM less than or similar to 0 ''.8), which constitute similar to 10% of the total data set. For subsequent mosaics, we added in data with larger seeing values until the final, deepest mosaic included all images with FWHM less than or similar to 1 ''.8 (similar to 94% of the total data set). From the mosaics, we made object catalogs to compare the optimal-resolution, yet shallower image to the lower-resolution but deeper image. We show that the number counts for both images are similar to 90% complete to U-AB less than or similar to 26 mag. Fainter than U-AB similar to 27 mag, the object counts from the optimal-resolution image start to drop-off dramatically (90% between U-AB = 27 and 28 mag), while the deepest image with better surface-brightness sensitivity (mu(AB)(U) less than or similar to 32 mag arcsec(-2)) show a more gradual drop (10% between U-AB similar or equal to 27 and 28 mag). For the brightest galaxies within the GOODS-N field, structure and clumpy features within the galaxies are more prominent in the optimal-resolution image compared to the deeper mosaics. We conclude that for studies of brighter galaxies and features within them, the optimal-resolution image should be used. However, to fully explore and understand the faintest objects, the deeper imaging with lower resolution are also required. Finally, we find-for 220 brighter galaxies with U-AB less than or similar to 23 mag-only marginal differences in total flux between the optimal-resolution and lower-resolution light-profiles to mu(AB)(U) less than or similar to 32 mag arcsec(-2). In only 10% of the cases are the total-flux differences larger than 0.5 mag. This helps constrain how much flux can be missed from galaxy outskirts, which is important for studies of the Extragalactic Background Light.

    • Citrus tristeza virus (CTV) is a severe threat to the citrus industry. Disease symptoms and severity may vary depending on the CTV isolates. These are responsible for the decline of trees grafted on sour orange rootstock, or stem pitting on some citrus commercial cultivars regardless of rootstock. In the Calabria region (Italy), CTV was first reported on cultivars imported from other countries. However, recent observations suggested that natural spread of CTV was occurring and a study was needed to determine the epidemiological status and aphid transmission of CTV in Calabria. The role played by local A. gossypii in the spread of CIV was analyzed in the laboratory using various viral acquisition, inoculation periods with three different CTV isolates. Single aphid vectors acquired CTV after a minimum of 30 min acquisition access period (AAP) and were able to transmit the virus after a 60 min inoculation access period (IAP) to healthy plants. A minimum of four aphid vectors were needed to reach 50% transmission probability. The results suggested that the three tested strains are transmitted by A. gossypii in a semi-persistent mode. The results demonstrated that local A. gossypii population can acquire and transmit efficiently the tested virus isolates with serious implications on the virus spread.     • Abstract Laboratory diagnostics (i.e., the total testing process) develops conventionally through a virtual loop, originally referred to as "the brain to brain cycle" by George Lundberg. Throughout this complex cycle, there is an inherent possibility that a mistake might occur. According to reliable data, preanalytical errors still account for nearly 60%-70% of all problems occurring in laboratory diagnostics, most of them attributable to mishandling procedures during collection, handling, preparing or storing the specimens. Although most of these would be "intercepted" before inappropriate reactions are taken, in nearly one fifth of the cases they can produce inappropriate investigations and unjustifiable increase in costs, while generating inappropriate clinical decisions and causing some unfortunate circumstances. Several steps have already been undertaken to increase awareness and establish a governance of this frequently overlooked aspect of the total testing process. Standardization and monitoring preanalytical variables is of foremost importance and is associated with the most efficient and well-organized laboratories, resulting in reduced operational costs and increased revenues. As such, this article is aimed at providing readers with significant updates on the total quality management of the preanalytical phase to endeavour further improvement for patient safety throughout this phase of the total testing process.

  • Micronuclei in Cord Blood Lymphocytes and Associations with Biomarkers of Exposure to Carcinogens and Hormonally Active Factors, Gene Polymorphisms, and Gene Expression : The NewGeneris Cohort

    • Background: Leukemia incidence has increased in recent decades among European children, -suggesting that early-life environmental exposures play an important role in disease development. Objectives: We investigated the hypothesis that childhood susceptibility may increase as a result of in utero exposure to carcinogens and hormonally acting factors. Using cord blood samples from the NewGeneris cohort, we examined associations between a range of biomarkers of carcinogen exposure and hormonally acting factors with micronuclei (MN) frequency as a proxy measure of cancer risk. Associations with gene expression and genotype were also explored. Methods: DNA and protein adducts, gene expression profiles, circulating hormonally acting factors, and GWAS (genome-wide association study) data were investigated in relation to genomic damage measured by MN frequency in lymphocytes from 623 newborns enrolled between 2006 and 2010 across Europe. Results: Malondialdehyde DNA adducts (M(1)dG) were associated with increased MN frequency in binucleated lymphocytes (MNBN), and exposure to androgenic, estrogenic, and dioxin-like compounds was associated with MN frequency in mononucleated lymphocytes (MNMONO), although no monotonic exposure-outcome relationship was observed. Lower frequencies of MNBN were associated with a 1-unit increase expression of PDCD11, LATS2, TRIM13, CD28, SMC1A, IL7R, and NIPBL genes. Gene expression was significantly higher in association with the highest versus lowest category of bulky and M(1)dG-DNA adducts for five and six genes, respectively. Gene expression levels were significantly lower for 11 genes in association with the highest versus lowest category of plasma AR CALUX (R) (chemically activated luciferase expression for androgens) (8 genes), ER alpha CALUX (R) (for estrogens) (2 genes), and DR CALUX (R) (for dioxins). Several SNPs (single-nucleotide polymorphisms) on chromosome 11 near FOLH1 significantly modified associations between androgen activity and MNBN frequency. Polymorphisms in EPHX1/ 2 and CYP2E1 were associated with MNBN. Conclusion: We measured in utero exposure to selected environmental carcinogens and circulating hormonally acting factors and detected associations with MN frequency in newborns circulating T lymphocytes. The results highlight mechanisms that may contribute to carcinogen-induced leukemia and require further research.
