    • OBJECTIVES: The aim of this study was to identify clinical characteristics of depression in elderly people that could be useful for case finding in general practice. DESIGN: A cross-sectional study of clinical characteristics through review of medical records. SETTING: Herrhagen health centre, Karlstad, Sweden. SUBJECTS: Seventy-one persons with a high depressive score in a screening of depressive symptoms and an age-matched and sex-matched control group of 138 persons with a low depressive score. RESULTS: The high depressive score group had an increased relative risk for "mental health problems" (RR 3.4; CI 95% 1.7-7.2), "many contacts with the health care centre" (> or = 14/3 years) (RR 2.9; CI 95% 1.4-6.1), and prescriptions of benzodiazepines (RR 1.7; CI 95% 1.0-2.9). Two-thirds of those in the high depressive score group had at least one of these characteristics. However, three-quarters of those with any of these characteristics had a low depressive score. In our population of elderly people with an estimated prevalence of 10.2% the positive predictive value would be 21% and the negative predictive value 95%. CONCLUSION: General practitioners should suspect a possible depressive disorder in elderly patients with mental health problems, with frequent contacts with the health centre or with prescriptions of benzodiazepines. Despite the high occurrences of these prominent clinical characteristics in the high-score group, they did not unfortunately have sufficient discriminatory power to be useful for case finding.

    • Studies have found that depressive states in old age that do not fulfil the criteria for major depression are clinically important depressive states. These depressive states are common in primary care patients. Therefore, studies of late-life depression from a primary care perspective are needed. A depressive rating scale, Hopkins Symptom Checklist-25 (HSCL-25), was used in a community survey in 1993. The prevalence of high depressive score was 10.2%. In a medical record review, the high score group and an age- and sex-matched low score group were compared in order to identify clinical characteristics. The high score group often had many contacts with health care, recognised mental health problems and/or prescribed psychotropic medication. However, these characteristics did not have the discriminatory power for case-finding. In a 6-year outcome study in 1999 the risk of dying were nearly twice as high for the high score group as for the low score group. The overall outcome was poor in the high score group: 72% were either dead, had moved to long-term accommodation or were depressed. HSCL-25 was compared with Montgomery-Åsberg Depression Rating Scale (MADRS) and with the diagnostic criteria for depressive disorders in an interview study. HSCL-25, with cut-off > 1.75, was found to be a sensitive case finder for clinically important depressive states. The data from the interview study were also used for identifying the most characteristic symptoms of clinically important late-life depression. We found that the symptoms “hopelessness”, “everything is an effort” and “lassitude” were in good agreement with depression, of which “hopelessness” could be useful as a key symptom.

    • Objectives We examined cognitive functions before and in acute phase of stroke studying frequency and profile of cognitive impairment and relationships between cognitive status.MethodsSeventy-four patients with early phase after stroke and 49 healthy controls were included and examined using the Mini-Mental State Examination (MMSE) and a battery of neuropsychological tests. Cognitive status before stroke-onset was investigated using Cognitive Impairment Questionnaire.ResultsCognitive impairments were present in 96% of patients after stroke onset using the battery of neuropsychological tests and in 39% of patients using the MMSE, but in only 9% of controls. Seventy-six percent exhibited reduced executive function and 75% reduced psychomotor tempo. Cognitive dysfunction was present in 52% before stroke onset without any impact on the frequency of impairment in the various cognitive areas in early phase after stroke.ConclusionsCognitive impairment is frequent before the onset of stroke among older people and may partially explain the very high frequency of cognitive impairment observed after stroke onset.
