  • Acutely reduced locomotor activity is a major contributor to Western diet-induced obesity in mice

    • The aim of the present study was to investigate the short- and long-term effects of a high-fat Western diet (WD) on intake, storage, expenditure, and fecal loss of energy as well as effects on locomotor activity and thermogenesis. WD for only 24 h resulted in a marked physiological shift in energy homeostasis, including increased body weight gain, body fat, and energy expenditure (EE) but an acutely lowered locomotor activity. The acute reduction in locomotor activity was observed after only 3–5 h on WD. The energy intake and energy absorption were increased during the first 24 h, lower after 72 h, and normalized between 7 and 14 days on WD compared with mice given chow diet. Core body temperature and EE was increased between 48 and 72 h but normalized after 21 days on WD. These changes paralleled plasma T3 levels and uncoupling protein-1 expression in brown adipose tissue. After 21 days of WD, energy intake and absorption, EE, and body temperature were normalized. In contrast, the locomotor activity was reduced and body weight gain was increased over the entire 21-day study period on WD. Calculations based on the correlation between locomotor activity and EE in 2-h intervals at days 21–23 indicated that a large portion of the higher body weight gain in the WD group could be attributed to the reduced locomotor activity. In summary, an acute and persisting decrease in locomotor activity is most important for the effect of WD on body weight gain and obesity in mice.

    • The adipocyte-derived hormone adiponectin regulates glucose and lipid metabolism and influences the risk for developing obesity, type 2 diabetes, and cardiovascular disease. Adiponectin binds to two different seven-transmembrane domain receptors termed AdipoR1 and AdipoR2. To study the physiological importance of these receptors, AdipoR1 gene knockout mice (AdipoR1−/−) and AdipoR2 gene knockout mice (AdipoR2−/−) were generated. AdipoR1−/− mice showed increased adiposity associated with decreased glucose tolerance, spontaneous locomotor activity, and energy expenditure. However, AdipoR2−/− mice were lean and resistant to high-fat diet–induced obesity associated with improved glucose tolerance and higher spontaneous locomotor activity and energy expenditure and reduced plasma cholesterol levels. Thus, AdipoR1 and AdipoR2 are clearly involved in energy metabolism but have opposing effects.

    • Objective: The study focus on oral health-related quality of life (OHRQOL) and how oral health effect individuals quality of life. The aim of this presentation was to describe the distribution of oral health impact profile (OHIP-14) in a population in the city of Jönköping, Sweden. Research Methods: A random selected sample of 572 (276 male, 296 female) individuals in age groups 20, 30, 40, 50, 60, 70 and 80 years answered a OHIP-14questioinnaire. The questionnaire capture five dimensions and 14 items. The dimensions are “functioning”, “discomfort”, “psychological aspects”, “social aspects” and “disabilities”. Results: 423 individuals answered all 14 items and 95 individuals answered 1-13 items. Of the individuals that had answered all items 23% were without problems. Highest frequency of answers with problems was found within the dimension “functioning” and the item “pain problems” (43% of individuals) and lowest frequency of answers with problems was found in the dimension “disability” and the item “totally unable to function”, (11% of individuals). Within the dimension “discomfort” the item “been self-consious during the last year” got answered from 39% of the individuals. Seventeen % of these individuals answered that they had problems “very often” or “quite often”. Outcomes: The results from this epidemiological study covering a random sample of a normal adult swedish population is expected to increase the knowledge about OHIP-14 and the possibility to study factors of importants for oral health-related quality of life.

    • Bakgrund: Munhälsan har stor betydelse för den allmänna hälsan och välbefinnandet och påverkar därmed livskvaliteten .Syftet med studien var att beskriva munhälsorelaterad livskvalitet i en svensk vuxen population.Metod: Munhälsorelaterad livskvalitet mättes med frågeformuläret Oral Health Impact Profile (OHIP-14). Med OHIP-14 poängsätts munbesvär (0-56 p). Mer besvär ger högre poäng. Ett slumpmässigt urval av 910 individer, 20-80 år tillfrågades att deltaResultat: Totalt 516 individer deltog. 78% uppgav att de hade någon form av orala besvär relaterat till livskvaliteten.  20-åriga kvinnor var de som hade mest besvär. 20% av 30-åriga män och 59% av 70-åriga kvinnor uppgav att de haft besvär från munnen som inneburit att livet varit mindre tillfredställande. Andelen individer med svåra besvär, 17-41 poäng, var 9 %.Slutsatser: Den studerade populationen uppvisade i sin helhet en god munhälso-   relaterad livskvalitet. Det förekom ett relativt stort antal både unga och äldre individer med munbesvär som påverkade livskvaliteten.

  • Oral health impact on quality of life in an adult Swedish population

    • Objectives: Quality of life is an  expression with many meanings  and it creates positive associations  for most people. Oral health has a major impact on general quality of life. The aim of this study was to describe  oral health-related quality of life measured with Oral Health Impact  profile with 14 questions (OHIP-14)  in an Swedish adult population. Methods: This is an epidemiological study and it comprised a stratified random sample of 260 men and 259 women, 20-80 years of age. The OHIP-14 questionnaire was used. Results: Twenty-one percent of the respondents stated that they had no oral problems that were negative related to their well-being, and 79% had some form of problem related to quality  of life. The mean value for the entire population was 6.4 (SD=7.1). Of the individuals who stated that they had oral problems, the majority were among women aged 20-yearsInternal reliability of the OHIP scale showed good internal consistency with a Cronbach alpha coefficient of 0.90. Conclusions: In this Swedish population, a number of individuals, young and old, experienced oral problems that had a negative impact on their well-being.

The aspects of oral health that are most important for each individual vary, and quality of life is a construct and not a measurable variable. The aim of this study was to describe the impact of oral health on quality of life in an adult Swedish population. Material and methods. The study comprised a stratified random sample of 519 individuals. The OHIP-14 questionnaire was used and the answers were rated on a scale of 0-4. Results. Twenty-one percent of the respondents stated that they had no oral problems related to well-being, and 79% had some form of problem related to quality of life. The mean value for the entire population was 6.4 (SD = 7.1); 5.9 (SD = 7.1) for men and 6.8 (SD = 7.2) for women. Of individuals who stated that they had oral problems, the majority were women aged 20 years. Nineteen percent of 30-year-old men and 53% of 70-year-old women stated that they had had problems that had made life less satisfactory. Individuals who frequently experienced problems related to oral health, with scores of 16-41 points, accounted for 10%. Conclusion. In this Swedish population, a number of individuals, young and old, experienced oral problems that had an impact on their well-being.

    • The impact of oral health on quality of life is one aspect when it comes to understanding the significance of oral health. The aim of this sudy was to analyse the self-reported oral discomfort and clincial status of individuals reporting oral problems never/very seldom affexting quality of life during the last year and compar ehem with individuals who reported oral problems hardly ever/occasiaonally or often/very often during the same period. The study comprised a stratified random sample of 515 individuals who lived in four parishes in the City of Jönköping, Sweden, and tyrned 20, 30, 40, 50, 60, 70 and 80 years of age in 2003. The impact of oral health on quality of life was examined using the OHIP-14 questionnaire. The individuals were also examined clinically and radiographically. Of the participants, 21 % reorted no experience of impaired quality of life and 20 % of the indivudals reported that they had expperienced impaires quality of life often or very often during the last year. The highest frequency of oral problems was found among individuals aged 20 and 80 years. Subjective symptoms, such as grinding/clenching and headache, were found among 20- and 30-year-olds. Edentulous individuals with many missing teeth, individuals with severe periodontal disease or subjective dry mouth answered that they experienced problems accordning to the OHIP-14 often or very often. A number of individuals, young and old, had thus experienced subjective or clinically verified oral conditions associated with a negative experience of quality of life. This complementary information will provide a deeper understanding of the importance of oral health in the population.

    • The aim: The overall aim of this licentiate thesis was to describe and analyze oral health-related quality of life measured with OHIP-14. The thesis comprises two studies, each with a specific aim.   Study I: To describe oral health-related quality of life measured with OHIP-14 in an Swedish adult population Study II: To study the relationship between oral health-related quality of life, measured with OHIP-14, and subjective as well as objective dry mouth conditions in fragile old people. Furthermore, the aim was to study the reliability and validity of a new instrument (VAS) to measure dry mouth   Material and methods: Study I comprised a stratified random sample of 519 individuals 20-80 years of age. In Study II, 41 randomly selected fragile old people, residents at three different community care centers, participated. In both studies, the questionnaire OHIP-14 was used for measurement of oral health-related quality of life. The participants in Study II answered a questionnaire for subjectively experienced dry mouth (VAS). For objective dry mouth measurements, saliva was absorbed into a preweighted cotton roll. Results: In Study I, 21% of the respondents stated that they had no oral problems that had a negative impact on their well-being. In Study II, the corresponding figure was 71%. In study I the mean value for OHIP-14 was 6.4 (SD=7.1) for the entire population; 5.9 (SD=7.1) for men and 6.8 (SD=7.2) for women in. Subjects, who frequently experienced problems related to oral health, with scores ranging from 16 to 41 points, accounted for 10% of the study group. In Study II, significant associations were identified between both objectively measured respective subjectively experienced dry mouth and oral health-related quality of life. The validity of the VAS instrument was good for subjective mouth dryness, but poor for objectively measured dry mouth in fragile old people. Conclusion: From Study I it can be concluded that, in this Swedish population, a number of individuals, both young and old, experience oral problems that have a negative impact on their well-being. From Study II, the conclusion is that dry mouth (both objective and subjective) is significantly associated with poorer oral health-related quality of life, underlining the value of monitoring dry mouth conditions in the care of fraigile old people.

    • Syphilis is currently a treatable disease, with a low incidence in most developed countries, although the prevalence has increased recently, especially among men -who -have -sex -with -men. In many of the least developed countries, however, syphilis is still a major health problem, although the problem is not comparable to the desperate situation worldwide less than 80 years ago. At that time, and for many centuries previously, syphilis dramatically affected the lives and health of individuals and threatened the well-being of many societies. This review examines the aetiology, transmission, and many manifestations of syphilis from a historical perspective, emphasizing morbidity, treatment, psychosocial and cultural manifestations, as well as ethical issues uncovered in the clinical search for knowledge about the manifestations of the disease.

    • Objective. Studies of oral health developments increasingly include self-reported assessments of how oral health affects quality of life (QoL), referred to as “oral health-related QoL”. People with dementia are often excluded in studies of oral health-related QoL and thus our aim in this study was to explore this area in elderly persons with dementia. Material and methods. Eighteen elderly individuals (aged between 78 and 94 years) with dementia of varying degrees of severity were interviewed with the aid of an interview guide; pictures and objects were used as stimulus material (triggers). The material was analyzed using grounded theory as point of departure, and a professional assessment of the oral health of the participants was used as reference. Results. Four categories were identified: the ability to chew and eat, independence, oral problems, and teeth are important. These factors are largely consistent with those that have emerged in earlier studies of the elderly, but in some cases less pronounced in persons with dementia. Conclusion. The use of triggers is a positive way to communicate oral health-related QoL among persons suffering from dementia, although the material used in this study needs further evaluation and development.

  • Preoperative beta blockers and other drugs in relation to anastomotic leakage after anterior resection for rectal cancer

    • Aim: Previous research has indicated that preoperative beta blocker therapy is associated with a decreased risk of complications after surgery for rectal cancer. This is thought to arise because of the anti-inflammatory activity of the drug. These results need to be reproduced and analyses extended to other drugs with such properties, as this information might be useful in clinical decision-making. The main aim of this work was to replicate previous findings of beta blocker use as a prognostic marker for postoperative leakage. We also investigated whether drug exposure might induce anastomotic leaks.Method: This is a retrospective multicentre cohort study, comprising 1126 patients who underwent anterior resection for rectal cancer between 2014 and 2018. The use of any preoperative beta blocker was treated as the primary exposure, while anastomotic leakage within 12 months of surgery was the outcome. Secondary exposures comprised angiotensin-converting enzyme inhibitors/angiotensin receptor blockers, statins and metformin. Using multivariable regression, we performed a replication analysis with a predictive aim for beta blockers only, while adjustment for confounding was done in more causally oriented analyses for all drugs. We estimated incidence rate ratio (IRR) and relative risk (RR) with 95% confidence intervals (CIs).Results: Anastomotic leakage occurred in 20.6% of patients. Preoperative beta blockers were used by 22.7% of the cohort, while the leak distribution was almost identical between exposure groups. In the main replication analysis, no association could be detected (IRR 0.95, 95% CI 0.68–1.33). In the causally oriented analyses, only metformin affected the risk of leakage (RR 1.59, 95% Cl 1.31–1.92).Conclusion: While previous research has suggested that preoperative beta blocker use could be prognostic of anastomotic leakage, this study could not detect any such association. On the contrary, our results indicate that preoperative beta blocker use neither predicts nor causes anastomotic leakage after anterior resection for rectal cancer.

    • Background and aims: The importance of efficient and safe treatment of Crohns disease is highlighted by its chronicity. Both medical and surgical treatments have shown good results in the symptomatic control of limited ileocaecal Crohns disease. The aim of this study was to compare medical treatment with surgical treatment of ileocaecal Crohns disease. Methods: Thirty-six patients from seven hospitals with primary ileocaecal Crohns disease were randomized to either medical or surgical treatment. The medical treatment was induction of remission with budesonide and thereafter maintenance treatment with azathioprine. The surgical treatment was open ileocaecal resection. Crohns disease activity index over time, expressed as area under the curve at 1, 3 and 5 years, was the primary endpoint. Subjective health measured with the 36-item Short Form Survey Instrument (SF36) and a visual analogue scale (VAS) were secondary endpoints. Results: There were no differences between the treatment groups in Crohns disease activity index over time. General health, measured as SF36 score, was higher in patients receiving surgical treatment than in those receiving medical treatment at 1 year, but there was no corresponding difference in VAS. Due to the slow inclusion rate and changes in clinical practice, the study was t = erminated prematurely. Conclusion: The study ended up being underpowered and should be interpreted with caution, but there was no clinically significant difference between the two treatment arms. Further studies are needed to address this important clinical question.

    • Background: Antonovskys concept of "sense of coherence (SOC) has been shown to be related to health. The aim of this study was to describe the distribution of SOC scores and their components in an adult Swedish population aged 20-80 years. Methods: A random sample of 910 individuals from Jonkoping, Sweden, aged 20, 30, 40, 50, 60, 70 and 80 years, of which 589 agreed to participate in an oral health examination. The participants answered Antonovskys 13-item version of "the life orientation questionnaire scale. The response to the items and the distribution of the three components of comprehensibility, manageability and meaningfulness were analyzed for different age groups and genders using mean values and standard deviations, Students t-test and ANOVA. Results: A total of 526 individuals, 263 men and 263 women, answered all 13 questions and constituted the final material for the study (response rate 89%). The individual SOC score increased with age. The 20 year olds had a statistically significantly lower SOC score compared with the other age groups and 55% of them had a low SOC (andlt;= 66 points) compared with 17% of the 80 year olds. Men in the 60 and 70 year age groups had a statistically significantly higher SOC score compared with women of the same age. Conclusions: The individual distribution of SOC varied with age and gender. Twenty year olds had a significantly lower SOC score compared with elderly age groups. Elderly men had a statistically significantly higher SOC score compared with women of the same age.

    • Time lapse video recordings of cultured adult human and guinea pig spiral ganglion (hSG and gpSG) show that mitogen responsive progenitor/stem cells develop in the form of spheres that proliferate and differentiate into mature neurons and glia cells. Neurospheres, cultured with EGF and bFGF showed expression of nestin and incorporation of 5'-Bromo-2-deoxyuridine (BrdU). Newly formed BrdU labelled cells were positive for beta-tubulin, and also for GFAP demonstrating that neuronal cells were derived from a dividing population of progenitor cells. Dissociated spheres cultured either with glia cell line-derived neurotrophic factor (GDNF) or brain-derived neurotrophic factor (BDNF) and neurotrophin-3 (NT-3), induced differentiation of the progenitor cells. Video microscopy showed that neurons develop from subcultured spheres maintained for up to four weeks. Neurons showed fasciculation and migration with a speed of 10-30 microm/h, and some cells had up to 6 mm long neurites coexpressing TrkB and TrkC receptors. Precise dissection suggests that the neurons formed are cochlea-specific. The results suggest that the mammalian auditory nerve has the capability for self-renewal and replacement. Transplantation of progenitor cells together with established means to induce neural differentiation and fiber growth may facilitate strategies for better repair and treatment of auditory neuronal damage.

  • Robotic low anterior resection with complete splenic flexure mobilization and defunctioning left-sided loop colostomy : a case series

    • A defunctioning stoma is used to alleviate the consequences of anastomotic leakage after low anterior resection for rectal cancer. A loop ileostomy is often preferred but may lead to dehydration and kidney injury. Here, we present a case series for an alternative: the left-sided loop colostomy. A convenience sample of four patients underwent robotic low anterior resection for rectal cancer. A complete splenic flexure mobilization and a total mesorectal excision were performed. To defunction the anastomosis, the redundant left colon was brought up to a stoma site in the left iliac fossa and matured as a loop colostomy. Two patients experienced minor stoma leaks and one also had a small prolapse, while all patients had their colostomies reversed on average 7 months after surgery without complications. There were no dehydration episodes and creatinine levels remained within baseline levels at end of follow-up (on average 18 months).

    • Our aim was to study risk factors for Dupuytren's contracture (DC) by assessing cancer morbidity in a group of Swedish patients treated surgically for Dupuytren's contracture. The risk of cancer was determined in 15,212 patients operated on for Dupuytren's contracture, identified in the nationwide Swedish Inpatient Register during the period 1965 to 1994 by means of record linkage to the Swedish Cancer Register. Standardized incidence ratios (SIRs) were computed using age-, sex- and period-specific incidence rates derived from the entire Swedish population. The overall relative risk of cancer was increased by 24%. There were significantly increased risks for malignancies related to smoking such as buccal, oesophageal, gastric, lung and pancreatic cancers. Significantly increased risks were present for both prostate and rectal cancer in men and an increase risk for breast cancer in women was noted 1 year or more after surgery for Dupuytren's contracture. The present study confirms smoking and alcohol abuse as probable risk factors for DC. There are characteristics in patients with DC that alter the risks for other malignancies compared with the general population.
