Introduction: With the goal of generate uniform criteria among centers dealing with pituitary tumors and to enhance patient care, the Pituitary Society decided to generate criteria for developing Pituitary Tumors Centers of Excellence (PTCOE). Methods: To develop that task, a group of ten experts served as a Task Force and through two years of iterative work an initial draft was elaborated. This draft was discussed, modified and finally approved by the Board of Directors of the Pituitary Society. Such document was presented and debated at a specific session of the Congress of the Pituitary Society, Orlando 2017, and suggestions were incorporated. Finally the document was distributed to a large group of global experts that introduced further modifications with final endorsement. Results: After five years of iterative work a document with the ideal criteria for a PTCOE is presented. Conclusions: Acknowledging that very few centers in the world, if any, likely fulfill the requirements here presented, the document may be a tool to guide improvements of care delivery to patients with pituitary disorders. All these criteria must be accommodated to the regulations and organization of Health of a given country.

  • A planetesimal orbiting within the debris disc around a white dwarf star

    • Many white dwarf stars show signs of having accreted smaller bodies, implying that they may host planetary systems. A small number of these systems contain gaseous debris discs, visible through emission lines. We report a stable 123.4-minute periodic variation in the strength and shape of the Ca II emission line profiles originating from the debris disc around the white dwarf SDSS J122859.93+104032.9. We interpret this short-period signal as the signature of a solid-body planetesimal held together by its internal strength.

    • In statistics, samples are drawn from a population in a data-generating process (DGP). Standard errors measure the uncertainty in estimates of population parameters. In science, evidence is generated to test hypotheses in an evidence-generating process (EGP). We claim that EGP variation across researchers adds uncertainty-nonstandard errors (NSEs). We study NSEs by letting 164 teams test the same hypotheses on the same data. NSEs turn out to be sizable, but smaller for more reproducible or higher rated research. Adding peer-review stages reduces NSEs. We further find that this type of uncertainty is underestimated by participants.

  • Detection and Classification of Measurement Errors in Bioimpedance Spectroscopy

    • Bioimpedance spectroscopy (BIS) measurement errors may be caused by parasitic stray capacitance, impedance mismatch, cross-talking or their very likely combination. An accurate detection and identification is of extreme importance for further analysis because in some cases and for some applications, certain measurement artifacts can be corrected, minimized or even avoided. In this paper we present a robust method to detect the presence of measurement artifacts and identify what kind of measurement error is present in BIS measurements. The method is based on supervised machine learning and uses a novel set of generalist features for measurement characterization in different immittance planes. Experimental validation has been carried out using a database of complex spectra BIS measurements obtained from different BIS applications and containing six different types of errors, as well as error-free measurements. The method obtained a low classification error (0.33%) and has shown good generalization. Since both the features and the classification schema are relatively simple, the implementation of this pre-processing task in the current hardware of bioimpedance spectrometers is possible.

    • Since there are several applications of Electrical Bioimpedance (EBI) that use the Cole parameters as base of the analysis, to fit EBI measured data onto the Cole equation is a very common practice within Multifrequency-EBI and spectroscopy. The aim of this paper is to compare different fitting methods for EBI data in order to evaluate their suitability to fit the Cole equation and estimate the Cole parameters. Three of the studied fittings are based on the use of Non-Linear Least Squares on the Cole model, one using the real part only, a second using the imaginary part and the third using the complex impedance. Furthermore, a novel fitting method done on the impedance plane, without using any frequency information has been implemented and included in the comparison. Results show that the four methods perform relatively well but the best fitting in terms of standard error of estimate is the fitting obtained from the resistance only. The results support the possibility of measuring only the resistive part of the bioimpedance to accurately fit Cole equation and estimate the Cole parameters, with entailed advantages.

    • We describe the Sloan Digital Sky Survey IV (SDSS-IV), a project encompassing three major spectroscopic programs. The Apache Point Observatory Galactic Evolution Experiment 2 (APOGEE-2) is observing hundreds of thousands of Milky Way stars at high resolution and. high signal-to-noise ratios in the near-infrared. The Mapping Nearby Galaxies at Apache Point Observatory (MaNGA) survey is obtaining spatially resolved spectroscopy for thousands of nearby galaxies (median z similar to 0.03). The extended Baryon Oscillation Spectroscopic Survey (eBOSS) is mapping the galaxy, quasar, and neutral gas distributions between z similar to 0.6 and 3.5 to constrain cosmology using baryon acoustic oscillations, redshift space distortions, and the shape of the power spectrum. Within eBOSS, we are conducting two major subprograms: the SPectroscopic IDentification of eROSITA Sources (SPIDERS), investigating X-ray AGNs. and galaxies in X-ray clusters, and the Time Domain Spectroscopic Survey (TDSS), obtaining spectra of variable sources. All programs use the 2.5 m Sloan Foundation Telescope at the. Apache Point Observatory; observations there began in Summer 2014. APOGEE-2 also operates a second near-infrared spectrograph at the 2.5 m du Pont Telescope at Las Campanas Observatory, with observations beginning in early 2017. Observations at both facilities are scheduled to continue through 2020. In keeping with previous SDSS policy, SDSS-IV provides regularly scheduled public data releases; the first one, Data Release 13, was made available in 2016 July.

  • Immune-mediated febrile response in female rats: Role of central hypothalamic mediators

    • Lipopolysaccharide (LPS) induces fever through cytokines like receptor-activator of nuclear factor kappa B ligand (RANKL), triggering mediators like prostaglandins (PG), endothelin-1 (ET-1), corticotrophin-releasing factor (CRF), substance P (SP) and endogenous opioids. LPS-induced fever is reduced in females compared with males except in ovariectomized (OVX) females which show increased fever mediated by PG. The present study aimed to identify the mediators involved in fever in intact and OVX female rats. Fever was induced with LPS (50 mu g/kg) intraperitoneally or CRF (2.5 mu g), ET-1 (1 pg), morphine (10 mu g) and SP (500 ng) intracerebroventricularly in sham-operated and OVX rats. The role of RANKL was evaluated with osteoprotegerin (OPG, 1 mu g, intracerebroventricularly). Expression of RANK, CRFI/II, ETB, mu-opioid (MOR) and NK1 receptors was evaluated by confocal microscopy. Besides LPS, only morphine induced fever in OVX rats while all mediators induced fever in sham-operated animals. OPG abolished LPS-induced fever in OVX but not sham-operated animals. Overall, fever involves similar central mediators in cycling females and males but only morphine induced fever in OVX females. Importantly, RANK/RANKL participates in LPS-induced fever in OVX females, as in males but not in cycling females.

Such Hook Effect is noticeable at high frequencies but it also causes a data deviation at lower measurement frequencies. In order to perform any accurate analysis of the EBI spectroscopy data, the influence of the Hook Effect must be removed. An established method to compensate the hook effect is the well known Td compensation, which consist on multiplying the obtained spectrum, Zmeas() by a complex exponential in the form of exp[jTd]. Such a method cannot correct entirely the Hook Effect since the hook-alike deviation occurs a broad frequency range in both magnitude and phase of the measured impedance, and by using a real value for Td. First, a real value only modifies the phase of the measured impedance and second, it can only correct the Hook Effect at a single frequency. In addition, the process to select a value for Td by an iterative process with the aim to obtain the best Cole fitting lacks solid scientific grounds. In this work the Td compensation method is revisited and a modified approach for correcting the Hook Effect that includes a novel method for selecting the correcting values is proposed. The initial validation results confirm that the proposed method entirely corrects the Hook Effect at all frequencies.

    • Activities around applications of Electrical Bioimpedance Spectroscopy (EBIS) have proliferated in the past decade significantly. Most of these activities have been focused in the analysis of the EBIS measurements, which eventually might enable novel applications. In Body Composition Assessment (BCA) the most common analysis approach currently used in EBIS is based on the Cole function, which most often requires curve fitting. One of the most implemented approaches for obtaining the Cole parameters is performed in the impedance plane through the geometrical properties that the Cole function exhibit in such domain as depressed semi-circle. To fit the measured impedance data to a semi-circle in the impedance plane, obtaining the Cole parameters in an indirect and sequential manner has several drawbacks. Applying a Non-Linear Least Square (NLLS) iterative fitting on the spectroscopy measurement, obtains the Cole parameters considering the frequency information contained in the measurement. In this work, from experimental total right side EBIS measurements, the BCA parameters have been obtained to assess the amount and distribution of whole body fluids. The values for the BCA parameters have been obtained using values for the Cole parameters estimated with both approaches: circular fitting on the impedance plane and NLLS impedance-only fitting. The comparison of the values obtained for the BCA parameters with both methods confirms that the NLLS impedance-only is an effective alternative as Cole parameter estimation method in BCA from EBIS measurements. Using the modulus of the Cole function as the model for the fitting would eliminate the need for performing a phase detection in the acquisition process, simplifying the hardware specifications of the measurement instrumentation when implementing a bioimpedance spectrometer.

    • Applications based on measurements of Electrical Bioimpedance Spectroscopy (EBIS) analysis are proliferating. The most spread and known application of EBIS is the non-invasive assessment of body composition. Fitting to the Cole function to obtain the Cole parameters, R0 and R∞, is the core of the EBIS analysis to obtain the body fluid distribution. An accurate estimation of the Cole parameters is essential for the Body Composition Assessment (BCA) and the estimation process depends on several factors. One of them is the upper frequency limit used for the estimation and the other is the number of measured frequencies in the measurement frequency range. Both of them impose requirements on the measurement hardware, influencing largely in the complexity of the bioimpedance spectrometer. In this work an analysis of the error obtained when estimating the Cole parameters with several frequency ranges and different number of frequencies has been performed. The study has been done on synthetic EBIS data obtained from experimental Total Right Side (TRS) measurements. The results suggest that accurate estimations of R0 and R∞ for BCA measurements can be achieved using much narrower frequency ranges and quite fewer frequencies than electrical bioimpedance spectrometers commercially available nowadays do.

    • Often when performing electrical bioimpedance (EBI) spectroscopy measurements, the obtained EBI data present a hook-like deviation, which is most noticeable at high frequencies in the impedance plane. The deviation is due to a capacitive leakage effect caused by the presence of stray capacitances. In addition to the data deviation being remarkably noticeable at high frequencies in the phase and the reactance spectra, the measured EBI is also altered in the resistance and the modulus. If this EBI data deviation is not properly removed, it interferes with subsequent data analysis processes, especially with Cole model-based analyses. In other words, to perform any accurate analysis of the EBI spectroscopy data, the hook deviation mustbe properly removed. Td compensation is a method used to compensate the hook deviation present in EBI data; it consists of multiplying the obtained spectrum, Zmeas(ω), by a complex exponential in the form of exp(–jωTd). Although the method is well known and accepted, Td compensation cannot entirely correct the hook-like deviation; moreover, it lacks solid scientific grounds. In this work, the Td compensation method is revisited, and it is shown that it should not be used to correct the effect of a capacitive leakage; furthermore, a more developed approach for correcting the hook deviation caused by the capacitive leakage is proposed. The method includes a novel correcting expression and a process for selecting the proper values of expressions that are complex and frequency dependent. The correctness of the novel method is validated with the experimental data obtained from measurements from three different EBI applications. The obtained results confirm the sufficiency and feasibility of the correcting method.

    • The analysis of measurements of Electrical Bioimpedance (EBI) is on the increase for performing non-invasive assessment of health status and monitoring of pathophysiological mechanisms. EBI measurements might contain measurements artefacts that must be carefully removed prior to any further analysis. Cole model-based analysis is often selected when analysing EBI data and might lead to miss-conclusion if it is applied on data contaminated with measurement artefacts. The recently proposed Correction Function to eliminate the influence of the Hook Effect from EBI data and the fitting to the real part of the Cole model to extract the Cole parameters have been validated on experimental measurements. The obtained results confirm the feasible experimental use of these promising pre-processing tools that might improve the outcome of EBI applications using Cole model-based analysis.

    • Objective: The aim of this clinical trial is to examine whether it is possible to reduce postoperative complications using an individualized perioperative ventilatory strategy versus using a standard lung-protective ventilation strategy in patients scheduled for thoracic surgery requiring one-lung ventilation. Design: International, multicenter, prospective, randomized controlled clinical trial. Setting: A network of university hospitals. Participants: The study comprises 1,380 patients scheduled for thoracic surgery. Interventions: The individualized group will receive intraoperative recruitment maneuvers followed by individualized positive end-expiratory pressure (open lung approach) during the intraoperative period plus postoperative ventilatory support with high-flow nasal cannula, whereas the control group will be managed with conventional lung-protective ventilation. Measurements and Main Results: Individual and total number of postoperative complications, including atelectasis, pneumothorax, pleural effusion, pneumonia, acute lung injury; unplanned readmission and reintubation; length of stay and death in the critical care unit and in the hospital will be analyzed for both groups. The authors hypothesize that the intraoperative application of an open lung approach followed by an individual indication of high-flow nasal cannula in the postoperative period will reduce pulmonary complications and length of hospital stay in high-risk surgical patients.     • Genetic discoveries of Alzheimer’s disease are the drivers of our understanding, and together with polygenetic risk stratification can contribute towards planning of feasible and efficient preventive and curative clinical trials. We first perform a large genetic association study by merging all available case-control datasets and by-proxy study results (discovery n = 409,435 and validation size n = 58,190). Here, we add six variants associated with Alzheimer’s disease risk (near APP, CHRNE, PRKD3/NDUFAF7, PLCG2 and two exonic variants in the SHARPIN gene). Assessment of the polygenic risk score and stratifying by APOE reveal a 4 to 5.5 years difference in median age at onset of Alzheimer’s disease patients in APOE ɛ4 carriers. Because of this study, the underlying mechanisms of APP can be studied to refine the amyloid cascade and the polygenic risk score provides a tool to select individuals at high risk of Alzheimer’s disease. © 2021, The Author(s).

  • Blood Selenium Concentrations Are Inversely Associated with the Risk of Undernutrition in Older Adults

    • Background: Selenium is an essential trace element with an antioxidant and anti-inflammatory capacity that has been associated in experimental studies with beneficial effects on appetite control, the regulation of the gut microbiota, and control of the anabolic–catabolic balance. The main aim of the present study was to evaluate the association between circulating selenium concentrations and the risk of developing undernutrition in older adults. Methods: This was a cohort study with 1398 well-nourished community-dwelling individuals aged ≥ 65 years residing in Spain in 2017, who were followed for a mean of 2.3 years. Whole blood selenium was measured at baseline using inductively coupled plasma-mass spectrometry. Undernutrition was assessed at baseline and at follow-up, and defined as having at least one of the three GLIM phenotypic criteria (involuntary weight loss, a low body mass index, and a reduced muscle mass) and at least one of the two etiologic criteria (reduced food consumption or nutrient assimilation and inflammation/disease burden). Results: During the follow-up, 142 participants (11%) developed moderate undernutrition and 113 (8.8%) severe undernutrition. The standardized relative risks of moderate and severe undernutrition at the 75th percentile of Se levels versus the 25th were 0.90 and 0.70, respectively. In dose–response analyses, the risk of severe undernutrition decreased linearly with increasing selenium concentrations. This association was independent of protein intake or diet quality and was stronger among participants with a diagnosis of a musculoskeletal disorder. Conclusions: The results suggest that an adequate dietary selenium status is needed to prevent undernutrition in older adults. Also, this may open the door for clinical trials with selenium supplementation, at doses considered as safe, to prevent undernutrition.

    • WD 1145+017 is currently the only white dwarf known to exhibit periodic transits of planetary debris as well as absorption lines from circumstellar gas. We present the first simultaneous fast optical spectrophotometry and broad-band photometry of the system, obtained with the Gran Telescopio Canarias (GTC) and the Liverpool Telescope, respectively. The observations spanned 5.5 h, somewhat longer than the 4.5-h orbital period of the debris. Dividing the GTC spectrophotometry into five wavelength bands reveals no significant colour differences, confirming grey transits in the optical. We argue that absorption by an optically thick structure is a plausible alternative explanation for the achromatic nature of the transits that can allow the presence of small-sized (~µm) particles. The longest (87 min) and deepest (50 per cent attenuation) transit recorded in our data exhibits a complex structure around minimum light that can be well modelled by multiple overlapping dust clouds. The strongest circumstellar absorption line, Fe II λ5169, significantly weakens during this transit, with its equivalent width reducing from a mean out-of-transit value of 2 to 1 Å in-transit, supporting spatial correlation between the circumstellar gas and dust. Finally, we made use of the Gaia Data Release 2 and archival photometry to determine the white dwarf parameters. Adopting a helium-dominated atmosphere containing traces of hydrogen and metals, and a reddening E(B - V) = 0.01 we find T_eff=15 020 ± 520 K, log g = 8.07 ± 0.07, corresponding to M_WD=0.63± 0.05 M☉ and a cooling age of 224 ± 30 Myr.

    • Assessing emotional state is an emerging application field boosting research activities on the topic of analysis of non-invasive biosignals to find effective markers to accurately determine the emotional state in real-time. Nowadays using wearable sensors, electrocardiogram and thoracic impedance measurements can be recorded, facilitating analyzing cardiac and respiratory functions directly and autonomic nervous system function indirectly. Such analysis allows distinguishing between different emotional states: neutral, sadness, and disgust. This work was specifically focused on the proposal of a k-fold approach for selecting features while training the classifier that reduces the loss of generalization. The performance of the proposed algorithm used as the selection criterion was compared to the commonly used standard error function. The proposed k-fold approach outperforms the conventional method with 4% hit success rate improvement, reaching an accuracy near to 78%. Moreover, the proposed selection criterion method allows the classifier to produce the best performance using a lower number of features at lower computational cost. A reduced number of features reduces the risk of overfitting while a lower computational cost contributes to implementing real-time systems using wearable electronics.

    • Activity and emotion recognition based on physiological signal processing in health care applications is a relevant research field, with promising future and relevant applications, such as health at work or preventive care. This paper carries out a deep analysis of features proposed to extract information from the electrocardiogram, thoracic electrical bioimpedance, and electrodermal activity signals. The activities analyzed are: neutral, emotional, mental and physical. A total number of 533 features are tested for activity recognition, performing a comprehensive study taking into consideration the prediction accuracy, feature calculation, window length, and type of classifier. Feature selection to know the most relevant features from the complete set is implemented using a genetic algorithm, with a different number of features. This study has allowed us to determine the best number of features to obtain a good error probability avoiding over-fitting, and the best subset of features among those proposed in the literature. The lowest error probability that is obtained is 22.2%, with 40 features, a least squares error classifier, and 40 s window length.

  • Assessment of Mental, Emotional and Physical Stress through Analysis of Physiological Signals Using Smartphones

    • Determining the stress level of a subject in real time could be of special interest in certain professional activities to allow the monitoring of soldiers, pilots, emergency personnel and other professionals responsible for human lives. Assessment of current mental fitness for executing a task at hand might avoid unnecessary risks. To obtain this knowledge, two physiological measurements were recorded in this work using customized non-invasive wearable instrumentation that measures electrocardiogram (ECG) and thoracic electrical bioimpedance (TEB) signals. The relevant information from each measurement is extracted via evaluation of a reduced set of selected features. These features are primarily obtained from filtered and processed versions of the raw time measurements with calculations of certain statistical and descriptive parameters. Selection of the reduced set of features was performed using genetic algorithms, thus constraining the computational cost of the real-time implementation. Different classification approaches have been studied, but neural networks were chosen for this investigation because they represent a good tradeoff between the intelligence of the solution and computational complexity. Three different application scenarios were considered. In the first scenario, the proposed system is capable of distinguishing among different types of activity with a 21.2% probability error, for activities coded as neutral, emotional, mental and physical. In the second scenario, the proposed solution distinguishes among the three different emotional states of neutral, sadness and disgust, with a probability error of 4.8%. In the third scenario, the system is able to distinguish between low mental load and mental overload with a probability error of 32.3%. The computational cost was calculated, and the solution was implemented in commercially available Android-based smartphones. The results indicate that execution of such a monitoring solution is negligible compared to the nominal computational load of current smartphones.

    • Conducting an analysis of human behavior is an intriguing topic for many researchers. Within this field, machine learning can be applied to classify activities and emotions by analyzing physiological signals. However, the limited size of available databases poses challenges for the generalization of classifiers. This paper proposes a method to enhance the generalization of neural network-based classifiers by intelligently initializing weights for emotion and activity recognition. The signals under consideration are electrocardiogram, thoracic electrical bioimpedance, and electrodermal activity. The database used comprises recordings from 40 subjects performing various tasks that induce emotions and activities. The performance of the proposed method is compared with several standard machine learning and deep learning classifiers typically employed in emotion and activity recognition. This study involves two primary assessments. First is the activity recognition task, encompassing classes such as neutral, emotional, mental, and physical activity, where results close to 20% accuracy are achieved using the three physiological signals. Second, the emotion recognition assessment aims to differentiate between emotions like neutral, sadness, and disgust. An error probability close to 15% is obtained using thoracic electrical bioimpedance and electrodermal activity. The proposed method yields the best results among the approaches evaluated.

    • Background Eosinophilic granulomatosis with polyangiitis (EGPA) is often associated with glucocorticoid-dependent asthma and/or ear, nose and throat (ENT) manifestations. When immunosuppressants and/or mepolizumab are ineffective, dupilumab could be an option. We describe the safety and efficacy of off-label use of dupilumab in relapsing and/or refractory EGPA.Patients and methods We conducted an observational multicentre study of EGPA patients treated with dupilumab. Complete response was defined by Birmingham Vasculitis Activity Score (BVAS)=0 and prednisone dose ≤4 mg/day, and partial response by BVAS=0 and prednisone dose >4 mg/day. Eosinophilia was defined as an eosinophil count >500/mm3.Results Fifty-one patients were included. The primary indication for dupilumab was disabling ENT symptoms in 92%. After a median follow-up of 13.1 months, 18 patients (35%) reported adverse events (AEs), including two serious AEs. Eosinophilia was reported in 34 patients (67%), with a peak of 2195/mm3 (IQR 1268–4501) occurring at 13 weeks (IQR 4–36) and was associated with relapse in 41%. Twenty-one patients (41%) achieved a complete response and 12 (24%) a partial response. Sixteen (31%) patients experienced an EGPA relapse while on dupilumab, which was associated with blood eosinophilia in 14/16 (88%) patients. The median eosinophil count at the start of dupilumab was significantly lower in relapsers than in non-relapsers, as was the median time between stopping anti-IL-5/IL-5R and switching to dupilumab.Conclusion These results suggest that dupilumab may be effective in treating patients with EGPA-related ENT manifestations. However, EGPA flares occurred in one-third of patients and were preceded by eosinophilia in 88%, suggesting that caution is required.

    • This work has the objective to show development of a non-conventional subscale aircraft tests and compare a preliminary numerical analysis with wind tunnel tests. The subscale flight tests can be used as a low cost tool and that can improve the learning curve, considering the future aircrafts are characterized by the use of new technologies and radical configurations. For this validation was built the ITA-BWB aircraft, a single engine and tailless concept. This project demonstrates all steps of preliminary analysis with VLM software, preliminary flight tests and wind tunnel tests, where these data were used for simulations of specific maneuvers used in parameter identification. Finally, the simulations demonstrated consistency between the static derivatives acquired by VLMsoftware and wind tunnel tests, which were used to estimate dynamic derivatives that allowed the simulation of specific maneuvers for subscale flight tests.

    • Purpose While computational methods are prevalent in aircraft conceptual design, recent advances in mechatronics and manufacturing are lowering the cost of practical experiments. Focussing on a relatively simple property, the lift curve, this study aims to increase understanding of how basic aerodynamic characteristics of a complex stealth configuration can be estimated experimentally using low-cost equipment, rapid prototyping methods and remotely piloted aircraft. Design/methodology/approach Lift curve estimates are obtained from a wind tunnel test of a three-dimensional-printed, 3.8%-scale model of a generic fighter and from flight testing a 14%-scale demonstrator using both a simple and a more advanced identification technique based on neural networks. These results are compared to a computational fluid dynamics study, a panel method and a straightforward, theoretical approach based on radical geometry simplifications. Findings Besides a good agreement in the linear region, discrepancies at high angles of attack reveal the shortcomings of each method. The remotely piloted model manages to provide consistent results beyond the physical limitations of the wind tunnel although it seems limited by instrumentation capabilities and unmodelled thrust effects. Practical implications Physical models can, even though low-cost experiments, expand the capabilities of other aerodynamic tools and contribute to reducing uncertainty when other estimations diverge. Originality/value This study highlights the limitations of commonly used aerodynamic methods and shows how low-cost prototyping and testing can complement or validate other estimations in the early study of a complex configuration.

  • Impact of SPECT corrections on 3D-dosimetry for liver transarterial radioembolization using the patient relative calibration methodology

    • Purpose: Many centers aim to plan liver transarterial radioembolization (TARE) with dosimetry, even without CT-based attenuation correction (AC), or with unoptimized scatter correction (SC) methods. This work investigates the impact of presence vs absence of such corrections, and limited spatial resolution, on 3D dosimetry for TARE. Methods: Three voxelized phantoms were derived from CT images of real patients with different body sizes. Simulations of 99mTc-SPECT projections were performed with the SIMIND code, assuming three activity distributions in the liver: uniform, inside a "liver's segment," or distributing multiple uptaking nodules ("nonuniform liver"), with a tumoral liver/healthy parenchyma ratio of 5:1. Projection data were reconstructed by a commercial workstation, with OSEM protocol not specifically optimized for dosimetry (spatial resolution of 12.6 mm), with/without SC (optimized, or with parameters predefined by the manufacturer; dual energy window), and with/without AC. Activity in voxels was calculated by a relative calibration, assuming identical microspheres and 99mTc-SPECT counts spatial distribution. 3D dose distributions were calculated by convolution with 90Y voxel S-values, assuming permanent trapping of microspheres. Cumulative dose-volume histograms in lesions and healthy parenchyma from different reconstructions were compared with those obtained from the reference biodistribution (the "gold standard," GS), assessing differences for D95%, D70%, and D50% (i.e., minimum value of the absorbed dose to a percentage of the irradiated volume). γ tool analysis with tolerance of 3%/13 mm was used to evaluate the agreement between GS and simulated cases. The influence of deep-breathing was studied, blurring the reference biodistributions with a 3D anisotropic gaussian kernel, and performing the simulations once again. Results: Differences of the dosimetric indicators were noticeable in some cases, always negative for lesions and distributed around zero for parenchyma. Application of AC and SC reduced systematically the differences for lesions by 5%-14% for a liver segment, and by 7%-12% for a nonuniform liver. For parenchyma, the data trend was less clear, but the overall range of variability passed from -10%/40% for a liver segment, and -10%/20% for a nonuniform liver, to -13%/6% in both cases. Applying AC, SC with preset parameters gave similar results to optimized SC, as confirmed by γ tool analysis. Moreover, γ analysis confirmed that solely AC and SC are not sufficient to obtain accurate 3D dose distribution. With breathing, the accuracy worsened severely for all dosimetric indicators, above all for lesions: with AC and optimized SC, -38%/-13% in liver's segment, -61%/-40% in the nonuniform liver. For parenchyma, D50% resulted always less sensitive to breathing and sub-optimal correction methods (difference overall range: -7%/13%). Conclusions: Reconstruction protocol optimization, AC, SC, PVE and respiratory motion corrections should be implemented to obtain the best possible dosimetric accuracy. On the other side, thanks to the relative calibration, D50% inaccuracy for the healthy parenchyma from absence of AC was less than expected, while the optimization of SC was scarcely influent. The relative calibration therefore allows to perform TARE planning, basing on D50% for the healthy parenchyma, even without AC or with suboptimal corrections, rather than rely on nondosimetric methods.

    • Even though NMR has found countless applications in the field of small molecule characterization, there is no standard file format available for the NMR data relevant to structure characterization of small molecules. A new format is therefore introduced to associate the NMR parameters extracted from 1D and 2D spectra of organic compounds to the proposed chemical structure. These NMR parameters, which we shall call NMReDATA (for nuclear magnetic resonance extracted data), include chemical shift values, signal integrals, intensities, multiplicities, scalar coupling constants, lists of 2D correlations, relaxation times, and diffusion rates. The file format is an extension of the existing Structure Data Format, which is compatible with the commonly used MOL format. The association of an NMReDATA file with the raw and spectral data from which it originates constitutes an NMR record. This format is easily readable by humans and computers and provides a simple and efficient way for disseminating results of structural chemistry investigations, allowing automatic verification of published results, and for assisting the constitution of highly needed open-source structural databases.

    • Multiple sclerosis is a common disease of the central nervous system in which the interplay between inflammatory and neurodegenerative processes typically results in intermittent neurological disturbance followed by progressive accumulation of disability. Epidemiological studies have shown that genetic factors are primarily responsible for the substantially increased frequency of the disease seen in the relatives of affected individuals, and systematic attempts to identify linkage in multiplex families have confirmed that variation within the major histocompatibility complex (MHC) exerts the greatest individual effect on risk. Modestly powered genome-wide association studies (GWAS) have enabled more than 20 additional risk loci to be identified and have shown that multiple variants exerting modest individual effects have a key role in disease susceptibility. Most of the genetic architecture underlying susceptibility to the disease remains to be defined and is anticipated to require the analysis of sample sizes that are beyond the numbers currently available to individual research groups. In a collaborative GWAS involving 9,772 cases of European descent collected by 23 research groups working in 15 different countries, we have replicated almost all of the previously suggested associations and identified at least a further 29 novel susceptibility loci. Within the MHC we have refined the identity of the HLA-DRB1 risk alleles and confirmed that variation in the HLA-A gene underlies the independent protective effect attributable to the class I region. Immunologically relevant genes are significantly overrepresented among those mapping close to the identified loci and particularly implicate T-helper-cell differentiation in the pathogenesis of multiple sclerosis.

    • Several applications of Electrical Bioimpedance (EBI) make use of Cole parameters as base of their analysis, therefore Cole parameters estimation has become a very common practice within Multifrequency- and EBI spectroscopy. EBI measurements are very often contaminated with the influence of parasitic capacitances, which contributes to cause a hook-alike measurement artifact at high frequencies in the EBI obtained data. Such measurement artifacts might cause wrong estimations of the Cole parameters, contaminating the whole analysis process and leading to wrong conclusions. In this work, a new approach to estimate the Cole parameters from the real part of the admittance, i.e. the conductance, is presented and its performance is compared with the results produced with the traditional fitting of complex impedance to a depressed semi-circle. The obtained results prove that is feasible to obtain the full Cole equation from only the conductance data and also that the estimation process is safe from the influence capacitive leakage.

    • Advances in textile materials, technology and miniaturization of electronics for measurement instrumentation has boosted the development of wearable measurement systems. In several projects sensorized garments and non-invasive instrumentation have been integrated to assess on emotional, cognitive responses as well as physical arousal and status of mental stress through the study of the autonomous nervous system. Assessing the mental state of workers under stressful conditions is critical to identify which workers are in the proper state of mind and which are not ready to undertake a mission, which might consequently risk their own life and the lives of others. The project Assessment in Real Time of the Stress in Combatants (ATREC) aims to enable real time assessment of mental stress of the Spanish Armed Forces during military activities using a wearable measurement system containing sensorized garments and textile-enabled non-invasive instrumentation. This work describes the multiparametric sensorized garments and measurement instrumentation implemented in the first phase of the project required to evaluate physiological indicators and recording candidates that can be useful for detection of mental stress. For such purpose different sensorized garments have been constructed: a textrode chest-strap system with six repositionable textrodes, a sensorized glove and an upper-arm strap. The implemented textile-enabled instrumentation contains one skin galvanometer, two temperature sensors for skin and environmental temperature and an impedance pneumographer containing a 1-channel ECG amplifier to record cardiogenic biopotentials. With such combinations of garments and non-invasive measurement devices, a multiparametric wearable measurement system has been implemented able to record the following physiological parameters: heart and respiration rate, skin galvanic response, environmental and peripheral temperature. To ensure the proper functioning of the implemented garments and devices the full series of 12 sets have been functionally tested recording cardiogenic biopotential, thoracic impedance, galvanic skin response and temperature values. The experimental results indicate that the implemented wearable measurement systems operate according to the specifications and are ready to be used for mental stress experiments, which will be executed in the coming phases of the project with dozens of healthy volunteers.

    • The Spanish Ministry of Defense, through its Future Combatant program, has sought to develop technology aids with the aim of extending combatants' operational capabilities. Within this framework the ATREC project funded by the Coincidente program aims at analyzing diverse biometrics to assess by real time monitoring the stress levels of combatants. This project combines multidisciplinary disciplines and fields, including wearable instrumentation, textile technology, signal processing, pattern recognition and psychological analysis of the obtained information. In this work the ATREC project is described, including the different execution phases, the wearable biomedical measurement systems, the experimental setup, the biomedical signal analysis and speech processing performed. The preliminary results obtained from the data analysis collected during the first phase of the project are presented, indicating the good classification performance exhibited when using features obtained from electrocardiographic recordings and electrical bioimpedance measurements from the thorax. These results suggest that cardiac and respiration activity offer better biomarkers for assessment of stress than speech, galvanic skin response or skin temperature when recorded with wearable biomedical measurement systems.
