    • BACKGROUND: There is currently a lack of evidence-based guidelines regarding surveillance for recurrence after esophageal and gastric (OG) cancer surgical resection, and which symptoms should prompt endoscopic or radiological investigations for recurrence. The aim of this study was to develop a core symptom set using a modified Delphi consensus process that should guide clinicians to carry out investigations to look for suspected recurrent OG cancer in previously asymptomatic patients.METHODS: A web-based survey of 42 questions was sent to surgeons performing OG cancer resections at high volume centers. The first section evaluated the structure of follow-up and the second, determinants of follow-up. Two rounds of a modified Delphi consensus process and a further consensus workshop were used to determine symptoms warranting further investigations. Symptoms with a 75% consensus agreement as suggestive of recurrent cancer were included in the core symptom set.RESULTS: 27 surgeons completed the questionnaires. A total of 70.3% of centers reported standardized surveillance protocols, whereas 3.7% of surgeons did not undertake any surveillance in asymptomatic patients after OG cancer resection. In asymptomatic patients, 40.1% and 25.9% of centers performed routine imaging and endoscopy, respectively. The core set that reached consensus, consisted of eight symptoms that warranted further investigations included; dysphagia to solid food, dysphagia to liquids, vomiting, abdominal pain, chest pain, regurgitation of foods, unexpected weight loss and progressive hoarseness of voice.CONCLUSION: There is global variation in monitoring patients after OG cancer resection. Eight symptoms were identified by the consensus process as important in prompting radiological or endoscopic investigation for suspected recurrent malignancy. Further randomized controlled trials are necessary to link surveillance strategies to survival outcomes and evaluate prognostic value.

    • Highlights are presented from studies of the electric field data from various regions along the CLUSTER orbit. They all point towards a very high coherence for phenomena recorded on four spacecraft that are separated by a few hundred kilometers for structures over the whole range of apparent frequencies from I mHz to 9 kHz. This presents completely new opportunities to study spatial-temporal plasma phenomena from the magnetosphere out to the solar wind. A new probe environment was constructed for the CLUSTER electric field experiment that now produces data of unprecedented quality. Determination of plasma flow in the solar wind is an example of the capability of the instrument.

    • We use multi-point electric field observations from the four Cluster satellites to study the dynamical behavior of the high-latitude magnetosphere on February 13-14, 2001, 20-02 UT. At 20:00 UT the vehicles enter the cusp where three satellites observe a 500-volt potential drop. It implies that at lower altitudes there likely exist some parallel electric fields that accelerate electrons downward and ions upward. In the following 2-3 hours the satellites move over the southern polar cap where all four satellites pass through a number of stationary, large-scale density enhancements that are associated with 200-volt potential drops. The observed events are possibly ionospheric ion outflows, triggered by geomagnetic activity. At 23:20 UT, the satellites move in the distant plasma sheet, and an hour later they have a brief encounter with the auroral region where a density cavity of a few degrees wide is observed. At the equatorward edge of the cavity, large electric fields of 100 mV/m are observed, which are likely related to an auroral arc. Similar observations are collected from all four satellites within a few minutes, but a detailed comparison reveals plenty of differences, apparently due to small spatial and temporal scale sizes. Near the perigee pass, the vehicles traverse the plasma trough near local midnight, where they all detect a ULF wave event. A preliminary analysis of the event shows that it is a resonant mode of a 120-sec period. Surprisingly the observations from four satellites are not well correlated, which suggests a short spatial and temporal scale for the event. A possible source mechanism for ULF waves at this local time sector is drifting ring current protons.

    • Misuse and overuse of antibiotics in aquaculture has proven to be an unsustainable practice leading to increased bacterial resistance. An alternative strategy involves the inclusion of immunostimulants in fish diets, especially fungal and herbal compounds already authorized for human consumption, hence without environmental or public health concerns. In this study, we used a holistic and cross-disciplinary pipeline to assess the immunos-timulatory properties of two fungi: Trametes versicolor and Ganoderma lucidum; one herbal supplement, capsaicin in the form of Espelette pepper (Capsicum annuum), and a combination of these fungal and herbal additives on rainbow trout (Oncorhynchus mykiss). We investigated the impact of diet supplementation for 7 weeks on sur-vival, growth performance, cellular, humoral, and molecular immune parameters, as well as the intestinal mi-crobial composition of the fish. Uptake of herbal and fungal compounds influenced the expression of immune related genes, without generating an inflammatory response. Significant differences were detected in the spleen-tlr2 gene expression. Supplementation with herbal additives correlated with structural changes in the fish in-testinal microbiota and enhanced overall intestinal microbial diversity. Results demonstrated that the different treatments had no adverse effect on growth performance and survival, suggesting the safety of the different feed additives at the tested concentrations. While the mechanisms and multifactorial interactions remain unclear, this study provides insights not only in regard to nutrition and safety of these compounds, but also how a combined immune and gut microbiota approach can shed light on efficacy of immunostimulant compounds for potential commercial inclusion as feed supplements.

    • Mercury's environment is a complex system, resulting from the interaction between the solar wind, magnetosphere, exosphere and surface. A comprehensive description of its characteristics requires a detailed study of these four elements. This paper illustrates and discusses the key processes that are implicated in the strong coupling of the Hermean magnetosphere with the other elements. The magnetosphere of Mercury, frequently called mini- magnetosphere, when compared to that of Earth, plays a significant role in controlling the planet source and loss processes, by means of both particle and field interactions. We review the status of our knowledge, and give possible interpretations of the still-limited data set presently available.

    • The main objective of the Mutual Impedance Probe (MIP), part of the Rosetta Plasma Consortium (RPC), is to measure the electron density and temperature of Comet 67P/Churyumov-Gerasimenko's coma, in particular inside the contact surface. Furthermore, MIP will determine the bulk velocity of the ionised outflowing atmosphere, define the spectral distribution of natural plasma waves, and monitor dust and gas activities around the nucleus. The MIP instrumentation consists of an electronics board for signal processing in the 7 kHz to 3.5 MHz range and a sensor unit of two receiving and two transmitting electrodes mounted on a 1-m long bar. In addition, the Langmuir probe of the RPC/LAP instrument that is at about 4 m from the MIP sensor can be used as a transmitter (in place of the MIP ones) and MIP as a receiver in order to have access to the density and temperature of plasmas at higher Debye lengths than those for which the MIP is originally designed.
