    • gamma-Secretase inhibition represents a major therapeutic strategy for lowering amyloid beta (A beta) peptide production in Alzheimers disease (AD). Progress toward clinical use of gamma-secretase inhibitors has, however, been hampered due to mechanism-based adverse events, primarily related to impairment of Notch signaling. The gamma-secretase inhibitor MRK-560 represents an exception as it is largely tolerable in vivo despite displaying only a small selectivity between A beta production and Notch signaling in vitro. In exploring the molecular basis for the observed tolerability, we show that MRK-560 displays a strong preference for the presenilin 1(PS1) over PS2 subclass of gamma-secretases and is tolerable in wild-type mice but causes dose-dependent Notch-related side effect in PS2-deficient mice at drug exposure levels resulting in a substantial decrease in brain A beta levels. This demonstrates that PS2 plays an important role in mediating essential Notch signaling in several peripheral organs during pharmacological inhibition of PS1 and provide preclinical in vivo proof of concept for PS2-sparing inhibition as a novel, tolerable and efficacious gamma-secretase targeting strategy for AD.

    • The proof of the pudding: A proof-of-concept study using γ-secretase inhibitors as a model has shown that sulfonimidamides act as bioisosteres for sulfonamides. Detailed in vitro and in vivo profiling reveal that the sulfonimidamide motif imparts desirable properties such as decreased lipophilicity and plasma protein binding, accompanied by increased solubility. Our data support a wider use of this unique functional group in the design of new pharmacologically active agents.

    • Som vi visat har utvecklingen av metoder och strukturer för öppna prioriteringar i Sverige kommit långt. Många frågor återstår likväl. Under vårt arbete med denna bok har vi identifierat ett antal förbättringsområden och utmaningar som vi avslutningsvis vill lyfta fram. Det rör sig om vilka som ska delta i prioriteringarna, tydliggörande av värdegrunden, behov av bättre kunskap, baserad på både vetenskaplig metod och erfarenhet, och fortsatt utveckling av prioriteringsprocesser på olika nivåer och i olika sammanhang. Även om vi i Sverige skulle nå en god enighet kring principer och kriterier för prioriteringar så kommer vi alltid finna många olika sätt att praktiskt lösa specifika prioriteringsproblem.

  • Young Athletes Who Return to Sport Before 9 Months After Anterior Cruciate Ligament Reconstruction Have a Rate of New Injury 7 Times That of Those Who Delay Return

    • OBJECTIVE: To investigate the association between sustaining a second anterior cruciate ligament (ACL) injury and (1) time to return to sport, (2) symmetrical muscle function, and (3) symmetrical quadriceps strength at the time of return to sport in young athletes after primary ACL reconstruction. DESIGN: Prospective cohort study. METHODS: Patient demographics and results from 5 tests of muscle function (2 strength tests and 3 hop tests) were extracted from a rehabilitation registry. A questionnaire was sent to athletes (1530 years old) who were involved in knee-strenuous sport before the injury and had undergone primary ACL reconstruction to determine time of return to knee-strenuous sport (preinjury Tegner Activity Scale score of 6 or greater). We used the Cox proportional hazard regression model to analyze time to event. RESULTS: One hundred fifty-nine (32% of initial sample) athletes (mean +/- SD age, 21.5 +/- 4.4 years; 64% female) were included. Athletes with a higher preinjury Tegner Activity Scale score had a higher rate of second ACL injury (hazard ratio = 2.1; 95% confidence interval: 1.2, 3.6; P<.01). Athletes who returned to knee-strenuous sport before 9 months after reconstruction had a higher rate of second ACL injury (hazard ratio = 6.7; 95% confidence interval: 2.6,16.7; P<.001). There was no association between symmetrical muscle function or quadriceps strength and second ACL injury. CONCLUSION: Returning to knee-strenuous sport before 9 months after ACL reconstruction was associated with an approximately 7-fold increased rate of sustaining a second ACL injury. Achieving symmetrical muscle function or quadriceps strength was not associated with new ACL injury in young athletes.

    • Studiens övergripande syfte är att undersöka samt kritiskt granska digitaliseringens utmaningar och möjligheter vid simulatorstödd undervisning i gymnasial yrkesutbildning. I yrkesutbildning inom naturbruk ska elever utveckla kunskap i att använda de olika typer av maskiner eller fordon som förekommer inom relaterade yrken (t ex traktor, skördetröska, skotare). Användning av körsimulatorer i undervisningen förväntas öka elevers förutsättningar för kunskapsutveckling till exempel genom ökad tillgång till körträning, större möjligheter till individualiserad undervisning, fler tillfällen och möjligheter att pröva färdigheter i enskilda moment. Användningen av körsimulatorer i yrkesutbildning motiveras också av miljöskäl och ekonomiska skäl. De körsimulatorer som utvecklas idag har en hög grad av likhet med autentiska maskiner vilket ytterligare motiverar användning i undervisning. Tillgänglighet och användning av digital teknik och acceptans för tekniken räcker dock inte för att förbättra undervisningen (Säljö, 2010). En central fråga är vilka nya möjligheter ett digitalt verktyg, i detta fall körsimulator erbjuder och vilken betydelse dessa möjligheter kan ha för elevens lärande och yrkesrelaterade kunskaper. En annan fråga är då vilken betydelse simulatormiljön får för lärarens planering och genomförande av undervisning. I de tre skolor där studien genomförs sker en större satsning på implementering av körsimulatorer, vilket då direkt påverkar såväl utbildningsmiljö som undervisningspraktik. Satsningen motiverar ett lokalt utvecklingsarbete där lärare systematiskt och i samarbete med forskare prövar och granskar simulatorstödd undervisning. Studien genomförs som aktionsforskning (Rönnerman, 2008; Söderström, 2012). Målet är att bidra till kunskapsutveckling om simulatorstödd undervisning i yrkesutbildning, men också till den lokala kontextens egen kvalitetssäkring av simulatorstödd undervisning. Studien består av fyra undervisningsnära projekt som behandlar simulatorövningar som komplement till autentisk körning, simulatorövningar och ämnesintegrering samt simulatorövningar för bedömning. Lärares erfarenheter och frågor är utgångspunkt för designen av aktionsforskningen. Studien genomförs med utgångspunkt i yrkeslärares erfarenheter och frågor samt aktioner i den egna undervisningen. Lärare prövar metoder och modeller i simulatorstödd undervisning samt analyserar implikationer för planering, genomförande och bedömning i samarbete med forskare. Data för analys har skapats genom observationer, intervjuer och etnografisk dokumentation. Genomförande och analys av de fyra projekten pågår under våren 2018. Presentationen kommer att beskriva respektive projekts syfte, genomförande och resultat, en övergripande analys och slutsats samt öppna för en diskussion om digitala verktyg för att utveckla relevant yrkeskunnande i framtidens yrkesutbildning

  • Vocational teacher students’ critical reflections in site-based education

    • Critical reflection is an essential element in the professional teacher’s practice. This article investigates vocational teacher education (VTE) students´ ability to reflect when engaged in site-based education and considers how arrangements in vocational teacher education enable or constrain this ability. The study was conducted at one Swedish university, where 78 VTE students’ written self-evaluation reports from three practicum courses were analyzed on the basis of the concept of critical reflection and the theory of practice architectures. The findings emphasize the value of a strong relationship between the learning at university and in the teaching workplace, and the progression of critical reflection throughout the education. These findings support the conclusion that practice architectures that improve VTE students’ ability to critical reflection need to include tasks that focuse on both reflection and on bridging the contexts of research-based and experience-based knowledge and practice.

    • ABSTRACT The purpose of this article is to describe the emerging science of improvement in health care and to add a perspective from the industrial quality improvement movement, the use of data from quality registers, and to give some personal reflections and suggestions. Furthermore, we want to introduce to the broader quality management community what is happening in healthcare with respect to quality improvement. We will discuss some of the challenges of the health care system and the current status of a science ofimprovement and give some suggestions for further improvements to the area.We discuss a possible extension of improvement knowledge and thetheoretical and practical arsenal of a science of improvement, in particular, understanding variation and implications for the use of, for, example controlcharts. In addition, the normative side of a science of improvement is discussed. The article ends with some brief reflections of use for future research agendas.

    • The authors agree that operations management is a discipline of great importance to a science of improvement in health care. They also believe that action research (AR) aiming for sustainable system changes and also providing a more beneficial context for process improvements is an important and fruitful way forward. AR is an approach to inquiry where the researcher(s) engage in collaborative communities with practitioners on an equal grounding. The role of the patient as a co-producer illuminates the importance of person-centered care, where patients and health care professionals collaborate on stated care plans. When the patient is included in the care plan as an equal partner there can be room for increased self-management and empowered patients contributing to safe care. In conclusion, the authors agree with Does and colleagues about the importance of operations management and its different practices.

    • Under sommaren 2020 vandrar Vänermuseets utställning Min plats i biosfären runt i biosfärområdet Vänerskärgården med Kinnekulle. Utställningen kommer stå på tre omtyckta platser och lyfta forskning om vilka favoritplatser människan har och vad de platserna betyder för människan. Hur påverkar dessa platser människan och hur kan människan påverka dem? Ett forskarlag vid Göteborgs universitet och Högskolan i Gävle har undersökt vilka attityder och tankar boende i biosfärområdet har kring sina favoritplatser, kulturmiljö och hållbar utveckling. Forskningen har finansierats av Riksantikvarieämbetet och spridningen av forskningsresultaten finansieras av Formas. En favoritplats blir viktig att bevara och utställningen lyfter fram hur biosfärområdets kommuner jobbar med detta. Utställningens besökare kommer inte bara kunna ta del av vilka platser andra gillar, de kommer själva kunna dela med sig om vilka platser de tycker är viktiga att skydda och ha kvar. Utställningen Min plats i biosfären kommer visas vid Stenhusbacken i Fröslunda i Lidköpings kommun under juni, vid Bäcken vid Ekudden i Mariestad under juli och avslutningsvis i Stora stenbrottet på Kinnekille i Götene kommun under augusti månad. I utställningen får besökarna även möjlighet att se alla platser under olika årstider genom filmklipp, ta del av biosfärområdet och dela med sig om hur de tycker det ska förvaltas och utvecklas på ett hållbart sätt!

  • High frequency of contact allergy to gold in patients with endovascular coronary stents

    • Background Stent implantation is an effective method for treatment of atherosclerotic disease. Factors predisposing to in-stent restenosis are still largely unknown. Contact allergy to metal ions eluted from the stent has been suggested to be a risk factor. Objectives To explore whether there is a possible induction of contact allergy to metals used in stents among patients with a stainless steel stent containing nickel (Ni stent) and patients with a gold-plated stent (Au stent). Methods Adults (n = 484) treated with coronary stent implantation participated in the study with patch testing. The study design was retrospective and cross-sectional with no assessment of contact allergy before stenting. Age- and sex-matched patch-tested patients with dermatitis (n = 447) served as controls. Results Of Au-stented patients, 54 of 146 (37%) were allergic to gold compared with 85 of 447 (19%) controls (P < 0.001). Within the stented population there were no statistically significant differences in contact allergy to gold or nickel between Ni-stented and Au-stented patients. In multivariate models where other risk factors for contact allergy to gold were considered, the Au stent showed a trend towards statistical significance (odds ratio 1.43, 95% confidence interval 0.95-2.16; P = 0.09). Conclusions As the frequency of contact allergy to gold is higher in stented patients independent of stent type it suggests a previous sensitization. However, several pieces of circumstantial evidence as well as statistical analysis indicate the possibility of sensitization in the coronary vessel by the Au stent. Ni stents and Au stents should not be ruled out as risk factors for induction of contact allergy to these metals.

  • Cultural Values of Landscapes in the Practical Work of Biosphere Reserves

    • The purpose of this paper is to investigate the role of the landscape’s cultural values in the practical work of biosphere reserves and to identify what opportunities there are to increase awareness and knowledge about these values. The paper draws upon data collected in a Swedish biosphere reserve, including a survey of residents, interviews with public officials involved in cultural heritage management, and an analysis of documents produced by the Biosphere Reserve Association. Residents showed a broad knowledge about the landscape’s cultural values, and they linked immaterial heritage to material objects. The residents’ strong identity and pride in relation to the landscape were confirmed by the officials, who argued that it is the deep layers of history and the cultural diversity of the landscape that make the biosphere reserve attractive. However, concepts related to the landscape’s cultural values were barely touched upon in the documents analysed; the landscape’s cultural values were presented as a background—as an abstract value. The findings reveal several unexplored opportunities and practical implications to increase awareness and knowledge of the landscape’s cultural values. Suggested actions include definition of goals, articulation and use of concepts, inventories of actors, increased collaboration, and use of residents’ knowledge. Cultural values of landscapes are often neglected in the practical work of biosphere reserves, despite the social and cultural dimensions of sustainable development being an important component of UNESCO’s Man and the Biosphere (MAB) Programme. This research indicates several ways of bridging this gap between theory and practice

  • The Need to Articulate Historic and Cultural Dimensions of Landscapes in Sustainable Environmental Planning - A Swedish Case Study

    • Ignoring the historic and cultural dimensions of landscapes makes environmental planning unsustainable, which in the long run, will have a negative impact on both the environment and society. This paper examines the work and perceptions of practitioners with a focus on the role of historic and cultural landscape dimensions and their relation to the recent implementation of the ecosystem service framework in sustainable environmental planning. Semi-structured interviews with officials at local and regional planning levels in a Swedish case study showed that the historical landscape forms the basis for environmental work. Respondents expressed an integrated view of the landscape, and historic and cultural landscape dimensions were considered important in the initial planning process. However, several challenges existed later in the planning process and final decision-making, such as conceptual ambiguities, unclear policy and assignments, limited cross-sectorial coordination and lack of awareness, knowledge, resources and other priorities. The results also show that the respondents worked regularly with intangible landscape dimensions, which can be defined as cultural ecosystem services, but they do not label them as such. Furthermore, established knowledge and expertise of heritage planning was not activated in the implementation of the ecosystem service approach. We conclude that historic and cultural landscape dimensions are not ignored in practice, but there is a need to articulate these aspects more clearly in order to achieve sustainable environmental planning. There is also an unexplored opportunity to connect skills and create new forms of cross-sectorial collaboration between heritage planning and the ES approach.

    • The degree project represents an important part of higher education since it will examine the student´s knowledge in relation to the degree goals and the student´s ability to work independently under supervision. The supervision that is requested and given constitutes a central part of the student´s working process and depending on the supervision´s character it can also impact on the student´s possibilities to independence. Using as a point of departure the intention to make visible and to problematize our students’ ability to be independent, the aim of the study presented here is to identify the specific features of the written supervision which we as supervisors give to students in the form of comments. The attempt is auto-ethnographic, which means that we reflect over our own way to supervise. As support for the analysis parts of Bernstein´s theory have been used. The article describes and problematizes how we as supervisors have used comments in form of demands, exhortations and questions, the motives for the different kinds of comments and how different comments impact on the student´s possibility to use and show his/her ability to work independently. The article contributes with knowledge about what is happening during the supervision, it can be used as foundation for discussion about supervision in relation to the student´s independence and gives an example on self-reflection over one´s supervision.

    • Background: Patients’ complaints have been highlighted as important for constructively improving healthcare services. In so doing, it may be important to identify disparities in experiences based on patients’ demographics, such as sex. Aim: To explore hospital recorded complaints addressing potential sex differences and whether complaints were reported by the patient or a relative. Methods: Quantitative study of all 835 closed patient complaints during 2013 at three mid-sized hospitals in Sweden. The complaints were categorisation based on perceived quality theory and analysed using a probabilistic graphical model. The findings were validated through qualitative interviews. Findings: Female patients were more likely than male patients to report dissatisfaction with interpersonal issues, whereas male patients were more likely to report dissatisfaction with administration. If a complaint from a male patient had been reported by a relative, the matter was more likely to be interpersonal. Improvement suggestions were predominantly reported by staff. However, patients and relatives proved more likely than staff to report improvement suggestions when dissatisfied with interpersonal matters. Conclusion: Using a Bayesian network, this article suggests that complaints in health care should be more holistically understood and the factors should be viewed as interconnected.     • Non-invasive prenatal testing (NIPT) was recently introduced for prenatal testing of genetic disorders. Cell-free fetal DNA is present in maternal blood during pregnancy and enables detection of fetal chromosome aberrations in a maternal blood sample. The public perspective to this new, simple method has not been illuminated. The views of young people (i.e. future parents) are important to develop suitable counseling strategies regarding prenatal testing. The aim was to explore Swedish high school students' attitudes, knowledge and preferences regarding NIPT. A questionnaire was completed by 305 students recruited from one high school in Stockholm, November and December 2014. Most students (80 %) considered prenatal testing as good. The majority (65 %) was positive or very positive towards NIPT and 62 % stated that they potentially would like to undergo the test if they or their partner was pregnant. The vast majority (94 %) requested further information about NIPT. Most students (61 %) preferred verbal information, whereas 20 % preferred information via the Internet. The majority of the high school students was positive towards prenatal testing and most was positive towards NIPT. Further, information was requested by the vast majority before making a decision about NIPT. Most of the students preferred verbal information and to a lesser extent information via the Internet. The attitudes, knowledge and preferences for risk information concerning NIPT in young adults are important, in order to increase knowledge on how to educate and inform future parents.

    • This improvement project took place at Skaraborgs hospital group (SkaS) in the Western Region of Sweden. The hospital group serves about 250 000 citizens and has 4 500 employees. The improvement initiative could be regarded as a hospital-wide project involving healthcare professionals from different areas of the hospital group. The project team consisted of two team managers/black belts, physicians, nurses, one hospital statistician/master black belt, and an external researcher. ProblemAccording to a recent national report, around 8.6 % of patients in Sweden suffer from care-related injuries each year. One-fourth of the injuries are caused by medication errors. According to a study at the Karolinska University Hospital, one-third of all hospitalizations can be classified as drug-related problems. The medication process has thus captured the attention from a patient safety perspective due to its poor design, evidently causing a lot of adverse events.Assessments of problems and analysis of its causes Various types of data on needs related to the medication process were collected at SkaS. The main emphasis was placed on the analysis of the present way of working, reported errors and compliance to regulations/legislation. A detailed flow chart was used to map the identified problems. A list of 58 identified problems was created. As a result, the following suggestions for improvement were proposed and implemented:•Process organisation has been established for the medication process•Hospital general practice for the medication process has been created•Computerized order-entry system generating one digital medication list per patient at the hospital has been instigated•5S in the medication storage has been carried out.•“Do not disturb” signs for the rooms where medication is prepared have been printed•A Global Trigger Tool study/analysis has been initiated•Automated medication management system launched InterventionThis was a hospital-wide project supported from the top management team.. The intervention was performed using the Design for Six Sigma method (DFSS). The project followed the Define-Measure-Analyse-Design-Verify (DMADV) cycle, which is the most common project model in DFSS.Study designThis is a case study. The empirical material consists of field notes, interviews with project participants, meeting notes, diaries etc. The analysis was based on the construction of an event sequence, linking activities, tools and experiences together.Strategy for changeUpon the completion of the project a process organisation for the medication process has been established. The medication process at SkaS is considered to be an important supporting process to the hospital group’s main patient processes.Measurements of improvementThe project organization has considered several ways of measuring the implemented changes. One of them was to follow up the admissions caused by an adverse drug event at the medical emergency care unit.. As of today, there has been a reduction of such cases from initially approximately 13,6% to 8,1%. Effects of changesOne of the effects of this project was the establishment of a permanent process organization including a process owner, a process leader and a process improvement team, the latter being in charge of the continuous improvement of the medication process. Lessons learnt One of the lessons we have learnt was that it is important to delimit the project early on in the process. The other lesson was that the customer needs in a medication process are not explicitly expressed and that a thorough search is needed to identify them through multiple sources such as adverse event databases, electronic medical record system, medication reports, etc.Message for othersStructured approach with related tools support innovative ideas for a new process. Moreover, the adaption of DFSS tools to healthcare context are needed. Finally, there is need of mature organizations to support and care for the results of DFSS projects in healthcare.

    • Objectives This study examined the relationship between professionals' perceptions of a strengthened role for the patient and of patient involvement in quality improvement (QI) and whether professionals' experiences in improvement science were a moderator on such a relationship. Design From a predominantly close-ended, 44-item questionnaire, 4 questions specifically concerning professionals′ perception on patient involvement in QI were analysed. Setting Three Swedish regions. Participants 155 healthcare professionals who had previously participated in courses in improvement science. Results The covariate patient involvement was significantly related to a perceived strengthened patient role. There was also a significant interaction effect between degree of patient involvement and professionals' experience in the area of improvement science on a strengthened patient role. The result shows that there is a relationship between the perceived level of patient involvement in improvements and professionals' perceptions of a strengthened patient role. In this study, the covariate, perceived patient involvement, was significantly related to experiences of more equal relationships between patients and healthcare professionals. There was also a significant interaction effect between the degree of patient involvement and professionals' experience in the area of improvement science, for a more equal relationship between patients and healthcare professionals. Conclusion Increased patient involvement in QI is a means of strengthening the patient role and supporting a more equal relation between patients and healthcare professionals. Furthermore, empirical evidence shows that the healthcare professionals' experiences in the area of improvement science support a strengthened patient role and a more equal power relationship, but for this to happen, the mindset of professionals is key. Future research is needed to capture and investigate the experiences from patients and relatives about being involved in QI in healthcare, and to study the effects on quality in care processes.

    • Objectives Involving patients in quality improvement is often suggested as a critical step for improving healthcare processes. However, this comes with challenges related to resources, tokenism, validity and competence. Therefore, to optimise the use of available resources, there is a need to understand at what stage in the improvement cycle patient involvement is most beneficial. Thus, the purpose of this study was to identify the phase of an improvement cycle in which patient involvement had the highest impact on radicality of improvement. Design An exploratory cross-sectional survey was used. Setting and methods A questionnaire was completed by 155 Swedish healthcare professionals (response rate 34%) who had trained and had experience in patient involvement in quality improvement. Based on their replies, the impact of patient involvement on radicality in various phases of the improvement cycle was modelled using the partial least squares method. Results Patient involvement in quality improvement might help to identify and realise innovative solutions; however, there is variation in the impact of patient involvement on perceived radicality depending on the phase in which patients become involved. The highest impact on radicality was observed in the phases of capture experiences and taking action, while a moderate impact was observed in the evaluate phase. The lowest impact was observed in the identify and prioritise phase. Conclusions Involving patients in improvement projects can enhance the quality of care and help to identify radically new ways of delivering care. This study shows that it is possible to suggest at what point in an improvement cycle patient involvement has the highest impact, which will enable more efficient use of the resources available for patient involvement.

The study concluded that DFSS was beneficial in designing the medication process in the sense that; it generated concrete operational improvements, but also led to the establishment of a process organisation that was responsible for continuous development of the process. A further benefit was a greater understanding of the complexity of several customer groups with variation between and within groups. However, the challenges in applying DFSS in a healthcare setting included assessing implicit patient needs and the need for organisational maturity in order to support the DFSS project and handle project results.

    • BACKGROUND: Globally, there is a growing use of Information and Communication Technology (ICT), including mobile phones, tablets and computers, which are being integrated into people's daily activities. An ICT-based intervention called F@ce was developed in order to provide a structure for the process in stroke rehabilitation and facilitate change by integrating a global problem-solving strategy using SMS alerts. The aim of the study was to evaluate the feasibility of i) F@ce within in-patient and primary care rehabilitation after stroke, ii) the study design and outcome measures used, and iii) the fidelity, adherence and acceptability of the intervention.METHODS: Three teams comprising occupational therapists and physiotherapists who work in neurological rehabilitation participated in a preparatory workshop on F@ce and then enrolled 10 persons with stroke to participate in the intervention. Goals were set using the Canadian Occupational Performance Measure (COPM) and the participants with stroke rated their performance and satisfaction with the activities associated with the three goals every day for 8 weeks. Data were collected at inclusion, at four and 8 weeks, using the COPM, Stroke Impact Scale, Frenchay Activities Index, Life Satisfaction Checklist, Self-Efficacy Scale, Hospital Anxiety and Depression Scale, Fatigue Severity Scale, follow-up survey, daily ratings on the web platform and logbooks.RESULTS: All of the participants showed increased scores in the primary outcome (COPM) and a clinically meaningful improvement of ≥2 points was found in four participants regarding performance and in six participants regarding satisfaction. Overall fidelity to the components of F@ce was good. The response rates to the F@ce web platform were 44-100% (mean 78%). All of the participants stated that F@ce had supported their rehabilitation.CONCLUSION: The results indicate that the most beneficial part of F@ce was the person-centred, goal-setting process and SMS alerts. All participants were satisfied with F@ce and highlighted the benefits of receiving daily alerts about their goals. This encouraged them to be more active. The only downside mentioned was that they felt under an obligation to practice, although this was described as "a positive obligation".

They are e.g. free to decide how to organise their studies. The aim of the study presented here is to discuss how students answer on a survey concerning education for sustainability. In the first and fourth semester the ECTE students at Kristianstad University are particularly directed to learning about sustainability and education for sustainability. Critical aspects of sustainability and education for sustainability are pointed out by the teachers and by the textbooks. The survey will give answer to questions about what aspects of sustainability the students had discerned in the fifth semester. The first results of the survey indicates that the students’ experience is that the ECTE had developed their knowledge about sustainability although we can see there still are aspects that are not yet discerned. This indicates the importance of problematizing sustainability from a didactic perspective in relation to ECTE.

    • BackgroundPhysical therapy-led orthopedic triage is a care model used to optimize pathways for patients with hip or knee osteoarthritis. However, scientific evidence of the effectiveness of this model of care is still limited and only a few studies report patients' perception of it. The aim of this study was to compare patients' perceived quality of care after physical therapy-led triage with standard practice in a secondary care setting for patients with primary hip or knee osteoarthritis.MethodsIn this randomized study, patients with hip or knee osteoarthritis referred for an orthopedic consultation received either physical therapy-led triage (n = 344) or a standard care assessment by an orthopedic surgeon (n = 294). To evaluate the patients' perceived quality of care, a short version of the Quality from the Patient's Perspective (QPP) questionnaire was sent to the patients within a week after their assessment. The primary outcome was the statement "I received the best examination and treatment" on QPP.ResultsA total of 348 patients (70%, physical therapy-led triage: n = 249, standard care: n = 199) answered the questionnaire. No significant difference was found in the primary outcome between the groups (p = 0.6). Participants in the triage group perceived themselves to have received significantly better information about how to take care of their osteoarthritis (p = 0.017) compared with the standard care group. The standard care group reported that they participated in the decision-making process to a greater extent (p = 0.005), that their expectations were met to a greater degree (p = 0.013), and that their care depended more on their need for care rather than the caregivers' routines (0.007).ConclusionBoth groups report high perceived quality of care. Significant differences were found in four of 14 questions, one in favor of the physical therapist and three in favor of the standard care group. The findings of this study are in line with previous research and support the use of this care model for patients with hip or knee OA in secondary care. However, due to the dropout size, the results should be interpreted with caution.

    • Detta projekt har genomförts med syftet att få en uppfattning om vilka pannor i storleksintervallet 50 kW – 1 MW på den europeiska marknaden som kan bedömas ha förutsättningar för eldning av rörflen i briketterad eller riven form. Förbränningstekniskt har rörflen besvärligare egenskaper än träbränslen, framför allt i form av hög askhalt, 2 – 9 %. Avsevärt större askmängder per tidsenhet måste därmed föras ut ur pannan än för träbränslen. Askan har också en annan struktur är träaska, är voluminös och bygger lätt upp strukturer som håller samman och inte självmant faller ned i t.ex. askskruvar etc. De praktiska erfarenheterna i Sverige från rörflenseldning i små anläggningar är begränsade till endast ett par panntyper och med mindre goda erfarenheter. Totalt har ett tjugotal pannor och brännare studerats översiktligt mot bakgrund av en preliminär kravspecifikation. Därefter har elva produkter identifierats som intressanta för vidare undersökning. Dessa har delats in i de tre kategorierna 1-3 enligt följande: 1. Bedöms fungera med rörflen och inga behov föreligger kring djupare tester och utvärderingar med rörflen. 2. Bedöms ha potential att fungera med rörflen men det finns behov att djupare utvärdera och långtidstesta tekniken för att kontrollera dess kapacitet med rörflen (askhalt 2-9 %). 3. Bedöms ej fungera tillfredsställande med rörflen med varierande askhalt (2-9 %) utan anpassning/ombyggnad. Bedömningen ledde till följande resultat för pannor: Kategori 1: Ökotherm Compact Kategori 2: Reka HKRST, Linka, Schmid UTSW Kategori 2(3): ETA Hack, Fröling, Heizomat RHKAK, Biokompakt Kategori 3: Catfire För brännare erhölls följande bedömning: Kategori 2: Veto Flisomat, Ariterm Multijet Ovan redovisade resultat utgör inte en absolut sanning utan är resultatet av en kvalificerad bedömning på basis av tillgängligt underlag.

    • The Swedish school system offers curriculum-based early childhood education (ECE) organised as preschool (for 0–5-year-olds) and preschool class (for 6-year-olds). The intention to create a playful and educational environment based on children’s perspectives, interests, and questions is strongly based on historical and cultural traditions. This article develops knowledge of ECE teachers’ approaches to science-learning situations. The study applies a phenomenographic approach. The analysis is based on approximately 9.5 hours of video documentation of teacher-led and child-initiated Swedish ECE science activities. We identified two descriptive categories and four subcategories dealing with science-learning situations: (A) making anything visible, containing the three subcategories (Aa) addressing everyone, (Ab) addressing everything, and (Ac) addressing play and fantasy; and (B) creating a shared space for learning (Ba) addressing common content. These categories are related to how efforts to take advantage of children’s perspectives are interpreted and addressed in educational practice.     • The Swedish school system offers curriculum-based early childhood education (ECE) organised as preschool (for 0–5-year-olds) and preschool class (for 6-year-olds). The intention to create a playful and educational environment based on children’s perspectives, interests, and questions is strongly based on historical and cultural traditions. This article develops knowledge of ECE teachers’ approaches to science-learning situations. The study applies a phenomenographic approach. The analysis is based on approximately 9.5 hours of video documentation of teacher-led and child-initiated Swedish ECE science activities. We identified two descriptive categories and four subcategories dealing with science-learning situations: (A) making anything visible, containing the three subcategories (Aa) addressing everyone, (Ab) addressing everything, and (Ac) addressing play and fantasy; and (B) creating a shared space for learning (Ba) addressing common content. These categories are related to how efforts to take advantage of children’s perspectives are interpreted and addressed in educational practice. The article discusses and exemplifies the use of various categories and their potential implications for ECE learning practice.

    • Purpose: The purpose of this study was to identify how people 6–12 months after stroke were using and integrating information and communication technology (ICT) in their everyday lives.Method: To capture the participants’ experiences, one focus group and 14 individual interviews were carried out in Sweden and Denmark regarding the use of ICT in everyday life. The participants comprised 11 men and seven women aged 41–79 years. A grounded theory approach was used throughout the study and a constant comparative method was used in the analysis.Results: Five categories were identified from the analysis of the interviews with the participants: 1) Using the mobile phone to feel safe, 2) Staying connected with others, 3) Recreating everyday life, 4) A tool for managing everyday life, and 5) Overcoming obstacles for using ICT. From these categories one core category emerged: The drive to integrate ICT in everyday life after stroke.Conclusions: People with stroke had a strong drive to integrate ICT in order to manage and bring meaning to their everyday lives, although sometimes they needed support and adaptations. It is not only possible but also necessary to start using ICT in rehabilitation in order to support people’s recovery and promote participation in everyday life after stroke.Implications for rehabilitation People with stroke have a strong drive for using information and communication technology in their everyday lives, although support and adaptations are needed.The recovery process of people with stroke could benefit from the use of ICT in the rehabilitation and ICT could possibly contribute to independence and promote participation in everyday life.Knowledge from this study can be used in the development of an ICT-based stroke rehabilitation model.

    • Background: Using Information and Communication Technology (ICT) could increase the intensity of rehabilitation, the level of patient activity and participation in everyday life after stroke and is in line with having a person-centred approach.Aim: To explore how healthcare professionals use and could potentially use ICT to enable a person-centred rehabilitation process after stroke.Methods: Six individual and two focus group interviews were conducted with a group of healthcare professionals working within rehabilitation after stroke. A grounded theory approach was used to collect and analyze the data.Results: The professionals described their current use of ICT as well as their vision of how ICT could be incorporated as a tool in rehabilitation for sharing in four categories: (1) Sharing of information, (2) Collaborating from a distance, (3) Having transparency in the documentation and (4) Supporting patients’ use of ICT.Conclusion: Professionals state that using ICT solutions in rehabilitation after stroke could increase sharing between professionals and their patients and increase patient participation in the rehabilitation process.Significance: This study highlights the importance of developing ICT that healthcare professionals could use along with a person-centred approach. The results will be used to develop an ICT-supported multidisciplinary intervention for rehabilitation after stroke.

  • Factors affecting outcome in participation one year after stroke : a secondary analysis of a randomized controlled trial

    • Objective:To explore the importance of client characteristics (age, sex, stroke severity and participation before stroke), rehabilitation context (in-patient or client's home) and approach (enhanced client-centeredness or not) on participation in everyday life after stroke.Methods:A secondary analysis of data on 237 participants from a previous randomized controlled trial evaluating an enhanced client-centred intervention after stroke. Plausible associations between client characteristics, context and rehabilitation approach, and a positive outcome regarding participation were explored. Three different outcome measures for participation were used: Stroke Impact Scale 3.0, domain participation; Frenchay Activities Index; and Occupational Gaps Questionnaire.Results:For all participants there was a significant association between mild stroke and a positive outcome using the Frenchay Activities Index. Among participants who had not received the enhanced client-centred approach, there was a significant association between mild stroke and a positive outcome using the Stroke Impact Scale. The context of rehabilitation, i.e. receiving home rehabilitation, was also associated with a positive outcome in the Frenchay Activities Index for the control group.Conclusion:In order to increase participation in everyday life for people after stroke it appears to be important to use an enhanced client-centred approach, particularly when working within in-patient rehabilitation and with people with moderate or severe stroke.

  • Organic Matter Chlorination Rates in Different Boreal Soils: The Role of Soil Organic Matter Content

    • Transformation of chloride (Cl-) to organic chlorine (Cl-org) occurs naturally in soil but it is poorly understood how and why transformation rates vary among environments. There are still few measurements of chlorination rates in soils, even though formation of Cl-org has been known for two decades. In the present study, we compare organic matter (OM) chlorination rates, measured by Cl-36 tracer experiments, in soils from eleven different locations (coniferous forest soils, pasture soils and agricultural soils) and discuss how various environmental factors effect chlorination. Chlorination rates were highest in the forest soils and strong correlations were seen with environmental variables such as soil OM content and Cl- concentration. Data presented support the hypothesis that OM levels give the framework for the soil chlorine cycling and that chlorination in more organic soils over time leads to a larger Cl-org pool and in turn to a high internal supply of Cl- upon dechlorination. This provides unexpected indications that pore water Cl- levels may be controlled by supply from dechlorination processes and can explain why soil Cl- locally can be more closely related to soil OM content and the amount organically bound chlorine than to Cl- deposition.
