  • Distribution and evaluation of sense of coherence among older immigrants before and after a health promotion intervention - results from the RCT study promoting aging migrants' capability

    • Background: The migration process can be a threat to a person's sense of coherence (SOC) and to their ability to experience life as comprehensible, manageable, and meaningful. Seen from a salutogenic perspective, this may have a negative impact on the experience of health. Purpose: We describe the distribution of SOC and its components among older persons with an immigrant background now aging in Sweden. In addition, we evaluated whether a group-based health promotion program with a person-centered approach could support the SOC among older persons in this group. Materials and methods: A randomized controlled trial with postintervention follow-ups at 6 and 12 months was conducted with 131 independently living persons aged >= 70 years from Finland and the Balkan Peninsula. Participants were randomly allocated to an intervention group (4 weeks of group intervention and one follow-up home visit) and a control group (no intervention but access to ordinary health care services). The outcome measure was the SOC measured by SOC-13. Chi-square and ORs were calculated. Results: There was a significant improvement in total SOC scores for the intervention group at 6-month follow-up. Also, the ORs for the SOC components were higher in the person-centered intervention group. However, we found no significant between-group differences nor did the effect last until the 12-month follow-up. Conclusion: Persons who have lived a long time in a host country after migration seem to have a SOC similar to native-born persons. Interventions with a person-centered approach could support the SOC by capturing individual life situations. Such interventions could support older persons by making everyday life more comprehensible and manageable and helping them to cope with challenges in daily life caused by aging.

    • Background: There is a growing need to support the health and wellbeing of older persons aging in the context of migration. Objectives: We evaluated whether a group-based health promotion program with person-centred approach, maintained or improved life satisfaction and engagement in activities of older immigrants in Sweden. Methods: A randomised controlled trial with post-intervention follow-ups at 6 months and 1 year was conducted with 131 older independently living persons aged ≥70 years from Finland and the Balkan Peninsula. Participants were randomly allocated to an intervention group (4 weeks of group intervention and a follow-up home visit) and a control group (no intervention). Results: The odds ratios for maintenance or improvement of life satisfaction (for social contact and psychological health) were higher in the person-centred intervention group. However, no significant between-group differences were found. Conclusion: Person-centred interventions can support older person’s capability to maintain their health in daily life when aging in migration. Further research is needed with a larger sample and longer intervention period to determine the effectiveness of the intervention.

    • In the context of migration, ageing is described as a negative stressor for subjective perceptions of health in everyday life. Most research has adopted a biomedical perspective. An occupational perspective of health was the starting point for this study. The aim was to explore how older persons from Finland who are ageing in Sweden experienced health from an occupational perspective. Qualitative interviews were conducted with 16 persons who had migrated from Finland to Sweden as labour immigrants several decades ago. The interviews were analysed and interpreted using a hermeneutic approach. Participants experienced health when they managed their daily life occupations. Health was also reinforced through affiliation with others who shared similar life situations. A key health resource was maintaining a sense of dual belonging to the country of birth and to the host country. The present study revealed that the experience of health could be seen as a continuum in time and place, which creates a sense of belonging and continuity in life. Connections between health and doing, being and becoming are fundamental aspects to consider when enabling health in everyday life among older persons ageing in the context of migration.

    • © 2016 Nordic College of Caring Science.The proportion of older immigrants in the population is expected to increase in Sweden. Research indicates that older immigrants face the double burden of frailty, caused by a weaker health status, and the immigration process. Health promotion interventions can be used to prevent frailty and support healthy ageing in this population. Healthcare professionals are a crucial part of health promotion and the present study aimed to explore healthcare professionals' experiences of health in context of daily life among older immigrants to and provide a basis for the development of a health promotion intervention targeted at older immigrants. Four focus group discussions were conducted with professionals who worked with home-dwelling older people from Finland, currently living in Sweden. Participants represented a variety of healthcare professions. Data were analysed using qualitative content analysis. The healthcare professionals perceived that health in daily life was (i) connected to the view of oneself as a capable person and (ii) participants strove to maintain their own origin. The findings highlighted that when planning for health promotion interventions for older immigrants, the point of departure should be to recognise the person as a person and the background of immigration is not the first issue to address. Therefore, a person-centred health promotion intervention is recommended to open a dialogue between healthcare professionals and older immigrants, so that a common view of the intervention can be reached.

    • Introduction: The topic of migration concerns many people with an immediate impact on culture, politics, education, and healthcare around the world. The geographical mobility associated with migration arguably challenges the form, function, and meaning of everyday occupations and thus has a direct impact upon peoples health. This workshop will tackle some of these topics by drawing on a series of ongoing community and intervention projects. Aims: to explore topics of everyday occupations in the context of migration from multiple perspectives. Methods: workshop consists of short presentations followed by groupwork. Outcomes: at the end of this workshop, participants will have: 1) Reflected on challenges concerning research in areas of migration and occupational therapy/occupational science, 2) applied concepts of diversity to own practice, 3) applied concepts and ideas from presentations through groupwork with a WFOT position paper, and 4) initiated contact with occupational therapists from around the world who share common interests.

  • For whom is a health-promoting intervention effective? : Predictive factors for performing activities of daily living independently

    • Background: Health-promoting interventions tailored to support older persons to remain in their homes, so-called “ageing in place” is important for supporting or improving their health. The health-promoting programme “Elderly Persons in the Risk Zone,” (EPRZ) was set up for this purpose and has shown positive results for maintaining independence in activities of daily living for older persons 80 years and above at 1- and 2 year follow-ups. The aim of this study was to explore factors for maintaining independence in the EPRZ health-promoting programme. Methods: Total of 459 participants in the original trial was included in the analysis; 345 in the programme arm and 114 in the control arm. Thirteen variables, including demographic, health, and programme-specific indicators, were chosen as predictors for independence of activities of daily living. Logistic regression was performed separately for participants in the health promotion programme and in the control arm. Results: In the programme arm, being younger, living alone and self-rated lack of tiredness in performing mobility activities predicted a positive effect of independence in activities of daily living at 1-year follow-up (odds ratio [OR] 1.18, 1.73, 3.02) and 2-year, (OR 1.13, 2.01, 2.02). In the control arm, being less frail was the only predictor at 1-year follow up (OR 1.6 1.09, 2.4); no variables predicted the outcome at the 2-year follow-up. Conclusions: Older persons living alone - as a risk of ill health - should be especially recognized and offered an opportunity to participate in health-promoting programmes such as “Elderly Persons in the Risk Zone”. Further, screening for subjective frailty could form an advantageous guiding principle to target the right population when deciding to whom health-promoting intervention should be offered.     • Background: There are inequities in health status associated with ethnicity, which may limit older foreign-born persons' ability to age optimally. Health promotion for older persons who have experienced migration is thus an area of public health importance. However, since research related to this issue is very limited, the study 'Promoting Aging Migrants' Capabilities' was initiated to improve our understanding. The study aims to implement and evaluate a linguistically adapted, evidence-based, health-promoting intervention with a person-centred approach for two of the largest groups of aging persons who have migrated to Sweden: persons from Finland and persons from the Balkan Peninsula. Methods/Design: This study has a descriptive, analytical, and experimental design. It is both a randomised controlled trial and an implementation study, containing the collection and analysis of both qualitative and quantitative data. The setting is an urban district in a medium-sized Swedish city with a high proportion of persons who were born abroad and whose socio-economic status is low. The intervention comprises four group meetings ('senior meetings') and one follow-up home visit made by a multi-professional team. For the randomised controlled trial, the plan is to recruit at least 130 community-dwelling persons 70 years or older from the target group. Additional persons from involved organisations will participate in the study of the implementation. Both the intervention effects in the target group (outcome) and the results of the implementation process (output) will be evaluated. Discussion: The results of this forthcoming randomised controlled trial and implementation study may be useful for optimising implementation of person-centred, health-promoting initiatives for older persons who have experienced migration. It is also hoped that this combined study will show that the capabilities for optimal aging among older persons born in Finland and the Balkan countries can be improved in the Swedish healthcare context.     • OBJECTIVES: The study Elderly Persons in the Risk Zone was designed to evaluate if it is possible to delay deterioration if a health-promoting intervention is made when the older adults (80+) are at risk of becoming frail, and if a multi-professional group intervention is more effective in delaying deterioration than a single preventive home visit. This paper examined the outcome with regard to frailty, self-rated health, and Activities of Daily Living (ADL) at the three-month follow-up. DESIGN: A randomized, three-armed, single-blind, and controlled trial performed between November 2007 and May 2011. SETTING: Two municipalities of Gothenburg, Sweden. PARTICIPANTS: A total of 459 older adults were included. They were 80 years or older, living in their ordinary housing, and not dependent on the municipal home help service. INTERVENTION: A preventive home visit or four weekly multi-professional senior group meetings with one follow-up home visit. MEASUREMENTS: The change in frailty, self-rated health, and ADL between baseline and the three-month follow-up. RESULTS: Both interventions delayed deterioration of self-rated health (OR=1.99, 95% CI=1.12 to 3.54). As regards postponing dependence in ADL, senior meetings were found to be the most beneficial intervention (OR=1.95, 95% CI=1.14 to 3.33). No effect on frailty could be demonstrated. CONCLUSION: Health-promoting interventions, made when older adults are at risk of becoming frail, can delay deterioration of self-rated health and ADL in the short term. Also, a multi-professional group intervention such as the senior meetings described seems to have a greater impact on delaying deterioration in ADL than a single preventive home visit. Further research is needed to examine the outcome in the long term, and in different contexts.

To further enhance the long-term effects of the senior meetings and support the process of self-change in health behavior, it is suggested that booster sessions might be a good way of reinforcing the intervention.

    • Introduction: There are numerous ways to utilize virtual reality (VR) in rehabilitation. For clients and professionals VR promotes digital inclusion in rehabilitation programs. Aim: to evaluate the evidence for the effectiveness of virtual rehabilitation on occupational performance in adult stroke patients' rehabilitation. Methods: The systematic review was conducted to identify published randomized controlled trials (RCT) from several databases. The quality of evidence was graded with GRADE (Grading of Recommendation Assessment, Development and Evaluation) method. Results: Ten RCT studies identified. The quality of evidence on the effectiveness of using VR in the stroke rehabilitation was very low or low. Conclusion: The current evidence on the effectiveness of using VR in the stroke rehabilitation is limited. Any recommendations of using VR in stroke rehabilitation cannot be made due to limited evidence. Further research concerning virtual rehabilitation in group-based rehabilitation and organizing rehabilitation in rural areas is needed.

  • An Occupational Perspective on Enculturation and Habitus - a Scoping Review

    • Increasingly, scientific interest has been focused on the meaningfulness of engaging in occupations of daily life in individual ways, and especially so for people with migration backgrounds. This is because people who migrate to a new country will likely find themselves in a position in which they notice that some occupations are done differently from how they were used to prior to moving. Enculturation, a process in which cultural elements are recognized and internalized, informs the way immigrants engage in occupations. This study aimed to scope the occupational science and occupational therapy literature on the phenomenon in which enculturation leads to adjustments of habitus among immigrants. Descriptions of the phenomenon and its impact on daily occupations were qualitatively synthesized. The included studies entail documentations on how people who migrated enculturate and adjust their habitus regarding social etiquette, social expectations, women and gender perception, feeling of acceptance and belonging, disclosure of values and beliefs, work moral and sense of competency, and occupations involving meals. Despite the findings on the impact of occupations in migration contexts, further rigorous research is needed to investigate the way in which enculturation informs habitus adjustments and strengthen the evidence base on cultural sensitivity in occupational science.

  • Aspects Affecting Food Choice in Daily Life as Well as Drivers and Barriers to Engagement with Fungi-Based Food-A Qualitative Perspective

    • Fungi-based food is expected to contribute to more sustainable food systems. The present study has three focus areas: (i) aspects that affect food choices food in daily life, (ii) aspects that affect choices of fungi-based food in particular, and (iii) drivers that motivate, and barriers that prevent, engagement in cultivating fungi and cooking fungi-based food at home. One hundred and sixty participants, who were recruited using convenience sampling, filled out qualitative questionnaires. The results show that there are numerous aspects (e.g., environmental benefits, nutrition, sensory characteristics, production practices and ingredients) that are important when people choose food in daily life. In addition to curiosity, many of these aspects also affect the choice of fungi-based food. The study identified more drivers (e.g., self-providing, curiosity, awareness of ingredients) than barriers (time, knowledge, concerns about contamination) to cultivation and cooking of fungi-based food at home. The findings are relevant for the development of fungi-based food so that this type of food is engaged with, and so that it can contribute to more sustainable food systems.

  • Aspects that Affect Tasting Studies of Emerging Food : A Review

    • Providing food security to the growing global population, and the resource depletion associated with current food systems, let to calls for more sustainable food sources. Food that can be produced in a sustainable way (taking all three aspects of sustainable development into consideration) is currently emerging in Western societies. Through tastings, insight can be gathered not only into sensory characteristics but also other aspects that aid innovation and development of food. The current study identified aspects that can affect tastings of emerging food by reviewing relevant literature. General aspects; meat alternatives; ingredients or processing technologies; information, prior knowledge and (un)familiarity; taste and liking; emotional factors; and willingness to engage with emerging food can affect tastings of emerging food. Awareness of the effect that these aspects can have on methodological considerations and results can be constructive in future research that use tastings as a platform to develop new and emerging food. The findings are significant for food science in terms of cornerstones towards potential industrial applications. These include innovating new types of food, assessing most effective technologies in the context of such food, developing new products, and understanding engagement with emerging food products.

  • Glocal and ecoethical perceptions of engagement with fungi-based food

    • Fungal fermentation is a promising strategy to secure affordable, nutritious and sustainable food. Encouraging engagement with fungi-based food is crucial to contribute to social, economic, and environmental sustainability. Reflections can trigger a sense of meaning in engaging in activities and with resources. The aim of this mixed methods study was to explore perceptions of whether participants think their own engagement with fungi-based food is consequential. To do so, the study explored ecoethical reflections relating to whether participants thought engaging with fungi-based food is beneficial or not beneficial for the environment. This study also explored glocal reflections of whether participants thought their own engagement with this kind of food is beneficial or not beneficial in ways that extend to people around them (i.e., local population) or people in other parts of the world (i.e., global population). N = 160 participants completed questionnaires. Most participants expressed a positive outlook, believing that embracing fungi-based food could promote increased sustainability and overall well-being for humans and the environment in numerous different ways. The perceptions that participants shared can affect and trigger conscious engagement with fungi-based food locally with awareness of its global impact which, in turn, can promote well-being for individuals and extend to the population level and thereby contribute to efforts at archiving sustainable development. Nevertheless, the findings highlight a necessity for more information to enable individuals to engage in knowledgeable reflections and, ultimately, act upon their values and what is meaningful to them. The results are important for future development and conceptualization of not only fungi-based food but also other food that is expected to contribute to sustainable development.

    • Resource dependency of food production is aggravated when food is wasted. In Sweden, it is estimated that 37% of the total bread waste is generated at the household level. This work aimed to assess whether fermentation using edible filamentous fungi at households can provide a solution to valorize leftover bread in the production of fungi-based food for consumption. Bread was fermented in household and laboratory conditions with Neurospora intermedia and Rhizopus oligosporus. The results show that bread can be successfully and easily fermented at households, without signs of microbial contamination even though the conditions were not sterile. Fermentation at the household resulted in higher protein, fat and fiber content as well as greater starch reduction compared to the samples fermented under laboratory conditions. Household engagement in bread fermentation will likely depend on values that motivate reusing leftover bread. Perceived values that are expected to motivate engagement vary across individuals, but may include improved nutritional benefits, food waste prevention, convenience, responsibilities, and being part of sustainable societies and actions. © 2022 The Author(s)

    • The aim of this scoping review was to gain an overview of the current state of the literature on the engagement in waste sorting post migration from an occupational perspective, in the light of two aspects sustainability efforts currently face: Increased human migration and environmental degradation. Both the resource recovery and occupational science literature were reviewed and analyzed. However, despite the current lack on studies on how migrants’ transition into waste sorting schemes at the household level, this scoping review was able to provide a broad picture of the engagement in daily activities that support sustainability, such as household waste sorting. Given the current initiatives to develop efficient resource recovery from waste, such knowledge contributes to efforts to engage households with different cultures and experiences in waste sorting. The results highlight the importance of future research to better understand how people who are new to waste management schemes experience these, and study the way that engagement in waste sorting shifts and transforms. This is because providing such knowledge can contribute to raising awareness of the environmental impact of waste sorting, and inform policies aimed at sustainable waste management.

    • The objective of this study was to describe how elderly users of assistive devices perceive and relate to their assistive devices in occupational performance. Study design.The research approach used was phenomenography, which endeavours to describe different ways of peoples experience. Result.User attitude is described as: the assistive devices are well incorporated; the elderly are forced to accept; the necessity of using the device gives the feeling of inadequacy. Accommodation to the use of assistive devices in daily activities are described as: modification and resignation. The Support of assistive device is perceived in three different ways; external safety, internal security and respect. Conclusion. Elderly women's experience of what it is like to rely on an artificial device is important when planing for support. The main feature that appears is an endeavour to gain control of, or a feeling of being able to take control of, one's occupational performance

    • The aim was to explore experiences of daily occupations among the oldest old. Ten 99-year-old persons were interviewed in their living environment about an ordinary day. The interviews were analysed according to the phenomenographic approach. The result showed that participants regarded themselves as competent and that they felt proud because they were involved in daily occupations. Being challenged was experienced as a way of performing difficult tasks, thus being confirmed as a capable person who performs and learns new things. The participants' occupational patterns preserve occupational ability and continuity in life in that they have a rhythm and allow the individuals to predict and handle interruptions. Participants experienced being incapable and being restricted as a result of personal, environmental, and social hindrances. The participants adapted to and reshaped their sense of self, which has been disrupted due to the discrepancy between self, the person's ability, and the real world "outside". Experience of daily occupation is unique, supports the sense of self, builds identities, and describes engagement and creative processes. Individual experience must be recognized as it can mean the difference between success and failure in maintaining meaningful daily occupation.

    • Bakgrund: Klientcentrering är ett inarbetat begrepp och arbetssätt inom arbetsterapin sedan många år tillbaka. Hur begreppet är implementerat i praxis beskrivs i litteraturen och det finns en konsensus att det är det arbetsterapeuter bör sträva efter i mötet med klienter. Personcentrering är ett annat begrepp med ökad användning inom forskning och klinisk hälso- och sjukvård, men ibland används begreppet oreflekterat och ersätter andra liknande begrepp. Innebörden av begreppet kräver ytterligare uppmärksamhet och det pågår en debatt om dess tillämpning. Klient- och personcentrering har närliggande innebörder, men begreppen har sina ursprung från olika kunskapsområden och har anpassats efter de områden de används inom. Vi behöver förstå vilka svårigheter och möjligheter olika begrepp kan ge oss i praxis och det finns ett behov av att utforska närliggande begrepp från flera synvinklar. Syfte: Att tillsammans problematisera kring begrepp som står för liknande fenomen i olika kontext i syfte att fånga komplexiteten. Deltagarna skall kritiskt utforska möjligheter och illustrera olika begrepp som används i praxis samt identifiera vad vi redan vet, hur vi tänker samt vad vi behöver utveckla. Tillvägagångssätt: Gestaltning och diskussion i mindre grupper för att belysa erfarenheter och uppfattningar om närliggande fenomen inom arbetsterapeutisk praxis. Det handlar om att tillsammans ordsätta fenomenets innebörd i olika kontexter. Workshopen innefattar även en kort presentation av olika begrepps ursprung samt hur de beskrivs i litteraturen. Efter slutförd workshop ska deltagarna ha: - Kritiskt granskat klientcentrering och närliggande begrepps betydelse för arbetsterapeutisk praxis och forskning - Identifierat faktorer som kan vidareutveckla arbetsterapeutisk praxis och forskning - Förtydligat hur arbetsterapeutiska erfarenheter kan vara en resurs för utveckling av personcentrerad praxis inom olika verksamhetsområden

    • The overall aim of the present thesis was to examine, extend and deepen the understanding of very old persons? experience of participation in everyday life from an occupational perspective. The thesis consists of four empirical studies. The participants are mainly very old persons (80+) living at home and were derived from the gerontological and geriatric population studies in Göteborg (H70), Sweden. Study I (n=11) and study IV (n=10) are qualitative studies in which a phenomenographical approach was adopted. Study II is a cross-sectional study (n=205), and study III is a cross-sectional and longitudinal study (n=201), which were subjected to qualitative content- and statistical analysis. The findings in study I and II showed that very old persons used assistive devices to a high degree, and that the use of devices increased with age. Most common were devices used in hygiene- and mobility related occupations. The experiences of being a user of assistive devices varied greatly and various contradictions were found. On the one hand, the assistive devices were seen as natural or normal for the age, the devices gave support, made the person feel safe and facilitated their daily occupation. On the other hand, the assistive devices were experienced as cumbersome, gave a feeling of incapability, were a mark of old age, and made the person concerned avoid participation in everyday life. The findings in III study showed that the participants had a broad range of occupations of interest, media and individual leisure interests being the most common. Personal and environmental factors were reasons for giving up interests. Persons who regarded their health as good, or had no problems in daily activities or in mobility outdoors, had more interests than those with poor health, limited abilities in mobility and in managing daily occupations. The findings in study IV showed how 99-year old persons regarded themselves as competent and proud of their ability to participate in everyday life. Many signs of involvement in daily occupations were found; how they challenged; how occupational patterns preserve occupational abilities, and how incapability and restrictions as a result of personal, environmental and social hindrances were experienced. In conclusion these studies revealed that very old persons live a creative and varied life and appear to have a variety of management/coping ability for handling the balance between abilities, limitations and environmental demands in everyday life. Daily occupations are mainly performed with the support of assistive devices, though these may be sometimes seen in a negative light. What very old persons experience and how they experience their participation in everyday life greatly affect their self-images, and this is a challenge to everyone who works with elderly persons.
