  • Satisfactory outcome after intensive chemotherapy with pragmatic use of minimal residual disease (MRD) monitoring in older patients with Philadelphia-negative B cell precursor acute lymphoblastic leukaemia : a Swedish registry-based study

    • The introduction of minimal residual disease (MRD) monitoring, in the Swedish national guidelines for acute lymphoblastic leukaemia, was evaluated in 35 patients aged 46-79 years (median 61), who were diagnosed from 2007 to 2011 and treated with high-intensity, block-based chemotherapy (ABCDV/VABA induction). Both a high complete remission rate (91 %) and acceptable overall survival (OS) rate (47 %) at 5 years were achieved. MRD by flow cytometry was measured in 73 % of the patients reaching complete remission after the first course, but was omitted by the clinicians for eight patients who were either over 70 years of age or already met conventional high-risk criteria. Factors negatively influencing OS were age over 65 years and WHO status >= 2. MRD < 0.1 % after induction had positive impact on continuous complete remission but not on OS. Only five patients were allocated to allogeneic haematopoietic stem cell transplantation in first remission, mainly due to conventional high risk factors. Thus, use of intensive remission induction therapy is effective in a selection of older patients. In a population for whom the possibilities of treatment escalation are limited, the optimal role of MRD monitoring remains to be determined.

  • Slipped capital femoral epiphysis : A population-based study

    • Background: Slipped capital femoral epiphysis (SCFE) is the most common hip disorder in children 9-15 years old. This is a population-based study in Sweden presenting the epidemiology for SCFE. Methods: In a prospective cohort study, we analysed pre- and postoperative radiographs, medical records for all children treated for SCFE in Sweden 2007-2013, demographic data, severity of slip and surgical procedures performed. Results: We identified 379 Swedish children with primary SCFE 2007-2013; 162 girls, median age 11.7 (7.2-15.4) years, and 217 boys, median age 13 (3.8-17.7) years. The average annual incidence was 4.4/10000 for girls and 5.7/10000 for boys 9-15 years old. Obesity or overweight was found in 56% of the girls and in 76% of the boys. As an initial symptom, 66% of the children had hip/groin pain and 12% knee pain. At first presentation, 7% of the children had bilateral SCFE. Prophylactic fixation was performed in 43%. Of the remaining children, 21% later developed a contralateral slip. Fixation with implants permitting further growth was used in >90% of the children. Femoral neck osteotomy was performed for 11 hips. Conclusions: The annual average incidence 2007-2013 in Sweden showed a mild increase for girls. The male-to-female ratio was lower than previous regional data from Sweden. Overweight or obesity is one major characteristic for boys with SCFE but to a less extent for girls. Knee pain as initial symptom cause a delay in diagnosis. Most hospitals in Sweden treat <2 children annually.

  • Determination of uncertainty of different CFD codes by means of comparison with experimental fire scenarios

    • This article will summarise the evaluation of four CFD software codes (CFX, FDS4, SMAFS and SOFIE). Evaluation was performed by means of comparing the simulation data with experimental scenarios. The scenarios were chosen to represent scenarios frequently assessed using fire safety design based on performance. The greatest difficulty during validation of CFD-codes is to find well documented relevant experiments. It is true that many large scale experiments including smoke spread have been published but many of them are not described sufficiently and are therefore impossible to use for validation. It is very unusual that data on reproducibility and repeatability is available. After an extensive discussion between the participants in the project five scenarios were chosen. Scenarios 1A (large room with vents), and scenarios 1B1 and 1B2 (corridor with vents) were well documented experiments (repeatability) with ceiling vents and ceiling jets performed earlier at SP in Boras. Scenario 2 is a tunnel fire taken from an experimental study in Boston, US. Scenario 3 is an atrium fire taken from an experimental study in Japan. Lack of well documented fires in retail premises lead us to perform a series of tests in 1/2 scale, 4.1 and 4.2 at SP in Boras. Scenario 5 is an experimental study from Australia including smoke spread from a fire room to an adjacent corridor and a room. The conditions for the simulations included information about the scenarios given above. No information about experimental results was available until the results from the simulations had been sent in to the project manager. The intention was to imitate the working conditions for a consultant as far as possible when using a CFD-simulation to estimate fire safety. In this manner the simulations are dependent on many factors such as the CFD-code used, judgement of input data of the scenarios done by the operator, operator skill etc. The results from the evaluation of the CFD codes showed that the simulation results generally were a good description of the experimental fires studied, but that limitations and specific properties of the different codes, together with the operator factor, can strongly influence the results. The detailed results of all comparisons are given in the final report of the project. It was not possible to include these detailed evaluations in this conference paper but these results are available in the main report as well as in future scientific articles.
