  • ZD – Directional Testing of Paperboard, Using Refined Methods to Revisit Out of Plane Properties

    • The ZD-tensile strength is tested by attaching the top and bottom sides of a paperboard to rigid blocks that are pulled apart. In a production laboratory the strength is recorded using a tape as an adhesive. In specialized laboratories a more thorough method is available that also measures the force-displacement curve of the sample. The advanced method involves laminating and gluing the paperboard sample to metal blocks which are mounted in a universal testing machine. In this study the advanced ZD-tensile method was refined by removing the glue step and laminating the paperboard directly to the blocks. The new method was validated against the regular method with adequate results. The limits of the refined method were explored with regards to ZD-strength and paper/paperboard thickness. In an attempt to unify the ZD-tensile and -compressive behaviour of paperboard, samples were laminated and tested in combined compression and tension testing. The compressive properties were compared to non-laminated samples. The laminated samples showed a different behaviour than the non-laminated samples. The flat slope seen in the initial part of the pure compression curve disappeared, replaced by a continuous response passing 0 N. The stiffness in this region resembled the response in tensile testing.

  • Additive Manufacturing of a Removable Partial Prosthesis in Titanium Using Binder Jetting Technology: A Brief Research Report

    • Objective: The purpose of this study was to measure the surface roughness and compare the fit of a cobalt-chromium selective laser-melted removable partial denture framework and a titanium binder jetting removable partial denture framework. Materials and methods: A design for a removable partial prosthesis framework was made using 3Shape software, and thereafter, one framework was additively manufactured in cobalt-chromium with selective laser melting and one in titanium using binder jetting technology. The weight of the frameworks was measured, and the surface parameters Sa, Sds, and Sdr were measured by white light interferometry. The fit of the frameworks was analyzed using visual inspection and the pressing test. Results: The weight of the frameworks in their post-printed state was 15.66 g and 7.43 g for cobalt-chromium and titanium, respectively. After finishing and polishing, the cobalt-chromium framework’s outer surface showed a high shine and smooth surface, with lower values in the Sa and Sdr parameters. The visual inspection showed gaps in the titanium framework, and the pressing test detected movement greater than 0.5 mm for the same material. Conclusion: The Ti framework showed higher surface roughness and was left with a lackluster finish after polishing. Also, the fit of the Ti framework was not clinically acceptable. The additive manufacturing binder jetting technology for titanium needs improvement or an adjustment of the settings to be used for clinically removable partial dentures. The cobalt-chromium framework manufactured by selective laser melting showed an acceptable clinical fit and surface roughness.

  • Club cell secretory protein (CC16) in gastric fluid at birth and subsequent lung disease in preterm infants

    • BackgroundClub cell secretory protein (CC16) probably has a role in protecting the lung from inflammation. AimTo evaluate if low levels of CC16 in gastric fluid at birth, reflecting low levels of CC16 in the lung, would be associated with lung inflammation and respiratory morbidity. MethodsA study of 64 infants with mean gestational age 26.1 weeks. CC16 was analyzed in gastric fluid at birth. CC16, pro-inflammatory cytokines, and MMP-9 were analyzed in tracheal aspirate within 24h from birth. ResultsCC16 in gastric fluid increased with gestational age (P=0.033). Lower concentrations of CC16 in gastric fluid at birth were associated with higher concentrations of IL-1 (P=0.028), TNF- (P=0.034), and MMP-9 (P=0.015) in tracheal aspirate. Infants who needed mechanical ventilation at 24 and 72h of age had lower CC16 in gastric fluid than those not ventilated at these ages (P=0.011 and P=0.024, respectively). Lower CC16 in gastric fluid was associated with higher FiO(2) at 6h (P=0.009), higher PaCO2 at 24h (P=0.03), more ventilator days (P=0.012) and more days with supplemental oxygen (P=0.03). Infants who had either died or were still treated with supplemental oxygen at 36 weeks postmenstrual age had lower CC16 in gastric fluid than infants with none of these outcomes (P=0.049). ConclusionA low CC16 concentration in gastric fluid at birth was associated with increased inflammation in the trachea within the first 24h of life and with more need for respiratory support in the neonatal period.

  • Lung function deficits and bronchodilator responsiveness at 12 years of age in children born very preterm compared with controls born at term

    • INTRODUCTION: Very preterm birth is associated with lung function impairment later in life, but several aspects have not been studied. We aimed to comprehensively assess lung function at school age in very preterm infants and term controls, with special emphasis on bronchopulmonary dysplasia (BPD), sex, and bronchodilator response.METHODS: At 12 years of age, 136 children born very preterm (85 with and 51 without BPD) and 56 children born at term performed spirometry, body plethysmography, impulse oscillometry, measurement of diffusion capacity, and multiple breath washout, before and after bronchodilator inhalation.RESULTS: Airway symptoms and a diagnosis of asthma were more common in children born very preterm. These children had more airflow limitation, seen as lower forced expiratory volume in 1 s (FEV1 ) (p < .001), FEV1 /forced vital capacity (FVC) (p = .011), and mean forced expiratory flow between 25% and 75% of FVC (p < .001), and a higher total and peripheral airway resistance compared with term-born controls. There was no difference in total lung capacity but air trapping and lung clearance index were higher in children born very preterm. Diffusion capacity was lower in children born very preterm, especially in those with a diagnosis of BPD. In most other tests, the differences between preterm-born children with or without BPD were smaller than between children born preterm versus at term. Boys born preterm had more lung function deficits than preterm-born girls. In children born very preterm, airway obstruction was to a large extent reversible.CONCLUSION: At 12 years of age, children born very preterm had lower lung function than children born at term in most aspects and there was only little difference between children with or without BPD. Airway obstruction improved markedly after bronchodilator inhalation.

    • Aim: Very preterm birth may be associated with lung function impairment later in life. It is not known if this is caused by prematurity per se or by associated perinatal events, such as maternal–foetal inflammation and severity of early neonatal lung disease. We assessed these factors in a prospective cohort of very preterm infants followed from birth to middle school age. Methods: In 71 infants with a gestational age of median 27.4 (range 23.9–31.7) weeks, pro-inflammatory and modulatory cytokines were measured in umbilical cord blood and in arterial blood sampled at 6, 24 and 72 h after birth, and cumulated cytokine concentrations were calculated as area under the curve (AUC). At median 12.6 (range 12.3–13.5) years of age, pulmonary function testing was done in 53 children. Results: There was a positive correlation between days on mechanical ventilation and AUC for IL-6 (p = 0.001), IL-8 (p = 0.015) and IL-10 (p = 0.006). Infants with bronchopulmonary dysplasia (BPD; n = 32) had higher AUC for the cytokines IL-6, IL-8 and IL-10 than those without BPD (all p < 0.01). Higher levels of AUC for IL-6 at birth correlated with lower forced expiratory volume in 1 s (p = 0.030) and lower mean expiratory flow rate between 25 and 75% of forced vital capacity (p = 0.034). Conclusion: Perinatal inflammation, assessed by circulating cytokines in the first three days of life, was associated with BPD and with airway obstruction at 12 years of age.

    • An empirical study investigates the extent to which affective–symbolic and instrumental–independencepsychologicalmotives mediate effects of socio-demographicvariables on dailycar use in Sweden. Questionnaire data from a mail survey to 1134 car users collected in 2007 were used to assess the relationships dailycar use as driver or passenger have to sex, household type (single or cohabiting with or without children), and residential area (urban, semi-rural or rural). Reliable measures of affective–symbolic and instrumental–independencemotives were constructed. The results show that households with children use the car more than households with no children, that men make more car trips as drivers than women who use the car as passenger more than men, and that households living in rural areas use the car more than households living in semi-rural areas who use the car more than households living in urban areas. An affective–symbolicmotive partially mediates the relationship between the number of weekly car trips and sex, the instrumental–independencemotive partially mediates the relationships between weekly car use and percent car use as driver and several of the socio-demographicvariables (living in urban vs. rural residential area for both measures; sex and living in urban vs. semi-rural residential area for percent car use as driver). Of several other socio-demographicvariables (age, employment, and income) affecting car use, only the relationship of the number of cars to percent car use as driver was (partially) mediated by the instrumental–independencemotive.

    • A survey of a random sample of 1,330 Swedish residents assessed the relationships between affect associated with performance of routine out-of-home activities, mood, and judgments of life satisfaction (cognitive subjective wellbeing, CSWB). Regression analyses showed that sociodemographic variables accounted for most variance in CSWB (7%) and least in mood (2%). In agreement with previous research, CSWB increased with income, employment, and cohabiting with a spouse, and had a U-formed relationship with age. Affect associated with routine activities accounted for more variance than the socio-demographic variables in mood (30%) and in CSWB (13%). Mood partially mediated the effect on CSWB of affect associated with the activities. The results suggest that future policy-related research should consider the possibility that community-provided resources that facilitate performance of routine out-of-home activities would increase life satisfaction

    • Previous research demonstrates an impact on subjective well-being (SWB) of affect associated with routine performance of out-of-home activities. A primary aim of the present study is to investigate whether satisfaction with daily travel has a positive impact on SWB, either directly or indirectly through facilitating the performance of out-of-home activities. A secondary aim is to determine whether emotional-symbolic or instrumental reasons for car use results in higher satisfaction with daily travel than other travel modes. A survey of a population-based sample of 1,330 Swedish citizens included measures of car access and use, satisfaction with daily travel, satisfaction with performance of out-of-home routine activities, and affective and cognitive SWB. The results confirmed that the effect on affective and cognitive SWB of satisfaction with daily travel is both direct and indirect via satisfaction with performance of activities. Percent weekly car use had a small effect on satisfaction with daily travel and on affective SWB, although fully mediating the effect of satisfaction with performance of the activities. This suggests that car use plays a minor role for satisfaction with daily travel and its effect on SWB. This role may be larger if investigated after a forced reduced car use.

    • The aim of this systematic review was to evaluate the assessment of trabecular bone patterns in dental radiographs, for fracture risk prediction, compared with the current diagnostic methods.The PRISMA guidelines were followed. According to predefined inclusion criteria (PICO), literature searches were focussed on published studies with analyses of trabecular bone patterns on intraoral and/or in panoramic radiographs, compared with Dual X-ray Absorptiometry (DXA) and/or Fracture Risk Assessment Tool (FRAX), with the outcomes; fracture and/or sensitivity and specificity for osteoporosis prediction. The included studies were quality-assessed using the QUADAS-2 tool and the certainties of evidence was assessed using the GRADE approach.The literature searches identified 2913 articles, whereas three were found to meet the inclusion criteria. Two longitudinal cohort studies evaluated the use of trabecular bone patterns to predict bone fractures. In one of the studies, the relative risk of fracture was significantly higher for women with sparse bone pattern, identified by visual assessment of dental radiographs, and in the other study by digital software assessment. Visual assessment in the second study did not show significant results. The cross-sectional study of digital analyses of trabecular bone patterns in relation to osteoporosis reported a sensitivity of 0.70 and a specificity of 0.69.Based on low certainty of evidence, trabecular bone evaluation on dental radiographs may predict fractures in adults without a prior diagnosis of osteoporosis, and based on very low certainty of evidence, it is uncertain whether digital image analyses of trabecular bone can predict osteoporosis.

  • Mechanical response of paperboard in rapid compression – the rapid ZD-tester, a measurement technique

    • Paperboard is a common material for packages and other carriers of information. During rotary printing processes, the paperboard is subjected to rapid deformations in the out-of-plane direction as it passes through the nip between the rolls of the printer. Being viscoelastic in nature, the mechanical response of the material to high deformation rates differs from what is measured with conventional testing conducted at slower deformation rates. In this work, a device called the rapid ZD-tester is used to show the response of paperboards subjected to a rapid pressure pulse and compare this to measurements made at lower strain rates in a common universal testing machine. All the tested paperboards show complete recovery within 5 s when being rapidly compressed, while the slower compression to the same pressure leaves a deformation that remains after 5 s. The stiffness response differs between the paperboards, but does not consistently increase or decrease between slow or rapid compressions. The difference in response between slow and rapid compression appears larger for the low-density paperboard in the study.

    • The out-of-plane properties of paperboard are important in several converting applications such as printing, sealing, creasing, and calendering. A juxtaposed tensile and compression curve in the z direction (ZD) will, however, appear to have a kink or discontinuity at 0 stress. The purpose of the present work is to capture the continuous transition between tension and compression and to increase the understanding of the complex ZD properties of paperboard by cyclic testing. In this attempt to unify the ZD tensile and compressive behavior of paperboard, samples were laminated to the testing platens using heat seal laminate film. The method for adhering the samples was compared to samples that were laminated and glued to the testing platens. The edge effects of the cutting method were evaluated in compression testing with samples not attached to the testing platens. The flat slope seen in the initial part of the pure compression curve disappeared when the samples were laminated to the testing platens. The flat slope was instead replaced by a continuous response in the transition across 0 N. The stiffness in the transition region resembled the response in tensile testing. When the testing is cycled, the material exhibits a history dependence. Starting the cycle in either compression or tensile will show an effect on the stiffness at the transition, as well as the compressive stiffness. However, the ultimate tensile strength is unaffected.

  • Key outcomes in childbirth : Development of a perinatal core outcome set for management of labor and delivery at or near term

    • Introduction: Consistency and relevance of perinatal outcome measures are necessary basics for obstetric research, audit, and clinical counseling. Still, there is an unwarranted variation in reported perinatal outcomes, which impairs research synthesis, validity, and implementation, as well as clinical benchmarking and longitudinal comparisons. The aim of this study was to develop a short-term perinatal (fetal and neonatal) Core Outcome Set to be used in research and quality assurance of management of labor and delivery at or near term.Material and methods: The methods were guided by the Core Outcome Measures in Effectiveness Trials Initiative Handbook. The project was prospectively registered on July 2, 2020 in the Core Outcome Measures in Effectiveness Trials (COMET) data base (reference number 1593). A list of potential outcomes was created based on a systematic review of studies evaluating interventions for peripartum management at or near term (>= 34 weeks of gestation), including decisions regarding timing and type of onset of labor, intrapartum care, and mode of delivery. The list was entered into a two-round Delphi survey with predefined consensus criteria. Participants (n = 67) included clinicians, researchers, lay persons with experience of childbirth (patient representatives), and other stakeholders. A consensus meeting was held to reach a final agreement.Results: Response rates were 82.1% (55/67) and 92.7% (51/55) for the first and second Delphi rounds, respectively. In total, 17 outcomes were included in the final core outcome set, reflecting mortality, health or morbidity, including asphyxia, central nervous system status, infection, neonatal resuscitation and admission, breastfeeding and mother-infant interaction, operative delivery due to fetal distress, as well as birthweight and gestational age. Two of these outcomes were suggested by patient representatives.Conclusions: The Swedish Perinatal Core Outcome Set (SPeCOS) study involved a broad circle of relevant stakeholders and reached consensus on a minimal set of perinatal outcomes that should be collected and reported in a standardized way in all future studies on management of labor and delivery at or near term, regardless of the specific population or condition studied. This could improve obstetric research, evidence synthesis, uptake, implementation, and adherence, as well as clinical practice, audit, and comparisons in childbirth care.

    • Alveolar capillary dysplasia with misalignment of pulmonary veins (ACD/MPV) is a rare and lethal developmental disorder of the lung defined by a constellation of characteristic histopathological features. Nonpulmonary anomalies involving organs of gastrointestinal, cardiovascular, and genitourinary systems have been identified in approximately 80% of patients with ACD/MPV. We have collected DNA and pathological samples from more than 90 infants with ACD/MPV and their family members. Since the publication of our initial report of four point mutations and 10 deletions, we have identified an additional 38 novel nonsynonymous mutations of FOXF1 (nine nonsense, seven frameshift, one inframe deletion, 20 missense, and one no stop). This report represents an up to date list of all known FOXF1 mutations to the best of our knowledge. Majority of the cases are sporadic. We report four familial cases of which three show maternal inheritance, consistent with paternal imprinting of the gene. Twenty five mutations (60%) are located within the putative DNA-binding domain, indicating its plausible role in FOXF1 function. Five mutations map to the second exon. We identified two additional genic and eight genomic deletions upstream to FOXF1. These results corroborate and extend our previous observations and further establish involvement of FOXF1 in ACD/MPV and lung organogenesis.

However, measuring and evaluating the relevant properties in the thickness direction of paperboard are not necessarily straightforward or easy. Measuring at relevant, millisecond deformation rates further complicates the problem. The aim of the present work is to elucidate some of the influences on the compressive stiffness. Both the initial material response and the overall compressibility of the paperboard is studied. In this project, the effect on the material response from the surface structure and the millisecond timescale recovery is explored. The method utilized is a machine called the Rapid ZD-tester. The device drops a probe in freefall on the substrate and records the probe position, thus acquiring the deformation of the substrate. The probe is also allowed to bounce several times on the surface for consecutive impacts before being lifted for the next drop. To investigate the time dependent stiffness behavior, the probe is dropped several times at the same XY position on the paperboard from different heights, thus achieving different impact velocities. The material response from drops and bounces combined allows study of the short-term recovery of the material. The material in the study is commercial paperboard. The paperboard samples are compared to material where the surface has been smoothed by grinding it. Our study shows that there is a non-permanent reduction in thickness and a stiffening per bounce of the probe, indicating a compaction that has not recovered in the millisecond timescale. Additionally, a higher impact velocity has an initial stiffening effect on the paperboard, and this is reduced by smoothing the surface.
