    • BACKGROUND: Generalised anxiety disorder (GAD) is a frequent and severe disorder among older adults. For older adults with GAD the effect of the recommended treatment, cognitive behaviour therapy (CBT), is reduced. Physical exercise (PE) may enhance the effect of CBT by improving cognitive function and increasing levels of brain-derived neurotrophic factor (BDNF), a predictor of the effect of CBT in patients with anxiety. The aim of the study was to assess the feasibility of a randomized controlled trial (RCT) investigating treatment effect of the combination of CBT and PE for GAD in a sample of older adults, including procedures for assessment and treatment.METHODS: Four participants aged 62-70 years (M = 65.5, SD = 3.2) with a primary diagnosis of GAD were included. Participants received 15 weeks of PE in combination with 10 weeks of CBT. Participants completed self-report measures, and clinical, biological, physiological and neuropsychological tests at pre-, interim- and post-treatment.RESULTS: Procedures, protocols, and results are presented. One participant dropped out during treatment. For the three participants completing, the total adherence to PE and CBT was 80% and 100%, respectively. An independent assessor concluded that the completers no longer fulfilled the criteria for GAD after treatment. Changes in self-report measures suggest symptom reduction related to anxiety and worry. The sample is considered representative for the target population.CONCLUSIONS: The results indicate that combining CBT and PE for older adults with GAD is feasible, and that the procedures and tests are suitable and manageable for the current sample.TRIAL REGISTRATION: ClinicalTrials.gov, NCT02690441.     • Distinct tissue-specific mechanisms mediate insulin action in fasting and postprandial states. Previous genetic studies have largely focused on insulin resistance in the fasting state, where hepatic insulin action dominates. Here we studied genetic variants influencing insulin levels measured 2 h after a glucose challenge in >55,000 participants from three ancestry groups. We identified ten new loci (P < 5 × 10-8) not previously associated with postchallenge insulin resistance, eight of which were shown to share their genetic architecture with type 2 diabetes in colocalization analyses. We investigated candidate genes at a subset of associated loci in cultured cells and identified nine candidate genes newly implicated in the expression or trafficking of GLUT4, the key glucose transporter in postprandial glucose uptake in muscle and fat. By focusing on postprandial insulin resistance, we highlighted the mechanisms of action at type 2 diabetes loci that are not adequately captured by studies of fasting glycemic traits.

    • Acreo has a long tradition of working with quantum structure based infrared (IR) detectors and arrays. This includes QWIP (quantum well infrared photodetector), QDIP (quantum dot infrared photodetector), and InAs/GaInSb based photon detectors of different structure and composition. It also covers R&D on uncooled microbolometers. The integrated thermistor material of such detectors is advantageously based on quantum structures that are optimised for high temperature coefficient and low noise. Especially the SiGe material system is preferred due to the compatibility with silicon technology. The R&D work on IR detectors is a prominent part of Acreo's centre of excellence "IMAGIC" on imaging detectors and systems for non-visible wavelengths. IMAGIC is a collaboration between Acreo, several industry partners and universities like the Royal Institute of Technology (KTH) and Linkoping University. (C) 2010 Elsevier B.V. All rights reserved.

    • Insulin secretion is critical for glucose homeostasis, and increased levels of the precursor proinsulin relative to insulin indicate pancreatic islet beta-cell stress and insufficient insulin secretory capacity in the setting of insulin resistance. We conducted meta-analyses of genome-wide association results for fasting proinsulin from 16 European-ancestry studies in 45,861 individuals. We found 36 independent signals at 30 loci (p value < 5 × 10-8), which validated 12 previously reported loci for proinsulin and ten additional loci previously identified for another glycemic trait. Half of the alleles associated with higher proinsulin showed higher rather than lower effects on glucose levels, corresponding to different mechanisms. Proinsulin loci included genes that affect prohormone convertases, beta-cell dysfunction, vesicle trafficking, beta-cell transcriptional regulation, and lysosomes/autophagy processes. We colocalized 11 proinsulin signals with islet expression quantitative trait locus (eQTL) data, suggesting candidate genes, including ARSG, WIPI1, SLC7A14, and SIX3. The NKX6-3/ANK1 proinsulin signal colocalized with a T2D signal and an adipose ANK1 eQTL signal but not the islet NKX6-3 eQTL. Signals were enriched for islet enhancers, and we showed a plausible islet regulatory mechanism for the lead signal in the MADD locus. These results show how detailed genetic studies of an intermediate phenotype can elucidate mechanisms that may predispose one to disease.

    • We report on the design and fabrication of (Al)GaAs(Sb)/InAs tensile strained quantum-dot (QD) based detector material for thermal infrared imaging applications in the long-wavelength infrared (LWIR) regime. The detection is based on transitions between confined dot states and continuum states in a type-II band lineup, and we therefore refer to it as a dot-to-bulk (D2B) infrared photodetector with expected benefits including long carrier lifetime due to the type-II band alignment, suppressed Shockley-Read-Hall generation-recombination due to the relatively large-bandgap matrix material, inhibited Auger recombination processes due to the tensile strain and epitaxial simplicity. Metal-organic vapor-phase epitaxy was used to grow multiple (Al)GaAs(Sb) QD layers on InAs substrates at different QD nominal thicknesses, compositions, doping conditions and multilayer periods, and the material was characterized using atomic force and transmission electron microscopy, and Fourier-transform infrared absorption spectroscopy. Dot densities up to 1 x 10(11) cm(-2), 1 x 10(12) cm(-2) and 3 x 10(10) cm(-2) were measured for GaAs, AlGaAs and GaAsSb QDs, respectively. Strong absorption in GaAs, AlGaAs and GaAsSb multilayer QD samples was observed in the wavelength range 6-12 mu m. From the wavelength shift in the spectral absorption for samples with varying QD thickness and composition it is believed that the absorption is due to an intra- valance band transition. From this it is possible to estimate the type-II inter-band transition wavelength, thereby suggesting that (Al)GaAs(Sb) QD/InAs heterostructures are suitable candidates for LWIR detection and imaging.

    • Historically IRnova has exclusively been a company, focused on manufacturing of QWIP detectors. Nowadays, besides continuous improvements of the performance of QWIP FPAs and development of new formats IRnova is involved in development of QWIP detectors for special applications and has started the development of the next generation infrared detectors, as well. In the light of the development of new formats we validate experimentally theoretical calculations of the response of QWIPs for smaller pixel size. These results allow for the development of high performance megapixel QWIP FPA that exhibit the high uniformity and operability QWIP detectors are known for. QWIP is also being considered for space applications. The requirements on dark current and operating temperature are however much more stringent as compared to the terrestrial applications. We show ways to improve the material quality with as a result a higher detector operating temperature. IRnova is also looking at antimony-based strained superlattice material for the LWIR region together with partners at theIMAGIC centre of excellence. One of the ways to overcome the problem with surface currents is passivating overgrowth. We will report the status and results of overgrowing the detector mesas with AlGa(As)Sb in a MOVPE system. At the same centre of excellence a novel material concept is being developed for LWIR detection. This new material contains a superlattice of vertically aligned and electronically coupled InAs and GaSb quantum dots. Simulations show that it should be possible to have LWIR detection in this material. We will present the current status and report results in this research.

The aim of the current research project is to investigate whether augmenting CBT with physical exercise will lead to improved effects of CBT on GAD in older adults in a randomized controlled trial (RCT). Participants between 60-75 years of age with a primary diagnosis of GAD will be randomised to one of two treatment conditions. The effects of treatment will be assessed on outcome measures, biological, physiological and cognitive measures at pre- interim-, and post-treatment, and follow-up assessments at 6- and 12-months post intervention. Participants in both groups will receive five weeks of pre-treatment intervention consisting of either physical exercise or weekly telephone contact. Participants thereafter receive either ten weeks of manualised CBT for GAD combined with manualised physical exercise or ten weeks of manualised CBT for GAD combined with weekly telephone contact. We expect that the treatment effect of the physical exercise augmented CBT will be greater than that of CBT combined with weekly telephone contact, as measured by a reduction in GAD symptoms on the Penn State Worry Questionnaire and in the proportion of remitted patients. The study also aims to determining the possible beneficial and augmenting properties of physical exercise in combination with CBT, and our understanding of clinical characteristics of GAD and mechanisms involved in treatment effect. Treatment rationale, procedures and protocols will be presented in detail together with preliminary results from the initial feasibility study comprises eight participants

    • The realization of silicon-based light sources has been the subject of a major research and development effort worldwide. Such sources may help make integrated photonic and electronic circuitry more cost-effective, with higher performance and greater energy efficiency. The hybrid approach, in which silicon is integrated with a III-V gain medium, is an attractive route in the development of silicon lasers because of its potential for high efficiency. Hybrid lasers with good performance have been reported that are fabricated by direct growth or direct wafer-bonding of the gain medium to silicon. Here, we report a membrane reflector surface-emitting laser on silicon that is based on multilayer semiconductor nanomembrane stacking and a stamp-assisted transfer-printing process. The optically pumped laser consists of a transferred III-V InGaAsP quantum-well heterostructure as the gain medium, which is sandwiched between two thin, single-layer silicon photonic-crystal Fano resonance membrane reflectors. We also demonstrate high-finesse single-or multiwavelength vertical laser cavities.

    • Flexible photodetectors were demonstrated experimentally on large-area crystalline silicon nanomembranes (3 mm×3 mm), based on wet transfer and metal-frame supported transfer processes. Very low dark current (a few nA) and linear photoresponses were demonstrated for both Si MSM and InP PIN photodiodes on flexible PET substrates.

InP nanomembranes as large as 2cm×2cm was transferred successfully. Large area flexible photodetectors, solar cells and LED arrays all have been demonstrated experimentally, based on transferred InP nanomembranes.

    • BACKGROUND: Dyslipidaemia commonly coexists with diabetes. We investigated the association of lipid profiles with glucose levels in populations of Asian origin without a prior history of diabetes. METHODS: Cross-sectional data of 10,374 men and 12,552 women aged 30-74 years from 14 cohorts, representing seven populations of Asian origin were jointly analysed. Multivariable adjusted linear regression analyses with standardized regression coefficients (beta) were performed to estimate relationships between lipids and plasma glucose. RESULTS: Within each glucose category, fasting plasma glucose (FPG) levels were correlated with increasing levels of triglycerides (TGs), total cholesterol (TC), TC to high-density lipoprotein (HDL) ratio and non-HDL cholesterol (non-HDL-C) (p < 0.05 in most of the ethnic groups) and inversely associated with HDL-C (p < 0.05 in some, but not all, of the populations). The association of lipids with 2-h plasma glucose (2hPG) followed a similar pattern as that for the FPG, except that an inverse relationship between HDL-C and glucose was more commonly observed for 2hPG than for FPG among different ethnic groups. CONCLUSIONS: Hyperglycaemia is associated with adverse lipid profiles in Asians without a prior history of diabetes. The 2hPG appears to be more closely associated with lipid profiles than does FPG. When assessing the risk of cardiovascular disease, the association of the dyslipidaemia with intermediate hyperglycaemia needs to be considered.

    • Objectives: The aim of the study was to explore the role of GLP-1 receptor activation on the counter-regulation and symptoms of hypoglycemia in subjects who have undergone gastric bypass surgery (GBP).Design: Experimental hyperinsulinemic–hypoglycemic clamp study.Methods: Twelve post-GBP subjects participated in a randomized cross-over study with two hyperinsulinemic, hypoglycemic clamps (glucose nadir 2.7 mmol/L) performed on separate days with concomitant infusions of the GLP-1 analog exenatide or with saline, respectively. Continuous measurements of metabolites and counter-regulatory hormones as well as assessments of heart rate variability and symptoms of hypoglycemia were performed throughout the clamps.Results: No effect of GLP-1 receptor activation on counter-regulatory hormones (glucagon, catecholamines, cortisol, GH) or glucose infusion rate was seen, but we found indications of a downregulation of the sympathetic relative to the parasympathetic nerve activity, as reflected in heart rate variability. No significant differences in symptom of hypoglycemia were observed.Conclusions/interpretation: Short-term exposure to a GLP-1 receptor agonist does not seem to impact the counter-regulatory hormonal and metabolic responses in post-GBP subjects during hypoglycemic conditions, suggesting that the improvement in symptomatic hypoglycemia post-GBP seen following treatment with GLP-1 receptor agonists may be mediated by mechanism not directly involved in counter-regulation.

  • A framework for regulating underwater noise during pile driving

    • Pile driving is a common technique used during the construction of bridges, offshore wind power, and underwater infrastructure or shoreline structures. It is the process by which a foundation, beam or pole is hammered or vibrated down into the bottom, which can generate extremely loud noise that propagates throughout the surrounding water and sediment. The noise can reach such high levels that marine animals are at risk of disturbance, injury or even death.Sweden currently lacks established thresholds stating the level at which underwater noise potentially disturbs or injures marine animals. Hence, there are no guidance values for allowable underwater noise levels from noiseproducing activities to avoid serious environmental impacts. Several countries in Europe have defined thresholds for when underwater noise can result in severe negative environmental impacts as well as standards for measuring, analysing and reporting underwater noise levels.The purpose of this study is to review the scientific literature on underwater noise from pile driving and its effects on marine life. The study aims to define the noise levels that can cause injury and other negative effects and, on this basis, recommend noise levels that can be used to establish guidance values for regulating underwater noise for Swedish waters and species. The study presents examples of the factors that contribute to sound propagation in Swedish waters and how this influences the noise level from a pile strike as a function of distance at four study areas along the Swedish coast. Additionally, the study contains a thorough technical description of pile driving activities, basic underwater acoustics and noise effects on marine animals. These effects (injury and behavioral, e.g., flight, but not subtle effects) are demonstrated on representative species such as the harbour porpoise (Phocoena phocoena), Atlantic cod (Gadus morhua), Atlantic herring (Clupea harengus) and on fish larvae and eggs. The study’s authors look atthe original sources of information that other countries base their guidelines and thresholds on, so the recommendations follow scientifically determined levels rather than values that have been rounded off or otherwise altered.The study presents sound levels in three different units, each with different biological relevance to the effects caused by a pile driving activity. None of the sound levels have been frequency weighted for a specific species, as this method is not yet fully established. The first unit used is the sound pressure level SPL(peak), which is the maximum overpressure or underpressure of the noise pulse generated by the pile strike. This unit has a high relevance for behavioural effects. The sound exposure level, SEL, is the calculated energy level over a period of time and expresses the energy of the entire sound pulse. SEL is the unit most related to hearing impairing effects. SEL(ss) is the value for a single strike while SEL(cum) is the cumulative value of a determined number of pulses over a period of time. The review revealed that for Atlantic cod and Atlantic herring there are currently no studies that can be used to determine a species’ specific threshold value for injury, but studies show that loud noise can affect both species negatively. Because of this, the recommended noise levels for injury are based mainly on studies on other species exposed to pile driving noise in laboratory environments, supported by studies conducting large-scale experiments in tanks and oceans. The levels at which fish are at risk of death or sustaining serious injury to internal organs is SPL 207 dB re 1 μPa, SEL(ss) 174 dB re 1 μPa2s and SEL(cum) 204 dB re 1 μPa2s. Note that for injury in fish, the cumulative sound exposure level has higher relevance than the single-strike level as the cited studies found injuries after a certain time period of exposure. The thresholds for fish larvae and eggs are based on the fact that no negative effects were observed at exposures of up to SPL(peak) 217 dB re 1 μPa, SEL(ss) 187 dB re 1 μPa2s and SEL(cum) 207 dB re 1 μPa2s. However, there are relatively few studies on early life stages of fish. There are more species-specific studies on harbour porpoises regarding noise than there are for Atlantic cod and Atlantic herring. Nonetheless, only a few can be used to determine thresholds that will lead to injury or negative behavioural effects. The levels at which there is a risk of a temporary impact on hearing, i.e. temporary threshold shift (TTS), for the harbour porpoises is SPL(peak) 194 dB re 1 μPa, SEL(ss) 164 dB re 1 μPa2s and SEL(cum) 175 dB re 1 μPa2s. When it comes to TTS, the cumulative sound exposure level, SEL(cum), is of primary importance. However, this unit is dependent on a specific time and number of pulses. For permanent threshold shift (PTS), the level is set to SPL(peak) 200 dB re 1 μPa, SEL(ss) 179 dB re 1 μPa2s and SEL(cum) 190 dB re 1 μPa2s. The recommended level should be revised as new relevant studies are conducted.

    • Vid byggnation av broar, havsbaserad vindkraft och andra havsbaserade eller strandnära konstruktioner används det oftast någon form av pålningsteknik för att få ner konstruktionen i botten. Detta innebär att ett fundament, balk eller spont hamras eller vibreras ner i botten, vilket kan generera mycket höga ljudnivåer som sprids ut i vattnet och ner i botten. Ljudnivåerna är så höga att marina organismer kan störas, skadas eller till och med dödas. Idag saknar Sverige fastställda ljudnivåer för när undervattensbuller blir så högt att de kan skada djur i havet. Det saknas därför också vedertagna begränsningsvärden som anger vilka nivåer av undervattensbuller som kan tillåtas för bullrande aktiviteter utan att riskera allvarliga miljöeffekter. Flera länder i Europa har någon form av gränsvärden för när bullernivån under vattnet kan ge upphov till allvarlig miljöpåverkan liksom standarder för hur undervattensbuller skall mätas och rapporteras. Syftet med denna studie var att ta fram ett vetenskapligt underlag rörande ljudet från pålning i havet och dess påverkan på det marina livet. Slutmålet var att utifrån den vetenskapliga information som finns idag ge förslag på ljudnivåer för skador och negativ påverkan som sedan kan användas för att ta fram begränsningsvärden för reglering av undervattensbuller anpassade för svenska vatten och arter. Studien ger ett antal exempel på vilka faktorer som påverkar ljudutbredningen i svenska vatten och hur detta påverkar ett pålningsslags ljudnivå som funktion av avstånd i fyra typområden kring den svenska kusten. Vidare presenteras ingående både tekniska beskrivningar av pålningsaktiviteter, undervattensakustik samt påverkan på marina djur. Denna påverkan (skada och flyktbeteende men ej subtila effekter) demonstreras med hjälp av ett antal typarter som tandvalen tumlare (Phocoena phocoena) och fiskarterna torsk (Gadus morhua) och sill (Clupea clupea) samt fiskägg och fisklarver. I denna studie har författarna gått tillbaka till de originalkällor av information som andra länders gränsvärden grundas på, så att rekommendationerna bygger på vetenskapliga nivåer och inte värden som har avrundats eller på annat sätt ändrats.Studien presenterar ljudnivåer i tre olika enheter då dessa har olika biologisk relevans för påverkan från en pålningsaktivitet. Inga av dessa värden har frekvensviktats för att anpassas för en specifik art då denna metod ännu inte är helt vedertagen. Den första enheten är ljudtrycksnivå SPL(topp), d.v.s. det maximala över- eller undertryck som den av pålningsslaget genererade ljudpulsen har. Denna enhet har hög relevans för beteendepåverkan. För ljudexponeringsnivå SEL, beräknas ljudnivån över en viss tid och tar då med energin i hela ljudpulsen. SEL är den enhet som visats vara bäst relaterad till hörselskador. SEL(enkel) är värdet för en enkel puls och för det kumulativa SEL(kum) har antalet pulser under en viss tid summerats.Litteraturstudien på torsk och sill visar att det i dagsläget inte finns några studier som kan användas för att fastställa en artspecifik ljudnivå för skada men litteraturen visar tydligt på att höga bullernivåer kan påverka torsk och sill negativt. Istället baseras de föreslagna nivåerna i huvudsak på studier på andra arter som har exponerats för pålningsljud i laboratoriemiljö med stöd av studier från mer storskaliga experiment i tankar och hav. De nivåer då fisk riskerar att dödas eller få allvarliga skador på inre organ är 207 dB re 1 μPa SPL(topp), 174 dB re 1 μPa2s SEL(enkel) och 204 dB re 1 μPa2s SEL(kum). Notera att för skada på fisk har det kumulativa värdet högre relevans än enkelvärdet för SEL eftersom studier visar att skador uppkommer efter en viss tids exponering. Nivåerna för påverkan på fiskägg och larver grundas i att inga negativa effekter har observerats vid exponering för ljudtryck från pålning upp till 217 dB re 1 μPa SPL(topp), 187 dB re 1 μPa2s SEL(enkel) och 207 dB re 1 μPa2s SEL(kum). Det finns emellertid mycket få studier relaterat till pålningsljud för dessa livsstadier. För tumlare finns det fler artspecifika studier gjorda relaterat till buller än för torsk och sill. Det är dock endast ett fåtal som kan användas för att bestämma ljudnivåer som leder till skada eller negativ beteendepåverkan. De ljudnivåer som riskerar ge tillfällig hörselnedsättning (TTS) hos tumlare är 194 dB re 1 μPa SPL(topp), 164 dB re 1 μPa2s SEL(enkel) och 175 dB re 1 μPa2s SEL(kum). Det är framförallt den kumulativa ljudexponeringsnivån SEL(kum) som har stor betydelse för just TTS, dock hänger detta värde ihop med en specifik tid och antalet pulser vilket kan vara svårt att uppskatta i förväg. Vidare avseende permanent hörselskada (PTS) är ljudnivån 200 dB re 1 μPa SPL(topp), 179 dB re 1 μPa2s SEL(enkel) och 190 dB re 1 μPa2s SEL(kum). Föreslagna nivåer bör uppdateras när nya relevanta forskningsstudier tillkommer.

    • Inflammation is a well-documented driver of cancer development and progression. However, little is known about its role in prostate carcinogenesis. Thus, we examined the association of C-reactive protein (CRP), haptoglobin, albumin and white blood cells (WBC) with prostate cancer (PCa) severity (defined by PCa risk category and clinicopathological characteristics) and progression (defined by PCa death). We selected 8,471 Swedish men with newly diagnosed PCa who had exposure measurements taken approximately 14 years prior to diagnosis. We calculated odds ratio (OR) and 95% confidence interval (CI) for the associations between the inflammatory markers and PCa severity using logistic regression, while Cox proportional hazard regression was used for the associations with overall and PCa death. Serum CRP levels were associated with increased odds of high risk and metastatic PCa, and high PSA levels (20 mu g/L) (OR: 1.29; 95% CI: 1.06-1.56, 1.32; 1.05-1.65 and 1.51; 1.26-1.81, respectively). Similarly, higher haptoglobin levels were associated with increased odds of metastatic PCa, high PSA level and possibly high grade PCa (1.38; 1.10-1.74, 1.50; 1.17-1.93 and 1.25; 1.00-1.56, respectively). Albumin was positively associated with Gleason 4+3 tumour (1.38; 1.02-1.86) and overall death (HRunit increase in log: 1.60; 95% CI: 1.11-2.30), but inversely associated with high risk PCa and high PSA levels (20 mu g/L) (0.71; 0.56-0.89 and 0.72; 0.5 9-0.90). WBC was associated with increased odds of T3-T4 PCa. Except for albumin, none of these markers were associated with PCa death or overall death. Systemic inflammation as early as 14 years prior to diagnosis may influence prostate cancer severity. What's new? High levels of C-reactive protein can presage a particularly malignant prostate cancer, new results show. Cancers certainly arise in the wake of chronic inflammation, but it's not known exactly how markers of inflammation initiate prostate cancer. Here, the authors show that systemic inflammation can worsen the severity of the cancer, even if it occurred long before the cancer's onset. High levels of CRP and haptoglobin, they found, were associated with prostate cancer with high PSA and metastasis. The question remains whether inflammation pushes cancer cells into a more malignant mode, or selects for the more dangerous cells early on.

    • We report on the application of cross-sectional Scanning Capacitance Microscopy (SCM) for studying two-dimensional doping variations in Si and InP device structures. Different sample preparation methods were evaluated and the response of the SCM signal from various test structures, including epitaxially grown layers with n- and p-doping concentrations ranging from 5 × 1014 to 2 × 1019 cm-3, were examined under different imaging conditions. The technique was further evaluated by imaging a Si bipolar transistor structure and an InP-based buried heterostructure diode laser. We conclude that valuable information can be gained also from complex device structures.
