    • Background: Measurement of changes in health across locations is useful to compare and contrast changing epidemiological patterns against health system performance and identify specific needs for resource allocation in research, policy development, and programme decision making. Using the Global Burden of Diseases, Injuries, and Risk Factors Study 2016, we drew from two widely used summary measures to monitor such changes in population health: disability-adjusted life-years (DALYs) and healthy life expectancy (HALE). We used these measures to track trends and benchmark progress compared with expected trends on the basis of the Socio-demographic Index (SDI). Methods: We used results from the Global Burden of Diseases, Injuries, and Risk Factors Study 2016 for all-cause mortality, cause-specific mortality, and non-fatal disease burden to derive HALE and DALYs by sex for 195 countries and territories from 1990 to 2016. We calculated DALYs by summing years of life lost and years of life lived with disability for each location, age group, sex, and year. We estimated HALE using age-specific death rates and years of life lived with disability per capita. We explored how DALYs and HALE difered from expected trends when compared with the SDI: the geometric mean of income per person, educational attainment in the population older than age 15 years, and total fertility rate. Findings: The highest globally observed HALE at birth for both women and men was in Singapore, at 75·2 years (95% uncertainty interval 71·9-78·6) for females and 72·0 years (68·8-75·1) for males. The lowest for females was in the Central African Republic (45·6 years [42·0-49·5]) and for males was in Lesotho (41·5 years [39·0-44·0]). From 1990 to 2016, global HALE increased by an average of 6·24 years (5·97-6·48) for both sexes combined. Global HALE increased by 6·04 years (5·74-6·27) for males and 6·49 years (6·08-6·77) for females, whereas HALE at age 65 years increased by 1·78 years (1·61-1·93) for males and 1·96 years (1·69-2·13) for females. Total global DALYs remained largely unchanged from 1990 to 2016 (-2·3% [-5·9 to 0·9]), with decreases in communicable, maternal, neonatal, and nutritional (CMNN) disease DALYs ofset by increased DALYs due to non-communicable diseases (NCDs). The exemplars, calculated as the fve lowest ratios of observed to expected age-standardised DALY rates in 2016, were Nicaragua, Costa Rica, the Maldives, Peru, and Israel. The leading three causes of DALYs globally were ischaemic heart disease, cerebrovascular disease, and lower respiratory infections, comprising 16·1% of all DALYs. Total DALYs and age-standardised DALY rates due to most CMNN causes decreased from 1990 to 2016. Conversely, the total DALY burden rose for most NCDs; however, age-standardised DALY rates due to NCDs declined globally. Interpretation: At a global level, DALYs and HALE continue to show improvements. At the same time, we observe that many populations are facing growing functional health loss. Rising SDI was associated with increases in cumulative years of life lived with disability and decreases in CMNN DALYs ofset by increased NCD DALYs. Relative compression of morbidity highlights the importance of continued health interventions, which has changed in most locations in pace with the gross domestic product per person, education, and family planning. The analysis of DALYs and HALE and their relationship to SDI represents a robust framework with which to benchmark location-specific health performance. Country-specific drivers of disease burden, particularly for causes with higher-than-expected DALYs, should inform health policies, health system improvement initiatives, targeted prevention eforts, and development assistance for health, including fnancial and research investments for all countries, regardless of their level of sociodemographic development. The presence of countries that substantially outperform others suggests the need for increased scrutiny for proven examples of best practices, which can help to extend gains, whereas the presence of underperforming countries suggests the need for devotion of extra attention to health systems that need more robust support. © The Author(s).     • BACKGROUND: Timely assessment of the burden of HIV/AIDS is essential for policy setting and programme evaluation. In this report from the Global Burden of Disease Study 2015 (GBD 2015), we provide national estimates of levels and trends of HIV/AIDS incidence, prevalence, coverage of antiretroviral therapy (ART), and mortality for 195 countries and territories from 1980 to 2015.METHODS: For countries without high-quality vital registration data, we estimated prevalence and incidence with data from antenatal care clinics and population-based seroprevalence surveys, and with assumptions by age and sex on initial CD4 distribution at infection, CD4 progression rates (probability of progression from higher to lower CD4 cell-count category), on and off antiretroviral therapy (ART) mortality, and mortality from all other causes. Our estimation strategy links the GBD 2015 assessment of all-cause mortality and estimation of incidence and prevalence so that for each draw from the uncertainty distribution all assumptions used in each step are internally consistent. We estimated incidence, prevalence, and death with GBD versions of the Estimation and Projection Package (EPP) and Spectrum software originally developed by the Joint United Nations Programme on HIV/AIDS (UNAIDS). We used an open-source version of EPP and recoded Spectrum for speed, and used updated assumptions from systematic reviews of the literature and GBD demographic data. For countries with high-quality vital registration data, we developed the cohort incidence bias adjustment model to estimate HIV incidence and prevalence largely from the number of deaths caused by HIV recorded in cause-of-death statistics. We corrected these statistics for garbage coding and HIV misclassification.FINDINGS: Global HIV incidence reached its peak in 1997, at 3·3 million new infections (95% uncertainty interval [UI] 3·1-3·4 million). Annual incidence has stayed relatively constant at about 2·6 million per year (range 2·5-2·8 million) since 2005, after a period of fast decline between 1997 and 2005. The number of people living with HIV/AIDS has been steadily increasing and reached 38·8 million (95% UI 37·6-40·4 million) in 2015. At the same time, HIV/AIDS mortality has been declining at a steady pace, from a peak of 1·8 million deaths (95% UI 1·7-1·9 million) in 2005, to 1·2 million deaths (1·1-1·3 million) in 2015. We recorded substantial heterogeneity in the levels and trends of HIV/AIDS across countries. Although many countries have experienced decreases in HIV/AIDS mortality and in annual new infections, other countries have had slowdowns or increases in rates of change in annual new infections.INTERPRETATION: Scale-up of ART and prevention of mother-to-child transmission has been one of the great successes of global health in the past two decades. However, in the past decade, progress in reducing new infections has been slow, development assistance for health devoted to HIV has stagnated, and resources for health in low-income countries have grown slowly. Achievement of the new ambitious goals for HIV enshrined in Sustainable Development Goal 3 and the 90-90-90 UNAIDS targets will be challenging, and will need continued efforts from governments and international agencies in the next 15 years to end AIDS by 2030.

    • The width of spiral-patterned zinc oxide (ZnO) nanorod coatings on plastic optical fiber (POF) was optimized theoretically for light-side coupling and found to be 5 mm. Structured ZnO nanorods were grown on large core POFs for the purpose of alcohol vapor sensing. The aim of the spiral patterns was to enhance signal transmission by reduction of the effective ZnO growth area, thereby minimizing light leakage due to backscattering. The sensing mechanism utilized changes in the output signal due to adsorption of methanol, ethanol, and isopropanol vapors. Three spectral bands consisting of red (620 to 750 nm), green (495 to 570 nm), and blue (450 to 495 nm) were applied in measurements. The range of relative intensity modulation (RIM) was determined to be for concentrations between 25 to 300 ppm. Methanol presented the strongest response compared to ethanol and isopropanol in all three spectral channels. With regard to alcohol detection RIM by spectral band, the green channel demonstrated the highest RIM values followed by the blue and red channels, respectively.

    • Controlled light coupling from surrounding to the cladding mode of zinc oxide (ZnO) nanorod coated multimode optical fiber induced by the light scattering properties of the nanorod coating and their applications of sensing are reported here. A dense and highly ordered array of ZnO nanorods is grown on the cladding of silica fibers by using low temperature hydrothermal process and the effect of the hydrothermal growth conditions of the nanorods on the light scattering and coupling to the optical fibers is experimentally investigated. The nanorod length and its number per unit area are found to be most crucial parameters for the optimum side coupling of light into the fibers. Maximum excitation of the cladding mode by side coupling of light is obtained with ZnO nanorods of length similar to 2.2 mu m, demonstrating average coupling efficiency of similar to 2.65%. Upon exposure to different concentrations of various chemical vapors, the nanorod coated fibers demonstrated significant enhancement in the side coupled light intensity, indicating the potential use of these ZnO nanorod coated fibers as simple, low cost and efficient optical sensors. The sensor responses to methanol, ethanol, toluene and benzene vapor were investigated and compared, while the effect of humidity in the sensing environment on the sensor performance was explored as well.

    • A novel concept is introduced that utilizes the scattering properties of zinc oxide nanorods to control light guidance and leakage inside optical fibers coated with nanorods. The effect of the hydrothermal growth conditions of the nanorods on light scattering and coupling to optical fiber are experimentally investigated. At optimum conditions, 5% of the incident light is side coupled to the cladding modes. This coupling scheme could be used in different applications such as distributed sensors and light combing. Implementation of the nanorods on fiber provides low cost and controllable nonlithography-based solutions for free space to fiber coupling. Higher coupling efficiencies can be achieved with further optimization.

    • Side coupling to core modes through zinc oxide (ZnO) nanorods grown around the fiber is demonstrated in this work. The scheme utilizes wet etching of the cladding region followed by hydrothermal growth of the nanorods. The combination of nanostructures and the optical fiber system is used to demonstrate a simple wide field of view (FOV) optical receiver. Core modes are excited by the light scattered in the region where the fiber core is exposed. The angular response of the receiver was tested using a nephlometer. Light coupling efficiency was extracted by deconvoluting the finite beam extinction from the measured power. The results were compared to a first-order analytical model in which the phase function is assumed to linearly shift with the incident angle. The trend of the experimental measurements agrees with the model. 180 degrees FOV is verified, and maximum coupling efficiency of around 2.5% for a single fiber is reported. Excitation of core modes through side coupling shows potential for the application of these devices in optical receivers and sensors.

    • Realization of a simple integrated and low-cost intensity modulation/direct detection-based humidity and vapor detection system utilizing zinc oxide (ZnO) nanorods as the active material is demonstrated. The sensing device comprises of ZnO nanorods optimally grown on a glass substrate and mounted on 3D printed platform for the alignment with a green light-emitting diode setup for an edge excitation. An Arduino platform was used for the signal processing of the detection of the transmitted light. Both forward and backward scattering are affected due to light leakage while propagating through the glass substrate which are further attenuated in the presence of humidity. In this paper, backward scattering was found to be dominant, and thus, with increasing humidity, a reduction in the transmitted light was monitored. When the sensor was tested in a humidity controlled environment, it was found that the output voltage drops by approximately 750 mV upon changing the relative humidity (RH) level from 35% to 90% in a non-linear fashion. The average sensitivity of the sensor was observed to be-12 mV/% throughout the tested RH levels. Sensitivity was found to be higher at-24.6 mV/% for RH's beyond 70%. An average response time of 3.8 s was obtained for RH levels of 85% with respect to the standard ambient humidity conditions (RH 50%), which showed a quicker recovery time of 2.2 s. The proposed sensor device provides numerous advantages, including low-cost production, simplicity in design, ease of use, and stability during handling.

    • Introduction: Ectopic parathyroid adenoma is rare during pregnancy but poses multiple challenges in treatment. It presents as primary hyperparathyroidism which leads to symptoms and complications of hypercalcemia in both the mother and fetus. Presentation of case: A 38-year-old Sudanese female presented with diffuse bone pain and polyuria. Laboratory investigations revealed elevated serum calcium and parathyroid hormone. Ultrasound of the neck did not show any abnormal lesion, however 99mTc-sestamibi scan showed a right sided parathyroid adenoma, and an earlier CT scan showed the adenoma to be in an ectopic paraesophageal position. Focused surgical neck exploration was done, and the ectopic parathyroid adenoma was excised. Discussion: Preoperative localization of the ectopic parathyroid adenoma allows for a focused surgical procedure. Ultrasound is the safest during pregnancy, but 99mTc-sestamibi and CT scan may be necessary if ultrasound or initial bilateral neck exploration do not detect any adenoma. Mild elevations in maternal serum calcium can have detrimental effects on the fetus which suggests that a surgical approach may be necessary in the majority of cases. Conclusions: Ectopic parathyroid adenoma is rare during pregnancy and is detrimental to both the mother and fetus. Preoperative localization allows for a focused surgery which is a definitive treatment and can safely be performed during the 2nd trimester of pregnancy.

    • In this study a 3D printed capsule designed to break from the physiological pressures in the antropyloric region was evaluated for its ability to deliver the synthetic octapeptide octreotide in beagle dogs when co-formulated with the permeation enhancer sodium caprate. The pressure sensitive capsules were compared to traditional enteric coated hard gelatin capsules and enteric coated tablets. Paracetamol, which is completely absorbed in dogs, was included in the formulations and used as an absorption marker to give information about the in vivo performance of the dosage forms. The pressure sensitive capsules released drug in 50% of the dogs. In the cases where drug was released, there was no difference in octreotide bioavailability or C-max compared to the enteric coated dosage forms. When comparing all dosage forms, a correlation was seen between paracetamol C-max and octreotide bioavailability, suggesting that a high drug release rate may be beneficial for peptide absorption when delivered together with sodium caprate.     • We present a global atlas of 4,728 metagenomic samples from mass-transit systems in 60 cities over 3 years, representing the first systematic, worldwide catalog of the urban microbial ecosystem. This atlas provides an annotated, geospatial profile of microbial strains, functional characteristics, antimicrobial resistance (AMR) markers, and genetic elements, including 10,928 viruses, 1,302 bacteria, 2 archaea, and 838,532 CRISPR arrays not found in reference databases. We identified 4,246 known species of urban microorganisms and a consistent set of 31 species found in 97% of samples that were distinct from human commensal organisms. Profiles of AMR genes varied widely in type and density across cities. Cities showed distinct microbial taxonomic signatures that were driven by climate and geographic differences. These results constitute a high-resolution global metagenomic atlas that enables discovery of organisms and genes, highlights potential public health and forensic applications, and provides a culture-independent view of AMR burden in cities.

  • Dispositional and Contextual Antecedents of Individual-level Absorptive Capacity

    • An overwhelming majority of past research has conceptually and empirically treated Absorptive Capacity (AC) at the organizational level of analysis. This exclusive focus on organizational level is problematic given the central role of individuals in developing AC. Adopting micro-foundations perspective, this paper probes into individual-level dispositional antecedents (i.e., "who you are") as well as contextual elements (i.e., "where you are") facilitating motivation for effective implementation of practices and routines that could build up AC. Based on Goal Orientation Theory and Componential Theory of Creativity, we develop and test our hypotheses using a novel dataset collected from individual engineers working at a high-tech manufacturing company. Our results are largely in line with our theoretical predictions.

    • This paper outlines the development of an analytical model of multimodal, multicommodity freight transportation in Indonesia. The model is used to evaluate strategic planning of inter-island freight transportation. The demand for inter-island freight transportation is estimated using econometric demand models which account for the socioeconomic characteristics of the regions and commodity types. The supply side is concerned with modeling the inter-island transportation network, including links, such as the land access network, and transfer points which are connected by the links, and a generalized cost function. System optimization is obtained by identifying the combination of links and transfers that minimize the total generalized transport cost. The model is applied to the evaluation of the impacts of several policy scenarios which contribute to the improvement of the inter-island freight transportation network. The model has shown its capability of estimating system costs of transportation network that can be utilized to assess the policy implications.

  • Multidrug Antimicrobial Resistance and Molecular Detection of mcr-1 Gene in Salmonella Species Isolated from Chicken

    • Colistin (polymyxin E) is widely used in animal and human medicine and is increasingly used as one of the last-resort antibiotics against Gram-negative bacilli. Due to the increased use of colistin in treating infections caused by multidrug-resistant Gram-negative bacteria, resistance to this antibiotic ought to be monitored. The study was undertaken to elucidate the molecular mechanisms, genetic relationships and phenotype correlations of colistin-resistant isolates. Here, we report the detection of the mcr-1 gene in chicken-associated Salmonella isolates in Bangladesh and its in-silico functional analysis. Out of 100 samples, 82 Salmonella spp. were isolated from chicken specimens (liver, intestine). Phenotypic disc diffusion and minimum inhibitory concentration (MIC) assay using different antimicrobial agents were performed. Salmonella isolates were characterized using PCR methods targeting genus-specific invA and mcr-1 genes with validation for the functional analysis. The majority of the tested Salmonella isolates were found resistant to colistin (92.68%), ciprofloxacin (73.17%), tigecycline (62.20%) and trimethoprim/sulfamethoxazole (60.98%). When screened using PCR, five out of ten Salmonella isolates were found to carry the mcr-1 gene. One isolate was confirmed for Salmonella enterica subsp. enterica serovar Enteritidis, and other four isolates were confirmed for Salmonella enterica subsp. enterica serovar Typhimurium. Sequencing and phylogenetic analysis revealed a divergent evolutionary relationship between the catalytic domain of Neisseria meningitidis lipooligosaccharide phosphoethanolamine transferase A (LptA) and MCR proteins, rendering them resistant to colistin. Three-dimensional homology structural analysis of MCR-1 proteins and molecular docking interactions suggested that MCR-1 and LptA share a similar substrate binding cavity, which could be validated for the functional analysis. The comprehensive molecular and in-silico analyses of the colistin resistance mcr-1 gene of Salmonella spp. of chicken origin in the present study highlight the importance of continued monitoring and surveillance for antimicrobial resistance among pathogens in food chain animals.

    • International diversification is a fundamental pillar of multinational corporations' (MNCs) growth strategies. Consequently, there is a considerable body of research on the performance implications of MNCs' international diversification strategies. We extend this literature by adopting a relational view where we compare the diversification profiles of firms in an inter-organizational context. We argue that the relative characteristics of firms' and their partners' diversification profiles is an indicator of parties' resource bases and thereby can explain if and when inter-organizational ties yield optimum performance outcomes. We examine these relative characteristics and propose a conceptual refinement by differentiating between the degree and content dimensions of international diversification. Analyzing data from 202 manufacturing firms from the S&P 500 list, we find that firms achieve optimum performance when their partners have moderately higher degrees of international diversification and a moderate overlap of presence in foreign markets.

    • Research Summary This paper examines an understudied aspect of network relationships-that is, direction of relational ties. Tie direction is important since it can shape when and how firms can benefit from the international experience of other firms. We focus on a specific type of network relationship-that is, interlocking directorates, which provides a clinical context to study directionality. We show that, due to their higher familiarity, identification, and executive power, focal firm directors serving in other firms' boards (i.e., outgoing ties) are more beneficial for utilizing partners' international experience. However, outside directors sitting on the boards of focal firms (i.e., incoming ties) can bring more useful first-hand experience and facilitate international expansion once these ties get stronger. Theoretical and practical implications of these results are discussed. Managerial Summary As they grow internationally, firms need to manage risks and uncertainties of doing business abroad. In this regard, they can potentially benefit from the international experience of other firms in their network. We show how firms can realize these benefits by means of interlocking ties (i.e., shared board memberships). To that end, we examine the directionality of interlocking ties. Specifically, we argue that a firm's ability to utilize partners' experience for its own international expansion is greater when its directors sit on the boards of other firms (so-called outgoing ties) compared to when other firms' directors sit on its own board (so-called incoming ties). However, experience coming through incoming ties is more effective for a firm's international expansion once these ties get stronger.

    • Although received research emphasizes direct experiential knowledge as a key driver of firms’ internationalization, the role of indirect experience has been increasingly recognized in recent studies. In this paper, we extend these studies by examining the role of source and context of experiential knowledge in relation to firms’ internationalization into specific host markets, and offer a fine-grained analysis of when and how indirect experience complement or substitute direct experience. We test our hypotheses with data from 1,478 Swedish SMEs. Our results reveal that a firm can address its knowledge gaps and increase its extent of internationalization into a host market by combining direct and indirect experience, especially when they yield different types of knowledge needed for internationalization. We further show that direct and indirect experience derived from comparable contexts could substitute for each other but may also create knowledge redundancies.

    • An experimental study of the dynamic range maximization with Zinc Oxide (ZnO) nanorods coated glass substrates for humidity and vapor sensing is reported. Growth time of the nanorods and the length of the coated segments were controlled to study the differences between a reference environmental condition (normal humidity or dry condition) and water vapor concentrations. In order to achieve long dynamic range of detection with respect to nanorods coverage, several substrates with triangular patterns of ZnO nanostructures were fabricated by selective hydrothermal growth over different durations of time (5 h, 10 h and 15 h). It was found that maximum dynamic range for the humidity sensing occurs for the combination parameters of normalized length (Z) of 0.23 and normalized scattering coefficient (zeta) of 0.3. A reduction in transmittance by 38% at humidity levels of 80% with reference point as 50% humidity was observed. The results could be correlated to a first order approximation model that assumes uniform growth and the optimum operating conditions for humidity sensing device. This study provides an option to correlate ZnO growth conditions for different vapor sensing applications which can set a platform for compact sensors where modulation of light intensity is followed. rights reserved.
