  • Back Pain and Quality of Life 10 Years After Segmental Pedicle Screw Instrumentation for Adolescent Idiopathic Scoliosis

    • Study Design.Comparative cohort study. Objective.The aim of the present study was to evaluate pain and health-related quality of life (HRQoL) in surgically managed patients with a minimum follow-up of 10 years compared with patients with untreated adolescent idiopathic scoliosis (AIS) and a healthy control group. Summary of Background Data.Posterior spinal fusion with pedicle screws is the standard treatment for AIS, although it remains unclear whether this procedure results in improved long-term HRQoL compared with untreated patients with AIS. Patients and Methods.Sixty-four consecutive patients at a minimum follow-up of 10 years, who underwent posterior pedicle screw instrumentation for AIS were prospectively enrolled. Fifty-three (83%) of these patients completed Scoliosis Research Society (SRS) 24 questionnaires, clinical examination, and standing spinal radiographs. Pain and HRQoL were compared with age and sex-matched patients with untreated AIS and healthy individuals. Results.The mean major curve was 57 degrees preoperatively and 15 degrees at the 10-year follow-up. SRS-24 self-image domain score showed a significant improvement from preoperative to 2 years and remained significantly better at the 10-year follow-up (P < 0.001). Patients fused to L3 or below had lower pain, satisfaction, and total score than patients fused to L2 or above (P < 0.05), but self-image, function, and activity scores did not differ between groups at 10-year follow-up. Pain, self-image, general activity, and total SRS domains were significantly better at 10-year follow-up in the surgically treated patients as compared with untreated patients (all P < 0.05). Healthy controls had significantly higher total scores than those surgically treated at 10-year follow-ups (P < 0.001). Conclusion.Patients undergoing segmental pedicle screw instrumentation for AIS maintain high-level HRQoL during a 10-year follow-up. Their HRQoL was significantly better than in the untreated patients with AIS, except for the function domain. However, HRQoL remained at a lower level than in healthy controls.

  • Surgical and Health-related Quality of life Outcomes in Children With Congenital Scoliosis During 5-year Follow-up. Comparison to Age and Sex-matched Healthy Controls

    • Background:Congenital spinal anomalies represent a heterogeneous group of spinal deformities, of which only progressive or severe curves warrant surgical management. Only a limited number of studies have investigated the impact of surgery on the health-related quality of life and very limited data exists comparing these outcomes to healthy controls.Methods:A single surgeon series of 67 consecutive children with congenital scoliosis (mean age at surgery 8.0 y, range: 1.0 to 18.3 y, 28 girls) undergoing hemivertebrectomy (n = 34), instrumented spinal fusion (n = 20), or vertical expandable prosthetic titanium rib procedure (n = 13) with a mean follow-up of 5.8 years (range: 2 to 13 y). The comparison was made to age and sex-matched healthy controls. Outcome measures included the Scoliosis Research Society questionnaire both pre and postoperatively, radiographic outcomes, and complications.Results:The average major curve correction was significantly better in the hemivertebrectomy (60%) and instrumented spinal fusion (51%) than in the vertical expandable prosthetic titanium rib group (24%), respectively (P < 0.001). Complications were noted in 8 of 67 (12%) children, but all patients recovered fully during follow-up. Pain, self-image, and function domains improved numerically from preoperative to final follow-up, but the pain score was the only one with a statistically significant change (P = 0.033). The Scoliosis Research Society pain, self-image, and function domain scores remained at a significantly lower level at the final follow-up than in the healthy controls (P <= 0.05), while activity scores improved to a similar level.Conclusions:Surgery for congenital scoliosis improved angular spinal deformities with a reasonable risk of complications. Health-related quality of life outcomes improved from preoperative to final follow-up, but especially pain and function domains remained at a significantly lower level than in the age and sex-matched healthy controls.

  • Postoperative outcomes of pedicle screw instrumentation for adolescent idiopathic scoliosis with and without a subfascial wound drain: a multicentre randomized controlled trial

    • AimsThe aim of this study was to evaluate whether, after correction of an adolescent idiopathic scoliosis (AIS), leaving out the subfascial drain gives results that are no worse than using a drain in terms of total blood loss, drop in haemoglobin level, and opioid consumption.MethodsAdolescents (aged between 10 and 21 years) with an idiopathic scoliosis (major curve ≥ 45°) were eligible for inclusion in this randomized controlled noninferiority trial (n = 125). A total of 90 adolescents who had undergone segmental pedicle screw instrumentation were randomized into no-drain or drain groups at the time of wound closure using the sealed envelope technique (1:1). The primary outcome was a drop in the haemoglobin level during first three postoperative days. Secondary outcomes were 48-hour postoperative oxycodone consumption and surgical complications.ResultsAll 90 patients were included in the primary outcome analysis (no drain = 43; drain = 47). The mean total postoperative blood loss (intraoperative and drain output) was significantly higher in the group with a subfascial drain than in the no-drain group (1,008 ml (SD 520) vs 631 ml (SD 518); p < 0.001). The drop in haemoglobin level did not differ between the study groups over the postoperative timepoints (p = 0.290). The 48-hour opioid consumption was significantly higher in the no-drain group (2.0 mg/kg (SD 0.9) vs 1.4 (SD 0.6); p = 0.005). Two patients in the no-drain and one patient in the drain group developed a surgical site infection.ConclusionLeaving the subfascial drain out after pedicle screw instrumentation for AIS is not associated with higher postoperative haemoglobin levels. Patients treated without a subfascial drain needed 30% more opioids during the first 48 hours than those who had a drain.

Some predictions shown here have been updated by including the same experimental cuts as the data. Some additional predictions are also presented, especially for quarkonia, that were provided to the experiments before the data were made public but were too late for the original publication.

    • The Large Hadron Electron Collider (LHeC) project is the proposal to use the existing LHC proton/ion beams and construct a new electron beam line to perform high-energy electron-proton/ion collisions. In this talk, we consider some of the physics topics that could be studied in the electronion mode. In particular, we estimate how much the current nuclear parton distribution fits could be improved with the deeply inelastic scattering measurements at the LHeC by including pseudodata into a global analysis. In addition, we discuss briefly other topics that would help to better understand some aspects of heavy-ion collisions, namely small-x physics and hadron production with a nuclear target.

  • Hard diffraction in photoproduction with Pythia 8

    • We present a new framework for modeling hard diffractive events in photoproduction, implemented in the general purpose event generator Pythia 8. The model is an extension of the model for hard diffraction with dynamical gap survival in p p and p p ¯ collisions proposed in 2015, now also allowing for other beam types. It thus relies on several existing ideas: the Ingelman–Schlein approach, the framework for multiparton interactions and the recently developed framework for photoproduction in γp , γγ, e p and e + e - collisions. The model proposes an explanation for the observed factorization breaking in photoproduced diffractive dijet events at HERA, showing an overall good agreement with data. The model is also applicable to ultraperipheral collisions with p p and p Pb beams, and predictions are made for such events at the LHC.

  • Neutron-skin effect and centrality dependence of high-pT observables in nuclear collisions

    • We report on our studies of the neutron-skin effects in high-pT observables at the LHC. We study the impact of the neutron-skin effect on the centrality dependence of inclusive direct photon, highpT hadron and W± production in nuclear collisions at the LHC. The neutron-skin effect refers to the observation that in spherical heavy nuclei, the tail of the neutron distribution extends farther than the distribution of protons, which can affect observables sensitive to electroweak phenomena in very peripheral collisions. We quantify this effect for direct photons, charged hadrons and W bosons as a function of the collision centrality. In the case of direct photons we find that it will be difficult to resolve the neutron-skin effect, given the uncertainties in the nuclear PDFs and their spatial dependence. With charged hadrons and W's, however, up to 20 % unambiguous effects are expected for most peripheral collisions.

    • A well-established observation in nuclear physics is that in neutron-rich spherical nuclei the distribution of neutrons extends farther than the distribution of protons. In this work, we scrutinize the influence of this so called neutron-skin effect on the centrality dependence of high-pT direct-photon and charged-hadron production. We find that due to the estimated spatial dependence of the nuclear parton distribution functions, it will be demanding to unambiguously expose the neutron-skin effect with direct photons. However, when taking a ratio between the cross sections for negatively and positively charged high-pT hadrons, even centrality-dependent nuclear-PDF effects cancel, making this observable a better handle on the neutron skin. Up to 10% effects can be expected for the most peripheral collisions in the measurable region.

    • An updated analysis regarding the expected nuclear PDF constraints from the future Large Hadron Electron Collider (LHeC) experiment is presented. The new study is based on a more flexible small-x parametrization which provides less biased uncertainty estimates in the region where there are currently no data constraints. The effect of the LHeC is quantified by directly including a sample of pseudodata according to the expected precision of this planned experiment. As a result, a significant reduction of the small-x uncertainties in sea quarks and gluons is observed.

  • Nuclear PDF constraints from p+Pb collisions at the LHC

    • As the current nuclear PDF analyses are mainly constrained by fixed-target Drell-Yan and deeply inelastic scattering data only the quark nuclear modifications at fairly large x values are in a good control. Inclusive pion production in d+Au collisions at RHIC provides some constraints for gluons but due to the limited kinematic reach of the data the gluon modifications remain uncertain especially at small values of x. In this talk, we discuss how the existing data from p+Pb collisions at the LHC can improve the nuclear PDF fits and which measurements would be sensitive to the small-x gluons. In particular, we consider inclusive hadron production, compare this to direct photons, and show estimates of the effect of CMS dijet measurements to the uncertainty of nuclear gluon distributions.

    • The duration of pregnancy is influenced by fetal and maternal genetic and non-genetic factors. Here we report a fetal genome-wide association meta-analysis of gestational duration, and early preterm, preterm, and postterm birth in 84,689 infants. One locus on chromosome 2q13 is associated with gestational duration; the association is replicated in 9,291 additional infants (combined P=3.96×10-14). Analysis of 15,588 mother-child pairs shows that the association is driven by fetal rather than maternal genotype. Functional experiments show that the lead SNP, rs7594852, alters the binding of the HIC1 transcriptional repressor. Genes at the locus include several interleukin 1 family members with roles in pro-inflammatory pathways that are central to the process of parturition. Further understanding of the underlying mechanisms will be of great public health importance, since giving birth either before or after the window of term gestation is associated with increased morbidity and mortality.

    • The present paper discusses the results of an R&D project (fi PULU) in south western Finland to develop a general purpose mobile application framework for elementary and grammar schools with a special emphasis on distributed content creation and a usage model anchored in the convergence of pedagogics and location-based cultural heritage information. Since the beginning of 2001, the LAMB project and its predecessors have studied, developed and evaluated several different technological and content design approaches starting from stand-alone applications on palmtop computers, tablet PCs, and smart phones to modern small light-weight portable tablet computers. The technological focus of the effort has been to develop a simple and robust position-aware multiplatform software application based on a simple general purpose data model based on locations. The Java application designed for the LAMB project can be run on diverse portable computers and used by a group of pupils together. Interactions with the application are recorded on the device and the teacher can afterwards evaluate pupil input and device use data. The software is based on freely available and Open Source components. An important aspect of the system is that it can be updated and maintained on site by the local staff without extensive technical knowledge.
