  • What is best for Esther? What Canada can learn from the Swedish health care service

    • Health care systems in Canada and Sweden are facing increasing challenges to do with access and wait times for non-emergent issues. Both are seeing the erosion of single-payer health care systems as a consequence of these challenges, and both are emphasizing moving care out of hospitals into community and home care. Sweden has moved ahead in this area and Canadians can learn two key lessons from its experience. First, rather than thinking in terms of what’s best for the system or the provider, the Jönköping region in Sweden emphasizes the perspective of “Esther,” a hypothetical patient with a life beyond the walls of the institution. Second, viewing patients as copilots in their care has patients learning treatments, such as selfdialysis, with benefits to themselves and the system.

Men hur styr man en verksamhet som drivs av berättelser, lyssnande, överenskommelser och dokumentationer? Är det styrning underifrån, ovanifrån eller genom partnerskap? Det är en utmaning att leda en verksamhet som involverar patienten som aktiv partner och en del av vårdteamet.

The purpose is to identify factors that enable the development of macro level PMsystems in health care. Based on a case study in nine top management teams in Swedishhealth care, four enabling factors are identified: building an infrastructure, using tools forvisualizing measures, selecting useful macro level measures, and expressing the purpose asa means to improve clinical practice rather than the PM system per se.Despite the increased interest for performance measurement in health care, few authorshave studied how top managers on a macro level develop performance measurement (PM)systems.     • BACKGROUND: When the 21 Swedish county councils decided to collaborate in the creation of a national system for knowledge-based management, patient participation was mandatory. Patient and next-of-kin representatives (PR) co-produced person-centred and cohesive clinical pathways together with healthcare professionals (HPR). Research on co-production in healthcare at the national level is scarce. The aim of this study is to explore experiences of patient participation from the perspectives of both PRs and HPRs when co-producing clinical pathways within the Swedish nationwide healthcare system for knowledge-based management.METHODS: A qualitative study was conducted. A strategic sample of nine PRs and eight HPRs were interviewed individually between August 2022 and January 2023 using a semi-structured interview guide. We analysed data using an inductive content analysis.RESULTS: Three main categories were identified: (1) Finding appropriate patient representativeness; (2) Working methods that facilitate a patient perspective; and (3) Influence of the patient perspective in the clinical pathways.CONCLUSIONS: The study demonstrates the importance of patient and next-of-kin participation in the construction of clinical pathways at the national level. The results provide a platform for further research on patient participation on the national level and add to studies on if and how patient participation on this level has an impact on how the clinical pathways are put into practice at the micro level, and the support provided at the meso level. The study contributes to the growing body of literature studying patient participation and co-production.TRIAL REGISTRATION: Region Örebro County ID 276,940. An advisory opinion was obtained from the Swedish Ethical Review Authority (2021-05899-01).

  • Improvement work in mental healthcare : an example from Region Jönköping County, Sweden

    • Region Jönköping County (RJC) in Sweden is a healthcare system that is characterised by sustainable work with quality in healthcare and long-term system-wide improvement. This article describes important factors behind the improvement work in RJC, and how the improvement methods and initiatives have been adopted also in mental healthcare. For example, patients otherwise eligible for admission to a psychiatric department were treated at home after introduction of home treatment teams. Patient satisfaction was high and the number of visits to the emergency department, hospital admissions and hospital stay decreased.

    • Kundnöjdhet är en viktig faktor för kliniska laboratorier. En betydelsefull värdemätare är turnaround tid för prover. I ett samarbetsprojekt mellan klinisk mikrobiologi och kemi ändrades rutinerna för omhändertagandet av blododlingar under kvällar och nätter. Ny teknik för bakterietypning och snabb resistensbestämning infördes. Medeltid från provtagning till resistensbestämning för cefotaximresistenta enterobacteriaceae minskades från 55 till 43 timmar. För att undersöka personalen upplevelser genomfördes intervjuer vilka visade att personalen kan hantera förändringar väl.
