  • Organizational preconditions and supportive resources for Swedish healthcare managers. : Factors that contribute to or counteract changes

    • Swedish Healthcare managers’ organizational preconditions and supportive resources are important for their ability to work with planned change in a sustainable way. This thesis further investigates these factors together with an output measure, healthcare process quality (HPQ).The overall aim was to investigate how healthcare managers’ organizational preconditions and support contribute to or counteract managers’ work with planned change in order to implement process development in a sustainable way. Specific aims were: to improve knowledge of managers’ views of and approaches to increasing their employees’ influence on and engagement in models for improving care processes (study I); to investigate relationships among managers’ organizational preconditions, support, and work to improve quality of care and HPQ over time (study II); to investigate whether managers’ coaching style, preconditions, implementation strategy, appraisal of change, and clinical autonomy are associated with HPQ (study III ); and to assess the influence of support from superiors, colleagues, external sources, subordinates, and private life on managers’ own health (study IV ).The data for Studies I – III came from five hospitals collected over a three-year period. The data were collected by means of interviews (Study I, qualitative analysis) and annual questionnaires (Studies II and III, quantitative and mixed-method analyses). The data for Study IV were based on questionnaires administered to first- and second-line managers in municipal care, twice during a two-year period.The results revealed that the healthcare managers were key actors in implementing planned change, but were dependent on their employees’ engagement in order to succeed. Managers’ appraisal of work with planned change became more positive with strong support from other managers, employees, and the organization as well as with long managerial experience. Support from private life and networks, as well as the managers’ attitudes towards their managerial role, predicted their own health. For new managers or managers with many employees, organizational support predicted their health-related sustainability. Managers practising a more distanced style of coaching (e.g., clearly delegating responsibility for implementation work to employees) were associated with better HPQ outcomes than were managers who were more involved in implementation. In conclusion, implementation of planned change are facilitated by, engaged managers, employees with knowledge of implementation work and of the healthcare system, as well as organizational structures that support the managers. Strong support from various sources as well as managerial experience are important for managers’ appraisal of work with planned change. Strong managerial support and a more delegated leadership style are both important factors related to higher estimated HPQ.

    • This study is an analysis of how television journalism, specifically Swedish publictelevision (SVT) and the commercial channel TV4, have approached environmental,consumption and lifestyle issues from 1997 to 1998. A central issue considered is howtelevision journalism in Sweden has contributed to an ideologieal change in societywhereby ecological considerations have become individualized, as well as reformulatedand re-ftamed such that they are no longer (or not) regarded as a hinder to technical andeconomic progress. Environmental politics on an internationallevel, as well as bills inthe Swedish government have played an important role in this development. Anoverarching ambition of this thesis is an examination of how a consumption-orientedenvironmental discourse has been communieated in television news. This question raisesa nurnber of others: what are the themes that can be identified in the news stories? Howare these themes articulated? Which actors have access to this discourse? How areperspectives on the threat against society and nature that environmental problems implydepicted, including visions ofthe society and natural world that we aspire to have?In order to examine how television news reporting contributes to theconsumption-oriented environmental discourse I exarnine the actual form of the newsreporting, Le. how the message is packaged. What patterns are observable in influentialtext and pictures and what do they have to say with respect to environmental issues? Thepoint of departure in this case is that the tools employed to create meaning, such asmetonymy, metaphors, c\icMs and symbols, are geared at creating understanding andpersuasion, at the same time as they must be meaningful within a cuiturai context. Insuch a manner they reveal pertinent perceptions of a foundational character with respectto environmental issues and how we regard the society in which we wish to live. Themeswhich I have identified as receiving a great deal of coverage in television news reportsare explored in four different empirical chapters covering media depiction of recycling,consurner durables, environmental politics and automobile use.This study conc\udes that television news stories are a co-creator of building a"green people's home" in Sweden together with the formal politics. They provide thearena for formal politics to be framed and to provide the public with moral guidelinesabout how to deal with the environmental question on an individual basis. Theirdepiction of visions and threats to Nature and Society says as much about whatenvironment is worth preserving, as how the economical and social welfare system characteristicfor Sweden during the post-W.W.II years - could be maintained.

  • Case fatality of myocardial infarction among shift workers

    • PURPOSE: Shift work has been associated with an excess risk of cardiovascular disease (CVD) and more specifically myocardial infarction (MI). The majority of the studies that found a positive association between shift work and CVD have been based on incidence data. The results from studies on cardiovascular-related mortality among shift workers have shown little or no elevated mortality associated with shift work. None of the previous studies have analysed short-term mortality (case fatality) after MI. Therefore, we investigated whether shift work is associated with increased case fatality after MI compared with day workers.METHODS: Data on incident cases with first MI were obtained from case-control study conducted in two geographical sites in Sweden (Stockholm Heart Epidemiology Program and Västernorrland Heart Epidemiology Program), including 1,542 cases (1,147 men and 395 women) of MI with complete working time information and 65 years or younger. Case fatality was defined as death within 28 days of onset of MI. Risk estimates were calculated using logistic regression.RESULTS: The crude odds ratios for case fatality among male shift workers were 1.63 [95 % confidence interval (CI) 1.12, 2.38] and 0.56 (95 % CI 0.26, 1.18) for female shift workers compared with day workers. Adjustments for established cardiovascular risk factors such as diabetes type II and socio-economic status did not alter the results.CONCLUSION: Shift work was associated with increased risk of case fatality among male shift workers after the first MI.

    • BACKGROUND: Shift work is associated with increased risk of cardiovascular disease, but the causes have not yet been fully established. It has been proposed that the coronary risk factors are more hazardous for shift workers, resulting in a potential interaction effect with shift work.OBJECTIVE: To analyse interaction effects of work schedule and established risk factors for coronary artery disease on the risk of myocardial infarction.METHODS: This analysis was conducted in SHEEP/VHEEP, a case-control study conducted in two counties in Sweden, comprising all first-time cases of myocardial infarction among men and women 45-70 years of age with controls stratified by sex, age, and hospital catchment area, totalling to 4648 participants. Synergy index (SI) was used as the main outcome analysis method for interaction analysis.RESULTS: There was an interaction effect between shift work and physical inactivity on the risk of myocardial infarction with SI of 2.05 (95% CI 1.07 to 3.92) for male shift workers. For female shift workers, interaction effects were found with high waist-hip ratio (SI 4.0, 95% CI 1.12 to 14.28) and elevated triglycerides (SI 5.69, 95% CI 1.67 to 19.38).CONCLUSION: Shift work and some established coronary risk factors have significant interactions.

    • Shift work is often defined as working time outside daytime hours (06:00 to 18:00). In recent years, shift work has been associated with an increased risk of cardiovascular disease (CVD), type II diabetes, and the metabolic syndrome. While some studies support the associations, others do not. Therefore, more research is needed. The aim of this thesis was to further study the association between shift work and CVD. This was addressed by performing four studies, one analysed if shift workers had an increased risk of ischemic stroke, the second study analysed whether shift workers had an increased risk of short-term mortality (case fatality)after a myocardial infarction (MI). The third study analysed if shift work interacts with other risk factors for MI and the fourth study analysed if parental history of CVD interacted with shift work on the risk of MI. The studies were performed using logistic regression analyses and additive interaction analyses in two different case-control databases. Shift workers did not have an increased risk of ischemic stroke. Male shift workers had an increased risk of death within 28 days after a MI. Shift work interacts with some CVD risk factors and interacts with paternal history of CVD and the risk of MI for males. The findings from this thesis provide new evidence showing that shift work is in different ways associated with an increased risk of MI and related mortality, but not with ischemic stroke. However, more research is needed to clarify and characterise these results.

    • Shift work is a work schedule being the opposite of normal daytime work, often defined as working time outside normal daytime hours (06:00 to 18:00). In recent years, shift work has been associated with an increased risk of numerous chronic conditions including for example cardiovascular disease, some types of cancer, type II diabetes, and the metabolic syndrome. While some studies on the association between shift work and chronic disease have found results supporting it, others have not. Therefore, more research is needed to clarify potential associations.The aim of this thesis was to further study the proposed association between shift work and cardiovascular disease. This was addressed by performing two studies, one analysing if shift workers had an increased risk of ischemic stroke compared to day workers. The other study analysed whether shift workers had an increased risk of short-term mortality (case fatality) after a myocardial infarction compared to day workers. The studies were performed using logistic regression analysis in two different case-control databasesThe findings from the first study indicated that shift workers did not have an increased risk of ischemic stroke. The findings from the second study showed that male shift workers had an increased risk of death within 28 days after a myocardial infarction; the results did not indicate an increased risk for female shift workers. The results from both studies were adjusted for both behavioural and medical risk factors without affecting the results. The findings from this thesis provide new evidence showing that male shift workers have an increased risk of death 28 days after a myocardial infarction, however more research is needed to clarify and characterise any such potential associations.

    • BACKGROUND: Despite a decreasing incidence of peptic ulcer disease, most previous studies report a stabile incidence of ulcer complications. We wanted to investigate the incidence of peptic ulcer complications in Sweden before and after the introduction of the proton pump inhibitors (PPI) in 1988 and compare these data to the sales of non-steroid anti-inflammatory drugs (NSAID) and acetylsalicylic acid (ASA).METHODS: All cases of gastric and duodenal ulcer complications diagnosed in Sweden from 1974 to 2002 were identified using the National hospital discharge register. Information on sales of ASA/NSAID was obtained from the National prescription survey. RESULTS: When comparing the time-periods before and after 1988 we found a significantly lower incidence of peptic ulcer complications during the later period for both sexes (p < 0.001). Incidence rates varied from 1.5 to 7.8/100000 inhabitants/year regarding perforated peptic ulcers and from 5.2 to 40.2 regarding peptic ulcer bleeding. The number of sold daily dosages of prescribed NSAID/ASA tripled from 1975 to 2002. The number of prescribed sales to women was higher than to males. Sales of low-dose ASA also increased. The total volume of NSAID and ASA, i.e. over the counter sale and sold on prescription, increased by 28% during the same period.CONCLUSION: When comparing the periods before and after the introduction of the proton pump inhibitors we found a significant decrease in the incidence of peptic ulcer complications in the Swedish population after 1988 when PPI were introduced on the market. The cause of this decrease is most likely multifactorial, including smoking habits, NSAID consumption, prevalence of Helicobacter pylori and the introduction of PPI. Sales of prescribed NSAID/ASA increased, especially in middle-aged and elderly women. This fact seems to have had little effect on the incidence of peptic ulcer complications.

  • Quasi-static path optimization for industrial robots with dress packs

    • Problems with robot dress packs are a major reason for on-line adjustments of robot programs and down-time in robot stations. It is therefore of high value if the physical behaviour of the dress packs can be considered with simulation methods already during the off-line programming process for a robot station. This paper presents a method for quasi-static path optimization for an industrial robot with respect to its deformable dress pack. Given an initial collision-free path generated by an automatic path planner, the via point configurations of the path are optimized with respect to the performance aspects of the dress pack. The method is derived from a general framework for parameter optimization of a mechanical system subject to quasi-static motions and deformations. The optimal parameter values are obtained from numerical solutions to a non-linear programming problem in which the static equilibrium equations of the system hold at discrete times. Due to the large-scale nature of this problem, a dress pack is modelled as a discrete Cosserat rod, which is the preferred choice for modeling large spatial deformations of a slender flexible structure with coarse discretization. The method is applied to an industrial robot moving in-between stud welding operations in a stud welding station. The optimized path reduces the stress in the dress pack and keeps the dressed robot from the surrounding geometry with a prescribed safety clearance during the entire robot motion.

  • Manufacturing Consensus : The Making of the Swedish Reformist Working Class

    • The 1910s were a precarious time for the labor movement. The Russian Revolution in 1917 sparked a trend towards radicalization among labor organizations and communist organizations spread all over Europe. These organizations challenged existing notions of the “worker,” causing an identity crisis in class organizations. Suddenly, there were not only workers, but different kinds of workers, promoting not only social democracy and syndicalism, but also communism. The labor movement became fragmented. This was very much the case in Sweden. However, despite the conflict situation during the interwar period, the Swedish workers were integrated into a strong cohesive labor movement, united under the banner of reformism which, in turn, paved the way for a strong social democracy.How was this possible? This study presents an explanation of why the Swedish working class so unanimously adopted reformism. Its thesis is that the leadership of the Trade Union Confederation (LO) was well aware of the identity problems the leftwing factions had created for the reformist unions. Because of this, the leadership decided to take actions.  As “identity entrepreneurs” the leaders re-formed the notion of the worker by constructing an organizational identity that downplayed class struggle and embraced discipline, peaceful solutions to labor market problems, and cooperation with the employers. This notion was inculcated in the workers through popular education. The book shows that ABF study circles became the main tool of the Trade Union Confederation’s identity policy in the 1920s and 1930s and its successful outcome paved the way for the renowned “Swedish Model.”

  • Experiences of being a mentor for newly qualified nurses : An interview study

    • Background: Challenges of retaining registered nurses in the profession is a global issue[1]. Support is an important factor for retaining nurses[2], and even though mentorship have been found to reduce turnover rate for nurses[3], it is still not a fully integrated routine in all health care units. To fill the knowledge gap, the aim of this study is to describe registered nurses’ experiences of being a mentor for newly qualified nurses.Methods: Members of the collaborative project Becoming a Professional Nurse performed individual interviews in health care units in Sweden and Norway. Participants were registered nurses (n=21) who had mentored at least one newly qualified nurse. Transcribed interview data were analysed using qualitative content analysis[4]. Preliminary results: To be a mentor were described as; being a role model for the mentees, creating an inclusive and open environment, and developing as a professional. To be successful mentors, they described a need of; scheduled time, support from colleagues, education about mentorship, arenas for exchange of experience, and attendant leaders. Conclusions: There is a need of a structured organization to promote mentorship. More research is needed to develop, test and evaluate evidence-based mentorship programs in the health care units. [1] World Health Organisation. (2020).   • Exploring registered nurses’ perspectives as mentors for newly qualified nurses : a qualitative interview study

    • Objective: Existing research has focused mostly on mentees’ experiences of mentoring rather than mentors’ experiences. Therefore, this study describes registered nurses’ experiences of being a mentor for newly qualified nurses.Design: A qualitative interview study based on semistructured individual interviews. Interviews were analysed using qualitative content analysis.Participants and setting: A purposive sample of experienced registered nurses (n=21) from healthcare units in northern Sweden and northern Norway. Inclusion criteria were to have been a mentor to at least one newly qualified nurse, hold permanent employment of 75%–100% as a registered nurse and to be able to communicate in Swedish or Norwegian.Results: Our study’s findings suggest that being a mentor plays a crucial role in establishing safety in complex work environments. The main theme consists of three themes: feeling motivated in being a mentor; continuously developing the learning environment; and navigating obstacles and cultivating support.Conclusion: Being a mentor is a complicated role for registered nurses. The mentoring role is beneficial—ie, positive and rewarding—if facilitated sufficiently in the context of a structured organisation. This study brings a more profound understanding of and provides new insights into registered nurses’ perspectives and needs regarding being a mentor and the study’s findings make an important contribution to the field of nursing regarding the facilitation of mentoring.

    • Background: Healthcare organizations worldwide face persistent challenges relating to turnover and intention to leave the nursing profession among registered nurses. Factors contributing to their retention and well-being at work include high job satisfaction, professional competence, and self-efficacy. Few multicenter studies have investigated these factors in relation to work experience in a Nordic context. Therefore, this study aimed to investigate job satisfaction, professional competence, and self-efficacy among registered nurses.Methods: This multicenter cross-sectional study survey was part of a larger overarching Swedish-Norwegian project, and was conducted among registered nurses (n = 1137) in September 2021. The participants worked in a variety of health care units, e.g., hospital units, primary health care, and home care. Data was subjected to descriptive and comparative statistical analysis; chi-square test, one-way between-groups analysis of variance (ANOVA) and Kruskal-Wallis test.Results: The findings show that job satisfaction is reported as lowest in registered nurses with medium-term work experience as compared to newly qualified and long-term work-experienced registered nurses. Professional competence and self-efficacy are reported as higher among registered nurses with long-term work experience as compared to those with medium-term work experience and newly qualified registered nurses. However, the participants reported their professional competence as highest in relation to the same factor – “Value-based nursing care” – regardless of their work experience.Conclusions and implications: This study underscores the need for continuous support and professional development for registered nurses throughout their careers. Proactive support for newly qualified nurses may improve job satisfaction as they progress to being registered nurses with medium-term work experience. Tailored interventions to address the distinct needs of both newly qualified and medium-term work-experienced registered nurses are crucial for nurturing a sustainable nursing workforce.

  • The impact of a mentorship program with digital solutions : Mentors' experiences, self-reported self-efficacy, and clinical teaching behavior

    • Background: In 2021, a two-fold mentorship program was developed in collaboration with new and experienced registered nurses and leaders in healthcare units and municipalities in two regions in northern Sweden and northern Norway. The mentorship program aimed to support newly qualified nurses into working life as well as support mentors and was tested in intervention units from October-21 to April-22. The aim of this sub-study was to evaluate the mentors´ experiences, self-reported self-efficacy, and clinical teaching behavior.Methods: A mixed methods design was used where data was collected via focus-group interviews (n=5) directly after the intervention. A follow-up, self-reported questionnaire study was conducted at three different time points: pre-test, baseline, and post-test I, directly after the intervention, and post-test II, eight months after the intervention. Transcribed interview data will be analyzed using qualitative content analysis, and self-reported answers will be analyzed using suitable statistical methods.Results and conclusion: Preliminary results will be presented at the conference. The evaluation will highlight the potential effects and meaning of the mentorship program from the mentors´ perspective as well as what it means to be a mentor.

  • How agencies inspect : a comparative study of inspection policies in eight Swedish government agencies

    • BackgroundThe regulatory approach used by SKI has an impact on the licensee and, therefore, indirectly on the safety of the nuclear facilities. The regulatory policy and its implementation also influences the way SKI is working and the competency needed by the regulator as well as by the industry. One of SKI´s tasks is to be able adjust the regulatory approach to current and future needs.Little research is available on the use of regulatory strategies, methods and activities used by regulatory agencies in their work (to assure safety), as well as their impacts. To address this gap in knowledge, SKI has contracted two separate research projects. This project, where the focus is on comparing inspection policies in Swedish government agencies, and another one where regulatory strategies across selected countries, are studied and compared.SKI´s purpose and goalsThe goal of this research project is to gain knowledge about selected Swedish agencies and their inspection policies. All the selected agencies are working with issues regarding health, environmental protection or safety. A number of persons at each agency have been interviewed about the regulatory work at their organizations. A questionnaire was developed as a first part of the project, and it was used during the interviews.In addition to provide insights as a basis for improvement within SKI, the purpose has been to promote exchange of experience between agencies with similar tasks. By this study SKI also wanted to contribute to the development of competency in this field of research.ResultsA lot of information has been gathered and analyzed during the project. The focus of the research is on comparing agencies and their practices. The report includes an analysis regarding similarities and differences in a number of areas across the agencies. The report also provides information and evidence on the problems related to comparison between agencies, partly due to the difference in the use of definitions. The recommendations resulting from this study focus on improving cooperation among Swedish agencies and on improving inspection methodologies, areas where further analysis is suggested. The conclusions and recommendations in the report belong to the persons participating in this work, and are not necessarily the same as the once drawn by SKI.

    • Background: Early interventions for young children with autism have been shown to enhance developmental outcomes. However, opportunities for targeted interventions in autism, both in care and preschool, are often lacking, particularly in immigrant communities. The early start denver model (ESDM) stands as one of the most well-established intervention models, including improvement in core developmental domains and reduction of maladaptive behaviours, also delivered in preschool settings. An intervention program based on the ESDM was implemented in collaboration with parents, preschool staff, and health care professionals locally in a multiethnic immigrant and socioeconomically disadvantaged area in Gothenburg, Sweden. Purpose: The present study aimed to describe a low intensity intervention program based on the ESDM for young children with autism in a multiethnic immigrant setting and capture the experiences of the preschool staff involved in implementing the program. Method: Fifteen preschool professionals were interviewed through focus group interviews. Data were analyzed using content analysis. The interviews focused on capturing the crucial factors in the intervention program and to get more in-depth information about the intervention program's influence on the children with autism, their parents, the preschool staff, and preschool activities. Results: Central components of the intervention program were emphasized by the preschool staff. These encompassed contextual prerequisites, such as the preschool staff's participation in intervention program fostered by the local environment and features linked to the ESDM methodology. The preschool staff emphasized that shared objectives and regular network meetings as fundamental components of the model. They also underscored the seamless alignment between the ESDM and the preschool curriculum emphasizing how ESDM strategies could be easily integrated into the preschool's daily routines. The staff's experience indicated that the implementation of ESDM enriched the learning experiences of children with autism and yielded benefits to their parents, fellow peers in the preschool, and the preschool staff. Conclusions: The intervention program based on the ESDM presents a promising model for young children with autism in a multiethnic immigrant setting. Several critical factors based on the preschool staff's experiences were essential for implementation: contextual prerequisites such as organizational support, close collaboration with healthcare professionals, good competence among preschool staff, and the ESDM itself.

  • Sustainable building engineering by blended learning

    • The sustainable building engineering education at Mid Sweden University has since it started 1994 a strong focus on sustainable building. It is also committed to be a pilot case for blended learning. All courses in the building engineering program were from the autumn 2012 adapted to a blended learning concept. That means that all courses are available in an integrated mix of ordinary campus and online distance education in real time. Blended learning is a way of taking in account different needs concerning student study situation. Adult students, bound with house and families, often prefer more distance education while younger students need more campus education. The results of the case study show that blended learning attracts more students, and allows students with different backgrounds to study and gives a more solid base of students for the program. It also allows a more flexible way of allocating resources by integrating two previous modes of delivery for the same program. This provides an education with a well thought through quality assurance. Another strength of blended learning is the possibility to connect individual students to a larger collective of students, making it possible for them to relate to and learn with and from others by social interactions that are difficult to achieve in solely online education. The pilot case is so far limited to the sustainable building engineering program but in a next stage the concept is planned to be spread to other engineering programs and other education programs at Mid Sweden University.

  • Kan Hälsostöd främja en positiv utveckling av psykisk hälsa och hälsolitteracitet hos asylsökande och nyanlända? Utvärdering av effekter och processer

    • Sedan 2015 har Sveriges Kommuner och Landsting (SKL) genomfört ett omfattande arbete för att identifiera, sprida och stötta implementeringen av metoder som kan stödja en positiv hälsoutveckling hos asylsökande och nyanlända. I Västra Götalandsregionen (VGR) har HiS spridits utifrån beslut i Hälso-och sjukvårdsstyrelsen september 2016. Som en del av HiS-programmet ges Hälsoinformation vid ett tillfälle och Hälsostödets vid fem gruppträffar. Hälsoinformationen omfattar främst information om det svenska hälso- och sjukvårdssystemet, medan Hälsostödet har ett mer långtgående syfte i att förstärka en positiv utveckling av den psykiska hälsan genom öka begripligheten över den situation gruppdeltagarna befinner sig i, ge dem redskap att i vardagen hantera vissa svårigheter samt också få kunskap om hjälp som finns att få. Föreliggande utvärderings specifika mål har varit (a) att utvärdera effekter på psykisk hälsa och hälsolitteracitet före-och-efter deltagande i HiS-programmets Hälsostöd, samt (b) att utvärdera deltagarnas egen upplevelse av Hälsoinformationens och Hälsostödets genomförande och godtagbarhet. Effektutvärderingen visar på en positiv association mellan deltagande i Hälsostöd och förbättring av nyanländas psykiska hälsa och hälsolitteracitet. Processutvärderingen indikerar att denna kan öka ytterligare efter förbättringar i genomförande. Utvärderingens slutsats är därför att: •satsningen på Hälsostöd fortsätter då den kan anses vara positiv både på individ- och samhällsnivå, för mottagande och integration av asylsökande och nyanlända. Specifikt kan Hälsostöd erbjudas som en dokumenterat fungerande åtgärd för att främja nyanländas psykiska hälsa och hälsolitteracitet. •man överväger om gruppen asylsökande ska erbjudas mer eller annan hjälp. Innehållet och deltagandet i Hälsostöd kan enligt föreliggande utvärdering inte fullt ut hjälpa personer i den mycket påfrestande och utdragna asylprocessen. •åtgärder vidtas för att klargöra prioriteringar mellan uppdragets målsättningar och medel för att nå dessa. •ökade resurser och fokus läggs på att stärka stödjande kultur med syfte att öka projektets effektivitet liksom att motverka hälsorisker för teamledare och Hälsokommunikatörer.

    • Alkaline electrolyte solutions are important components in rechargeable batteries and alkaline fuel cells. As the ionic conductivity is thought to be a limiting factor in the performance of these devices, which are often operated at elevated temperatures, its temperature dependence is of significant interest. Here we use NaOH as a prototypical example of alkaline electrolytes, and for this system we have carried out reactive molecular dynamics simulations with an experimentally verified high-dimensional neural network potential derived from density-functional theory calculations. It is found that in concentrated NaOH solutions elevated temperatures enhance both the contributions of proton transfer to the ionic conductivity and deviations from the Nernst-Einstein relation. These findings are expected to be of practical relevance for electrochemical devices based on alkaline electrolyte solutions.

    • Objective: Despite the wide use of telephone call-back services in Swedish healthcare, there has been little research on how it affects patients. This study explores individual experiences of a call-back service, concentrating on barriers to healthcare, and healthcare-seeking behavior. Methods: The study was conducted at Angered Hospital and Angered Primary Care Rehabilitation Center in Gothenburg, Sweden. Ten informants, 28-82 years old, who had used the call-back service participated in interviews about their experience of the call-back service. Thematic analysis was used to analyze data from the interviews. Results: Three themes were identified in the analysis: (1) features and functions of the call-back service; (2) the call-back service as a barrier to or facilitator of healthcare; and (3) adjustments to the call-back service. Most informants were content with the call-back function. Negative experiences were related to language difficulties and the length of time allowed during the phone call. Lack of available appointments and telephone access were problems reported. Informants suggested a longer time frame for calls, longer opening hours regarding telephone access, more language and voicemail options, and the possibility of speaking to a person. Conclusions: Informants in this study mostly had a positive impression of seeking healthcare using call-back services. Barriers related to language and time frame for calls could be explored in larger studies. The results from this explorative study suggests that a combination of approaches – with other options added to the call-back services - might increase equal access to health care. The use and effects of call-back services warrant further investigation.

  • Regulating Multiple Externalities: The Case of Nordic Fisheries

    • Open access is a well-known externality problem in fisheries causing excess capacity and overfishing. Due to global warming, externality problems from CO2 emissions have gained increased interest. With two externality problems, a first-best optimum can be achieved by using two regulatory instruments. However, solving the open-access externality problem also affects CO2 emissions. By using a bio-economic model covering Iceland, Norway, Denmark, Sweden, and the Faroe Islands, it is shown that regulations of the open-access externality problem have a large effect on both economic performance and CO2 emissions, while an additional CO2 regulation only has minor effects. The second-best solution achieved by only regulating open access reduces emissions by approximately 50% compared to current fisheries, with the exception of Iceland, which already has a well-developed fisheries management system.

  • Long-Term Follow Up of Sexual Function and Steroid Levels in Women after Perimenopausal Hysterectomy with or without Concomitant Oophorectomy

    • Hysterectomy, most often performed because of bleeding disorders or uterine leiomyoma, is one of the most common major surgical procedures in women and is usually performed during the perimenopausal period on ages 45-55 years. Hysterectomy may be combined with bilateral salpingo-oophorectomy, as a risk-reducing procedure to minimize the risk of ovarian cancer. An open question is whether concomitant oophorectomy, with cessation of ovarian androgen secretion, has any long-term effects on sexual function. In the present prospective cohort study of women undergoing benign hysterectomy, the long-term (10-12 years) effects on sexual function and changes in sex hormone levels were investigated in women having undergone perimenopausal hysterectomy, with or without concomitant bilateral salpingo-oophorectomy. Originally, 491 women (mean age around 50 years) were operated with (patient preference) either only hysterectomy (HYST) or hysterectomy plus bilateral salpingo-oophorectomy (HYST + BSO), and 441 women (90%; HYST; n = 271 and HYST + BSO; n = 170) completed a one-year survey. In the present study, 185 women (42%) of the cohort with one-year follow up participated in the long-term follow up after 10-12 years. Follow-up was with the 10-item McCoy Female Sex Questionnaire and blood analysis of levels of testosterone, estradiol and sexual-hormone-binding globulin. The results showed that specific aspects of sexual function were lower after HYST + BSO compared to HYST 10-12 years after surgery. These lowered items were frequency of sexual fantasies, enjoyment of sexual activity, sexual arousal, and orgasmic frequency. No long-term differences in sex hormone levels were found between the two groups. In conclusion, some items related to sexual function were lower after HYST + BSO in a long-term perspective study, although the levels of testosterone were unaltered. This finding may have implications for clinical recommendations concerning prophylactic salpingo-oophorectomy or for hysterectomy during the perimenopausal age.
