    • The cryosphere (including, snow, glaciers, permafrost, lake and river ice) is an integral element of high- mountain regions, which are home to roughly 10% of the global population. Widespread cryosphere changes affect physical, biological and human systems in the mountains and surrounding lowlands, with impacts evident even in the ocean. Building on the IPCC’s Fifth Assessment Report (AR5), this chapter assesses new evidence on observed recent and projected changes in the mountain cryosphere as well as associated impacts, risks and adaptation measures related to natural and human systems. Impacts in response to climate changes independently of changes in the cryosphere are not assessed in this chapter. Polar mountains are included in Chapter 3, except those in Alaska and adjacent Yukon, Iceland, and Scandinavia, which are included in this chapter.

    • Plant traits-the morphological, anatomical, physiological, biochemical and phenological characteristics of plants-determine how plants respond to environmental factors, affect other trophic levels, and influence ecosystem properties and their benefits and detriments to people. Plant trait data thus represent the basis for a vast area of research spanning from evolutionary biology, community and functional ecology, to biodiversity conservation, ecosystem and landscape management, restoration, biogeography and earth system modelling. Since its foundation in 2007, the TRY database of plant traits has grown continuously. It now provides unprecedented data coverage under an open access data policy and is the main plant trait database used by the research community worldwide. Increasingly, the TRY database also supports new frontiers of trait-based plant research, including the identification of data gaps and the subsequent mobilization or measurement of new data. To support this development, in this article we evaluate the extent of the trait data compiled in TRY and analyse emerging patterns of data coverage and representativeness. Best species coverage is achieved for categorical traits-almost complete coverage for 'plant growth form'. However, most traits relevant for ecology and vegetation modelling are characterized by continuous intraspecific variation and trait-environmental relationships. These traits have to be measured on individual plants in their respective environment. Despite unprecedented data coverage, we observe a humbling lack of completeness and representativeness of these continuous traits in many aspects. We, therefore, conclude that reducing data gaps and biases in the TRY database remains a key challenge and requires a coordinated approach to data mobilization and trait measurements. This can only be achieved in collaboration with other initiatives.

  • The Randolph Glacier Inventory : a globally complete inventory of glaciers

    • The Randolph Glacier Inventory (RGI) is a globally complete collection of digital outlines of glaciers, excluding the ice sheets, developed to meet the needs of the Fifth Assessment of the Intergovernmental Panel on Climate Change for estimates of past and future mass balance. The RGI was created with limited resources in a short period. Priority was given to completeness of coverage, but a limited, uniform set of attributes is attached to each of the similar to 198 000 glaciers in its latest version, 3.2. Satellite imagery from 1999-2010 provided most of the outlines. Their total extent is estimated as 726 800 +/- 34 000 km(2). The uncertainty, about +/- 5%, is derived from careful single-glacier and basin-scale uncertainty estimates and comparisons with inventories that were not sources for the RGI. The main contributors to uncertainty are probably misinterpretation of seasonal snow cover and debris cover. These errors appear not to be normally distributed, and quantifying them reliably is an unsolved problem. Combined with digital elevation models, the RGI glacier outlines yield hypsometries that can be combined with atmospheric data or model outputs for analysis of the impacts of climatic change on glaciers. The RGI has already proved its value in the generation of significantly improved aggregate estimates of glacier mass changes and total volume, and thus actual and potential contributions to sea-level rise.

  • Predictors of mortality in solid organ transplant recipients with bloodstream infections due to carbapenemase-producing Enterobacterales : The impact of cytomegalovirus disease and lymphopenia

    • Treatment of carbapenemase-producing Enterobacterales bloodstream infections in solid organ transplant recipients is challenging. The objective of this study was to develop a specific score to predict mortality in solid organ transplant recipients with carbapenemase-producing Enterobacterales bloodstream infections. A multinational, retrospective (2004-2016) cohort study (INCREMENT-SOT, ClinicalTrials.gov NCT02852902) was performed. The main outcome variable was 30-day all-cause mortality. The INCREMENT-SOT-CPE score was developed using logistic regression. The global cohort included 216 patients. The final logistic regression model included the following variables: INCREMENT-CPE mortality score >= 8 (8 points), no source control (3 points), inappropriate empirical therapy (2 points), cytomegalovirus disease (7 points), lymphopenia (4 points), and the interaction between INCREMENT-CPE score >= 8 and CMV disease (minus 7 points). This score showed an area under the receiver operating characteristic curve of 0.82 (95% confidence interval [CI] 0.76-0.88) and classified patients into 3 strata: 0-7 (low mortality), 8-11 (high mortality), and 12-17 (very-high mortality). We performed a stratified analysis of the effect of monotherapy vs combination therapy among 165 patients who received appropriate therapy. Monotherapy was associated with higher mortality only in the very-high (adjusted hazard ratio [HR] 2.82, 95% CI 1.13-7.06, P = .03) and high (HR 9.93, 95% CI 2.08-47.40, P = .004) mortality risk strata. A score-based algorithm is provided for therapy guidance.

  • TEFM variants impair mitochondrial transcription causing childhood-onset neurological disease

    • Mutations in the mitochondrial or nuclear genomes are associated with a diverse group of human disorders characterized by impaired mitochondrial respiration. Within this group, an increasing number of mutations have been identified in nuclear genes involved in mitochondrial RNA biology. The TEFM gene encodes the mitochondrial transcription elongation factor responsible for enhancing the processivity of mitochondrial RNA polymerase, POLRMT. We report for the first time that TEFM variants are associated with mitochondrial respiratory chain deficiency and a wide range of clinical presentations including mitochondrial myopathy with a treatable neuromuscular transmission defect. Mechanistically, we show muscle and primary fibroblasts from the affected individuals have reduced levels of promoter distal mitochondrial RNA transcripts. Finally, tefm knockdown in zebrafish embryos resulted in neuromuscular junction abnormalities and abnormal mitochondrial function, strengthening the genotype-phenotype correlation. Our study highlights that TEFM regulates mitochondrial transcription elongation and its defect results in variable, tissue-specific neurological and neuromuscular symptoms.

  • 21st-century increase in glacier mass loss in the Wrangell Mountains, Alaska, USA, from airborne laser altimetry and satellite stereo imagery

    • Alaskan glaciers are among the largest regional contributors to sea-level rise in the latter half of the 20th century. Earlier studies have documented extensive and accelerated ice wastage in most regions of Alaska. Here we study five decades of mass loss on high-elevation, land-terminating glaciers of the Wrangell Mountains (similar to 4900 km(2)) in central Alaska based on airborne center-line laser altimetry data from 2000 and 2007, a digital elevation model (DEM) from ASTER and SPOT5, and US Geological Survey topographic maps from 1957. The regional mass-balance estimates derived from center-line laser altimetry profiles using two regional extrapolation techniques agree well with that from DEM differencing. Repeat altimetry measurements reveal accelerated mass loss over the Wrangell Mountains, with the regional mass-balance rate evolving from -0.07 +/- 0.19 m w.e. a(-1) during 1957-2000 to -0.24 +/- 0.16 m w.e. a(-1) during 2000-07. Nabesna, the largest glacier in this region (similar to 1056 km(2)), lost mass four times faster during 2000-07 than during 1957-2000. Although accelerated, the mass change over this region is slower than in other glacierized regions of Alaska, particularly those with tidewater glaciers. Together, our laser altimetry and satellite DEM analyses demonstrate increased wastage of these glaciers during the last 50 years.

    • The Aerodyne aerosol mass spectrometer (Q-AMS) was coupled with a counterflow virtual impactor (CVI) for the first time to measure cloud droplet residuals of warm tropospheric clouds on Mt. Areskutan in central Sweden in July 2003. Operating the CVI in different operational modes generated mass concentration and species-resolved mass distribution data for non-refractory species of the ambient, interstitial, and residual aerosol. The ambient aerosol measurements revealed that the aerosol at the site was mainly influenced by long-range transport and regional photochemical generation of nitrate and organic aerosol components. Four different major air masses were identified for the time interval of the experiment. While two air masses that approached the site from northeastern Europe via Finland showed very similar aerosol composition, the other two air masses from polar regions and the British Islands had a significantly different composition. During cloud events the larger aerosol particles were found to be activated into cloud droplets. On a mass basis the activation cut-off diameter was approximately 150 nm for nitrate and organics dominated particles and 200 nm for sulfate dominated particles. Generally nitrate and organics were found to be activated into cloud droplets with higher efficiency than sulfate. While a significant fraction of the nitrate in ambient particles was organic nitrates or nitrogen-containing organic species, the nitrate found in the cloud droplet residuals was mainly ammonium nitrate. After passage of clouds the ambient aerosol size distribution had shifted to smaller particle sizes due to the predominantly activation of larger aerosol particles without a significant change in the relative composition of the ambient aerosol.

    • Glaciers distinct from the Greenland and Antarctic Ice Sheets are losing large amounts of water to the world's oceans. However, estimates of their contribution to sea level rise disagree. We provide a consensus estimate by standardizing existing, and creating new, mass-budget estimates from satellite gravimetry and altimetry and from local glaciological records. In many regions, local measurements are more negative than satellite-based estimates. All regions lost mass during 2003-2009, with the largest losses from Arctic Canada, Alaska, coastal Greenland, the southern Andes, and high-mountain Asia, but there was little loss from glaciers in Antarctica. Over this period, the global mass budget was -259 +/- 28 gigatons per year, equivalent to the combined loss from both ice sheets and accounting for 29 +/- 13% of the observed sea level rise.

    • We determine the geodetic mass balance of surge-type Black Rapids Glacier, Alaska, for the time periods 1980-2001 and 2001-2010 by combining modern interferometric synthetic aperture radar (InSAR)-derived digital elevation models (DEMs), DEMs derived from archival aerial imagery, laser altimetry data, and in situ surface elevation measurements. Our analysis accounts for both the large rockslides and terrain displacements caused by the 2002 M7.9 earthquake on the Denali fault, which runs through Black Rapids Glacier. To estimate uncertainties, we apply Monte Carlo simulations. For the earthquake-triggered rockslides we find a volume of 56.62 +/- 2.86 x 10(6) m(3), equivalent to an average debris thickness of 4.44 +/- 0.24 m across the 11.7 km(2) deposit area on the glacier. Terrain displacement due to the earthquake corresponds to an apparent glacier volume change of -53.1 x 106 m(3), which would cause an apparent specific mass balance of -0.19 meter water equivalent (mwe) if not taken into account. The geodetic mass balance of Black Rapids Glacier is -0.48 +/- 0.07 mwe a(-1) for the entire 30 year period, but more negative for the period 2001-2010 (-0.64 +/- 0.11 mwe a(-1)) than the period 1980-2001 (-0.42 +/- 0.11 mwe a(-1)), in agreement with trends indicated by in situ mass balance measurements. Elevation data indicate no net thickening of the surge reservoir between 1980 and 2010, in contrast to what is expected during the quiescent phase. A surge of Black Rapids Glacier in the near future is thus considered unlikely.

    • BACKGROUND: The Alberta Stroke Program Early CT Score (ASPECTS) on baseline imaging is an established predictor of functional outcome in anterior circulation acute ischemic stroke (AIS). We studied ASPECTS before intravenous thrombolysis (IVT) and at 24 hours to assess its prognostic value.METHODS: Data for consecutive anterior circulation AIS patients treated with IVT from 2006 to 2013 were extracted from a prospectively managed registry at our tertiary center. Pre-thrombolysis and 24-hour ASPECTS were evaluated by 2 independent neuroradiologists. Outcome measures included symptomatic intracranial hemorrhage (SICH), modified Rankin Scale (mRS) at 90 days, and mortality. Unfavorable functional outcome was defined by mRS >1. Dramatic ASPECTS progression (DAP) was defined as deterioration in ASPECTS by 6 points or more.RESULTS: Of 554 AIS patients thrombolyzed during the study period, 400 suffered from anterior circulation infarction. The median age was 65 years (interquartile range (IQR): 59-70) and the median National Institutes of Health Stroke Scale score was 18 points (IQR: 12-22). Compared with the pre-IVT ASPECTS (area under the curve [AUC] = .64, 95% confidence interval [CI]: .54-.65, P = .001), ASPECTS on the 24-hour CT scan (AUC = .78, 95% CI: .73-.82, P < .001), and change in ASPECTS (AUC = .69, 95% CI: .64-.74, P < .001) were better predictors of unfavorable functional outcome at 3 months. DAP, noted in 34 (14.4%) patients with good baseline ASPECTS (8-10 points), was significantly associated with unfavorable functional outcome (odds ratio [OR]: 9.91, 95% CI: 3.37-29.19, P ≤ .001), mortality (OR: 21.99, 95% CI: 7.98-60.58, P < .001), and SICH (OR: 8.57, 95% CI: 2.87-25.59, P < .001).CONCLUSION: Compared with the pre-thrombolysis score, ASPECTS measured at 24 hours as well as serial change in ASPECTS is a better predictor of 3-month functional outcome.

    • A molecularly imprinted polymer (MIP) film for domoic acid (DA) was synthesised by direct photo-grafting onto a gold chip suitable for a surface plasmon resonance (SPR) based bioanalytical instrument system, the BlAcore 3000(TM). The gold surface was first functionalised with a self-assembled monolayer of 2-mercaptoethylamine and subsequent carbodiimide chemistry was performed for covalent attachment of the photoinitiator, 4,4-azobis(cyanovaleric acid). This ensured that the formation of the MIP thin film, comprising 2-(diethyl amino) ethyl methacrylate as functional monomer and ethylene glycol dimethacrylate as cross-linker, occurred only at the surface level. Optimisation and control over the grafting procedure were achieved using contact angle measurements and atomic force microscope (AFM) imaging. The surface grafting resulted in the formation of thin and homogeneous MIP film with thickness of 40 nm. A competitive binding assay was performed with free DA and its conjugate with horseradish peroxidase, which was used as a refractive label. The sensor was evaluated for its sensitivity, cross-reactivity, and robustness by using a BlAcore 3000(TM). Likewise, monoclonal antibodies acting as natural receptors for the toxin were studied with the same BlAcore system. Results of a comparison between the artificial and natural receptors are reported. In contrast to monoclonal antibodies, the regeneration of MIP chip did not affect its recognition properties and continuous measurement was possible over a period of at least 2 months.   • Using surface velocities to infer ice thickness and bed topography : A case study a Columbia Glacier, Alaska, USA

    • Information about glacier volume and ice thickness distribution is essential for manyglaciological applications, but direct measurements of ice thickness can be diffcult and costly. Wepresent a new method that calculates ice thickness via an estimate of ice fux. We solve the familiarcontinuity equation between adjacent fowlines, which decreases the computational time requiredcompared to a solution on the whole grid. We test the method on Columbia Glacier, a large tidewaterglacier in Alaska, USA, and compare calculated and measured ice thicknesses, with favorable results.This shows the potential of this method for estimating ice thickness distribution of glaciers for whichonly surface data are available. We fnd that both the mean thickness and volume of Columbia Glacierwere approximately halved over the period 1957–2007, from 281m to 143m, and from 294km3to134km3, respectively. Using bedrock slope and considering how waves of thickness change propagatethrough the glacier, we conduct a brief analysis of the instability of Columbia Glacier, which leads usto conclude that the rapid portion of the retreat may be nearing an end.

  • International pooled patient-level meta-analysis of ketamine infusion for depression: In search of clinical moderators

    • Depression is disabling and highly prevalent. Intravenous (IV) ketamine displays rapid-onset antidepressant properties, but little is known regarding which patients are most likely to benefit, limiting personalized prescriptions. We identified randomized controlled trials of IV ketamine that recruited individuals with a relevant psychiatric diagnosis (e.g., unipolar or bipolar depression; post-traumatic stress disorder), included one or more control arms, did not provide any other study-administered treatment in conjunction with ketamine (although clinically prescribed concurrent treatments were allowable), and assessed outcome using either the Montgomery-Åsberg Depression Rating Scale or the Hamilton Rating Scale for Depression (HRSD-17). Individual patient-level data for at least one outcome was obtained from 17 of 25 eligible trials [pooled n = 809]. Rates of participant-level data availability across 33 moderators that were solicited from these 17 studies ranged from 10.8% to 100% (median = 55.6%). After data harmonization, moderators available in at least 40% of the dataset were tested sequentially, as well as with a data-driven, combined moderator approach. Robust main effects of ketamine on acute [~24-hours; β*(95% CI) = 0.58 (0.44, 0.72); p < 0.0001] and post-acute [~7 days; β*(95% CI) = 0.38 (0.23, 0.54); p < 0.0001] depression severity were observed. Two study-level moderators emerged as significant: ketamine effects (relative to placebo) were larger in studies that required a higher degree of previous treatment resistance to federal regulatory agency-approved antidepressant medications (≥2 failed trials) for study entry; and in studies that used a crossover design. A comprehensive data-driven search for combined moderators identified statistically significant, but modest and clinically uninformative, effects (effect size r ≤ 0.29, a small-medium effect). Ketamine robustly reduces depressive symptoms in a heterogeneous range of patients, with benefit relative to placebo even greater in patients more resistant to prior medications. In this largest effort to date to apply precision medicine approaches to ketamine treatment, no clinical or demographic patient-level features were detected that could be used to guide ketamine treatment decisions.Review Registration: PROSPERO Identifier: CRD42021235630

  • Speleothems as environmental recorders : A study of Holocene speleothems and their growth environments in Sweden

    • The main aim of this thesis was to contribute with detailed information of regional environmental change during the Holocene through studies of speleothems and their growth environments from two caves, Korallgrottan and Labyrintgrottan in northwestern Sweden, and a cellar vault in Uppsala. This was done through studies of stable isotopes and luminescence properties in the speleothems in combination with a detailed monitoring study in Korallgrottan.The monitoring study suggests that stalagmites fed by stalactites with slow and stable drip rates from deep inside the cave may be suitable as palaeoclimate archives. Similarities between oxygen isotope signals of contemporary samples from Labyrintgrottan and Korallgrottan emphasize the potential of speleothems from Labyrintgrottan to also provide high resolution regional palaeoclimate information.Except for a number of cold events stalagmite δ18O records from northern Scandinavia indicate that temperatures were warmer than today between 9500 and 6000 years ago. During this period the interval between 7800 and 6000 years ago seems to have been the warmest. The area above Labyrintgrottan was most likely covered by much denser vegetation than today at the time of stalagmite growth (9500-7500 years ago) and was, unlike today, probably situated below the local tree-limit between 9000 and 8000 years ago. The δ18O record of a stalagmite from Korallgrottan covering the last 4000 years agrees with the concept of a warmer period, the so called Medieval Warm Period, centred around AD1000 and a colder period, the so called Little Ice Age, somewhere between AD1000 and today.Studies of luminescence properties in fast growing speleothems from Uppsala indicate that the variations in luminescence intensity are annual and that the annual lamiae of the luminescent record represent a flush of organic material.
