    • Objective: Aberrant subclavian artery (ASA) and Kommerell's diverticulum (KD) are rare vascular anomalies that may be associated with lifestyle-limiting and life-threatening complications. The aim of this study is to report contemporary outcomes after invasive treatment of ASA/KD using a large international dataset.Methods: Patients who underwent treatment for ASA/KD (2000-2020) were identified through the Vascular Low Frequency Disease Consortium, a multi-institutional collaboration to investigate uncommon vascular disorders. We report the early and mid-term clinical outcomes including stroke and mortality, technical success, and other operative outcomes including reintervention rates, patency, and endoleak.Results: Overall, 285 patients were identified during the study period. The mean patient age was 57 years; 47% were female and 68% presented with symptoms. A right-sided arch was present in 23%. The mean KD diameter was 47.4 mm (range, 13.0-108.0 mm). The most common indication for treatment was symptoms (59%), followed by aneurysm size (38%). The most common symptom reported was dysphagia (44%). A ruptured KD was treated in 4.2% of cases, with a mean diameter of 43.9 mm (range, 18.0-100.0 mm). An open procedure was performed in 101 cases (36%); the most common approach was ASA ligation with subclavian transposition. An endovascular or hybrid approach was performed in 184 patients (64%); the most common approach was thoracic endograft and carotid-subclavian bypass. A staged operative strategy was employed more often than single setting repair (55% vs 45%). Compared with endovascular or hybrid approach, those in the open procedure group were more likely to be younger (49 years vs 61 years; P < .0001), female (64% vs 36%; P < .0001), and symptomatic (85% vs 59%; P < .0001). Complete or partial symptomatic relief at 1 year after intervention was 82.6%. There was no association between modality of treatment and symptom relief (open 87.2% vs endovascular or hybrid approach 78.9%; P = .13). After the intervention, 11 subclavian occlusions (4.5%) occurred; 3 were successfully thrombectomized resulting in a primary and secondary patency of 95% and 96%, respectively, at a median follow-up of 39 months. Among the 33 reinterventions (12%), the majority were performed for endoleak (36%), and more reinterventions occurred in the endovascular or hybrid approach than open procedure group (15% vs 6%; P = .02). The overall survival rate was 87.3% at a median follow-up of 41 months. The 30-day stroke and death rates were 4.2% and 4.9%, respectively. Urgent or emergent presentation was independently associated with increased risk of 30-day mortality (odds ratio [OR], 19.8; 95% confidence interval [CI], 3.3-116.6), overall mortality (OR, 3.6; 95% CI, 1.2-11.2) and intraoperative complications (OR, 8.3; 95% CI, 2.8-25.1). Females had a higher risk of reintervention (OR, 2.6; 95% CI, 1.0-6.5). At an aneurysm size of 44.4 mm, receiver operator characteristic curve analysis suggested that 60% of patients would have symptoms.Conclusions: Treatment of ASA/KD can be performed safely with low rates of mortality, stroke and reintervention and high rates of symptomatic relief, regardless of the repair strategy. Symptomatic and urgent operations were associated with worse outcomes in general, and female gender was associated with a higher likelihood of reintervention. Given the worse overall outcomes when symptomatic and the inherent risk of rupture, consideration of repair at 40 mm is reasonable in most patients. ASA/KD can be repaired in asymptomatic patients with excellent outcomes and young healthy patients may be considered better candidates for open approaches versus endovascular or hybrid modalities, given the lower likelihood of reintervention and lower early mortality rate.

  • International Multi-Institutional Experience with Presentation and Management of Aortic Arch Laterality in Aberrant Subclavian Artery and Kommerell's Diverticulum

    • BACKGROUND: Aberrant subclavian artery (ASA) with or without Kommerell's diverticulum (KD) is a rare anatomic aortic arch anomaly that can cause dysphagia and/or life-threatening rupture. The objective of this study is to compare outcomes of ASA/KD repair in patients with a left versus right aortic arch.METHODS: Using the Vascular Low Frequency Disease Consortium methodology, a retrospective review was performed of patients ≥18 years old with surgical treatment of ASA/KD from 2000 to 2020 at 20 institutions.RESULTS: 288 patients with ASA with or without KD were identified; 222 left-sided aortic arch (LAA), and 66 right-sided aortic arch (RAA). Mean age at repair was younger in LAA 54 vs. 58 years (P = 0.06). Patients in RAA were more likely to undergo repair due to symptoms (72.7% vs. 55.9%, P = 0.01), and more likely to present with dysphagia (57.6% vs. 39.1%, P < 0.01). The hybrid open/endovascular approach was the most common repair type in both groups. Rates of intraoperative complications, death within 30 days, return to the operating room, symptom relief and endoleaks were not significantly different. For patients with symptom status follow-up data, in LAA, 61.7% had complete relief, 34.0% had partial relief and 4.3% had no change. In RAA, 60.7% had complete relief, 34.4% had partial relief and 4.9% had no change.CONCLUSIONS: In patients with ASA/KD, RAA patients were less common than LAA, presented more frequently with dysphagia, had symptoms as an indication for intervention, and underwent treatment at a younger age. Open, endovascular and hybrid repair approaches appear equally effective, regardless of arch laterality.

    • The Zn-61(30)31 nucleus has been studied via the combined data of two fusion-evaporation reaction experiments using a Ar-36 beam and a Si-28 target foil. The experimental setups involved the Ge array GAMMASPHERE and neutron and charged particle detectors placed around the target position. The resulting level scheme comprises about 120 excited states connected via some 180 gamma-ray transitions. In total, seven rotational structures were identified up to I similar to 25 or higher and compared with predictions from cranked Nilsson-Strutinsky calculations.

    • A largely extended experimental knowledge of the Cu-61(29)32 nucleus has been obtained from three experiments. Excited states in Cu-61 were produced via the fusion-evaporation reaction Si-28(Ar-36, 3p)Cu-61. In addition to the Ge array GAMMASPHERE, neutron and charged-particle detectors placed around the target position were used for high-performance particle spectroscopy. The constructed level scheme includes more than 160 energy levels and 320 gamma-ray transitions belonging to both normally deformed as well as superdeformed rotational structures. The multipolarities have been determined for the gamma-ray transitions and as a result spin-parity assignments are given for nearly all energy levels. Experimental results in the normally deformed region are compared with predictions from large-scale shell model calculations. The collective structures are compared with results from cranked Nilsson-Strutinsky calculations. The results reveal the need to modify the standard Nilsson parameters in the mass A similar to 60 region.

  • Diversity and Life Habits of Silurian Strophomenide Brachiopods of Gotland

    • The Superfamily Strophomenoidea is a very diverse group of brachiopods in the Early Palaeozoic. In the Silurian succession on Gotland, they are among the most easily identifiable, and commonly found fossils. However, there are few detailed studies of this group from Gotland, and no new strophomenide taxa have been described from this area since 1869. The life habits of strophomenides are also poorly understood, partly because their morphology differs greatly from that of living brachiopods. The succession on Gotland yielded 27 species belonging to the Strophomenoidea, of which two species and two subspecies are new. The remaining species have been described earlier from Gotland or Great Britain. The most important group is the Leptaeninae, which occurs commonly throughout the entire succession on Gotland. Five species (one new), two new subspecies and two taxa treated under open nomenclature were found. Leptaena rhomboidalis and Lepidoleptaena poulseni were specialized for life in shallow water environments, retaining a large apical pedicle; the remaining leptaenines were ambitopic. The Furcitellininae is represented by three genera comprising six species, of which only one persisted into the Ludlow. All were ambitopic, except Pentlandina loveni, which was specialized for high-energy environments. Six species of “strophodontids” from Gotland, belonging to the Leptostrophidae, the Strophodontidae and the Shaleriidae, share a shallow-bodied, variably concavo-convex shell with costellate ornament. Mesoleptostrophia and Brachyprion (Brachyprion) were long-ranged and ecologically tolerant, while B. (Erinostrophia), Strophodonta and Shaleria had short ranges and were ecologically specialized. The two earliest known cementing strophomenides occur on Gotland, and their hitherto unknown dorsal valves have been identified: Liljevallia was found to belong to the Douvillinidae. The cementing Leptaenoidea silurica was found to be conspecific with the ambitopic Scamnomena rugata; it was able to live ambitopically if removed from the substrate. The thickened dorsal valves allow reconstruction of its lophophore.

    • Twelve species of Brachiopods are described from the Silurian of Gotland, six furcitellinines and six "strophodontids." One is new-Strophodonta hoburgensis n. sp. The furcitellinines are moderately common and diverse in the lower part of the succession, but the last species disappears in the middle Hemse beds (similar to middle Ludlow). Three genera are represented: Bellimurina, Pentlandina and Katastrophomena, with the species and subspecies B. wisgoriensis, P. tartana, P. loveni, P. lewisii lewisii, K. penkillensis and K. antiquata scabrosa. Most of the taxa are confined to low energy environments, but P. loveni was evidently specialized for the high energy reef environments of the Hogklint Formation. B. wisgoriensis displays environmentally induced morphological variability in developing strong, frilly growth lamellae in high-energy environments. The "strophodontids," although belonging to three different families, share a common morphology consisting of denticles along the hinge line, a semi-circular outline, unequally to finely costellate ornament and, most importantly, a concavo-convex profile with both valves of the same curvature, enclosing a very small body chamber. Two leptostrophiids are generalists, occurring in both high- and low-energy environments and with long stratigraphical ranges [Mesoleptostrophia filosa; latest Llandovery through the entire Ludlow. Brachyprion (Brachyprion) semiglobosa; latest Llandovery to latest Wenlock]. The third leptostrophiid (Brachyprion (Erinostrophia) walmstedti) is short ranged and occurs in low-energy environments in the latest Llandovery. The species belonging to the Strophodontidae (Strophodonta hoburgensis n. sp.) and Shaleriidae [Shaleria (Janiomya) ornatella and S. (Shaleriella) ezerensis] occur only in high-energy environments and have a short range within the late Ludlow.

    • Background: Birth by caesarean section (CS) is associated with development of allergic diseases, but its role in the development of atopic dermatitis (AD) is less convincing. Objective: Our primary aim was to determine if birth mode was associated with AD in 3-year-olds and secondarily to determine if birth mode was associated with early onset and/or persistent AD in the first 3 years of life. Methods: We included 2129 mother–child pairs from the Scandinavian population-based prospective PreventADALL cohort with information on birth mode including vaginal birth, either traditional (81.3%) or in water (4.0%), and CS before (6.3%) and after (8.5%) onset of labor. We defined early onset AD as eczema at 3 months and AD diagnosis by 3 years of age. Persistent AD was defined as eczema both in the first year and at 3 years of age, together with an AD diagnosis by 3 years of age. Results: AD was diagnosed at 3, 6, 12, 24, and/or 36 months in 531 children (25%). Compared to vaginal delivery, CS was overall associated with increased odds of AD by 3 years of age, with adjusted odds ratio (95% confidence interval) of 1.33 (1.02-1.74), and higher odds of early onset AD (1.63, 1.06-2.48). The highest odds for early onset AD were observed in infants born by CS after onset of labor (1.83, 1.09-3.07). Birth mode was not associated with persistent AD. Conclusion: CS was associated with increased odds of AD by 3 years of age, particularly in infants presenting with eczema at 3 months of age.

    • High-spin states in the doubly magic N=Z nucleus Ni-56 have been investigated with three fusion-evaporation reaction experiments. New gamma-ray transitions are added, and a confirmation of a previously suggested prompt proton decay from a rotational band in Ni-56 into the ground state of Co-55 is presented. The rotational bands in Ni-56 are discussed within the framework of cranked Nilsson-Strutinsky calculations.

    • A new decay mode, the so called prompt proton decay, was discovered in 1998. It has since proven to be an important decay mechanism for several neutron deficient nuclei in the A similar to 60 region. To measure with high accuracy the energies and angular distributions of these protons, a state-of-the-art charged particle detector - LuWuSiA - was developed. It was first utilized during a fusion-evaporation reaction experiment performed at Argonne National Laboratory, U.S.A. In this contribution, the characteristics of the prompt proton decay are discussed along with the special features of LuWuSiA as well as a revisit to the prompt proton decay in Cu-58.
