    • Abnormal expression of several key regulators essential for G1/S transitions has been implicated in tumorigenesis. A critical role of cyclin A1 in the development of acute myeloid leukemia (AML) has previously been demonstrated in transgenic mice. Our present study focused on the expression and prognostic significance of cyclin A1 and a panel of cell cycle regulatory proteins including cyclin A2, cyclin B1, cyclin E, CDK1, CDK2, p21 and p27 in bone marrow samples from 40 patients with AML. Freshly isolated CD34+ hematopoietic cells and bone marrow samples from 10 healthy donors were also assessed for cell type- and subcellular-specific expression of the cell cycle regulatory proteins. The level of cyclin A1 expression was the only factor that showed a significant correlation with patient outcome. In log-rank test stratified by levels of cyclin A1 expression, patients with high levels of cyclin A1 had significantly worse overall survival (OS) (P = 0.012) compared to those with low levels. Further, patients with high levels of cyclin A1 had significantly lower disease-free survival (DFS) (P = 0.028). Multivariate analysis indicated that cyclin A1 protein expression was an independent prognostic factor for predicting DFS (P = 0.035) and OS (P = 0.045). No correlation between cyclin A1 expression and age was found. However, expression of cyclin A2, cyclin B1, cyclin E, CDK1, CDK2, p21 and p27 did not show prognostic significance in these AML patients.

    • An important role of the cell cycle regulatory protein cyclin A1 in the development of acute myeloid leukemia (AML) was previously demonstrated in a transgenic mouse model. We have now turned our attention to study specific aspects of the activity and subcellular distribution of cyclin A1 using bone marrow samples from normal donors and patients with AML, as well as leukemic cell lines. We show that the localization of cyclin A1 in normal hematopoietic cells is nuclear, whereas in leukemic cells from AML patients and cell lines, it is predominantly cytoplasmic. In leukemic cell lines treated with all-trans retinoic acid (ATRA), cyclin A1 localized to the nucleus. Further, there was a direct interaction between cyclin A1 and cyclin-dependent kinase 1, as well as a major ATRA receptor, RARalpha, in ATRA-treated cells but not in untreated leukemic cells. Our results indicate that the altered intracellular distribution of cyclin A1 in leukemic cells correlates with the status of the leukemic phenotype.

    • Den svenska fjärrvärmesektorn har stor potential att bidra med negativa koldioxidutsläpp genom bio-CCS, minst 10 Mton per år. Den största osäkerheten beträffande möjligheterna för bio-CCS gäller marknads förutsättningarna. Uppvärmningsbranschen har en vision om att år 2045 utgöra en kolsänka. Ett sätt att åstadkomma detta är genom att avskilja och lagra koldioxidutsläpp med biogent ursprung. Ett antal fjärrvärmeföretag har redan olika långt gångna planer på att satsa på bio-CCS. De har sett ett värde i att samarbeta kring hur detta kan åstadkommas. Ett led i detta är projektet Bio-CCS i fjärrvärmesektorn som består av ett gediget underlag baserat på forskning kring olika aspekter av frågan samt en strategi baserad på det underlaget. I denna rapport redovisas en syntes av detta forskningsarbete. Projektet visar att fjärrvärmesektorn har stor teoretisk potential att bidra med negativa koldioxidutsläpp, minst 10 Mton per år. I huvudsak är avskiljning, transport och lagring av koldioxid beprövad teknik även om tillämpningen i detta fall är ny. Även om bio-CCS är förknippad med energianvändning så bidrar tekniken sett ur ett systemperspektiv med stor nytta för att minska koldioxid[1]utsläppen. Bio-CCS är en relativt dyr teknik och det är angeläget att utnyttja samverkan och kluster för att exempelvis skapa ökad kostnadseffektivitet i transport och mellanlagring. Tillgång till lagringsplatser är en förutsättning för framgång och flera alternativ bedöms bli tillgängliga. Det kan dock uppstå konkurrens om tillgången till lagringsplatserna. De regelmässiga förutsättningarna för bio-CCS i Sverige har förbättrats avsevärt de senaste dryga decenniet. Flera regelmässiga hinder kvarstår dock. En del utgör mindre barriärer, andra är av mer betydande karaktär. Den största osäkerheten beträffande möjligheterna för bio-CCS gäller ekonomin. Flera potentiella finansieringsmetoder har studerats, både stöd, regleringar och frivilligmarknader. Det finns fortfarande oklarheter kring syftet med planerade stöd och det framtida ägandet av de negativa utsläppen. Det genomförda projektet har skapat ett forum för kunskapsuppbyggnad, erfarenhetsutbyte och nätverkande, vilket de deltagande företagen bedömt vara mycket värdefullt.

  • Fångst av vegetationsdata och Natura 2000-habitat i fjällen genom flygbildstolkning i IRF med punktgittermetodik

    • I denna rapport visas att flygbildstolkning med punktgittermetodik har stor potential för att ta fram tillförlitliga och kostnadseffektiva data på tillstånd och förändringar i fjällens vegetation. Projektet är finansierat av Naturvårdsverkets svenska miljöövervakning.Inom denna studie har följande delmål nåtts1) Utprovning av försöksdesign har gjorts med antal och fördelning av punkter, samt design och val av tolkningsvariabler.2) Ett operativt tolkningssystem för punktgittertolkning har tagits fram.3) Tolkningsprecisionen för olika variabler och Natura 2000-habitat är utvärderade mot fältdata.4) Metodens kvaliteter är utvärderade.5) Beräkning av tidsåtgång och kostnadsberäkningar är gjorda och ställda mot en polygonbaserad metod för översiktlig tolkning av NILS 5x5 km ruta.Förutom dessa delmål har även säkerheten ytterligare undersökts via ett personvariationstest av tolkade data.Resultaten från de tolkade fältpunkterna visar att överensstämmelsen med fältinventeringen är mycket god. Ett gott resultat fanns från testet med personvariation, med några få problemområden. Metoden har några begränsningar. För flygbildstolkning i punktgitter med den metodik som föreslås här förutsätts att det finns välutbildade tolkare. För en van vegetationstolkare krävs det i storleksordning en tilläggsutbildning på minst 1 månad i tolkning av fjällvegetation, inklusive fältbesök. En exempelsamling av bilder och ett antal nycklar för tolkning bör tas fram.Punktgittermetoden är operativt användbar, vissa kompletterande fältstudier behövs. Metoden har följande generella kvaliteter.Metoden är enkel att implementera.Punktgittermetoden är en kostnadseffektiv metod för att fånga landskapsdata. Den är snabbare än polygontolkning.Datafångst från en 5 x 5 km ruta kan göras på 1-3 dagar, beroende på antal punkter i gittret.Det är enkelt att bearbeta och analysera punktgitterdata.Precisionen i arealskattningarna kommer att kunna enkelt beräknas.Tolkningen i punktgitter kan verifieras genom jämförelse med NILS fältdata.Metoden är lämplig för förändringsstudier. Exempelvis kan den utgöra ett snabbt och effektivt sätt att statistiskt uppdatera information om area av vegetationstyper från vegetationskartorna över de svenska fjällen.Metoden kan fånga upp ovanligare naturtyper i 5 x 5 km ytan förutsatt att många punkter tolkas.Metoden kan användas för urval av objekt för riktade fältinventeringar till ovanligare habitat.Resultaten från metoden kan utgöra viktiga data för annan forskning, exempelvis som träningsdata till satellitbildsklassificeringar.

    • Several studies have shown slight differences in language skills between genders, favouring females. In order to investigate gender differences in speech production for Swedish children, the productive vocabulary size of 295 children, aged 18-24 months, was measured by the validated instrument SECDI-2. The size of the productive vocabulary was found to grow rapidly during this age. Significant gender differences were found at 21 and 24 months, but not at 18 months. The girls’ mean scores were higher.

  • Outcomes After Elective Inguinal Hernia Repair Performed by Associate Clinicians vs Medical Doctors in Sierra Leone A Randomized Clinical Trial

    • IMPORTANCE Task sharing of surgical duties with medical doctors (MDs) without formal surgical training and associate clinicians (ACs; health care workers corresponding to an educational level between that of a nurse and an MD) is practiced to provide surgical services to people in low-resource settings. The safety and effectiveness of this has not been fully evaluated through a randomized clinical trial. OBJECTIVE To determine whether task sharing with MDs and ACs is safe and effective in mesh hernia repair in Sierra Leone. DESIGN, SETTING, AND PARTICIPANTS This single-blind, noninferiority randomized clinical trial included adult, healthy men with primary inguinal hernia randomized to receiving surgical treatment from an MD or an AC. In Sierra Leone, ACs practicing surgery have received 2 years of surgical training and completed a 1-year internship. The study was conducted between October 2017 and February 2019. Patients were followed up at 2 weeks and 1 year after operations. Observers were blinded to the study arm of the patients. The study was carried out in a first-level hospital in rural Sierra Leone. Data were analyzed from March to June 2019. INTERVENTIONS All patients received an open mesh inguinal hernia repair under local anesthesia. The control group underwent operations performed by MDs, and the intervention group underwent operations performed by ACs. MAIN OUTCOMES AND MEASURES The primary end point was hernia recurrence at 1 year. Outcomes were assessed by blinded observers at 2 weeks and 1 year after operations. RESULTS A total of 230 patients were recruited (mean [SD] age, 43.0 [13.5] years), and all but 1 patient underwent inguinal hernia repair between October 23, 2017, and February 2, 2018, performed by 5 MDs and 6 ACs. A total of 114 patients were operated on by MDs, and 115 patients were operated on by ACs. There were no crossovers between the study arms. The follow-up rate was 100% at 2 weeks and 94.1% at 1 year. At 1 year, hernia recurrence occurred in 7 patients (6.9%) operated on by MDs and 1 patient (0.9%) operated on by ACs (absolute difference, -6.0 [95% CI, -11.2 to 0.7] percentage points; P < .001). CONCLUSIONS AND RELEVANCE These findings demonstrate that task sharing of elective mesh inguinal hernia repair with ACs was safe and effective. The task sharing debate should progress to focus on optimizing surgical training programs for nonsurgeons and building capacity for elective surgical care in low- and middle-income countries.

  • Standardised Competency-Based Training of Medical Doctors and Associate Clinicians in Inguinal Repair with Mesh in Sierra Leone

    • IntroductionIn low-income settings, there is a high unmet need for hernia surgery, and most procedures are performed with tissue repair techniques. In preparation for a randomized clinical trial, medical doctors and associate clinicians received a short-course competency-based training on inguinal hernia repair with mesh under local anaesthesia. The aim of this study was to evaluate feasibility, safety and effectiveness of the training.MethodsAll trainees received a one-day theoretical module on mesh hernia repair under local anaesthesia followed by hands-on training. Performance was assessed using the American College of Surgeons Groin Hernia Operative Performance Rating System. Patients were followed up two weeks and one year after surgery. Outcomes of the patients operated on during the training trial were compared to the 229 trial patients operated on after the training.ResultsDuring three surgical camps, seven medical doctors and six associate clinicians were trained. In total, 129 patients were operated on as part of the training. Of the 13 trainees, 11 reached proficiency. Patients in the training group had more wound infections after two weeks (8.5% versus 3.1%; p = 0.041). There was no difference in recurrence and mortality after one year, and none of the deaths were attributed to the surgery.Discussion and conclusionMesh repair is the international standard for inguinal hernia repair worldwide. Nevertheless, this is not widely accessible in low-income settings. This study has demonstrated that short-course intensive hands-on training of MDs and ACs in mesh hernia repair is effective and safe.Trial Registration: International Clinical Trial Registry ISRCTN63478884.

  • OSA Is Associated With the Human Gut Microbiota Composition and Functional Potential in the Population-Based Swedish CardioPulmonary bioImage Study

    • BACKGROUND: Obstructive sleep apnea (OSA) is a common sleep-breathing disorder linked to increased risk of cardiovascular disease. Intermittent hypoxia and intermittent airway obstruction, hallmarks of OSA, have been shown in animal models to induce substantial changes to the gut microbiota composition and subsequent transplantation of fecal matter to other animals induced changes in blood pressure and glucose metabolism.RESEARCH QUESTION: Does obstructive sleep apnea in adults associate with the composition and metabolic potential of the human gut microbiota?STUDY DESIGN AND METHODS: We used respiratory polygraphy data from up to 3,570 individuals aged 50-64 from the population-based Swedish CardioPulmonary bioImage Study combined with deep shotgun metagenomics of fecal samples to identify cross-sectional associations between three OSA parameters covering apneas and hypopneas, cumulative sleep time in hypoxia and number of oxygen desaturation events with gut microbiota composition. Data collection about potential confounders was based on questionnaires, on-site anthropometric measurements, plasma metabolomics, and linkage with the Swedish Prescribed Drug Register.RESULTS: We found that all three OSA parameters were associated with lower diversity of species in the gut. Further, the OSA-related hypoxia parameters were in multivariable-adjusted analysis associated with the relative abundance of 128 gut bacterial species, including higher abundance of Blautia obeum and Collinsela aerofaciens. The latter species was also independently associated with increased systolic blood pressure. Further, the cumulative time in hypoxia during sleep was associated with the abundance of genes involved in nine gut microbiota metabolic pathways, including propionate production from lactate. Lastly, we observed two heterogeneous sets of plasma metabolites with opposite association with species positively and negatively associated with hypoxia parameters, respectively.INTERPRETATION: OSA-related hypoxia, but not the number of apneas/hypopneas, is associated with specific gut microbiota species and functions. Our findings lay the foundation for future research on the gut microbiota-mediated health effects of OSA.

    • The environmental toxicants examined in this report can be classified into five groups –heavy metals, chlorinated compounds, brominated flame retardants, polyaromatichydrocarbons and perfluorinated compounds. Each of these contaminants has beenexamined from various sites for up to six different fish species, in blue mussels, and inguillemot eggs, for varying lengths of time. The following summary examines overalltrends, spatial and temporal, for the five groups.Condition and Fat ContentCondition and fat content in different species tended to follow the same pattern at the samesites, with a few exceptions. Most of the fish species generally displayed a decreasing trendin both condition and fat content at most sites examined. Exceptions to this were increasesin fat content seen in herring (the last ten years) and cod at Fladen; an increase in conditionfor herring at Ängskärsklubb in spring; and in perch, a decrease in fat content atKvädöfjärden but no trend in condition for the same site.Heavy MetalsDue to a change in methods for metal analysis (not Hg) in 2004, values between 2003 and2007 should be interpreted with care. From 2009 metals are analyzed at ITM, Stockholmuniversity.The longer time series in guillemot egg and spring-caught herring from the southernBothnian Sea and southern Baltic Proper show significant decreases of mercury. Theherring site in the southern Bothnain Sea indicates a local Hg-source. The rest of the timeseries show varying concentrations over the study period, and even increasing trends in e.g.cod muscle and blue mussels, but the concentrations are fairly low compared to measuredconcentrations in perch from fresh water and coastal sites. However, in most cases, theseconcentrations are above the newly suggested EU-target level of 20 ng/g wet weight.Lead is generally decreasing over the study period (in time series of sufficient length),supposedly due to the elimination of lead in gasoline. Elevated lead concentrations between2003 and 2007 (e.g. Harufjärden) should be viewed with caution (see above regardingchange in analysis methods).Cadmium concentrations show varying non-linear trends over the monitored period. It isworth noting that despite several measures taken to reduce discharges of cadmium,generally the most recent concentrations are similar to concentrations measured 30 yearsago in the longer time series.The reported nickel concentrations show no consistent decreasing trends. Some series beginwith two elevated values that exert a strong leverage effect on the regression line and maygive a false impression of decreasing trends. Chromium generally shows decreasing trends,possibly explained by a shift in analytical method. The essential trace metals, copper andzinc, show no consistent trends during the monitored period.

  • Regulation of Decay Accelerating Factor Primes Human Germinal Center B Cells for Phagocytosis

    • Germinal centers (GC) are sites for extensive B cell proliferation and homeostasis is maintained by programmed cell death. The complement regulatory protein Decay Accelerating Factor (DAF) blocks complement deposition on host cells and therefore also phagocytosis of cells. Here, we show that B cells downregulate DAF upon BCR engagement and that T cell-dependent stimuli preferentially led to activation of DAF(lo) B cells. Consistent with this, a majority of light and dark zone GC B cells were DAF(lo) and susceptible to complement-dependent phagocytosis, as compared with DAF(hi) GC B cells. We could also show that the DAF(hi) GC B cell subset had increased expression of the plasma cell marker Blimp-1. DAF expression was also modulated during B cell hematopoiesis in the human bone marrow. Collectively, our results reveal a novel role of DAF to pre-prime activated human B cells for phagocytosis prior to apoptosis.

  • Från datafångst till datavärdskap – översyn av datahanteringen i Nationell Inventering av Landskapet i Sverige (NILS)

    • Syftet med denna utredning har varit att se över hela datahanteringen i NILS för att kunna identifiera särskilt kritiska processer och för att få en bild av helheten. I rapporten presenteras en kravspecifikation för de nya system (datahantering, statistik/redovisning och datavärdskap) som behöver byggas upp, de resurser som behövs för detta samt ges ett förslag på hur och i vilken tidsordning som systemen bör byggas upp.I rapporten presenteras den rekommenderade framtida datahanteringen inom NILS, från datafångst till datavärdskap. Data som idag ingår i datahanteringen för NILS är fältdata, tolkningsdata samt medföljande metadata, stöddata och annan nödvändig dokumentation. I denna rapport föreslås att datahantering i NILS skall delas in i fyra olika huvudprocesser.Den första processen ärDatafångstprocessen som samlar in rådata, samt kvalitetssäkrar och levererar detta. I NILS datafångstprocess ingår i dagsläget två delar, dels fältinventering, dels flygbildstolkning av 1 km x 1 km rutan. Det är nödvändigt att uppbyggnaden av fältdatabas och tolkningsdatabas samordnas vad gäller såväl systemstruktur, variabelnamn mm för att underlätta samkörningar av data. Den största kraften och resurserna skall läggas på att effektivisera datahanteringen inom datafångsten för då uppnås den största nyttan med insatserna.Den andra processen ärDatabasprocessen som tar emot rådata, metadata och stöddata från fält- och flygbildsprocessen i datafångsten och lagrar detta i ett databassystem. I systemet lagras även beräknade data, alternativt beräkningsfunktioner (script) som utför beräkningar direkt beroende på olika användares behov. För både flygbildstolkning och fältinventering skall normaliserade databaser upprättas. Detta är en absolut nödvändighet och ett krav för att Databas och Datavärdskap skall vara genomförbart.Den tredje processen ärStatistikprocessen som producerar skattningar, bearbetningar, sammanställningar och resultatrapporter från NILS grunddata i databasen.Den fjärde processen utgörs av självaDatavärdskapet där det ingår att ta emot och kontrolleradata, tillgängliggöra data för externa användare (via webb-portal bl.a.) samt att permanent arkivera kvalitetssäkrade data. I datavärdskapet ingår grunddata från NILS rådatabas samt bearbetade data, resultatrapporter, sammanställningar av data, tabeller, figurer mm som producerats i Statistikprocessen.Denna rapport ger följande övergripande förslag till hur huvudprocesserna i NILS datahantering skall genomföras och byggas upp:Beställarens uppdrag skall vara klart definierat för utförarenTydligt uppsatta mål för arbetetUtförlig planering - baserat på tillräckligt kunskapsunderlagTydligt definierade roller för personalenAtt personalen har de personella, ekonomiska och materiella resurser som behövs för genomförandet (kompetens, tid, verktyg, utrustning mm)Planera helheten, men bygg från litet till stort, undvik onödig komplexitetDelleveranser till avnämare ska ske tidigt och fortlöpandeKvalitetssäkring ska finnas i alla processerI samtliga huvudprocesser skall det ingå tillräcklig säkerhetskopiering av data, anpassad till varje process särskilda kravTillräcklig dokumentation av alla ingående delprocesserTydlig strategi för att minimera riskerFortlöpande dialog och adaptiv utveckling – för att säkerställa att målen nåsKostnadseffektivitet i alla led – beakta möjligheter till intern och extern samordningSamverkan – såväl inom som utom SLU.Under arbetets gång har särskilt kritiska moment i utvecklingsarbetet identifierats bl.a. lednings- och organisationsstrukturen, val av programvara för databaserna samt kvalitetssäkring av rådata. Dessa bör lyftas fram i det fortsatta arbetet.Rapporten ger ett förslag till en 5-årsplan för uppbyggnad av huvudprocesserna. De mest kritiska processerna är uppbyggnaden av kvalitetsäkrade rådatabaser för fältinventeringen och flygbildstolkning. Fokus de första åren ligger därför på NILSbasen och statistikdelen.     • WISP2 is a novel adipokine, most highly expressed in the adipose tissue and primarily in undifferentiated mesenchymal cells. As a secreted protein, it is an autocrine/paracrine activator of canonical WNT signaling and, as an intracellular protein, it helps to maintain precursor cells undifferentiated. To examine effects of increased WISP2 in vivo, we generated an aP2-WISP2 transgenic (Tg) mouse. These mice had increased serum levels of WISP2, increased lean body mass and whole body energy expenditure, hyperplastic brown/white adipose tissues and larger hyperplastic hearts. Obese Tg mice remained insulin sensitive, had increased glucose uptake by adipose cells and skeletal muscle in vivo and ex vivo, increased GLUT4, increased ChREBP and markers of adipose tissue lipogenesis. Serum levels of the novel fatty acid esters of hydroxy fatty acids (FAHFAs) were increased and transplantation of Tg adipose tissue improved glucose tolerance in recipient mice supporting a role of secreted FAHFAs. The growth-promoting effect of WISP2 was shown by increased BrdU incorporation in vivo and Tg serum increased mesenchymal precursor cell proliferation in vitro. In contrast to conventional canonical WNT ligands, WISP2 expression was inhibited by BMP4 thereby allowing normal induction of adipogenesis. WISP2 is a novel secreted regulator of mesenchymal tissue cellularity.

    • This article aims to investigate how Nordic countries – exemplified by Denmark, Finland, and Sweden – govern the education and inclusion of newly arrived students. Approaching this through policy as text and practice, we identify the subject positions of the newly arrived students. Our research data consist of national policy documents, legislation, and evaluation reports on the education of newly arrived students, which we compare and analyse. We conclude that newly arrived students are subject to underachievement, bullying, discrimination, and at risk of not continuing their education. Despite all good intentions the Nordic policies do not seem to include all students.

    • A critical cell cycle regulatory protein, cyclin A1, has been implicated in the development of acute myeloid leukemia (AML). Here, we have examined the expression and clinical significance of cyclin A1 in childhood acute lymphoblastic leukemia (ALL). Cyclin A1 was highly expressed in lymphoblastic leukemic cell lines and in 22 of 30 ALL patients (73%). Cyclin A1 expression correlated with patient age (P=0.006), but not with cytogenetic abnormalities. Patients with high levels of cyclin A1 had poorer event-free survival (57.9%) compared to patients with lower levels (75%).

    • Introduction: Bacteraemia is a frequent infectious condition that strongly affects morbidity and mortality. The incidence is increasing worldwide. This study explores all-cause 30-day mortality after bacteraemia in two out of Denmark’s five healthcare regions with approximately 2.4 million inhabitants. Methods: Clinically significant bacteraemia episodes (n = 55,257) were identified from a geographically well-defined background population between 2000 and 2014, drawing on population-based data regarding bacterial species and vital status. All-cause 30-day mortality was assessed in relation to bacteraemia episodes, number of patients with analysed blood cultures and the background population. Results: We observed a decreasing trend of all-cause 30-day mortality between 2000 and 2014, both in relation to the number of bacteraemia episodes and the background population. Mortality decreased from 22.7% of the bacteraemia episodes in 2000 to 17.4% in 2014 (annual IRR [95% CI]: 0.983 [0.979–0.987]). In relation to the background population, there were 41 deaths per 100,000 inhabitants in 2000, decreasing to 39 in 2014 (annual IRR [95% CI]: 0.988 [0.982–0.993]). Numbers of inhabitants, bacteraemia episodes, and analysed persons having BCs increased during the period. Conclusions: All-cause 30-day mortality in patients with bacteraemia decreased significantly over a 15-year period.

    • I Sverige skadas 40 000 människor av brännskador per år, och 1 300 patienter vårdas på sjukhus. Tidigare studier har beskrivit ett samband mellan socioekonomisk status och skaderisk hos barn, där brännskador inkluderas. I de flesta höginkomstländer har incidensen av brännskador minskat. I Sverige minskade antalet brännskador med 45 procent mellan 1987 och 2010 [1]. Förebyggande strategier med bland annat branddetektorer, elsäkerhet, vattentemperaturreglering och säkrare byggnader samt utbildning har bidragit till denna positiva utveckling.I Sverige finns två brännskadecentrum – ett i Linköping och ett i Uppsala. Man hade noterat ett ökat inflöde av barn med brännskador, och detta behövde undersökas närmare.En retrospektiv studie [2] med uppgifter från dessa två centrum kombinerade med uppgifter från Statistiska centralbyrån och Migrationsverket genomfördes därför för perioden 2010–2020. Alla barn (0–17 år) som hade vårdats inom öppenvård eller slutenvård ingick i studien. Studien omfattade 2 455 patienter. De flesta av barnen var under 5 år (76 procent) och var pojkar (60 procent). Studiedeltagarnas postnummer registrerades, och 83 procent (n = 1 974) bodde i områden med en inkomstnivå under den nationella medianen (Figur 1).Barn med asylstatus hade närmare 10 gånger ökad risk (medelvärde 9,6) att drabbas av brännskador jämfört med andra barn. Det är känt sedan tidigare studier runt om i världen att risken för att få en brännskada är kopplad till låg socioekonomisk status. Detta stämmer, enligt studien, även i Sverige.Brännskador hos barn kan undvikas genom prevention. Givet den ökade risken för brännskador hos barn bland asylsökande och dem som bor i områden med låg inkomst bör preventiva åtgärder designas för och implementeras med dessa grupper i åtanke.Den ökade risken kan vara kopplad till hur barnen och deras familjer bor, då trångboddhet är en riskfaktor för brännskador hos barn.

Children with asylum status were over-represented compared with residents and/or Swedish citizens.Conclusions: In Sweden, most pediatric burns occur in families that live in areas with low-income levels. Pediatric burns affect children with asylum status disproportionally compared with those who are residents in and/or citizens of Sweden. Prevention strategies should be designed and implemented to alleviate this health inequity.

    • IRL790 ([2-(3-fluoro-5-methanesulfonylphenoxy)ethyl](propyl)amine, mesdopetam) is a novel compound in development for the clinical management of motor and psychiatric disabilities in Parkinson disease. The discovery of IRL790 was made applying a systems pharmacology approach based on in vivo response profiling. The chemical design idea was to develop a new type of DA D3/D2 receptor type antagonist built on agonist rather than antagonist structural motifs. We hypothesized that such a dopamine antagonist with physicochemical properties similar to agonists would exert antidyskinetic and antipsychotic effects in states of dysregulated dopaminergic signaling while having little negative impact on physiologic dopamine transmission and, hence, minimal liability for side effects related to dopamine-dependent functions. At the level of in vivo pharmacology, IRL790 displays balancing effects on aberrant motor phenotypes, reducing L-DOPA-induced dyskinesias in the rodent 6-hydroxydopamine lesion model and reducing psychostimulant-induced locomotor hyperactivity elicited by pretreatment with either d-amphetamine or dizocilpine, without negatively impacting normal motor performance. Thus, IRL790 has the ability to normalize the behavioral phenotype in hyperdopaminergic as well as hypoglutamatergic states. Neurochemical and immediate early gene (IEG) response profiles suggest modulation of DA neurotransmission, with some features, such as increased DA metabolites and extracellular DA, shared by atypical antipsychotics and others, such as increased frontal cortex IEGs, unique to IRL790. IRL790 also increases extracellular levels of acetylcholine in the prefrontal cortex and ventral hippocampus. At the receptor level, IRL790 appears to act as a preferential DA D3 receptor antagonist. Computational docking studies support preferential affinity at D3 receptors with an agonist-like binding mode. SIGNIFICANCE STATEMENT This paper reports preclinical pharmacology along with molecular modeling results on IRL790, a novel compound in clinical development for the treatment of motor and psychiatric complications in advanced Parkinson disease. IRL790 is active in models of perturbed dopaminergic and glutamatergic signaling, including rodent 6-hydroxydopamine L-DOPA-induced dyskinesias and psychostimulant-induced hyperactivity, in a dose range that does not impair normal behavior. This effect profile is attributed to interactions at dopamine D2/D3 receptors, with a 6- to 8-fold preference for the D3 subtype.

    • We propose that analysing interviews with subjects who have been exposed to anthropomorphic characters from a metaphorical point of view can provide insights into how characters in the interface are perceived. In a study of the Agneta & Frida system (two characters that comment contents of web pages in an ironic, humorous manner) we found that subjects who used Agneta & Frida used more narrative verbs and adverbs than users who only browsed the web pages. In the latter case, more spatial verbs and adverbs were used. This may imply that normal web browsing is perceived as navigation through a space, while Agneta & Frida provides for a more narrative experience.

    • Recombinant single-chain fragment variable (scFv) antibodies with specificity to tumor antigens can be used to target tumors in vivo. The approach to use administration of complexes of idiotypic-anti-idiotypic scFvs when targeting tumors has not been tested earlier, and from a theoretical point it could contribute to longer in vivo circulation and improved targeting efficiency by dissociation, when in contact with the target antigen. In this study two models to evaluate the targeting efficiency of such complexes were used. HeLa HEp-2 tumor cells were grown as multicellular tumor spheroids (MCTS) and exposed to the antibody constructs in vitro. The behavior in vivo was tested in an in vivo tumor xenograft model. To increase the size of the anticytokeratin 8 scFv, TS1-218, complexes were formed between TS1-218 and its anti-idiotype, alphaTS1 scFv. The functionality of (125)I-labeled TS1-218 alone and in complex was studied in both models. The uptake patterns were similar in both models. The idiotypic TS1-218 was able to localize to the MCTS and xenografted tumors, both alone and in complex with alphaTS1 scFv. TS1-218 in complex, however, demonstrated a significantly higher uptake than the monomeric TS1-218 in both models (p < 0.0005 and p < 0.0089, respectively). When complexes were administered in vivo, a slower clearance and an increased tumor half-life could be observed. The present investigation indicates that administration of targeting antibodies, with initially blocked antigen-binding sites by complex formation with their anti-idiotypes, may improve targeting efficiency.

    • Protection of the large intestine with its enormous amount of commensal bacteria is a challenge that became easier to understand when we recently could describe that colon has an inner attached mucus layer devoid of bacteria (Johansson et al. (2008) Proc. Natl. Acad. Sci. USA 105, 15064-15069). The bacteria are thus kept at a distance from the epithelial cells and lack of this layer, as in Muc2-null mice, allow bacteria to contact the epithelium. This causes colitis and later on colon cancer, similar to the human disease Ulcerative Colitis, a disease that still lacks a pathogenetic explanation. Dextran Sulfate (DSS) in the drinking water is the most widely used animal model for experimental colitis. In this model, the inflammation is observed after 3-5 days, but early events explaining why DSS causes this has not been described.

Det här projektet har utrett möjligheterna för fjärrvärmebranschen att bidra med negativa utsläpp genom avskiljning, transport och lagring av biogen koldioxid från kraft‐ och fjärrvärmeanläggningar inklusive avfallsförbränningsanläggningar. Uppvärmningssektorn har som vision att år 2045 vara en kolsänka som hjälper till att minska de totala svenska växthusgasutsläppen. Inom sektorn finns stor potential för bio‐CCS eftersom en betydande andel av Sveriges biogena utsläpp kan härledas till punktutsläpp inom sektorn. Men för att realisera potentialen och uppvärmningssektorns vision om att bli en kolsänka krävs konkreta insatser. Målet med projektet har varit att ta fram ett kunskapsunderlag som visar hur fjärrvärmesektorn kan bli en kolsänka till år 2045 genom avskiljning och lagring av biogen koldioxid från förbränning av biobränslen och avfall. Underlaget har legat till grund för fjärrvärmesektorns arbete med att utveckla en strategi för bio‐CCS vid kraft‐ och värmeproduktion inklusive förbränning av avfall. Projektet ger konkret vägledning till fjärrvärmeföretagen när det gäller implementering av bio‐CCS. I arbetet har ingått att visa på robusta utvecklingsvägar för utbyggnaden av bio‐CCS och hur bio‐CCS från år 2030 kan drivas på affärsmässiga grunder. Arbetet har omfattat sex arbetspaket samt en sammanfattande syntes kring ekonomi, teknik, infrastruktur, policy, regelverk och hållbarhetsaspekter för storskalig introduktion och användning av bio‐CCS i fjärrvärmebranschen. En mindre kunskapssammanställning kring CCU har också genomförts inom projektet.

    • Bone area is one measure of bone size that is easily derived from dual-energy X-ray absorptiometry (DXA) scans. In a GWA study of DXA bone area of the hip and lumbar spine (N ≥ 28,954), we find thirteen independent association signals at twelve loci that replicate in samples of European and East Asian descent (N = 13,608 – 21,277). Eight DXA area loci associate with osteoarthritis, including rs143384 in GDF5 and a missense variant in COL11A1 (rs3753841). The strongest DXA area association is with rs11614913[T] in the microRNA MIR196A2 gene that associates with lumbar spine area (P = 2.3 × 10 −42 , β = −0.090) and confers risk of hip fracture (P = 1.0 × 10 −8 , OR = 1.11). We demonstrate that the risk allele is less efficient in repressing miR-196a-5p target genes. We also show that the DXA area measure contributes to the risk of hip fracture independent of bone density.

    • The human antimicrobial peptide cathelicidin LL-37 has, besides its antimicrobial properties, also been shown to regulate apoptosis in a cell type-specific manner. Mechanisms involved in LL-37-regulated apoptotic signaling are not identified. Here, we show that LL-37 reduces the human osteoblast-like MG63 cell number and cell viability in the micromolar concentration range with an IC50 value of about 5 µM. Treatment with 4 µM LL-37 increased the number of annexin V-positive cells and stimulated activation of caspase 3 showing that LL-37 promotes apoptosis. Treatment with 4 µM LL-37 caused an acute and sustained rise in intracellular Ca(2+) concentration assessed by laser-scanning confocal microscopy of Fluo-4-AM-loaded MG63 cells. LL-37 increased Ca(2+) also in the presence of the respective L- and T-type voltage-sensitive Ca(2+) channel blockers nifedipine and NiCl2. LL-37 had no effect on Ca(2+) in cells incubated with Ca(2+)-free solution. LL-37 (4 and 8 µM) reduced the MG63 cell number both in the presence and absence of Ca(2+) in the medium. In conclusion, LL-37 reduces the osteoblast cell number by promoting apoptosis, and furthermore, LL-37 stimulates Ca(2+) inflow via a mechanism independent of voltage-sensitive Ca(2+) channels. Interestingly, LL-37-induced lowering of the cell number seems to be mediated via a mechanism independent of Ca(2+).

    • Background: The Autism Diagnostic Interview-Revised (ADI-R) is considered a first choice assessment tool in autism spectrum disorder. Nevertheless, despite its wide use in psychiatric practice and recommendations by various clinical guidelines, its interrater reliability has predominantly been confirmed in research settings by specially trained, research reliability interviewers. The reliability of ADI-R assessments among clinicians has not yet been established. Therefore, this study examined the spontaneous interrater reliability of the ADI-R in a naturalistic clinical multicenter setting. Sampling and Methods: Ten video-recorded ADI-R administrations were rated by 5 different raters each from a pool of 11 raters affiliated to 8 different clinical sites. Results: The interrater reliability for the 12 diagnostic criteria operationalizing autism spectrum disorders according to DSM-IV/ICD-10 in the ADI-R algorithms ranged between G(q,k) (analogous to intraclass correlations) = 0.96 and 0.99 for reciprocal social interaction, 0.96 and 1.00 for communication, and 0.91 and 0.97 for repetitive and restricted behavior. Reliability of diagnostic classification was KCohen 0.83. Conclusions: The findings endorse the psychometric properties of ADI-R in terms of interrater reliability previously reported from research settings and support their generalization to common clinical settings. Limitations of this study include an unbalanced sample composition.
