  • The Risk of Serious Infections Before and After Anti-TNF Therapy in Inflammatory Bowel Disease : A Retrospective Cohort Study

    • Lay Summary: The incidence rate of serious infection among inflammatory bowel disease patients did not increase with anti-TNF therapy compared with 1 year before treatment start. A decrease in incidence rate could be seen more than 1 year after initiation of anti-TNF.Background: Serious infections have been observed in patients with inflammatory bowel disease (IBD) on anti-TNF use-but to what extent these infections are due to anti-TNF or the disease activity per se is hard to disentangle. We aimed to describe how the rates of serious infections change over time both before and after starting anti-TNF in IBD.Methods: Inflammatory bowel disease patients naive to anti-TNF treatment were identified at 5 centers participating in the Swedish IBD Quality Register, and their medical records examined in detail. Serious infections, defined as infections requiring in-patient care, the year before and after the start of anti-TNF treatment were evaluated.Results: Among 980 patients who started their first anti-TNF therapy between 1999 and 2016, the incidence rate of serious infections was 2.19 (95% CI,1.43-3.36) per 100 person years the year before and 2.11 (95% CI, 1.33-3.34) per 100 person years 1 year after treatment start. This corresponded to an incidence rate ratio 1 year after anti-TNF treatment of 0.97 (95% CI, 0.51-1.84). Compared with before anti-TNF therapy, the incidence of serious infection was significantly decreased more than 1 year after treatment (incidence rate ratio 0.56; 95% CI, 0.33-0.95; P = .03).Conclusions: In routine clinical practice in Sweden, the incidence rate of serious infection among IBD patients did not increase with anti-TNF therapy. Instead, serious infections seemed to decrease more than 1 year after initiation of anti-TNF treatment.

    • OBJECTIVES: To investigate absolute and relative risk of serious infections in adult/elderly inflammatory bowel disease (IBD) diagnosed 2002-2017.METHODS: Nationwide, register-based cohort study of Swedish patients with IBD compared with general population matched reference individuals with regard to time to first serious infection, equal to hospital admission. Multivariable Cox regression estimated hazard ratios (HRs) for any serious infection. Secondary outcomes included site-specific infections, opportunistic infections and sepsis.RESULTS: We identified 47 798 individuals with IBD. During a follow-up of 329 000 person-years, they had 8752 first serious infections (26.6 per 1000 person-years). This compared with an incidence rate of 10.7 per 1000 person-years in matched reference individuals, corresponding to a 2.53-fold increased hazard of serious infections (95%CI = 2.47-2.59). The HR for serious infection in elderly-onset IBD was 2.01 (95%CI = 1.95-2.08). The relative hazard of serious infection was somewhat higher in Crohn's disease (2.94; 95%CI = 2.81-3.06) than in ulcerative colitis (2.24; 95%CI = 2.17-2.31). The HR for serious infections was high in the first year of follow-up (5.17; 95%CI = 4.93-5.42). Individuals with IBD were at a particularly high relative hazard of gastrointestinal and opportunistic infections. The HR for sepsis was 2.47 (95%CI = 2.32-2.63). The relative rates for serious infections in IBD increased in recent years.CONCLUSIONS: Patients with adult-onset IBD are at increased risk of serious infections, particularly gastrointestinal and opportunistic infections. Relative rates were highest just after IBD diagnosis, and seem to have increased in recent years.

In this paper, the results of the measurements of the response of the barrel calorimeter to hadrons with energies in the range 20-350 GeV and beam impact points and angles corresponding to pseudo-rapidity values in the range 0.2-0.65 are reported.     • This paper summarises the mechanical construction and installation of the Tile Calorimeter for the ATLAS experiment at the Large Hadron Collider in CERN, Switzerland. The Tile Calorimeter is a sampling calorimeter using scintillator as the sensitive detector and steel as the absorber and covers the central region of the ATLAS experiment up to pseudorapidities +/- 1.7. The mechanical construction of the Tile Calorimeter occurred over a period of about 10 years beginning in 1995 with the completion of the Technical Design Report and ending in 2006 with the installation of the final module in the ATLAS cavern. During this period approximately 2600 metric tons of steel were transformed into a laminated structure to form the absorber of the sampling calorimeter. Following instrumentation and testing, which is described elsewhere, the modules were installed in the ATLAS cavern with a remarkable accuracy for a structure of this size and weight.

    • The Tile Calorimeter, covering the central region of the ATLAS experiment up to pseudorapidities of +/-1.7, is a sampling device built with scintillating tiles that alternate with iron plates. The light is collected in wave-length shifting (WLS) fibers and is read out with photomultipliers. In the characteristic geometry of this calorimeter the tiles lie in planes perpendicular to the beams, resulting in a very simple and modular mechanical and optical layout. This paper focuses on the procedures applied in the optical instrumentation of the calorimeter, which involved the assembly of about 460,000 scintillator tiles and 550,000 WLS fibers. The outcome is a hadronic calorimeter that meets the ATLAS performance requirements, as shown in this paper.

    • Erfarenheter från Södra Länken och andratunnelprojekt har visat på ett behov av bättrekontroll över krympegenskaper hos sprutbetong, främst för att undvika sprickbildningi samband med dräner för vatten- och frostsäkring. Två delprojekt har genomförts medsyfte att bättre förstå de grundläggande mekanismerna för krympning hos sprutbetong ochundersöka möjligheter att minska denna medbibehållen sprutbarhet. Effekten av fiberinblandning som kan bidra till att fördela krympningenpå flera fina sprickor och därmed undvika korrosion på stålfibrer undersöktes också. Undersökningarna visar att man i laboratorieskalakan åstadkomma en sprutbar betong med lägrecementinnehåll än vid vanlig proportionering,vilket begränsar krympningen. En begränsadförsöksserie visade att risken för sprickbildningmärkbart kan reduceras genom inblandning aven kombination av stål- och glasfibrer.

    • Continuous fibre wrapping technique is becoming a common repair and strengthening technique for concrete structures. In this method continuous fibre sheets are bonded to the surface of a concrete structure using organic resins. The main disadvantages of this method are: (i) the use of resins (organic compounds) which are hazardous for the manual worker, and (ii) the low permeability of this type of organic materials. These problems could be solved if a cement-based matrix is used instead of epoxy matrix. In the current development stage one of the limiting parameter in a cementitious carbon fibre composite is the bond between the fibres and the cementitious matrix. The main objective of the study presented in this paper was to improve the bond between the carbon fibres and the cementitious matrix. Two possible methods were studied: (i) the design of a complex binder based on Portland cement with polymer and silica fume additions and (ii) surface treatment of the carbon fibres prior to their casting in the cementitious composite.

  • Prestressed hybrids of AAC and HPC : The BCE (Block Composed Element) building system. A conceptual study

    • There is an important potential for a development of a building system, if the present AACblock plants are completed with high performance concrete and pre-stressing technique. This can be done as a continuation of a present AAC production or in a site factory. Hybrid cooperation between AAC and concrete is not a new technology. Traditionally, AAC is covered with rendering. The wetted material is first sprayed with cement slurry after which comes lime/cement-based rendering which strengthens the wall and supplies a mechanical protection. AAC blocks can be used as infill members in concrete deck plates and concrete framed walls (Hellers, 1993), in which the shrinkage of the surrounding concrete locks completely the cured blocks into a stable composite. A good cooperation between AAC and concrete is a requirement for the pre-stressed hybrid. This combination has been investigated with reference to bond and moisture content. The interface must have sufficient bond to supply shear strength to the structural member. Also, the concrete should be suitable for pre-stressing which requires a minimum strength class K40. Here, K60 is applied in order to reduce creep and avoid creep failure of the AAC. A production layout for the purpose is suggested. This research consists of three parts: 1- Hybrid concrete elements The principal formulation of hybrids, built on cooperation between two concrete materials, a weak AAC and a strong HPC poured on top, shows that this combination unites the most favourable qualities of the two concretes into a structural element with rational building technology. Load-bearing capacity is good, and the fire protection is excellent. Through pre-stressing of the structural concrete, a crack-free behaviour is guaranteed up to the service limit, and deflection from dead weight (incl. floor covering and possibly half the service load) be eliminated. The most important structural elements needed in a building system could be taken as hybrids. In drawings, different members like floor- roof- and wall members, window and door lintels are shown. 2- Production system for hybrids AAC blocks form a bed containing the pre-stressing steel in slits and on which high performance concrete is poured. Pre-stressing brings the two concretes together. This is especially important for the shear capacity of a building member, by which extra dowels can be omitted. The pre-stressing force is anchored by plates directly against the cured AAC blocks. A special pre-stressing bed is not required. The production is arranged in such a way that necessary equipment (trays, form strips, wedges, locks) are circulated within twenty-four hours. The necessary manpower for this facility is analysed. Normally 8 men are needed per shift. A detailed conceptual production layout for a hybrid production plant is included for discussion. See figure 6.2 (suggested production layout). 3- Application in building The hybrid members are united at joints and through seams. Continuity over connections is achieved by filling seams with mortar grout. Reinforcement may be included to achieve ductility. By a similar method, it is possible to make connections between horizontal and vertical building members. Common connection details are shown in the report. This building method replaces the equivalent method with concrete or AAC members, and it is in fact a coordination between these two. The maximum span of floor members is up to 9 m. It makes the system suitable for modern residential house production, but also suitable for office buildings, industrial halls and other applications. See attached drawings, part 2 and part 3.

    • The response of pions and protons in the energy range of 20–180 GeV, produced at CERN's SPS H8 test-beam line in the ATLAS iron–scintillator Tile hadron calorimeter, has been measured. The test-beam configuration allowed the measurement of the longitudinal shower development for pions and protons up to 20 nuclear interaction lengths. It was found that pions penetrate deeper in the calorimeter than protons. However, protons induce showers that are wider laterally to the direction of the impinging particle. Including the measured total energy response, the pion-to-proton energy ratio and the resolution, all observations are consistent with a higher electromagnetic energy fraction in pion-induced showers. The data are compared with GEANT4 simulations using several hadronic physics lists. The measured longitudinal shower profiles are described by an analytical shower parametrization within an accuracy of 5–10%. The amount of energy leaking out behind the calorimeter is determined and parametrized as a function of the beam energy and the calorimeter depth. This allows for a leakage correction of test-beam results in the standard projective geometry.

    • This study investigates shrinkage of accelerated shotcrete (sprayed concrete), especially in the case of shotcrete sprayed on drains, a part of a tunnel lining not continuously bonded to the rock. One of the goals is to find methods of avoiding shotcrete shrinkage cracks in such drain structures. If cracks yet develop the crack distribution is of great importance, i.e. several fine cracks instead of one wide. By using both steel and glass fibres this may be achieved. A newly developed test set-up for shrinking, end-restrained shotcrete slabs is also presented and evaluated. The performed tests show that the addition of very fine glass fibres could be a solution to the cracking problem. The newly developed test equipment using concrete interacting with an instrumented granite slab represents a realistic way of testing restrained shrinkage. The on-going research focuses on the optimization of the glass fibre addition and the understanding of the interaction between shrinkage and creep of shotcrete.

    • This study investigates early age bond strength of shotcrete (sprayed concrete), in the case of shotcrete sprayed on hard rock. Shotcrete differs from ordinary, cast concrete through the application technique and the addition of set accelerators which give immediate stiffening. The bond between shotcrete and rock is one of the most important properties. During the very first time after spraying the physical properties and the bond to the rock depend on the set accelerator and the micro structure that is formed. In this work a laboratory test method for measuring early bond strength for very young or early age shotcrete is presented. The newly developed method was tested and evaluated and proved that it can be used for bond strength testing already from a couple of hours after shotcreting.

    • This study investigated the shrinkage of shotcrete (sprayed concrete), especially the case of shotcrete sprayed on drains – a part of tunnel lining not continuously bonded to the rock. A newly developed method for testing the shrinkage of end-restrained shotcrete slabs is presented and evaluated. The test setup, which is designed to capture in situ behaviour, consists of shotcrete sprayed on an instrumented granite slab partly covered with a plastic sheet. The primary goal was to practically use and evaluate the test method with shotcreted test samples. Results from restrained shrinkage tests are presented along with results for free shrinkage. It is shown that the method realistically captures the behaviour of shotcrete drains on hard rock in situ. In addition, the corresponding compressive strength and flexural crack strength as functions of shotcrete age are also reported. The test method can be used for an evaluation of different solutions for avoiding shrinkage cracks in shotcreted soft drains, or in shotcrete that is fully bonded to a rock surface, with respect to preventing cracking or distributing the shrinkage strain into several fine cracks instead of one wide crack.

    • Stabilization of arsenic contaminated soils by iron oxides has been proposed as a remediation technique to prevent leaching of arsenate into the environment. Fundamental studies are needed to establish under which conditions the complexes formed are stable. In the present work, a powerful technique, viz. ATR-FTIR spectroscopy, is adapted to studies of adsorption of arsenate species on iron oxides. This technique facilitates acquisition of both quantitative and qualitative in situ adsorption data.In the present work, about 800 nm thick films of 6-lineferrihydrite were deposited on ZnSe ATR crystals. Arsenate adsorption on the ferrihydrite film was studied at pD values ranging from 4 to 12 and at an arsenate concentration of 0.03 mM in D2O solution. The amount of adsorbed arsenate decreased with increasing pD as a result of the more negatively charged iron oxide surface at higher pD values. The adsorption and desorption kinetics were also studied. Arsenate showed a higher adsorption rate within the first 70 minutes and a much lower adsorption rate from 70 up to 300 minutes. The low adsorption rate at longer reaction times was partly due to a low desorption rate of already adsorbed carbonate species adsorbed at the surface. The desorption of carbonate species was evidenced by the appearance of negative absorption bands. The desorption of adsorbed arsenate complexes was examined by flushing with D2O at pD 4 and 8.5 and it was found that the complexes were very stable at pD 4 suggesting formation of mostly inner-sphere complexes whereas a fraction of the complexes at pD 8.5 were less stable than at pD 4, possibly due to the formation of outer-sphere complexes.In summary, the ATR technique was shown to provide in situ information about the adsorption rate, desorption rate and the speciation of the complexes formed within a single experiment, which is very difficult to obtain using other techniques.

    • In the present study, in-situ ATR-FTIR spectroscopy was used for the first time to study the competitive adsorption of phosphate and arsenate on ferrihydrite. Deuterium oxide was used as solvent to facilitate the interpretations of recorded infrared spectra.It was found that arsenate and phosphate adsorbed more strongly at lower pD values, showing similarities in the adsorption behavior as a function of pD. However, arsenate complexes were found to be more strongly adsorbed than phosphate complexes in the pD range studied. About five times higher concentration of phosphate in solution was needed to reduce the absorbance due to pre-adsorbed arsenate to the same relative level as for pre-adsorbed phosphate, which was desorbed using a solution containing equal (molar) concentrations in arsenate and phosphate. At pD 4, two phosphate complexes were adsorbed on the iron oxide, one deuterated and one de-deuterated. When phosphate was pre-adsorbed and arsenate subsequently added to the system, the deuterated phosphate complex desorbed rapidly while the de-deuterated phosphate complex was quite stable. At pD 8.5, only the de-deuterated phosphate complex was adsorbed on the iron oxide. Moreover, the arsenate adsorbed was also predominantly de-deuterated as opposite to the arsenate adsorbed at pD 4. During the substitution experiments the configuration of these complexes on the iron oxide surface did not change. To the best of our knowledge, this is the first time this difference in stability of the different phosphate complexes is reported and shows the power of employing in-situ spectroscopy for this kind of studies.
