    • The aim of this study was to illuminate the experiences of occupational therapy interventions in individuals with psychoses. Repeated tape-recorded narrative interviews were conducted with six women participating in occupational therapy immediately after an intervention. The subsequent analyses followed a phenomenological approach. Key constituents integrated in two structures, are the main ®ndings. The meaning of occupational therapy as expressed in the key constituents relief, selfknowledge, belief in the future, capability, resistance and satisfaction formed one structure. The form of occupational therapy as expressed in the key constituents time, environment, guidance, voluntariness and collaboration represented the other structure. These ®ndings con®rm and give empirical support to beliefs and assumptions expressed in occupational therapy literature. The results form a conceptual base for developing an evaluative assessment instrument for individuals with psychoses participating in occupational therapy.

  • Contraceptive counselling to teenagers at abortion visits : a qualitative content analysis

    • Objective: Contraceptive counselling at the time of an abortion is crucial for preventing new unintended pregnancies especially in teenagers. What is discussed on this occasion should be recorded in the patient's file. In this study we examined what was documented in medical records (MRs) about previous contraceptive use and further plans as a reflection of what was discussed between the gynaecologist and the teenager.Methods: Thirty-six MRs were consecutively selected in 2006. The study design was descriptive, retrospective and qualitative. Data were analysed using qualitative content analysis. The text areas were transcribed and analysed in several steps.Results: Two themes were generated from the analysis: 'Contraceptive methods previously used' and 'Plan for future contraceptive use'. Information on previous contraceptive use was absent in ten MRs. In five MRs data about future contraceptive use were missing and eight teenagers had not started a contraceptive method at follow-up.Conclusion: The MR often lacked information about contraception; this could be a reflection of insufficient contraceptive counselling at the abortion-visit. Both contraceptive counselling and documentation thereof must be improved to enhance contraceptive use in teenagers.

    • The majority of occupational therapists in Sweden previously worked on large occupational therapy wards. Health care policy has changed over the years and the system has been reorganized accordingly. The employment situation for occupational therapists has also changed. This paper focuses on the perception of professional self among occupational therapists. The objective was to explore the professional self and to suggest components important to the occupational therapist profession. The Professional Self Description Form (PSDF) was used for the exploration of self. The 19 items in the PSDF cover areas relevant to professional functioning and activity. Sixty-eight employed occupational therapists participated. The results of the PSDF were subjected to factor analysis and five factors were obtained; Ability to solve problems, Professionalism, Management, Empathy, and Working capacity. We believe that these five factors can function as improving domains of the role of the professional occupational therapist in Sweden

    • The aim of the present study was to investigate how Swedish occupational therapists’ describe their work with persons suffering from cognitive impairment following acquired brain injury. A qualitative descriptive approach was used and interviews were conducted with 12 occupational therapists working in community and county council care. Qualitative content analysis was used and revealed three main themes: 1) “To make the invisible visible”, 2) “To collaborate- a prerequisite for success” and 3) “Dilemmas to handle”. The findings showed a complex scenario where the occupational therapists worked to make the cognitive impairments visible to themselves, the clients, and persons close to the client. Collaboration was perceived as a key factor. The dilemmas concerned different aspects in the rehabilitation process, which affected the occupational therapists’ work with the clients. Identified areas in need of improvement are prioritisations and additional education regarding both intervention methods and theory. A reluctance to use standardised assessments was expressed and research that identifies and overcomes those hindrances in clinical practice is needed. Therapeutic use of self was described as important. To understand and illuminate the occupational therapists’ comprehension of the concept further research is required.

    • Objectives: Studies have shown that occupational therapy interventions for clients with cognitive impairment following acquired brain injury (CIABI) have a positive effect on performance of activities of daily living, although the exact nature of the interventions are not clearly described. A better understanding of current practice is therefore needed to move further in the search for evidence based occupational therapy for clients with CIABI. Research on occupational therapists’ (OTs’) practice can play an important role to explore and describe current practice and furthermore define and evaluate the effectiveness of different components in OT interventions. The aim of this study was to survey Swedish OTs’ practice patterns for clients having CIABI.Methods: The study had a cross-sectional design using a questionnaire developed in two former studies. A stratified random sample was done with 250 OTs each from the following areas: regional care, somatic county care, primary care and municipal eldercare. The questionnaire was sent out using on-line survey software and 462 participants responded. Data is currently analyzed by descriptive and comparative statistics.Results: Preliminary results show that the interventions targeted a wide range of cognitive impairments and its consequences. Interventions were both remedial and compensatory with graded activity as the most common remediating intervention. To compensate for activity limitations prescription, or recommendation, of assistive devices (AD) as well as supporting clients in finding internal strategies were used extensively. The ADs used consisted of both high and low technology devices. Eighty-two percent also included different specific interventions to improve clients’ decreased self-awareness in their practice. A vast majority of the OTs (94%) preferred to use observations in daily activities to assess both activity limitations and cognitive impairment. Almost all participants also used interviews and sixty-two percent used tests on impairment level. To support practice general Occupational Therapy models were widely used while theories focusing on cognitive impairments specifically were used less. The participants’ answers reflected a client-centered attitude with collaborative goal-setting and feedback discussions after each session. Eighty- four percent felt dependent on relatives to reach a successful outcome and most of them worked deliberately to involve relatives in the rehabilitation. The OTs responses were evenly distributed over the spheres of activity. Ninety-two percent of the participants worked in team and the most common diagnoses were stroke and traumatic brain injury. There were some differences in responses between the spheres.Conclusions: Preliminary conclusions are that the OTs practice patterns included interventions targeting most cognitive impairments and their consequences in daily activities. The use of daily activities as a mean was preferred irrespective of whether the interventions were remediative or compensatory. Interventions targeting decreased self-awareness as well as the use of ADs were common. A client-centered approach was dominating including collaboration with relatives.For personalcentered approach was dominating including collaborationwith relatives.

    • Clients with cognitive impairment following acquired brain injury (CIABI) are a common group to receive occupational therapy services. Research has shown that occupational therapy has a positive effect on occupational performance for these clients, but the exact nature of the interventions is not clearly described and needs to be better understood and defined. The aim of this study was to develop and evaluate an empirically derived questionnaire for the purpose of surveying occupational therapists' (OTs') practice patterns in relation to CIABI. The questionnaire was developed from the results of a former qualitative study. It was evaluated for content validity by a group of six OT researchers with experience in CIABI, using the content validity index (CVI). Reliability was evaluated by a test-retest design with a group of 51 OTs. Data were analysed by non-parametric statistical methods. Initially the questionnaire consisted of 90 items dealing with OT practice and nine demographic questions. After the reliability and content validity process the OT practice items were reduced to 44. The revised questionnaire will be used to survey and explicitly describe occupational therapy practice for clients with CIABI.

    •  The aims of this study were to explore the intended meaning of the terms “occupation”, “activity”, “doing”, and “task” used in international occupational therapy literature and from this perspective explore which Swedish terms best capture these meanings. A literature review of occupational therapy-related journals was performed to gain a basic understanding of the term occupation and related terms. In addition, a semantic review was used: English and Swedish dictionaries were reviewed to explore the semantic meaning of the English terms “occupation”, “activity”, “doing”, and “task”, and the Swedish terms “aktivitet”, “syssla/sysselsättning”, “görande”, and “uppgift”. A comparison was also performed by searching for parallels between the results of the literature review, the semantic definitions of the English and Swedish terms and the comprehensive meaning of the Swedish terms aktivitet and syssla/sysselsättning. An overarching idea of the concept of occupation was found in the literature review and for the purposes of this study we have identified this as Occupation for survival. From this overarching idea, three themes were identified: The feature of occupation, Impact of occupation and Occupation an occupational therapy concept. Each theme could be subdivided into sub-themes. The Swedish term aktivitet was found to have more power, strength, and spirit connected to the synonyms found in the semantic analysis than to those connected with the Swedish term syssla/sysselsättning. According to the findings in this study we found that the term “aktivitet” is the best comparable term in Swedish to the English term “occupation”.

    • The purpose of this study was to assess the validity and reliability of a new tool called the Experiences of Occupational Performance Questionnaire (EOPQ) and also to investigate whether there were any gender differences in the answers. The EOPQ was completed by 32 women and 73 men with severe mental disorders, who participated in an occupation arranged by the healthcare system in Sweden. The EOPQ was originally developed from interviews with six women participating in occupational therapy. An analysis of the content validity of the EOPQ was performed with the help of seven occupational therapists. Forty-six of the 49 items in the EOPQ were valued as representative of the concept occupation as a resource in therapy. Principal component analysis performed in two steps yielded seven factors called: "Satisfaction with own ability"; "Confidence in own ability"; "Capability to perform"; "Challenge"; "Ability to handle difficult situatio"; "Forgetting time and self"; and "Identification with others". The internal consistency calculated by Cronbach's f values varied between 0.63 and 0.91 in the seven factors. No gender differences were found on the factor level but differences in five items were found. The results showed that EOPQ could be used as a tool to measure therapeutic potentials in occupations and could also contribute to knowledge to secure the use of occupations as a therapeutic resource for individuals with severe mental disorders.

    • The purpose of the study was to analyse the content of occupational therapy records documenting treatment for individuals with psychoses. A retrospective content analysis of occupational therapy records regarding treatment goals, interventions and outcome was undertaken using a coding scheme, the Template of Occupational Therapy (TOT). Twenty-five occupational therapists chosen at random contributed 64 occupational therapy records, generating 2992 statements.The patients’ disorders and functional and activity limitations (n=1723) were comprehensively described. Ability to manage self-care and the home environment, and to engage in a daily occupation, emerged as the prominent treatment goals. The content of the occupational therapy interventions was characterized by descriptions of a wide range of activities, which were used as therapeutic resources.The occupational therapist had a client-centred approach, combining the use of the current activity with the use of herself/himself as a resource. A few statements in the records were interpreted as relating to outcomes of treatment for this patient group. Statements related to outcome were the patients’ self-reports and how they were able to manage activities of daily living. This retrospective study was found to be a useful conceptual model for designing studies of evidence-based occupational therapy in mental health

    • This study is the second in a series of studies carried out in Sweden concerning the dimensions of the concept occupation. The specific aim of this study was to explore new and confirm previously found dimensions of the concept of occupation in the context of occupational therapy. Occupational therapy students, a few weeks from their completion of studies, were asked to write down spontaneously what they personally considered to be occupation. Forty-two women and three men, aged between 25 and 33 years, participated. Grounded theory with the constant comparative method was used to analyse the data. A coding scheme of 40 codes was used to compare new data with previously found data concerning the concept of occupation as described by occupational therapy students. Six new codes concerning occupation expanded the dimensions of the concept. Five of those were found within the doing and context dimensions. These codes defined occupation as something that depends on who is performing the occupation and where the occupation is performed. Thus, occupation is not a permanent state but also very much depends on subjective experience. Additional studies with experienced occupational therapists have been planned to further expand these findings and aim to give a stronger foundation to the concept of occupation built on empirical grounds.

  • Performance of occupations in daily life among individuals with severe mental disorders

    • The aims of this study were to describe self-perceived capability to perform daily occupations, functional cognitive ability and differences between self-perceived capability to perform daily occupations and cognitive levels among individuals with severe mental disorders. Fifty-one persons participated in the study. The participants completed a structured interview concerning their own perceived experiences of daily occupations, they were also observed during performance of a standardized task and assigned a cognitive level score. The results showed that most participants perceived small problems in Activities of Daily Living. On the other hand, Play and Leisure Activities seemed problematic to them. The participants' cognitive levels seem to correlate with their performance of Work and Productive Activities. The results also showed that a structural tool focusing on capability to perform daily occupations enabled mentally disabled persons to have a perspective of their own. Together with observed assessment it gave complementary information about these people's occupational performance. These shared identified occupational problems could serve as a foundation for planning individually adapted interventions

    • The objective of this study was to construct a coding schema for analysing the occupational therapy process, often described as formulating the treatment goal, implementing treatment and evaluating the effect of treatment. The concepts underlying the terms treatment goal, treatment intervention and effect or outcome of treatment, as used in occupational therapy, were defined and an analytical template of questions was constructed, using the analysis method designed by Walker and Avant (1995). The method included a third part; definitions of the three concepts were formulated on the basis of excerpts from occupational therapy textbooks in mental health that are well known in Sweden. The results showed that the concept of treatment goal included the attributive components planning goals, step-by-step goal, and group goal; the concept of treatment intervention included the components planning process, available resources and level; and the concept of effect or outcome of treatment included type and measurement. The excerpts for each of the components were then summarized as statements. These statements were converted into the questions that constituted the template. Third, these questions were used to test the usefulness of the template in analysing the occupational therapy content of 10 scientific reports on individuals with psychosis. All but two questions served to identify attributes of the three concepts. The template was useful for analysing the content of 10 research studies relevant to the occupational therapy process in mental health

  • The development of the Self Assessment of Occupational Ability : a tool to measure occupation as a medium in therapy among persons with mental disability

    • To add knowledge on how occupations contribute to health and wellbeing and to explicit describe thetherapeutic potential in occupations deeper knowledge is needed. One way is to ground the knowledgeon narratives from people’s experiences. In consequence a questionnaire was constructed based onnarratives from persons with mental disorders who have taken part in occupational therapy. A new version of the questionnaire was developed and as the items have been reformed a new underlying structure was expected.The aim of the present study was to investigate the construct validity and the internal consistency of the questionnarie in a new sample of individuals with severe mental disorders. Persons (n=106) with mental disorders participated in the study by answering the questionnarie after performing an occupation in therapy. The correlation between each of the 35 items was analyzed and items with high correlation were excluded from further analysis. Twenty-five items were left to explorethe underlying factor structure of the questionnarie.A principal component analysis with varimaxrotation resulted in the five factors, “Ability to perform occupation”, “Feeling of belongingness”, “Engaging in occupation”, “Influencing on occupation” and “Insight into ability”. A second order factoranalysis resulted in a three component structure explaining 79 % of the variance. In summery, a pattern of occupation as a therapeutic medium was found and gave a ground for the construction of the questionnaire Self Assessment of Occupational Ability (SAO).The pattern found is constructed by the three components, “Sense of competence, “Feeling of belongingness” and “Insight into ability”including the five factors and 25 items. The SAO will be presented the first time in the WFOT 2014, a tool to bee used by occupational therapist in their cooperation with persons with mental disability to find out how occupation can be of help in their recovery.

    • The aims of this study were to test the construct validity and the homogeneity of the Swedish version of the Burden Assessment Scale (BAS) and study perceived burden in daily life among family caregivers of individuals with severe mental disorders. A total of 256 caregivers took part in the study. The participants completed two questionnaires. One was the BAS, a brief scale that focuses on specific subjective and objective consequences of families caring for individuals with severe mental disorders. The other one was a questionnaire constructed for the study including demographic data. A principal component analysis yielded three factors: 'activity limitation', 'feelings of worry and guilt' and 'social strain'. The internal consistency calculated by Cronbach's alpha coefficients varied between 0.73 and 0.88 for the three factors. The findings indicated that the caregivers experienced more 'activity limitation' and 'social strain' in relation to the youngest and to those who had an elementary educational level than they did for comparison groups. There were no differences found between the caregivers in relation to clients' daily occupation outside home. The results confirm that family caregiver burden is complex and includes several areas such as activities in daily life, worry and social strain. Given this knowledge about caregivers' situation, it is critical that families be given opportunities to identify what support might help. Further research is needed to identify such support.

  • The use of creative activities in occupational therapy practice in Sweden

    • Occupation as a mean in therapy contributes to personal development, stimulates people’s senses, and generates positive energy, happiness and satisfaction. Creative activities might be one form of occupation that mediates these outcomes. The aim of this study was to describe the prevalence of creative activities in occupational therapy in Sweden and how often occupational therapists use creative activities as a means of intervention. A web-mail survey was sent to 2975 occupational therapists working in health care and 63 % answered the questionnaire, of these 44 % did use creative activities. The extent to which the respondents used creative activities varied; 48 % used creative activities once a week or more, 36 % once a month or more, and 14 % at some point every six months. There was a significant relationship between the level of health care the respondent worked at and use of creative activities. Creative activities were more often used by respondents working at county council, regional and “other” health care levels than by those working in primary health care or with vocational rehabilitation. Respondents who worked with patients in psychiatric rehabilitation used creative activities significantly more often than those working with patients in somatic rehabilitation, stressed patients/patients with chronic pain or other patients. Occupational therapists working in psychiatric rehabilitation reported using creative activities more frequently compared to the other areas of health care. No relationships were found between the use of creative activities and the variables gender or years of experience as occupational therapists. Further research should focus on more in-depth inquiries about how occupational therapists and their patients perceive the use of creative activities as a means of intervention in occupational therapy

    • Primary objective: To describe Swedish occupational therapist practice patterns for clients with cognitive impairment following acquired brain injury.Research design: A cross-sectional stratified random sample of 462 occupational therapists.Methods: An online questionnaire was used to collect data.Main results: The predominant practice pattern was the use of ADL-activities for assessment and therapy regardless of whether limitations in occupational performance or cognitive function were assessed or whether the approach was remedial or compensatory. For assessment, general ADL-instruments were used more often than instruments that assessed cognitive function. Instruments were used less often within municipal rehabilitation facilities compared to regional, county and primary care facilities. The most common focus of the therapies was in regard to abilities related to executive functioning. Another prominent practice pattern was a collaborative approach involving clients, relatives and other staff. The theories used in practice were, to a large extent, general in nature and did not focus specifically on cognitive functioning.Conclusions: Swedish occupational therapy practice for clients with cognitive impairments following acquired brain injury focuses highly on occupational performance. Therapies targeting executive functioning seem particularly important in practice and a collaborative approach involving clients, relatives and other staff is a prominent feature in practice.

Aims: One aim was to investigate mentally disordered offenders' background factors, confidence in and how they value occupations. Another aim was to investigate MDOs background factors' in relation to and the influences on Occupational Performance and Social Participation. Method: Data was collected with an explorative, correlative design after informed consent, from 74 mentally disordered offenders (mean age 34,2) cared for in forensic psychiatric hospitals. Assessments were Allen Cognitive Level Screen, Capability to Perform Daily Occupations, Interview Schedule of Social Interaction, Manchester Short Assessment of Quality of Life, Self-efficacy Scale and Importance scale. Eight background factors were assembled from the individual forensic psychiatric investigation. Findings: Most of the investigated background factors relate to and half of them influence occupational performance, particular the cognitive aspect of occupational performance. The influences on occupation originate from adulthood, such as suffering from schizophrenia, psycho/social problems, and having performed violent crimes. These findings indicate that staff in forensic hospital care should initiate rehabilitation with knowledge about MDOs' complex daily occupations. For avoiding information bias, information gathering preceding treatment planning should be performed in collaboration between caring staff and mentally disordered offenders.

    • Goal Attainment Scaling (GAS) är en metodik för att beskriva behov och mål för personer med funktionshinder. Målen kan med denna metodik systematiskt utvärderas och följas upp över tid. Fördelen med GAS är dess anpassningsbarhet, vilket innebär att olika typer av mål kan skrivas och varierande former av dokumentation kan användas. Med GAS kan målrelaterade under- och överprestationer konkretiseras. I denna manual presenteras GAS-metodiken genom att beskriva hur mål formuleras, hur måluppfyllelse graderas, hur GAS dokumenteras och följs upp. Avsikten med manualen är att avsnitten skall studeras i den ordningsföljd som är beskriven i den detta för att ge en god introduktion till metodiken. För att underlätta inlärningen ges exempel samt möjligheter till att öva metodiken. Förhoppningen är att övningarna även skall uppmuntra till tankeväckande reflektion om hur det är att arbeta med GAS-metodiken. Denna manual vänder sig i första hand till personal inom psykiatrisk vård och rehabilitering.

    • Purpose – The purpose of this paper is to test the psychometric properties and dimensionality of the instrument Quality in Psychiatric Care-Housing (QPC-H) and briefly describe the residents’ perception of quality of housing support. Design/methodology/approach – A sample of 174 residents from 22 housing support services in nine Swedish municipalities participated in the study. Confirmatory factor analysis (CFA) revealed that the QPC-H consisted of six dimensions and had a factor structure largely corresponding to that found among other instruments in the Quality in Psychiatric Care (QPC) family of instruments. Findings – CFA revealed that the QPC-H consisted of six dimensions and had a factor structure largely corresponding to that found among other instruments in the QPC family of instruments. The internal consistency of the factors was acceptable except in the case of secure and secluded environment, probably due to few numbers of items. With this exception, the QPC-H shows adequate psychometric properties. Social implications – The residents’ ratings of quality of housing service were generally high; the highest rating was for secluded environment and the lowest for participation. This dimension would thus seem to indicate an important area for improvement. Originality/value – The QPC-H includes important aspects of residents’ assessment of quality of housing service and offers a simple and inexpensive way to evaluate housing support services from the residents’ perspective. © 2016, © Emerald Group Publishing Limited.

    • Background/Aims: Community-based day centres in Sweden are well-established arenas for psychiatric rehabilitation, but little is known of the attendees' perception of the quality of the service provided. The aim of the study was thus to describe and investigate the quality of the services in community-based day centre for people with psychiatric disabilities.Methods: A sample of 218 attendees in 14 community-based day centre services in Sweden completed the Quality in Psychiatric Care - Daily Activities (QPC-DA).Results: The results showed that people with psychiatric disabilities perceived the quality of community-based day centre services as high. Most notably, quality of service was rated higher by those with lower educational level, had waited shorter time to attend the centre, and had better mental and physical health. However, particularly aspects of a secluded environment and participation (information) may be areas with potential for improvement.Conclusion/Significance: From an occupational science perspective, the results adhere to the importance of occupational balance, with periods of rest/privacy during the time at the centre.

    • A core assumption in occupational therapy is that occupation enhances health. This study is one of a series of planned studies carried out in Sweden, with the overall aim to develop a theory, based on empirical grounds, concerning the dimensions of occupation that may be useful in occupational therapy. The specific aim for this study is to examine the dimensions of occupation as generally understood among novice occupational therapy students. Grounded Theory was the chosen method. Data were collected among occupational therapy students. The students wrote down spontaneously what they considered to be included in activity. A preliminary core category ‘‘The what, why, and how of occupation’’ and five other categories emerged from the analysis: (1) The doing and context of occupation; (2) Motive for occupation; (3) Time and place for occupation; (4) Type of participation; and (5) Outcome of occupation. Relationships between the categories were established between the Motive for occupation and Outcome of occupation, which constitute a kind of prerequisite for occupation. The three remaining categories: The doing and context of occupation, Time and place for occupation, and Type of participation established a form for occupation. In conclusion, the study results illuminated the richness of the concept of occupation and support the core assumption in occupational therapy that occupation enhances health.

    • The aim of this study was to describe the prevalence of creative activities in occupational therapy in Sweden and how often Swedish occupational therapists use creative activities as a means of intervention. A web-mail survey was sent to 2975 Swedish occupational therapists working in health care at regional, county council or primary health care level, and those working in vocational rehabilitation. A total of 1867 (63%) answered the questionnaire and showed that 44% did use creative activities as a means of intervention and most often by practitioners working in psychiatric health care. The most commonly used form of creative activity was arts and crafts followed by gardening. This web-mail survey was based on a limited amount of items regarding creative activities. Further research should focus on in-depth inquiries about how occupational therapists and their patients perceive the use of creative activities as a means of treatment in occupational therapy.
